1. Exploring the Impact of TPACK on Education 5.0 during the Times of COVID-19: A Case of Zimbabwean Universities

Abstract

The development of ICTs that can be used for Education 5.0 is rapid. Current educational practice suggests that many lecturers experience challenges in implementing the use of some form of ICT in teaching and learning. The research explores the impact of TPACK on the implementation of Education 5.0 in universities during the time of the COVID-19 pandemic in Zimbabwe. The qualitative research study was conducted with university lecturers. Data were collected through semi-structured interviews. Mishra & Koehler's (2006) TPACK framework which focuses on technological knowledge, pedagogical knowledge, and content knowledge was used as the lens of the study. During the COVID-19 pandemic, online delivery of education was found to be effective, convenient, and facilitated access to information. However, the universities experienced poor Internet connectivity, lack of infrastructure, unreliable devices and unavailability of software, as challenges to effectively implement Education 5.0. The training of lecturers to empower them to engage students effectively during the implementation of Education 5.0 was vital.

Abstract

BACKGROUND: We evaluated dolutegravir pharmacokinetics in infants with human immunodeficiency virus (HIV) receiving dolutegravir twice daily (BID) with rifampicin-based tuberculosis (TB) treatment compared with once daily (OD) without rifampicin. METHODS: Infants with HIV aged 1-12 months, weighing ≥3 kg, and receiving dolutegravir BID with rifampicin or OD without rifampicin were eligible. Six blood samples were taken over 12 (BID) or 24 hours (OD). Dolutegravir pharmacokinetic parameters, HIV viral load (VL) data, and adverse events (AEs) were reported. RESULTS: Twenty-seven of 30 enrolled infants had evaluable pharmacokinetic curves. The median (interquartile range) age was 7.1 months (6.1-9.9), weight was 6.3 kg (5.6-7.2), 21 (78%) received rifampicin, and 11 (41%) were female. Geometric mean ratios comparing dolutegravir BID with rifampicin versus OD without rifampicin were area under curve (AUC)0-24h 0.91 (95% confidence interval, .59-1.42), Ctrough 0.95 (0.57-1.59), Cmax 0.87 (0.57-1.33). Abstract

BACKGROUND AND OBJECTIVE: HIV treatment options remain limited in children. Dolutegravir is a potent and well-tolerated, once-daily HIV-1 integrase inhibitor recommended for HIV-1 infection in both adults and children down to 4 weeks of age. To support pediatric dosing of dolutegravir in children, we used a population pharmacokinetic model with dolutegravir data from the P1093 and ODYSSEY clinical trials. The relationship between dolutegravir exposure and selected safety endpoints was also evaluated. METHODS: A population pharmacokinetic model was developed with data from P1093 and ODYSSEY to characterize the pharmacokinetics and associated variability and to evaluate the impact of pharmacokinetic covariates. The final population pharmacokinetic model simulated exposures across weight bands, doses, and formulations that were compared with established adult reference data. Exploratory exposure-safety analyses evaluated the relationship between dolutegravir pharmacokinetic parameters and selected clinical laboratory parameters and adverse events. RESULTS: A total of N = 239 participants were included, baseline age ranged from 0.1 to 17.5 years, weight ranged from 3.9 to 91 kg, 50% were male, and 80% were black. The final population pharmacokinetic model was a one-compartment model with first-order absorption and elimination, enabling predictions of dolutegravir concentrations in the pediatric population across weight bands and doses/formulations. The predicted geometric mean trough concentration was comparable to the adult value following a 50-mg daily dose of dolutegravir for all weight bands at recommended doses. Body weight, age, and formulation were significant predictors of dolutegravir pharmacokinetics in pediatrics. Additionally, during an exploratory exposure-safety analysis, no correlation was found between dolutegravir exposure and selected safety endpoints or adverse events. CONCLUSIONS: The dolutegravir dosing in children ≥ 4 weeks of age on an age/weight-band basis provides comparable exposures to those historically observed in adults. Observed pharmacokinetic variability was higher in this pediatric population and no additional safety concerns were observed. These results support the weight-banded dosing of dolutegravir in pediatric participants currently recommended by the World Health Organization.

