4. Perspectives of Rehabilitation Professionals on Long COVID Interventions to Facilitate Return-to-Work.

- Abstract
Background. The severe functional impact of long COVID presents a significant challenge for clients seeking to return to work. Despite emerging clinical management guidelines, long COVID remains a concern in the rehabilitation field. There is a need to establish optimal practices for sustainable rehabilitation paths that enhance the recovery of clients with long COVID, all while understanding the challenges faced by rehabilitation professionals working with this population. Purpose. This study aimed to explore the perspectives of rehabilitation professionals intervening in long COVID rehabilitation with the goal of returning to work. Methods. A qualitative study was conducted involving online semi-structured interviews with rehabilitation professionals in Quebec from public and private sectors across various regions who had experience treating individuals with long COVID. Thematic analysis was employed for data analysis. Findings. Nine rehabilitation professionals participated in the study, yielding five themes: (a) reassessment of RTW goals; (b) education and self-management as primary interventions; (c) gradually reintegrating daily activities and life habits; (d) progression of interventions and dealing with post-exertional malaise (PEM); and (e) challenges in long COVID rehabilitation. Conclusion. Education, gradual activation and self-management appear as central components in supporting patient recovery, however, achieving return to work remains challenging without proper accommodations.

Coronary heart disease remains one of the leading causes of morbidity and mortality, and is responsible for significant social costs. Resumption of work is an essential objective when this pathology concerns working patients. French data remain patchy and relatively old. The French Society of Cardiology's Exercise, Rehabilitation, Sport and Prevention Group has proposed a multicentre study to update these data. Following an acute coronary syndrome (ACS), the cardiology team asked the patient, who was currently working, to complete a questionnaire on his or her pathology, occupation and plans to return to work. An interview after 6 months enabled the clinical and professional situation of the patient to be analyzed, in order to study the factors predictive of a return to work. 364 patients were included in 6 interventional and 17 cardiac rehabilitation centres between 2018 and 2019. The resumption rate was 81% (n = 295), 93% of them in the same position, with a mean delay of 106 ± 56 days. The cardiologic independent factors for non-return were left ventricular ejection fraction, the presence of an anticoagulant, angina or heart failure, and occupational factors, shift work, exposure to cold, and imposed work rates. Factors that lengthened the time taken to return to work included delayed access to rehabilitation, the carrying of heavy loads, difficult postures and imposed work rates, as well as the patient's lack of a project, the absence of a cardiologist's opinion and the request for a modified workstation. The rate of return to work remains fairly stable despite the evolution of disease management, and the time to return to work relatively high. One way of improving the situation is to enhance access to cardiac rehabilitation programs, for example by offering alternatives such as tele-rehabilitation for a proportion of patients. This will free up more time for more severe patients, to better prepare them physically and psychologically for a return to work, which will also have a beneficial economic effect. Introduction: The growing number of people diagnosed with a cancer of working age prompts us to explore the sustainability of their working conditions and avenues of intervention in their work situations to promote sustainable job retention in a context of work intensification., Methods: Narrative review of the literature on people diagnosed with a cancer, their work situation, participation in the labour market, return to work (RTW) and sustainable employment., Results: Transformations in the world of work are increasing the unpredictability and variability of work situations, making strategies for managing functional deficiencies after diagnosis of the disease more complex. Reinforcing socio-technical and organizational leeways (STOL), i.e. self-regulation capacities to cope with the demands of the work situation by deploying effective and healthy work strategies, implies optimizing the (external) resources available at the level of the work situation through ergonomic actions and/or improving individual (internal) resources through supportive care or rehabilitation and/or vocational training., Discussion: Interventions to help people diagnosed with a cancer to stay at work in the long term must give primacy to the accommodation of the work situation and identify the ergonomic levers for improving their STOL., (Copyright © 2024 Société Française du Cancer. La disponibilité d’évidences scientifiques sur les Accidents de la Route (AR) est essentielle à la mise en place des politiques de sécurité routière pertinentes, efficaces et adaptées au contexte des pays en développement. Ce travail a visé trois objectifs essentiels :celui de décrire les caractéristiques des accidents de la route au Bénin, ensuite, identifier leurs facteurs de risque et de gravité et enfin, identifier les facteurs associés aux conséquences chez les victimes d’accident de la route au Bénin. Il permettra de mettre à la disposition des décideurs des données probantes, scientifiquement objectives et susceptibles d’orienter les prises de décisions en matière de sécurité routière. Afin d’atteindre les objectifs fixés, nous avons mis en place et suivi entre juillet 2019 et janvier 2021, une cohorte de 1871 victimes d’accidents de la route qui a servi de base