Ein hoher Prozentsatz von Patienten weist zu Beginn einer chronischen Nierenersatztherapie keinen permanenten Gefaszugang bzw. Peritonealdialyse (PD)-Katheter auf, obwohl grose Anstrengungen unternommen werden, die Verwendung zentralvenoser Dialysekatheter (ZVDK) einzudammen. Es konnte gezeigt werden, dass ein ungeplanter Dialysebeginn unter Einsatz temporarer Hamodialyse (HD)-Katheter mit einer hohen Rate an infektiosen Komplikationen und Mortalitat vergesellschaftet ist. Trotz ihres mit der HD vergleichbaren Outcomes wird die PD nur selten eingesetzt. Um den Bedarf an ZVDK zu reduzieren, sind ein HD-Beginn mittels ZVDK und eine PD mit PD-Katheter hinsichtlich Komplikationen und Uberleben in akuten und ungeplanten Situationen miteinander verglichen worden. Es zeigte sich, dass der akute, ungeplante PD-Beginn unter sofortiger Verwendung des PD-Katheters unmittelbar nach dessen Einbringen weder das Risiko fur infektiose Komplikationen erhoht noch langfristig die Lebensdauer der PD-Technik oder das Patientenuberleben beeintrachtigt. Die Etablierung eines akuten, ungeplanten PD-Beginns erfordert besondere infrastrukturelle und logistische Voraussetzungen wie die schelle Implantation eines PD-Katheters, den fruhen Start der PD-Therapie mit modifizierter Rezeptierung sowie adaquater personeller und pflegerischer Ausstattung. Die Studien belegen, dass die PD-Therapie unter einer engagierten und entsprechenden Herangehensweise fur Patienten, die dringend eine Nierenersatztherapie benotigen, nicht nur im chronischen, sondern auch im akuten, ungeplanten Setting eine praktikable, sichere und effiziente Alternative zur HD darstellt. In diesem Beitrag werden klinische Berichte uber den Beginn einer akuten, ungeplanten PD-Therapie und wesentliche infrastrukturelle und logistische Anforderungen vorgestellt.

5. Comparison of effects of digital vaginal examination with transperineal ultrasound during labor on pain and anxiety levels: a randomized controlled trial

To evaluate whether routine vaginal examination during labor is associated with increased levels of anxiety and pain compared with transperineal ultrasound assessment.This was a single-blinded, parallel, randomized controlled trial conducted in a tertiary care facility. Parous pregnant women without a known psychiatric condition who were seen at the care facility between November 2015 and March 2016 were included in the trial. Participants had an uneventful pregnancy and were assigned randomly to routine digital vaginal examination or transperineal ultrasound assessment during labor. Psychological distress levels, measured by the Symptom Checklist-90-Revised, and anxiety levels, measured by State-Trait Anxiety Inventory (STAI), were recorded before admission, and pain, measured using a visual analog scale, and anxiety were recorded during the latent phase of labor, the beginning of active labor and the postpartum period. A sample size of 45 women per group (n = 90) was planned to compare methods of assessment.Ninety women were randomized (1:1 allocation) to one or other of the interventions. Preadmission psychological distress and anxiety levels were similar between the two groups (P = 0.93 and 0.65, respectively). Most of the studied characteristics were similar in each group including duration of labor, number of examinations, analgesic administration during labor, episiotomy rate and interval between deliveries. Visual analog scale scores revealed that pain perception was reduced during latent (mean difference, -1.5 (95% CI, -2.51 to -0.57); P0.01) and active (mean difference, -1.2 (95% CI, -2.45 to -0.09); P = 0.03) stages of labor and during the postpartum period (mean difference, -0.5 (95% CI, -1.02 to -0.06); P = 0.02) in participants who had a transperineal ultrasound assessment compared with participants who had a digital vaginal examination. STAI scores revealed that anxiety levels were similar between the two groups during the latent and active phases of labor and during the postpartum period (P = 0.07, P = 0.38 and P = 0.13, respectively).The perception of pain was significantly reduced with the use of a transperineal ultrasound assessment compared with routine digital vaginal examination. However, only during the latent stage of labor was the magnitude of the observed effect sufficiently great to be considered clinically significant. Our results indicate that transperineal ultrasound assessment could be preferred to digital examination for the evaluation of progression of labor during this phase. 6. The Effect of Pentoxifylline on Random-Pattern Skin-Flap Necrosis Induced by Nicotine Treatment in the Rat

Cigarette smoke, and specifically nicotine, has been shown to reduce skin-flap survival. The purpose of this study was to determine if the preoperative administration of pentoxifylline can counteract the deleterious effects of nicotine on skin-flap survival in the rat. Sixty rats were distributed into four groups (n = 15). The survival of modified McFarlene skin flaps was assessed on postoperative day 7. The administration of nicotine (0.6 mg/kg) for 24 weeks preoperatively produced an average skin-flap survival of 59 percent; this was significantly decreased compared with controls (p < 0.05). When similarly treated animals were given pentoxifylline (20 mg/kg) for 30 days preoperatively, the mean skin-flap survival improved significantly to 80 percent (p < 0.05). Withholding nicotine for 2 weeks preoperatively also was found to significantly improve skin-flap survival to 73 percent (p < 0.05). Blood filterability was measured as an indicator of viscosity. The blood filterability in rats that received nicotine for 24 weeks was significantly decreased compared with controls (p < 0.05). Both the addition of pentoxifylline preoperatively and the withholding of nicotine for 2 weeks preoperatively were found to significantly improve blood filterability compared with rats that received nicotine alone for 24 weeks postoperatively (p < 0.05).

