World wide, the number of CT studies in children and the radiation exposure by CT increases. The same energy dose has a greater biological impact in children than in adults, and scan parameters have to be adapted to the smaller diameter of the juvenile body. Based on seven rules, a practical approach to paediatric CT is shown: Justification and patient preparation are important steps before scanning, and they differ from the preparation of adult patients. The subsequent choice of scan parameters aims at obtaining the minimal signal-to-noise ratio and volume coverage needed in a specific medical situation; exposure can be divided in two aspects: the CT dose index determining energy deposition per rotation and the dose-length product (DLP) determining the volume dose. DLP closely parallels the effective dose, the best parameter of the biological impact. Modern scanners offer dose modulation to locally minimise exposure while maintaining image quality. Beyond the selection of the physical parameters, the dose can be kept low by scanning the minimal length of the body and by avoiding any non-qualified repeated scanning of parts of the body. Following these rules, paediatric CT examinations of good quality can be obtained at a reasonable cost of radiation exposure.

Zusammenfassung: Die rasante technische Weiterentwicklung der CT hat in den letzten Jahren zu einer deutlichen Zunahme der diagnostischen Möglichkeiten geführt, mit dem Resultat, dass die CT-Untersuchungszahlen weltweit angestiegen sind. Dies hat ebenfalls Auswirkung auf die Strahlenexposition der Bevölkerung. Bis heute sind zahlreiche Publikationen erschienen, die gezeigt haben, dass eine Dosisreduktion erreicht werden kann, ohne dadurch die Bildqualität und Sensitivität der CT zu beeinträchtigen. Die Mehrzahl der Strategien zur Dosisoptimierung sind einfach anzuwenden und unabhängig von der Detektorkonfiguration des CT-Scanners. Im vorliegenden Übersichtsartikel werden die wichtigsten Methoden vorgestellt: indikationsabhängige Methoden (z.B. rechtfertigende Indikation, Reduktion der Röhrenspannung für die CT-Angiographie, Wahl von Kollimation und Pitchfaktor, Minimierung der Untersuchungsphasen, Senkung der Röhrenspannung und des -stroms für die Nativphase), herstellerabhängige Methoden (z.B. automatische Röhrenstrommodulation, adaptive Filter zur Reduktion des Bildrauschens, iterative Bildrekonstruktion) und allgemeine Methoden (z.B. Patientenzentrierung im Isozentrum der CT-Gantry, Reduktion der Scanlänge, Anwendung von Röntgenschutzmitteln, Reduktion der Röhrenspannung und/oder des -stroms für den CT-Planungsscan)

OBJECTIVE Imaging registries afford opportunities to study large, heterogeneous populations. The purpose of this study was to examine the American College of Radiology CT Dose Index Registry (DIR) for dose-related demographics and metrics of common pediatric body CT examinations. MATERIALS AND METHODS Single-phase CT examinations of the abdomen and pelvis and chest submitted to the DIR over a 5-year period (July 2011-June 2016) were evaluated (head CT frequency was also collected). CT examinations were stratified into five age groups, and examination frequency was determined across age and sex. Standard dose indexes (volume CT dose index, dose-length product, and size-specific dose estimate) were categorized by body part and age. Contributions to the DIR were also categorized by region and practice type. RESULTS Over the study period 411,655 single-phase pediatric examinations of the abdomen and pelvis, chest, and head, constituting 5.7% of the total (adult and pediatric) examinations, were submitted to the DIR. Head CT was the most common examination across all age groups. The majority of all scan types were performed for patients in the second decade of life. Dose increased for all scan types as age increased; the dose for abdominopelvic CT was the highest in each age group. Even though the DIR was queried for single-phase examinations only, as many as 32.4% of studies contained multiple irradiation events. When these additional scans were included, the volume CT dose index for each scan type increased. Among the studies in the DIR, 99.8% came from institutions within the United States. Community practices and those that specialize in pediatrics were nearly equally represented. CONCLUSION The DIR provides valuable information about practice patterns and dose trends for pediatric CT and may assist in establishing diagnostic reference levels in the pediatric population.

SUMMARY We describe a 33-yr-old man suffering from severe vertebral osteoporosis and urticaria pigmentosa due to systemic mast cell disease (SMCD). Because i.v. clodronate therapy could not prevent further vertebral fractures, an additional treatment with interferon alpha-2b was initiated. During 24 months of treatment, our patient had no further pain episodes, no new vertebral fractures were discovered, trabecular bone mineral density (BMD) increased significantly and urticarial symptoms improved. Nevertheless, the extent of skin lesions remained unchanged. On histological examination, a remarkable decrease of mast cells was observed in the bone marrow, but not in the skin. Five months after discontinuation of interferon alpha-2b, trabecular BMD decreased and urticarial symptoms deteriorated. These findings illustrate a beneficial effect of interferon alpha-2b on SMCD-induced osteoporosis as well as urticarial symptoms, and raise the question whether this treatment may have a diverse impact on mast cell populations in different tissues.

The ACR, the European Society of Radiology, and the International Society of Radiology held the first joint Global Summit on Radiological Quality and Safety in May 2013. The program was divided into 3 day-long themes: appropriateness of imaging, radiation protection/infrastructure, and quality and safety. Participants came from global organizations, including the International Atomic Energy Agency, the World Health Organization, and other institutions; industry and patient advocacy groups with an interest in imaging were also represented. The goal was to exchange ideas and solutions and share concerns to arrive at a better and more uniform approach to quality and safety. Participants were asked to use the information presented to develop strategies and tactics to harmonize and promote best practices worldwide. These strategies were summarized at the conclusion of the meeting.

Background A precise detection of volume change allows for better estimating the biological behavior of the lung nodules. Postprocessing tools with automated detection, segmentation, and volumetric analysis of lung nodules may expedite radiological processes and give additional confidence to the radiologists. Purpose To compare two different postprocessing software algorithms (LMS Lung, Median Technologies; LungCARE®, Siemens) in CT volumetric measurement and to analyze the effect of soft (B30) and hard reconstruction filter (B70) on automated volume measurement. Material and Methods Between January 2010 and April 2010, 45 patients with a total of 113 pulmonary nodules were included. The CT exam was performed on a 64-row multidetector CT scanner (Somatom Sensation, Siemens, Erlangen, Germany) with the following parameters: collimation, 24x1.2 mm; pitch, 1.15; voltage, 120 kVp; reference tube current-time, 100 mAs. Automated volumetric measurement of each lung nodule was performed with the two different postprocessing algorithms based on two reconstruction filters (B30 and B70). The average relative volume measurement difference (VME%) and the limits of agreement between two methods were used for comparison. Results At soft reconstruction filters the LMS system produced mean nodule volumes that were 34.1% ( P 0.05). Conclusion There is a substantial inter-software (LMS/LungCARE®) as well as intra-software variability (B30/B70) in lung nodule volume measurement; therefore, it is mandatory to use the same equipment with the same reconstruction filter for the follow-up of lung nodule volume.