à six (06) études présentées dans ce travail. De plus une base nationale de données des accidents de la route, provenant des constats policiers réalisés entre 2008 et 2017, a été exploitée pour la réalisation de deux (02) autres études relatives aux facteurs environnementaux et géographiques. Les résultats de ces études montrent que :o Les victimes d’accidents de la route au Bénin étaient en majorité des jeunes âgés de 15 à 44 ans (67,1%), de sexe masculin (72,7%) et avaient un emploi au moment de l’accident (71,1%) ;o La prévalence du port de protection était de 57,0% et environ 11,3% des victimes de la cohorte utilisaient leur téléphone au moment de l'accident ;o Les accidents étaient plus fréquents dans la journée entre 07 heures et 19 heures (67,0%) ;o Les principaux types d’usagers de la route impliqués dans les accidents étaient des motocyclistes (75,3%) suivis des piétons (20,1%) ;o La proportion de blessés graves parmi les victimes de la cohorte était de 19,9% ;o Les proportions de victimes suivies à 6 et 12 mois étaient respectivement de 73,9% et 62,3% o L’analyse réalisée grâce aux Systèmes, The availability of scientific evidence on road crashes is essential for the implementation of relevant and effective road safety policies adapted to the context of developing countries. This study has three main objectives: to describe the characteristics of road crashes in Benin, to identify their risk and severity factors and, finally, to identify the factors associated with the consequences of road crashes in Benin. It will provide decision-makers with scientifically objective evidence to guide decision-making on road safety. In order to achieve the set objectives, we have set up and followed up between July 2019 and January 2021, a cohort of 1871 road crash victims which served as a basis for six (06) studies presented in this work. In addition, a national data base of road crashes recorded by the police from 2008 to 2017 was used for two (02) other studies on environmental and geographical factors. Introduction : Les troubles mentaux courants (TMC) sont l’une des causes principales d’incapacité au travail. Lorsque les travailleuses et les travailleurs âgés de 50 ans et plus s’absentent du travail en raison des TMC, ils sont à risque élevé d’une incapacité prolongée. Bien que l’importance de l’environnement de travail sur l’incapacité soit reconnue, les facteurs liés à ce système entravant le retour au travail des personnes de 50 ans et plus demeurent méconnus. Cette étude visait à décrire les obstacles associés à l’environnement de travail chez des travailleuses et travailleurs âgés de 50 ans et plus s’étant absentés du travail en raison des TMC. Les objectifs spécifiques étaient 1) de caractériser les obstacles de l’environnement de travail les plus fréquents chez ces travailleuses et travailleurs et 2) d’explorer auprès d’actrices et d’acteurs clés de l’environnement de travail leur compréhension des principaux obstacles au retour au travail chez cette population. Méthode : Un devis mixte séquentiel explicatif a été utilisé. La phase quantitative a consisté en l’analyse secondaire d'une base de données. Des analyses descriptives ont été réalisées pour identifier les principaux obstacles de l'environnement de travail des travailleuses et travailleurs âgés de 50 ans et plus. La phase qualitative a consisté en des entretiens portant sur ces principaux obstacles avec des superviseurs et superviseures ainsi que du personnel des ressources humaines d'un Centre Intégré de Santé et de Services Sociaux. Une analyse par questionnement analytique a été réalisée. Résultats : Le premier échantillon était composé de 44 personnes: 24 femmes, 20 hommes, âge moyen de 55 ans (écart-type 3,4). Les cinq principaux obstacles liés à l'environnement de travail sont : conflits interpersonnels (39%), craintes dues aux événements négatifs antérieurs à l'arrêt maladie (34%), perception par le travailleur d'une surcharge de travail (25%), atmosphère compétitive et exigences de performa, Abstact: Purpose: Common mental disorders (CMDs) are one of the leading causes of work disability. When workers aged 50 and over are absent from work due to CMDs, they are at high risk for prolonged disability. Although the importance of the work environment on disability is recognized, the factors related to this system impeding the return to work of workers 50 years and older remain poorly understood. The purpose of this study was to describe the work environment-related obstacles for workers aged 50 years and over on sick leave due to CMDs. The specific objectives were 1) to characterize the most common work environment obstacles among these workers and 2) to explore with key stakeholders from the work environment their understanding of the main obstacles to the return to work for this population. Methods: A mixed-methods sequential explanatory design was used. The quantitative phase consisted of a secondary analysis of a database. Descriptive analyses were conducted to highlight the main work environment obstacles of workers aged 50 and over. The qualitative phase consisted of semi-structured interviews addressing these main obstacles with management and human resources personnel from an Integrated Health and Social Services Centre. Analytical questioning analysis was performed. Results: The first sample consisted of 44 individuals: 24 women, 20 men, mean age 55 years (SD 3,4). The five main work environment obstacles were: interpersonal conflicts (39%), overload (25%), competitive atmosphere and high performance perception of little recognition from his organization (23%). The interviews revealed that three of the five obstacles manifest themselves differently for these workers, are interrelated, and seem to act as an incentive of early retirement. 20. Employer‐oriented labour market policies in Sweden: Creating jobs and the division of labour in the public sector.

