A Insuficiência Cardíaca (IC) é uma das principais causas de internação hospitalar no mundo e tem um elevado grau de morbidade e mortalidade, sendo um grave problema de saúde pública. Os lncRNAs (RNAs longo não codificantes), têm funções regulatórias transcricionais e/ou pós transcricionais bem complexas e que ainda não são totalmente claras, mas que podem exercer influência sobre as doenças cardiovasculares, dentre elas a IC. Assim o estudo teve como objetivo identificar na literatura o papel dos lncRNAs na patogênese da IC por meio de uma revisão integrativa com busca sistemática. Foram considerados elegíveis para leitura e composição do estudo 33 artigos e os principais papéis dos lncRNA na IC foram relatados como possíveis marcadores biológicos para diagnóstico e prognóstico da doença devido a sua expressividade na corrente sanguínea. Além disso, os lncRNAs podem estar relacionados à capacidade funcional uma vez que o aumento ou diminuição de sua expressão promove redução da apoptose de células endoteliais, melhora a disfunção cardíaca, distúrbios de contratilidade e dos canais de cálcio em pacientes com IC. Portanto, os lncRNAs parecem estar envolvidos na patogênese e/ou fisiopatologia da IC, podendo ser utilizados como biomarcadores genéticos com sensibilidade e especificidade semelhantes ou superiores aos empregados atualmente no diagnóstico e prognóstico da IC.

Introdução: A Hipomineralização Molar-Incisivo (HMI) é caracterizada como uma alteração qualitativa do esmalte de origem sistêmica que envolve de 1 até 4 primeiros molares permanentes, podendo, também, atingir incisivos permanentes. Objetivos: Avaliar as evidências disponíveis na literatura sobre os fatores etiológicos associados à HMI. Métodos: Realizou-se uma revisão integrativa da literatura a partir de pesquisas nas bases de dados Pubmed, Web of Science, Scopus, Lilacs e Scielo, entre os meses março e maio de 2020. Utilizaram-se os descritores “Dental enamel hypoplasia”, “Tooth Demineralization”, “Molar”, “Incisor” e “Etiology”. Os critérios de inclusão foram: artigos publicados em português, inglês ou espanhol indexados nas bases de dados selecionadas e disponíveis de maneira gratuita, acessados via Portal de Periódicos Capes. Foram descartados todos os estudos que não apresentassem relação com a temática de interesse e que não fossem estudos primários. Resultados: Inicialmente, obteve-se um quantitativo de 673 publicações. Após avaliação criteriosa, a amostra final constou de 19 artigos. Influência genética e condições ambientais tais como renda familiar, problemas durante o parto e de saúde durante os três primeiros anos de vida da criança são fatores associados à HMI. Conclusão: Problemas durante o parto, hipocalcemia no recém-nascido prematuro, uso de antibióticos, fatores ambientais, febre alta e doenças respiratórias na primeira infância, além de fatores genéticos, estão associados ao surgimento da HMI.

To examine whether free (3-PD-5free) and/or liposomal (3-PD-5lipo) 6,7-dihydroxy-3-[3′,4′-methylenedioxyphenyl]-coumarin (3-PD-5) (1) modulate the effector functions of neutrophils from patients with rheumatoid arthritis under remission (i-RA) and with active disease (a-RA), in vitro; and (2) exert anti-inflammatory effect in a rat model of zymosan-induced acute joint inflammation. Incorporation of 3-PD-5 into unilamellar liposomes of soya phosphatidylcholine and cholesterol was efficient (57.5 ± 7.9%) and yielded vesicles with low diameter (133.7 ± 18.4 nm), polydispersity index (0.39 ± 0.06), and zeta potential (− 1.22 ± 0.34 mV). 3-PD-5free (1 µM) and 3-PD-5lipo (3 µM) equally suppressed elastase release and reactive oxygen species generation in neutrophils from healthy subjects and i-RA and a-RA patients, stimulated with immune complexes. 3-PD-5free (20 µM) suppressed the release of neutrophil extracellular traps and chemotaxis in vitro, without clear signs of cytotoxicity. 3-PD-5lipo (1.5 mg/kg, i.p.) diminished joint edema and synovial infiltration of total leukocytes and neutrophils, without changing the synovial levels of TNF-α, IL-1β, and IL-6. Altogether, the results reported herein indicate that 3-PD-5 is a promising modulator of the early stages of acute joint inflammation that can help to diminish not only excessive neutrophil infiltration in the synovia but also neutrophil activation and its outcomes in RA patients.

The COVID-19 pandemic has imposed urgency to answer numerous questions about the control of viral infections and the dual immune response of protection of and damage to the host, with the purpose of finding specific and efficient treatments and strategies for prevention of infection. The association between severe cases of the disease and overactivation of the immune response has raised the hypotheses that the inflammatory response participates in the pathogenesis of SARS-CoV-2 infection, and that neutrophils are important effector cells of such response. In this sense, neutrophils are potential targets for pharmacological interventions to treat this infection. The present paper briefly describes the neutrophil biology and discusses the possible roles that neutrophils play in the pathogenesis of COVID-19, based on literature data.

