Purpose To compare the three-dimensional (3D) heads-up surgery with the traditional microscopic (TM) surgery for various vitreoretinal diseases. Methods A medical record review of patients that underwent 3D heads-up or TM vitreoretinal surgeries was performed from May 2020 to October 2021 in this retrospective case–control study. Main outcome measures included surgery-related characteristics, efficacy, safety, and satisfaction feedback from the surgical team. Results A total of 220 (47.6%) and 242 (52.4%) eyes were included in the 3D and TM groups, respectively. The 3D heads-up system significantly benefits delicate surgical steps, like the epiretinal membrane (ERM) peeling for ERM and internal limiting membrane peeling for idiopathic macular holes (P P = 0.049), while no difference by TM surgery (P = 0.45). For the satisfaction feedback, the 3D heads-up system was rated significantly higher in most subscales and the overall score (P P P > 0.05). Conclusion The efficacy and safety of the 3D heads-up surgery were generally comparable to the TM surgery. The 3D heads-up system could significantly benefit delicate surgical steps and achieve better surgical team satisfaction.

In this paper, the physical and chemical analysis method was used to analyze the leakage of a regeneration gas heater device accident. The result shows that this device was heated seriously at local area, and oxidized severely. The carbon content around the leakage is obviously lower than the limit of standard, and the grain deformation at crack tip of the leakage is obvious.It shows that reason of device failure is caused by typical plastic Instability, and low carbon content leads to insufficient mechanical properties at high temperature. According to data from manufacturer, the unreasonable arrangement of thermo-couples may be the root cause of this leakage.It can be considered that replace the higher grade materials conservatively, while optimizing the arrangement of thermo-couples.

OBJECTIVE: To study the clinical and gastroscopic features of children with cyclic vomiting syndrome. METHODS: A retrospective analysis was performed on the medical data of 63 children with cyclic vomiting syndrome who were hospitalized and followed up in Beijing Tsinghua Changgung Hospital, School of Clinical Medicine, Tsinghua University from August 2019 to March 2022. RESULTS: Among the 63 children, there were 30 boys and 33 girls, with a mean age of 6.11 years, a mean course of disease of 2.57 years, and a mean vomiting period of 4.04 days. The most common accompanying symptom was listlessness or somnolence (55/63, 87%), followed by anorexia (45/63, 71%), abdominal pain or abdominal discomfort (40/63, 63%), constipation (19/63, 30%), salivation (12/63, 19%), nausea (11/63, 17%), headache (11/63, 17%), fever (6/63, 10%), and rash (1/63, 2%). All 63 children underwent gastroscopy, among whom 3 had no marked abnormalities, 22 (35%) had chronic superficial gastritis or chronic non-atrophic gastritis alone, and 38 (60%) had other abnormal changes aside from chronic gastritis (16 children with reflux esophagitis, 12 with bile reflux gastritis, 13 with duodenitis, 10 with erosive gastritis, and 5 with gastric or duodenal ulcer). Among the 63 children, 42 underwent pathological examinations of gastric mucosa, among whom 5 had no marked abnormalities, 34 had mild chronic gastritis, 2 had moderate chronic gastritis, and 1 had severe chronic gastritis. Among the 63 children, 15 received 24-hour dynamic esophageal pH monitoring during the interictal period, among whom 9 children were found to have pathological acid reflux. CONCLUSIONS: In addition to recurrent vomiting, most children with cyclic vomiting syndrome also have the symptoms such as somnolence or listlessness, anorexia, and abdominal pain. The main manifestation on gastroscopy is chronic gastritis, and most children may also have reflux esophagitis, bile reflux gastritis, and erosive gastritis. Mild chronic gastritis is the main pathological change of gastric mucosa.