In many European countries, greater importance is accorded to labour market policies in which employers are involved in activating unemployed people. Such employer‐oriented policies target employers' demand for labour and attempt to influence their willingness to hire, train or guide (often disadvantaged) unemployed groups. Using data from a qualitative interview study of an employer‐oriented programme in a medium‐size city in Sweden, the present article aims to develop knowledge about how these policies are used to influence employers to hire unemployed workers and how jobs created in this context differ from regular jobs. The article argues that creating jobs through new arrangements for the division of labour, with the promise of relieving regular staff of unskilled tasks, may influence employers' willingness to hire the unemployed when used alongside other kinds of policy instruments. However, the article also shows that this new division of labour, with programme participants performing mainly unskilled tasks, has been difficult to realize, as new staff gradually come to perform an increasing number of regular working tasks. Le milieu de travail est un déterminant essentiel de la santé de ses acteurs (p.ex., troubles musculosquelettiques, dépression, stress, épuisement professionnel). Le personnel de sécurité publique, par son rôle et sa proximité avec le public, est plus à risque d’être touché par des problématiques de santé mentale. La présente revue systématique aborde la question de l’effectivité des procédures organisationnelles de retour au travail du personnel de sécurité publique, à la suite d’une absence pour une problématique de santé mentale. Afin d’y répondre, une recherche exhaustive a répertorié l’ensemble des études présentes dans la littérature scientifique de 1990 à 2019. Trois études ont été retenues selon les critères d’inclusion et l’évaluation de leur qualité. Les procédures efficaces recensées démontrent la priorité de la communication et de l’inclusion de tous les acteurs d’une organisation dans le processus de retour au travail durable; l’enjeu du suivi exemplaire de la procédure établie en amont, par les membres de l’organisation, tels les médecins du travail ou les gestionnaires; et enfin, l’impact d’une meilleure confiance et connaissance de son rôle et des problématiques de santé mentale, sur l’efficacité du gestionnaire ou du médecin du travail, dans la procédure de retour au travail. La principale conclusion de cette revue systématique est que les procédures actuelles ont un effet significatif limité sur le retour au travail durable des travailleurs de ces milieux. Des constats et des recommandations sont avancés., The workplace is a key determining factor in personnel health issues (e.g., musculoskeletal problems, depression, stress, burnout). Because of their duties, public safety personnel have an increased risk of developing mental health problems compared to the general working population. The following systematic review addresses the issue of the effectiveness of organizational procedures for public safety personnel resuming work, following an absence due to a mental health problem. An exhaustive search was conducted to gather the studies in the scientific literature from 1990 to 2019. Three studies were selected based on inclusion criteria and quality assessment. The effective procedures found indicate the following: 1) communication and inclusion is a priority for all the personnel in an organization in the long-term work resumption process; 2) the importance of exemplary follow-up of the procedure established upstream, by members of the organization such as occupational physicians or managers; 3) the impact of better trust; and 4) the impact of knowledge of mental health issues and of one’s role on the effectiveness of the manager or occupational physician in the work resumption process. The main conclusion of this systematic review is that current procedures have a limited significant effect on worker long-term work resumption in these workplaces. Findings and recommendations are given.

The Swedish Social Insurance Agency (Försäkringskassan – SSIA) and its frontline staff have a key role in the implementation of activation policy. Drawing on ethnographic research conducted at local offices, this article investigates how the transparency ideal, as an integral part of the organizational governance of the activation policy, is negotiated and enacted in the everyday life of a welfare bureaucracy. The analysis shows the central role that the transparency ideal plays in the alignment of frontline staff with the normative regime of the agency. While the transparency ideal is central to the internal organizational life of the SSIA, the analysis shows how transparency is much less salient in relation to clients and other relations with the outside world. Although policy‐makers and scholars have directed increasing attention towards collaborative innovation and knowledge development between frontline agencies and workers and other stakeholders such as citizens and researchers, empirical research has not focused on the (varying) assessment of collaborators regarding what knowledge is "appropriate" to develop. In this article, we examine such knowledge assessments by drawing on a comparative case study of two local innovation projects conducted by the Norwegian Labour and Welfare Administration (NAV) in a four‐year service innovation programme. Although they responded to the same call, the projects involved development of two very distinct types of knowledge; one dealt with practice‐based knowledge and the other with evidence‐based knowledge. We show that whereas the former knowledge type was contested and difficult to transform into practice, the latter prompted few (if any) challenges and was implemented on a relatively large scale. These two projects point to the possible existence of a hierarchy of knowledge in labour and welfare services, where evidence‐based forms of knowledge and methods are regarded as more legitimate and appropriate than forms of knowledge placed "lower" in the hierarchy. We discuss the reasons for and implications of this apparent hierarchy of knowledge for frontline labour and welfare services. [ABSTRACT FROM AUTHOR]

