Research articles in English have witnessed a growing emphasis on self-promotion, posing challenges for Spanish social scientists when writing for English-medium journals. One difficulty lies in effectively promoting their own research without compromising necessary content and exceeding word limits. To address this issue, this study explores strategies for making room for research promotion in the discussion/closing sections of social science research articles. A comparative analysis of these sections in English and Spanish was conducted, focusing on the communicative functions fulfilled by different segment types. The findings confirm that authors in English prioritise promoting the quality and applicability of their research over its contribution and relevance. They also reveal that these authors tend to include fewer non-promotional segments restating results, providing background information, and elaborating ideas compared to their Spanish counterparts. However, both groups of authors consistently include segments commenting on the results, as well as making recommendations for future research/practice. These segments are considered essential in this part-genre. This study provides insights for Spanish social scientists balancing self-promotion and other communicative goals in English research writing. The article analyzes linguistic means used for boosting which is considered a crucial metadiscourse strategy regularly employed in the genre of research article abstracts. The study aims at identifying the types of booster and the frequency of occurrence of boosting markers in Russian-language research article abstracts in the field of soft and hard sciences. The study makes an attempt to reveal differences in the use of these metadiscourse patterns and suggest reasons for the uneven distribution of boosters in the two corpora. The study is based on the assumption that disciplinary conventions affect the choice of rhetorical strategies. The study draws on a corpus of 180 research article abstracts published in 12 soft and hard science journals. The theoretical basis of the study is the taxonomy of metadiscourse markers proposed by Hyland. The findings showed that soft science authors used boosting markers much less frequently than their hard science counterparts. Differences were also revealed in terms of the types of boosting. The results of the analysis confirmed the assumption that boosting as a metadiscourse strategy is determined by disciplinary conventions. The results obtained can be used both by teachers of academic writing and academic writing course designers and become an avenue for further research into metadiscourse as an important rhetorical strategy.

Background/Objectives A drug-related problem (DRP) is an event or circumstance involving drug therapy that actually or potentially interferes with desired health outcomes. The aim of this study is to identify and categorize the DRPs associated with the use of antibiotics in women undergoing cesarean delivery and to analyze the causes of these problems, making an improvement proposal of preoperative antibiotic prophylaxis (PAP) use in accordance with international guidelines. Methods Observational, retrospective, descriptive study of 814 women undergoing cesarean delivery. Prescription adequacy was assessed based on the selection, administration, dose and duration of the antibiotic therapy. DRP analysis and characterization was conducted using the Pharmaceutical Care Network Europe (PCNE) Classification V 9.1. Results DRPs were detected in all women (1125, 1.38 DRPs/patient), the most frequent ones associated to failure to administer PAP (30.10% of patients) and to unnecessary postsurgical antibiotic (100%). Of the 1935 possible causes for DRPs identified, most of them were related to inappropriate drug selection. Cefazolin was administered to 98.07% of women with PAP and 71.45% of patients received postsurgical cefalexin, with 13 different antibiotics being used in total. Conclusions The implementation of therapeutic protocols and early identification of DRPs is proposed in order to optimize antibiotic use in the institution.

Background The utilization of zein and gum arabic has grown in an attempt to formulate wall materials based on protein–polysaccharide complexes. This mixture provides a versatile shelter for hydrophilic (guava leaf extract, GLE) or lipophilic (fish oil, FO) bioactive compounds from unwanted environmental factors, and it can be used as an edible film-forming polymer. This study was undertaken to characterize FO-containing edible films that were double-layered with a film containing GLE. Methods Modified zein and gum arabic solutions (MG complex) were mixed at a ratio of 1:1.5 (v/v), adjusted to pH 5, added with glycerol (20% of the complex) and FO (5% of the complex), and finally adjusted to pH 5. This was prepared as the bottom/lower layer. The upper/active layer was prepared by mixing MG complex, glycerol, and GLE (1, 3, and 5% w/v of the complex). Physical, mechanical, microstructural, thermal, microbiological, and oxidative measurements were also performed. Results The total phenolic and flavonoid contents in GLE were 15.81 mg GAE/g extract and 6.99 mg QE/g extract, respectively. The IC 50 of the DPPH radical scavenging activity of GLE was 26.86 ppm with antibacterial activity against Bacillus subtilis and Escherichia coli of 9.83 and 12.55 mm. The total plate counts of films double-layered with a film containing GLE were retained below 3 log CFU/g during 28-day storage. The peroxide values of these films were dimmed for no more than 9.08 meq/kg sample on day 28 of storage. Thickness (872.00-971.67 μm), water vapor transmission rate (12.99-17.04 g/m 2/day), tensile strength (1.56-2.02 kPa), elongation at break (61.53-75.41%), glass transition (52.74-57.50°C), melting peak (131.59-142.35°C), inhibition against B. subtilis (33.67-40.58 mm), and inhibition against E. coli (2.05-9.04 mm) were obtained by double-layer films. Conclusions GLE can be successfully incorporated into the active layer of a double-layer film to improve its characteristics while significantly slowing down the microbial contamination and oxidation rate.

Background The profound impacts of climate change on the environment, economy, and society are anticipated to significantly influence smallholder farmers, whose livelihoods and traditional ways of life are inextricably intertwined with the natural environment. This comprehensive study delves into the multifaceted factors shaping the adoption of climate-smart agricultural practices and their subsequent effects on crop productivity among the small-scale farming communities in Nyimba District, situated within Zambia. Methods The study collected data from 194 smallholder farmer households across 12 villages in 4 agricultural camps of Nyimba District between June and July 2022. A logistic regression model was utilized to assess the factors influencing crop production and the adoption of climate-smart agricultural practices in response to climate change and variability within the study area. Furthermore, propensity score matching was performed to evaluate the impacts of adopting climate-smart agriculture by comparing adopter households with their non-adopter counterparts. Results The logit regression model’s findings from the research indicate that several factors influence smallholder farmer households’ adoption of climate-smart agricultural techniques and crop productivity. These factors include the farmer’s educational level, household size, fertilizer utilization, the household head’s age and gender, farming experience duration, livestock ownership status, annual household income, farmland size, the household head’s marital status, and access to climate-related information. Additionally, the propensity score matching analysis revealed that crop yields among smallholder farmers who adopted climate-smart agricultural practices were 20.20% higher compared to non-adopters. Furthermore, the analysis showed that implementing such practices in the study area led to a 21.50% increase in maize yields for adopter households relative to their non-adopter counterparts. Conclusion The findings of this research offer valuable insights to policymakers, guiding them in enhancing farmers’ climate change adaptation strategies and formulating relevant policies. Nevertheless, these initiatives and concerted efforts possess the potential to mitigate the detrimental impacts posed by climate change and climatic variability. Background: Insomnia is difficulty initiating or maintaining sleep for at least three nights a week or more and lasting for at least 3 months. One of the molecules that play a role in the circadian rhythm of arousal system is hypocretin/orexin. Orexin activates the p38-MAPK signaling pathway and increases phosphorylated ERK1/2 levels. Centella asiatica (CA) has a role in the signal work of the MAPK/ERK, Akt, and p38 path in many various diseases. Methods: The research method used is true laboratory experimental. The research approach used was randomized control group post-test only. Zebrafish embryos aged 0-7 dpf were used in this study. The treatment group consisted of 5 groups: normal, insomnia, insomnia + 2.5 μg/mL CA, insomnia + 5 μg/mL CA, and insomnia + 10 μg/mL CA. The locomotor motion of zebrafish larvae was observed using Basler cameras on days five-, six- and seven-day post fertilization (dpf), then analyzed by using Western Blot method. Results: The results proved that exposure to CA extract was able to reduce the expression of orexin (91963 ± 9129) and p38 (117425 ± 6398) as an arousal trigger in the sleep-wake cycle, with the most optimal concentration of CA 5 μg/mL. Exposure to CA extract was also able to reduce the expression of ERK (94795 ± 30830) and Akt (60113.5 ± 27833.5) with an optimum concentration of CA 2.5 μg/mL. Conclusion: Exposure to CA extract was able to improve the sleep activity of zebrafish larvae insomnia model by extending the total inactivity time ( cumulative duration) and shortening the duration of first sleep ( latency to first) in light and dark phases through inhibition of orexin, ERK, p38, and Akt.