Caspase-9 is the initiator caspase for the intrinsic apoptotic cell death pathway, and is critical to the activation of effector caspases during apoptosis, but how its levels and activities are maintained remains unclear. The gigantic Inhibitor of Apoptosis Protein (IAP) BIRC6/BRUCE/Apollon not only inhibits apoptosis, but also promotes ubiquitination of the key autophagic protein LC3 and inhibits autophagy. Here we show that BIRC6 forms an anti-parallel U-shaped dimer in a 3.6-Å cryo-EM structure with multiple previously unannotated domains, including a ubiquitin-like domain, and discover that the mitochondria-derived pro-apoptotic factor Smac/DIABLO binds BIRC6 by interacting with one BIR domain, two carbohydrate-binding modules and two helices in the central cavity. Notably, Smac outcompetes the effector caspase 3 and the pro-apoptotic protease HtrA2, but not caspase 9, for binding BIRC6. BIRC6 strongly inhibits cellular activity of caspase 9, but weakly suppresses that of caspase 3. Meanwhile, BIRC6 binds LC3 through an LC3-interacting region, probably following dimer disruption of this BIRC6 region. Deficiency in LC3 ubiquitination promotes autophagy and autophagic degradation of BIRC6, and inhibits apoptosis. Moreover, induction of autophagy promotes autophagic degradation of both procaspase-9 and active caspase-9, but not of effector caspases. These results are important to understand how the balance between apoptosis and autophagy is regulated under pathophysiological conditions.

To investigate the efficacy of high-risk myelodysplastic syndrome (MDS) patients treated by different doses of decitabine (DAC) and its safety.Thirty patients with high-risk MDS were all treated by demethylation drug DAC. According to the doses of DAC, 30 patients were divided into 10-day regimen [6 mg/(mThe patients were followed up to May 2020, in the 10-day regimen group, 10 cases achieved complete remission (CR), 3 cases achieved partial remission (PR), and 2 cases were progressive disease (PD). Four cases died, including 1 case for heart failure, 2 cases for respiratory failure and 1 case for serious infection. In the 5-day regimen group, 11 cases achieved CR, 1 case achieved PR, 3 cases were PD. Five cases died, including 2 cases for heart failure and 3 for serious infection. The CR rate and ORR of the patients in the two groups were 66.67% vs 73.33%, 86.67% vs 80.00%, respectively, which showed no significant differences, and the efficacy also showed no significant difference. After treatment, the levels of WBC, NE, Hb and PLT of the patients in 10-day regimen group were higher than those in 5-day regimen. In the 10-day regimen group, there were 11 cases of pneumonia, 2 cases of bacteremia, 1 case of skin infection and 1 case of urinary tract infection. While in the 5-day regimen group, 13 cases of pneumonia, 5 cases bacteremia, 1 case of skin infection and 3 cases of urinary tract infection. There were 2 cases with mild gastrointestinal response in the 10-day regimen group, and 7 cases with obvious nausea and anorexia in the 5-day regimen group. The symptoms were relieved after the treatment of acid suppression, stomach protection and antiemetic. The liver, kidney and heart function were monitored. One case liver function damage and 2 cases cardiac insufficiency were observed in the 10-day regimen group. Seven cases regimen cardiac insufficiency and 4 cases regimen liver function damage were observed in the 5-day regimen group.10-day regimen and 5-day regimen are equally effective, but 10-day regimen is less myelosuppressive and more safer, which can be applied in clinical.不同剂量地西他滨治疗高危骨髓增生异常综合征患者疗效及安全性的回顾性分析.探讨不同剂量地西他滨治疗高危骨髓增生异常综合征（MDS）患者的临床疗效及安全性。.选取高危MDS患者30例，均使用去甲基化药物地西他滨治疗。根据患者使用地西他滨剂量不同，分为10 d [6 mg/（m随访至2020年5月，10 d方案组获完全缓解10例，部分缓解3例，疾病进展2例；完全缓解率为66.67%，总反应率为86.67%，死亡4例，其中1例死于心衰，2例死于呼吸衰竭，1例死于重症感染。5 d方案组获完全缓解11例，部分缓解1例，疾病进展3例，完全缓解率为73.33%，总反应率为80.00%，死亡5例，2例死于心衰，3例死于重症感染。两组疗效和生存率比较差异均无统计学意义（P0.05）。治疗后10 d方案组白细胞数、中性粒细胞数、血红蛋白及血小板数均值均高于5 d方案组。10 d方案组发生肺炎11例，菌血症2例，皮肤感染1例，泌尿系感染1例。5 d方案组肺炎13例，菌血症5例，皮肤感染1例，泌尿系感染3例。10 d方案组有2例患者出现轻微消化道反应， 5 d方案组有7例出现明显恶心、食欲减退，经抑酸护胃、止吐等对症处理后，症状缓解。监测肝肾功能及心功能，10 d方案组1例出现肝功能损伤，2例出现心功能不全；5 d方案组4例出现肝功能损伤，7例发生心功能不全。.10 d方案组和5 d方案组疗效相当，但10 d方案组骨髓抑制程度及不良反应发生率均较5 d方案组少，安全性更高，具有广泛的临床应用价值。.