This themed issue contributes to European research on the role of front‐line work in the implementation of welfare‐to‐work policies. A number of factors underline the relevance of such study. First, the focus on activating and disciplining the unemployed seen in many countries may on the surface look similar. However, a closer look at these policies and how they unfold in different contexts reveals many and interesting differences. While all contain a certain level of disciplining and coercive elements, they also to a varying degree contain elements that focus on the upgrading of skills, building human capital and providing other types of support in promoting labour‐market participation. In turn, these policies contain both people processing and people changing technologies that are used for different aspects of policy delivery. In addition, policy developments have gradually expanded the client group of these policies, including more hard‐to‐place unemployed, thus making the client group more heterogeneous. Finally, we have seen a strong political belief in the positive effects of using punitive sanctions. Research supports this belief when it comes to clients with high employability and limited problems besides unemployment, but the knowledge‐base is rather shaky when it comes to the hard‐to‐place clients with substantial problems. Using punitive sanctions or other disciplining or coercive measures in frontline work has caused controversy and resistance. In order to qualify our understanding of welfare‐to‐work policies, we need to take a step closer to where these policies are translated into reality for the target group. [ABSTRACT FROM AUTHOR]

26. Facteurs influençant le délai de retour au travail du personnel hospitalier après une infection à la Covid-19 durant la première vague épidémique

Objectif : le délai de retour au travail après une infection à la Covid-19 peut varier selon les individus et les circonstances. Cette étude descriptive vise à identifier les facteurs pouvant influencer le délai de retour au travail des agents hospitaliers ayant été contaminés par la Covid-19 dans un centre hospitalo-universitaire du sud de la France.Méthodes : les données sociodémographiques et professionnelles, la symptomatologie, les comorbidités, la prise en charge médicale et les thérapeutiques reçues ont été renseignées par un questionnaire soumis entre le 16 mars et le 1er juin 2020 aux 170 sujets inclus. Les résultats des tests RT-PCR au Sars-Cov2 réalisés par le laboratoire de virologie de l'hôpital universitaire ont été transmis au service de santé au travail investigateur de l’étude.Résultats : le délai moyen de retour au travail des 170 agents hospitaliers ayant répondu (taux de réponse : 100%) était de 15.6 jours. Le délai moyen de négativation du test RT-PCR après l’infection était de 9,4 jours.Au moment de leur dépistage, la plupart (79,29%) des agents hospitaliers étaient symptomatiques et présentaient principalement de la fatigue (64,2%), des myalgies (52,2%), une anosmie (52,2%) et des céphalées (50%). Au moment du retour au travail, 60,48 % des patients étaient symptomatiques avec la fatigue (53,5 %) comme symptôme majoritairement persistant. L'analyse multivariée montre une association positive significative entre le délai de retour au travail et le délai de négativation du test RT-PCR (p=0,024). Un délai de retour au travail plus court a été observé chez le personnel hospitalier travaillant en laboratoire (p=0,019) ou aux urgences (p=0,020). Un délai de retour au travail plus long a été mis en évidence chez les salariés âgés de 40 à 49 ans (p=0,045), présentant un risque de forme sévère de la maladie (p

Introduction Les troubles mentaux courants (TMC) représentent de 30 % à 50 % des absences maladies toutes confondues. Le succès du retour au travail (RAT) n’est pas dû au seul fait de l’individu, mais plutôt au résultat de l’interaction entre les acteurs impliqués dans le processus. De nombreuses applications en santé mentale ont été développées pour améliorer la prise en charge des patients et optimiser la communication entre les professionnels, sans toutefois être validées. De plus, aucune solution technologique n’a été développée à ce jour pour faciliter à la fois la concertation des acteurs du RAT (p. ex. gestionnaire, professionnels de la santé) et l’accompagnement systématique de l’employé dans son RAT.Objectif Pour combler ces lacunes, l’objectif de cet article est double : 1) décrire le développement de l’application PRATICAdr, Plateforme de Retour Au Travail axée sur les Interactions et la Communication entre les Acteurs : un programme Durable favorisant le Rétablissement ; 2) documenter les tests utilisateurs de l’application PRATICAdr.Méthode Le développement de PRATICAdr se décline en 3 phases : 1) l’analyse des besoins ; 2) la conceptualisation des mécanismes internes à l’application et des techniques de programmation ; 3) le test de l’application en situation réelle. L’application est évaluée par des questionnaires et entrevues pour mesurer la satisfaction des utilisateurs.Résultats PRATICAdr permet de suivre en temps réel le parcours des acteurs impliqués dans l’accompagnement personnalisé de l’employé dans son RAT. La schématisation du processus de RAT et l’inclusion d’outils d’évaluation validés systématisent la concertation et la prise de décision partagée des acteurs, ainsi que le suivi et les actions posées en vue d’entreprendre un RAT favorisant le rétablissement. L’interface de PRATICAdr a été développée pour simplifier l’expérience utilisateur de l’employé en absence maladie et des acteurs du RAT. Les résultats de satisfaction des premiers utilisateurs de PRATICAdr, 16 employés d’une grande organisation dans le domaine de la santé en processus de RAT à la suite d’un TMC, apprécient (moyenne > 9/10) non seulement la Plateforme Web, mais également la participation des acteurs du RAT et les questionnaires inclus dans PRATICAdr. Des éléments d’amélioration sont aussi proposés.Conclusion PRATICAdr est implantée dans 2 grandes organisations (> 15 000 employés) afin d’évaluer son efficacité auprès d’employés en absence maladie en raison d’un TMC et en processus de RAT. Dans le cadre de cet article, l’objectif était de présenter non seulement le développement de PRATICAdr, mais aussi de mesurer la satisfaction des utilisateurs. Les premiers résultats indiquent une appréciation élevée chez les employés en absence maladie, utilisateurs de PRATICAdr. En termes de pistes futures, l’intégration de l’apprentissage automatique sera abordée dans l’objectif de personnaliser le programme de RAT selon les prédictions de durée d’absence maladie et de RAT durable., Introduction Common mental disorders (CMDs) account for 30% to 50% of all illness absences. The success of RTW is not only due to the individual but rather to the result of the interaction between the stakeholders involved in the RTW process. Numerous mental health applications have been developed to improve patient management and optimize communication between professionals, but have not been validated. Moreover, no technological solution has been developed to date to facilitate both consultation among the RTW stakeholders (e.g., managers, health professionals) and systematic support for the employee in his or her RTW.Objective To address these shortcomings, the purpose of this article is twofold: 1) to describe the development of the PRATICAdr application (Return-to-Work Platform focused on Stakeholder Interaction and Communication: a Sustainable Recovery Program) and 2) to document PRATICAdr application user testing.Method The development of PRATICAdr has been operationalized in three phases: 1) needs assessment, 2) conceptualization of the internal mechanisms of the application and programming techniques and 3) testing of the application in real situation. The application is evaluated through questionnaires and interviews to measure user satisfaction.Results PRATICAdr allows to follow in real time the path of RTW stakeholders involved in the personalized support of the employee in his RTW. The operationalization of the RTW process and the inclusion of validated assessment tools help systematize the stakeholders’ consultation and shared decision-making, as well as the monitoring and actions taken to undertake a recovery-promoting RTW. The PRATICAdr interface was developed to simplify the user experience for the employee on sick leave and all RTW stakeholders. Regarding user satisfaction, results show that the first 16 users of PRATICAdr, employees in a large healthcare organization returning to work following a CMD, were very satisfied (average>9/10) with the Web application, as well as the participation of RTW stakeholders and the questionnaires included in PRATICAdr. Improvements were also suggested.Conclusion PRATICAdr is implemented in two large organizations (>15,000 employees) in order to evaluate its effectiveness with employees on sick leave due to CMD registered in a RTW process. The aim of this article was to present not only the development of PRATICAdr, but also to measure user satisfaction. Preliminary results indicate a high level of satisfaction among employees on sick leave who used PRATICAdr. In terms of future avenues, the integration of e-learning will be addressed with the objective of customizing the RTW program according to the predictions of duration of sick leave and sustainable RTW.

