[ES] Introducción: La ventilación mecánica no invasiva (VMNI) se ha convertido durante los últimos años en una alternativa a la intubación orotraqueal (IOT) y conexión a ventilación mecánica invasiva (VMI) para el tratamiento de la insuficiencia respiratoria (IR). Aunque en algunas patologías, como la reagudización de la Enfermedad Pulmonar Obstructiva Crónica (EPOC) o el edema agudo de pulmón (EAP) la evidencia es muy amplia, en otro tipo de pacientes existen más discrepancias. Objetivo: Se plantean dos objetivos principales: - Conocer cómo se está utilizando la VMNI y su grado de eficacia. - Identificar marcadores predictores precoces de fracaso de la VMNI, para mejorar su utilización en futuros pacientes. Material y métodos: Se analizaron de forma retrospectiva todos los pacientes que recibieron tratamiento con VMNI en la Unidad de Cuidados Intensivos (UCI) del Hospital Clínico de Salamanca entre los años 2006 y 2011. Se realizó un análisis estadístico mediante la versión 19 del programa de IBM SPSS Statistics. - Para el análisis descriptivo y el comparativo se obtuvieron medias, desviaciones típicas y porcentajes, y se aplicaron los test adecuados a las distintas variables. - Posteriormente se realizó un estudio de regresión logística binaria, para identificar marcadores de fracaso de la VMNI. De forma independiente se realizó un estudio mediante técnicas de minería de datos, con dos tipos de algoritmos: - Algoritmos de selección de atributos, para identificar las variables más importantes en el resultado de la VMNI. - Algoritmos de clasificación, que predecirán éxito o fracaso de la VMNI en futuros pacientes. Resultados: En total se contabilizaron 410 casos de VMNI, con una edad media de 66,69 ± 13,38 años, y un APACHE II medio de 20,83 ± 13,38. La causa más frecuente de uso fue la IR aguda hipoxémica (38,05%), seguida de lejos de la IR post-extubación (21,22%) y la reagudización de EPOC (19,02%). El grupo menos numeroso fue el del EAP (6,83%). El porcentaje de fracaso total fue del 50,73%, con una mortalidad total del 33,41%. Los pacientes en los que la VMNI fue exitosa tuvieron menor mortalidad, con menor estancia en UCI y hospitalaria que los pacientes que fracasaron. Por tipo de IR, la que tuvo un mayor fracaso fue la IR aguda hipoxémica (73,72%), y la que menos el EAP (21,43%). El estudio de regresión logística identifica como variables predictoras de fracaso: el tipo de IR, el APACHE II, la existencia de alteraciones radiológicas al inicio de la VMNI, la bilirrubina al inicio de la VMNI, la necesidad de sedación para tolerarla, y el cambio de nivel de consciencia, PAFI, frecuencia respiratoria y frecuencia cardiaca del inicio a las 2 horas de VMNI. Las técnicas de minería de datos identifican multitud de variables importantes en el resultado de la VMNI, entre las que destacan los balances a las 12 y a las 24 horas. De los distintos modelos de clasificación, el que mejores resultados obtuvo fue el de Bagging con J48, con una precisión del 76,83%. Conclusión: La VMNI es una técnica útil para el tratamiento de la IR, siendo más eficaz en el EAP y en la reagudización de EPOC. Existen predictores de fracaso que pueden ayudar a la toma de decisiones en estos pacientes. Las técnicas de minería de datos pueden ser una herramienta útil para el manejo de grandes cantidades de variables., [EN]Introduction: Noninvasive mechanical ventilation (NIV) has become in recent years an alternative to endotracheal intubation (OTI) and invasive mechanical ventilation (IMV) for the treatment of respiratory failure (IR). Although some diseases, such as acute exacerbation of chronic obstructive pulmonary disease (COPD) or pulmonary edema (EAP) is extensive evidence in other types of patients are more discrepancies. Objective: They raise two main objectives: - Know how you are using the NIV and their effectiveness. - Identify early markers predictors of NIV failure to improve future use in patients. Material and methods: We retrospectively analyzed all patients who were treated with NIV in the Intensive Care Unit (ICU) of the Hospital Clinico de Salamanca between 2006 and 2011. Independently conducted a study using data mining techniques, with two types of algorithms: - Feature selection algorithms, to identify the most important variables in the outcome of NIV. - Classification algorithms that predict success or failure of NIV in future patients. Results: In total there were 410 cases of NIV, with a mean age of 66.69 ± 13.38 years, and average APACHE II 20.83 ± 13.38. The most common use was the hypoxemic ARF (38.05%), followed distantly by the IR post-extubation (21.22%) and exacerbation of COPD (19.02%). The smallest group was the EAP (6.83%). The overall failure rate was 50.73%, with a total mortality of 33.41%. The patients in whom NIV was successful had lower mortality, less ICU and hospital stay than patients who failed. By type of IR, which had a major failure was the hypoxemic ARF (73.72%), and the least the EAP (21.43%). The logistic regression analysis identified as predictors of failure: the type of IR, the APACHE II, the presence of radiographic abnormalities at the start of NIV, bilirubin at the start of NIV, the need for sedation to tolerate it, and change level of consciousness, PAFI, respiratory rate and heart rate beginning at 2 hours of NIV. The data mining techniques identify many important variables in the outcome of NIV, among which the balance sheets at 12 and 24 hours. Of different classification models, the best performing was the Bagging with J48, with an accuracy of 76.83%. Conclusion: The NIV is a useful technique for the treatment of IR, being more effective in the EAP and the exacerbation of COPD. There predictors of failure that can help decision making in these patients. The data mining techniques can be a useful tool for handling large numbers of variables.