Chronic administration of methamphetamine (METH) leads to physical and psychological dependence. It is generally accepted that METH exerts rewarding effects via competitive inhibition of the dopamine transporter (DAT), but the molecular mechanism of METH addiction remains largely unknown. Accumulating evidence shows that mitochondrial function is important in regulation of drug addiction. In this study, we investigated the role of Clk1, an essential mitochondrial hydroxylase for ubiquinone (UQ), in METH reward effects. We showed that Clk1(+/−) mutation significantly suppressed METH-induced conditioned place preference (CPP), accompanied by increased expression of DAT in plasma membrane of striatum and hippocampus due to Clk1 deficiency-induced inhibition of DAT degradation without influencing de novo synthesis of DAT. Notably, significantly decreased iron content in striatum and hippocampus was evident in both Clk1(+/−) mutant mice and PC12 cells with Clk1 knockdown. The decreased iron content was attributed to increased expression of iron exporter ferroportin 1 (FPN1) that was associated with elevated expression of hypoxia-inducible factor-1α (HIF-1α) in response to Clk1 deficiency both in vivo and in vitro. Furthermore, we showed that iron played a critical role in mediating Clk1 deficiency-induced alteration in DAT expression, presumably via upstream HIF-1α. Taken together, these data demonstrated that HIF-1α-mediated changes in iron homostasis are involved in the Clk1 deficiency-altered METH reward behaviors.

In order to reveal the fouling problem on the outer surface of the steam generator (SG) tube in the secondary circuit condition of pressurized water reactor (PWR) nuclear power plant, based on the density functional theory (DFT) method, the Cambridge sequential total energy program package (CASTEP) is used to simulate seven kinds of highly symmetric adsorption structure models of termination with tetrahedral Fe (A termination) and termination with octahedral Fe (B termination) on Fe3O4 (0 0 1) surface. The adsorption energies and stable adsorption conformations are calculated. The results show that the most stable adsorption structures of the Fe2+/Fe3O4 (0 0 1) configurations are Fe2+ above Fe-O bond of B layer termination (Fe3O4(001) A-b). During the adsorption, the Fe-Fe, Fe-O bond length, and Fe-Fe-O bond angle of (0 0 1) surface change, and the atomic positions parallel and perpendicular to (0 0 1) surface change correspondingly. The change happened to the surface layer is the most drastic one. The calculation of charge population, the density of states (DOS), and electron local function of Fe2+/Fe3O4 (0 0 1) optimal adsorption configuration show that there is electron transfer between Fe2+ and Fe3O4 (0 0 1), and the adsorption type is chemisorption. Among them, Fe (Fe2+)-Fe (Fe3O4) forms a metal bond, and Fe (Fe2+)-O (Fe3O4) forms the ionic bond. The results illustrate the interaction between free Fe2+ and Fe3O4 is the reason of the nucleation and agglomeration of Fe3O4 scale and it provides the foundation for the further research on Fe3O4 scale deposition.