Objective: To evaluate the scientific production on university social responsibility (USR) from institutions in Latin America and the Caribbean. Methods: A bibliometric analysis was conducted on documents published in indexed journals in the Scopus database from its inception until April 2023. Eligible documents included those on USR describing experiences carried out by universities in Latin America and the Caribbean. The number of articles per author, average authors per article, average citations per article, and the number of documents with one or more author were described. Bibliometric indicators regarding authors per article, co-authors per article, and institutional collaboration were presented. Bibliometric networks were constructed based on bibliographic coupling analysis of documents by countries and term co-occurrence in titles and abstracts. Results: Of a total of 4075 documents retrieved from Scopus, 150 were included. Documents published between 1997 and 2023 were identified, with an average annual growth rate of 2.7%. A total of 439 authors were identified, 18 articles had a single author, and an average of 0.3 articles per author and a co-authorship index of 3.13 were found. The percentage of international collaborations was 30.7%. Brazil had the highest proportion of publications (26.4%), followed by Chile (17%) and Colombia (13.2%). Opción and Revista de Ciencias Sociales were the journals with the highest number of articles published (13 each). In the analysis of term co-occurrence, recent years showed an increase in the use of terms related to e-learning, information and communication technologies, virtual education, COVID-19, sustainable development goals, and URSULA (initiative on USR in institutions in Latin America and the Caribbean). Conclusions: A growth in scientific production on USR in Latin America and the Caribbean was identified. The interest in USR documents in recent years has been focused on COVID-19 and the challenges of virtual education and sustainable development.

Background: Acute pancreatitis (AP) has unpredictable severity. Its management is based on initial assessment of disease severity. It ranges from mild interstitial to severe necrotic form; the latter is associated with poor prognosis. Contrast-enhanced computed tomography (CT) of the abdomen is the gold standard in early detection of pancreatic necrosis and in assessing the severity of AP. Two CT grading systems exist to assess the severity of AP: CT severity Index (CSI) and modified CSI (MCSI). This study compares the usefulness of these two systems in predicting the severity and clinical outcome in AP in comparison with Ranson’s criteria and clinical outcome parameters. Methods: This is a prospective hospital-based screening study of 80 patients aged >12 years with clinical diagnosis of AP who underwent contrast-enhanced CT study of the abdomen. Comparative analysis between MCSI and CSI with Ranson’s criteria and clinical outcome parameters was assessed by Chi-Squared test. Results: The accuracy of CSI and MCSI in predicting the requirement of critical care, superadded infection, multiple organ dysfunction syndrome (MODS) and requirement of intervention were 73.0%, 64.5%, 69.8% 60.9% and 77.2%, 76.0%, 74.4% & 56.6% respectively. Area under the curve for MCSI score was significantly higher (AUC: 0.861; 95% CI: 0.736-0.986) than CSI score (AUC:0.815;95% CI:0.749-0.941). MCSI and CSI showed significant correlation with Ranson’s criteria; however, MCSI correlation was better (r:0.53; p Conclusion: CSI and MCSI are better predictors of severity, clinical outcome and mortality compared with Ranson’s criteria, with MCSI being more accurate and better predictor than CSI. The accuracy of MCSI is better than CSI for prediction of requirement of critical care, development of superadded infection and development of MODS in AP. However, CSI and MCSI have low accuracy in predicting intervention in AP.

Background This research conducted an extensive exploration in 2021 and 2022 to investigate the implications of integrating Chinese checkers into Comparative Politics courses across the Asia-Pacific region. The primary aim was to enhance students’ strategic thinking and negotiation skills and determine if active engagement with Chinese checkers could significantly improve academic performance. Methods The study employed paired-samples t-tests and independent-samples t-tests to evaluate the impact of Chinese checkers. It involved assessing the academic performance and strategic thinking abilities of students who engaged with the game. The study encompassed a thorough design, participant selection, and procedures related to the intervention, including various outcome measures. Results The investigation revealed notable improvements in academic performance, especially in strategic thinking, among students who actively participated in Chinese checkers gameplay. This research also substantiated the enhanced strategic thinking abilities of students who integrated Chinese checkers into their curriculum, contributing to the corpus of knowledge on innovative pedagogical practices. Conclusions The findings of this study are relevant to educators, students, and policymakers, emphasizing the educational value of unconventional teaching methods like Chinese checkers. In light of the increasing importance of the Asia-Pacific region in global politics, this research underscores the significance of nurturing strategic thinking skills among undergraduate students. The study contributes to the cultivation of critical thinking and strategic acumen within higher education, potentially benefiting society by fostering superior academic performance and the development of strategic thinking abilities.

Background Natural forest resources in northwestern Ethiopia are currently under threat, requiring immediate conservation measures and a management strategy. Thus, an understanding of the present floristic diversity status is required. There is lack of such type of studies specific to fragmented forest patches and across environmental factors. Consequently, this study aimed to examine the existing state of these resources to apply sustainable forest management practices. Methods To achieve this, transects with 200 square plots measuring 400 m 2 were set up at nine forest patches with 200m spacing between plots and transects. Vegetation and environmental data were collected and analyzed using R version 4.1. Results Significant variations (p 0.008) were found in vegetation features along the slope, forest patches, and agroclimatic zone. When compared to other forest patches, highland, and mid-altitude forest patches had the highest species diversity (2.48) and stocking (2578 trees/ha). The gentle slope has the most species diversity (2.83). The species similarity between highland and mid-altitude forest habitats was found significant (69%). The vegetation cover in the mid-altitude forest patches was also high (abundance: 5-12%). In all the forest patches examined, tree life forms exceeded shrubs, climbers, and herbs. Conclusions According to the study’s findings, the state of forest resources varies considerably across different environmental variations. Despite the impression of entire forest patches from the outside, the interiors are open, with only huge and mature trees covering the canopy. This state has been triggered by deforestation, degradation, and inappropriate human and grazing operations.

Aims To study the association between AIM2 gene polymorphisms and the tendency for periodontal infection and coronary heart disease, and to determine whether males or females are more susceptible to these diseases. Additionally, we examined its association with the features of periodontal disease. Methods 140 patients were enrolled in this study, and those who took part were divided into four groups as follows: healthy (c), periodontal disease (P), coronary heart disease with intact periodontium (AS-C), and coronary heart disease with periodontal disease (AS-P). Information on entrants, including age, sex, body mass index, and indicators of periodontal disease severity, was documented. Blood samples were collected, and AIM2 gene polymorphisms were evaluated by polymerase chain reaction test, gel phase, and sequences. Results Genetic analysis of AIM2 G/T (rs 2793845) revealed a high frequency of the (T) allele and (GT and TT) genotypes that were detected in the periodontal disease and coronary heart disease groups in males. The Hardy-Weinberg equilibrium of alleles and genotypes did not differ significantly between the study groups. Gene polymorphisms were also significantly correlated with indicators of periodontal disease severity. Conclusion High frequenting of (T) alleles and (GT, TT) genotypes in AIM2 single nucleotide polymorphisms (SNP) were associated with an increased tendency to develop periodontal disease and coronary heart disease. It can be supposed that it has a causative function in the pathophysiology of both disorders, and the validity of SNP as a potential genomic factor for the risk of both disorders in Iraqi males.

Background Selfies have been defined as “a photograph that one has taken of oneself, and are a continuing growing trend among Millennials. Narcissism refers to the personality trait of an extraordinary self-awareness and adoration. Studies have shown a close relationship between selfie-taking behaviour, narcissism and oral health. Hence, a study was conducted with aim of the influence of selfie-taking behavior and narcissism on oral health. Methods An analytical cross-sectional questionnaire study was designed using relevant literature for collating information on sociodemographic characteristics, frequency of dental visits, selfie-taking behavior and perceived oral health. Oral health examination was performed to assess decay and plaque. Narcissism was assessed using The Five-Factor Narcissism Inventory Short Form Results The study included a sample of 163 participants. The associations between selfies taken per day and perceived oral health was statistically significant (p value 0.047*). Binary logistic regression models were constructed, which were significant for perceived oral health and grandiose fantasy. Plaque Scores showed significant corelations between indifference, manipulativeness, thrill seeking and age. Step forward binary regression demonstrated a significant odds ratio for grandiose narcissism followed by selfie-taking behavior Conclusion This study revealed a correlation between perceived oral health and selfie-taking behaviour. Further studies are required to assess selfie-taking behaviour and oral health.