To assess the ability of low-dose CT to detect and characterize the most common CT patterns of pulmonary disease.Sixty patients with nodules, consolidations or interstitial disease were scanned using a low-dose (128 mm × 0.6 mm, 40 reference mAs, 120 kVp) and standard-dose CT protocol (150 reference mAs, 120 kVp). Two radiologists with 3 and 10 years of thoracic imaging experience searched both exams in consensus for the most commonly observed CT patterns according to the Fleischner Society criteria, which consisted of 46 different subgroups of ground-glass opacities, nodules, interstitial and airspace diseases. The standard of reference was established by consensus of a panel of two experienced chest radiologists (9 and 12 years of experience).The lung segments (1080) showed 813 nodules, 596 ground-glass opacities, 74 airspace and 575 interstitial diseases and 64 normal segments. In particular, air-space disease and nodules were unaffected by the increase in noise. However, the sensitivity to detect ground-glass opacities, ground-glass nodules and interstitial opacities decreased significantly, from 89% to 77%, 86% to 68% and 91% to 71%, respectively (all p-values0.00001). Using iterative reconstruction instead of the applied filtered back projection sensitivity for ground-glass nodules rose to the sensitivity of standard-dose CT in an additional phantom study.A low-dose CT of 40 mAs/120 kVp is feasible for detecting solid nodules, airspace, airways and pleural disease. For diagnosing pathologies consisting of ground-glass opacities or interstitial opacities, higher tube current or iterative reconstruction is required.

Imaging during pregnancy has increased in frequency. Radiation protection is extremely important although tissue reactions of the conceptus, requiring a threshold dose of around 100mGy, are unlikely in the diagnostic use of X-rays and stochastic effects of cancerogenesis have a rather low risk (around 10-4/mGy for childhood cancer due to in utero exposure). This article will review the risk depending on dose and phase of pregnancy and the exposure by frequent examinations; it will then concentrate on the duties of an imaging department: screening for pregnancy, examination justification, planning and optimization, patient information, counseling, involving the patient in the decisions, and managing the situation of pregnant staff members. Typical flowcharts of investigating frequent clinical questions will finally be presented and critically discussed.

Purpose: Converting the measurable quantities to patient organ doses in projection radiography is usually based on a standard-sized patient model and a specific radiation quality, which are likely to differ from the real situation. Large inaccuracies can therefore be obtained in organ doses, because organ doses are dependent on the exposure parameters, exposure geometry and patient anatomy. In this study, the effect of radiation quality and patient thickness on the organ dose conversion factors were determined. Methods: In this study, the posterior-anterior projection radiograph of the thorax was selected in order to determine the effect of radiation quality (tube voltages of 70-130 kV and total filtrations of 3 mmAl to 4 mmAl + 0.2 mmCu) and patient thickness (anterior-posterior thicknesses of 19.4-30.8 cm) on the breast and lung dose conversion factors. For this purpose, Monte Carlo simulation programs ImpactMC and PCXMC were used with computed tomography examination data of adult male and female patients and mathematical hermaphrodite phantoms, respectively. Results: Compared to the reference beam quality and patient thickness, the relative variation range in organ dose conversion factors was up to 74% for different radiation qualities and 122% for different patient thicknesses. Conclusions: Conversion factors should only be used with comprehensive understanding of the exposure conditions, considering the exposure parameters, exposure geometry and patient anatomy they are valid for. This study demonstrates that patient thickness-specific and radiation quality-specific conversion factors are needed in projection radiography. (C) 2016 Associazione Italiana di Fisica Medica. To assess the effect of radiation dose reduction on the appearance and visual quantification of specific CT patterns of fungal infection in immuno-compromised patients.Raw data of thoracic CT scans (64 × 0.75 mm, 120 kVp, 300 reference mAs) from 41 consecutive patients with clinical suspicion of pulmonary fungal infection were collected. In 32 patients fungal infection could be proven (median age of 55.5 years, range 35-83). A total of 267 cuboids showing CT patterns of fungal infection and 27 cubes having no disease were reconstructed at the original and 6 simulated tube currents of 100, 40, 30, 20, 10, and 5 reference mAs. Eight specific fungal CT patterns were analyzed by three radiologists: 76 ground glass opacities, 42 ground glass nodules, 51 mixed, part solid, part ground glass nodules, 36 solid nodules, 5 lobulated nodules, 6 spiculated nodules, 14 cavitary nodules, and 37 foci of air-space disease. The standard of reference was a consensus subjective interpretation by experts whom were not readers in the study.The mean sensitivity and standard deviation for detecting pathological cuboids/disease using standard dose CT was 0.91 ± 0.07. Decreasing dose did not affect sensitivity significantly until the lowest dose level of 5 mAs (0.87 ± 0.10, p=0.012). Nodular pattern discrimination was impaired below the dose level of 30 reference mAs: specificity for fungal 'mixed nodules' decreased significantly at 20, 10 and 5 reference mAs (p0.05). At lower dose levels, classification drifted from 'solid' to 'mixed nodule', although no lesion was missed.Our simulation data suggest that tube current levels can be reduced from 300 to 30 reference mAs without impairing the diagnostic information of specific CT patterns of pulmonary fungal infections.

To assess the effect of tumor size and tumor-to-liver contrast of simulated hypovascular liver tumors on the diagnostic accuracy of hepatic computed tomography (CT).This retrospective study was approved by the institutional review board, and informed consent was waived. A total of 153 simulated hypovascular liver tumors were embedded in 70 hepatic CT data sets that were acquired during the portal venous phase. The simulated tumors had 3 different diameters (6, 10, and 14 mm) and 3 different tumor-to-liver contrast values (20, 35, and 50 HU). There were also 30 hepatic CT data sets without liver tumors. Three radiologists independently performed tumor detection on the randomized 100 hepatic CT data sets.The lowest sensitivity was obtained for the 6-mm tumors with a tumor-to-liver contrast of 20 HU (4.1%), and the highest sensitivity was obtained for the 10- and 14-mm tumors with a tumor-to-liver contrast of 50 HU (100%). Increasing the contrast from 20 to 35 to 50 HU in the 6-mm tumors yielded a significant increase in sensitivity (4.1%, 48.8%, and 92.4%, respectively; P0.0001). The sensitivity for the 10- and 14-mm tumors also increased significantly as the tumor-to-liver contrast value increased from 20 to 35 HU (P0.01). However, no significant increase in sensitivity was seen for the 10- and 14-mm tumors as the tumor-to-liver contrast values increased from 35 to 50 HU (P = 0.733 and P = 1.0, respectively).Increasing the tumor-to-liver contrast from 20 to 35 HU results in a significant increase in the detection of hypovascular liver tumors ranging from 6 to 14 mm in diameter. Optimization of the tumor-to-liver contrast is necessary for improved detection of hypovascular liver tumors.