Soil slips are widely distributed on the Loess Plateau. Most of them have experienced an ecological succession process, which has strong effects on the near-surface characteristics, and thereby influences soil detachment capacity by overland flow (Dc). This study quantified the effects of progressive succession of the deposition zone of soil slip on Dc and soil resistance to flowing water erosion reflected by rill erodibility (Kr) and critical shear stress (τc) on the Loess Plateau. Soil samples (diameter 10 cm, height 5 cm) were taken from seven deposition zones of soil slips covered by typical plant communities with different succession stages restored for 1 to 31 years. A 37-year grassland where no slip occurred covered by climax community was selected as the control. A hydraulic flume (4.0 m in length, 0.35 m in width) with a slope ranging from 0 to 60% was applied to determine Dc. Biological crusts thickness, soil texture, bulk density, cohesion (CH), water stability ability (WSA), organic matter content, root mass density (RD), and plant litter density (LD) were measured for each site. The measured Dc decreased rapidly when the soils were restored for 1 to 21 years, and then gradually leveled off. Dc could be well estimated by flow shear stress, CH, WSA, RD, and LD (NSE = 0.95). Kr decreased rapidly with succession till attained a stable stage after 21 years of succession. Kr could be well simulated by CH, WSA, RD, and LD (NSE = 0.92). τc increased generally with succession, although it fluctuated from 10 to 31 years. The temporal variation of τc was dominantly controlled by the biological crusts thickness and the content of WSA. Natural ecological succession is an effective approach to promote soil resistance to flowing water erosion for the deposition zone of soil slip. The near-surface characteristics, such as CH, WSA, RD, and LD, were the key factors to control soil erosion for the deposition zone of soil slip on the Loess Plateau.

The adjustment of land use can alter the near soil surface characteristics and thus likely affects soil erodibility. However, few studies have been carried out to assess the variations in soil erodibility under different land uses on the Loess Plateau after many slope farmlands have been converted to green lands in the past several decades. This study was conducted to quantify comprehensively the effects of five typical land uses (cropland, orchard, grassland, shrubland and woodland) on soil erodibility based on soil cohesion (Coh), saturated conductivity (Ks), the number of drop impact (NDI), the mean weight diameter of soil aggregates (MWD), soil penetration resistance (PR) and soil erodibility K factor from different aspects, as well as a comprehensive soil erodibility index (CSEI) produced by a weighted summation method. The results showed that Coh of grassland, Ks of woodland, and MWD and NDI of shrubland were the maximum, while PR of woodland and K of shrubland were the minimum among five tested land uses. Soil erodibility (reflected by CSEI) of shrubland was the minimum, while cropland had the maximum CSEI. Compared to cropland, soil erodibility decreased by 15.5%, 53.0%, 77.2% and 59.6% for orchard, grassland, shrubland and woodland, respectively. The variation in soil erodibility under different land uses was closely related to near soil surface characteristics. Soil erodibility increased with soil bulk density, and decreased with biological soil crust thickness, organic matter content, plant litter density, and root density. The results are helpful for optimizing spatial planning and improving eco-environment on the Loess Plateau.

Natural restoration age of abandoned farmlands has significant effects on near soil surface characteristics and thus affects soil erodibility. However, few studies have been conducted to investigate the potential effects of natural restoration age on soil erodibility on the Loess Plateau where many slope farmlands have been abandoned for soil erosion control in the past decades. This study was performed to quantify the effects of natural restoration age on soil erodibility reflected by soil cohesion (Coh), saturated conductivity (Ks), the number of drop impact (NDI), the mean weight diameter of soil aggregates (MWD), soil penetration resistance (PR), and soil erodibility K factor. One slope farmland (as the control) and six abandoned farmlands restored for 3 to 33 years were selected for soil indicators measurements. A weighted summation method was used to produce one comprehensive soil erodibility index (CSEI) to demonstrate comprehensively the temporal variation in soil erodibility with natural restoration age. Recently, medicinal peptide molecules are of great interest to many international pharmaceutical companies, mainly because of their relatively lower research costs, shorter research cycles, and the greater likelihood of being drugs, when compared with traditional small molecules. Due to the great variety in molecule structures and the diverse biological functions, disulfide-rich peptide toxins have become a shining molecular library for the development of polypeptide drugs. In view of the increasing amount of related publications, here we summarize the discovery, structural elucidation and chemical synthesis of disulfide-rich peptide toxins and their analogs.