The tumescent technique consists of injection of large amounts of dilute lidocaine and epinephrine into the subcutaneous fat prior to performing liposuction. This preinjection provides local anesthesia and reduction in postoperative pain. Blood loss, bruising, and swelling are significantly reduced. The large volume of injectate is a complete fluid replacement, eliminating the need for intravenous fluid support. In the authors' experience, patients are most comfortable when the tumescent technique is combined with monitored intravenous sedation.

Summary Background Adenovirus keratoconjunctivitis outbreaks have rarely been reported in preterm infants. An outbreak of adenovirus conjunctivitis occurred between 15 January and 25 February at a neonatal intensive care unit of a university hospital in Turkey. Aim To describe the evolution, investigation and management of the outbreak. Methods Adenovirus type 8 was identified in 14 samples by polymerase chain reaction analysis. A case–control study was performed to determine the risk factors. Findings Fifteen preterm neonates, five healthcare workers (HCWs) and four parents suffered from conjunctivitis signs such as lacrimation, swelling and redness of the eye. A retinopathy of prematurity (ROP) examination was found to be the most important risk factor for adenovirus conjunctivitis (odds ratio: 17.5; 95% confidence interval: 1.9–163.0; P =0.012). The eyelid speculum (blepharostat) used during the ROP examination was not sterilized between each patient and was found to be the cause of contamination. Conclusion The outbreak was controlled by measures such as barrier precautions, hand hygiene, sterilization of the blepharostat, suspending patient transfer to other units, and excluding infected HCWs for at least 15 days.

The formation of reactive oxygen species (ROS) is increased in heart failure (HF). However, the causal and mechanistic relationship of ROS formation with contractile dysfunction is not clear in detail. Therefore, ROS formation, myofibrillar protein oxidation and p38 MAP kinase activation were related to contractile function in failing rabbit hearts.Three weeks of rapid left ventricular (LV) pacing reduced LV shortening fraction (SF, echocardiography) from 32 +/- 1% to 13 +/- 1%. ROS formation, as assessed by dihydroethidine staining, increased by 36 +/- 8% and was associated with increased tropomyosin oxidation, as reflected by dimer formation (dimer to monomer ratio increased 2.28 +/- 0.66-fold in HF vs. sham, P0.05). Apoptosis (TdT-mediated dUTP nick end labelling staining) increased more than 12-fold after 3 weeks of pacing when a significant increase in the phosphorylation of p38 MAP kinase and HSP27 was detected (Western blotting). Vitamins C and E abolished the increases in ROS formation and tropomyosin oxidation along with an improvement of LVSF (19 +/- 1%, P0.05 vs. untreated HF) and prevention of apoptosis, but without modifying p38 MAP kinase activation. Inhibition of p38 MAP kinase by SB281832 counteracted ROS formation, tropomyosin oxidation and contractile failure, without affecting apoptosis.Thus, p38 MAP kinase activation appears to be upstream rather than downstream of ROS, which impacts on LV function through myofibrillar oxidation. p38 MAP kinase inhibition is a potential target to prevent or treat HF.

10. A case with three primary tumors of the ovary, endometrium and gallbladder

A case with three synchronous tumors is presented. A 52-year-old patient underwent a total abdominal hysterectomy with bilateral salpingo-oophorectomy, bilateral pelvic and paraaortic lymph node dissection, and partial omentectomy for endometrial carcinoma accompanied by an adnexal mass. She further underwent cholecystectomy for a perioperative incidental suspicious nodule on the serosal surface of the gallbladder. Histopathology revealed a uterine endometrioid adenocarcinoma, a mucinous adenocarcinoma of the gallbladder, and an ovarian endometrioid carcinoma with a clear cell component. The progress of the patient until the time of death is discussed.

Intraventricular meningiomas have often been reported; however, literature reveals very few cases localized within foramen of Monro. Herein we report a 57-year-old woman admitted with obstructive hydrocephalus-related symptoms. Strikingly, the lesion was completely calcified in CT and had no marked solid component on MRI. The lesion was completely removed by surgical resection with a transfrontal intraventricular approach. The resected mass was histopathologically diagnosed as meningioma. The patient's symptoms resolved immediately after the operation.

respectively. The reticulocyte count reached 3%. Computed tomography of the chest showed a minimal response to chemotherapy. Two additional cycles of paclitaxel were given. The patient’s haematological values and disease have remained stable for 6 months. The best therapy for thymomaassociated cytopenia has not yet been established. We think that antitumour therapies may be effective as in other paraneoplastic syndromes. To our knowledge, this is the first report showing that paclitaxel may be beneficial in the treatment of thymoma-associated haematologic dyscrasias. The role of paclitaxel in the treatment of thymoma is unknown. On the basis of this case, we suggest that paclitaxel at 175 mg/m 2 every 3 weeks may be a promising chemotherapy for advanced or recurrent thymoma.