The purpose of the study was to quantify and compare the effect of CT dose and of size and density of nodules on the detectability of lung nodules and to quantify the influence of CT dose on the size of the nodules.From 50 patients a total of 125 cuboidal regions of interest (3 × 3 × 1.5 cm volumes) showing a single nodule (≤ 8 mm) and 27 normal cuboids were selected. Image sets were reconstructed with the software from raw data simulating different dose levels: 300 (original dose), 220, 180, 140, 100, 80, 60, 50, 40, 30, 20, 10, and 5 reference mAs. A logistic regression model was used to analyze detectability for three blinded readers. Odds ratios were calculated for nodule size smaller than 3 mm versus 3 mm and larger and for nodule attenuation of -300 HU and greater versus less than -300 HU.Tube current-time settings of 10 mAs and greater were not associated with a significant difference in individual reader sensitivity compared with the standard setting of 300 mAs. At 5 mAs only one reader had a significant decrease in sensitivity, from 82% to 77% (p = 0.0035). According to the odds ratios and logistic regression results, the strongest negative effect on sensitivity can be assumed for low nodule density followed by small nodule size and dose level. The mean nodule volume measurement error between 5 and 300 mAs was 2.2% ± 18% (SD) and much lower than the interobserver volume measurement error rate of 38% ± 45%.The results show the feasibility of a low-dose CT protocol at 10 mAs for follow-up of lung nodules. Computer-aided volume measurement in follow-up of lung nodules decreases interobserver variability.

To assess the diagnostic accuracy, image quality, and radiation dose of an iterative reconstruction algorithm compared with a filtered back projection (FBP) algorithm for abdominal computed tomography (CT) at different tube voltages.A custom liver phantom with 45 simulated hypovascular liver tumors (diameters of 5, 10, and 15 mm; tumor-to-liver contrast of 10, 25, and 50 HU) was placed in a cylindrical water container that mimicked an intermediate-sized patient. The phantom was scanned at 120, 100, and 80 kVp. The CT data sets were reconstructed with FBP and iterative reconstruction. The image noise was measured, and the contrast-to-noise ratio (CNR) of the tumors was calculated. The radiation dose was assessed with the volume CT dose index. Tumor detection was independently performed by three radiologists. Statistical analysis included analysis of variance.Compared with the FBP data set at 120 kVp, the iterative reconstruction data set collected at 100 kVp demonstrated significantly lower mean image noise (20.9 and 16.7 HU, respectively; P.001) and greater mean CNRs for the simulated tumors (P.001). The iterative reconstruction data set collected at 120 kVp yielded the highest sensitivity for tumor detection, while the FBP data set at 80 kVp yielded the lowest. The sensitivity for the iterative reconstruction data set at 100 kVp was comparable with that for the FBP data set at 120 kVp (79.3% and 74.9%, respectively; P.99). The volume CT dose index decreased by 39.8% between the 120-kVp protocol and the 100-kVp protocol and by 70.3% between the 120-kVp protocol and the 80-kVp protocol.Results of this phantom study suggest that a 100-kVp abdominal CT protocol with an iterative reconstruction algorithm for simulated intermediate-sized patients increases the image quality and maintains the diagnostic accuracy at a reduced radiation dose when compared with a 120-kVp protocol with an FBP algorithm.

The effect of copper (Cu) filtration on image quality and dose in different digital X-ray systems was investigated. Two computed radiography systems and one digital radiography detector were used. Three different polymethylmethacrylate blocks simulated the pediatric body. The effect of Cu filters of 0.1, 0.2, and 0.3 mm thickness on the entrance surface dose (ESD) and the corresponding effective doses (EDs) were measured at tube voltages of 60, 66, and 73 kV. Image quality was evaluated in a contrast-detail phantom with an automated analyzer software. Cu filters of 0.1, 0.2, and 0.3 mm thickness decreased the ESD by 25-32%, 32-39%, and 40-44%, respectively, the ranges depending on the respective tube voltages. There was no consistent decline in image quality due to increasing Cu filtration. The estimated ED of anterior-posterior (AP) chest projections was reduced by up to 23%. No relevant reduction in the ED was noted in AP radiographs of the abdomen and pelvis or in posterior-anterior radiographs of the chest. Cu filtration reduces the ESD, but generally does not reduce the effective dose. Cu filters can help protect radiosensitive superficial organs, such as the mammary glands in AP chest projections.

To study the effect of a nonlinear noise filter on the detection of simulated endoleaks in a phantom with 80- and 100-kVp multidetector computed tomographic (CT) angiography.An aortic aneurysm phantom, including iodinated endoleaks, was constructed. Multidetector CT angiography with use of 80-, 100-, and 120-kVp tube voltages was performed for simulated intermediate-sized and large patients (estimated body weights, 72-85 kg and 118-142 kg, respectively). Images obtained with 80 and 100 kVp were postprocessed by using a nonlinear noise filter. CT images containing 1152 endoleaks and images with no endoleaks were randomized and independently analyzed by three radiologists blinded to the location of the endoleaks. Diagnostic confidence and image quality were rated by using subjective scales. Analysis of variance was used for statistical assessment.In simulated intermediate-sized patients, energy reduction from 120 to 100 kVp and from100 to 80 kVp did not decrease image quality when images with reduced kilovoltage were filtered (P = .2692 and P.99, respectively). Readers detected more endoleaks on the filtered 100-kVp images than on the nonfiltered images in simulated large patients (83 vs 75 lesions, P = .041). The number of detected endoleaks and the confidence rate were similar at 100 kVp with a filter and at 120 kVp in simulated large patients (P = .339 and P = .211, respectively).In a phantom, the nonlinear noise filter can prevent decreased image quality with use of 80- and 100-kVp abdominal multidetector CT angiography at a wide range of simulated body weights and may facilitate a better detection rate of endoleaks in heavy patients.

CT pulmonary angiography is the currently accepted standard in ruling out acute pulmonary embolism. Issues of radiation dose received by patients via CT have been extensively disputed by radiologists and reported by the media. In recent years there has been considerable research performed to find ways for reducing radiation exposure from CT. Herein, we will discuss specific measures that have been shown to be valuable for CT pulmonary angiography. The limitations and the potential benefits of reduced CT peak tube kilovoltage will be detailed as this method is capable of reducing both radiation exposure and iodine load to the patient simultaneously. We discuss some of the emerging tools, which will hopefully play a significant role in wider acceptance of low-dose CT pulmonary angiography protocols.