Chemical reduction of graphene oxide (GO) is a simple and inexpensive method for the large-scale production of graphene-based materials. A suitable reducing agent, especially a green reductant, is in high demand for the production of reduced GO (RGO). Vancomycin, a glycopeptide antibiotic used to treat a variety of Gram-positive bacterial infections, was used to chemically reduce GO at a weak alkaline pH. As far as we know, this is the first report of reduction of GO by a glycopeptide antibiotic. The resulting vancomycin-decorated RGO (RGO-Van) was characterized by UV-visible adsorption and Raman spectroscopies, X-ray diffraction pattern and atomic force microscopy (AFM). The antibacterial effect of the RGO-Van suspension was investigated by the bacterial growth curves. The RGO-Van sheet can be fabricated into paper-like film through vacuum filtration. The antibacterial property of the as-obtained RGO-Van film was assessed by the inhibition zone test, and the bacterial adhesion assay. The antibacterial efficacy of the RGO-Van film was also verified by treatment of wound infection caused by Staphylococcus aureus (S. aureus) in a rat infection model.

Although phenolic copigments are rarely colored, they can still influence color expression in red wine copigmentation. Here, prefermentative agents, such as caffeic and rosmarinic acids, were added as copigments during Cabernet Sauvignon red winemaking. The evolution of colors and polyphenols was analyzed by tristimulus colorimetry and high-performance liquid chromatography-tandem mass spectrometry during brewing and aging periods. The results showed that the addition of caffeic and rosmarinic copigments was beneficial to obtain more saturated and vivid hue of wines. The changes in color were correlated with a significant increase in anthocyanins concentration. Moreover, the results demonstrated that the addition of the two phenolic copigments increased the concentration of total phenols and enhanced the copigmentation effect, indicating improved chromatic intensity and color stability of wines. Therefore, the addition of caffeic and rosmarinic acids is a simple and useful enological technique to obtain red wines with high color quality and aging potential.

Based on the scaling problem on the outer surface of the steam generator tube in the secondary circuit condition of pressurized water reactor, the molecular dynamic was used to calculate the adsorption characteristics of Fe3O4. The trajectory, mean squared displacement, radial distribution function and adsorption energy of Fe3O4 agglomeration and deposition process were further analyzed through variation of temperature and concentration. When the temperature was between 510 K and 530 K, the diffusion coefficient of Fe2+ and OH− could reach the minimum value of 2.255 × 10-5 cm2/s and 2.638 × 10-5 cm2/s, respectively. During variation of Fe2+ concentration, the size of agglomerate particles increased continuously, and the diffusion coefficient of H2O can reach the minimum value of 3.78 × 10-5 cm2/s. Fe2+ with positive charges can effectively agglomerate with H2O and OH−, forming the tetrahedral and octahedral structures of Fe3O4 crystal. At this time, a small number of OH− were adsorbed at the interface of the matrix, forming the double electrical layer and affecting the deposition of agglomerated Fe3O4. Apart from this, high temperature is beneficial to inhibit scaling deposition in part. Likewise, low concentration of the dissolved Fe2+ and OH− can also lead to weaker deposition of agglomerated Fe3O4 on the outer scaling layer of the secondary side in the steam generator.

Based on the fouling problem on the outer surface of the steam generator tube in the secondary circuit condition of pressurized water reactors, the density functional theory was used to calculate the adsorption characteristics of Fe2+, OH− on Fe3O4 surface. Eight kinds of highly symmetric adsorption configurations of Fe2+ and OH− on Fe3O4 (1 1 1) surface under two kinds of Fetet1- and Feoct2-terminated surfaces were established. Meantime, the interaction mechanisms and electronic structures of Fe2+/Fe3O4(1 1 1), OH−/Fe3O4(1 1 1) before and after the adsorption process were studied. The results demonstrated that the most stable adsorption site of Fe2+/Fe3O4(1 1 1) is the Fe3O4(111) tet-b configuration. After Fe2+ adsorbed on Fe3O4(111) tet-b sites, the lengths of Fe-Fe and Fe-O bonds and the Fe-Fe-O angle changed. The parallel and vertical directions of top atoms in Fe3O4(1 1 1) surface changed and the shift of topmost atoms was the largest, indicating that the Fe3O4 crystal lattice parameters can be affected by the Fe2+ adsorption. The charge population, density of states, and electron local function results indicated that there were 0.5e to 0.81e electron transfer between Fe2+ and Fe3O4 (1 1 1). The electron transfer of OH-/Fe3O4(111) oct-Oh is from 0.18e to 0.63e. Both of the Fe2+ and OH- can form the chemical bond on the Fe3O4(1 1 1) surface. The results illustrate the nucleation and agglomeration of Fe3O4 scale and provide the foundation for the further research on Fe3O4 scale deposition.