Bardet-Biedl syndrome (BBS) is a rare autosomal recessive disorder. End-stage renal failure has been reported as the most frequent cause of death in this disorder. There are few reports of kidney transplantation in these patients. Renal transplant patients are known to be at increased risk for the development of malignancies. Although a few patients with BBS have been described to develop malignant disease, there was no previous association with lymphoma. We report a 20-year-old patient in whom primary central nervous system lymphoma was diagnosed 20 months after renal transplantation.

15. [Acute renal failure in hantavirus infections]

The acute renal failure remains a diagnostic challenge for the clinician.Between 1991 and 1996, acute renal failure caused by hantavirus infection was diagnosed in 4 previously healthy male patients. Main symptoms consisted of fever, headache, arthralgia, lumbar and abdominal pain as well as a decline in diuresis. The ultrasonography showed a slight splenomegaly in 2 patients. The clinical chemistry showed elevated serum creatinine from 2.2 mg/dl to 6.7 mg/dl and thrombocytopenia from 4000 to 150,000/microliter. The examination of the urine showed slight proteinuria and microhematuria. The kidney biopsy of 1 patient showed a reversible damage of the tubuli. The pathologic findings normalized within 3 weeks in 3 patients without need for dialysis. One patient developed a severe clinical course with acute renal failure and pulmonary edema requiring dialysis. In all patients, the renal function improved.

16. [Incidence of cardiovascular risk factors and complications after kidney transplantation]

Cardiovascular disease is a leading cause of death after renal transplantation (RTx), and the incidence is considerably higher than in the general population. Aim of this study was to evaluate the incidence of atherosclerotic cardiovascular complications after RTx, the prevalence of cardiovascular risk factors, prior to and following RTx, and the association between the risk factors and complications.Analysis of atherosclerotic cardiovascular diseases (coronary artery disease, cerebral and peripheral vascular disease) and cardiovascular risk factors before and after transplantation in 427 renal transplant recipients between 1987 and 1992 (mean age at transplantation 45 +/- 12 years, 58% male, 7% diabetics) with a mean posttransplant follow-up of 28 +/- 20 months.Following RTx 11.7% developed atherosclerotic cardiovascular diseases, the majority coronary artery disease (9.8%). The comparison of risk factors 12 months before and 24 months following transplantation showed: The prevalence of systemic hypertension (from 67% to 86%), diabetes mellitus (from 7% to 16%) and obesity with a body mass index25 kg/m2 (from 26% to 48%) had increased significantly whereas the number of smokers halved to 20%. The triglycerides decreased significantly (from 235 +/- 144 mg/dl to 217 +/- 122 mg/dl). The total and HDL cholesterol rose significantly (from 232 +/- 65 mg/dl to 273 +/- 62 mg/dl and from 47 +/- 29 mg/dl to 56 +/- 21 mg/dl, respectively). The LDL cholesterol increase was insignificant (from 180 +/- 62 mg/dl to 189 +/- 53 mg/dl). In the univariate analysis, cardiovascular diseases were significantly associated with male gender, age over 50 years, diabetes mellitus (DM), smoking, total cholesterol200 mg/dl, LDL cholesterol180 mg/dl, HDL cholesterol55 mg/dl, fibrinogen350 mg/dl, body mass index25 kg/m2, and more than 2 antihypertensive agents per day. The Cox proportional hazards model revealed DM with a relative risk (RR) of 4.3, age50 years (RR = 2.7), body mass index25 kg/m2 (RR = 2.6), smoking (RR = 2.5), and LDL cholesterol180 mg/dl (RR = 2.3) as independent risk factors.The high incidence of cardiovascular disease following renal transplantation is mainly due to a high prevalence and accumulation of classical risk factors before and following transplantation. The treatment of the risk factors must be effective and introduced early in the course of renal failure, further, they must be continued following transplantation. The tumescent technique consists of injection of large amounts of dilute lidocaine and epinephrine into the subcutaneous fat prior to performing liposuction. This preinjection provides local anesthesia and reduction in postoperative pain. Blood loss, bruising, and swelling are significantly reduced. The large volume of injectate is a complete fluid replacement, eliminating the need for intravenous fluid support. In the authors' experience, patients are most comfortable when the tumescent technique is combined with monitored intravenous sedation.