Background: Common low back pain is often associated with significant psychosocial factors, particularly when it reaches the chronic stage. At that point, symptoms become more persistent and are responsible for a large majority of the socioeconomic costs. Consequently, it seems essential to intervene as early as possible in the evolution of low back pain. Biopsychosocial management leads to the development of multidisciplinary programs that bring together different health care professionals. Objectives: To assess the potential benefits of multidisciplinary management based on the biopsychosocial model in reducing pain, improving functional capacities, reintegrating patients into the workplace, and addressing psychosocial behaviours in patients with subacute low back pain of 3 to 12 weeks' duration.Methods: Four scientific databases (PubMed, Cochrane, PEDro, Kinedoc) were searched until November 2021. Results: Five randomized clinical trials meeting the inclusion and exclusion criteria were analysed. The results show that multidisciplinary treatment significantly improves pain symptoms and functional abilities in the long term. The results in favor of a multidisciplinary program remain uncertain concerning work reintegration, kinesiophobia, anxiety and depression. No effect was found on quality of life and on the fear and beliefs. Conclusion: Multidisciplinary programs provide physical, psychological and social benefits for patients with common subacute low back pain. The integration of a professional alliance is fully consistent with the evolution.; Contexte : la lombalgie commune est souvent associée à de lourds facteurs psychosociaux, en particulier lorsqu'elle atteint le stade de chronicité. Dès lors, les symptômes deviennent plus persistants et sont responsables d’une grande majorité des coûts socioéconomiques. Par conséquent, il semble primordial d'intervenir le plus précocement possible dans l’évolution de la lombalgie. La prise en charge biopsychosociale conduit au développement de programmes multidisciplinaires alliant différents acteurs de la santé. Objectifs : évaluer quel pourrait être l’intérêt d’une prise en charge multidisciplinaire centrée sur le modèle biopsychosocial dans la diminution des douleurs, l’amélioration des capacités fonctionnelles, la réinsertion professionnelle et face aux comportements psychosociaux pour les patients souffrant de lombalgie subaiguë commune de 3 à 12 semaines d’évolution.Méthodologie : quatre bases de données scientifiques (PubMed, Cochrane, PEDro, Kinédoc) ont été explorées jusqu’en Novembre 2021. Résultats : cinq essais cliniques randomisés répondant aux critères d’inclusion et d’exclusion imposés ont été analysés. Les résultats rapportent qu’une prise en charge multidisciplinaire améliorerait durablement et significativement les symptômes douloureux et les capacités fonctionnelles. Les résultats en faveur d’un programme multidisciplinaire restent incertains concernant la réinsertion au travail, la kinésiophobie, l’anxiété et la dépression. Aucun effet n’est retrouvé sur la qualité de vie ainsi que sur les peurs et les croyances.Conclusion : les programmes multidisciplinaires apportent des bienfaits physiques, psychologiques et sociaux pour les patients souffrant de lombalgie subaiguë commune. L’intégration d’une alliance professionnelle est totalement en accord avec l’évolution de la pratique professionnelle.

