Objective The aim of this randomized experimental study is to analyze the effect of massage, wipe bathing and tub bathing on physiological measurements of late premature newborns. Design and Methods: This randomized experimental study was conducted on 192 newborns at the gestational week of 34 or higher in the neonatal intensive care unit of a university hospital. The researchers applied massage (n: 48), tub bathing (n: 48), and wipe bathing (n: 48) to the newborns. The control group (n: 48) received no intervention except for the routine clinical practices. The newborns' physiological measurements before, immediately after and 30 min after the interventions were taken and recorded on the “Newborn Follow-up Form.” The newborns’ physiological measurements were assessed by independent evaluators who were blinded to the purpose of the study at different phases across the massage, wipe bathing and tub bathing protocols. The statistical analysis was performed using percentages, means, repeated analysis, variance analysis, Bonferroni analysis. Results No difference was found in the massage, wipe bathing, tub bathing and control groups' mean physiological measurements (pulse, respiration, oxygen saturation, blood pressure, body temperature) (p > 0.05).

INTRODUCTION In the treatment of patients with acute bronchiolitis there is great variability in clinical practice. Treatments whose efficacy has not been demonstrated are frequently used despite the recommendations contained in the Clinical Practice Guidelines. MATERIAL AND METHODS A quality improvement strategy is implemented in the care of patients with acute bronchiolitis in the Emergency Department, which is maintained for five years and is periodically updated to be increasingly restrictive regarding the use of bronchodilators. To evaluate the impact of the intervention, a retrospective study of the rates of prescription of bronchodilators in children diagnosed with acute bronchiolitis in the month of December of four epidemic periods (2012, 2014, 2016 and 2018) was carried out. RESULTS 1767 children are included. There were no differences regarding age, respiratory rate, oxygen saturation or the estimated severity in each of the study seasons. Objective Intracordal injection under local anesthesia is widely performed; however, few studies show hemodynamic changes in the heart rate, blood oxygen saturation, and blood pressure during intracordal injection under local anesthesia. This study examined changes in vital signs (heart rate, blood oxygen saturation, systolic blood pressure, diastolic blood pressure) during intracordal injection under local anesthesia among high-risk patients and investigated whether intracordal injection under local anesthesia could be safely conducted. Methods A retrospective chart review was adopted as the research design. We investigated the changes in vital signs (heart rate, blood oxygen saturation, blood pressure) before and after intracordal injection with basic fibroblast growth factor (bFGF) preparations under local anesthesia in 46 patients who visited our institution and developed unilateral vocal cord paralysis after a thoracic aortic aneurysm, thoracic aortic dissection surgery, thyroid disease, esophageal disease, idiopathic disease, etc. Results The average operation time for the high-risk group was 3.67 minutes, with the shortest operating time being 2 minutes and the maximum operating time being 13 minutes. The average operation time for the control group was 3.73 minutes, with the shortest operating time being 1 minute and the maximum operating time being 9 minutes. Results before and after intracordal injection with bFGF preparations under local anesthesia for heart rate, blood oxygen saturation, systolic blood pressure, and diastolic blood pressure had P-values of 0.324, 0.394, 0.215, and 0.508, respectively, in the high-risk group, and no significant differences were found. Conversely, heart rate, blood oxygen saturation, systolic blood pressure, and diastolic blood pressure had P-values of 0.057, 0.232, 0.265, and 0.091, respectively, in the control group, and no significant differences were found. Conclusion Intracordal injection under local anesthesia may be safe, even for patients who require blood pressure management after thoracic aortic disease surgery.

Introduction Classic cardiovascular risk factors do not explain all the cardiovascular events. Obstructive sleep apnoea (OSA) has been proposed as a potential and prevalent cardiovascular risk factor. Our study aimed to describe the prevalence of OSA in a middle-aged cohort with mild–moderate cardiovascular risk and evaluate its association with atherosclerotic disease. Methods This is an observational cross-sectional ancillary study of the ILERVAS project which was aimed to study subclinical arterial disease in a cohort with mild–moderate cardiovascular risk. In a sample of consecutive subjects, we performed a sleep study and evaluate OSA prevalence and its association with carotid and femoral atheroma plaques and atherosclerotic burden. Results Overall, 966 subjects with a median age of 57 years (25–75th percentile; 52–62) and a body mass index (BMI) of 28.5 kg/m2 (25.6–31.6) were included. Of these, 72.6% (69.7%–75.3%) had OSA (apnoea–hypopnoea index (AHI) ≥ 5/h); 35.7% (32.8%–38.8%) had mild OSA (AHI 5–14.9/h) and 36.9% (33.9%–39.9%) had moderate/severe OSA (AHI ≥ 15/h). Mean oxygen saturation and the percentage of time with oxygen saturation Conclusions This study confirms a high prevalence of OSA in patients with mild–moderate cardiovascular risk and shows an association between atherosclerotic burden, total and femoral plaque with CT90 and mean oxygen saturation, suggesting the importance of OSA-related hypoxaemia in the induction of atherosclerotic disease.

STUDY OBJECTIVES: Biological models suggest that obstructive sleep apnea (OSA) is potentially carcinogenic. We aimed to clarify the inconsistent epidemiological literature by considering various traditional and novel OSA severity indices. METHODS: We systematically searched PubMed, Embase, Scopus, and the Cochrane Library for observational or randomized studies of associations of OSA, measured by diagnostic codes or any index, each with all-cancer incidence or mortality in adults, compared with participants with no/mild OSA. Two reviewers independently selected studies, extracted data, and evaluated study bias using the Newcastle-Ottawa scale and quality of evidence using GRADE (Grading of Recommendations Assessment, Development, and Evaluation). We performed inverse variance-weighted, random-effects meta-analyses and sensitivity analyses. RESULTS: We included 20 observational studies (5,340,965 participants), all with moderate/low bias, from 1,698 records. Based on T90 (sleep duration with oxygen saturation < 90%), patients with OSA who had moderate (T90 > 1.2%, hazard ratio [HR] = 1.28, 95% confidence interval [CI] = 1.07–1.54) and severe nocturnal hypoxemia (T90 > 12%, HR = 1.43, 95% CI = 1.16–1.76) experienced 30%–40% higher pooled all-cancer risk than normoxemic patients, after multiple adjustment for covariates including obesity. Furthermore, severe nocturnal hypoxemia nearly tripled all-cancer mortality (HR = 2.66, 95% CI = 1.21–5.85). Patients with apnea-hypopnea index–defined severe OSA, but not moderate OSA, had higher all-cancer risk (HR = 1.18, 95% CI = 1.03–1.35) but similar all-cancer mortality as patients without OSA. An OSA diagnosis was not associated with all-cancer risk. Evidence quality ranged from low to moderate. Insufficient evidence was available on the oxygen desaturation index, lowest/median saturation, and arousal index. CONCLUSIONS: In patients with OSA, nocturnal hypoxemia is independently associated with all-cancer risk and mortality. Future studies should explore if risk differs by cancer type, and whether cancer screening and OSA treatment are beneficial. BACKGROUND: Recently, the first report of lung ultrasound (LUS) guided recruitment during open lung ventilation in neonates has been published. LUS guided recruitment can change the approach to open lung ventilation, which is currently performed without any measure of lung function/lung expansion in the neonatal population. METHODS: We included all the newborn infants that underwent a LUS-guided recruitment maneuver during mechanical ventilation as a rescue attempt for an extremely severe respiratory condition with oxygen saturation/fraction of inspired oxygen (SpO2/FIO2) ratio below 130 or the inability to wean off mechanical ventilation. RESULTS: We report a case series describing 4 LUS guided recruitment maneuvers, underlying crucial aspects of this technique that can improve the effectiveness of the procedure. In particular, we describe a novel pattern (the S-pattern) that allows us to distinguish the recruitable from the unrecruitable lung and guide the pressure titration phase. Additionally, we describe the optimal LUS-guided patient positioning. CONCLUSIONS: We believe that the inclusion of specifications regarding patient positioning and the S-pattern in the LUS-guided protocol may be beneficial for the success of the procedure.