Abstract

BackgroundAlthough super-boosted lopinavir/ritonavir (LPV/r; ratio 4:4 instead of 4:1) is recommended for infants living with HIV and receiving concomitant rifampicin, in clinical practice, many different LPV/r dosing strategies are applied due to poor availability of pediatric separate ritonavir formulations needed to superboost. We evaluated LPV pharmacokinetics in infants with HIV receiving LPV/r dosed according to local guidelines in various sub-Saharan African countries with or without rifampicin-based tuberculosis (TB) treatment.MethodsThis was a 2-arm pharmacokinetic substudy nested within the EMPIRICAL trial (#NCT03915366). Infants aged 1-12 months recruited into the main study were administered LPV/r according to local guidelines and drug availability either with or without rifampicin-based TB treatment; during rifampicin cotreatment, they received double-dosed (ratio 8:2) or semisuperboosted LPV/r (adding a ritonavir 100 mg crushed tablet to the evening LPV/r dose). Six blood samples were taken over 12 hours after intake of LPV/r.ResultsIn total, 14/16 included infants had evaluable pharmacokinetic curves; 9/14 had rifampicin cotreatment (5 received double-dosed and 4 semisuperboosted LPV/r). Abstract

BackgroundPneumonia is the primary cause of death among HIV-infected children in Africa, with mortality rates as high as 35-40% in infants hospitalized with severe pneumonia. Bacterial pathogens and Pneumocystis jirovecii are well known causes of pneumonia-related death, but other important causes such as cytomegalovirus (CMV) and tuberculosis (TB) remain under-recognized and undertreated. The immune response elicited by CMV may be associated with the risk of developing TB and TB disease progression, and CMV may accelerate disease caused both by HIV and TB. Minimally invasive autopsies confirm that CMV and TB are unrecognized causes of death in children with HIV. CMV and TB may also co-infect the same child. The aim of this study is to compare the impact on 15-day and 1-year mortality of empirical treatment against TB and CMV plus standard of care (SoC) versus SoC in HIV-infected infants with severe pneumonia.MethodsThis is a Phase II-III, open-label randomized factorial (2 × 2) clinical trial, conducted in six African countries. The trial has four arms. Infants from 28 to 365 days of age HIV-infected and hospitalized with severe pneumonia will be randomized (1:1:1:1) to (i) SoC, (ii) valganciclovir, (iii) TB-T, and (iv) TB-T plus valganciclovir. The primary endpoint of the study is all-cause mortality, focusing on the short-term (up to 15 days) and long-term (up to 1 year) mortality. Secondary endpoints include repeat hospitalization, duration of oxygen therapy during initial admission, severe and notable adverse events, adverse reactions, CMV and TB prevalence at enrolment, TB incidence, CMV viral load reduction, and evaluation of diagnostic tests such as GeneXpert Ultra on fecal and nasopharyngeal aspirate samples and urine TB-LAM.DiscussionGiven the challenges in diagnosing CMV and TB in children and results from previous autopsy studies that show high rates of poly-infection in HIV-infected infants with respiratory disease, this study aims to evaluate a new approach including empirical treatment of CMV and TB for this patient population. BACKGROUND: Chronic respiratory symptoms are common among children living with human immunodeficiency virus (HIV). We investigated the radiological features of chronic lung disease in children aged 6-16 years receiving antiretroviral therapy for ≥6 months in Harare, Zimbabwe. METHODS: Consecutive participants from a HIV clinic underwent clinical assessment and chest radiography. Participants with an abnormal chest radiograph (assessed by a clinician) and/or those meeting a clinical case definition for chronic lung disease underwent high-resolution computed tomography (HRCT). Abstract