Background Anxiety has been reported to be one of the most common epidemics in recent years. The present study focused on understanding the association between early menarche and the prevalence of anxiety and anxiety symptoms among adult undergraduate students. Methods This was an observational, case-control study. The sample included 146 young female adults aged more than or equal to 18 years pursuing the Bachelor of Medicine and Bachelor of Surgery (MBBS) and Bachelor of Dental Sciences (BDS). Using an online questionnaire, participants were asked to recall and enter the age at which they attained menarche. We used the Generalized Anxiety Disorder 7- Item Questionnaire (GAD-7) to measure the severity of their present anxiety symptoms. Results The results showed a significant increase in anxiety symptoms in participants who had early menarche compared to those who did not have early menarche. The mean score on the GAD-7 Questionnaire for the cases was 9.93 and the control group was 6.89. The GAD-7 scores among the cases group were significantly higher in the GAD-7 scores than in the control group. Conclusions This study concluded that early menarche is associated with higher anxiety levels in young adults.

Background Staphylococcus aureus infections, including Methicillin-Resistant S. aureus (MRSA) and Methicillin-Sensitive S. aureus (MSSA), present significant challenges in healthcare due to rising antimicrobial resistance. This study evaluates the genetic basis of antibiotic resistance in S. aureus, focusing on key resistance-associated genes mecA and PVL. Methods A total of 568 clinical specimens were analyzed for the presence of S. aureus. Demographic data were collected to assess age-dependent prevalence. Antimicrobial susceptibility testing was conducted to evaluate resistance patterns. The prevalence of the mecA and PVL genes was determined using molecular techniques. Results S. aureus was identified in 37.9% of cases, with the highest prevalence (60-79 age group). All S. aureus isolates showed 100% resistance to penicillin. Multidrug-resistant (MDR) strains accounted for 84.19% of isolates, with a significant presence of extensively drug-resistant (XDR) strains. The mecA gene was prevalent in 82.79% of MRSA isolates, indicating its strong association with methicillin resistance. Additionally, 41.86% of all S. aureus isolates were positive for the PVL gene, highlighting its widespread distribution. Conclusions The high prevalence of mecA and PVL genes in S. aureus strains underscores the challenges in managing these infections. These findings emphasize the necessity for judicious antibiotic use and enhanced collaborative efforts to combat antimicrobial resistance. Understanding the genetic basis of resistance can inform more effective diagnostic, therapeutic, and preventive strategies, ultimately improving patient outcomes in S. aureus infections.

The finite-volume theory has shown to be numerically efficient and stable for topology optimization of continuum elastic structures. The significant features of this numerical technique are the local satisfaction of equilibrium equations and the employment of compatibility conditions along edges in a surface-averaged sense. These are essential properties to adequately mitigate some numerical instabilities in the gradient version of topology optimization algorithms, such as checkerboard, mesh dependence, and local minima issues. Several computational tools have been proposed for topology optimization employing analysis domains discretized with essential features for finite-element approaches. However, this is the first contribution to offer a platform to generate optimized topologies by employing a Matlab code based on the finite-volume theory for compliance minimization problems. The Top2DFVT provides a platform to perform 2D topology optimization of structures in Matlab, from domain initialization for structured meshes to data post-processing. This contribution represents a significant advancement over earlier publications on topology optimization based on the finite-volume theory, which needed more efficient computational tools. Moreover, the Top2DFVT algorithm incorporates SIMP and RAMP material interpolation schemes alongside sensitivity and density filtering techniques, culminating in a notably enhanced optimization tool. The application of this algorithm to various illustrative cases confirms its efficacy and underscores its potential for advancing the field of structural optimization.

Background: Optimum productivity of beef cattle is achieved with adequate feed supply, both in quality and quantity. Consumption of local feedstuff Neptunia plena L. Benth and Leersia hexandra Swartz as a ration by the animal subject is expected to promote cost efficiency and production, as well as provide essential nutrition needs. Therefore, this research aimed to evaluate dry matter digestibility (DMD), organic matter digestibility (OMD), N-NH 3 production, and volatile fatty acid (VFA) in beef cattle. Methods: Feed and rumen inoculum samples were prepared and analyzed for their proximate contents. There were five treatment groups based on the diet received by beef cattle. In vitro approaches were used to determine the DMD, OMD, N-NH3 production, and VFA in beef cattle. The research was conducted in the Laboratory of Feed Nutrient Science, Faculty of Animal Husbandry and Agriculture, Diponegoro University, Semarang. The data were analyzed using ANOVA at a significance level of 95%, and a Duncan Multiple Range Test. Results: The results showed that the highest DMD (P5 (56.47%), followed by T 4 (56.45%) and T 3 (55.90%). T 5=62.40% significantly (P4=61.95% and T 3=60.82%. This treatment had the highest N-NH 3 value, namely 5.02 mM, compared with T 3=4.55 mM, T 4=4.50 mM, T 2=4.22 mM, and T 1=3.99 mM. Furthermore, T 5 had the highest VFA (P4, T 3, T 2, and T 1 with the value of 150.5, 133.0, 130.5, 130.0, and 123.5 mM, respectively. Conclusions: The local feedstuff-based ration contributed to beef cattle production.

Background: Contributing factors for improved survival of human adipocytes mesenchymal stem cells (h-AMSCs) cultured through hypoxia preconditioning, in example apoptosis inhibition involving BCL2 and HSP27 expression, trigger signal expression (VEGF), SCF expression, OCT-4 expression, and CD44+ expression. The objective if this study was to explain the mechanism and role of hypoxic preconditioning and the optimal duration of hypoxic preconditioning exposure to improve survival of h-AMSCs. Methods: An experimental laboratory explorative study ( in vitro) with hypoxic preconditioning in h-AMSCs cultures. This research was conducted through four stages. First, isolation of h-AMSCs culture from adipose tissue of patients. Second, the characterization of h-AMSCs from adipose tissue by phenotype (flowcytometry) through CD44+, CD90+ and CD45-expression before being pre-conditioned for hypoxic treatment. Third, the hypoxic preconditioning in h-AMSCs culture ( in vitro) was performed with an oxygen concentration of 1% for 24, 48 and 72 hours. Fourth, observation of survival from h-AMSCs culture was tested on the role of CD44+, VEGF, SCF, OCT-4, BCL2, HSP27 with Flowcytometry and apoptotic inhibition by Tunnel Assay method. Results: The result of regression test showed that time difference had an effect on VEGF expression ( pβ=-0.482) and hypoxia condition also influenced VEGF expression ( pβ=0.774). The result of path analysis showed that SCF had effect on OCT-4 expression ( pβ=0.985). The regression test results showed that time effects on HSP27 expression ( pβ=0.398) and hypoxia precondition also affects HSP27 expression ( pβ=0.847). Pathway analysis showed that BCL2 expression inhibited apoptosis ( p=0.030; β=-0.442) and HSP27 expression also inhibited apoptosis ( pβ=-0.487). Conclusion: Hypoxic preconditioning of h-AMSC culture has proven to increase the expression of VEGF, SCF, OCT-4, and BCL2 and HSP27. This study demonstrated and explained the existence of a new mechanism of increased h-AMSC survival in cultures with hypoxic preconditioning (O2 1%) via VEGF, SCF, OCT-4, BCL2, and HSP 27.

Background The consensus in scientific literature is that each child undergoes a unique linguistic development path, albeit with shared developmental stages. Some children excel or lag behind their peers in language skills. Consequently, a key challenge in language acquisition research is pinpointing factors influencing individual differences in language development. Methods We observed children longitudinally from 3 to 24 months of life to explore early predictors of vocabulary size. Based on the productive vocabulary size of children at 24 months, 30 children met our sample selection criteria: 10 late talkers and 10 early talkers, and we compared them with 10 typical talkers. We evaluated interactive behaviors at 3, 6, 9 and 12 months, considering vocal production, gaze at mother’s face, and gestural production during mother-child interactions, and we considered mothers’ report of children’s actions and gestures and receptive-vocabulary size at 15 and 18 months. Results Results indicated early precursors of language outcome at 24 months identifiable as early as 3 months in vocal productions, 6 months for gaze at mother’s face and 12 months for gestural productions. Conclusions Our research highlights both theoretical and practical implications. Theoretically, identifying the early indicators of belonging to the group of late or early talkers underscores the significant role of this developmental period for future studies. On a practical note, our findings emphasize the crucial need for early investigations to identify predictors of vocabulary development before the typical age at which lexical delay is identified.