Acromegaly produces a syndrome of physiological and physical derangements. Patients afflicted with this disease are often as concerned with the changes in their appearance as with the pituitary tumor. The external rhinoplasty approach to transphenoid hypophysectomy has recently been shown to improve visualization and to have fewer complications compared with the sublabial approach devised initially by Cushing. Three patients are presented in whom this approach was used to simultaneously treat the physiological derangement of acromegaly and to correct the nasal deformity.

Forty boys with hypospadias received 2 mg/kg testosterone esters intramuscularly 5 and 2 weeks before surgical correction of the hypospadias. The effects on penile development were determined 3 and 5 weeks after the first injection and 3 and 12 months post-operatively. A significant effect on penile development was shown. This effect disappeared during the first year following treatment. There was no effect on bone maturation, children's height and the ultimate testosterone level. All adverse effects disappeared after cessation of therapy. Androgen receptor deficiency as a possible cause of hypospadias is not supported by our observations.

This paper describes three sisters, triplets, age 7 years, 2 months, two of whom derive from a single ovum, the third from a second ovum. As such, they provide an opportunity to examine the relative impact of heredity on certain of their physical, developed, and acquired abilities and characteristics. In addition to a comparison of such physical traits as height, weight, refractive status, axial length, and so on, certain (developed) sensory-motor and (acquired) cognitive and language measures are reported, such as binocular status, visual and auditory analysis skills, intelligence quotient, and (expressive and receptive) language abilities. Because all three children have lived in (more or less) the same pre- and post-natal environment, we assume that whatever concordance the monozygotic (MZ) pair displays that is not shared by their dizygotic (DZ) sister is attributable to heredity. (It does not necessarily follow, however, that any discordance between the two MZ sisters or any concordance between one of them and their DZ sister is attributable to environment.

It is more common in various sociodemographic situations., Aims: This study investigated the risk factors associated with depressive symptoms and changes over the years based on data from a sample from the Republic of Türkiye. It was also intended to compare depressive symptoms and inflation rates by years., Method: Data for 2016, 2019 and 2022 from the Turkish Health Survey performed by the Turkish Statistical Institute were used in the study. Data for age, sex, marital status, education level, employment status, accompanying chronic health problems, restrictions in daily activities associated with health problems and problems accessing psychological treatment due to difficulty in paying were evaluated in terms of depressive symptoms., Results: Higher depressive symptoms were determined in women, the widowed and divorced, individuals with a low level of education, the unemployed, individuals with chronic health problems, those with restrictions in daily activities associated with chronic health problems and those with problems accessing psychological treatment due to difficulty in paying. Although inflation rates increased over the years, the severity of depressive symptoms decreased. 33. Response to treatment with grapiprant as part of a standard multimodal regimen in young dogs with appendicular joint osteoarthritis associated pain.

Introduction: The response to medical management of young dogs with osteoarthritis (OA) associated pain has not been evaluated. Using an open-label design, the effectiveness, over a 4-month period, of standardized management (grapiprant/fish oil/exercise) for treating OA pain in young dogs was evaluated., Methods: Included dogs were 9 months-4 years of age; ≥3.6 kg body weight; had ≥1 appendicular joint with radiographic OA and obvious joint pain; had a Liverpool Osteoarthritis in Dogs (LOAD) score of ≥5. The non-steroidal anti-inflammatory piprant (grapiprant) was given at the recommended dose daily, omega-3 fatty acid supplementation was initiated at 100 mg/kg and then increased to 200 mg/kg daily, and leash exercise was gradually increased to a target of 60 min daily. Client-reported outcome measures (CROMs) and force plate gait analysis were collected at baseline and monthly for 4 months. The index limb was defined as the most severely affected limb at baseline., Results: Forty-eight dogs were enrolled (mean ± SD age of 30.7 ± 10.7 months). Hips, elbows, and stifles were commonly affected. Medication and supplement compliance was excellent (≥95% of target administered), and treatments were well-tolerated. CROMs showed significant improvement over time and at each time point. Overall, peak vertical force (PVF) increased significantly (<0.001), and vertical impulse increased numerically. Increase in PVF from baseline was significant at all time points except 4-months., Discussion: This study demonstrates a clinically meaningful benefit of a multimodal treatment regimen over a 4-month period for young dogs (<4 years old) with OA-pain. - Abstract
Problem : Gay men (GMs) and lesbian women (LWs) can be exposed to misunderstanding, pressure, condemnation, obstructed access to public services, social isolation, and discrimination in many countries. They may also encounter various difficulties in accessing health services. Being GM and LW is generally unacceptable in Turkey, and both are perceived as abnormal. Medical students may require education on the subject of sexual orientation to improve their knowledge of and attitudes toward LGBT individuals, to help them remain neutral and avoid prejudice in providing health services for such individuals, and to ensure that such care is unbiased. Intervention : This one-group pretest-post-test design study was performed with third-year students at the Ondokuz Mayıs University Medical Faculty (Samsun-Turkey) on 01-31 September, 2021. Three hundred twenty-five students took part. We evaluated students' attitudes toward homophobia and being GM and LW following a two-week multidisciplinary education program. The program included such topics as "Marginalization," "Interaction between cultures," "Sexual orientation," "Faith-based marginalization," "Sub-cultures," "Health protection," "Gender," and "Marginalized groups." For objective acquisition, we organized small group work, experience-sharing sessions (such as different orientation groups, different ethnic groups, and different behavioral templates), presentations, and panel activities. Context : Some circles in Türkiye regard LGBT individuals as representing an attack on national and spiritual values, and they are used as part of the political discourse. Studies are being performed in some medial faculties in Türkiye concerning the inclusion of subjects related to LGBT individuals in the educational curriculum. However, these studies have not yet assumed the form of a curriculum design including content, method, and testing. It is important for subjects concerning LGBT individuals to be considered more extensively in medical education in Türkiye, and for awareness of the rights of these individuals in the community and of combating discrimination to be improved. Outcome : We observed a significant decrease in students' homophobia after education. Significant decreases were observed in agreement with statements to the effect that being a GM or LW is a disease, that it can be treated, that people can be identified as GMs and LWs based on their behavior, and that they pose a major threat to society. Lessons Learned : It is unclear whether education aimed at all marginalized groups will produce different results to those of programs aimed specifically at GMs and LWs, but we think that applying programs directed toward discrimination and prejudice together will yield more effective results.