Objetivo: Investigar o papel da fisioterapia na condução diagnóstica e terapêutica de pacientes chagásicos com constipação intestinal crônica. Métodos: Trata-se de um estudo de revisão sistemática que seguiu a Declaração PRISMA (2020). Com isso, realizou-se uma busca de alta sensibilidade nas bases de dados Medline (PubMed), Web Of Science, Cochrane Library, Embase (Elsevier), Biblioteca Virtual em Saúde (BVS), mecanismos de busca do Google Scholar e PEDro. Além disso, contou com a busca na literatura cinzenta, nas bases CAPES e Open Grey, e nas listas de referenciais teóricos de estudos primários. Resultados: A busca sistemática de alta sensibilidade resultou em 479 estudos. Após aplicação dos critérios de elegibilidade, foram incluídos quatro estudos primários. Esta revisão abarcou o total de 112 pacientes chagásicos com CI crônica, submetidos às intervenções fisioterapêuticas, tanto diagnósticas, com a realização do exame físico longitudinal e uso da manometria anorretal, quanto terapêuticas, com uso da massoterapia abdominal e prática de exercícios físicos supervisionados. Considerações Finais: Os protocolos e modalidades fisioterapêuticas avaliadas trazem benefícios substanciais ao paciente chagásico constipado, proporcionando-lhe melhor qualidade de vida.

Introduction Chagas disease (CD) is a global health concern with approximately 12 000 deaths per year worldwide. In the chronic phase, about 30% of patients develop the cardiac clinical form, which presents symptoms associated with the presence of inflammatory cells in the cardiac tissue. Neutrophils are inflammatory cells able to modulate the chronic immune response against pathogens. These cells are capable of interacting with Trypanosoma cruzi, the aetiological agent of CD, and perform several effector functions, such as NET release. However, few studies have been carried out to investigate the role of these cells in the disease. Aims To investigate the release of NETs by neutrophils from CD patients by measuring the amount of DNA and elastase released. Methods and results Measurement of DNA release by neutrophils from chronic CD patients presenting the indeterminate (IND group; n = 18) and cardiac (CARD group; n = 15) clinical forms and nonchagasic subjects (n = 18) stimulated with soluble antigen of T. cruzi was quantified using the Quant-iT™ PicoGreen® dsDNA assay kit. Patients from CARD group release less DNA (117.3 ± 21.85 ng/mL; *P = .0131) than neutrophils from control (177.7 ± 58.41 ng/mL). Elastase enzyme degranulation was measured using the substrate N-methoxysuccinyl-Ala-Ala-Pro-Val p-nitroanilide (SAAVNA). Absorbance values of elastase degranulation activity showed that only cells from healthy individuals presented a high release profile of elastase. Also, we found a negative correlation between DNA released concentration and risk of death (r = -.6574; *P = .0173); the lower the neutrophil DNA release from chagasic patients with cardiac event, the higher the risk of death. Conclusion These preliminary data show that patients with the cardiac form of CD release less NETs than nonchagasic individuals, raising the possibility that lower release of NETs enhances risk of death in CD patients with cardiac events.

Introdução: O linfedema relacionado ao câncer de mama (LRCM) é uma condição crônica que pode surgir em decorrência de alterações na estrutura e função linfática provenientes do tratamento desta neoplasia. O tratamento padrão-ouro desta sequela é a terapia física complexa (TFC); entretanto, pode haver dificuldade de adesão a esta terapêutica por fatores como temperatura, aplicação e estética. Nesse contexto, o Linfotaping® é uma terapêutica alternativa. Objetivo: Avaliar a eficiência do Linfotaping® no tratamento do LRCM, analisando os seus efeitos na qualidade de vida, volume e funcionalidade dos braços afetados. Metodologia: Trata-se de uma revisão da literatura, com busca sistemática, nas bases PubMed, Web of Science, Embase, Scopus, PEDro, SciELO e LILACS. Os termos de busca foram “Breast Cancer Lymphedema, Athletic Tape, Rehabilitation, Recovery of Function, Quality of Life e Clinical Laboratory Techniques”. Inicialmente, obtiveram-se 8.363 publicações iniciais. Após a eliminação das duplicatas e aplicação dos critérios de inclusão e exclusão ficaram 17 artigos. Resultados: o Linfotaping® pode promover benefícios na qualidade de vida e no volume e funcionalidade dos braços acometidos pelo LRCM, apesar de haver resultados conflitantes que podem estar relacionados às diferenças metodológicas dos artigos analisados. Conclusão: Alguns estudos mostraram que o Linfotaping® se apresentou como uma alternativa terapêutica interessante para o LRCM. No entanto, faz-se necessária a obtenção de evidências mais sólidas, com a utilização de ensaios clínicos randomizados adicionais, amostragem maior, populações melhor definidas, metodologias mais descritivas e a utilização de outros parâmetros clínicos e laboratoriais para avaliar o efeito da terapia.

Objectives To examine whether the hydroalcoholic extract from Baccharis dracunculifolia leaves (BdE) modulates the human neutrophil oxidative metabolism, degranulation, phagocytosis and microbial killing capacity. Methods In-vitro assays based on chemiluminescence, spectrophotometry, flow cytometry and polarimetry were used, as well as docking calculations. Key findings At concentrations that effectively suppressed the neutrophil oxidative metabolism elicited by soluble and particulate stimuli ( Conclusions BdE selectively modulates the effector functions of human neutrophils, inhibits the activity of key enzymes and scavenges physiological oxidant species. Caffeic acid contributes to lower the levels of oxidant species. Our findings help to unravel the mechanisms by which these natural products exert immunomodulatory action towards neutrophils.