Objective The aim of the study was to examine the clinical forensic findings of strangulation according to their ability to differentiate between life-threatening and non-life-threatening strangulation, compare clinical and MRI findings of the neck and discuss a simple score for life-threatening strangulation (SLS). Materials and methods Forensic pathologists classified a continuous sample of 56 survivors of strangulation into life-threatening cases by clinical history and examination alone. Subjective, objective and radiological signs were evaluated for discriminating the two groups. Results In 27% of the cases life was in danger. The most significant signs of danger to life were congestive petechial hemorrhage in the face (eyes); hematoma of the neck and loss of consciousness. The radiological score could compete with the clinical scores. Conclusion Clinical forensic findings differ significantly in non-life-threatening and life-threatening manual strangulation. Thanks to MRI a rather objective test for life-threatening strangulation can be added to a SLS.

X-ray computed tomography (CT) using phase contrast can provide images with greatly enhanced soft-tissue contrast in comparison to conventional attenuation-based CT. We report on the first scan of a human specimen recorded with a phase-contrast CT system based on an x-ray grating interferometer and a conventional x-ray tube source. Feasibility and potential applications of preclinical and clinical phase-contrast CT are discussed.A hand of an infant was scanned ex vivo at 40 kVp tube voltage. The simultaneously recorded attenuation and phase-contrast CT images were quantitatively compared with each other, by introducing a specific Hounsfield unit for phase-contrast imaging.We observe significantly enhanced soft-tissue contrast in the phase images, when compared with the attenuation data. Particularly, tendons and ligaments appear with strongly increased contrast-to-noise ratio.Our results demonstrate the huge potential of phase-contrast CT for clinical investigations of human specimens and, potentially, of humans. Because the applied technique works efficiently with conventional x-ray tubes and detectors, it is suitable for the realization of preclinical and clinical phase-contrast CT systems.

The objective of our study was to compare the performance of low-dose linear slit digital radiography (DR) with computed radiography (CR) for the detection of trauma sequelae in the chest including rib fractures, pneumothorax, and lung contusion.Eighty trauma victims (62 males, 18 females; mean age, 51.5 years) with a total of 612 rib fractures and 80 consecutive patients without rib fractures (59 males, 21 females; mean age, 39.5 years) were retrospectively analyzed. All patients had undergone whole-body linear slit DR and consecutive chest CT, and 87 patients underwent follow-up CR of the chest within 24 hours of DR and CT. Four blinded readers assessed image quality, rib fracture localization with diagnostic confidence, and the presence of pneumothorax and lung contusion on linear slit DR and CR images. Sensitivity for rib fractures and image quality were compared using the Wilcoxon's test. For the detection of pneumothorax and lung contusion, the difference in the areas under the receiver operating characteristic curves were calculated.The rate of correctly identified rib fractures was higher (true-positive findings per image, 2.55 vs 2.21, respectively; p = 0.02), the rate of missed fractures was lower (false-negative findings per image, 4.98 vs 6.19; p = 0.02), and the diagnostic confidence was greater (2.03 vs 1.73 on a 3-point scale; p = 0.01) with linear slit DR than with CR, respectively. Image quality and performance for detecting pneumothorax and lung contusion with both techniques were not statistically different (p = 0.22, 0.85, and 0.55, respectively).Linear slit DR is a reliable substitute for CR in the initial evaluation of chest trauma, with better sensitivity for detecting rib fractures and similar performance in assessing pneumothorax and lung contusion.

OBJECTIVE. In this experimental study we assessed the diagnostic performance of digital linear slit scanning radiography compared with computed radiography (CR) for the detection of urinary calculi in an anthropomorphic phantom imitating patients weighing approximately 58–88 kg.CONCLUSION. Compared with CR, linear slit scanning radiography is superior for the detection of urinary stones and may be used for pretreatment localization and follow-up at a lower patient exposure.

The aim of the study was to determine the sensitivity and specificity for typical abdominal injuries after major blunt trauma in postmortem multislice computed tomography (MSCT) and magnetic resonance imaging (MRI).Thirty-four cases of accidental death underwent postmortem pre-autopsy MSCT and MRI. The imaging findings were correlated with the autopsy findings.Sensitivity and specificity for liver injury in computed tomography (CT) alone were 53% and 84%. In MRI, a sensitivity of 58% and a specificity of 46% were found. CT and MRI together (when either one was positive, liver-injury was considered) had a sensitivity of 73% and a specificity of 63%. For major liver lacerations (grades II-VI) a slightly higher sensitivity of 81% and a better specificity of 100% were noted. CT and MRI together showed a sensitivity of 50% for injuries of the spleen, specificity was 89%. CT and MRI together had a sensitivity of only 25% for trauma of the kidney, but a specificity of 100%.A lot of cases with small organ injuries (such as superficial liver-laceration) seemed that could not be found by MSCT or by MRI, leading to a rather low sensitivity. Nevertheless, most of the life-threatening liver injuries could be detected, which is essential in forensic pathology.

BACKGROUND AND OBJECTIVE The preoperative investigation for choledocholithiasis in patients undergoing elective laparoscopic cholecystectomy is still a matter of debate. PATIENTS AND METHODS In a prospective clinical trial the accuracy of intravenous cholangiography (IVC), ultrasonography and liver function tests in the preoperative diagnosis of choledocholithiasis was assessed in 98 patients undergoing elective cholecystectomy. Only patients with uncomplicated cholecystolithiasis considered to be at low risk for having bile duct stones were investigated. A 2-year follow-up clinical survey (mean) was performed in 92 of the 98 patients to investigate the occurrence of postoperative choledocholithiasis (gold standard: clinically manifest choledocholithiasis). RESULTS In this patient cohort the incidence of choledocholithiasis was found to be 5.1%. Among the three diagnostic tests IVC proved to be more accurate with higher sensitivity and a better positive predictive value than ultrasonography and liver function tests. The sensitivity for IVC was 100% compared to 20% for ultrasonography and 40% for liver function tests, respectively. The positive predictive value for IVC was 83.3% in comparison to 20% for ultrasonography and 25% for liver function tests. Mild side effects caused by intravenous contrast media were observed in 2.0%. During a mean postoperative follow-up of 2 years no clinically manifest and initially overlooked choledocholithiasis could be detected in the 92 investigated patients. CONCLUSION IVC is a reliable method to detect unsuspected common bile duct stones and should be used in the preoperative diagnosis prior to elective laparoscopic cholecystectomy. IVC may play a role in decreasing the rate of preoperative ERCP or intraoperative cholangiography in these patients.