This qualitative study was conducted to examine the relationship between Muslim midwives' spiritual values and ethical orientations and their attitudes towards uterine transplantation. The phenomenological study sample group consisted of students in the Midwifery undergraduate programme of a public university in Istanbul (n:26). The data were collected in semi-structured focus group interviews. It was among the student midwives, it was determined that some students rejected uterine transplantation with thoughts such as "I am a Muslim, according to my religious belief, I should go to the grave without damaging my organs" or "This transplantation is not a vital necessity", as well as students who considered uterine transplantation as religious and ethically appropriate. It was determined that midwife candidates who will provide reproductive health services have different ethical and moral views regarding uterus transplantation, and there are some who see this method as religiously acceptable, and there are also students who emphasize that it is not suitable for Islam. - Abstract
This study aimed to determine the prevalence of osteoarthritis (OA) and associated clinical signs in young dogs. Owners of dogs aged 8 months-4 years from a single practice, were contacted in random order, to participate in a general health screen. Clinical and orthopedic examinations were performed. Each joint was scored for pain reactions (0-4). Orthogonal radiographs of all joints were made under sedation. Each joint was scored for radiographic OA (rOA) severity on an 11-point scale. Clinical OA (cOA) was defined as an overlap of rOA and joint pain in ≥ 1 joint. Owners completed OA questionnaires. The owners of 123 dogs agreed to participate. Overall, 39.8% (49/123) of dogs had rOA in ≥ 1 joint, and 16.3% (20/123) or 23.6% (29/123) dogs had cOA, depending on the cut-off value of joint pain; moderate (2), or mild (1), respectively. Owners of dogs with cOA observed signs of impairment in approximately 30% of cases. Only 2 dogs with cOA were receiving OA pain management. The most commonly affected joints in descending order of frequency were elbow, hip, tarsus, and stifle. Radiographically visible OA is common in young dogs, and 40-60% of dogs with rOA had cOA. Background: The purpose of this study was to determine intern physicians' attitudes and beliefs toward obesity and individuals with obesity and the frequency at which they encounter discriminatory language in their work environments., Methods: This cross-sectional study was performed with intern physicians at the Ondokuz Mayıs University Medical Faculty in Samsun, Türkiye. A questionnaire was employed for data collection (Appendix). Two hundred eighteen (82.2%) interns took part in the study. The questionnaire consisted of 53 questions in five sections, and included the attitudes toward obese persons (ATOP) and beliefs about obese persons (BAOP) scales., Results: The interns most frequently cited genetic factors (98.1%) and endocrine disorder-metabolic disorders (98.1%) as non-behavioral causes of obesity, and overeating (96.2%) and physical immobility (96.2%) as behavioral causes. Lifestyle changes and exercise were most frequently recommended for treatment. The interns' attitudes toward individuals with obesity were very slightly negative, but close to neutral. Analysis showed that 46.8% of the interns reported hearing members of academic staff, intern physicians, or health workers make negative or derogatory comments or jokes about obese patients during their education, while 22.0% reported witnessing an obese patient being subjected to discriminatory treatment in the hospital environment., Conclusion: Medical faculties must develop curricula aimed at comprehensively addressing obesity-related bias. Such a curriculum should allow students to reflect on their biases, be aimed at reducing the effect of those biases on patient communication, and involve strategies directed toward eliminating those effects from physicians' treatment decisions., (© 2023. 38. Improving Flame Retardant Properties of Aliphatic Polyketone (POK)-Based Composites.