PURPOSE To investigate the effects in cataract surgery using local anesthesia along with a 30% low-concentration nitrous oxide (N2O) anesthesia compared with local anesthesia only. SETTING Saneikai Tsukazaki Hospital. DESIGN Retrospective, consecutive study. METHODS Patients who underwent bilateral cataracts surgeries were enrolled. 37 patients using room air inhalation (Air group) and 45 patients using 30% low-concentration N2O anesthesia (70% oxygen, total 6 L/min) at the surgery's beginning (N2O group) were retrospectively reviewed. Systolic blood pressure (BPs), diastolic blood pressure (BPd), and heart rate (HR) at the surgery's beginning and end, and mean intraoperative oxygen saturation (% SpO2) were examined. Immediately following surgery, a questionnaire using the Visual Analog Scale score was done to determine intraoperative pain, anxiety, memory, and nausea. RESULTS No systemic symptoms and ocular complications requiring treatment were observed. For the N2O and Air groups, changes in BPs were -5.38 ± 11.07(P = 0.01) and 1.27 ± 13.61 mmHg, and HR were -2.24 ± 6.76 and 0.89 ± 5.18 bpm (P = 0.001), respectively; intraoperative SpO2 was 99.05 ± 0.74% and 97.44 ± 1.31% (P < 0.001), intraoperative anxiety was 21.76 ± 23.2 and 37.17 ± 32.79(P = 0.002), and intraoperative memory was 55.24 ± 36.8 and 68.91 ± 33.81(P = 0.01), respectively. No patients experienced intraoperative nausea. There was no statistically difference in BPd and intraoperative pain. CONCLUSIONS Low-concentration N2O anesthesia may not cause respiratory depression, abnormal vital signs, or nausea in cataract surgery. It can suppress intraoperative anxiety and memory and decrease and stabilize vital signs.

Background Strict isolation precautions limit formal echocardiogram use in the setting of COVID-19 infection. Information on the importance of the utilization of a hand-held focused ultrasound for cardiac evaluation in these patients is scarce. This study investigated the utility of a hand-held echocardiogram device in COVID-19 hospitalized patients in diagnosing cardiac pathologies and predicting the composite endpoint of in-hospital death, mechanical ventilation, shock, and acute decompensated heart failure. Methods From April 28th through July 27th, 2020, consecutive patients diagnosed with COVID-19 underwent evaluation using a hand-held ultrasound (Vscan Extend™ with Dual Probe; GE Healthcare) within 48-h of admission. The patients were divided into two groups: 'Normal' and 'Abnormal' (as defined by biventricular systolic dysfunction/enlargement, or moderate/severe valvular regurgitation/stenosis) echocardiogram study. Results Among 102 patients, 26 (25.5%) had an abnormal study. They were older, with more co-morbidities, and more severe presenting symptoms, as compared to the group with a normal echocardiogram. A 70-year-old man with severe interstitial pneumonia attributed to limited cutaneous systemic sclerosis was referred to our institution because of worsening dyspnea. High-resolution computed tomography did not show considerable progression compared with previous images, whereas transthoracic echocardiography showed severe right ventricular dysfunction. Oxygen saturation was decreased to 84% at room air. A blood test showed an increase in the plasma brain natriuretic peptide level (289.4 pg/mL). Right heart catheterization (RHC) showed a remarkably high mean pulmonary arterial pressure (mPAP) of 48 mmHg at room air. A vaso reactivity test using inhaled nitric oxide showed improvement of mPAP, pulmonary vascular resistance (PVR), and partial pressure of arterial oxygen. These findings suggested that the patient responded to pulmonary hypertension (PH)-targeted drugs. We then prescribed tadalafil 10 mg and inhaled iloprost 5 µg six times daily. Three weeks after initiating PH-targeted drugs, RHC indicated hemodynamic improvement similar to hemodynamic changes in the vaso reactivity test (mPAP: 28 mmHg; PVR: 4.2 W.U.). He was discharged with improved symptoms. Inhaled nitric oxide during RHC might be helpful to consider the treatment strategy when patients have PH comorbid systemic sclerosis and severe interstitial lung disease.

INTRODUCTION Chest physiotherapy (CPT) may benefit children aged below five years who suffer from lower respiratory tract infection (LRTI). However, its effects depend on the technique used. This study aimed to determine whether mechanical CPT using the LEGA-Kid® mechanical percussion device is superior to manual CPT in children with LRTI. METHODS Children aged five months to five years who were admitted and referred for CPT from January to April 2017 were randomised to either manual CPT or mechanical CPT with LEGA-Kid. Outcomes measured before intervention and two hours after intervention were respiratory rate (RR), oxygen saturation and modified Respiratory Distress Assessment Instrument (mRDAI) score. RESULTS All 30 enrolled patients showed significant reduction in post-intervention RR and mRDAI scores. There was an 8% reduction in RR for the manual CPT group (p = 0.002) and a 16.5% reduction in the mechanical CPT group (p = 0.0001), with a significantly greater reduction in the latter (p = 0.024). mRDAI scores decreased by 2.96 in the manual group (p = 0.0001) and 3.62 in the mechanical group (p = 0.002), with no significant difference between the groups. There was no significant improvement in oxygen saturation, and no adverse events were observed after CPT. CONCLUSION Children receiving both manual and mechanical CPT showed improvements in respiratory distress symptoms, with no adverse effects. A combined strategy of nebulised hypertonic saline followed by CPT for LRTI removes airway secretions and results in improvements in moderately severe respiratory distress. The LEGA-Kid mechanical CPT method is superior to manual CPT in reducing the RR.

An 89-year-old woman was admitted to our hospital for subacute onset of right upper and lower limb weakness and was diagnosed with acute cerebral infarction. During rehabilitation, close observation revealed that her oxygen saturation decreased in the sitting position and improved in the recumbent position without any subjective symptoms of dyspnea. Transthoracic and transesophageal echocardiography and cardiac catheterization revealed a large patent foramen ovale with an atrial septal aneurysm with right-to-left shunting through the defect, and she was diagnosed with platypnea-orthodeoxia syndrome. Her right hemiplegia caused the trunk to collapse, so the patient slumped when in sitting position, and the trunk tilted to the right forward, resulting in an increased right-to-left shunt. Her peripheral capillary oxygen saturation improved in the upright sitting position supported by therapists. This case suggests that right hemiplegia may exacerbate the symptoms of platypnea-orthodeoxia syndrome.

Chronic wounds are a major health problem that cause the medical infrastructure billions of dollars every year. Chronic wounds are often difficult to heal and cause significant discomfort. Although wound specialists have numerous therapeutic modalities at their disposal, tools that could 3D-map wound bed physiology and guide therapy do not exist. Visual cues are the current standard but are limited to surface assessment; clinicians rely on experience to predict response to therapy. Photoacoustic (PA) ultrasound (US) is a non-invasive, hybrid imaging modality that can solve these major limitations. PA relies on the contrast generated by hemoglobin in blood which allows it to map local angiogenesis, tissue perfusion and oxygen saturation—all critical parameters for wound healing. This work evaluates the use of PA-US to monitor angiogenesis and stratify patients responding vs. not-responding to therapy. We imaged 19 patients with 22 wounds once a week for at least three weeks. Our findings suggest that PA imaging directly visualizes angiogenesis. Patients responding to therapy showed clear signs of angiogenesis and an increased rate of PA increase (p = 0.002). These responders had a significant and negative correlation between PA intensity and wound size. Hypertension was correlated to impaired angiogenesis in non-responsive patients. The rate of PA increase and hence the rate of angiogenesis was able to predict healing times within 30 days from the start of monitoring (power = 88%, alpha = 0.05) This early response detection system could help inform management and treatment strategies while improving outcomes and reducing costs.