Background Studies have this reported the presence of death anxiety, ranging from near-death experiences to physically ill people, to a simple thought about death, as well as when it is associated with states of loneliness. The Templer’s Death Anxiety Scale (DAS), based on Eysenk’s incubation theory, has been adapted and validated in different contexts and is still insufficient in countries such as Peru. This study aimed to determine the psychometric properties of the DAS, its internal structure, and factorial invariance. Methods An online scale was used in 1248 Peruvians aged between 18 and 70 years (M= 27.37, SD= 11.29) from all regions. DAS was validated using exploratory factor analysis (EFA), confirmatory factor analysis (CFA), convergent validity, measurement invariance, and internal consistency (reliability). Results Psychometric properties were found with adequate values in its structure and validity based on the internal structure through exploratory factor analysis, where it was found that the components of the scale are interrelated and the data matrix is factorizable. model of three specific factors and a general factor is presented, which is consistent with the theory and has practical utility, revealing adequate statistical values that reflect acceptable levels of reliability. Finally, there was fairness according to sex and age group of the scale by means of factorial invariance analysis. Conclusions Our results indicate adequate psychometric properties and facilitate a better data collection process for future research.

Background The Rorschach test is one of the most popular tests used in clinical settings for psychopathology and personality assessment; however, there is still little evidence regarding the functional correlates of test responses. Functional magnetic resonance imaging (fMRI) allows for the investigation of biological correlates associated with different psychological functions. Among other applications, fMRI has been used to identify different networks that reflect brain activity in the resting state (rs-fMRI), that is, when an individual is not engaged in any specific task. Among these resting-state networks (RSN), the best-known and most studied are the Default Mode Network (DMN), along with other networks such as salience, frontoparietal, sensorimotor, temporo-parietal, visual, and cerebellar networks. Methods We used this type of analysis to obtain new evidence regarding Rorschach. This study aimed to analyze the functional brain architecture underlying Rorschach’s personality variables, administered according to Exner’s Comprehensive System (CS). In particular, the aims are: 1) to ascertain the existence of correlations between CS variables and activity of the RSN, and 2) to use these profiles of activity to develop a new data-driven clustering of the CS variables. Archival data from twenty-four non-clinical subjects were analyzed. Independent Component Analysis (ICA) and partial least squares regression (PLS) were used to analyze the fMRI data. Results The results showed specific associations with the given Rorschach variables (several of which could be grouped into higher-order latent factors) and activity of the main RSN. Moreover, the cluster analysis outlined important groupings of Rorschach variables, particularly regarding their clinical implications. Conclusions Our study could be comprised in the existing literature providing strong evidence about the neurobiological validity of the Rorschach test.

Background The tomato, Solanum lycopersicum L., is one of the most important horticultural crops that can be consumed fresh or after being processed worldwide. The tomato leaf miner (Tuta absoluta) is one of the most devastating pest to tomato plants due to its mine-feeding nature in the mesophyll tissue of the plant. Fungal entomopathogens can exist naturally in plants as an asymptote. This study aimed to detect the endophytic colonization of Beauveria bassiana and Metarhizium robertsii within tomato plants via artificial inoculation and their virulence effects on Tuta absoluta. Methods Isolates with the highest percent germination and virulence against T. absoluta were selected for endophytic evaluation within tomato plants by different artificial inoculation techniques. Results This study revealed that, isolates with the highest percent germination and virulent to Tuta absoluta had the potential to colonize tomato plants. The result showed that, the maximum mortality rate (97.5%) of Tuta absoluta larvae was achieved by Metarhizium robertsii isolate K-61 at a concentration of 1x10 8conidial/ml at 7 days post inoculated. However, the highest cumulative mortality (100%) was recorded by Beauveria bassiana isolate APPRC-27 at 10 days post inoculated through the direct contact method. The highest endophytic colonization was registered by isolate APPRC-27 (76.67%) at 7 days post-inoculated using the leaf spray technique, but it declined to 11.67% after 28 days of inoculated. In the case of the seedling inoculation technique, the highest endophytic colonization was obtained in the root tissues of tomatoes at 28 days of inoculated by isolate K-61. Conclusions This study revealed that the leaf spray inoculation technique was the most effective method, followed by seedling inoculation, for the deployment of Beauveria bassiana and Metarhizium robertsii endophytes in tomato plant tissues. Therefore, virulent Beauveria bassiana and Metarhizium robertsii, are promising bioagents for the control of Tuta absoluta if deployed as endophytes.

Background: First line (1L) TP-Ex-like regimen followed by 2nd-line (2L) immunotherapy represents one of the standards of care in platinum-sensitive recurrent and/or metastatic squamous cell carcinoma of head and neck (R/M SCCHN). We report our experience from 2 tertiary care institutions of India. Methods: This is a retrospective analysis of consecutive patients of platinum-sensitive R/M SCCHN treated with 1L weekly paclitaxel, carboplatin, and cetuximab (PCC) regimen followed by cetuximab maintenance (if non-progressive) or 2L nivolumab or oral metronomic chemotherapy (OMCT) on progression. Overall response rates (ORR), progression-free survival after 1L and 2L (PFS-1 & PFS-2), overall survival (OS), and safety were evaluated. Results: The study included 54 patients; median age 56.5 years; 89% men; 11% had cardiac dysfunction; 13% had renal dysfunction. After 1L PCC, ORR was 59.3%; median PFS-1 was 7.031 months; 61% had progression; 35% were treated with nivolumab and 18% with OMCT. The ORR was 26.3% (nivolumab) and 10% (OMCT). Median PFS-2 was 6.5 months (nivolumab) and 2 months (OMCT). The median OS was 15.01 months (entire cohort), 20.6 months (nivolumab), and 7 months (OMCT). Grade III/IV adverse events on PCC included neutropenia (31.4%), anaemia (35.1%), thrombocytopenia (7.4%), febrile neutropenia (11.1%), and skin reaction (16.6%); no Grade-III/IV treatment-related toxicities on 2L. Conclusions: 1L weekly PCC is an effective regimen for palliative therapy of platinum- sensitive R/MSCCHN with an acceptable toxicity profile. The addition of 2L nivolumab on progression further improves the outcomes.

* Background The level of liver fibrosis is the basis for the treatment of chronic hepatitis B (CHB), and it is necessary to adapt non-invasive liver fibrosis modalities. We aimed to investigate the use of M2BPGi as a single or combined diagnostic modality for liver fibrosis in CHB patients through a stepwise diagnostic analysis. Methods Cross-sectional data were taken from patients between October 2021 and August 2022. Demographic data, blood profile, liver function, and liver stiffness were measured in CHB patients over 18 years old, willing to take part in the research, and had complete data. APRI, FIB-4, and AAR were calculated using the well-known formulas. Serum M2BPGi-levels were converted into a cut-off index (COI). The patients were divided into low-risk (LR) and high-risk fibrosis (HR) groups. A cut-off for each predictor variable to differentiate between the LR and HR groups was determined. The obtained cut-off was assessed for its association with the grouping of liver elastography results. Models to diagnose the liver stiffness measurement (LSM) ≥8 kPa were created and compared through multivariate and ROC analyses. Results The number of patients that met the inclusion and exclusion criteria was 143 (HR = 65, LR = 78). The cut-off for diagnosing LSM ≥8kPa was 0.311, 0.742, 0.635, and 1.434 for APRI, FIB-4, AAR, and M2BPGi, respectively. This cut-off was significantly associated with the results of the HR and LR groupings. A multivariate analysis found that FIB4, AAR, and M2BPGi added significantly to the model. Statistically, the most optimal use of M2BPGi was combined with FIB-4, with an AUC of 0.835. Conclusions The optimal cut-off of M2BPGi for diagnosing high-risk liver fibrosis in this study was 1.434. M2BPGi should be used with FIB-4 as a diagnostic tool for diagnosing liver fibrosis, especially in the absence of a liver biopsy or elastography.

Background Currently there are 1.3 billion adolescents worldwide, which makes up 16% of the world population. Over 20% of adolescents around the world are thought to have behavioural or mental health issues. Addressing mental health issues is very important for the promotion of positive health in adolescents. This study aimed to estimate the prevalence of depression, anxiety and stress among adolescents in urban and rural areas of Shivamogga. Methods A cross-sectional study was conducted among 350 adolescents aged 16 to 19 years each from urban and rural areas of Shivamogga. Results Depression, anxiety and stress were found to be 23.1%, 29.4% and 26.6% in urban areas and 19.1%, 24% and 21.1% in rural areas respectively. Depression was more common among females in both urban and rural settings, with a significant difference observed in rural areas. Anxiety and stress were also more prevalent among females, with anxiety significantly higher in urban females. Urban adolescents exhibited higher levels of depression, anxiety, and stress compared to their rural counterparts. Conclusions About a quarter of the adolescent population suffers from depression anxiety and stress. Adopting and implementing better education and health policies are necessary to enhance adolescent mental health.