The establishment of physical training programs for people living with HIV/AIDS (PLWHA) has several benefits. The study aimed to analyze the effect of resistance training using prediction of intensity by subjective perception of effort (SPE) on body composition, muscle strength, and TCD4+ lymphocyte levels in PLWHA. This a randomized controlled trial study. Participants (11 men and 8 women), were divided in two groups: exercise group (EG) and control group (CG). The EG was submitted to 12 weeks of a resistance-training program based in the prediction of intensity by SPE. Body mass percentages were evaluated using tetrapolar bioelectrical impedance and skinfold methods. We used Flow Cytometry to quantify CD4+ T lymphocytes. Patients showed significant changes in Body Fat Percentage (Δ%=-6.23%), Lean Body Mass (Δ%=2.45%), and CD4T lymphocytes levels (Δ%=15.77%). They also showed significant increase in muscular strength presented in the test for one repetition maximum in all the evaluated exercises. Our data suggest that exercising program prescribed by SPE is capable of improving immune function, body composition, and muscular strength in PLWHA.

Este estudo buscou investigar a contribuição do Peer-Mentoring na formação acadêmica de base e educação continuada de profissionais da saúde, bem como identificar as principais limitações, desafios e alternativas para atingir o sucesso de sua implementação. Trata-se de um estudo de revisão integrativa com busca sistemática, realizada nas bases de dados PubMed, Scopus, Web Of Science, Education Resources Information Center (ERIC) e Biblioteca Virtual em Saúde (BVS). Foram incluídos artigos eletrônicos, disponíveis na íntegra, nos idiomas português, inglês e/ou espanhol e publicados nos últimos 5 anos. Excluiu-se os estudos do tipo revisão de literatura, resumos, carta ao editor, manuscritos duplicados e estudos que não apresentaram relação com a temática. Os seguintes descritores foram combinados com os operadores booleanos AND e OR: “Peer-Mentoring”, “Peer Group”, “Mentoring”, “Mentors”, “Health Education” e "Education, Medical”. Foram coletados 31 artigos elegíveis, que passaram por uma análise na íntegra e fichamento dos dados. Os estudos foram agrupados em cinco categorias de análise: (1) contribuição do Peer-Mentoring no processo de formação acadêmica; (2) impacto do Peer-Mentoring na educação continuada de profissionais da saúde educação continuada; (3) limitações são encontradas durante a execução do Peer-Mentoring; (4) desafios pedagógicos que devem ser vencidos, no transcurso da mentoria entre pares; (5) Como atingir o sucesso durante a implantação e execução de programas de Peer-Mentoring?. O Peer-Mentoring mostrou-se capaz de melhorar significativamente o processo de ensino em saúde. Perspectivas futuras apontam para uma maior adesão, por parte das instituições de ensino, em inserir a mentoria entre pares nos programas pedagógicos.

Objetivos: O presente trabalho pretende apresentar um protocolo de uma revisão sistemática que busca comparar as alterações hemodinâmicas ocorridas em pacientes que foram submetidos à técnica de sedação consciente por via oral com o diazepam versus o midazolam, durante a exodontia de terceiros molares inferiores. Métodos: Protocolo para uma revisão sistemática de ensaios clínicos randomizados, sendo utilizado as bases de dados Embase, PubMed/MEDLINE, Cochrane Library, Portal Regional da BVS, literatura cinzenta e buscas de forma manual nos principais periódicos da área de cirurgia bucomaxilofacial. O referido protocolo foi registrado no International Prospective Register of Systematic Reviews (PROSPERO), ressaltando-se que esta revisão será conduzida de acordo com as diretrizes do Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Considerações finais: A necessidade de se manter a maior estabilidade hemodinâmica durante a exodontia de terceiros molares inferiores, frente a sedação consciente por via oral, torna relevante que esta revisão sistemática seja realizada.

Objetivou-se identificar o perfil de funcionalidade da pessoa idosa brasileira residente na comunidade acompanhada pelas equipes de saúde da atenção primária. Tratou-se de revisão integrativa desenvolvida entre março e julho de 2020, utilizando-se a estratégia de busca “Avaliação geriátrica” AND “Atenção Primária à Saúde”; “Geriatric Assessment” AND “Primary Health Care”. Obteve-se um achado inicial de 8.146 artigos que, após aplicação dos filtros e dos critérios de elegibilidade, cinco artigos foram incluídos para a análise final, considerando os aspectos físicos e psicológicos da pessoa idosa. Observou-se que fatores sociodemográficos influenciam diretamente na dependência do idoso. Sintomas depressivos, número de comorbidades, prática atividade física, auto avaliação da sua saúde e amparo familiar são influenciadores para fragilidade e dependência desencadeando baixa funcionalidade física e psicológica da pessoa idosa comprometendo sua autonomia para desempenhar funções do dia a dia. Concluiu-se que a funcionalidade do idoso é diretamente influenciada por fatores extrínsecos, sendo necessário um redirecionamento das práticas profissionais para a efetivação de uma atenção integral à saúde.