Objective To compare the potential healing prognosis of the different routine noninvasive techniques implemented in the International Working Group Diabetic Foot Guidelines with the novel use of hyperspectral imaging (HSI) in patients with diabetic foot ulcers (DFUs). Methods Twenty-one patients with active DFUs participated in this 1-year prospective study in a specialized diabetic foot unit between December 2018 and January 2020. HSI was performed at baseline to quantify tissue oxygenation and should be presented on an anatomical map by analyzing the following parameters: (1) oxygen saturation of the hemoglobin, (2) tissue hemoglobin index, (3) the near-infrared perfusion index, and (4) tissue water index. In addition, transcutaneous oxygen pressure (TcpO2), systolic toe and ankle pressures, ankle-brachial index, and toe-brachial index values were calculated for the ulcerated limb. The primary outcome measure was wound healing, defined as complete epithelization without any drainage confirmed for at least 10 days after closure was first documented at 24 weeks. Results During the follow-up period 14 patients (66.66 %) healed and 7 patients did not heal (33.3%) by 24 weeks. The TcpO2 optimal cut-off point as determined by a balance of sensitivity and specificity of 28.5 mm Hg that yielded a sensitivity of 91% and a specificity of 100%, and area under the curve of 0.989 (P = .005; 95% confidence interval [CI], 0.945-1.000). Followed by the oxygen saturation of the hemoglobin optimal cut-off point as determined by a balance of sensitivity and specificity of 48.5 mm Hg that yielded a sensitivity of 93% and a specificity of 0.71%, and area under the curve of 0.932 (P = .013; 95% CI, 0.787-1.000). The logistic regression analyses showed that TcpO2 was the only variable associated with wound healing at 24 weeks (P Conclusions The HSI was shown to be effective in the prognosis of DFU healing compared with other noninvasive test; only TcpO2 values resulted in better diagnosis potential in wound healing.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has caused a pandemic since early 2020. The coronavirus disease 2019 (COVID-19) has already caused more than three million deaths worldwide and affected people's physical and mental health. COVID-19 patients with mild symptoms are generally required to self-isolate and monitor for symptoms at least for 14 days in the case the disease turns towards severe complications. In this work, we overviewed the impact of COVID-19 on the patients' general health with a focus on their cardiovascular, respiratory and mental health, and investigated several existing patient monitoring systems. We addressed the limitations of these systems and proposed a wearable telehealth solution for monitoring a set of physiological parameters that are critical for COVID-19 patients such as body temperature, heart rate, heart rate variability, blood oxygen saturation, respiratory rate, blood pressure, and cough. This physiological information can be further combined to potentially estimate the lung function using artificial intelligence (AI) and sensor fusion techniques. The prototype, which includes the hardware and a smartphone app, showed promising results with performance comparable to or better than similar commercial devices, thus potentially making the proposed system an ideal wearable solution for long-term monitoring of COVID-19 patients and other chronic diseases.

The prevalence of sleep-disordered breathing (SDB) is higher in older adults compared with younger individuals. The increased propensity for ventilatory control instability in older adults may contribute to the increased prevalence of central apneas. Reductions in the cerebral vascular response to CO(2) may exacerbate ventilatory overshoots and undershoots during sleep. Thus, we hypothesized that hypercapnia-induced cerebral vasodilation (HCVD) will be reduced in older compared with younger adults. In 11 older and 10 younger adults with SDB, blood flow velocity in the middle cerebral artery (MCAV) was measured using Doppler transcranial ultrasonography during multiple steady-state hyperoxic hypercapnic breathing trials while awake, interspersed with room air breathing. Changes in ventilation, MCAV, and mean arterial pressure (MAP) via finger plethysmography during the trials were compared with baseline eupneic values. For each hyperoxic hypercapnic trial, the change (Δ) in MCAV for a corresponding change in end-tidal CO(2) and the HCVD or the change in cerebral vascular conductance (MCAV divided by MAP) for a corresponding change in end-tidal CO(2) was determined. The hypercapnic ventilatory response was similar between the age groups, as was ΔMCAV/Δ [Formula: see text]. However, compared with young, older adults had a significantly smaller HCVD (1.3 ± 0.7 vs. 2.1 ± 0.6 units/mmHg, P = 0.004). Multivariable analyses demonstrated that age and nadir oxygen saturation during nocturnal polysomnography were significant predictors of HCVD. Thus, our data indicate that older age and SDB-related hypoxia are associated with diminished HCVD. We hypothesize that this impairment in vascular function may contribute to breathing instability during sleep in these individuals. NEW & NOTEWORTHY This study demonstrates, for the first time, in individuals with sleep-disordered breathing (SDB) that aging is associated with decreased hypercapnia-induced cerebral vasodilation (HCVD). In addition to advanced age, the magnitude of nocturnal oxygen desaturation due to SDB is an equal independent predictor of HCVD.

Background: The relationship between sleep disorders and diabetes is bidirectional; sleep disorders can raise the risk of developing insulin resistance, while diabetes would worsen sleep quality. Objectives: Assessment of the prevalence and severity of sleep apnea among patients with type II diabetes mellitus. Patients and Methods: a prospective cross-sectional study was carried out on 45 cases with type II diabetes mellitus that were either admitted in Aswan University hospital or sought medical advice in the out-patient clinics during the period from 2017 to 2019. For every patient, the demographic and clinical data were collected, blood glucose levels and HBA1c were measured and full night attended polysomnography was done. Results: 77.78% of the diabetic cases had sleep apnea syndrome. The most common pattern was obstructive sleep apnea (82.86%). Apnea hypopnea index, obstructive sleep apnea and mixed sleep apnea were considerably higher among cases who had clinical picture of neuropathy compared to those without clinical picture of neuropathy. There was a moderate positive correlation between HbA1c with both AHI (P =0.005, r= 0.464) and obstructive sleep apnea (P value 0.016, r= 0.405). BackgroundNon-invasive oxygen saturation (SpO2) measurement is a central vital sign that supports the management of COVID-19 patients. However, reports on SpO2 characteristics (patterns and dynamics) are scarce and none, to our knowledge, has analysed high resolution continuous SpO2 in COVID-19. MethodsSpO2 signal sampled at 1Hz and clinical data were collected from COVID-19 departments at the Rambam Health Care Campus (Haifa, Israel) between May 1st, 2020 and February 1st, 2021. Data from a total of 367 COVID-19 patients, totalling 27K hours of continuous SpO2 recording, could be retrieved, including 205 non-critical and 162 critical cases. Desaturations based on different SpO2 threshold definitions and oximetry derived digital biomarkers (OBMs) were extracted and compared across severity and support levels. FindingsAn absolute SpO2 threshold at 93% was the most efficient in discriminating between critical and non-critical patients without support or under oxygen support. Under no support, the non-critical group depicted a fold change (FC) of 1 {middle dot}8 times more frequent desaturations compared to the critical group. However, the hypoxic burden was 1 {middle dot}6 times more important in critical versus non-critical patients. Other OBMs depicted significant differences, notably the percentage of time below 93% SpO2 (CT93) was the most discriminating OBM. Mechanical ventilation depicted a strong effect on SpO2 by significantly reducing the frequency (1 {middle dot}85 FC) and depth (1 {middle dot}21 FC) of desaturations. OBMs related to periodicity and hypoxic burden were markedly affected up to several hours before the initiation of the mechanical ventilation. InterpretationThis is the first report investigating continuous SpO2 measurements in hospitalized patients affected with COVID-19. SpO2 characteristics differ between critical and non-critical patients and are impacted by the level of support. OBMs from high resolution SpO2 signal may enable to anticipate clinically relevant events, monitoring of treatment response and may be indicative of future deterioration. BACKGROUND Children with chronic neurological diseases present motor disability and increased respiratory morbidity and mortality. The aim of this study was to assess whether children with chronic neurological impairment benefit from respiratory physiotherapy and postural hygiene. METHODS Quasi-experimental study in which six fortnightly respiratory physiotherapy sessions and workshops on postural hygiene were carried out on children aged 0 to 6 years with chronic neurological disease and respiratory impairment. A PedsQL questionnaire assessed respiratory clinical variables, expectorated secretions, respiratory exacerbations and quality of life. An 89-year-old woman was diagnosed with subacute cerebellar infarction. Her blood oxygen saturation decreased to 88% in a sitting position, resulting in dyspnea. Cardiological thoracic computed tomography revealed an unruptured aortic aneurysm, an enlarged ascending aorta, right atrial compression, and counterclockwise rotation of the heart. An anatomical distortion of the atrial septum induced by these abnormalities directed the atrial venous inflow such that the right-left shunt flow was exacerbated in a sitting position.