Background: The phylogenetic bootstrap, first proposed by Felsenstein in 1985, is a critically important statistical method in assessing the robusticity of phylogenetic datasets. Core to its concept was the use of pseudo sampling - assessing the data by generating new replicates derived from the initial dataset that was used to generate the phylogeny. In this way, phylogenetic support metrics could overcome the lack of perfect, infinite data. With infinite data, however, it is possible to sample smaller replicates directly from the data to obtain both the phylogeny and its statistical robusticity in the same analysis. Due to the growth of whole genome sequencing, the depth and breadth of our datasets have greatly expanded and are set to only expand further. With genome-scale datasets comprising thousands of genes, we can now obtain a proxy for infinite data. Accordingly, we can potentially abandon the notion of pseudo sampling and instead randomly sample small subsets of genes from the thousands of genes in our analyses. Methods: We introduce Scoutknife, a jackknife-style subsampling implementation that generates 100 datasets by randomly sampling a small number of genes from an initial large-gene dataset to jointly establish both a phylogenetic hypothesis and assess its robusticity. We assess its effectiveness by using 18 previously published datasets and 100 simulation studies. Results: We show that Scoutknife is conservative and informative as to conflicts and incongruence across the whole genome, without the need for subsampling based on traditional model selection criteria. Conclusions: Scoutknife reliably achieves comparable results to selecting the best genes on both real and simulation datasets, while being resistant to the potential biases caused by selecting for model fit. As the amount of genome data grows, it becomes an even more exciting option to assess the robusticity of phylogenetic hypotheses.

Background: Mycobacterial pathogens are among the top causes of diseases in humans. In Kenya, incidences of Non-Tuberculous Mycobacteria (NTM) species have steadily been on the increase. Most NTMare resistant to first line treatment of tuberculosis and have a challenge in timely and accurate diagnosis. Misdiagnosis has led to prescribing anti-tuberculosis regimens to patients suffering from NTM. We aimed to determine the most prevalent Non-Tuberculous Mycobacterium in Kenya. Methods: We reviewed records from the National Tuberculosis Reference Laboratory(NTRL ) Laboratory information management system (LIMS) between January 2018 and December 2019 for the patients on surveillance. All isolates were cultured in Mycobacterial Growth Indicator Tubes (MGIT) and incubated for detection using BACTEC™ MGIT™ system. Those with negative acid-fast bacilli (AFB) growth and negative for Mycobacterium Tuberculosis Complex Species (MTBC) protein-MPT64 were suggestive of NTM infections, which were sub-cultured in MGIT and characterized using Line Probe Assay (LPA) GenoType® MTBDR CM/AS. Descriptive and bivariate analysis was done. Results: Of the total 24,549 records reviewed, 167(0.7%) were NTM isolates. Males comprised of 74.2% (124/167), and the mean age was 42 years (SD±16), age group 35-44 years had the highest NTM at 26.3% (44/167). Nairobi had 12.6% (21/167), Mombasa 10.8% (18/167), Kilifi and Meru each had 7.8% (13/167). Eleven isolated species comprised of Mycobacterium intracellulare 35.3% (65/167), M. fortuitum at 27% (48/167), and M. scrofulaceum at 10.2% (17/167). Previously treated patients had higher NTM [63.5% (106/167)] than Drug-resistant follow-up patients [26.9% (45/167)]. Coinfection with HIV was at 27.5% (46/167). Conclusion: Previously treated patients should have an additional screening of NTMS, and drug susceptibility testing should be done before initiation of treatment.

Background PRAAN, a digital peak flow meter, was developed to measure peak expiratory flow rate (PEFR) for asthma monitoring. This study aimed to compare PRAAN’s accuracy to that of spirometry and the mini-Wright peak flow meter for PEFR measurement. Methods Two cross-sectional studies were conducted in 106 healthy adult participants. Study A (n=56) compared PRAAN with spirometry, while study B (n=50) compared PRAAN with the mini-Wright peak flow meter. PEFR values were collected using crossover studies. Pearson’s correlation and Bland-Altman plots were used to report the relationship and agreement between two measurements, respectively. Results In study A, 51.8% were female, mean age was 24.3±5.6 years, and PEFR was 480.3±86.8 L/min. PEFR measured by PRAAN had very strongly positive correlation with spirometry (r=0.980, P Conclusions The accuracy of PRAAN is in the agreement with spirometry and the mini-Wright peak flow meter. The clinical application of PRAAN may potentially lead to a monitoring strategy that healthcare providers can use to improve the management of asthma.

Background: Considering the importance of exploring the development of reasoning skills during preschool period and the suitability of using a culturally linguistically relevant story-based approach for the same, the present research intended to profile the reasoning skills in typically developing Indian preschool children between 36 and 72 months using a story-based approach. The specific objectives were to develop explanation, prediction, and inference based reasoning tasks around a story and assess the reasoning skills in typically developing Indian preschool children using the same. Method: Reasoning tasks across explanation, prediction, and inference domains were developed based on a story and evaluated for its psychometric properties. The developed tasks were then administered to 63 typically developing Indian preschool children attending English medium schools in Mangalore. The preschoolers were equally divided into three age groups, and the responses obtained across the age groups were analyzed quantitatively and qualitatively. Results: The developed tasks were confirmed to have good psychometric properties like content validity and reliability. The age comparisons of reasoning abilities using one-way ANOVA suggested an increase in reasoning abilities with age during the preschool period. The qualitative analysis further suggested that with increasing age, the nature of reasoning changed from content-based reasoning to reasoning based on prior knowledge which was integrated with the story content. Conclusion: The study describes reasoning skill development using a story-based task in Indian preschoolers. The study findings further provide clinical and educational implications to assess and foster reasoning abilities among preschoolers.

Background Recent evidence suggests that some flavonoid compounds obtained from crude methanol extract of mistletoe leaves ( Dendrophthoe pentandra L. Miq), also known as Benalu Duku (BD), have antimicrobial effects. Thus, the plant has the potential to eliminate viruses that may cause outbreaks in chicken farms. This study aimed to prove the in vitro ability of flavonoid compounds, namely quercetin-like compounds (QLCs), to eliminate field viruses, specifically the Newcastle disease virus (NDV). Methods This research was performed in two stages. An in vitro test was used with a post-test of the control groups designed at a significance of 0.05. BD leaves (5 kg) were extracted using a maceration method with methanol and then separated into hexane, chloroform, ethyl acetate, and methanol fractions. The final extracted products were separated using semi-preparative high-performance liquid chromatography (HPLC) to obtain QLCs. The QLCs were identified and compared with quercetin using HPLC, proton and carbon nuclear magnetic resonance spectrometry, Fourier transform infrared spectrophotometry and ultra-performance liquid chromatography-mass spectrometry. The activity of QLCs was tested in vitro against the NDV at a virulence titter of 10 −5 Tissue Culture Infectious Dose 50% (TCID50) in chicken kidney cell culture. Results Solutions of 0.05% (w/v) QLCs were discovered to have antiviral activity against NDVs, with an average cytopathogenic effect antigenicity at a 10 −5 dilution (p Conclusions QLCs from flavonoids from the leaves of BD have in vitro antiviral bioactivity against NDV at a virulence titter of 10-5 Tissue Culture Infectious Dose 50% (TCID50) in chicken kidney cell culture. QLCs may have the potential to be developed as medicinal compounds for the treatment of other human or animal viral infections.

Background This research investigated the dynamics of fee-free higher education in South Africa, aiming to elucidate the complexities surrounding its implementation and impact. By employing a causal loop diagram approach, the study examined the interplay of socioeconomic, political, and institutional factors influencing the provision of fee-free higher education. Method A participatory approach to developing the CLD was used. Through an extensive literature review, the research contextualised fee-free higher education enabling a preliminary CLD to be developed. Discussions were held to improve the preliminary model based on stakeholder and expert opinion. The model was then validated by stakeholders and experts. Results The CLD has explicitly mapped out the causal relationships which play a fundamental role in fee-free higher education in South Africa. Key findings revealed that fee-free higher education initiatives, such as managing funding constraints and administrative complexities, hold promise for fostering inclusivity and socioeconomic development but are hindered by bureaucratic policy establishments and inefficiencies. These challenges underscore the gap between policy formulation and implementation, highlighting the need for comprehensive reforms to streamline administrative processes and enhance financial sustainability within the higher education sector. Conclusion This research thoroughly examined the dynamics of fee-free higher education in South Africa, highlighting both the challenges and opportunities in its implementation. The study emphasized the need for systemic reforms to improve accessibility and effectiveness, while also underscoring the potential of such initiatives to promote social mobility and economic empowerment, aligning with global goals like the SDGs and the African Agenda 2063.