OBJECTIVE: Respiratory disease is a major cause of morbidity and mortality in HIV-infected children. Despite antiretroviral therapy (ART), children suffer chronic symptoms. We investigated symptom prevalence, lung function and exercise capacity among older children established on ART and an age-matched HIV-uninfected group. DESIGN: A cross-sectional study in Zimbabwe of HIV-infected children aged 6-16 years receiving ART for over 6 months and HIV-uninfected children attending primary health clinics from the same area. METHODS: Standardized questionnaire, spirometry, incremental shuttle walk testing, CD4 cell count, HIV viral load and sputum culture for tuberculosis were performed. RESULTS: A total of 202 HIV-infected and 150 uninfected participants (median age 11.1 years in each group) were recruited. Median age at HIV diagnosis and ART initiation was 5.5 (interquartile range 2.8-7.5) and 6.1 (interquartile range 3.6-8.4) years, respectively. Median CD4 cell count was 726 cells/μl, and 79% had HIV viral load less than 400 copies/ml. Chronic respiratory symptoms were rare in HIV-uninfected children [n = 1 (0.7%)], but common in HIV-infected participants [51 (25%)], especially cough [30 (15%)] and dyspnoea [30 (15%)]. ObjectiveWe evaluated the impact of Option A on HIV-free infant survival and mother-to-child transmission (MTCT) in Zimbabwe.DesignSerial cross-sectional community-based serosurveys.MethodsWe analyzed serosurvey data collected in 2012 and 2014 among mother-infant pairs from catchment areas of 132 health facilities from five of 10 provinces in Zimbabwe. Eligible infants (alive or deceased) were born 9-18 months before each survey to mothers at least 16 years old. We randomly selected mother-infant pairs and conducted questionnaires, verbal autopsies, and collected blood samples. 21. Engaging Youth to Examine Lifestyle Behaviors through Authentic Research with University Partnerships

Abstract

University researchers partnered with secondary students in West Virginia and Mali on an international science investigation to strengthen science education and public health practices. WV and Mali students made comparisons of diet, physical activity, BMI, and blood pressure gathered from study participants. Full IRB approval was provided by West Virginia University for this human subjects study. The mean systolic blood pressure for the Mali participants was significantly lower, especially when compared to African-American youth (p=0.0008), as was the mean BMI scores of Mali participants (F [subscript 1, 77]=11.43, p=0.0011). Student investigators analyzed results with guidance by university faculty and showcased their results to peer-audiences at school, for local community events, and at annual Symposia. The University partnership provided opportunities for secondary students and their teachers to discover the role that environment plays in influencing health via authentic research and exposure to international public health strategies. Viable alternatives to unhealthy behavior were shared. Results point to the importance of a nutrition-dense diet (low in fats and sugar) combined with simple walking, as beneficial for all humans. Such comparisons can enhance science education, global health awareness, and empower young adults to alter their unhealthy behavior so as to avoid chronic disease in adulthood. (Contains 3 tables.)

Abstract

ObjectiveWe estimated HIV-free infant survival and mother-to-child HIV transmission (MTCT) rates in Zimbabwe, some of the first community-based estimates from a UNAIDS priority country.MethodsIn 2012 we surveyed mother-infant pairs residing in the catchment areas of 157 health facilities randomly selected from 5 of 10 provinces in Zimbabwe. Enrolled infants were born 9-18 months before the survey. We collected questionnaires, blood samples for HIV testing, and verbal autopsies for deceased mothers/infants. Estimates were assessed among i) all HIV-exposed infants, as part of an impact evaluation of Option A of the 2010 WHO guidelines (rolled out in Zimbabwe in 2011), and ii) the subgroup of infants unexposed to Option A. We compared province-level MTCT rates measured among women in the community with MTCT rates measured using program monitoring data from facilities serving those communities.FindingsAmong 8568 women with known HIV serostatus, 1107 (12.9%) were HIV-infected. Among all HIV-exposed infants, HIV-free infant survival was 90.9% (95% confidence interval (CI): 88.7-92.7) and MTCT was 8.8% (95% CI: 6.9-11.1). Sixty-six percent of HIV-exposed infants were still breastfeeding. Among the 762 infants born before Option A was implemented, 90.5% (95% CI: 88.1-92.5) were alive and HIV-uninfected at 9-18 months of age, and 9.1% (95%CI: 7.1-11.7) were HIV-infected. In four provinces, the community-based MTCT rate was higher than the facility-based MTCT rate. In Harare, the community and facility-based rates were 6.0% and 9.1%, respectively.ConclusionBy 2012 Zimbabwe had made substantial progress towards the elimination of MTCT. Our HIV-free infant survival and MTCT estimates capture HIV transmissions during pregnancy, delivery and breastfeeding regardless of whether or not mothers accessed health services. These estimates also provide a baseline against which to measure the impact of Option A guidelines (and subsequently Option B+).