Background Previous research shows that a lung ultrasound score (LUS) can anticipate CPAP failure in neonatal respiratory distress syndrome. Research Question Can LUS also predict the need for surfactant replacement? Study Design and Methods Multicenter, pragmatic study of preterm neonates who underwent lung ultrasound at birth and those given surfactant by masked physicians, who also were scanned within 24 h from administration. Clinical data and respiratory support variables were recorded. Accuracy of LUS, oxygen saturation to F io 2 ratio, F io 2, and Silverman score for surfactant administration were evaluated using receiver operating characteristic curves. The simultaneous prognostic values of LUS and oxygen saturation to F io 2 ratio for surfactant administration, adjusting for gestational age (GA), were analyzed through a logistic regression model. Results Two hundred forty infants were enrolled. One hundred eight received at least one dose of surfactant. LUS predicted the first surfactant administration with an area under the receiver operating characteristic curve (AUC) of 0.86 (95% CI, 0.81-0.91), cut off of 9, sensitivity of 0.79 (95% CI, 0.70-0.86), specificity of 0.83 (95% CI, 0.76-0.89), positive predictive value of 0.79 (95% CI, 0.71-0.87), negative predictive value of 0.82 (95% CI, 0.75-0.89), positive likelihood ratio of 4.65 (95% CI, 3.14-6.89), and negative likelihood ratio of 0.26 (95% CI, 0.18-0.37). No significant difference was shown among different GA groups: 25 to 27 weeks’ GA (AUC, 0.91; 95% CI, 0.84-0.99), 28 to 30 weeks’ GA (AUC, 0.81; 95% CI, 0.72-0.91), and 31 to 33 weeks’ GA (AUC, 0.88; 95% CI, 0.79-0.95), respectively. LUS declined significantly within 24 h in infants receiving one surfactant dose. When comparing F io 2, oxygen saturation to F io 2 ratio, LUS, and Silverman scores as criteria for surfactant administration, only the latter showed a significantly poorer performance. The combination of oxygen saturation to F io 2 ratio and LUS showed the highest predictive power, with an AUC of 0.93 (95% CI, 0.89-0.97), regardless of the GA interval. Interpretation LUS is a reliable criterion to administer the first surfactant dose regardless of GA. Its association with oxygen saturation to F io 2 ratio significantly improves the prediction power for surfactant need.

Available evidence indicates that elevated blood ketones are associated with improved hypoxic tolerance in rodents. From this perspective, we hypothesized that exogenous ketosis by oral intake of the ketone ester ( R)-3-hydroxybutyl ( R)-3-hydroxybutyrate (KE) may induce beneficial physiological effects during prolonged exercise in acute hypoxia. As we recently demonstrated KE to deplete blood bicarbonate, which per se may alter the physiological response to hypoxia, we evaluated the effect of KE both in the presence and absence of bicarbonate intake (BIC). Fourteen highly trained male cyclists performed a simulated cycling race (RACE) consisting of 3-h intermittent cycling (IMT180′) followed by a 15-min time-trial (TT15′) and an all-out sprint at 175% of lactate threshold (SPRINT). During RACE, fraction of inspired oxygen ([Formula: see text]) was gradually decreased from 18.6% to 14.5%. Before and during RACE, participants received either 1) 75 g of ketone ester (KE), 2) 300 mg/kg body mass bicarbonate (BIC), 3) KE + BIC, or 4) a control drink in addition to 60 g of carbohydrates/h in a randomized, crossover design. KE counteracted the hypoxia-induced drop in blood ([Formula: see text]) and muscle oxygenation by ∼3%. In contrast, BIC decreased [Formula: see text] by ∼2% without impacting muscle oxygenation. Performance during TT15′ and SPRINT were similar between all conditions. In conclusion, KE slightly elevated the degree of blood and muscle oxygenation during prolonged exercise in moderate hypoxia without impacting exercise performance. Our data warrant to further investigate the potential of exogenous ketosis to improve muscular and cerebral oxygenation status, and exercise tolerance in extreme hypoxia.

Wearable devices with integrated sensors for tracking human vitals are widely used for a variety of applications, including exercise, wellness, and health monitoring. Photoplethysmography (PPG) sensors use pulse oximetry to measure pulse rate, cardiac cycle, oxygen saturation, and blood flow by passing a light beam of variable wavelength through the skin and measuring its reflection. A multi-channel PPG wearable system was developed to include multiple nodes of pulse oximeters, each capable of using different wavelengths of light. The system uses sensor fusion along with machine learning model to perform feature extraction of relevant cardiovascular metrics across multiple pulse oximeters and predict saturated oxygen (SpO 2 ). The developed model predicted SpO 2 with a root mean square (RMSE) of 0.07 and accuracy of 99.5%. The wearable system was applied to the plant of the foot for vascular assessment. Wearable PPG systems capable of sensor fusion demonstrates a potential capability for continuous evaluation/monitoring of wounds and diseases associated with abnormal blood flow.

Objectives - To observe changes in sleep from baseline and during an altitude training camp in elite endurance athletes. Design - Prospective, observational. Setting - Baseline monitoring at Participants - Thirty-three senior national-team endurance athletes (mean age 25.8 ± S.D. 2.8 years, 16 women). Measurements - Daily measurements of sleep (using a microwave Doppler radar at baseline and altitude), oxygen saturation (SpO2), training load and subjective recovery (at altitude). Results - At altitude vs. baseline, sleep duration (P = .036) and light sleep (P < .001) decreased, while deep sleep (P < .001) and respiration rate (P = .020) increased. During the first altitude week vs. baseline, deep sleep increased (P = .001). During the first vs. the second and third altitude weeks, time in bed (P = .005), sleep duration (P = .001), and light sleep (P < .001) decreased. Generally, increased SpO2 was associated with increased deep sleep while increased training load was associated with increased respiration rate. Conclusion - This is the first study to document changes in sleep from near-sea-level baseline and during a training camp at 1800 m in elite endurance athletes. Ascending to altitude reduced total sleep time and light sleep, while deep sleep and respiration rate increased. SpO2 and training load at altitude were associated with these responses. This research informs our understanding of the changes in sleep occurring in elite endurance athletes attending training camps at competition altitudes.

Patients with massive pulmonary embolism undergoing catheter-directed therapy are at high risk for cardiopulmonary arrest in the periprocedural period due to severe right ventricular dysfunction. We report the outcomes of three patients with massive pulmonary embolism treated successfully with catheter-directed thrombolytic therapy and veno-arterial extracorporeal membrane oxygenation.