The effect of zinc borate (ZB) and high-molecular-weight siloxane (SIL) on flame retardancy, mechanical, and thermal properties of aliphatic polyketone (POK)-containing aluminum diethyl phosphinate (OF) was investigated in this study. Ten wt % OF is sufficient to obtain V0 rating according to the UL94 test. As the weight fraction of OF was increased, the flame retardancy properties and LOI values improved, while the tensile and impact properties decreased. To avoid the degradation in mechanical and impact properties as much as possible and obtain the same and better flame retardancy properties, synergists such as SIL and ZB were used. Flame retardancy of POK-based composites was determined by the limiting oxygen index (LOI) test, UL94 measurement, and cone calorimeter test. The additions of 1 wt % SIL and ZB have not led to a considerable decrease in the tensile strength and impact properties of POK-10OF. While ZB and SIL are very efficient in decreasing the smoke density, ZB is more efficient than SIL in increasing the LOI value of the composite. The addition of 1, 2, and 4 wt % ZB and SIL synergists did not lower their UL94 ratings. Moreover, it can be added that ZB is more efficient than SIL in decreasing the fire growth rate (FIGRA) and maximum average rate of heat emission (MARHE) values. The purpose of the study was to evaluate the relationship between school burnout, sense of school belonging, and academic achievement in medical students. This cross-sectional study was performed with students at the Ondokuz Mayıs University Medical Faculty (Samsun, Turkey) between 1 and 31 May 2019. Six hundred one (71.0%) first, second, and third-year students were included in the study. A questionnaire was employed as the data collection method. The questionnaire consisted of two parts. The first part consisted of questions investigating sociodemographic information produced by the authors by scanning the literature. The second part consisted of the School Burnout Inventory and the Psychological Sense of School Membership Scale. The relationship between the sense of school belonging, school burnout, and academic achievement was evaluated using structural equation modeling. The structural equation modeling analysis determined that school burnout played a mediating role in the relationship between the sense of school belonging and academic achievement in preclinical medical students. As the sense of belonging to the school increases in preclinical medical students, academic achievement increases. School burnout in preclinical medical students increases as a sense of school belonging decreases. Academic achievement among preclinical medical students decreases as school burnout levels increase. - Abstract
In this study, it was aimed to evaluate the opinions of psychiatrists in Turkey on whether to change the name of schizophrenia in order to reduce stigma. This cross-sectional survey was conducted with psychiatrists (resident in psychiatry, specialist, and lecturer) in Turkey. An online survey was created via the Google Forms public web address. Online questionnaires were delivered through Google Forms by emailing and messaging on WhatsApp, Telegram, Google and Yahoo groups and asking them to pass the questionnaire to other possible participants in their network. The study was performed between June 20, 2021 and July 10, 2021. 460 psychiatrists participated in the study. Forty-five-point 2 % of psychiatrists think that the name of schizophrenia should be renamed to reduce stigma. Forty-two-point 8 % of those who support the name change state that this change should be done as soon as possible. While 64.1 % of psychiatrists stated that naming the disease with another (new) name instead of schizophrenia could increase the hopes of patients and their relatives for recovery, 12.6 % stated that renaming would not cause any positive or negative changes. There is no statistical difference between psychiatrists who have a relative diagnosed with schizophrenia and psychiatrists who do not, in terms of thinking that the name of schizophrenia should be renamed to reduce stigma. In order to remove the stigma on schizophrenia, many interventions are required in social, cultural, economic and political fields. - Abstract
The purpose of this study was to evaluate the prevalence of depression, anxiety, distress, and insomnia and related factors in healthcare workers during the COVID-19 pandemic in Turkey. We applied the study survey online to HCWs during the pandemic in Turkey between 23 April and 23 May 2020. We used the sociodemographic data form, Patient Health Questionnaire-9, General Anxiety Disorder-7, Insomnia Severity Index, and Impact of Event Scale-Revised. Six hundred twenty (66.0%) of the 939 HCWs taking part in the study were female, 580 (61.8%) were physicians, 569 (60.6%) were working on the front line. Seven hundred twenty-nine (77.6%) participants exhibited depression, 565 (60.2%) anxiety, 473 (50.4%) insomnia, and 717 (76.4%) distress symptoms. Depression, anxiety, insomnia, and distress symptoms were significantly greater among females, individuals with a history of psychiatric illness, and individuals receiving psychiatric support during the COVID-19 pandemic. HCWs serving in Turkey during the COVID-19 pandemic experienced high levels of depression, anxiety, insomnia, and distress symptoms. Female gender, being a nurse, working on the front line, history of psychiatric illness, and being tested for COVID-19 were identified as risk factors for mental health problems.