Background Primiparous mothers face diverse challenges during pregnancy and post-childbirth. There is a lack of comprehensive educational programs for primiparous mothers on maternal functioning and newborn care. This study aimed to explore the efficacy of a developed educational program on the attitude of primiparous mothers towards newborn communication, general health, feeding and swallowing. The objectives were (1) to develop an attitude questionnaire (AQ), a parent education program, and a feedback questionnaire (FQ); and (2) to estimate the efficacy of the education program pre- and post-delivery. Methods Ninety-eight primiparous mothers without any obstetric history, proficient in English or Kannada, and delivering healthy newborns were recruited for the study. Phase 1 involved the development and validation of AQ, the parent education program [Newborn Communication, Health, Feeding and Swallowing Education Program (N-CHFSEP)], and FQ; while Phase 2 comprised of administering them on the mothers. Both quantitative (descriptive statistics, paired t-test, and chi-square test) and qualitative analysis were done on the parameters of interest. Results The results of the study demonstrated a notable increase in the number of mothers (not all) reporting heightened confidence levels following receiving the N-CHFSEP (which was observed in all the domains). This observed change (pre and post) was statistically significant as per paired t-test analysis (p Conclusions This study demonstrates the effectiveness of N-CHFSEP in enhancing primiparous mothers' confidence in newborn care, thereby improving maternal and infant health.

Background Funding committees, comprising members with a range of knowledge, skills, and experience, are considered integral to the decision-making process of funding organisations for recommending or allocating research funding. However, there is limited research investigating the decision-making processes, the role of members and their social interactions during funding committee meetings conducted both virtually and face-to-face. Methods Using a mixed-methods design and following netnography principles, the study observed nine National Institute for Health and Care Research programmes funding committee meetings conducted virtually during October 2020 to December 2021; complemented by interviews with committee chairs and members (18 interviews) and NIHR staff (12 interviews); an online survey (50 responses); and documentary analysis. Personal reflections through immersive journals also formed part of the analysis. Results Three main themes were identified from the observations, interviews, and online survey: efficiency of virtual committee meetings (importance of preparation, and the role of formality, process, and structure); understanding the effect of virtual committee meetings on well-being (effects of fatigue and apprehension, and the importance of work life balance); understanding social interactions and engagement (levels of engagement, contribution and inclusivity, awareness of unconscious bias and the value of social networking). Conclusions Examining the decision-making practices of one funding organisation across several research programmes, across multiple committee meetings over one year has generated new insights around funding committee practices that previous studies have not been able to explore or investigate. Overall, it was observed that fair and transparent funding recommendations and outcomes can be achieved through virtual funding committees. However, whilst virtual funding committees have many benefits and opportunities, such as the potential to increase membership diversity and inclusivity, and be more environmentally sustainable, more evidence is needed to evaluate their effectiveness, with particular focus on issues of fatigue, engagement, and committee cohesion, especially when new committee members join.

Background The discovery of an ideal and effective therapy is urgently required for the treatment of Alzheimer’s disease. The main pathological hallmarks of Alzheimer’s disease that appear before the clinical symptoms are neurofibrillary tangles, amyloid plaques, brain inflammation, and neuronal atrophy throughout the cerebral cortex and hippocampus. GSK-3β (Glycogen Synthase Kinase-3β) is regarded as the most important and promising target for therapeutic use because GSK-3β expression levels increase with age and are the most abundant and hyperactive in the brains of patients with Alzheimer’s disease. Methods We used Maestro, which is Schrodinger, for our computational simulation studies. In the present work, we have used different modules that were used in previous studies with a little modification, the modules such as Protein Preparation with the help of Protein Preparation Wizard, Ligand Preparation with the help of LigPrep, for ADME (Absorption, Distribution, Metabolism and Excretion) prediction we used Qikprop, Docking studies we used Glide module, Binding energy prediction we used Prime and Molecular dynamic simulation studies by Desmond Results Our focus is mainly on an in-silico approach, focusing on library generation; we first drew an imidazo [1,5-a]pyridine-3-carboxamide (IMID 2) scaffold structure at Enamine and subjected it to a substructure search to target the receptor grid region (ATP-competitive site) of 6Y9R. They were then subjected to various screening processes. Finally, we selected nine compounds and subjected them to molecular dynamic simulation studies. Conclusions Nine compounds showed good results with the most stable interactions. Further experiments and studies are required to confirm these results.

Background Cardiac amyloidosis, characterized by the deposition of amyloid proteins in the heart tissue, presents in two main types: transthyretin (ATTR) and light-chain (AL) amyloidosis. The hemodynamic response to exercise and the relationship with pulmonary hypertension (PH) in these patients is not well understood. Methods This retrospective study analyzed 100 patients diagnosed with either ATTR or AL amyloidosis. We assessed the prevalence of PH at rest and its induction during exercise stress tests. Hemodynamic parameters were measured to identify differences in the cardiac response to exercise between the two subtypes. Results A higher prevalence of PH was noted in the ATTR group compared to the AL group. Exercise stress tests induced significant PH, particularly in the ATTR subgroup. Subtle yet clinically relevant hemodynamic differences were observed between the amyloidosis subtypes. Conclusions Our findings suggest that the amyloidosis subtype is an important factor in the management of PH. There is a need for tailored clinical approaches to address the distinct pathophysiological mechanisms in ATTR and AL amyloidosis. This study contributes to a better understanding of the hemodynamic changes during exercise in cardiac amyloidosis and underscores the importance of subtype-specific management strategies.

Background Because of its close relationship with vessels that supply the thyroid gland, it is necessary to understand and document the course of the laryngeal nerves to prevent accidental damage. Otherwise it can lead to weakness in vocal cord function if ligated along with the arteries during thyroidectomy or other surgical procedures. The aim of our study was to examine and document the origin, relationship, and branching patterns of the laryngeal nerves on the side of the neck. Method The present cross sectional anatomical study was conducted on 64 dissected necks of formalin-fixed cadavers that were utilized for undergraduate teaching purposes. Different parameters were measured in the neck along with the relationship between the laryngeal nerves and their accompanying arteries. Results We observed usual and unusual course of laryngeal nerves along with some variations. Some of the novel findings were an internal laryngeal nerve piercing the thyroid cartilage lamina and an external laryngeal nerve communicating with the superior cervical ganglion. Conclusion The present study observed anatomical variations in the course of the laryngeal nerves, which are relevant for clinical practice. The potential risks of damage to these laryngeal nerves during various surgical procedures make it necessary to review and compile its relationship with the surrounding structures.

Background COVID-19 has heavily impacted the financial and economic well-being of the world’s population. Various national relief programs and initiatives have been carried out to address society’s financial difficulties, especially the issue of poverty among the vulnerable, like refugees and immigrants. Consequently, being one of the main elements in Islamic social finance, waqf has been emphasised and proven throughout Islamic history as one of the practical tools for poverty reduction. However, recent waqf initiatives towards poverty reduction focused more on aiding the micro and small entrepreneurs, whilst limited waqf initiatives were available to help low-income households, refugees, and immigrants during this critical time in Malaysia. Methods This study introduces a novel waqf model, specifically designed to contribute to poverty reduction in society. It is tailored for low-income households, refugees, and immigrants, aiming to sustain their financial independence. By combining the elements of waqf and crowdfunding, this model proposes an innovative agriculture-based microfinance programme. It utilizes undeveloped waqf lands and equity-based crowdfunding to reach its objective of poverty reduction. This study has adopted a secondary research approach using the existing scholarly literature. Results This paper also discusses the potential of the proposed model to deliver more than its primary objective, as the framework illustrates that waqf institutions will have a continuous inflow of funds that will allow them to initiate other waqf-based projects, job creation through the microfinance programmes, and financial return for the investor of equity-based crowdfunding projects. Conclusion This study not only underscores the sustainability of the proposed model, but also its alignment with the increasing awareness of value-based and social impact objectives. It further explores the potential of waqf to be integrated with other financing instruments during the pandemic to help the vulnerable, specifically refugees and immigrants. This research thus contributes to the true purpose of Islamic social finance, highlighting the significance of its findings.