Le but de cette étude est d’explorer l’expérience de transition vers le retour au travail de survivantes du cancer du sein de moins de 50 ans. Huit survivantes ont participé à cette étude qualitative exploratoire. Les résultats de l’analyse de contenu itérative indiquent que ces survivantes veulent délaisser leur vie teintée par la maladie et réintégrer une vie « normale » incluant le retour au travail. La transition vers le retour au travail est aussi influencée par plusieurs facteurs d’ordre personnel, familial et sociétal qui peuvent faciliter ou entraver l’expérience. Enfin, plusieurs défis distincts et la présence d’une vulnérabilité plus marquée doivent être considérés par les professionnels de la santé pour mieux accompagner ces survivantes durant la transition vers le retour au travail.

The objective of this study was to provide a french translation and adaptation of the \"Return to Work Self-Efficacy’ scale - 11 items\" (RTWSE-11), validated in dutch language in its original version. METHODS: After translation-back translation steps, completed by experts’ consensus meetings, interviews were conducted with thirteen patients diagnosed with cancer in order to evaluate the degree of clarity, simplicity and ambiguity or the various elements of the french version of the RTW-SE-11. RESULTS: The main modifications inherent to the french adaptation of the questionnaire concerned the modalities of the Likert scale and the inversion of three negative items into positive items. DISCUSSION: The french translation and adaptation of the RTWSE-11 was particularly faithful to the semantic, idiomatic, functional, experiential, conceptual and operational aspects of the original version. Future work can therefore focus on the psychometrics evaluations of the questionnaire. However, this tool can already be used in clinical practice to establish an initial assessment of the ability of patients diagnosed with cancer to return to work.

The policies and practices of workers’ compensation have barely kept pace with the changing worker and employer needs created by the growth of precarious forms of employment. This study focused on how well workers’ compensation and RTW policies in Ontario fit the needs of precariously employed workers. A critical discourse analysis guided our study which consisted of in-depth interviews with 15 precariously-employed workers and 5 employers who had hired and managed these kinds of workers. Three domains where RTW policies fit uneasily with the experiences of precariously-employed workers were identified. These related to knowledge and power contrasts between well-informed employers and vulnerable workers, injury attribution challenges, and worker fear of speaking up about accidents. This study suggests that workers’ compensation and RTW policies rest uneasily with the circumstances of precariously-employed workers. In particular, it was difficult for workers to engage with/make a claim for workers’ compensation when employers resisted this process. Les politiques et pratiques concernant l’indemnisation des victimes de lésions professionnelles n’ont pas suivi le rythme des besoins changeants des travailleurs et employeurs résultant de la croissance de l’emploi précaire. Dans cette étude, nous avons cherché à savoir si les politiques concernant l’indemnisation et le retour au travail répondaient aux besoins des travailleurs ontariens qui occupent un emploi précaire. Une analyse critique du discours a guidé l’étude, qui a consisté en entrevues en profondeur menées auprès de quinze travailleurs occupant un emploi précaire et de cinq employeurs qui ont embauché et dirigé ce genre de travailleurs. Nous avons identifié trois domaines pour lesquels les politiques de retour au travail ne sont pas en adéquation avec les besoins des travailleurs précaires. Il s’agit de l’écart en matière de connaissances et de pouvoir entre les employeurs bien informés et les travailleurs vulnérables, des défis liés à l’attribution des blessures et de la crainte des travailleurs à parler ouvertement de leurs accidents de travail. The question of how workers’ compensation systems should account for language barriers is of increasing importance given Canada’s growing linguistic diversity. This study is the first to empirically examine, based on interviews with injured workers and key informants, return-to-work policies and practices through the lens of language barriers. By comparing two jurisdictions, Quebec and Ontario, the study highlights similarities and differences in language accommodation and return-to-work policies and practices that shape the experiences of injured workers with language needs. It argues that gaps in this regard, which are more pronounced in Quebec, contribute to poor return-to-work outcomes for these workers in both provinces. La façon dont les régimes d’indemnisation des victimes de lésion professionnelle doivent tenir compte des barrières linguistiques est d’une importance capitale à la lumière de la diversification linguistique croissante du Canada. Cet article, qui présente des résultats d’une étude québécoise sur le retour au travail après une lésion professionnelle subie par des personnes occupant un emploi précaire ou mobile, met en évidence certains effets inattendus des règles de droit et des incitatifs économiques y associés sur le processus de retour au travail. Il analyse les enjeux juridiques qui sous-tendent trois études de cas tirées d’une étude exploratoire comprenant des entrevues avec 11 victimes de lésions professionnelles et 18 informatrices et informateurs clés. Les résultats, basés sur une analyse en profondeur des mécanismes par lesquels le cadre juridique régissant la santé et la sécurité du travail (indemnisation et prévention) et le droit à l’égalité déterminent l’expérience rapportée par deux travailleurs d’agence de travail temporaire et une professionnelle de la santé engagée dans un travail de type « fly-in/fly-out », permettent d’identifier les mécanismes de production de résultats défaillants dans le processus de réadaptation professionnelle. L’étude découvre des conséquences inattendues de certaines règles sur l’expérience des trois victimes de lésion professionnelle et suggère des moyens pour réduire certaines pratiques sous-optimales en tenant mieux compte de l’environnement juridique dans lequel le processus de réadaptation se déroule, afin de rendre effectives les normes visant la prévention des incapacités au travail. This article reports on findings drawn from a Quebec study investigating the experiences of precariously employed or mobile workers who attempt to return to work after a work injury. It illustrates the unexpected effects of legal regulations and their associated economic incentives on the return-to-work process. Analyzing the legal context of three case studies drawn from interviews with 11 injured workers and 18 key informants, the study examines the ways in which the regulatory framework may shape the claimants’ experiences. It first provides a detailed account of the return-to-work experiences of three workers: a fly-in, fly-out health care worker and two employees of temporary employment agencies. It then provides an in-depth examination of the mechanisms by which specific parameters of the regulatory framework that governs workers’ compensation, occupational health and safety, and the right to equality shape events leading to less than satisfactory outcomes in disability prevention. The study shows that the experiences of the three injured workers were shaped by regulatory issues that had unforeseen consequences. Avenues are suggested to improve the regulatory effectiveness of laws designed to prevent work disability.

