Background: Traumatic Brain Injury (TBI) is a frequent cause of morbidity and hospitalizations. Victims of TBI admitted to an intensive care unit (ICU) require care from a highly complex multiprofessional team with the aim of optimizing brain perfusion, avoiding injuries secondary to the initial trauma and favoring the rehabilitation process. In this context, physiotherapy has been increasingly requested to participate in neuroprotection strategies and management of severe TBI. Objective: To describe the early treatment of a severe TBI victim, highlighting the role of physiotherapy in neuroprotection strategies and in the adequate and safe management/treatment of this patient profile. Case report: Female patient, 27 years old, victim of being hit by a motorcycle at high speed. Rescued by the mobile emergency care service with a diagnosis of severe TBI and fracture of the left tibia. Admitted to the ICU in the immediate postoperative period of decompressive craniectomy, with excision of an acute subdural hematoma and insertion of a catheter for continuous monitoring of intracranial pressure (ICP), in addition to orthopedic surgery with linear external fixation of the left tibia. Conclusion: In the critical phase after TBI, ICP monitoring and early performance of neuroprotection measures are crucial. It is essential to maintain the hemodynamic, respiratory and metabolic stability of the patient, who is submitted to deep sedation and mechanical ventilation (MV), and physiotherapy has fundamental importance and can be performed safely, ensuring adequate neuroprotective ventilation strategies and preventing complications related to MV, in addition to promoting early functional stimulation of this patient. Background: Traumatic brain injury (TBI) is a frequent cause of hospitalization and use of intensive physiotherapy care. Despite this, little has been studied about the role of respiratory therapy on assisting this kind of patient admitted to an intensive care unit (ICU). Objective: To investigate the main studies related to respiratory therapy in assisting severe TBI patients. Methods: Aliterature review was conducted, by means of SciELO, Medline and PubMed databases, in the period from 2004 to 2014. The keywords "Brain injuries", "Craniocerebral trauma", "Trauma centers", "Physical therapy modalities" and "Respiratory therapy" were used to identify the scientific articles, published in Portuguese and English, which investigated the role of physiotherapy on assisting TBI patients in ICU. Results: Using the previously described search strategy were included five studies, organized and presented in a table format. Intensive physiotherapy is an essential part in the treatment / rehabilitation of patients with severe TBI. Besides having important role in maintaining airway and bronchial hygiene, thus preventing pulmonary complications, physiotherapy team also assists in the management of integrated mechanical ventilation with neuroprotective measures. Conclusion: Respiratory therapy maneuvers (manual vibrocompression and increased expiratory flow) can be applied safely in patients with severe TBI. However, endotracheal aspiration leads to a transient increase in intracranial pressure and, therefore, some caution should be taken. Lung complications during postoperative period of cardiac surgery are frequently, highlighting atelectasis and hypoxemia. Alveolar recruitment maneuvers have an important role in the prevention and treatment of these complications. Thus, this study reviewed and updated the alveolar recruitment maneuvers performance in the immediate postoperative period of cardiac surgery. We noted the efficacy of alveolar recruitment through different specific techniques and the need for development of new studies.