OBJECTIVE: Postmortem examination of chest trauma is an important domain in forensic medicine, which is today performed using autopsy. Since the implementation of cross-sectional imaging methods in forensic medicine such as computed tomography (CT) and magnetic resonance imaging (MRI), a number of advantages in comparison with autopsy have been described. Within the scope of validation of cross-sectional radiology in forensic medicine, the comparison of findings of postmortem imaging and autopsy in chest trauma was performed. METHODS: This retrospective study includes 24 cases with chest trauma that underwent postmortem CT, MRI, and autopsy. Two board-certified radiologists, blind to the autopsy findings, evaluated the radiologic data independently. Each radiologist interpreted postmortem CT and MRI data together for every case. The comparison of the results of the radiologic assessment with the autopsy and a calculation of interobserver discrepancy was performed. RESULTS: Using combined CT and MRI, between 75% and 100% of the investigated findings, except for hemomediastinum (70%), diaphragmatic ruptures (50%; n=2) and heart injury (38%), were discovered. Although the sensitivity and specificity regarding pneumomediastinum, pneumopericardium, and pericardial effusion were not calculated, as these findings were not mentioned at the autopsy, these findings were clearly seen radiologically. The averaged interobserver concordance was 90%. CONCLUSION: The sensitivity and specificity of our results demonstrate that postmortem CT and MRI are useful diagnostic methods for assessing chest trauma in forensic medicine as a supplement to autopsy. Further radiologic-pathologic case studies are necessary to define the role of postmortem CT and MRI as a single examination modality.

Radiological identification is important in forensic medicine. Identification using comparison of individualising structures with ante- and post-mortem conventional radiographs has been known for a long time. New radiological procedures such as computed tomography (CT) and magnetic resonance imaging (MRI) are being increasingly used for identification. In this paper, a new comparative approach using various radiological methods is described and its application demonstrated. This new approach is the comparison of ante-mortem conventional radiographs with projected images calculated from post-mortem CT data. The identification procedure will be illustrated with reference to the frontal sinus and the pelvis.

After a mass fatality incident (MFI), all victims have to be rapidly and accurately identified for juridical reasons as well as for the relatives' sake. Since MFIs are often international in scope, Interpol has proposed standard disaster victim identification (DVI) procedures, which have been widely adopted by authorities and forensic experts. This study investigates how postmortem multislice computed tomography (MSCT) can contribute to the DVI process as proposed by Interpol. The Interpol postmortem (PM) form has been analyzed, and a number of items in sections D and E thereof have been postulated to be suitable for documentation by CT data. CT scans have then been performed on forensic cases. Interpretation of the reconstructed images showed that indeed much of the postmortem information required for identification can be gathered from CT data. Further advantages of the proposed approach concern the observer independent documentation, the possibility to reconstruct a variety of images a long time after the event, the possibility to distribute the work by transmitting CT data digitally, and the reduction of time and specialists needed at the disaster site. We conclude that MSCT may be used as a valuable screening tool in DVI in the future.

BACKGROUND: Until August 2004 there were 106 forensic cases examined with postmortem multislice computed tomography (MSCT) and magnetic resonance (MR) imaging before traditional autopsy within the Virtopsy project. Intrahepatic gas (IHG) was a frequent finding in postmortem MSCT examinations. The aim of this study was to investigate its cause and significance. METHODS: There were 84 virtopsy cases retrospectively investigated concerning the occurrence, location, and volume of IHG in postmortem MSCT imaging (1.25 mm collimation, 1.25 mm thickness). We assessed and noted the occurrence of intestinal distention, putrefaction, and systemic gas embolisms and the cause of death, possible open trauma, possible artificial respiration, and the postmortem interval. We investigated the relations between the findings using the contingency table (chi2 test) and the comparison of the postmortem intervals in both groups was performed using the t test in 79 nonputrefied corpses. RESULTS: IHG was found in 47 cases (59.5%). In five of the cases, the IHG was caused or influenced by putrefaction. Gas distribution within the liver of the remaining 42 cases was as follows: hepatic arteries in 21 cases, hepatic veins in 35 cases, and portal vein branches in 13 cases; among which combinations also occurred in 20 cases. The presence of IHG was strongly related to open trauma with systemic gas. Pulmonary barotrauma as occurring under artificial respiration or in drowning also caused IHG. Putrefaction did not seem to influence the occurrence of IHG until macroscopic signs of putrefaction were noticeable. CONCLUSIONS: IHG is a frequent finding in traumatic causes of death and requires a systemic gas embolism. Exceptions are putrefied or burned corpses. Common clinical causes such as necrotic bowel diseases appear rarely as a cause of IHG in our forensic case material.

Purpose Currently, in forensic medicine cross-sectional imaging gains recognition and a wide use as a non-invasive examination approach. Today, computed tomography (CT) or magnetic resonance imaging that are available for patients are unable to provide tissue information on the cellular level in a non-invasive manner and also diatom detection, DNA, bacteriological, chemical toxicological and other specific tissue analyses are impossible using radiology. We hypothesised that post-mortem minimally invasive tissue sampling using needle biopsies under CT guidance might significantly enhance the potential of virtual autopsy. The purpose of this study was to test the use of a clinically approved biopsy needle for minimally invasive post-mortem sampling of tissue specimens under CT guidance. Material and methods ACN III biopsy core needles 14 gauge × 160 mm with automatic pistol device were used on three bodies dedicated to research from the local anatomical institute. Tissue probes from the brain, heart, lung, liver, spleen, kidney and muscle tissue were obtained under CT fluoroscopy. Results CT fluoroscopy enabled accurate placement of the needle within the organs and tissues. The needles allowed for sampling of tissue probes with a mean width of 1.7 mm (range 1.2–2 mm) and the maximal length of 20 mm at all locations. The obtained tissue specimens were of sufficient size and adequate quality for histological analysis. Conclusion Our results indicate that, similar to the clinical experience but in many more organs, the tissue specimens obtained using the clinically approved biopsy needle are of a sufficient size and adequate quality for a histological examination. We suggest that post-mortem biopsy using the ACN III needle under CT guidance may become a reliable method for targeted sampling of tissue probes of the body.

Abnormally high pulmonary artery pressure (PAP) in hypoxia due to exaggerated hypoxic pulmonary vasoconstriction (HPV) is a key factor for development of high-altitude pulmonary edema (HAPE). It was shown that about 10% of a healthy Caucasian population has an exaggerated HPV that is comparable to the response measured in HAPE-susceptible individuals. Therefore, we hypothesized that those with exaggerated HPV are HAPE-susceptible.We screened 421 healthy Caucasians naïve to high altitude for HPV using Doppler echocardiography for assessment of systolic PAP in normobaric hypoxia (PASPHx; Po2 corresponding to 4500 m). Subjects with exaggerated HPV and matched controls were exposed to 4559 m with an identical protocol that causes HAPE in 62% of HAPE-S. Screening revealed 39 subjects with exaggerated HPV, of whom 33 (PASPHx 51±6 mmHg) ascended within 24 hours to 4559 m. Four (13%) of them developed HAPE during the 48 h-stay. This incidence is significantly lower than the recurrence rate of 62% previously observed in HAPE-S in the same setting. None of the control subjects (PASPHx 33±5 mmHg) developed HAPE.An exaggerated HPV cannot be considered a surrogate maker for HAPE-susceptibility although excessively elevated PAP is a hallmark in HAPE, while a normal HPV appears to protect from HAPE in this study.