Aging workers represent an emerging, growing, and essential population for the contemporary labour market. Considering their unique characteristics, these individuals are at risk of experiencing periods of disability at work due to different reasons (e.g., chronic diseases, occupational injuries) and a different return-to-work experience compared to younger workers. The scoping review presented in this article aimed to identify facilitators and barriers to returning to work in aging people who suffered a health impairment. Information from 34 manuscripts was extracted and analyzed, enabling the identification of factors hindering and facilitating the return to work of aging individuals, with regard to four systems: the individual, work, health, and compensation. The results suggest levers accessible to the various stakeholders involved in the process of returning to work for aging individuals to promote their healthy, safe, and satisfactory participation in work after a period of disability.

Après les traitements pour un cancer du sein, plusieurs survivantes désirent réintégrer une vie sociale active qui inclut le retour au travail. Bien que cette étape semble bénéfique, elle comporte plusieurs défis à surmonter tels que la gestion des effets secondaires des traitements au travail ou un soutien variable de la part des collègues et des professionnels de la santé. Toutefois, des facettes de cette expérience demeurent à être exploré au regard des survivantes, qui sont le plus susceptibles de réintégrer leur emploi. Il est même permis de croire que cette expérience peut être semblable à celle d’une transition. À notre connaissance, peu d’études abordent cette expérience sous l’angle de la transition et au regard des survivantes âgées de moins de 50 ans. Le but de cette étude était d’explorer l’expérience de transition vers le retour au travail de survivantes du cancer du sein. Une étude qualitative exploratoire a été effectuée. Des entrevues narratives (n=8) ont été menées auprès de survivantes qui étaient retournées au travail depuis au moins six mois et âgées de moins de 50 ans. Une analyse de contenu thématique a été réalisée. Les résultats ont été regroupés selon quatre thèmes : 1) une vie changée par le cancer, 2) le travail est un signe de normalité, 3) les facteurs influençant la transition et 4) les stratégies mobilisées pour réussir la transition vers le retour au travail. Les résultats suggèrent que les survivantes du cancer du sein veulent délaisser leur vie teintée par la maladie et réintégrer une vie « normale » qui inclut le retour au travail. L’expérience de transition vers le retour au travail est aussi influencée par plusieurs facteurs qui peuvent la faciliter ou l’entraver. Cette étude a permis de mettre en lumière l’expérience de transition vers le retour au travail de femmes de moins de 50 ans qui semblent comporter des défis distincts par rapport à celles des femmes plus âgées. À la lumière des résultats, de nombreuses recommandations ont été élaborées afin de mieux accompagner les survivantes de moins de 50 ans durant leur transition vers le retour au travail., After breast cancer treatments, many survivors wish to return to an active social life that includes return to work. Although this step seems beneficial, it has several challenges to overcome, such as managing the side effects of treatment at work or varying support from colleagues and health professionals. However, aspects of this experience remain to be explored for survivors, who are most likely to return to their jobs. It is even possible that this experience may be similar to that of a transition. To our knowledge, few studies have addressed this experience from a transition perspective and from the perspective of survivors under the age of 50. The purpose of this study was to explore the experience of transitioning breast cancer survivors back to work. An exploratory qualitative study was conducted. Narrative interviews (n=8) were conducted with youth survivors who had returned to work for at least six months and were under the age of 50. An analysis of the thematic content was carried out. The results were grouped into four themes: 1) a life changed by cancer, 2) work is a sign of normality, 3) factors influencing the transition and 4) strategies mobilized to successfully transition back to work. The results suggest that breast cancer survivors want to leave their illness-tinged lives and return to a “normal” life that includes returning to work. The experience of transitioning back to work is also influenced by several factors that may facilitate or hinder it. This study highlighted the transition experience of women under the age of 50 who appear to have distinct challenges to return to work. Based on the results, many recommendations were developed to better support survivors during their transition back to work.