Non-thyroid malignancies to the thyroid gland resulting from distant metastases are extremely rare, and such cases are rarely seen in clinical settings. The question of how a tumor metastasizes to the thyroid remains unanswered. Here we report a case of lung adenocarcinoma metastasizing to the thyroid gland. The article covers the pathological features, treatments, examination reports, and the postoperative follow-up reviews of the patient. In this article, we discuss the diagnostic method, the spread route, the prognosis, the mechanism and above all, the treatment. In addition, we searched the PubMed and ISI Web of Science databases for articles published in English using the key words "lung", "thyroid", and "metastasis", and we reviewed nearly all the reports about thyroid malignancies being metastasized from lung cancer. This rare case emphasizes the importance of the multifaceted comprehensiveness of the cephalometry diagnosis, pathological diagnosis, and immunohistochemical analysis to ensure that such rare cases are not missed. We declare that all cases of thyroid malignancies metastasized from the lungs shall be reported at large for further clinical research.

Objective To assess the efficacy and safety of apatinib in the treatment of advanced colorectal cancer(CRC). Methods The clinical data of 16 CRC patients treated with apatinib after failure of prior lines of treatment were retrospectively analyzed in terms of objective response rate,disease control rate,progression-free survival,overall survival,adverse events,and prognostic factors. Results The efficacy was evaluable in 14 patients,among whom the objective response rate was 7.1% and the disease control rate was 50%.The median progression-free survival was 3 months(95%CI=1.57-4.42),and the median overall survival was 6.5 months(95%CI=4.10-8.89).The safety was evaluable in 16 patients,among whom the most common grade 3 adverse events were hypertensinon(37.5%)and proteinuria(25%).No grade 4 adverse event was observed.Multivariate analysis did not show any factor directly related to survival.

Pulmonary fibrosis is a chronic and progressive interstitial lung disease of known and unknown etiology. Over the past decades, macrophages have been recognized to play a significant role in IPF pathogenesis. According to their anatomical loci, macrophages can be divided to alveolar macrophages (AMs) subtypes and interstitial macrophages subtypes (IMs) with different responsibility in the damage defense response. Depending on diverse chemokines and cytokines in local microenvironments, macrophages can be induced and polarized to either classically activated (M1) or alternatively activated (M2) phenotypes in different stages of immunity. Therefore, we hypothesize that there is a “phagocytosis-secretion-immunization” network regulation of pulmonary macrophages related to a number of chemokines and cytokines. In this paper, we summarize and discuss the role of chemokines and cytokines involved in the “phagocytosis-secretion-immunization” network regulation mechanism of pulmonary macrophages, pointing toward novel therapeutic approaches based on the network target regulation in the field. Therapeutic strategies focused on modifying the chemokines, cytokines and the network are promising for the pharmacotherapy of IPF. Some Traditional Chinese medicines may have more superiorities in delaying the progression of pulmonary fibrosis for their multi-target activities of this network regulation.

In terms of fault diagnosis of liquid-propellant rocket engine, the fault diagnosis accuracy of the traditional method is low the characteristics of engine fault data are small sample and nonlinear variation. In order to improve the accuracy of sensor fault diagnosis and overcome the scarcity of related samples, the particle swarm optimization (PSO) with strong global searching ability is used to optimize the LS-SVM parameters, and the LSSVM optimal parameter values are obtained by iteration to improve the model fitting accuracy and generalization ability. At the same time, the traditional support vector machine and BP neural network model are used for the detection. The simulation results show that the least squares support vector machine (SVM) detection method based on particle swarm optimization has the advantages of high precision and high speed. It has certain effect and positive significance for improving the safety of liquid-propellant rocket engine test and engine failure loss.