Von der konventionellen Röntgenaufnahme bis zu den modernen Schnittbildverfahren bietet die Radiologie ein breites Spektrum von diagnostischen Möglichkeiten in der Abklärung von Patienten mit Fieber. Um die besten Resultate und eine korrekte Diagnose zu erzielen, muss der Radiologe das diagnostische Protokoll an jeden Patient individuell anpassen. Die Auswahl der bestgeeigneten Methode und die Art und Weise der Kontrastmittel-Applikation werden durch die klinische Fragestellung und Verdachtsdiagnose stark beeinflusst. Aufgrund der Anamnese und Laborwerte sind bestimmte Untersuchungen kontraindiziert, oder die Patienten müssen entsprechend prämediziert werden. Die Autoren geben eine kurze Zusammenfassung über die diagnostischen Untersuchungsstrategien in der Abklärung der wichtigsten Fieberursachen und betonen die Wichtigkeit der engen Zusammenarbeit zwischen Zuweiser und Radiologen.

OBJECTIVE: Autopsy determination of fatal hemorrhage as the cause of death is often a difficult diagnosis in forensic medicine. No quantitative system for accurately measuring the blood volume in a corpse has been developed. MATERIALS AND METHODS: This article describes the measurement and evaluation of the cross-sectional areas of major blood vessels, of the diameter of the right pulmonary artery, of the volumes of thoracic aorta and spleen on MDCT, and of the volumes of heart chambers on MRI in 65 autopsy-verified cases of fatal hemorrhage or no fatal hemorrhage. RESULTS: Most cases with a cause of death of "fatal hemorrhage" had collapsed vessels. The finding of a collapsed superior vena cava, main pulmonary artery, or right pulmonary artery was 100% specific for fatal hemorrhage. The mean volumes of the thoracic aorta and of each of the heart chambers and the mean cross-sectional areas of all vessels except the inferior vena cava and abdominal aorta were significantly smaller in fatal hemorrhage than in no fatal hemorrhage. CONCLUSION: For the quantitative differentiation of fatal hemorrhage from other causes of death, we propose a three-step algorithm with measurements of the diameter of the right pulmonary artery, the cross-sectional area of the main pulmonary artery, and the volume of the right atrium (specificity, 100%; sensitivity, 95%). However, this algorithm must be corroborated in a prospective study, which would eliminate the limitations of this study. Quantitative postmortem cross-sectional imaging might become a reliable objective method to assess the question of fatal hemorrhage in forensic medicine.

Hypoxia of renal medulla is a key factor implicated in the development of drug-induced renal failure. Drugs are known to influence renal hemodynamics and, subsequently, affect renal tissue oxygenation. Changes in renal oxygenation can be assessed non-invasively in humans using blood oxygenation level-dependent magnetic resonance imaging (BOLD-MRI). This study was designed to test the acute effects of administration of specific drugs in healthy human kidney oxygenation using BOLD-MRI. Acute changes in renal tissue oxygenation induced by the non-steroidal anti-inflammatory drug indomethacin, the iodinated radio-contrast media (RCM) iopromidum, and the calcineurin inhibitors cyclosporine micro-emulsion (CsA-ME) and tracrolimus were studied in 30 healthy volunteers. A modified Multi Echo Data Image Combination sequence was used to acquire 12 T2*-weighted images. Four coronal slices were selected to cover both kidneys. The mean R2* (1/T2*) values determined in medulla and cortex showed no significant changes induced by indomethacin and tacrolimus administration. CsA-ME decreased medullary (P=0.008) and cortical (P=0.004) R2* values 2 h after ingestion. Iopromidum caused a significant increase in medullary R2* within the first 20 min after injection (P

The purpose of this study was to determine if the thoracic vertebral elements are altered in patients with Marfan's syndrome.Thirty patients underwent helical CT of the thorax because of suspected thoracic aortic dilatation and acute dissection. Thirteen had Marfan's syndrome and 17 did not. Two reviewers, unaware of the final diagnosis, evaluated the images by consensus for laminar thickness, foraminal width, dural sac ratios, and vertebral scalloping for T2-T12.At T9-T12, dural sac ratios at the midcorpus level (p = 0.031) and foraminal width (p = 0.0124) were significantly greater in the patients with Marfan's syndrome than in the patients without. Dural sac ratios at lower endplate levels (p = 0.0685), laminar thickness (p = 0.951), and vertebral scalloping (p = 0.24) were not significantly greater in the patients with Marfan's syndrome than in the patients without.Because the phenotypic expression of Marfan's syndrome is variable, information on the spine from thoracic studies in combination with major criteria may be helpful clinically.

We evaluated the incidence and clinical relevance of alternate or additional findings on unenhanced CT in patients with acute flank pain and suspected urinary calculi.A consecutive series of 1,500 patients underwent unenhanced CT due to acute flank pain. The absence or presence of urinary tract calculi and their localization were recorded. Alternate or additional CT findings were classified according to whether they required immediate or deferred treatment, or were of little or no clinical importance.Of the 1,500 patients 1,035 (69%) had urinary tract calculi, including 309 (30%) with nephrolithiasis, 377 (36%) with ureterolithiasis and 349 (34%) with the 2 conditions. Urolithiasis alone was found in 331 of these patients (32%) and additional pathological conditions were noted in 704 (68%). Of all patients 1,064 (71%) had other or additional CT findings. Of all patients 207 (14%) had nonstone related CT findings requiring immediate or deferred treatment, 464 (31%) had pathological conditions of little clinical importance and 393 (26%) had pathological conditions of no clinical relevance. CT was normal in 105 of all patients (7%).Unenhanced CT in patients with acute flank pain allows the accurate diagnosis of urinary stone disease and it can also provide further important information leading to emergency or deferred treatment in a substantial number of patients.

An objective analysis of image quality parameters was performed for a computed radiography (CR) system using both standard single-side and prototype dual-side read plates. The pre-sampled modulation transfer function (MTF), noise power spectrum (NPS), and detective quantum efficiency (DQE) for the systems were determined at three different beam qualities representative of pediatric chest radiography, at an entrance detector air kerma of 5 {mu}Gy. The NPS and DQE measurements were realized under clinically relevant x-ray spectra for pediatric radiology, including x-ray scatter radiations. Compared to the standard single-side read system, the MTF for the dual-side read system is reduced, but this is offset by a significant decrease in image noise, resulting in a marked increase in DQE (+40%) in the low spatial frequency range. Thus, for the same image quality, the new technology permits the CR system to be used at a reduced dose level.

