Healthy Start (HS) programs around the country seek to lower the risk of maternal mortality through services that screen for and address social determinants of health, including mental health. This study used secondary data from the University of Houston Healthy Start (UHHS) to examine postpartum depression screening and referral rates among program participants and feedback from participating mothers about the program’s mental health services.Patient and Methods: Secondary quantitative data from the program’s administration of services (n = 164) and secondary qualitative data from a sub-set of program participants (n = 23) were analyzed. Using data from the standardized HS tools (required for all sites), aggregate demographic data and screening results of depression and interpersonal violence were assessed. Anonymous qualitative data from focus groups were analyzed for themes related to mental health.Results: Screening for postpartum depression was 97% with a 78% referral rate. Screening for interpersonal violence (IPV), a known correlate of PMADs, was 98% with a 100% referral rate for the 4 women who screened positive for IPV. Qualitative themes revealed close-knit relationships with case managers and doulas that encouraged disclosure of symptoms, honest discussions, and an increased desire to ask for help when needed. Emergent themes revealed that Persistence and authenticity were critical to gain participant trust; a culture of sisterhood positively participants’ mental health; and case managers were proactive with emotional support and resource referral.Conclusion: High rates of mental health screening and referral at the UHHS site were documented and participant stories revealed that trust with case managers was instrumental to their wellbeing. More robust data is needed to allow statistical comparisons are needed in future research.Plain Language Summary: In this study, we wanted to know how well a strengths-based program, the University of Houston Healthy Start (UHHS), screened and referred mothers for postpartum depression needs. We also want to explore how mothers in the program perceived and experienced mental health services provided by UHHS. We used existing data from the program evaluation to answer both questions.We analyzed administrative data from 164 mothers in the program and analyzed anonymous qualitative data from focus groups (n=). We found that nearly all mothers (97%) were screened for postpartum depression, and among high-risk moms, the majority (78%) got referrals for further care. From the group interviews, we learned that mothers formed strong, trusting relationships with case managers. This close bond encouraged them to open up about their struggles and seek help. Abstract

Cosmic shear, galaxy clustering, and the abundance of massive halos each probe the large-scale structure of the universe in complementary ways. We present cosmological constraints from the joint analysis of the three probes, building on the latest analyses of the lensing-informed abundance of clusters identified by the South Pole Telescope (SPT) and of the auto- and cross-correlation of galaxy position and weak lensing measurements (3$\times$2pt) in the Dark Energy Survey (DES). We consider the cosmological correlation between the different tracers and we account for the systematic uncertainties that are shared between the large-scale lensing correlation functions and the small-scale lensing-based cluster mass calibration. Marginalized over the remaining $\Lambda$CDM parameters (including the sum of neutrino masses) and 52 astrophysical modeling parameters, we measure $\Omega_\mathrm{m}=0.300\pm0.017$ and $\sigma_8=0.797\pm0.026$. Compared to constraints from Planck primary CMB anisotropies, our constraints are only 15% wider with a probability to exceed of 0.22 ($1.2\sigma$) for the two-parameter difference. We further obtain $S_8\equiv\sigma_8(\Omega_\mathrm{m}/0.3)^{0.5}=0.796\pm0.013$ which is lower than the Planck measurement at the $1.6\sigma$ level. The combined SPT cluster, DES 3$\times$2pt, and Planck datasets mildly prefer a non-zero positive neutrino mass, with a 95% upper limit $\sum m_\nu<0.25~\mathrm{eV}$ on the sum of neutrino masses. Assuming a $w$CDM model, we constrain the dark energy equation of state parameter $w=-1.15^{+0.23}_{-0.17}$ and when combining with Planck primary CMB anisotropies, we recover $w=-1.20^{+0.15}_{-0.09}$, a $1.7\sigma$ difference with a cosmological constant. Abstract

We present cosmological constraints from the abundance of galaxy clusters selected via the thermal Sunyaev-Zel'dovich (SZ) effect in South Pole Telescope (SPT) data with a simultaneous mass calibration using weak gravitational lensing data from the Dark Energy Survey (DES) and the Hubble Space Telescope (HST). The cluster sample is constructed from the combined SPT-SZ, SPTpol ECS, and SPTpol 500d surveys, and comprises 1,005 confirmed clusters in the redshift range $0.25-1.78$ over a total sky area of 5,200 deg$^2$. Abstract

We present a Bayesian population modeling method to analyze the abundance of galaxy clusters identified by the South Pole Telescope (SPT) with a simultaneous mass calibration using weak gravitational lensing data from the Dark Energy Survey (DES) and the Hubble Space Telescope (HST). We discuss and validate the modeling choices with a particular focus on a robust, weak-lensing-based mass calibration using DES data. For the DES Year 3 data, we report a systematic uncertainty in weak-lensing mass calibration that increases from 1% at $z=0.25$ to 10% at $z=0.95$, to which we add 2% in quadrature to account for uncertainties in the impact of baryonic effects. We implement an analysis pipeline that joins the cluster abundance likelihood with a multi-observable likelihood for the Sunyaev-Zel'dovich effect, optical richness, and weak-lensing measurements for each individual cluster. We validate that our analysis pipeline can recover unbiased cosmological constraints by analyzing mocks that closely resemble the cluster sample extracted from the SPT-SZ, SPTpol ECS, and SPTpol 500d surveys and the DES Year 3 and HST-39 weak-lensing datasets. Abstract