23. Exploring the impact of TPACK on Education 5.0 during the times of COVID-19: a case of Zimbabwean universities.

Abstract

The development of ICTs that can be used for Education 5.0 is rapid. Current educational practice suggests that many lecturers experience challenges in implementing the use of some form of ICT in teaching and learning. The research explores the impact of TPACK on the implementation of Education 5.0 in universities during the time of the COVID-19 pandemic in Zimbabwe. The qualitative research study was conducted with university lecturers. Data were collected through semi-structured interviews. Mishra & Koehler’s (2006) TPACK framework which focuses on technological knowledge, pedagogical knowledge, and content knowledge was used as the lens of the study. During the COVID-19 pandemic, online delivery of education was found to be effective, convenient, and facilitated access to information. However, the universities experienced poor Internet connectivity, lack of infrastructure, unreliable devices and unavailability of software, as challenges to effectively implement Education 5.0. The training of lecturers to empower them to engage students effectively during the implementation of Education 5.0 was vital. [ABSTRACT FROM AUTHOR]

Abstract

Cohort studies in adults with HIV showed that dolutegravir was associated with neuropsychiatric adverse events and sleep problems, yet data are scarce in children and adolescents. We aimed to evaluate neuropsychiatric manifestations in children and adolescents treated with dolutegravir-based treatment versus alternative antiretroviral therapy.

Abstract

Air pollution is the second largest risk to health in Africa, and children with asthma are particularly susceptible to its effects. Yet, there is a scarcity of air pollution exposure data from cities in sub-Saharan Africa. We aimed to identify potential exposure reduction strategies for school children with asthma living in urban areas in sub-Saharan Africa.

28. India–Zimbabwe Relations: Recalibrating the Failing Engagement

Abstract

The India–Zimbabwe relations are rampant with obdurate omissions, commissions, misinformation, and proper attention, hence dismally productive. This investigation pins down the dismal relations to Zimbabwe’s economic quagmire, the inseparable China–Zimbabwe relations, Zimbabwe’s diplomatic isolation and the effects of sanctions enacted by the US and other Western countries. With both primary and secondary sources, an interpretive approach is used to generate and analyse data. The study outlines prospects that can recalibrate India–Zimbabwe relations, especially in the new dispensation era of Zimbabwe after Mugabe. The possibilities include leveraging the available business opportunities, energy and mineral resources, security, regional cooperation and conflict management, knowledge exchange, institutional building, and social cohesion.

Dolutegravir-based antiretroviral therapy is a preferred first-line treatment for adults and children living with HIV; however, very little pharmacokinetic data for dolutegravir use are available in young children. We therefore aimed to evaluate dolutegravir dosing and safety in children weighing 3 kg to less than 20 kg by assessing pharmacokinetic parameters and safety data in children taking dolutegravir within the ODYSSEY trial.

