1. Evolution of clusters of turbulent reattachment due to shear layer instability in flow past a circular cylinder

Abstract

We perform large eddy simulations of flow past a circular cylinder for the Reynolds number ($Re$) range, $2\times 10^3 \leq Re \leq 4\times10^5$, spanning subcritical, critical and supercritical regimes. We investigate the spanwise coherence of the flow in the critical and supercritical regimes using complex networks. In these regimes, the separated flow reattaches to the surface in a turbulent state due to the turbulence generated by the shear layer instability. In the early critical regime, the turbulent reattachment does not occur simultaneously at all span locations. It occurs incoherently along the span in clusters. We treat strong surface pressure fluctuations due to the shear layer instability as extreme events and construct time-varying spatial proximity networks where links are based on synchronization between events. This analysis unravels the underlying complex spatio-temporal dynamics by enabling the estimation of characteristics of clusters of turbulent reattachment via the concept of connected components. In the critical regime, the number and size of the clusters increase with increase in $Re$. At higher $Re$ in the supercritical regime, they coalesce to form bigger clusters, resulting in increase in spanwise coherence of turbulent reattachment. We find that the size and number of clusters govern the variation of the time-averaged coefficient of drag ($\overline{C}_D$) in the critical and supercritical regimes. $\overline{C}_D$ exhibits power-law distribution with the most probable cluster size ($\overline{C}_D \propto E(S_C)^{-\frac{2}{5}}$) and the largest cluster size ($\overline{C}_D \propto {\overline{S}_{CL}}^{-\frac{2}{5}}$)., Comment: 23 pages, 11 figures

Abstract

BackgroundApproximately half of the patients presenting with new-onset heart failure (HF) have HF with preserved left ventricular ejection fraction (HFpEF) and HF with mid-range left ventricular ejection fraction (HFmrEF). These patients have neurohormonal activation like that of HF with reduced ejection fraction; however, beta-blockers and angiotensin-converting enzyme inhibitors have not been shown to improve their outcomes, and current treatment for these patients is symptom based and empiric. Sympathoinhibition using parasympathetic stimulation has been shown to improve central and peripheral aspects of the cardiac nervous system, reflex control, induce myocyte cardioprotection, and can lead to regression of left ventricular hypertrophy. Beneficial effects of autonomic regulation therapy (ART) using vagus nerve stimulation (VNS) have also been observed in several animal models of HFpEF, suggesting a potential role for ART in patients with this disease.MethodsThe Autonomic Neural Regulation Therapy to Enhance Myocardial Function in Patients with Heart Failure and Preserved Ejection Fraction (ANTHEM-HFpEF) study is designed to evaluate the feasibility, tolerability, and safety of ART using right cervical VNS in patients with chronic, stable HFpEF and HFmrEF. Patients with symptomatic HF and HFpEF or HFmrEF fulfilling the enrolment criteria will receive chronic ART with a subcutaneous VNS system attached to the right cervical vagus nerve. Safety parameters will be continuously monitored, and cardiac function and HF symptoms will be assessed every 3 months during a post-titration follow-up period of at least 12 months.ConclusionsThe ANTHEM-HFpEF study is likely to provide valuable information intended to expand our understanding of the potential role of ART in patients with chronic symptomatic HFpEF and HFmrEF.

Abstract

Vortex-induced vibration (VIV) of a low mass ratio circular cylinder ( m * = 10), that is free to vibrate in crossflow and in-line directions, at supercritical Reynolds number (3 × 10 5 ) has been studied using large-eddy simulation for a range of reduced speed (2 ≤ U * ≤ 11). In the supercritical regime, the boundary layer transitions to a turbulent state via the formation of a laminar separation bubble (LSB). The regime is associated with weakened vortex shedding, resulting in subdued VIV response. Lock-in is observed for U * ≥ 3. The cylinder vibration frequency is identical in the crossflow and in-line directions, leading to an elliptical trajectory. The rms of the force coefficients is similar to that for a stationary cylinder in the desynchronization regime while it decreases with increase in U * during lock-in. The spatiotemporal dynamics of LSB is explored. The LSB is sedentary in the desynchronization regime, while it undergoes significant circumferential movement, in each cycle of cylinder oscillation, in the lock-in regime. The mode of vortex shedding, determined from the arrangement of vortices in the span-averaged instantaneous flow as well as the phase difference between the lift and cylinder response, is C(2S) in the desynchronization regime. Abstract