Despite providing cerebral tissue oxygen saturation (StOTo investigate if the additional monitoring of cerebral blood volume by TD-NIRS facilitates the understanding of cerebral hemodynamic conditions in patients with congenital heart disease.Preoperative TD-NIRS values were retrospectively reviewed in patients who underwent a cardiac surgery or catheter examination. We compared the values between patients with single and two ventricles. Moreover, we investigated the association of these values with the demographic and clinical variables.There was no significant difference in StO2 between single ventricle and two ventricles groups (median: 59.9 vs. 54.4, median difference [95% CI]: -4.06 [-9.90 to 2.90], p = .37). However, cerebral blood volume was significantly higher in the single ventricle group (median: 4.68 vs. 2.84, median difference [95% CI]: -2.01 [-2.88 to -1.06], p .001). Spearman's rank correlation analysis demonstrated an association between StO2 and postmenstrual age (r = 0.35, p = .03). In contrast, cerebral blood volume was correlated with single ventricle physiology (r = 0.62, p .001), postmenstrual age (r = 0.74, p .001), central venous pressure (r = 0.38, p = .02), and SaO2 (r = -0.38, p = .02). The multivariable regression analysis identified the postmenstrual age, single ventricle physiology, and SaO2 as independent factors associated with cerebral blood volume. In the logistic analysis, cerebral blood volume was identified as a significant predictor of unfavorable conditions.Cerebral blood volume monitoring detected differences in cerebral hemodynamic conditions, related to the age and the type of ventricle physiologies. Background: The coronavirus disease 2019 (COVID-19) pandemic is an ongoing global crisis challenging the worldwide healthcare systems. Many patients present with a mismatch of profound hypoxemia and few signs of respiratory distress (i.e., silent hypoxemia). This particular clinical presentation is often cited, but data are limited. Main Body: We describe dyspnea sensation as assessed by using the BORG scale in pulmonary patients admitted to the emergency room during a 4-week period and transferred to the respiratory department of Siloah Hospital, Hannover, Germany. From October 1 to November 1, 2020, 82 patients with hypoxemia defined as oxygen demand to achieve an oxygen saturation (SpO2) ≥92% were included. In 45/82 (55%) patients, SARS-CoV-2 was detected by PCR on admission. Among non-COVID patients, exacerbation of COPD was the main diagnosis (15/37, 41%). All subjects rated their perceived dyspnea using the modified Borg CR10 scale. Patients in the non-COVID group suffered from more dyspnea on the modified Borg CR10 scale (median 1, IQR: 0–2 vs. median 5, IQR: 3–6, p < 0.001). In multivariate analysis, “silent hypoxemia” as defined by the dyspnea Borg CR10 scale ≥5 was independently associated with COVID-19 and presence of severe hypocapnia with an odds ratio of 0.221 (95% confidence interval 0.054, 0.907, p 0.036). Conclusion: Among pulmonary patients with acute hypoxemia defined as oxygen demand, patients suffering from COVID-19 experience less dyspnea compared to non-COVID patients. “Silent” hypoxemia was more common in COVID-19 patients.

Introduction The present study was undertaken to investigate the prognostic value of the frontal QRS-T angle associated with adverse cardiac outcomes in patients with carbon monoxide (CO) poisoning in early stages in the emergency department. Materials and methods The data of 212 patients with CO poisoning who were admitted to the ED between January 2010 and May 2020 were retrospectively analyzed. The frontal QRS-T angle was obtained from the automatic reports of the EKG device. Results Compared to patients without myocardial damage, among patients with myocardial damage, statistically high creatinine, creatine kinase MB, cardiac troponin I, and frontal QRS-T angle values were found ( p < 0.001 for all parameters), while the saturation of arterial blood pH and arterial oxygen values were found to be lower ( p = 0.002 and p < 0.001, respectively). The frontal QRS-T angle values were correlated with creatine kinase, creatine kinase-MB, cardiac troponin I, and oxygen saturation (SpO2) in arterial blood (r = 0. 232, p = 0.001; r = 0. 253, p = < 0.001; r = 0. 389, p = < 0.001; r = −0. 198, p = 0.004, respectively). The optimum cut-off value of the frontal QRS-T angle was found to be 44.5 (area under the curve: 0.901, 95% confidence interval: 0.814–0.988, sensitivity: 87%, specificity: 84%). Conclusions The frontal QRS-T angle, a simple and inexpensive parameter that can be easily obtained from 12-lead surface electrocardiography, can be used as an early indicator in the detection of myocardial damage in patients with CO poisoning.

The present study compared the effects of eccentric cycling (ECC) and conventional concentric cycling (CONC) training on muscle function, body composition, functional performance, and quality of life (QOL) of patients with moderate chronic obstructive pulmonary disease (COPD). Twenty patients (age: 69.6 ± 10.1 years, forced expiratory volume in 1-s: 73.2 ± 11.4% of predicted) were randomly allocated to ECC (n = 10) or CONC (n = 10) group. They performed 12 weeks of ECC or CONC training at similar perceived exertion. The workload, heart rate (HR), blood oxygen saturation (SpO2), and dyspnea were monitored during cycling. Aim and objectives: To evaluate the behavior changes in children aged 4-6 years and correlate the physiological variations in children and their mothers during first dental visit, and assess the influence of maternal anxiety on child’s anxiety. Materials and Methods: 60 children of 4-6 years accompanied by their mother visiting the Department of Pedodontics and Preventive dentistry for the first time. Frankle’s behavior rating scale was used to record behavior of children. The blood pressure and heart rate was measured with digital blood pressure and heart rate monitor with wrist cuff and oxygen saturation with pulse oximeter and same were used to evaluate the physiological variations in the mother. Result: Significant Changes in systolic blood pressure and heart rate in children(p=0.0001 ,p=0.0001) were observed while waiting and on completion of the procedure Gradual lowering of blood pressure and heart rate after procedure (p=0.0001) Mothers were anxious while waiting and less anxious after procedure (p=0.0001,p=0.0001) Conclusion: Majority of the children exhibited high anxiety initially while waiting.The anxiety gradually diminished after procedure which was observed through their positive behavior. Maternal anxiety was high while waiting which gradually comes down after procedure Maternal anxiety does have some impact on the child’s anxiety at least initially. BACKGROUND: The optimal oxygen saturation target in preterm infants is not known. In this study, we aimed to assess the effect of lower oxygen saturation targets on the rate of bronchopulmonary dysplasia (BPD), retinopathy of prematurity (ROP), and pulmonary hypertension (PH) in preterm infants. METHODS: Retrospective cohort study comparing BPD, ROP, and PH incidence among two cohorts of infants born at≤32 weeks gestation with different oxygen saturation targets at≥34 weeks post-menstrual age (PMA): cohort 1, 94–98% (n = 126); cohort 2, 92–97% (n = 121). Groups compared by Chi-square test, t-test, and multivariable logistic regression. RESULTS: When comparing cohort 1 (average gestational age 29.8 weeks, average birth weight 1271g) with cohort 2 (average gestational age 29.6 weeks, average birth weight 1299g), there was no difference in rate of BPD (24% vs. 19%, p = 0.38), ROP (4% vs. 3%, p = 0.49), or PH (2% vs. 4%, p = 0.44). CONCLUSION: An oxygen saturation target of 92–97% at≥34 weeks PMA was not associated with a higher rate of PH or lower rate of BPD or ROP when compared with a higher target of 94–98%.

Introduction: Hypoxia, often referred to as “silent killer,” a common aeromedical stressor in aviation, may have catastrophic events in-flight unless recognized well in time. On exposure to hypoxia, an individual manifests a specific spectrum of symptoms referred to as “hypoxia signature.” The present study was conducted to assess the manifestation of “hypoxia signature” on repeated exposure to simulated hypobaric hypoxia for its potential usage as a tool for hypoxia recognition. Material and Methods: Twenty-two healthy adult volunteers were subjected to a simulated altitude of 22,000 feet for a duration of 5 min in the hypobaric altitude chamber. The symptoms experienced by the participants at the said altitude were recorded using a questionnaire. The heart rate (HR) and oxygen saturation (SpO2) were recorded during the exposure. The hypoxia exposure was repeated two more times with a minimum interval of 3 weeks between each. Paired t-test was used to compare the mean values of physiological parameters (HR and SpO2) between ground level and 22000 feet recorded in all the three exposures. The hypoxia symptoms and their severity reported during the exposures were compared with those of recalled symptoms (reported after 3 weeks of exposure) using McNemar test and Wilcoxon Signed Rank test, respectively. Results: Paired t-test revealed a statistically significant increase in HR and fall in SpO2 with rise in altitude from ground level to 22000 feet. The three most common symptoms consistently observed were lightheadedness, thinking slow, and warm feeling. The common hypoxic symptoms and their severity scores reported at 22,000 feet compared with recalled counterpart during subsequent exposures did not reveal any significant differences (P > 0.05). Conclusion: There was a high degree of similarity in the frequency and severity score of symptoms between acute exposure to hypobaric hypoxia and recall indicating evidence of repeatability of symptoms across the three sessions of hypoxia exposure within the individuals. This brings out the usefulness of “hypoxia signature” as a tool for hypoxia recognition and its application in hypoxia indoctrination and training for aircrew.