- Abstract
The aim of this study was to elicit the views of medical faculty students regarding the COVID-19 pandemic. This descriptive study was performed with Ondokuz Mayıs University Medical Faculty students on 24-27 March, 2020. The Medical Faculty currently has 2051 students. A questionnaire was used as a data collection tool. For that purpose, the authors designed a questionnaire specifically for this research via the "Google Forms" web. This consisted of 40 open- and close-ended questions. The questionnaire was completed by 1375 (67.1%) students. Accordingly, 52.4% of medical students reported feeling mentally unwell. Although 50.8% of medical students reported generally/usually obtaining information about COVID-19 through the social media, 82.0% did not trust information/messages arriving through the social media and WhatsApp. We found that 86.7% of students regarded frequent hand washing as the most important means of protection against COVID-19, and 19.3% of students did not regard the COVID-19 pandemic as a severe public health problem for Turkey at that moment. In addition, 61.6% of students stated that a suppression strategy involving tight restrictions need to be applied to being the pandemic under control in Turkey. Use can be made of medical students in the transmission of accurate information during the COVID-19 pandemic. Students can be excellent activists on these subjects in countries in which medical education is suspended. Measures therefore need to be taken concerning the transmission of up to date and accurate information to medical students.

Water from the Hickey Run Tributary of the Anacostia River is being collected quarterly (beginning August 2018) and analyzed to create high-resolution baseline taxonomic profiles of microbiota associated with this important aquatic ecosystem, which has a long history of exposure to residential and commercial effluents from Washington, DC. Introduction: Cancer is a chronic disease that requires long-term treatment and care. Caregivers of cancer patients are at greater risk of developing depression than the general population. The effect of caregivers' cognitive flexibility on depression and anxiety has not been well studied. We aimed to investigate the social characteristics, burden levels and cognitive flexibility of caregivers of advanced cancer patients, and determine the relationship between these factors and depression and anxiety. We hypothesised that factors such as cognitive flexibility and caregiver burden level significantly predict anxiety and depression., Methods: The study included 69 primary informal caregivers of patients with Stage 4 cancer. Methods utilised included diagnostic semi-structured interviews, Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), Zarit Caregiver Burden Inventory and cognitive flexibility inventory., Results: BDI scores were found to be significantly higher in caregivers who cared for men compared to those who cared for women (20.44 ± 2.06 vs. 13.29 ± 1.81; t = 2.60; p = 0.01). BDI mean scores were statistically lower in caregivers who received help with caregiving compared to those who did not (t = 2.62; p = 0.01). Cognitive flexibility level, burden level and lack of social support were found to be predictors of caregiver depression., Conclusion: The study showed that individuals with low cognitive flexibility levels are more likely to have depressive and anxiety symptoms. Based on our findings, we opine that evaluations of caregivers' cognitive strategies and social support are needed to determine the risk of depression in caregivers of cancer patients., (Copyright: © Singapore Medical Association.)

The purpose of this study was to assess family physicians' knowledge, attitudes, and practices toward colorectal cancer (CRC) screening. The population in this cross-sectional study consisted of 290 family physicians working in Samsun, Turkey, contacted between 15 June and 15 July 2015 and agreeing to participate. A questionnaire prepared by the authors on the basis of the relevant literature was applied at face-to-face interviews. The first part of the questionnaire inquired into sociodemographic information, while the second contained questions evaluating family physicians' knowledge, attitudes, and practices toward CRC screening. Physicians completed the questionnaire in approximately 10 min. 65.9 % of the family physicians in the study were men. Mean age of the participants was 43.40 ± 6.54 years, and mean number of years in service was 18.43 ± 6.42. The average number of patients seen by physicians on a daily basis was 51-99. CRC screening was performed by 83.1 % of physicians. The fecal occult blood test (FOBT) was recommended at the correct frequency by 30.7 % of physicians and colonoscopy by 11.7 %. A further 68.6 % of physicians followed no CRC guideline. Only 3.8 % of those reporting using a guideline were able to name it. The great majority of physicians in this study apply CRC screening. However, family physicians lack sufficient information concerning the ages at which screening tests should be started and concluded and how frequently they should be performed. They also do not attach sufficient importance to CRC guidelines. This results in excessive demand for screening tests.

- Abstract
Background: Graft-versus-host disease (GVHD) is common after allogeneic hematopoietic cell transplantation (HCT). Risk for death from GVHD has been associated with low bacterial diversity in the stool microbiota early after transplant; however, the specific species associated with GVHD risk remain poorly defined., Methods: We prospectively collected serial weekly stool samples from 66 patients who underwent HCT, starting pre-transplantation and continuing weekly until 100 days post-transplant, a total of 694 observations in HCT recipients. We used 16S rRNA gene polymerase chain reaction with degenerate primers, followed by high-throughput sequencing to assess the relative abundance of sequence reads from bacterial taxa in stool samples over time., Results: The gut microbiota was highly dynamic in HCT recipients, with loss and appearance of taxa common on short time scales. As in prior studies, GVHD was associated with lower alpha diversity of the stool microbiota. At neutrophil recovery post-HCT, the presence of oral Actinobacteria and oral Firmicutes in stool was positively correlated with subsequent GVHD; Lachnospiraceae were negatively correlated. A gradient of bacterial species (difference of the sum of the relative abundance of positive correlates minus the sum of the relative abundance of negative correlates) was most predictive (receiver operator characteristic area under the curve of 0.83) of subsequent severe acute GVHD., Conclusions: The stool microbiota around the time of neutrophil recovery post-HCT is predictive of subsequent development of severe acute GVHD in this study., (© The Author 2017. Smartphone addiction is a recent concern that has resulted from the dramatic increase in worldwide smartphone use. The purpose of this cross-sectional study was to evaluate psychosocial factors affecting smartphone addiction in university students. The study was performed among students at the Ondokuz Mayis University Samsun School of Health (Samsun, Turkey) on October-December 2015. Four hundred ninety-four students possessing smartphones and agreeing to participate were included. A sociodemographic data form produced by the authors and consisting of 10 questions was administered together with a questionnaire involving the Smartphone Addiction Scale-Short Version (SAS-SV), the Flourishing Scale, the General Health Questionnaire, and the Multidimensional Scale of Perceived Social Support. The questionnaires were applied in a class environment at face-to-face interviews. SAS-SV scores of 6.47% of students were "significantly higher" than the participating group mean SAS-SV score. Multiple regression analysis revealed that depression, anxiety and insomnia, and familial social support statistically, significantly predicted smartphone addiction. Further studies of smartphone addiction in different age groups and with different educational levels are now needed.