The rapid development of computed tomography (CT) and magnetic resonance imaging (MRI) led to the introduction and establishment in postmortem investigations. The objectives of this preliminary study were to describe the imaging appearances of the early postmortem changes of blood after cessation of the circulation, such as sedimentation, postmortem clotting, and internal livores, and to give a few first suggestions on how to differentiate them from other forensic findings. In the Virtopsy project, 95 human corpses underwent postmortem imaging by CT and MRI prior to traditional autopsy and therefore 44 cases have been investigated in this study. Postmortem alterations as well as the forensic relevant findings of the blood, such as internal or subcutaneous bleedings, are presented on the basis of their imaging appearances in multislice CT and MRI.

Decisions about the treatment of neck pain are largely made on the basis of information gained from plain X-rays and magnetic resonance imaging (MRI), which are used routinely as part of preliminary investigation. We performed a descriptive cadaveric study to compare histology with radiography and MRI. We correlated plain radiography, disc height [Farfan index (FI)] and MRI findings with histology to assess the ability of radiology to detect significant pathologic lesions. The study included 52 motion segments from nine subjects over the age of 50, who underwent routine hospital autopsy. Disc degeneration was assessed by histology, radiography, disc height (FI: anterior disc height plus posterior disc height divided by anterioposterior diameter) and MRI using established grading systems. Most of the discs were classified radiologically as grade 1 (19/52), grade 2 (13/52), grade 3 (9/52) or grade 4 (3/52). Eight of the discs were graded as normal. The distribution of MRI grades was grade 0 (9/36), grade 1 (9/36), grade 2 (7/36), grade 3 (8/36) and grade 4 (3/36). Half of the discs (26/52) showed advanced (grade 4) degeneration histologically. FI correlated with histological grade (P = 0.013), MRI grade (P = 0.02) and radiological grade (P0.001) of degeneration. Radiological and histological grade of degeneration showed a weak correlation (r = 0.3, P = 0.033). MRI correlated with overall histological grade (r = 0.41, P = 0.015, n = 34). Histological features (e.g., tears, rim lesions, prolapse of nucleus material) were poorly recognised by MRI, which had a sensitivity for disc material prolapse and annulus tears of less than 40%. Our study showed that discs from patients over 50 years are histologically severely degenerated; however, these changes may not be detected by conventional radiography and MRI.

MDCT is especially suited for emergency purposes because it allows rapid high-resolution scans of large areas, fast high-quality reformatting in every orientation, and 3D illustration of the data set. In a prospective study, we evaluated the reliability and workflow of a dedicated emergency department 16-MDCT scanner in the management of patients presenting to the emergency department.The use of a 16-MDCT scanner for 503 patients in the emergency department of a university clinic was evaluated. For reasons of workflow analysis, seven precise time intervals were recorded during the emergency examinations. A new setting for repositioning multiple-trauma patients after imaging of the head and neck from the head-first position to the feet-first position was introduced.Six (1.2%) of the 503 patients were excluded because of technical malfunction or patient noncompliance. Image quality in the remaining 497 cases, including CT angiography and CT of multiple-trauma patients, was outstanding. Positioning of the patients took from 3 to 13 min depending on the body region examined, representing 33-67% of the mean room time, which ranged from 8 to 21 min. In multiple-trauma patients, the initial positioning took a mean of 6 min and repositioning took 8 min, representing 19% and 26% of total room time, respectively.The use of a dedicated 16-MDCT scanner in the emergency department resulted in short examination times even for examinations of multiple body regions under emergency conditions. The introduced setting-repositioning of multiple-trauma patients-allowed high image quality to be maintained. The trade-off in multiple-trauma patients was prolonged room time because of patient repositioning.

Postmortem investigation is increasingly supported by Computed Tomography (CT) and Magnetic Resonance Imaging (MRI). This led to the idea to implement a noninvasive or minimally invasive autopsy technique. Therefore, a minimally invasive angiography technique becomes necessary, in order to support the vascular cross section diagnostic. Preliminary experiments investigating different contrast agents for CT and MRI and their postmortem applicability have been performed using an ex-vivo porcine coronary model. MSCT and MRI angiography was performed in the porcine model. Three human corpses were investigated using minimally invasive MSCT angiography. Via the right femoral artery a plastic tube was advanced into the aortic arch. Using a flow adjustable pump the radiopaque contrast agent meglumine-ioxithalamate was injected. Subsequent MSCT scanning provided an excellent anatomic visualization of the human arterial system including intracranial and coronary arteries. Vascular pathologies such as calcification, stenosis and injury were detected. Limitations of the introduced approach are cases of major vessel injury and cases that show an advanced stage of decay.

The authors of this article [1] have to be congratulated for their clear presentation of stepwise, process-based informed consent, including evidence-based justification, full patient information on benefits and risks, and interactive communication with enough time for the patient before taking the decision on whether to undergo an examination. Recchia et al. are in line with the clinical consent process [2], and even some radiological articles head in the same direction [3, 4]. The authors will likely get unanimous approval for their approach whenever risky interventions or complex examinations with an elevated dose level are considered. Non-invasive imaging examinations using low-dose levels, however, are not discussed differently by the authors, although this contrasts to the reality in radiology departments in Europe, the USA [5] and probably most countries. In other words, the theory is not applied in a large segment of the real world, and this brings up several questions

Objective: To assess the sensitivity of noninvasive virtual bronchoscopy based on multirow detector CT scanning in detecting and grading central and segmental airway stenosis using flexible bronchoscopic findings as the reference standard. Materials and methods: In a blinded controlled trial, multirow detector CT virtual bronchoscopy and flexible bronchoscopy were used to search for and grade airway stenosis in 20 patients. CT scan data were obtained with a multirow detector CT scanner using 4 1 mm collimation. Flexible bronchoscopy findings were graded by a pulmonologist and served as the reference standard for 176 central airway regions (ie, trachea, main bronchi, and lobar bronchi) and 302 segmental airway regions. The extent of airway narrowing was categorized as grade 0 (no narrowing), grade 1 ( 50%). Results: Flexible bronchoscopy revealed 30 stenoses in the central airways and 10 in the segmental airways. Virtual bronchoscopy detected 32 stenoses in the central airways (sensitivity, 90.0%; specificity, 96.6%; accuracy, 95.5%) and 22 in the segmental airways (sensitivity, 90.0%; specificity, 95.6%; accuracy, 95.5%). The number of false-positive findings was higher in the segmental airways (13 false-positive findings) than in the central airways (5 false-positive findings), which caused a lower positive predictive value for the segmental airways (40.9%) than for the central airways (84.4%). Flexible and virtual bronchoscopic gradings correlated better for central airway stenosis (r 0.87) than for segmental airway stenosis (r 0.61). Conclusion: Although a high sensitivity was found for the detection of both central and segmental airway stenosis, the number of false-positive findings was higher for segmental airways. However, noninvasive multirow detector CT virtual bronchoscopy enables high-resolution endoluminal imaging of the airways down to the segmental bronchi. (CHEST 2004; 125:704 –711)