A Realidade Virtual é umas das tecnologias mais inovadoras da atualidade, e pode ser aplicada na educação em diversas áreas do conhecimento, incluindo a formação de profissionais da saúde. Na educação em saúde, esta aplicação é foco de diversas pesquisas, por isso a necessidade de um estudo bibliométrico sobre o tema a fim de identificar os pontos de interesse neste tópico. O objetivo desse estudo é apresentar resultados de uma análise bibliométrica sobre uso da realidade virtual na docência em saúde e relatar as características destes estudos. Os artigos foram buscados na Web Of Science, e foram incluídos aqueles publicados entre 2004 e 2020 sobre o tema. Para a análise dos dados foram usados os softwares Histcite© e VOSviewer. No total, 1705 artigos foram identificados. A análise das características das publicações e das redes de co-citação e co-ocorrência de palavras mostrou que o principal uso dessa tecnologia no ensino da área da saúde vem sendo o treinamento de estudantes de cirurgia, e resultados positivos no aumento da performance dos alunos já foram identificados por vários estudos. Também foi possível identificar que em 2020, houve um aumento significativo no número de publicações sobre esse tema, possivelmente devido à pandemia do vírus SARS-CoV-2 e a consequente necessidade da implementação de novas abordagens metodológicas no ensino em saúde. Assim, a realidade virtual pode ser considerada uma ferramenta importante na formação e treinamento de novos profissionais da saúde, auxiliando na aquisição de conhecimento e habilidades em diversas áreas desse campo, principalmente na área cirúrgica.

Objetivo: Realizar uma revisão de literatura, com busca sistemática, observando as evidências quanto ao impacto da doença periodontal na qualidade de vida. Metodologia: A pesquisa foi feita nas bases de dados Medline/Pubmed, SciELO, LILACS, Web of Science e Scopus, nas quais utilizou-se os descritores “Periodontal Diseases” e “Quality of Life”. Foram incluídos artigos publicados em português, inglês e espanhol que retrataram a temática no período compreendido entre 2015-2020. Excluiu-se os artigos duplicados e todos os estudos de revisão. Inicialmente, identificou-se 1435 artigos, restando 36 artigos após a observação dos critérios de elegibilidade. Resultados: A doença periodontal causa um impacto negativo na qualidade de vida relacionada à saúde oral, visto que, do total de artigos (n=36), trinta evidenciaram essa afirmativa, e quanto mais severa é a destruição periodontal, pior é a qualidade de vida (n=11). Entretanto, alguns estudos (n=4) não encontraram relações significativas entre a doença e a qualidade de vida ou obtiveram baixo impacto. Além disso, há resultados com alto impacto na qualidade de vida nos seguintes grupos: fumantes, ausência de seguro de saúde, obesos e diabéticos, grávidas, ansiosos, idosos, pacientes com doença renal terminal, cárie, hipertensão arterial. Conclusão: As doenças periodontais interferem negativamente na qualidade de vida das pessoas afetadas e há evidências de quanto maior a severidade da doença, maior é o seu impacto na qualidade de vida. Contudo, são necessários estudos clínicos randomizados e estudos do tipo longitudinal para fortalecer esses achados.

The aerial parts of Baccharis dracunculifolia (BdE) is used in the Brazilian folk medicine to treat inflammatory conditions. Here we examined the ability of free and liposomal BdE to modulate reactive oxygen species generation in human neutrophils in vitro and zymosan-induced acute joint inflammation in Wistar rats. We prepared biocompatible liposomes of soya phosphatidylcholine and cholesterol with low diameter, homogeneous size distribution, and neutral surface charge. Free BdE decreased joint swelling, total leucocyte and neutrophil infiltration, and the synovial levels of tumour necrosis factor-α and interleukins 6 and 1β. Incorporation of BdE into liposomes preserved its capacity to inhibit the neutrophil superoxide anion and total reactive oxygen species generation, and improved its anti-inflammatory effect in vivo by decreasing the effective BdE dose by nearly sixfold. The same liposome type lowered the effective dose of caffeic acid by nearly sixteenfold. Therefore, incorporation of BdE into phosphatidylcholine-cholesterol liposomes improves its anti-inflammatory effect.

In this study, we report the ability of a set of eight 3-phenylcoumarin derivatives bearing 6,7- or 5,7-dihydroxyl groups, free or acetylated, bound to the benzopyrone moiety, to modulate the effector functions of human neutrophils. In general, (i) 6,7-disubstituted compounds (5, 6, 19, 20) downmodulated the Fcγ receptor-mediated neutrophil oxidative metabolism more strongly than 5,7-disubstituted compounds (21, 22, 23, 24), and (ii) hydroxylated compounds (5, 19, 21, 23) downmodulated this neutrophil function more effectively than their acetylated counterparts (6, 20, 22, 24, respectively). Compounds 5 (6,7-dihydroxy-3-[3',4'-methylenedioxyphenyl]-coumarin) and 19 (6,7-dihydroxy-3-[3',4'-dihydroxyphenyl]-coumarin) effectively downmodulated the neutrophil oxidative metabolism elicited via Fcγ and/or complement receptors. Compound 5 also downmodulated the immune complex-stimulated phagocytosis, degranulation of elastase, and production and release of neutrophil extracellular traps, as well as the human neutrophil chemotaxis towards n-formyl-methionyl-leucyl-phenylalanine, without altering the expression level of formyl peptide receptor type 1. Both compounds 5 and 19 did not impair the neutrophil capacity to recognize and kill Candida albicans. Docking calculations revealed that compounds 5 and 19 directly interacted with three catalytic residues - Gln-91, His-95, and Arg-239 - inside the myeloperoxidase active site. Together, these findings indicate that (i) inhibition of reactive oxygen species generation and degranulation of elastase are closely associated with downmodulation of release of neutrophil extracellular traps; and (ii) compound 5 can be a prototype for the development of novel immunomodulating drugs to treat immune complex-mediated inflammatory diseases.

