Abstract

Increasing efferent renal sympathetic nerve activity (ERSNA) increases afferent renal nerve activity (ARNA). To test whether the ERSNA-induced increases in ARNA involved norepinephrine activating [alpha]-adrenoceptors on the renal sensory nerves, we examined the effects of renal pelvic administration of the [[alpha].sub.1]- and [[alpha].sub.2]-adrenoceptor antagonists prazosin and rauwolscine on the ARNA responses to reflex increases in ERSNA (placing the rat's tail in 49[degrees]C water) and renal pelvic perfusion with norepinephrine in anesthetized rats. Hot tail increased ERSNA and ARNA, 6,930 [+ or -] 900 and 4,870 [+ or -] 670%'s (area under the curve ARNA vs. time). Renal pelvic perfusion with norepinephrine increased ARNA 1,870 [+ or -] 210%'s. Immunohistochemical studies showed that the sympathetic and sensory nerves were closely related in the pelvic wall. Renal pelvic perfusion with prazosin blocked and rauwolscine enhanced the ARNA responses to reflex increases in ERSNA and norepinephrine. Studies in a denervated renal pelvic wall preparation showed that norepinephrine increased substance P release, from 8 [+ or -] 1 to 16 [+ or -] 1 pg/min, and [PGE.sub.2] release, from 77 [+ or -] 11 to 161 [+ or -] 23 pg/min, suggesting a role for [PGE.sub.2] in the norepinephrine-induced activation of renal sensory nerves. Prazosin and indomethacin reduced and rauwolscine enhanced the norepinephrine-induced increases in substance P and [PGE.sub.2]. [PGE.sub.2] enhanced the norepinephrine-induced activation of renal sensory nerves by stimulation of EP4 receptors. Interaction between ERSNA and ARNA is modulated by norepinephrine, which increases and decreases the activation of the renal sensory nerves by stimulating [[alpha].sub.1]- and [[alpha].sub.2]-adrenoceptors, respectively, on the renal pelvic sensory nerve fibers. Abstract

A method to accurately measure the position and orientation of an acetabular cup implant from postoperative X-rays has been designed and validated. The method uses 2-D-3-D registration to align both the prosthesis and the preoperative computed tomography (CT) volume to the X-ray image. This allows the position of the implant to be calculated with respect to a CT-based surgical plan. Experiments have been carried out using ten sets of patient data. A conventional plain-film measurement technique was also investigated. A gold standard implant position and orientation was calculated using postoperative CT. Results show our method to be significantly more accurate than the plain-film method for calculating cup anteversion. Cup orientation and position could be measured to within a mean absolute error of 1.4 mm or degrees. Index Terms--Cup anteversion, implant position, 2-D-3-D registration.

13. Reducing artifacts in pelvic bone SPECT: an assessment of lesion detectability using numerical and human observers

Abstract

In pelvic bone SPECT using Tc-99m labeled compounds, physical effects such as nonhomogeneous attenuation and the accumulation of activity into the bladder during the data acquisition process can often result in data inconsistencies. With filtered backprojection (FBP) reconstruction, this may result in streak artifacts. Various methods for attenuation compensation can be employed to account for some of these streaks, but if the rate of uptake of tracer in to the bladder is sufficient, the resultant streaks may be significant enough to impair lesion detection. We have investigated various reconstruction methods in an effort to reduce these artifacts. In order to assess the impact of inconsistent bladder activities on the detection of pelvic lesions, SPECT imaging was simulated using the Zubal voxelized phantom, for cases of both a static bladder activity and a changing activity distribution. Reconstructions were performed using FBP, ordered subset-expectation maximization (OSEM) and dynamic expectation maximization (dEM) and was assessed for lesion detectability using a channelized, non-prewhitening (CNPW) numerical observer model. This observer model was used to optimize reconstruction strategies for a human LROC observer study. The human LROC observer study was performed in order to assess the various reconstruction methods in terms of lesion detectability. Three human observers were used in this test. The results of this test indicate that FBP performs significantly worse than static OSEM iterative reconstruction with attenuation correction when assessed using the area under the LROC curve (maximal [A.sub.LROC] = 0.47 for FBP versus 0.71 for OSEM). Visually, the dEM algorithm produces images with slightly reduced streak artifacts compared to OSEM, but this improvement was not reflected in significantly improved [A.sub.LROC] values. In fact, when assessed using human LROC methodology, detectability actually decreased slightly when using dEM (maximal [A.sub.LROC] = 0.71 for OSEM versus 0.66 for dEM), although this reduction was not determined to be statistically significant. It is possible that the slightly reduced performance of the dEM algorithm may be due, in part, to not performing an optimization in the number of reconstruction iterations as was performed for the OSEM method. Pregnancy-related back pain and pelvic girdle pain (PGP) are common across countries, irrespective of socioeconomic factors. (1) In general, the literature does not make a clear distinction between pregnancy-related low [...], Background. No appropriate measures have been specifically developed for pelvic girdle pain (PGP). There is a need for suitable outcome measures that are reliable and valid for people with PGP for use in research and clinical practice. Objective. The objective of this study was to develop a condition-specific measure, the Pelvic Girdle Questionnaire (PGQ), for use during pregnancy and postpartum. Design. This was a methodology study. Methods. Items were developed from a literature review and information from a focus group of people who consulted physical therapists for PGP. Face validity and content validity were assessed by classifying the items according to the World Health Organization's International Classification of Functioning, Disability and Health. After a pilot study, the PGQ was administered to participants with clinically verified PGP by means of a postal questionnaire in 2 surveys. The first survey included 94 participants (52 pregnant), and the second survey included 87 participants (43 pregnant). Rasch analysis was used for item reduction, and the PGQ was assessed for unidimensionality, item fit, redundancy, and differential item functioning. Test-retest reliability was assessed with a random sample of 42 participants. Results. The analysis resulted in a questionnaire consisting of 20 activity items and 5 symptom items on a 4-point response scale. The items in both subscales showed a good fit to the Rasch model, with acceptable internal consistency, satisfactory fit residuals, and no disordered threshold. Test-retest reliability showed high intraclass correlation coefficient estimates: .93 (95% confidence interval=0.86-0.96) for the PGQ activity subscale and .91 (95% confidence interval=0.84-0.95) for the PGQ symptom subscale. Limitations. The PGQ should be compared with low back pain questionnaires as part of a concurrent evaluation of measurement properties, including validity and responsiveness to change. Conclusions. The PGQ is the first condition-specific measure developed for people with PGP. The PGQ had acceptably high reliability and validity in people with PGP both during pregnancy and postpartum, it is simple to administer, and it is feasible for use in clinical practice.

