Abstract

We selected 29 medical imaging projects from 48 candidates, assessed 10 software qualities by answering 108 questions for each software project, and interviewed 8 of the 29 development teams. Based on the quantitative data, we ranked the MI software with the Analytic Hierarchy Process (AHP). The four top-ranked software products are 3D Slicer, ImageJ, Fiji, and OHIF Viewer. Generally, MI software is in a healthy state as shown by the following: we observed 88% of the documentation artifacts recommended by research software development guidelines, 100% of MI projects use version control tools, and developers appear to use the common quasi-agile research software development process. However, the current state of the practice deviates from the existing guidelines because of the rarity of some recommended artifacts, low usage of continuous integration (17% of the projects), low use of unit testing (about 50% of projects), and room for improvement with documentation (six of nine developers felt their documentation was not clear enough). From interviewing the developers, we identified five pain points and two qualities of potential concern: lack of development time, lack of funding, technology hurdles, ensuring correctness, usability, maintainability, and reproducibility. The interviewees proposed strategies to improve the state of the practice, to address the identified pain points, and to improve software quality. Combining their ideas with ours, we have the following list of recommendations: increase documentation, increase testing by enriching datasets, increase continuous integration usage, move to web applications, employ linters, use peer reviews, design for change, add assurance cases, and incorporate a "Generate All Things" approach., Comment: 73 pages, 14 figures, 12 tables

Abstract

Purpose: Concussion management is shifting away from a rest-is-best approach, as data now suggest that exercise-is-medicine for this mild brain injury. Despite this, we have limited data on habitual physical activity following concussion. Therefore, our objective was to quantify accelerometer-measured physical activity and sedentary time in children with concussion (within the first month of injury) and healthy controls. We hypothesized that children with concussion would be less active than their healthy peers. Methods: We performed a secondary analysis of prospectively collected accelerometer data. Our sample included children with concussion (n = 60, 31 females) and historical controls (n = 60) matched for age, sex, and season of accelerometer wear. Results: Children with concussion were significantly more sedentary than controls (mean difference [MD], 38.3 min/d, P =.006), and spent less time performing light physical activity (MD, −19.5 min/d, P =.008), moderate physical activity (MD, −9.8 min/d, P <.001), and vigorous physical activity (MD, −12.0 min/d, P <.001); these differences were observed from 8:00 AM to 9:00 PM. Sex-specific analyses identified that girls with concussion were less active and more sedentary than both boys with concussion (P =.010) and healthy girls (P <.010). Conclusion: There is an activity deficit observed within the first month of pediatric concussion. Physical activity guidelines should address this while considering sex effects. Abstract

Introduction: The ideal contrast agents for ventilation SPECT and MRI are Technegas and 129Xe gas, respectively. Despite increasing interest in the clinical utility of ventilation imaging, these modalities have not been directly compared. Therefore, our objective was to compare the ventilation defect percent (VDP) assessed by Technegas SPECT and hyperpolarized 129Xe MRI in patients scheduled to undergo lung cancer resection with and without pre-existing obstructive lung disease. Methods: Forty-one adults scheduled to undergo lung cancer resection performed same-day Technegas SPECT, hyperpolarized 129Xe MRI, spirometry, and diffusing capacity of the lung for carbon monoxide (DLCO). Ventilation abnormalities were quantified as the VDP using two different methods: adaptive thresholding (VDPT) and k-means clustering (VDPK). Correlation and agreement between VDP quantified by Technegas SPECT and 129Xe MRI were determined by Spearman correlation and Bland-Altman analysis, respectively. Results: VDP measured by Technegas SPECT and 129Xe MRI were correlated (VDPT: r = 0.48, p = 0.001; VDPK: r = 0.63, p < 0.0001). A 2.0% and 1.6% bias towards higher Technegas SPECT VDP was measured using the adaptive threshold method (VDPT: 23.0% ± 14.0% vs. 21.0% ± 5.2%, p = 0.81) and k-means method (VDPK: 9.4% ± 9.4% vs. 7.8% ± 10.0%, p = 0.02), respectively. For both modalities, higher VDP was correlated with lower FEV1/FVC (SPECT VDPT: r = -0.38, p = 0.01; MRI VDPK: r=-0.46, p = 0.002) and DLCO (SPECT VDPT: r=-0.61, p < 0.0001; MRI VDPK: r = -0.68, p < 0.0001). Subgroup analysis revealed that VDP measured by both modalities was significantly higher for participants with COPD (n = 13) than those with asthma (n = 6; SPECT VDPT: p = 0.007, MRI VDPK: p = 0.006) and those with no history of obstructive lung disease (n = 21; SPECT VDPT: p = 0.0003, MRI VDPK: p = 0.0003). Discussion: The burden of ventilation defects quantified by Technegas SPECT and 129Xe MRI VDP was correlated and greater in participants with COPD when compared to those without. Our observations indicate that, despite substantial differences between the imaging modalities, quantitative assessment of ventilation defects by Technegas SPECT and 129Xe MRI is comparable.