Semaglutide, a GLP-1 receptor agonist, reduces the risk of major adverse cardiovascular events (MACE) in people with overweight or obesity, but the effects of this drug on outcomes in patients with atherosclerotic cardiovascular disease and heart failure are unknown. We report a prespecified analysis of the effect of once-weekly subcutaneous semaglutide 2·4 mg on ischaemic and heart failure cardiovascular outcomes. We aimed to investigate if semaglutide was beneficial in patients with atherosclerotic cardiovascular disease with a history of heart failure compared with placebo; if there was a difference in outcome in patients designated as having heart failure with preserved ejection fraction compared with heart failure with reduced ejection fraction; and if the efficacy and safety of semaglutide in patients with heart failure was related to baseline characteristics or subtype of heart failure.

Abstract

Introduction: Molar intrusion has always been a complicated and difficult treatment modality. Posterior teeth that are supra-erupted due to the early loss of their antagonists are commonly seen in adults that have limited or no access to dentistry during childhood and adolescence. Severely over-erupted molars or entire posterior segments pose a great challenge to the treating orthodontist. With regard to intruding posterior teeth, molar intrusion is a treatment option for patients with Anterior Open Bite (AOB); a malocclusion often characterized by the overeruption of the posterior teeth or/and under eruption of the anterior teeth. Aim: To compile and summarize the existing and advanced molar intrusive techniques and appliances with respect to their advantages and disadvantages, and their possible clinical effectiveness. Abstract

The location of the aerodynamic center, with respect to the center of gravity, determines the longitudinal static stability of an aerial vehicle. While most studies in the past focused on high Reynolds number (Re) flows, we consider flows at low Re (100 ≤ R e ≤ 6000) past an end-to-end wing and a finite wing of various semi-aspect ratio (0.25 ≤ sAR ≤ 5). It is shown that two-dimensional computations, even for an end-to-end wing, are inadequate for accurate prediction of aerodynamic coefficients. The chordwise pressure distribution as well as the variation of aerodynamic coefficients with angle of attack (α) at low Re are significantly different from that at high Re. Beyond a certain α, the flow is associated with a large recirculation zone, resulting in a non-linear variation of the aerodynamic coefficients with α. The conventional definition of the aerodynamic center is too restrictive at low Re. Owing to the non-linear variation of the pitching moment with change in α, it is not possible to determine one single location where it is invariant over a large range of α. The aerodynamic center is, therefore, determined over various sub-regimes of α. It varies significantly over the sub-regimes. The wing-tip vortex significantly affects the vortex shedding, flow structures, and pressure distribution on a finite wing. The locations of the aerodynamic center and center of pressure move fore, toward the leading edge, with a decrease in aspect ratio compared to that for an end-to-end wing. [ABSTRACT FROM AUTHOR]

Heart failure (HF) is a significant public health concern characterized by notable rates of morbidity and mortality. Multimorbidity, ranging from 43% to 98% among HF patients, significantly impacts prognosis and treatment response. HF management requires a holistic approach, including guideline-directed medical therapy. Sacubitril/valsartan (angiotensin receptor neprilysin inhibitor [ARNI]) is a cornerstone of HF treatment, supported by robust evidence from large-scale clinical trials across different levels of left ventricular ejection fraction. The recommendations presented in this paper have been developed by a group of cardiologists in India who convened in expert opinion meetings to discuss the utilization of ARNI in chronic HF patients with five different comorbid conditions like type 2 diabetes mellitus (T2DM), chronic kidney disease, myocardial infarction (MI), obesity, and hypertension. Key focus areas include initiation, dose titration, and management across different HF phenotypes and comorbidities. Emphasis is placed on the efficacy of ARNI irrespective of glycemic status in the T2DM population, its role in HF patients with obesity, and addressing challenges related to renal function decline and hyperkalemia. 44. Comparing Four Cephalometric Methods Used to Evaluate the Change in the Lower Incisor Inclination.