Background: Nucleic acid amplification tests (NAATs) allow chlamydia screening in asymptomatic women who otherwise may not have pelvic examinations. How often these women have examination findings that may prompt empiric therapy is unclear. Objective: The objective of this study was to assess the relationship of chlamydia with pelvic examination findings in asymptomatic women screened by NAAT. Methods: The authors conducted a retrospective analysis of clinical data from 577 young asymptomatic women screened by chlamydial transcription-mediated amplification (TMA). Results: TMA was positive in 68 (11.8%). The most common examination finding was vaginal discharge (5.9%) followed by cervical ectopy (3.6%), endocervical mucopus (2.3%) or easily induced bleeding (2.3%), and cervical motion (1.4%) or adnexal (0.7%) tenderness. On multivariate analysis, only easily induced bleeding or mucopus predicted chlamydia (adjusted odds ratio [AOR] = 4.7; P = 0.010 and AOR = 4.4; P = 0.015, respectively). Conclusions: Abnormal pelvic examination findings were infrequent in asymptomatic young women screened by a chlamydial NAAT. However, endocervical bleeding or mucopus, when present, predicted chlamydia.

This paper, based on an analysis of cancer articles published in popular periodical literature since the early part of the century, argues that gender has played a key role in medical and popular understandings of cancer. Cancer education, the author finds, has taught women and men different things. Public health materials created with the intention of improving health through education actually send a multiplicity of messages, not all of them helpful. This essay suggests that public health messages targeted by sex are problematic. although perhaps necessary. A study to evaluate the stability of the maternal pelvis over the course of the third trimester and the puerperium is illustrated. Results reveal that serial magnetic resonance-based pelvimetry showed relative stability of pelvic measurements through the course of pregnancy and delivery but if comparative pelvimetry is to be useful as an antepartum predictor of labor success, then it might be possible to obtain reliable pelvimetry in those patients anytime after delivery.

The distribution of pelvic organ support in a gynecologic clinic population for defining the clinical disease state of pelvic organ prolapse is described and its epidemiologic condition is analyzed. Routine gynecological care is important for the maintenance of good health and integral to early detection and treatment of sexually transmitted diseases and cancer. Research indicates that both patients and clinicians find this examination anxiety provoking and, therefore, may fail to comply with optimal guidelines for routine care. Training techniques, effective for attenuating providers' and patients' anxiety, are reviewed. These include medical school programs for provides and seminars on women's health designed for lay people. In addition to providing training when there are fewer immediate stressors, methods for reducing anxiety may be employed proximate to exams. Providing informative, behavioral, and cognitive-behavioral techniques, emphasizing greater patient control, may be useful in decreasing anxiety associated with pelvic examinations. These latter techniques may be incorporated into providers' protocol for routine pelvic examinations and thereby may serve to enhance compliance for routine gynecological care.

Obese women are at higher risk for the development of both endometrial and ovarian carcinoma. Biochemical mechanisms represent documented causal factors but the role of psycho-social attitudes has received limited attention. This study examined the difference in the frequency of pelvic screening examinations between obese and non-obese women and the effect of physician and patient attitudes toward obesity on examination frequency. A total of 291 women subjects and 1316 physician subjects participated in this study. Data reported in this paper suggest that attitudes and behaviors are negatively influenced by weight. As subjects' weight went up, negative opinions about their appearance and reluctance to obtain pelvic examinations also increased while the likelihood of having annual pelvic examinations decreased. A substantial minority (17%) and an overwhelming majority (83%) of physicians indicated they were reluctant to perform pelvic examinations on obese and reluctant patients respectively. If physicians are more reluctant to perform pelvic examinations on obese and reluctant women and obese women are more reluctant to be examined, there may be a critical delay in detecting adenocarcinomas of the female genital tract.

A previous study of the effect of a new examination gown on patients' experienced discomfort during pelvic examination demonstrated that the gown was effective. This study replicated the previous study with a younger group of subjects. It was hypothesized that the new gown would reduce reported distress. Subjects were 147 patients at a university student health center. Age ranged from 18 to 31. Informed consent was obtained and patients were randomly assigned to either the experimental gown or the standard drape condition. Following examination, subjects completed questionnaires assessing demographic characteristics, state and trait anxiety, desired changes in pelvic examination procedures, and reactions to the examination. The attending nurse recorded blood pressure. Results supported the hypotheses. Experimental subjects rated the gown as more comfortable than control subjects rated the drape. Results indicate that this simple stimulus control intervention reduces one source of distress associated with pelvic examinations.

The attending nurse recorded heart rate and blood pressure. Results supported the hypotheses. Experimental subjects rated the gown as more comfortable than control subjects rated the drape. Experimental subjects desired fewer changes in exam procedures than control subjects, indicating that the gown provided them with an overall more comfortable experience.