Background Education of ‘108' ambulance personnel involved in transporting neonates may improve outcomes. We assessed i) perceptions/practices of ‘108’ ambulance personnel for transporting neonates, ii) clinical parameters of transported neonates at arrival, and iii) outcomes such as survival/mortality and NICU stay (before and after skill-based educational intervention). Methods We conducted a single-arm intervention study (pre-and post) over 18 months. We assessed the perceptions and practices of 77 ambulance personnel on neonatal transport pre- versus post-intervention. Checklists assessed ambulance equipment availability/usage in both phases. We compared clinical parameters and outcomes of transported neonates between the pre-intervention (n=62) and post-intervention (n=53) phases. We analyzed data using SPSS version 25. Results Post-intervention, there was a significant reduction in the levels of hypothermia (p < 0.001), hypoglycemia (p=0.010), and prolonged capillary refill time (p=0.042), along with improvement in the use of intravenous fluids (p (p < 0.001) in the perceptions/practices of ambulance personnel towards neonatal transport. There was an improvement in the ambulance equipment availability/usage post-intervention. Conclusions The perceptions and practices of the ‘108’ ambulance towards transporting neonates had significantly improved post-educational intervention. Further, a significant decrease in hypothermia, hypoglycemia, and duration of NICU stay was seen in neonates transported post-intervention.

Background Determinant and characteristic polynomials are important concepts related to square matrices. Due to the absence of additive inverse in max-plus algebra, the determinant of a matrix over max-plus algebra can be represented by a permanent. In addition, there are several types of square matrices over max-plus algebra, including triangular and diagonal strictly double ℝ -astic matrices. A special formula has been devised to determine the permanent and characteristic max-polynomial of those matrices. Another algebraic structure that is isomorphic with max-plus algebra is min-plus algebra. Methods Min-plus algebra is the algebraic structure of triple ( ℝ ε ′ , ⊕ ′ , ⊗ ) . Furthermore, square matrices over min plus algebra are defined by the set of matrices sized n × n , the entries of which are the elements of ℝ ε ′ . Because these two algebraic structures are isomorphic, the permanent and characteristic min-polynomial can also be determined for each square matrix over min-plus algebra, as well as the types of matrices. Results In this paper, we find out the special formulas for determining the permanent and characteristic min-polynomial of the triangular matrix and the diagonal strictly double ℝ -astic matrix. Conclusions We show that the formula for determining the characteristic min-polynomial of the two matrices is the same, for each triangular matrix and strictly double ℝ -astic matrix A , χ A ( x ) = ⨁ r = 0 , 1 , … , n ′ δ n − r ⊗ n r .

Background and aim Hypertension (HTN) is a significant public health concern worldwide, affecting approximately a third of people during their lifetimes. There are many factors that influence people with HTN to develop cardiovascular disease, such as lifestyle, genetics, economics, overall health, and medications; the authors did not find any previous study employing Multi-Attribute Utility Theory (MAUT) analysis using machine learning. There is a sense of urgency to understand the characteristics of HTN patients in Pakistan; by using MAUT to analyze and evaluate different factors related to developing HTN, we can gain valuable insights into the characteristics of HTN and cardiovascular HTN (HTN-CVD) patients in Pakistan. Methods and results This study utilized a cross-sectional survey with 98 participants diagnosed with HTN and HTN-CVD. The questionnaire included sociodemographic items, symptoms, heredity factors, and dietary habits. The study was approved by the Research Ethical Committees of Pir Mehr Ali Shah Arid Agriculture University Rawalpindi (PMAS-AAUR), Pakistan. The result shows that there is a wide difference between HTN and HTN-CVD patients’ scores for symptoms (37% vs. 63%) and economic status (39% vs. 61%) (respectively). Other than these, the attribute proportions of scores for HTN and HTN-CVD are almost the same. The highest MAUT scores were higher for patients with HTN-CVD (373) than for those with HTN alone (270), and the mean age of HTN patients is higher than HTN-CVD; almost all attributes’ groups of the 10 highest MAUT scores of HTN-CVD higher than HTN patients except for attribute of medication status. Conclusion Patients with HTN-CVD complications have higher MAUT scores for lifestyle, social status, health status, and economic status. This emphasizes the importance of educating patients (and indeed the general public) about risks, symptoms, and adopting healthy behaviors.

Background The degree of intratumoral lipiodol staining after conventional transarterial chemoembolization (cTACE) has the potential to predict tumor responses and disease prognosis. This study is aimed at evaluating the correlation between the lipiodol retention of the tumor with a complete response after cTACE and tumor recurrence by quantitative assessment. Methods From January 2013 to September 2023, every patient receiving cTACE for HCC was recognized. Inclusion criteria were patients with ≤6 HCCs and sizes 1-6 cm, with at least one tumor obtaining a complete response after cTACE, and available baseline and follow-up CT studies. Tumoral, cTACE procedural, and lipiodol staining parameters were analyzed. Using univariate and multivariate analysis, significant factors associated with tumor recurrence were identified. ROC curve analysis was used to identify the optimal cutoff point for the statistically significant factors, predicting tumor recurrence. Results The final population included 39 patients with 63 HCCs. Tumor recurrence was detected in 18/63 (28.6%) at a mean of 27.8 months. On univariate analysis, the incidence of tumor recurrence significantly increased with increased tumor size (p = 0.007), an inhomogeneous lipiodol staining pattern (p Conclusions Inhomogeneous lipiodol staining pattern and lipiodol washout rate of >6.44 HU/month were predictors for recurrence of HCC after a complete response after cTACE. These correlations may provide useful guidance for subsequent imaging surveillance and treatment approaches.

Background This study aims to develop a vulnerability map for Surabaya using GIS-based Multi-Criteria Decision Analysis (MCDA) to assess the city’s vulnerability to COVID-19. Methods Six key factors influencing vulnerability were identified and their relative importance determined through the Analytic Hierarchy Process (AHP) pairwise comparison matrix. GIS was utilized to classify Surabaya’s vulnerability into five levels: very low, low, medium, high, and very high. Results The resulting vulnerability map provides essential insights for decision-makers, healthcare professionals, and disaster management teams. It enables strategic resource allocation, targeted interventions, and formulation of comprehensive response strategies tailored to specific needs of vulnerable districts. Conclusions Through these measures, Surabaya can enhance its resilience and preparedness, ensuring the well-being of its residents in the face of potential emergency outbreaks.

Background Removal of the microbial deposits (plaque and calculus) by performing effective scaling and root planing (SRP) is the fundamental step for managing periodontal disease (gingivitis and periodontitis). Various adjuncts in the form of mouthwash, gels, and toothpaste are also being used as adjuncts to SRP for managing periodontitis. Recently, Lycium barbarum ( L. barbarum), commonly known as goji berry or wolfberry, has gained popularity for managing chronic inflammatory and infectious diseases. However, its efficacy in managing periodontitis has never been explored. Hence the present study aims to evaluate the efficacy of goji berry mouthwash compared to chlorhexidine mouthwash for managing periodontitis. Methods 60 adult participants were divided randomly using computer-generated random sequences into two groups (case group: L. barbarum mouthwash (Males: 16; Female: 14); control group: 0.2% chlorhexidine gluconate mouthwash (Males: 14; Females: 16)). The changes in the gingival index (Gi), plaque index (Pi), bleeding on probing (BOP), clinical attachment loss (CAL), probing pocket depth (PPD), microbial load, and antioxidant levels (protein thiol) in saliva were noted at the baseline, at 15 days and one month. Results A significant reduction in the mean PPD, Pi, and Gi was seen from baseline to one month in both the control (P-value=0.006, 0.027, and 0.036 respectively) and test groups (P-value=0.035, 0.000, and 0.000 respectively). CAL was reduced significantly only in the control group. However, the antioxidant levels (protein thiol) in saliva were significantly increased only in the test group. Conclusion Goji berry mouthwash along with SRP reduced the Gi, Pi, BOP, and PPD in patients with periodontitis. However, no statistically significant difference was noted between the use of goji berry and chlorhexidine mouthwash. Goji berry mouthwash was more effective than chlorhexidine in increasing the antioxidant levels in saliva.

Background This study applied geographical standards (coverage distance) and demographic standards to investigate the capabilities of primary healthcare facilities to cover the largest area of the study area and the service area that can be accessed. This study was conducted to find out the sufficient number of primary healthcare (PHC) centers required to provide healthcare services to the entire community. Methods Data was obtained by applying geographic information system (GIS) techniques to analyze primary care facilities using the demographic and geographic standards for primary care facilities. Results PHC centers cover 79% of the study area according to the geographical standard. The study area needs 41 additional centers to cover the shortfall in service provision per the demographic and geographic standards. Conclusions A significant deficiency in the number of primary care centers found in the study area compared to the large population at the geographical and demographic standards level.