Os neutrófilos são os leucócitos circulantes mais abundantes. Apesar de serem importantes no combate às infecções, o intenso recrutamento e a consequente ativação dessas células levam à liberação de mediadores inflamatórios que estão relacionados com o agravamento do quadro clínico de inúmeras doenças. Dessa forma, nos últimos anos tem-se intensificado a procura por substâncias terapêuticas que minimizem os danos teciduais ocasionados pela infiltração neutrofílica. Estudos prévios do grupo de pesquisa mostraram que as 3-fenilcumarinas, que constituem uma classe de produtos naturais de origem vegetal, são substâncias promissoras como moduladores do metabolismo oxidativo de neutrófilos. Em continuidade a essas investigações, o derivado sintético 3-fenilcumarínico 6,7-diidroxi-3-[3\',4\'- metilenodioxifenil]-cumarina (C13) foi selecionado, por apresentar o grupo substituinte ortodiidroxi nas posições C-6 e C-7 do esqueleto cumarínico e a metilenodioxila ligada ao anel fenílico, no intuito de avaliar o seu efeito nas demais funções efetoras dos neutrófilos, como também em um modelo animal de inflamação articular. Foi avaliado o efeito modulatório in vitro da C13 sobre a quimiotaxia, a produção de ERO e interação com o sítio ativo da mieloperoxidase (MPO), a fagocitose de imunocomplexos (IC), a desgranulação da enzima elastase, a atividade microbicida sobre Candida albicans, a mobilização e o influxo de cálcio, a polimerização do citoesqueleto, e a formação e liberação das NETs em neutrófilos humanos. A C13 inibiu, de forma dependente da concentração, o metabolismo oxidativo de neutrófilos estimulados via receptores de IgG (Fc?R) e/ou de complemento (CR), utilizando-se diferentes tipos de IC e foi capaz de interagir com o sítio ativo da MPO. Nas concentrações próximas da necessária para inibir 50% do metabolismo oxidativo (~ 2µmol/L), a C13 não interferiu nas demais funções efetoras dos neutrófilos avaliadas. No entanto, na maior concentração avaliada (20 µmol/L), a C13 inibiu cerca de: 30% da capacidade das células de migrar a favor dos agentes quimiotáticos n-formil-metionil-leucil-fenilalanina (fMLP) e leucotrieno B4; 70% da fagocitose de IC mediada por CR; 40% e 80% da desgranulação estimulada por fMLP e IC imobilizados, respectivamente; 50% do processo de formação e liberação das NETs. Esta concentração de C13 também interferiu na polimerização do citoesqueleto de actina, mas não inibiu: a quimiotaxia de neutrófilos frente à interleucina (IL) 8; a capacidade fagocítica de IC estimulada via Fc?R e Fc?R+CR; e a atividade microbicida sobre C. albicans. Na segunda parte deste trabalho, foi avaliada a toxicidade da C13 sobre neutrófilos mantidos em condições de cultura celular por 6 e 12 h. Nos períodos avaliados, essa substância não alterou a viabilidade dos neutrófilos. Por último, a C13 foi incorporada em vesículas lipossomais e sua atividade biológica foi avaliada em ratos Wistar com inflamação articular induzida por zimosan. O tratamento dos animais com a C13 lipossomal (1mg/Kg) reduziu a formação do edema e a infiltração de leucócitos e neutrófilos na sinóvia inflamada, mas não alterou a concentração sinovial das citocinas inflamatórias fator de necrose tumoral ?, IL-1? e IL-6. Portanto, o derivado 3-fenilcumarínico C13 pode servir de protótipo para o desenvolvimento de novos agentes terapêuticos com aplicação no tratamento de doenças onde há intensa participação dos neutrófilos. Although neutrophils are important to fight against infections, the massive recruitment and consequent activation of these cells result in the release of inflammatory mediators that are associated with worsening of the clinical condition in many diseases. In this sense, the search for new therapeutic compounds that minimize tissue damage caused by neutrophil infiltration has been intensified in the past few years. Previous studies have demonstrated that 3-phenylcoumarins, a class of plantderived natural compounds, are promising modulators of neutrophil oxidative metabolism. To continue these investigations, we selected the 3-phenylcoumarin derivative 6,7-dihydroxy-3- [3?,4?-methylenedioxyphenyl]-coumarin (C13), bearing the 6,7-dihydroxyl and the 3?,4?- methylenedioxyl groups, to further assess its effects on the other effector functions of neutrophils, as well as on in an animal model of articular inflammation. We examined the in vitro modulator effect of C13 on the human neutrophil chemotaxis, production of ROS and interaction with active site of myeloperoxidase (MPO), phagocytosis of immune complexes (IC), degranulation, killing of Candida albicans, calcium mobilization and influx, cytoskeleton polymerization, and the formation and release of NETs. C13 inhibited, in a concentration-dependent manner, the neutrophil ROS generation elicited via IgG (Fc?R) and/ or complement (CR) receptors using different types of IC and it interacted with active site of MPO. At concentrations near to that required to suppress the ROS production by 50% (~ 2µmol/L), C13 did not modulate the other neutrophil effector functions assessed. However, at the highest concentration tested (20 µmol/L), C13 inhibited nearly: 30% of the cell ability to migrate towards n-formyl-methionyl-leucyl-phenylalanine (fMLP) and leukotriene B4; 70% of CR-mediated phagocytosis of IC; 40% and 80% of cell degranulation triggered by fMLP and immobilized IC, respectively; and 50% of the NETs formation and release process. Such C13 concentration also interfered in the actin cytoskeleton polymerization, but it did not suppress the neutrophil: chemotaxis towards interleukin (IL) 8; capacity to phagocytose IC elicited via Fc?R and Fc?R+CR; and killing of C. albicans. In the second part of this study, we examined whether C13 was toxic towards neutrophils maintained in culture for 6 and 12 h. At the time points assessed, this compound did not change the viability of neutrophils. Finally, we incorporated C13 into liposomes and examined its biological activity in Wistar rats of zymosan-induced articular inflammation. Treatment of animals with liposomal C13 reduced edema formation, and leukocyte and neutrophil infiltration in the inflamed synovia, but it did not change the synovial concentration of the inflammatory cytokines tumor necrosis factor ?, IL-1?, and IL-6. Therefore, the 3-phenylcoumarin derivative C13 can be a prototype for the development of novel therapeutic agents to be used in the treatment of diseases where neutrophils have intense participation.