Hypoxia is the most frequently occurring adverse effect during endoscopic retrograde cholangiopancreatography (ERCP) under sedation; thus, oxygen must be properly supplied to prevent a reduction of oxygen saturation. In this study, we intend to verify the preventive effect for hypoxia during ERCP, using a high-flow nasal cannula (HFNC), in elderly patients. As a multicenter prospective randomized trial, patients who underwent ERCP with propofol-based sedation were randomly assigned into two groups: Patients in the HFNC group were supplied with oxygen via an HFNC, and those in the standard nasal cannula group were supplied with oxygen via a low-flow nasal cannula. The co-primary end points were the lowest oxygen saturation rate and hypoxia during the overall procedure. A total of 187 patients (HFNC group: 95; standard nasal cannula group: 92) were included in the analysis. Unexpected hypoxia events were more frequently observed among patients in the standard nasal cannula group than among patients in the HFNC group (13% vs. 4%, odds ratio 3.41, 95% confidence interval 1.06–11.00, p = 0.031). The mean of the lowest oxygen saturation rate during ERCP was significantly lower in the standard nasal cannula group than in the HFNC group (95% vs. 97%, p = 0.002). Oxygen supplementation with an HFNC can prevent oxygen desaturation and hypoxia events in patients undergoing ERCP under sedation. Background: Stepwise lung recruitment maneuvers (LRMs) may be used in ventilated preterm infants. However, its use in high-frequency oscillation with volume guarantee (HFO-VG) is not well studied. Methods: Preterm infants treated with HFO-VG who had LRMs were identified. Patient and respiratory parameters were recorded. Results: Ten infants, median GA 25+6 (IQR 24+2–27+0) weeks, and 21 LRMs were identified. LRMs were performed at a median age of 26 days, with a starting MAP of 16 (14–17) cm H2O and the highest MAP of 23.5 (22.0–24.8) cm H2O. Most (76%) resulted in immediate improved SpO2/FiO2. There were no sustained differences in median oxygen saturation index (8.4 vs. 9, p = 0.09), SpO2/FiO2 (1.8 vs. 1.8, p = 0.8), ∆P (21 vs. 23, p = 0.64), or transcutaneous CO2 (58 vs. 60, p = 0.84) in 24 h before and after LRMs. Conclusions: In preterm infants with evolving bronchopulmonary dysplasia, LRMs on HFO-VG did not result in sustained improvement to oxygenation or ventilation.

Purpose The purpose of this study is to examine the effects of fatigue on muscular performance, oxygenation saturation, and cognition following acute hypoxic exposure at Normoxia, Moderate Hypoxia (MH), and Severe Hypoxia (SH). Methods Twelve males performed 3 sets of leg extensions to failure under Normoxia (FiO2: 21%), MH (Fi02: 15.4%), and SH (Fi02: 12.9%). Heart rate, peripheral oxygenation saturation, total saturation index, psychomotor vigilance testing reaction time, psychomotor vigilance error rate, maximum strength, and repetitions to failure were measured throughout each visit. Results The primary findings indicated that MH and SH resulted in significant decreases in psychomotor vigilance test performance (MH: 388.25–427.17 ms, 0.41–0.33 error rate; SH: 398.17–445.42 ms reaction time, 0.25–1.00 error rate), absolute muscle tissue oxygen saturation (Abs-StO2) (MH:67.22% compared to SH:57.56%), but similar muscular strength, heart rate, and patterns of muscle tissue oxygen saturation responses (StO2%) during fatigue when compared to Normoxia. There was an acute decrease in the ability to remain vigilant and/or respond correctly to visual stimuli as indicated by the worsened reaction time (PVTRT) during MH (FiO2: 15.4%) and increased PVTRT and error rate (PVTE) during SH (FiO2: 12.9%) conditions. Conclusions Acute hypoxic exposure in the current study was not a sufficient stimuli to elicit hypoxic-related changes in HR, muscular strength (1-RM), or repetitions to failure. The SpO2 responses were hypoxic-level dependent with increasing levels of hypoxia resulting in greater and more sustained reductions in SpO2. The combined SpO2 and StO2 responses at MH and SH suggested a balance between the muscles metabolic demand remaining lower than the muscle oxygen diffusion capacity. During the SH condition, Abs-StO2 suggested greater metabolic stress than Normoxia and MH conditions during the fatiguing leg extensions. The patterns of responses for StO2% during the three sets of leg press to failure indicated that exercise is a more potent influencer to muscle oxygenation status than hypoxic conditions (FiO2: 15.4 and 12.9%)., Graphical abstract Image 1, Highlights • Moderate Hypoxia (FiO2: 15.4%) coupled with fatigue resulted in a decrease in reaction time. • Severe Hypoxia (FiO2: 12.9%) coupled with fatigue resulted in increased error rate and a decrease in reaction time. • Fatigue was the primary influencer on heart rate, muscular strength, and muscular endurance and not hypoxic exposure. • Severe Hypoxia (FiO2: 12.9%) resulted in greatly reduced muscle tissue oxygenation saturation before and after fatigue.

ObjectiveTo investigate the efficacy of automated control of inspired oxygen (FiO2) by Predictive Intelligent Control of Oxygenation (PRICO) on the Fabian ventilator in maintaining oxygen saturation (SpO2) in preterm infants on high flow nasal cannula (HFNC) support.DesignSingle-centre randomised two-period crossover study.SettingTertiary neonatal intensive care unit.Patients27 preterm infants (gestational age (GA) 0.25.InterventionA 24-hour period on automated FiO2-control with PRICO compared with a 24-hour period on routine manual control (RMC) to maintain a SpO2 level within target range of 88%–95% measured at 30 s intervals.Main outcome measuresPrimary outcome: time spent within target range (88%–95%). Secondary outcomes: time spent above and below target range, in severe hypoxia (SpO2 98%), mean SpO2 and FiO2 and manual FiO2 adjustments.Results15 patients received PRICO-RMC and 12 RMC-PRICO. The mean time within the target range increased with PRICO: 10.8% (95% CI 7.6 to 13.9). There was a decrease in time below target range: 7.6% (95% CI 4.2 to 11.0), above target range: 3.1% (95% CI 2.9 to 6.2) and in severe hypoxia: 0.9% (95% CI 1.5 to 0.2). We found no difference in time spent in severe hyperoxia. Mean FiO2 was higher during PRICO: 0.019 (95% CI 0.006 to 0.030). With PRICO there was a reduction of manual adjustments: 9/24 hours (95% CI 6 to 12).ConclusionIn preterm infants on HFNC support, automated FiO2-control by PRICO is superior to RMC in maintaining SpO2 within target range. Further validation studies with a higher sample frequency and different ventilation modes are needed.

Central sleep apnea (CSA) is universal during ascent to high altitude, with intermittent and transient fluctuations in oxygen saturation, but the consequences on mean sleeping blood oxygenation are unclear. We assessed indices of CSA and mean sleeping peripheral oxygen saturation ([Formula: see text]) during ascent to high altitude using two ascent profiles: rapid ascent and residence at 3,800 m and incremental ascent to 5,160 m. The severity of CSA was not correlated with mean sleeping [Formula: see text] with ascent.