Objective To explore the efficacy and prognostic factors of cetuximab therapy for KRAS or all RAS wild-type(WT)metastatic colorectal cancer(mCRC).Methods We screened mCRC patients receiving at least two cycles of cetuximb and chemotherapy from those with KRAS WT(before November 2013)or all-RAS-WT(after November 2013)in the Department of Medical Oncology,Peking Union Medical College Hospital from November 2007 to December 2016. The relationship between the clinicopathological characteristics and the efficacy was retrospectively analyzed.Results A total of 60 patients were included. For the 34 patients receiving cetuximab as first-line treatment,the objective response rate(ORR)was 55.9%,and the progression-free survival and overall survival(OS)was 10 and 24 months,respectively. All-RAS-WT mCRC had significantly lower risk of progression than those with KRAS-only-WT(P=0.012),and left-sided colorectal cancer had higher ORR than right-sided colon cancer(62.1% vs. 0,P=0.033)during the first-line treatment. The median OS of the eight patients continuing cetuximab beyond first-line progression was 35.0(95%CI:23.6-46.4)months.Conclusions The efficacy of cetuximab for left-sided colorectal cancer was better than for right-sided colon cancer,and patients with all-RAS-WT have lower risk of progression than those with KRAS-only-WT. Patients benefiting from first-line cetuximab and continuing cetuximab beyond progression survive longer.

Introduction Although HIV antibody tests have been widely accepted in clinical diagnosis of HIV infection, they may not be sufficient to diagnose all subjects with HIV infection. Except negative result of antibody test in the well-known" acute window phase", in rare cases, patients do not develop HIV antibodies despite demonstrable infection. Primary pulmonary Kaposi sarcoma (KS) without mucocutaneous involvement accounts for only 0-15% of all AIDS-related KS. KS is rare among Chinese subjects, especially in persons of Han descent. Methods A case of seronegative AIDS with primary pulmonary Kaposi sarcoma (KS) was reported. It's a 46-year-old Chinese man presented with sore throat, hemoptysis, fever, dyspnea and multiple lung nodules. The lung lesions grew over a 5-month period so as the symptoms worsened. The possibility of AIDS was discounted by his physicians because of the repeatedly negative HIV antibodies tests despite the ELISA tests or Western blot tests. Histopathologic diagnosis of fine needle lung biopsy in local hospital was undetermined. After admission, HIV infection was eventually confirmed by plasma HIV RNA testing. Histopathologic diagnosis of Lung Kaposi sarcoma was made through repeated fine needle aspiration biopsy as well as the review of former one. Multiple antibiotics and chemotherapy were administrated with no clinical effect due to advanced stage and the patient passed away soon after diagnosis. Results This is the first case of seronegative HIV-1 infection with presentation of primary pulmonary KS. Conclusion This case underscores the importance of plasma RNA test in conjunction with HIV antibody test for some rare patients with HIV infection who present with severe immunodeficiency and opportunistic infections or malignancy.

Background Recently, a nickel–titanium (NiTi) memory-shape device has been successfully used in gastrointestinal anastomosis. The aim of this study was to investigate the feasibility and safety of the device. Methods Four databases, reference lists, and the World Health Organization International Clinical Trials Registry Platform were systematically searched for randomized controlled trials assessing the clinical efficacy of a NiTi memory-shape device compared with that of a stapler in gastrointestinal or colorectal anastomosis. Results Seven randomized controlled trials regarding the use of compression anastomosis clips (CACs) were enrolled for meta-analysis. The use of CACs was associated with a significant reduction in hospital duration (mean = −0.88 d; 95% confidence interval [CI], −1.38 to −0.38), the time to flatus (mean = −0.36 d; 95% CI, −0.08 to −0.04), and the start of oral intake (mean = −0.45 d; 95% CI, −0.83 to −0.06), as well as a nonsignificant change in postoperative complications and mortality. These clinical outcomes did not significantly change with the use of compression anastomosis rings. Conclusions Colonic anastomosis with a CAC is likely to reduce hospital duration, time to flatus, and the start of oral intake without influencing mortality or postoperative complications and may be a safe and preferable choice in colonic anastomosis. Further well-designed trials should be performed to determine the safety and efficacy of the newly developed compression anastomosis ring in both ileocolic and colorectal anastomosis.