Chagas disease (CD) is a public health problem in Latin America. The acute phase presents nonspecific symptoms and most patients recover from acute parasitemia and undergo a prolonged asymptomatic phase. Several years later, about 30% of infected individuals develop chronic cardiopathy with progressive cardiomegaly, arrhythmia, thromboembolic events and heart failure. These symptoms suggest a persistent association with the presence of inflammatory infiltrate and tissue, and cellular destruction in the heart muscle. Nevertheless, few research studies have attempted to understand the role of inflammatory cells, such as neutrophils, in establishing the pathology and progression of CD. Only recently have some studies been performed with this intention. Despite this effort, the role of neutrophils in CD is still considered controversial. This review discusses the morphological and functional characteristics of neutrophils that describes their participation in the establishment and progression of Trypanosoma cruzi infection, through the development of its effector functions, such as release of lithic components, production of oxidative agents and release of inflammatory mediators capable of modulating the host immune response.

Rheumatoid arthritis (RA) is a highly disabling disease that affects all structures of the joint and significantly impacts on morbidity and mortality in RA patients. RA is characterized by persistent inflammation of the synovial membrane lining the joint associated with infiltration of immune cells. Eighty to 90% of the leukocytes infiltrating the synovia are neutrophils. The specific role that neutrophils play in the onset of RA is not clear, but recent studies have evidenced that they have an important participation in joint damage and disease progression through the release of proteolytic enzymes, reactive oxygen species (ROS), cytokines, and neutrophil extracellular traps, in particular during frustrated phagocytosis of immune complexes (ICs). In addition, the local and systemic activation of the complement system contributes to the pathogenesis of RA and other IC-mediated diseases. This review discusses (i) the participation of Fcγand complement receptors in mediating the effector functions of neutrophils in RA; (ii) the contribution of the complement system and ROS-dependent and ROS-independent mechanisms to joint damage in RA; and (iii) the use of plant extracts, dietary compounds, and isolated natural compounds in the treatment of RA, focusing on modulation of the effector functions of neutrophils and the complement system activity and/or activation.

Rheumatoid arthritis (RA) patients usually exhibit immune complex (IC) deposition and increased neutrophil activation in the joint. In this study, we assessed how four flavonols (galangin, kaempferol, quercetin, and myricetin) modulate the effector functions of healthy individuals' and active RA patients' IC-stimulated neutrophils. We measured superoxide anion and total reactive oxygen species production using lucigenin (CL-luc)- and luminol (CL-lum)-enhanced chemiluminescence assays, respectively. Galangin, kaempferol, and quercetin inhibited CL-lum to the same degree (mean IC50=2.5 μM). At 2.5 μM, quercetin and galangin suppressed nearly 65% CL-lum of active RA patients' neutrophils. Quercetin inhibited CL-luc the most effectively (IC50=1.71±0.36 μM). The four flavonols diminished myeloperoxidase activity, but they did not decrease NADPH oxidase activity, phagocytosis, microbial killing, or cell viability of neutrophils. The ability of the flavonols to scavenge hypochlorous acid and chloramines, but not H2O2, depended on the hydroxylation degree of the flavonol B-ring. Therefore, at physiologically relevant concentrations, the flavonols partially inhibited the oxidative metabolism of IC-stimulated neutrophils without affecting the other investigated effector functions. Using these compounds to modulate IC-mediated neutrophil activation is a promising safe therapeutic strategy to control inflammation in active RA patients.