Abstract

Aim: The assessment of change in the lower incisor inclination using cephalometry during any orthodontic treatment is used to determine the treatment outcome. The inclination change is measured using the lower border of the mandible even though it is subjected to remodeling in growing children. We have used Bjork's superimposition technique using "stable structures" to eliminate these growth changes. In this study, we have evaluated if there is any major difference between three routinely used methods to estimate the lower incisor inclination change influenced by orthodontic treatment (gonion-gnathion, gonion-menton, and tangent drawn to the lower border of the mandible) in contrast to Bjork's superimposition technique. Materials and Methods: This study was performed using the lateral cephalograms of total of 39 subjects undergoing orthodontic treatment using fixed appliances, aged 12-16 years at the beginning of the treatment. The pretreatment and mid-treatment cephalograms of these subjects were evaluated with a gap of at least 1 year between the two radiographs. Patients who had undergone lower incisor extractions were excluded from this study. Results: The ICC was calculated using Bjork's superimposition technique as references and subsequently the other techniques were compared. The linear regression model showed good ICC between all methods, with ICC being 0.92 for Go-Me, 0.94 for Go tangent, and 0.88 for Go-Gn. All the three methods showed a significant correlation with Bjork's method. Conclusion: The data in the current study showed that there was no significant difference in the findings obtained by Bjork's superimposition method when compared to other techniques. Abstract

Static and dynamic fluid–structure interaction of a flexible plate behind a stationary cylinder in uniform flow is explored for a body-to-fluid mass ratio of 10. Steady flow-structure computations for Re = 150, based on the diameter of cylinder and free-stream speed, reveal four regimes with respect to increasing flexibility of the plate. The plate does not undergo any lateral deflection in regime 1 beyond which it undergoes a symmetry-breaking bifurcation causing it to spontaneously deflect laterally. The curvature is of the same sign along the entire length of plate in regime 2 while it changes sign along the plate in regime 3. The lateral deflection, however, is still maximum at the plate tip. The location of maximum deflection moves away from the plate tip in regime 4. The evolution of flow structures including the strength of various standing vortices, with flexibility, is studied. The role of reattachment of flow on the surface of the plate and the modification of the pressure distribution is explored. The critical Re, beyond which the splitter plate spontaneously achieves a deflected shape, decreases with increase in flexibility. It is estimated to be Re = 122.33 for the limiting case of an infinitely flexible plate. Computations for dynamic fluid–structure interaction reveal several regimes of lock-in with different natural vibration modes of the plate and related hysteresis. No lateral bias in the time-averaged deflection is found during lock-in; it occurs in the desynchronization regime that precedes the lock-in regime with second mode. Abstract

Flow-induced vibration of a rotating cylinder that is free to oscillate in the stream-wise and cross stream directions is studied in the laminar flow regime via linear stability analysis (LSA) and direct time integration (DTI). LSA reveals that the instability can arise from fluid-, elastic-, or coupled fluid-elastic modes depending on the rotation rate of the cylinder, α , and reduced speed, U * . Beyond α = 2 , the U * -range of lock-in increases exponentially with an increase in α. DTI brings out the multiple regimes of lock-in at various α. Each lock-in regime extends for a certain range of U * and is associated with a different mode of vortex shedding. The modes differ in terms of the number of pair of vortices shed during each cycle of cylinder oscillation. The amplitude of cylinder oscillation increases with an increase in the number of shed vortices. With an increase in Re, the number of vortex shedding modes increase. Vortices are generally shed during the upstream movement of cylinder, while the shear layer wraps around it resulting in large lift during the downstream motion. The flow as well as the oscillation amplitude is found to be sensitive to the initial condition for a certain range of α and U * . A flow regime is discovered where three distinct response states can be realized depending on the initial condition. Hysteresis in response to the cylinder and flow, with respect to increase and decrease in U * , occurs near the transition between lock-in and desynchronization and during the switch in the mode of vortex shedding. Aim The present case report showcases the treatment results and biomechanics involved for en masse retraction and intrusion of anterior teeth using three-piece intrusion arch and temporary anchorage device. Conclusion The modified three piece base arch combined with TADS is effective in controlled translation and intrusion of anteriors and would be a preferable mechanotherapy in low angle case with deep bite, proclined anterior and Class II malocclusion.