The research adopted a qualitative approach using 87 key informant interviews and 1054 questionnaires administered to randomly selected households from deforestation hotspots in five districts. Based on a theoretical concept that collective action depends on resource system characteristics, actor networks, institutional arrangements and the external environment, the study explored patterns in the emergence of collective action. The study further explored actor networks to illustrate applicability of Social Network Analysis (SNA) as a proxy of collective action and institutional arrangements. Results suggest that actor networks reflect the existing strength and intensity of collective action and institutional arrangements. Forest tenure has a bearing on the intensity of collective action. Forest resources under state management, though more productive in terms of goods and services than those under communal management, have a low capacity for collective action and the emergence of new institutions. Results suggest that a high diversity of actors relates to a high intensity of collective action and an emergence of new institutional arrangements. These findings have a bearing on policy and forest management strategies for forest resources under community and state proprietorship.

Abstract

The moringa tree is a rising star in South Africa where it is increasingly playing a variety of social and economic roles, especially among the rural poor. Also known as the horseradish tree, moringa has been referred to as the miracle or magic tree because of its reported health, nutrition and some industrial applications. Through document review, this paper reviews these claims that include the treatment of cancer and diabetes, and its nutritional value to humans, livestock and crops plants, among other applications. Drawing from lessons in the commercialization of Rooibos and cannabis cases, the article highlights learning points that could aid South Africa identify and commercialize some aspects of the moringa as part of its economic development efforts and creating the envisaged knowledge economy.

We did nested pharmacokinetic substudies in patients enrolled in the ODYSSEY-trial to evaluate simplified dosing in children with HIV.

ABSTRACTTwo provenances of Moringa oleiferaLam., Malawi and Mutoko, were evaluated to determine the effect of plant spacing and cutting interval on the leaf biomass yield for fodder production. A split-split plot experimental design was set up with provenance as the main plot factor, plant spacing as sub-plot factor and cutting interval as sub-sub plot factor. The provenances were tested at two plant spacings of 15 cm × 15 cm and 20 cm × 20 cm, and two cutting intervals of 60 days and 75 days.Results showed that 90 days after planting, at 20 cm × 20 cm spacing, Malawi provenance plants had a higher (P < 0.05) leaf dry matter yield than Mutoko plants. A cutting interval of 60 days had a higher (P < 0.05) leaf dry matter yield for the two provenances than a 75 day cutting interval. Both provenances can be recommended for agroforestry because their leaf biomass yields were not significantly different at all subsequent cutting intervals of 60 and 75 days.

Abstract

Preterm birth (PTB) is the leading cause of morbidity and mortality in children globally, yet its prevalence has been difficult to accurately estimate due to unreliable methods of gestational age dating, heterogeneity in counting, and insufficient data. The estimated global PTB rate in 2020 was 9.9% (95% confidence interval: 9.1, 11.2), which reflects no significant change from 2010, and 81% of prematurity-related deaths occurred in Africa and Asia. PTB prevalence in the United States in 2021 was 10.5%, yet with concerning racial disparities. Few effective solutions for prematurity prevention have been identified, highlighting the importance of further research.

ABSTRACTThe Zimbabwean forestry industry, which contributes significantly to the economy, was introduced in the 1940s and is dominated by Pinusspp., Eucalyptusspp.and Acacia mearnsiiDe Wild. This paper assembles and consolidates literature on diseases and pests of forest plantations to serve as a foundation for management and future studies. Most of the outlined diseases and pests have been known since the early days of forest plantations (i.e. the 1960s) whilst some are recent, signifying their increasing importance to the forest industry. Pinusspp., Eucalyptusspp.and A. mearnsiiare affected by pathogens and pests originating from the native areas of the hosts while the origins of others are not known. Disease outbreaks resulted in discontinuous cultivation of Eucalyptus globulusLabill. and Pinus radiataD. Don. that were important species in eastern and southern Africa. Forest health issues impacting on forest production are poorly understood and there is an urgent need to implement a structured forest health survey program and appropriate support to identify current and emerging pest and disease issues. Additionally, this information is important to support the forestry seed and timber export program.