BackgroundIncreasing evidence recognizes the harm of excess oxygen to lungs, eyes, and brain of preterm infants, but not yet to the intestine. We assessed changes in splanchnic oxygenation during reoxygenation with 21% compared to 100% O-2 in a newborn piglet model of perinatal asphyxia.MethodsWe randomized 25 piglets to control or intervention. Intervention groups underwent global hypoxia until acidosis and hypotension occurred. Piglets were reoxygenated for 30 min with 21% or 100% O-2 and observed for 9 h. We continuously measured regional splanchnic oxygen saturation (r(s)SO(2)) using near-infrared spectroscopy (NIRS). We calculated mean r(s)SO(2) and r(s)CoVar (as SD/mean). We measured PaO2 and SaO(2), sampled from the right carotid artery. ResultsReoxygenation after global hypoxia restored r(s)SO(2). Reoxygenation with 100% O-2 increased r(s)SO(2) to values significantly higher than baseline. In intervention groups, r(s)CoVar decreased during observation compared to baseline. We found a correlation between r(s)SO(2) and PaO2 (r = 0.420, P < 0.001) and between r(s)SO(2) and SaO(2) (r = 0.648, P < 0.001) in pooled data from the entire experiment.ConclusionReoxygenation after global hypoxia improves splanchnic oxygenation, but is associated with reduced variability of r(s)SO(2). Reoxygenation with 100% O-2 exposes the intestine to hyperoxia. Splanchnic NIRS is able to detect intestinal hypoxia and hyperoxia.ImpactSplanchnic oxygenation improves during reoxygenation after global hypoxia, though reoxygenation with 100% O2 exposes the intestine to hyperoxia. Decreased variability of splanchnic oxygenation several hours after hypoxia and reoxygenation seems to be independent of the resuscitation strategy, and may indicate intestinal injury. Splanchnic NIRS monitoring was able to detect intestinal hypoxia and exposure to hyperoxia, as evidenced by a strong correlation between splanchnic oxygenation and arterial oxygen content.

– Patients with Covid-19 have several symptoms, one of which is a decrease in the oxygen content in the blood or oxygen saturation (SpO2). Detecting oxygen saturation in preventing the spread of Covid-19 is one of the first steps. In this study, researchers conducted an experiment to detect oxygen levels in the blood or oxygen saturation on the POLECTOR C-19 device that was made. By using a photoplethysmography (PPG) which is carried out non-invasively using the sensor, the MAX30102 aim is to get the oxygen saturation value which is used as a reactive and non-reactive decision maker. With the method, the moving average data used from the sensor readings in the form of analog data will be changed using the formula Ratio of Ratios by calculating the average oxygen level then the results can be used to calculate oxygen levels in the blood. The results of sensor readings on the POLECTOR C-19 tool with a digital oximeter from an average value of 10 samples have a difference in value of 0.4, the reading results are the same as the readings of existing medical devices (digital oximeter) so that this tool can be used as a reader. oxygen saturation parameters. In conclusion, this tool can detect a person's condition by predicting the risk or no risk of Covid-19 from the results of the sensor readings of oxygen levels in the blood.

Context Animal husbandry in thermally stressful environments can be both complex and difficult, and the anticipated effects of climate change are expected to increase this condition. Aim Thermoregulatory responses, and acid–base and electrolytic balance were evaluated in indigenous ewes of different coat colour during different moments of natural heat stress. Methods Fifteen ewes of different colour (white, red and black; five of each) were used and data were collected for six consecutive weeks in the dry and rainy periods at three times of the day [(Moment 1) 0500 hours to 0600 hours; (Moment 2) 1200 hours to 1300 hours; and (Moment 3) 1700 hours to 1800 hours)]. Thermoregulatory (clinical measures), acid–base and electrolytic variables (blood examinations) were recorded. Air temperature and relative humidity were measured, and the radiant heat load was calculated. Key results Black ewes showed higher averages of respiratory rate (RR) and rectal temperature (RT). RT was different between before and after heat stress for all ewes, while the RR was not affected and cutaneous evaporation presented minor variations. Between 1200 hours and 1300 hours, there was an increase in the hydrogen potential, oxygen pressure, oxygen saturation and total carbon concentration averages, and a decrease in partial pressure of carbon dioxide for all ewes. The base excess was not significantly different for each time of the day between the periods, and were not affected by the time of the day or the season. White ewes were highly affected by the time of the day and the period regarding electrolytic parameters, followed by the red ewes and black ewes. The first two discriminant functions showed that ewes, regardless of coat colour, show similarity in the animal responses at Moments 1 and 3, compared with Moment 2. RT was the main variable that distinguished the three genetic groups. Conclusions Indigenous ewes of different coat colour recover physiological equilibrium after heat challenges. These adjustments enable them to maintain thermoregulatory responses, and acid–base and electrolytic balance within the physiological equilibrium. Implications These variations indicated the ability of Brazilian indigenous ewes of different coat colour to recover thermoregulatory, and acid–base and electrolytic balance after an environmentally challenging condition in an equatorial semi-arid region, regardless of the period.

BACKGROUND Oxygen therapy is required to prevent hypoxaemia during the endobronchial ultrasonography (EBUS) procedure. AIMS To compare the effectiveness of oxygen therapy delivered through high-flow nasal cannula (HFNC) and conventional nasal cannula (CNC) in patients undergoing EBUS. The primary outcome was the proportion of patients who desaturated. Patient compliance and satisfaction were also evaluated. METHODS This single-centre prospective interventional study was conducted in a tertiary hospital among patients who presented to the EBUS unit in 2018 and 2019. Objectives: The coronavirus disease 2019 (COVID-19) pandemic is the current public health concern. Hypoxemia has been identified as an independent risk factor for mortality in COVID-19 patients regardless of age or sex. This study therefore aimed to assess the profile of COVID-19 patients with hypoxemia in Lagos, Nigeria and identify their associated socio-demographic and clinical risk factors, predictors, and outcomes. Materials and Methods: This was a retrospective cohort study in which data were extracted from medical records of real-time polymerase chain reaction confirmed COVID-19 positive patients admitted between April and October 2020. Data extracted included age, sex, comorbidities, disease category/classification, symptoms, lowest oxygen saturation (SPO2), and outcomes. Bivariate analysis was done to test associations between hypoxemia and other variables. Multivariate analysis was done to determine significant predictors of hypoxemia. Results: A total of 266 patients were included in the study; mean (SD) 49.80 (± 16.68) years. Hypoxemia (lowest SPO2 ≤ 90 in adults and < 92% in children) was found in 102 (38.3 %) of the cases. SPO2 of hypoxemic patients ranged from 33% to 90%, Mean ±SD of 77±13%. About half of the hypoxemic cases, 53 (52%) were ≥ 60 years and mostly male 70 (68.6%). Difficulty breathing was present in 56 (55%), while the common comorbidities were hypertension 86 (32.3%) and diabetes mellitus 47 (17.7%). Age ≥ 60, difficulty breathing, and fever were independent predictors of hypoxemia. Hypoxemia was significantly associated with death (X2-42.13; P < 0.001); odds ratio 14.5 (95% CI: 5.4–38.8). Conclusion: Hypoxemia occurred in 1 out of every 3 COVID-19 patients with poor prognosis. SPO2 monitoring and early presentation in hospital for those 60 years and above or with dyspnea may be essential for early identification and treatment of hypoxemia to reduce mortality.