Aim The purpose of this study was to assess the attitude of family physicians toward a child with delayed growth and development., Background: Primary healthcare professionals play a key role in monitoring growth and development, the best indicator of the child's health status. If delayed growth and development can be detected early, then it is usually possible to restore functioning., Methods: This descriptive study was performed in Samsun, Turkey, in May and June 2015. In total, 325 family physicians were included. The study consisted of two parts. In the first session of the research, the story of an 18-month-old child with delayed growth and development was presented using visual materials. An interview between the child's mother and a member of primary healthcare staff was then enacted by two of the authors using role-playing. Subsequently, participants were given the opportunity to ask the mother and member of primary healthcare staff questions about the case. During the sessions, two observers observed the participants, took notes and compared these after the presentation. In the second part of the study, the participants were asked to complete a questionnaire consisting of three open-ended questions. Findings When asking questions of the mother, family physicians generally used accusatory and judgmental language. One of the questions most commonly put to the mother was 'Do you think you are a good mother?' Family physicians were keen to provide instruction for the patient and relatives. Family physicians to a large extent thought that the problem of a child with delayed growth and development can be resolved through education., Conclusions: Family physicians' manner of establishing relations with the patient and relatives is inappropriate. We therefore think that they should receive on-going in-service training on the subject.

Colorectal cancer (CRC) is the third most common form of cancer in men worldwide and the second most common in women. The purpose of this study was to determine both barriers established by primary health care providers (PHCPs) and barriers perceived by them and to produce solutions for achieving the desired results. A four-part questionnaire was administered to family physicians (FPs) and family health personnels (FHPs) in Samsun, Turkey on 01-15 May, 2016. Sixty-six percent of PHCPs were contacted. Data were evaluated as numbers and percentages, and statistical significance was analyzed using the Chi square and t tests. 478 PHCPs participated; 49.4 % were FPs and 50.6 % FHPs. Of the participants, 86.6 % stated that they performed CRC screening on patients. The level of participants knowing that screening should start at age 50 and conclude at age 70 was 49.7 %. The level of subjects requesting the fecal occult blood test (FOBT) at the correct intervals was 29.7 %, but only 6.9 % recommended colonoscopy at the correct intervals. Additionally, 18.2 % of subjects knew that the test used is immunochemical FOBT, and 60.5 % reported not using reminders. PHCPs' low levels of knowledge, awareness and advice compatible with guidelines concerning CRC screening may represent an obstacle to such screening. Barriers perceived by PHCPs include patients' inability to access definite medical information, deficiencies in the reminder system and patients' lack of interest in CRC screening. Additions to the screening program will be useful in overcoming barriers.

Aim: To determine the perception of the term schizophrenia among university students., Methods: This cross-sectional study was performed in April 2015 with students from Canik Başarı University (Samsun/Turkey). A patient history was first established. We then investigated to what extent students agreed with 10 statements based on that patient history. Three separate questionnaire forms (versions A, B and C), differing only in terms of the diagnosis in the patient in the history, were prepared. The three diagnoses were 'Schizophrenia' (version A), 'A psychiatric disease by the name of Bleuler's syndrome' (version B) and 'Brain tumor' (version C). The questionnaires were administered in a class environment. In all, 771 students participated., Results: Statistically significant differences between the forms were determined in only two statements ('A.'s disease will represent a problem in A.'s future career' and 'A. will in all probability have problems with the law in the future'). While no difference was determined between versions A and B at two-way comparisons, a statistically significant difference was observed between versions A and B and version C., Conclusion: No difference was determined between students' attitudes toward a diagnosis of 'schizophrenia' and one of 'a psychiatric disease known as Bleuler's syndrome'. The focus in preventing stigmatization of schizophrenia should not concentrate on a name change alone. Changing the name schizophrenia may be of no use unless public ignorance and fear of psychiatric diseases can also be overcome., (© The Author(s) 2015.)