We present an extension to a Sunyaev-Zel'dovich Effect (SZE) selected cluster catalog based on observations from the South Pole Telescope (SPT); this catalog extends to lower signal-to-noise than the previous SPT-SZ catalog and therefore includes lower mass clusters. Optically derived redshifts, centers, richnesses and morphological parameters together with catalog contamination and completeness statistics are extracted using the multi-component matched filter algorithm (MCMF) applied to the S/N>4 SPT-SZ candidate list and the Dark Energy Survey (DES) photometric galaxy catalog. The main catalog contains 811 sources above S/N=4, has 91% purity and is 95% complete with respect to the original SZE selection. It contains 50% more total clusters and twice as many clusters above z=0.8 in comparison to the original SPT-SZ sample. The MCMF algorithm allows us to define subsamples of the desired purity with traceable impact on catalog completeness. As an example, we provide two subsamples with S/N>4.25 and S/N>4.5 for which the sample contamination and cleaning-induced incompleteness are both as low as the expected Poisson noise for samples of their size. The subsample with S/N>4.5 has 98% purity and 96% completeness, and will be included in a combined SPT cluster and DES weak-lensing cosmological analysis. We measure the number of false detections in the SPT-SZ candidate list as function of S/N, finding that it follows that expected from assuming Gaussian noise, but with a lower amplitude compared to previous estimates from simulations., Comment: 16 pages, 17 figures, submitted to MNRAS

Abstract

ObjectiveTo assess whether late surfactant treatment in extremely low gestational age (GA) newborn infants requiring ventilation at 7-14 days, who often have surfactant deficiency and dysfunction, safely improves survival without bronchopulmonary dysplasia (BPD).Study designExtremely low GA newborn infants (GA ≤28 0/7 weeks) who required mechanical ventilation at 7-14 days were enrolled in a randomized, masked controlled trial at 25 US centers. All infants received inhaled nitric oxide and either surfactant (calfactant/Infasurf) or sham instillation every 1-3 days to a maximum of 5 doses while intubated. The primary outcome was survival at 36 weeks postmenstrual age (PMA) without BPD, as evaluated by physiological oxygen/flow reduction.ResultsA total of 511 infants were enrolled between January 2010 and September 2013. 16. Effect of building airtightness and fan size on the performance of mechanical ventilation systems in new U.S. houses: a critique of ASHRAE standard 62.2-2003

Abstract

As many countries have already done, the U.S. is now undergoing a transition from homes ventilated by infiltration to homes that need mechanical ventilation to provide outdoor air when windows are closed. The American Society of Heating, Refrigerating and Air-conditioning Engineers (ASHRAE) developed, and the American National Standards Institute (ANSI) recently approved, a new residential ventilation Standard 62.2-2003, which would require many new homes to have a mechanical ventilation system. However, Standard 62.2 sizes ventilation fans in such a way that the tighter the home, and the more it needs mechanical ventilation, the less likely it is to receive the minimum ventilation rate of 0.35 air-changes/hour.This thesis uses the multi-zone building airflow software model CONTAMW to compare hourly air-change rates in a typical new home during a typical meteorological year in the moderate climate of Houston Texas. Two building parameters are varied in this analysis: leakage area (or airtightness), and ventilation fan size. There are three leakage scenarios: typical new construction, energy-efficient new construction, and tightest construction in the U.S.—and three fan scenarios: no ventilation fan, exhaust fan sized according to Standard 62.2, and an exhaust fan sized to deliver 0.35 air-changes per hour—for a total of nine scenarios. Air-change results are analyzed for time spent under-ventilated, adequately ventilated, and over-ventilated, and for exhaust ventilation efficiency, which is defined as percent of total ventilation attributable to the fan.Results indicate that exhaust fans sized according to Standard 62.2 improve ventilation only marginally in typical new construction, while energy-efficient or tighter homes would be significantly under-ventilated. This thesis suggests revising Standard 62.2 to: 1) size ventilation fans to ensure a minimum ventilation rate, regardless of house tightness, and 2) address the impact of tightness on ventilation system performance.

Abstract

The environments where galaxies reside crucially shape their star formation histories. We investigate a large sample of 1626 cluster galaxies located within 105 galaxy clusters spanning a large range in redshift (0.26 < z < 1.13). The galaxy clusters are massive (M 500 ≳ 2 × 1014 M ⊙) and uniformly selected from the SPT and ACT Sunyaev–Zel’dovich surveys. With spectra in hand for thousands of cluster members, we use the galaxies’ position in projected phase space as a proxy for their infall times, which provides a more robust measurement of environment than quantities such as projected clustercentric radius. We find clear evidence for a gradual age increase of the galaxy’s mean stellar populations (∼0.71 ± 0.4 Gyr based on a 4000 Å break, Dn4000) with the time spent in the cluster environment. This environmental quenching effect is found regardless of galaxy luminosity (faint or bright) and redshift (low or high-z), although the exact stellar age of galaxies depends on both parameters at fixed environmental effects. Such a systematic increase of Dn4000 with infall proxy would suggest that galaxies that were accreted into hosts earlier were quenched earlier due to longer exposure to environmental effects such as ram pressure stripping and starvation. Compared to the typical dynamical timescales of 1–3 Gyr of cluster galaxies, the relatively small age increase (∼0.71 ± 0.4 Gyr) found in our sample galaxies seems to suggest that a slow environmental process such as starvation is the dominant quenching pathway. Our results provide new insights into environmental quenching effects spanning a large range in cosmic time (∼5.2 Gyr, z = 0.26–1.13) and demonstrate the power of using a kinematically derived infall time proxy.