Introduction: Coronavirus disease 2019 (COVID-19) - a global pandemic has severely affected humankind. It basically affects both the upper (i.e. sinuses, nose and throat) and lower (i.e. trachea and lungs) airways hence causing respiratory tract diseases ranging from asymptomatic or cold to more severe lung diseases .Since it affects the respiratory system, so Dyspnea and fatigue are one of the most prominent symptoms for COVID-19. The aim of the study is to determine the effect of positioning exercises in COVID-19 patients which can contribute to relieve dyspnea and reduce fatigue Methodology: 80 Patients, age range 18 to 50 years, diagnosed with Covid 19 and undergoing treatment for the same with grade ≤ 3 NYHA dyspnea classifications were included and divided equally in two groups-Groups I (experimental) and Group II (Control). In Group I positioning exercises were administered 30 minutes twice a day for 7 days along with the regular Covid treatment protocol and in Group II followed regular Covid treatment protocol. Pre and post evaluation of oxygen saturation and respiratory rate with fatigue severity scale were taken Results: As the data was not normally distributed non parametric test was used. Wilcoxon test was used to compare the outcome measures within groups and Man-Whitney test was used for comparing between groups. Result shows that there was significant improvement noted in both the groups within and between comparison but Group I was found to be highly significant then Group II. Conclusion: Positioning exercises in COVID-19 patients helps to relieve dyspnea and reduce fatigue. Pulse oximetry screening (POS) has been shown to be an effective, non-invasive investigation that can detect up to 50–70% of previously undiagnosed congenital heart defects (CHDs). The aims of this study were to assess the accuracy of POS in detection of CHDs and its impact on clinical practice. All eligible newborn infants born between 1 Jan 2015 and 31 Dec 2019 in a busy regional neonatal unit were included in this prospective observational study. A positive POS was classified as two separate measurements of oxygen saturation 2% between pre- and post-ductal circulations. Overall, 23,614 infants had documented POS results. One hundred eighty nine (0.8%) infants had a true positive result: 6 had critical CHDs, 9 serious or significant CHDs, and a further 156/189 (83%) infants had significant non-cardiac conditions. Forty-three infants who had a normal POS were later diagnosed with the following categories of CHDs post-hospital discharge: 1 critical, 15 serious, 20 significant and 7 non-significant CHDs. POS sensitivity for detection of critical CHD was 85.7%, whereas sensitivity was only 33% for detection of major CHDs (critical and serious) needing surgery during infancy; specificity was 99.3%.Conclusion: Pulse oximetry screening showed moderate to high sensitivity in detection of undiagnosed critical CHDs; however, it failed to detect two-third of major CHDs. Our study further emphasises the significance of adopting routine POS to detect critical CHDs in the clinical practice. However, it also highlights the need to develop new, innovative methods, such as perfusion index, to detect other major CHDs missed by current screening tools. What is Known:• Pulse oximetry screening is cost effective, acceptable, easy to perform and has moderate sensitivity and high specificity in detection of critical congenital heart defects.• Pulse oximetry screening has been implemented many countries including USA for detection of critical congenital heart defects, but it is not currently recommended by the UK National Screening Committee. What is New:• To our knowledge, this is the first study describing postnatal detection and presentation of all the infants with congenital heart defects over a period of 5 years, including those not detected on the pulse oximetry screening, on the clinical practice. • It emphasises that further research required to detect critical congenital heart defects and other major CHDs which can be missed on the screening tools currently employed in clinical practice.

Objective Because of the abundance of complications associated with peri-intubation hypoxia, maintaining adequate oxygen saturation during endotracheal intubation (ETI) is of great concern. In addition to standard preoxygenation techniques, apneic oxygenation (AO), the continuous flow of passive oxygenation, is a potential tool that can be used to eliminate hypoxia during ETI. Although scarcely studied in the pediatric population, AO has proven effective in reducing the incidence of hypoxia in adult patients with minimal side effects. The objective of this study is to evaluate the use of apneic oxygenation in pediatric patients and to determine its efficacy in preventing or delaying oxygen desaturation during the apneic period of ETI. Methods This literature review examines 4 studies that evaluate the practice of AO in pediatric patients. A total of 712 patients across 3 randomized control trials and 1 observational study were assigned to either a control group that did not receive any form of AO, a group that did not receive 100% fraction of inspired oxygen (FiO2), or an intervention group where various methods of AO were delivered. Results Each AO method that provided 100% FiO2 saw a significantly longer time until initial desaturation when compared with those that did not receive any form of AO or those not receiving 100% FiO2. Conclusions The findings in this study confirm that the practice of AO is not only efficacious in increasing the time until initial desaturation but also reduces the overall incidence of hypoxia during laryngoscopy in children.

Background: Chronic Obstructive Pulmonary Disease (COPD) is a narrowing of the air flow caused by inflammation in the airways resulting in the shortness of breath and reduced oxygen levels in the blood thereby affecting patients’ quality of life.Purpose: The purpose of this study was to determine the effect of exercise on the PLB to Scale of Breathlessness and oxygen saturation (SaO2) in COPD patients.Methods: The research method utilized quasi experimental design with randomized pretest-posttest control group. Sample of 68 respondents (60 respondents PLB group, and 8 respondents control group) was recruited through total sampling technique.Result: Independent t-test showed difference of decreasing Scale of Breathlessness (p=0.018) & increasing of SaO2 (p=0.023) between PLB & control group. The results of Multiple linear regression test showed an effect of PLB on a Scale of Breathlessness (p=0.030) and SaO2 (p=0.002).Conclusion: It can be concluded that the PLB exercises influenced changes in the value Scale of Breathlessness and SaO2.

Background: Analysis of autonomic modulation after postural change may inform the prognosis and guide treatment in different populations. However, this has been insufficiently explored among adolescents with Duchenne muscular dystrophy (DMD). Objective: To investigate autonomic modulation at rest and in response to an active sitting test (AST) among adolescents with DMD. Methods: Fifty-nine adolescents were included in the study and divided into two groups: 1) DMD group: adolescents diagnosed with DMD; 2) control group (CG): healthy adolescents. Participants’ weight and height were assessed. Lower limb function, motor limitations and functional abilities of the participants in the DMD group were classified using the Vignos scale, Egen classification and motor function measurement, respectively. The following variables were assessed before, during and after AST: systolic blood pressure (SBP), diastolic blood pressure (DBP), respiratory rate (f), oxygen saturation and heart rate (HR). To analyze the autonomic modulation, the HR was recorded beat-by-beat. Heart rate variability (HRV) indices were calculated in the time and frequency domains. Results: Differences in relation to groups were observed for all HRV indices, except LF/HF, oxygen saturation, HR and f (p < 0.05). Differences in relation to time and the interaction effect between group and time were observed for RMSSD, SD1, SD2, SD1/SD2, LFms2 and LFnu, HFun, SBP and DBP (p < 0.05). Differences in relation to time were also observed for the indice SDNN, FC and f (p < 0.05). Conclusions: Performing the AST promoted reduced autonomic modulation and increased SBP, DBP and HR in adolescents with DMD. Métodos: 59 adolescentes foram incluídos no estudo e divididos em dois grupos: 1) Grupo DMD: adolescentes com diagnóstico de DMD; 2) Grupo controle: adolescentes saudáveis. O peso e a altura dos participantes foram avaliados. No grupo DMD, a funcionalidade de membros superiores, limitações motoras, e habilidades funcionais foram classificadas pela escala de Vignos, Egen Klassification, e motor function measure respectivamente. Problem: COVID-19 is caused by the SARS-Cov-2 virus which causes the main symptoms of fever, cough, chest tightness and dyspnea. Until the end of July 2021, there have been 3.331.206 confirmed positive cases of COVID-19, and the number of patients who have died is 90.552 with an increase of 1.893 people dying in a day1. Patients with COVID-19 who are critically ill and need to be transferred to the ICU are patients who experience severe illness characterized by dyspnea accompanied by hypoxemia. This condition allows the patient to experience decreased oxygen saturation and multi-organ failure. Objective: This study was conducted to analyze the pronation position to increase oxygenation of COVID-19 patients with respiratory disorders in the ICU who were not intubated. Method: The method used is a literature review. Researchers got 122 journals according to the chosen topic and screened according to the eligibility criteria and got five journals. Source searches were carried out through Google Scholar, PubMed web, Science Direct web, Springer Link web, Biomedcentral web and Medrxiv web. Results: The research showed that the pronation position was able to improve oxygenation in COVID-19 patients with respiratory disorders who were not intubated. Conclusion: Patients with COVID-19 provide a spectrum of symptoms that vary from asymptomatic, mild symptoms, pneumonia, severe pneumonia, ARDS, to septic shock. The use of the prone position in moderate to severe respiratory disorders experienced by COVID-19 patients is considered safe enough to do.
