4. P-219 Does artificial shrinkage prior to fresh blastocyst transfer improve ongoing pregnancy rate? A prospective double blind randomized controlled trial

Study question Does artificial shrinkage (AS) of blastocoelic cavity (BC) prior to fresh elective single blastocyst transfer (SBET) increase ongoing pregnancy rate? Summary answer Ongoing pregnancy rates were similar with or without AS of blastocoelic cavity. AS does not seem to provide benefit before fresh blastocyst transfer. What is known already AS of the vitrified blastocysts enhances success rate in frozen embryo transfer cycles. In vitro cultured embryos suffer changes in temperature, pH and osmotic pressure. Moreover, manipulations such as pipetting, fertilization, microscopic observations and changes of dishes can induce oxidative stress and apoptosis. The presence of cell free DNA (cfDNA) in blastocyst fluids could be the consequence of its release from dead cells. The quantity of cfDNA in blastocyst fluids could possibly be related to the rate of cell death. It is thus interesting to estimate whether AS of BC could improve the implantation rate in cycles with fresh blastocyst transfer. Study design, size, duration Prospective, randomized, double blind controlled study. From May 20th 2018 to June 30th 2021, 150 couples elected for fresh SBET were included in the study and were randomly selected as “AS +” group (n = 100), where AS of blastocoel was performed by laser pulse before fresh blastocyst transfer, and “AS -” group (n = 50), where fresh blastocysts were transferred without any additional intervention. Participants/materials, setting, methods On day 5 after fertilization, one blastocyst with a grade of expansion B3, B4, B5 or B6 and type A or B quality trophectoderm (Gardner and Schoolcraft classification, 1999) was selected for transfer. After replacement, the droplet that contained the embryo from day 3 was collected for cfDNA level quantification. Ongoing pregnancy rate was determined by the visualization of a gestational sac with a foetal heartbeat 6 weeks later and cfDNA was assessed by ALU-qPCR. Main results and the role of chance The two groups were similar for age, BMI, infertility duration and cause, stimulation characteristics and embryological parameters. The global ongoing pregnancy rate per transfer after SBET was 49.7 %. The pregnancy rate in the AS + group was slightly higher than in the control group but not significantly (respectively 50.00 % and 48,9 %, p = 0,91). cfDNA median value in the AS+ group was comparable to the control group (0.493 (0.219; 0.915) mg/ml and 0.595 (0.271; 1.129) mg/ml respectively (p = 0.45)). No link was found between cfDNA rate and clinical pregnancy rate. Limitations, reasons for caution Patients included in the study are still followed to evaluate the impact of AS on live birth rate, wastage rate, obstetrical and neonatal complications. cfDNA rate was evaluated in spent culture media and not by blastocentesis, which could provide a more accurate quantification. Wider implications of the findings To our knowledge, this is the first prospective randomized controlled study assessing the benefit of AS of BC before fresh blastocyst transfer. The inclusion of live birth rate is crucial to ascertain the interest of this technique and more studies are needed to improve the use of cfDNA in routine. Trial registration number NCT02988544

This review intends to introduce the changes of the new Bioethics law in the reproductive field and its application in French ART centers.The review details the main provisions of the Bioethics Law of August 2nd 2021 as well as the three decrees published since: the first one on September 29th 2021, which specifies in particular the age conditions to benefit from ART and self-preservation of one's gametes; another decree on December 31st, 2021, to set the terms and conditions for gamete self-preservation activities for non-medical reasons and the last decree on April 14th 2022, relating to the allocation of donated gametes and embryo donation.Since the law of August 2nd, 2021, access conditions to assisted reproductive technology (ART) have evolved in France. Previously based on pathological infertility or the risk of transmission of a serious disease, ART is now intended to respond to the parental project of a couple formed by a man and a woman, two women or an unmarried woman. With the widening of access conditions, the use of gamete donation will likely increase. The upcoming raise of children born from gamete donation has led the legislator to question their right to access their origin. From September 1st 2022, adults born from gamete donation will be able to request a special administrative authority in order to access the donor's non identifying data (age, physical characteristics, family and professional situation, motivation for the donation…) and/or the donor's identity. Moreover, the new bioethics law opens up the possibility of autologous gamete cryopreservation without medical reasons, under specific age conditions, in order to carry out an assisted reproduction technique later. If gametes are not used, autologous gamete preservation could also allow an increase in gamete donation. However, the modification of gamete donation conditions could suggest a short term decrease in donors' number. Finally, the new bioethics law further opens up research on human embryos and embryonic stem cells.The arrangements introduced by the Bioethics Law promulgated on August 2nd, 2021 represent a major revolution in the field of Reproductive Medicine and are expected to transform the practices of reproductive biology centers and CECOS in France.