Traumatic lesions of the subcutaneous fatty tissue provide important clues for forensic reconstruction. The interpretation of these patterns requires a precise description and recording of the position and extent of each lesion. During conventional autopsy, this evaluation is performed by dissecting the skin and subcutaneous tissues in successive layers. In this way, depending on the force and type of impact (right angle or tangent), several morphologically distinct stages of fatty tissue damage can be differentiated: perilobular hemorrhage (I), contusion (II), or disintegration (III) of the fat lobuli, and disintegration with development of a subcutaneous cavity (IV). In examples of virtopsy cases showing blunt trauma to the skin and fatty tissue, we analyzed whether these lesions can also be recorded and classified using multislice computed tomography (MSCT) and magnetic resonance imaging (MRI). MSCT has proven to be a valuable screening method to detect the lesions, but MRI is necessary in order to properly differentiate and classify the grade of damage. These noninvasive radiological diagnostic tools can be further developed to play an important role in forensic examinations, in particular when it comes to evaluating living trauma victims.

A man wearing no protective helmet was struck by a motor vehicle while riding a bicycle. He was loaded on his left side, and the impact point of his head was his occiput on the car roof girder. He was immediately transported to the general hospital, where he passed away. Postmortem examination using multi-slice computed tomography (MSCT) revealed an extensively comminuted fracture of the posterior part and the base of the skull. Observed were deep direct and contrecoup brain bruises, with the independent fractures of the roof of the both orbits. Massive subdural and subarachnoidal hemorrhage with cerebral edema and shifting of the mid-line towards left side were also detected. MSCT and autopsy results were compared and the body injuries were correlated to vehicle damages. In conclusion, postmortem imaging is a good forensic visualization tool with great potential for documentation and examination of body injuries and pathology.

It is impossible to obtain a representative anatomical documentation of an entire body using classical X-ray methods, they subsume three-dimensional bodies into a two-dimensional level. We used the novel multislice-computed tomography (MSCT) technique in order to evaluate a case of homicide with putrefaction of the corpse before performing a classical forensic autopsy. This non-invasive method showed gaseous distension of the decomposing organs and tissues in detail as well as a complex fracture of the calvarium. MSCT also proved useful in screening for foreign matter in decomposing bodies, and full-body scanning took only a few minutes. In conclusion, we believe postmortem MSCT imaging is an excellent vizualisation tool with great potential for forensic documentation and evaluation of decomposed bodies.

Our objective was to study the impact of low-dose multislice thoracic CT on image quality and lesion conspicuity in follow-up of patients with malignant lymphoma and extrapulmonary primary tumors. Forty consecutive patients with a history of malignant lymphoma or extrapulmonary malignant primaries who had undergone previous standard-dose thoracic spiral CT (120 kVp, 150 mAs, 8 mm) were subjected to low-dose multislice CT (15 mAs/rotation, 120 kVp, pitch 1.5, 4x2-mm collimation reconstructed to contiguous 8-mm slices. Image quality and lesion conspicuity were classified independently by two readers on a four-point ordinal scale (1=poor, 4=excellent). Mean image quality was significantly lower for low-dose CT, 2.35, than for standard-dose CT, 3.25 ( p0.001); however, all low-dose CT examinations were fully readable, none had to be repeated, and no lesions (including 51 lung lesions and 31 soft tissue lesions) remained undetected. Only in the apical lung did the lowered tube current significantly reduce lesion conspicuity. Even in pulmonary lesions smaller than 10 mm ( n=21) lesion conspicuity did not significantly differ between standard and low-dose scans. Low-dose thoracic multislice CT based on a 90% reduction in dose compared with standard-dose techniques was not associated with impaired detection of suspicious lesions and may be useful in follow-up of malignant lymphoma and extrapulmonary tumors.

The goal of this study was the full-body documentation of a gunshot wound victim with multislice helical computed tomography for subsequent comparison with the findings of the standard forensic autopsy. Complete volume data of the head, neck, and trunk were acquired by use of two acquisitions of less than 1 minute of total scanning time. Subsequent two-dimensional multiplanar reformations and three-dimensional shaded surface display reconstructions helped document the gunshot-created skull fractures and brain injuries, including the wound track, and the intracerebral bone fragments. Computed tomography also demonstrated intracardiac air embolism and pulmonary aspiration of blood resulting from bullet wound-related trauma. The "digital autopsy," even when postprocessing time was added, was more rapid than the classic forensic autopsy and, based on the nondestructive approach, offered certain advantages in comparison with the forensic autopsy.

The aim of analogue model experiments in geology is to simulate structures in nature under specific imposed boundary conditions using materials whose rheological properties are similar to those of rocks in nature. In the late 1980s, X-ray computed tomography (CT) was first applied to the analysis of such models. In early studies only a limited number of cross-sectional slices could be recorded because of the time involved in CT data acquisition, the long cooling periods for the X-ray source and computational capacity. Technological improvements presently allow an almost unlimited number of closely spaced serial cross-sections to be acquired and calculated. Computer visualization software allows a full 3D analysis of every recorded stage. Such analyses are especially valuable when trying to understand complex geological structures, commonly with lateral changes in 3D geometry. Periodic acquisition of volumetric data sets in the course of the experiment makes it possible to carry out a 4D analysis of the model, i.e. 3D analysis through time. Examples are shown of 4D analysis of analogue models that tested the influence of lateral rheological changes on the structures obtained in contractional and extensional settings.

Purpose: We determine long-term morphological changes of the upper urinary tract following ileal orthotopic bladder substitution with an afferent tubular segment without a flap-valve type antireflux procedure.Materials and Methods: A consecutive series of long-term survivors (5 years or greater) following ileal orthotopic bladder substitution underwent regular radiological followup with excretory urography. Preoperative and postoperative renal size, parenchymal thickness, pelvicaliceal dilatation and upper urinary tract obstruction were assessed by 2 radiologists.Results: A total of 76 patients with a median followup of 84 months (range 60 to 155) were evaluable. Four patients (2.6%) underwent unilateral nephroureterectomy during followup. Of the remaining 148 renoureteral units 141 (95%) revealed no significant changes in size or parenchymal thickness during followup. Renal size decreased in 6 (4%) and parenchymal thickness in 7 (5%) renoureteral units, 5 of which already had renal pathologies (d...