PEG-based hydrogels possess tissue-like mechanical elasticity, solute permeability, cytocompatibility and biocompatibility. In this work, PEG-based hydrogels were prepared via nucleophilic thiol–yne addition between a 4-arm PEG functionalized with thiols (PEG10k-4-SH) and an electron-deficient alkyne (PEG10k-4-PP). The as-fabricated hydrogels still possess residual functionalities, enabling a second nucleophilic thiol–yne addition on the gel matrix. A thiol-containing fluorescent dye was conjugated with the electron-deficient alkyne appended hydrogels. A thiol-containing antimicrobial peptide (AMP-SH) was also embedded into the gel matrix via nucleophilic thiol–yne addition. The inhibition of bacterial growth in suspensions and contaminated substrate surfaces by the AMP-embedded hydrogels was studied. The cytotoxicity of unmodified and AMP-embedded PEG-based hydrogels against 3T3 fibroblasts was investigated by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl-tetrazolium bromide (MTT) viability assay.

The copigmentation effects of polyphenol with different structures vary greatly. Therefore, the aim of this study is to investigate possible interactions in red wine model solutions between oenin and three phenolic compounds: danshensu, caffeic acid and rosmarinic acid. Our results show that the copigmentation of rosmarinic acid is the strongest among the compounds tested. The colourimetric parameters indicate that colour intensity becomes enhanced with increasing concentration of these copigments, leading to darker and more vivid bluish colours. Thermodynamic and quantum chemical investigations are performed to interpret the absorption properties in the visible range. Fluorescence spectroscopy confirms the interaction between caffeic acid and oenin, while FTIR spectroscopic results further suggest a role for hydrogen bonds in the overall process. To our knowledge, this is the first experimentally corroborated direct evidence of hydrogen bonds in copigmentation.

Macrophages are a group of immune cells with pluripotency and plasticity that can differentiate into different phenotypes under different microenvironments in vitro and in vivo. In the development of pulmonary fibrosis, there are alveolar macrophages and interstitial macrophages, which are polarized to different cell phenotypes at different stages of development. And their polarized phenotypes include M1 macrophages and M2 macrophages. In the inflammation early stages of pulmonary fibrosis, the increase of classical activated macrophages are helpful to clear pathogenic microorganisms and promote the progress of inflammation. In the fibrosis stage, the alternatively activated macrophages increased, which inhibiting the inflammatory reaction or directly promoting tissue fibrosis, on the other hand, it also promoting the fibrosis degradation. To clarify the polarization and polarization mechanisms of macrophages in pulmonary fibrosis will be conducive to the treatment of pulmonary fibrosis. In IPF, the polarization mechanism of M1 and M2 is closely related to TGF-β1/Smad. TGF-β1/Smad pathway plays an important regulatory role in liver fibrosis, renal fibrosis, myocardial fibrosis, scars, tumors and other diseases. Blocking the signaling of TGF-β1 by Smad3 and Smad4 is beneficial to inhibit the polarization of AM, which in turn helps to inhibit the progression of IPF.

Objective To investigate the clinicopathological features,treatment,and prognosis of patients with malignant peritoneal mesothelioma(MPM). Methods Clinical data of 25 MPM patients admitted to Peking Union Medical College Hospital from 1993 to 2017 were retrospectively analyzed.Results The mean age of these 25 patients with pathologically confirmed MPM(epithelioid subtype) was 50 years.The tumors were diffuse in 24 patients(96%) and localized in 1 patient(4%).Cytoreductive surgery was performed in 6 patients(24%),intraperitoneal chemotherapy in 12 patients(48%),and systemic chemotherapy in 24 patients(96%).The median overall survival was 26 months,with 1-year survival rate of 74.2% and 5-year survival rate of 16.7%.Cytoreductive surgery or intraperitoneal chemotherapy combined with systemic chemotherapy showed a significant survival advantage over intraperitoneal or intravenous chemotherapy alone(P=0.046,P=0.005).Cytoreductive surgery(P=0.018) showed statistical significance by multivariate analysis as a predictive factor in survival(hazard rate=6.889;95%CI=1.386-34.247).Conclusions MPM has its diverse clinical manifestations.Patients after cytoreductive surgery have longer survival time.Chemotherapy drugs(except for pemetrexed) and targeted therapy may be promising treatments.Cytoreductive surgery is an independent prognostic factor.