Problématique : La reprise du travail des survivantes du cancer du sein peut représenter un défi qui doit être compris davantage afin de favoriser le développement d’interventions permettant de mieux les soutenir. But : Modéliser une intervention visant la réappropriation du travail des survivantes du cancer du sein. Méthode : Une étude de cas a été réalisée. Le cas était défini comme étant le processus de réappropriation du travail après un cancer du sein durant les phases d’absence, de retour (RAT) et de maintien (MAT). Trois sources de données ont été privilégiées : 1) survivantes du cancer du sein en MAT, 2) écrits scientifiques sur les interventions visant le RAT et le MAT des survivants du cancer et 3) experts du terrain. Des entrevues ont été réalisées avec les sources 1 et 3. Une revue de la littérature inspirée de Whittemore et Knalf a été réalisée. Une intervention, basée sur l’ensemble des résultats de l’étude de cas, a été modélisée sous forme d’un modèle logique. Résultats : Trois principaux constats sont issus de l’étude de cas : 1) de multiples facteurs interreliés durant les phases d’absence, de RAT et de MAT favorisent ou nuisent au processus de réappropriation du travail, 2) il existe un écart entre le contenu des interventions visant le RAT et le MAT des survivants du cancer et les facteurs identifiés comme charnières par les survivantes du cancer du sein et 3) les milieux de travail semblent encore très peu mobilisés dans ces interventions. L’intervention est présentée sous la forme d’un modèle logique comprenant, entre autres, trois activités: 1) invitation, de l’employeur à la survivante, à maintenir la communication avec le milieu de travail pendant l’absence, 2) utilisation d’un guide commun de préparation des phases de RAT et de MAT, par l’employeur et la survivante, et 3) utilisation d’un guide d’autogestion par la survivante. Discussion : L’intervention élaborée est riche puisqu’elle est ancrée dans le vécu des survivantes, basée sur les d, Background: Returning and staying at work after treatments is a common challenge for cancer survivors. There is a need for a better understanding of the return-to-work process in order to be able to develop improved interventions to support breast cancer survivors returning to work. Goal: To model an intervention to support the return-to-work process of breast cancer survivors. Methods: A case study was conducted. The case was defined as the return-to-work process comprising the absence phase, the return-to-work phase (RTW) and the stay-at-work phase (SAW). Three sources of data have been prioritised: 1) breast cancer survivors, 2) literature on interventions designed to support the return-to-work process after a cancer diagnosis and 3) field experts. Semi-structured interviews were carried out with sources 1 and 3. A literature review was performed using a method inspired by the Whittemore and Knalf approach. An intervention was then modeled based on the overall results of the case study. Results: Three findings emerged from the case study: 1) there are multiple interrelated factors during the absence, RTW and SAW phases that can positively or negatively affect the return-to-work process, 2) there is a gap between the content of interventions designed to support the return-to-work process after cancer and the factors identified by the breast cancer survivors and 3) very few of these interventions integrate the workplaces. The modeled intervention is presented in the form of a logic model comprising activities, products, results and effect. The activities are: 1) an invitation, from the employer to the survivor, to maintain communication during the absence period, 2) the use of a common guide, by the employer and the survivor, to prepare the return-to-work and stay-at-work periods and 3) the use of a health self-management guide by the survivor. 49. Psychosocial Treatment Techniques to Augment the Impact of Physiotherapy Interventions for Low Back Pain.

- Abstract
Purpose: The present study examined the profile of physical and psychosocial changes that occur in physiotherapy intervention when patients also participate in a psychosocial intervention. The psychosocial intervention, delivered by physiotherapists, was designed to target catastrophic thinking, fear of pain, perceived disability, and depression. Methods: The study sample consisted of 48 individuals referred for the rehabilitation treatment of disabling back pain. Half the sample was enrolled in a physiotherapy intervention only; the other half was enrolled in a psychosocial intervention in addition to receiving a physiotherapy intervention. Results: At post-treatment, the two treatment groups did not differ significantly on measures of pain severity, physical function, or self-reported disability. Patients who participated in the psychosocial intervention in addition to physiotherapy showed significantly greater reductions in pain catastrophizing, fear of movement, and depression than patients who received only the physiotherapy intervention. Reductions in psychosocial risk factors contributed to reduced use of the health care system, reduced use of pain medication, and improved return-to-work outcomes. Conclusions: The findings of the present study suggest that a psychosocial intervention provided by physiotherapists can lead to meaningful reductions in psychosocial risk factors for pain and disability and may contribute to more positive rehabilitation outcomes.