Background Diagnosis of tuberculosis (TB) in resource-limited countries relies primarily on bacteriological confirmation using Ziehl-Neelsen (ZN) stain or culture. However, this method has low sensitivity due to suboptimal sampling and techniques. Neutrophils, monocytes, and lymphocytes are crucial in the pathogenesis of granulomatous inflammation and immune reactions. We investigated the usefulness of the haematological parameters and their ratios, like the Neutrophil to Lymphocyte ratio (NLR) and Monocyte to Lymphocyte ratio (MLR), for diagnosing tuberculosis. Methods We retrospectively grouped 114 patients with fever into those diagnosed with TB and control groups. We obtained their haematological data and calculated their derived ratios. The ratios obtained from the two groups were compared. Their sensitivity and specificity were calculated. Results Haematological parameters like MLR were higher in TB patients than in the control group. Although NLR was not significantly increased, MLR was significantly increased with p values Conclusion Serum NLR and MLR emerge as valuable tools in TB diagnosis. Their simplicity and cost-effectiveness render them particularly suitable for screening and recurrence monitoring in rural and remote settings, thereby mitigating loss to follow-up.

Background The goal of eliminating tuberculosis has not yet shown a bright spot. Specifically for HIV patients (PLHIV), WHO has made it a special category as a pillar for eliminating tuberculosis. The tuberculosis screening has not yet reached the national target because it is carried out manually by health workers, requiring optimization following technological developments. This study aims to determine the effectiveness of tuberculosis case finding using a self-assessment paradigm in HIV patients. Methods This research was cross-sectional. The data obtained from HIV patients before and after using the E-TIBI application for 3 months each was analyzed using the compare mean independent t-test. Then the Chi-squared test was carried out on the E-TIBI screening characteristic variables. Result In total, there were 921 respondents with 148 (16%) presumptive TB. It was found that there was a significant difference (p Conclusion E-TIBI can increase the tuberculosis case finding in HIV patients so it has the potential to be implemented as a screening tool in the PLHIV community.

Background Astrometeorology is an ancient science, that deals the relationship between planet position and weather events. Several Indian studies proved that Astrometeorology could be a complementary method to improve numerical weather forecast accuracy. Since 2011, Tamil Nadu Agricultural University is conducting astrometeorological research and devised a novel concept “Planet Activeness Chart”. The principle is that “planets’ influence on a location’s weather varies throughout the day and may be negative, inactive, active, highly active and rule depending on their angle to that location”. Most existing astromet studies used planetary position to predict the occurrence of weather events (yes/no) but failed to capture intensity of the event. The “Planet Activeness Concept” could address this limitation and enhance forecast usability. Methods A study was carried out from 2018 to 2021 with six years data (2011-16) to verify the “Planet activeness” on hourly rainfall and windspeed events in Tamil Nadu. The frequency of planet activeness for a weather event was calculated by dividing the number of times a planet was in the selected activeness during a specific event category by the total number of events. Results The results indicated that negative state of the Sun, active status of the Saturn, Uranus, Venus and Moon were positively associated with rainfall intensity. The windy planet Mercury and Neptune at active state, the Sun and Saturn at rule state, Venus and Uranus at negative state, Jupiter at highly active state had significant influence on the increased wind speed. Conclusion Applying the planet activeness concept with azimuth could enhance the accuracy and usability of Astrometeorological forecasts. This study establishes a mathematical relationship between planet activeness and weather as a first step to understand the science behind this relationship. It is suggested to study different combination of planet activeness during a weather event for more insights.

Background As a post-COVID complication in children, multisystem inflammatory syndrome in children (MIS-C) is important because of its varied and life-threatening manifestations. With this background, this study attempts to focus on MIS-C cases in an underprivileged rural setting in north-eastern India, with most patients being treated with methylprednisolone rather than intravenous immunoglobulin due to financial constraints. Methods In this prospective longitudinal cohort study at MGM Medical College, 27 MIS-C cases diagnosed following WHO criteria were included. Laboratory and radiological investigations, including echocardiography, were performed as required for diagnosis and to assess prognosis. Most patients were treated with methylprednisolone. A follow-up assessment was done six weeks after discharge for any residual impairment. Results The most frequently affected age group was 5–10 years (59.28%), while respiratory (48.14%) and cardiac (40.74%) were the most commonly involved systems. Logistic regression studies established a significant association between serum ferritin level and prolonged hospital stay (coefficient 0.0674, p=0.0041), possibly due to greater complications in cases with high ferritin levels. Organ impairment was found to increase the need for inotrope use (coefficient 3.8797, p=0.00584). Most cases were treated with methylprednisolone alone (85.18%) with a favourable response and no death occurred. Conclusion The favourable response in cases treated with methylprednisolone only affirms the effectiveness of the drug as a cheaper alternative in a resource-poor setting. The study highlights that higher ferritin levels in complicated cases leads to prolonged hospitalisation and the increased need for inotropes in cases with organ impairment.

Background This study seeks to examine the causal link between financial deepening, national security, ecological footprint, and economic growth in Nigeria, in order to contribute to the contradicting evidence on economic growth drivers. Methods The dynamic ARDL model was employed as the baseline model to establish the link between the phenomena, and the FMOLS and DOLS as robust models using time series data spanning from 1995 to 2022. The FMOLS and DOLS models take into consideration the presence of endogeneity, cross-sectional dependency, and heterogeneity. Results The results demonstrate that whereas ecological footprint has positive and significant long-term relationships with economic growth, financial deepening and national security have negative and significant long-run relationships with economic growth. There was a 34% and 50% correction to the short-term to long-term adjustment speed, respectively. The findings of the Granger causality, interaction terms, marginal (ME), and threshold (TH) effects demonstrate: a) a significant bidirectional causal relationship between national security and economic growth, as well as a one-way significant causal link between financial deepening and economic growth; b) changes in national security and ecological footprint have a significant and negative impact on the financial deepening effect on economic growth; c) significant negative and marginal effects of national security on economic growth, as well as significant positive marginal effects of ecological footprints on economic growth; and d) the impact of financial deepening on economic growth becomes apparent when the increases in the marginal effect of the ecological footprint reach a critical limit over the ( TH CO 2 = 0.981) threshold. Conclusions This study highlights the significant link between Nigeria’s financial deepening, national security, ecological footprint, and economic growth. It recommends that viable policies be put in place to support the development of financial institutions, increasing security infrastructure investment, and promoting renewable energy use for sustainable environment and reduced carbon emissions.

Background Higher education teachers dwell on various classroom assessment practices. Their assessment practices play a significant part of the teaching learning process and in determining student achievement. In this respect, their different attributes determine the nature of their classroom assessment practices. This cross-sectional survey study aimed to examine differences in assessment practices among public university teachers with respect to their academic status, teaching experience, assessment training and specializations. Methods The study adopted Assessment Practices Inventory (API) which was distributed to a sample of 380 randomly selected teachers from four public universities. The majorities (93.4%) of questionnaires which were properly completed were entered to SPSS version 25 software and used for analysis. Multivariate analysis of variance (MANOVA) was used for analysis and Pillai’s Trace was utilized for interpretation of the results. Results The results showed that academic status (P= .000); teaching experience (P = .000); and assessment training (P =.030) had significant differences for the combined dependent variables (assessment practice). However, field of specialization (P =.130) did not show significant difference in the overall assessment practice although differences were observed in assessment design and interpretation. Conclusion Academic status, teaching experience, and assessment training are important attributes of teachers that shape their assessment practices. Therefore, as teachers hold the sole responsibility of classroom assessment, training and development courses in assessment, experience sharing and learning opportunities are necessary regardless of their specializations, academic status, and teaching experience.

Background Healthcare, like other industries, emphasizes performance, quality, and consumer experience while also attempting to reduce costs. However, high-quality healthcare remains paramount for vulnerable and ill patients. This study aimed to investigate parents' and caregivers' level of satisfaction with physiotherapy services provided to neuropediatric outpatients on the United Arab Emirates (UAE). Methods This descriptive cross-sectional study included 103 parents/caregivers of children with neurological disabilities that were randomly selected from different Emirates Health Services Hospitals in the UAE. Data was collected using the long-form Patient Satisfaction Questionnaire (PSQ-III). Results The overall mean satisfaction was 159±7.73 (out of 250 points). Communication (20.36/25), interpersonal factors (20.17/35), and doctor-patient time (20.17/35) had the highest mean satisfaction scores (8.06/10). The lowest mean satisfaction scores were for access/availability/convenience (34.60/60), technical quality (33.17/50), and economic elements (23.83/40). Conclusion Despite participants’ overall satisfaction scores being positive, some service domains require improvement to improve satisfaction, specifically the access/availability/convenience, technical quality, and economic elements. These areas should be prioritized by service providers and managers to improve patients’ experiences and clinical outcomes.