Immune complex (IC) deposition in tissues triggers the release of harmful oxidant and lytic compounds by neutrophils. We examined how ten 3-phenylcoumarin derivatives affect the reactive oxygen species (ROS) production by IC-stimulated human neutrophils. Most of the 3-phenylcoumarins inhibited the luminol-enhanced chemiluminescence (CL-lum) more strongly than they inhibited the lucigenin-enhanced chemiluminescence (CL-luc), without clear signs of toxicity. The most effective CL-lum inhibitors, 6,7-dihydroxy-3-[3′,4′-methylenedioxyphenyl]-coumarin (5) and 6,7-dihydroxy-3-[3′,4′-dihydroxyphenyl]-coumarin (19), also inhibited myeloperoxidase activity more potently and had higher hypochlorous acid scavenging ability, but did not affect the NADPH-oxidase activity. The type, number, and position of the substituent influenced the pharmacological effects of 3-phenylcoumarins; however, the structural requirements for CL-lum and CL-luc inhibition were a little different. Compounds 5 and 19 are promising prototypes of therapeutic molecules to modulate ROS production by neutrophils in IC-mediated inflammatory diseases.

This article shows how six vehicles interfere in the oxidation of 3,3',5,5'-tetramethylbenzidine (TMB) by hypochlorous acid (HOCl) and taurine chloramine in vitro. All the tested vehicles inhibited TMB oxidation by HOCl; dimethyl sulfoxide had a remarkable effect at concentrations as low as 0.00005% (v/v). Cremophor EL and ethanol inhibited TMB oxidation by taurine chloramine at concentrations higher than 0.05 and 25% (v/v), respectively; the other vehicles did not affect this reaction. The results will help to guide the choice of solvent for the TMB oxidation assay performed under viable experimental conditions for evaluation of the HOCl and taurine chloramine scavenging ability of drugs.

A produção de espécies reativas de oxigênio (ERO) pelos neutrófilos, que representa um mecanismo essencial da imunidade inata contra agentes infecciosos, pode ser desencadeada por imunocomplexos (IC) associados ou não com componentes do sistema complemento. Os IC são normalmente eliminados da circulação pela ligação aos receptores de complemento do tipo 1 em eritrócitos, que os transportam até o baço e o fígado, onde são fagocitados por macrófagos residentes. Porém, defeitos neste mecanismo de eliminação podem levar à deposição de IC nos vasos e tecidos. Em algumas doenças inflamatórias, onde ocorre uma intensa deposição de IC, pode acontecer o recrutamento e a ativação maciça de neutrófilos pelos IC. Nessa situação, os mecanismos endógenos de defesa tecidual não conseguem controlar as alterações desencadeadas pelos fatores oxidantes e líticos liberados pelos neutrófilos, levando à possível lesão tecidual e consequências funcionais e estruturais para o organismo. Nessa perspectiva, tem-se intensificado, nos últimos anos, a procura por novos compostos terapêuticos com capacidade de reduzir os danos aos tecidos adjacentes ao acúmulo de IC, neutrófilos, ERO e enzimas líticas, expostos a níveis deletérios desses componentes. Assim, neste trabalho foi avaliado o potencial antioxidante de um conjunto de dez derivados 3-fenilcumarínicos no metabolismo oxidativo de neutrófilos humanos estimulados por ICs, empregando-se o ensaio de quimioluminescência dependente de luminol e de lucigenina (QLlum e QLluc, respectivamente). Posteriormente, foi avaliada a citoxicidade das amostras, através do ensaio de exclusão do azul de tripan e medida da atividade da lactato desidrogenase liberada, bem como sua capacidade de sequestrar HOCl e inibir a atividade das enzimas NADPH oxidase e mieloperoxidase (MPO). Observou-se que todas as amostras avaliadas inibiram a QLlum e a QLluc, sendo que cinco delas tiveram efeito inibitório maior que ou igual a 50% (C6, C13, C13a, C24 e C24a)*. Este efeito biológico não foi mediado pela citotoxicidade das 3-fenilcumarinas, nas condições avaliadas. Dentre essas amostras, o derivado 3-fenilcumarínico C13, que foi o mais efetivo em inibir a QLluc, possui duas características que parecem estar relacionadas com tal efeito: um grupo substituinte orto-diidroxi no esqueleto cumarínico e um grupo metilenodioxila no anel fenílico. Além do mais, todas as substâncias que possuem o grupo metilenodioxila no anel fenílico (C1, C6, C6a, C13, C13a) apresentaram efeito inibitório da QLluc maior que as respectivas análogas sem este substituinte (C15, C18, C18a, C24, C24a). A inibição da QLluc parece ser independente da inibição da atividade da NADPH oxidase, visto que as substâncias apresentaram efeito inibitório discreto no consumo de O2 pelos neutrófilos estimulados por IC. Por outro lado, as amostras C13 e C24, que têm em comum a presença do grupo substituinte 6,7-diidroxi, apresentaram os maiores efeitos inibitórios sobre a QLlum, apresentando IC50 semelhante. Essas duas substâncias também foram as mais ativas em inibir a atividade da enzima MPO e em sequestrar o HOCl. Para esses dois ensaios, a atividade biológica observada foi dependente do número de hidroxilas na estrutura do derivado 3-fenilcumarínico. Os resultados deste trabalho mostram que a atividade antioxidante dos derivados 3-fenilcumarínicos sobre o metabolismo oxidativo de neutrófilos humanos está relacionada às características estruturais dessas moléculas. Dentre a série de substâncias avaliadas, C13 e C24 são as amostras mais promissoras para uso como protótipos de moléculas com aplicação terapêutica na modulação desta função neutrofílica. Production of reactive oxygen species (ROS) by neutrophils, which is an essential mechanism of innate immunity against infectious agents, can be triggered by immune complexes (IC) associated or not with components of the complement system. IC are normally cleared from the circulation by binding to complement receptor type 1 on erythrocytes and transport to spleen and liver, where they are phagocytized by resident macrophages. However, defects in this clearance mechanism can lead to deposition of IC in vessels and tissues, favoring neutrophil chemotaxis. In some inflammatory diseases there is intense deposition of IC, which triggers massive recruitment and activation of neutrophils. In these situations, the endogenous tissue defense mechanisms are not able to regulate the local disorders provoked by the oxidant and lytic compounds released by neutrophils, and tissue damage as well as structural and functional consequences to the organism might occur. In this perspective, the last years have been marked by an intense search for new therapeutic compounds able to reduce the damage to tissues surrounding the site of accumulation of IC, neutrophils, ROS and lytic enzymes, exposed to deleterious levels of these components. So, the present work reports the evaluation of the antioxidant potential of ten 3-phenylcoumarin derivatives in the neutrophil oxidative metabolism stimulated with IC, through the luminol- and lucigenin-dependent chemiluminescence assay (QLlum and QLluc, respectively). Further, cytotoxicity of the samples was evaluated by the trypan blue exclusion assay and measurement of lactate dehydrogenase activity, as well as their ability to scavenge HOCl and inhibit NADPH-oxidase and myeloperoxidase (MPO) activity. We found that all the samples inhibited QL-lum and QLluc, being the inhibitory effect of five of them higher than or equal to 50% (C6, C13, C13a, C24 e C24a)*. This biological effect was not mediated by cytotoxicity of the 3-phenylcoumarins, under the conditions assessed. Compound C13, which was the most effective inhibitor of QLluc among the set of phenylcoumarins tested, has two structural features that seem to be related to this effect: an orto-dihydroxyl group in the coumarin moiety and a methylenedioxyl group attached to the phenyl ring. Moreover, the inhibitory effect of all 3-phenylcoumarins bearing the methylenedioxyl group (C1, C6, C6a, C13, C13a) was higher than that found to their analogues without this group (C15, C18, C18a, C24, C24a). The CLluc inhibition by phenylcoumarins seems to be independent of inhibition of NADPH-oxidase activity, since these compounds had a slight inhibitory effect in the O2 consumption by IC-stimulated neutrophils. On the other hand, compounds C13 and C24, which shares the 6,7-dihydroxyl group, had the highest inhibitory effects on CLlum with similar IC50 values among the samples tested. These compounds were also the most active ones to inhibit MPO activity and scavenge HOCl. Such biological effects were dependent on the number of hydroxyl groups in the 3-phenylcoumarin moiety. The results of this work show that the antioxidant activity of 3-phenylcoumarin derivatives in the oxidative metabolism of human neutrophils is related to the structural characteristics of these molecules. Among the set of compounds evaluated, C13 and C24 are the most promising samples to be used as prototypes of moleculse with therapeutic application in the modulation of neutrophil function.

