13. Surgical treatment of thoracic and thoracoabdominal aortic aneurysms: Experience with left atriofemoral bypass [Trattamento chirurgico degli aneurismi toracici e toraco-addominali: Esperienza con l'assistenza circolatoria mediante pompa centrifuga]

Abstract

Background. The aim of this study was to analyse perioperative morbidity and mortality in patients undergoing thoracic (TAA) and thoracoabdominal aortic aneurysm (TAAA) surgery at the Department of Vascular Surgery of IRCCS San Raffaele, Milan. Methods. The study included 258 patients (199 males and 59 females) with a mean age of 66 (range: 41-82 years) undergoing 259 operations for aneurysmectomy of TAA or TAAA between January 1988 and April 2000. Cerebral spinal fluid drainage (CSFD) was used in 166 cases (75% of TAAA, 53% of TAA; 183 patients (98 TAAA and 85 TAA) were operated on with the use of left atriofemoral bypass using a Biomedicus pump. Results. The overall mortality rate at 30 days was 33/259 (13%); a total of 26 deaths (11%) were recorded during elective surgery and 7 (32%) in patients undergoing emergency repairs. The following perioperative complications were reported: paraplegia/paraparesis in 20 cases (8%), respiratory failure requiring prolonged intubation in 62 cases (24%), cardiac complications (major arrhythmia, myocardial infarction) in 26 cases (10%), renal failure in 18 cases (7%), postoperative bleeding requiring redo surgery in 12 cases (5%), graft infection in 5 cases (2%). Conclusions. Morbidity and mortality consequent to TAAA and TAA surgery are still high. However, based on our experience, the use of atriodistal bypass, sequential cross-clamping and CFSD enables acceptable results to be achieved and reduces complications secondary to spinal cord and visceral ischemia without the need for expeditious clamping times. 14. Food Safety Knowledge of Cheese Consumers.

Abstract

The aim of this study was to evaluate habits and practices of cheese consumers toward their level of knowledge of food safety. A total of 1000 people were interviewed in several cities of the state of Rio de Janeiro, Brazil. Five hundred thirty-eight (53.8%) people consume cheese daily, while 318 (31.8%) and 144 (14.4%) consume cheese weekly and monthly, respectively. Five hundred twenty-two (52%) people reported its usage as an ingredient used in the preparation of other dishes, while 320 (32%) of the interviewed people consume cheese directly as a part of the diet. Typical Brazilian cheeses such as “Minas Frescal” and “Prato” cheese are preferred by 528 (52.8%) of the consumers. Of the total consumers, 764 (76.4%) purchase cheese from supermarkets, while 236 (23.6%) from open-air markets. Inspected cheese is purchased by 350 (35%) consumers, while 650 (65%) buy it without knowing if they were submitted to previous fiscalization. Four hundred thirty (43%) consumers do not know any disease transmitted by cheese that has not been inspected. Overall, educational campaigns must be developed by the Sanitary Surveillance and the Health Agencies to improve the knowledge of the consumer about food safety of cheeses. [ABSTRACT FROM AUTHOR]

Abstract

Purpose: To evaluate the intra-operative performance and clinical outcome of a new commercially available stent-graft for the treatment of thoracic aortic diseases., Methods and Patients: From January 2003 to October 2004, 45 consecutive patients received endovascular treatment with the Zenith TX1 device for diseases of the thoracic aorta at a single center in northern Italy. Indications included disease of the descending thoracic aorta in 26 cases, of the aortic arch in 17 cases and of the thoraco-abdominal aorta in two cases. We treated 38 atherosclerotic aneurysms, two post-traumatic aortic ruptures, two penetrating ulcers, two chronic dissections and one case was treated for aortic bleeding after voluntary acid ingestion for attempted suicide. General anesthesia was used in 20 cases. Combined or hybrid endovascular and open surgical repair was performed in 11 patients. Mean follow-up was 7 months (range 1-22 months)., Results: Technical success was obtained in 44 patients (98%). One primary type I endoleak occurred (2%). ICU was used in 12 cases with a mean stay of 1 day. The mean hospital stay was 6 days (range 4-13 days). There were no hospital deaths or strokes but one transient paraplegia (2%). A type II endoleak was observed in one case and resolved spontaneously 1 month later. No aneurysm enlargement, endograft migration or structural failures were observed during follow-up. Two late unrelated-deaths were observed., Conclusions: This stent-graft does not fulfill all the characteristics of the ideal graft, however, it proved to be safe and allowed satisfactory short term results in this group of patients treated at a single center.