Increased neutrophil activation significantly contributes to the tissue damage in inflammatory illnesses; this phenomenon has motivated the search for new compounds to modulate their effector functions. Coumarins are natural products that are widely consumed in the human diet. We have evaluated the antioxidant and immunomodulator potential of five 4-methylcoumarin derivatives. We found that the 4-methylcoumarin derivatives inhibited the generation of reactive oxygen species by human neutrophils triggered by serum-opsonized zymosan or phorbol-12-myristate-13-acetate; this inhibition occurred in a concentration-dependent manner, as revealed by lucigenin- and luminol-enhanced chemiluminescence assays. Cytotoxicity did not mediate this inhibitory effect. The 7,8-dihydroxy-4-methylcoumarin suppressed the neutrophil oxidative metabolism more effectively than the 6,7- and 5,7-dihydroxy-4-methylcoumarins, but the 5,7- and 7,8-diacetoxy-4-methylcoumarins were less effective than their hydroxylated counterparts. An analysis of the biochemical pathways suggested that the 6,7- and 7,8-dihydroxy-4-methylcoumarins inhibit the protein kinase C-mediated signaling pathway, but 5,7-dihydroxy-4-methylcoumarin, as well as 5,7- and 7,8-diacetoxy-4-methylcoumarins do not significantly interfere in this pathway of the activation of the human neutrophil oxidative metabolism. The 4-methylcoumarin derivatives bearing the catechol group suppressed the elastase and myeloperoxidase activity and reduced the 1,1-diphenyl-2-picrylhydrazyl free radical the most strongly. Interestingly, the 5,7-dihydroxy-4-methylcoumarin scavenged hypochlorous acid more effectively than the o-dihydroxy-substituted 4-methylcoumarin derivatives, and the diacetoxylated 4-methylcoumarin derivatives scavenged hypochlorous acid as effectively as the 7,8-dihydroxy-4-methylcoumarin. The significant influence of small structural modifications in the inhibitory potential of 4-methylcoumarin derivatives on the effector functions of neutrophil makes them interesting candidates to develop new drugs for the treatment of inflammatory diseases mediated by increased neutrophil activation.