7. [HLA-C KIR interactions and placental defects: Implications in ART pregnancy issues]

The aim of this review is to update data concerning the impact of HLA-C KIR system on placental disorders and assess the involvement on ART clinical outcomes.Ensuring the maintenance of human pregnancy requires the set up of immunological tolerance to prevent foetus rejection. This phenomenon involves different actors of the immune system: among them, uterine NK cells (uNK) hold specific KIR (killer-cell immunoglobulin-like) receptors linking to HLA molecules on the surface of trophoblastic cells at implantation. Many studies provided evidence that the specific interaction between maternal KIR and foetal HLA-C could influence the process of placentation; according to the KIR haplotype and the type of HLA-C, the interaction could be detrimental for placental function. We reviewed the latest data available regarding HLA-C KIR interactions and ART outcomes.The available results highlight a significant increase of preeclampsia risk and recurrent miscarriages when the maternal inhibitory haplotype KIR AA is present, this risk is all the more enhanced when the interaction occurs with foetal HLA-C2. Recent data suggest the consequences of this detrimental interaction in case of DET (double embryo transfer) or use of donor's oocytes in ART practice. On the other hand, maternal KIR AB or BB haplotypes haven't been related to an additional obstetrical risk, as well as the foetal HLA-C1 homozygous allotype.Despite the existence of many confoundings in current literature on the subject, interaction between maternal KIR and foetal HLA-C represent a promising target lead to broaden the spectrum of placental defects etiologies, especially in the reproductive health area.

Endocrine disruptor chemicals (EDCs) are ubiquitous contaminants in the environment, wildlife, and humans. During the last 20 years, several epidemiological, clinical and experimental studies have demonstrated the role of EDCs on the reduction of male and female fertility. The concept of foetal origins of adult disease is particularly topical in the field of reproduction. Moreover, exposure to EDCs during pregnancy has been shown to influence epigenetic programming of endocrine signalling and other important physiological pathways, and provided the basis for multi- and transgenerational transmission of adult diseases. However, the large panel of EDCs simultaneously present in the air, sol and water makes the quantification of human exposition still a challenge. Gas chromatography coupled with mass spectrometry, the measurement of total plasmatic hormonal bioactivity on stably transfected cell lines as well as the EDC analysis in hair samples are useful methods of evaluation. More recently, microRNAs analysis offers a new perspective in the comprehension of the mechanisms behind the modulation of cellular response to foetal or post-natal exposure to EDCs. They will help researchers and clinicians in identifying EDCs exposition markers and new therapeutic approaches in the future.

9. [Cesarean scare niche: Definition, diagnosis, risk factors, prevention, symptoms, adverse effects, and treatments]

To review the definitions, diagnostic methods, risk factors, symptoms, and treatments for caesarean scar niche.Review of the literature, critical reflection, and pragmatic advice.There is no consensus on the definition of caesarean scar niche. Some suggest an indentation≥2mm of the myometrium of the caesarean scar, but this is present in more than half of women with caesarean history and takes no account of woman's symptoms. The most popular diagnostic method is ultrasound±hysterosonography. Risks factors for niche are multiple Caesareans, Cesarean during labor with too low incision, and retroverted uterus. Symptoms include abnormal gynaecologic bleeding and pelvic pain, and their presence establish the "Caesarean scar syndrome". The risks of pregnancy with niche is poorly studied, but pregnancy is not contraindicated, even if the niche is untreated. The treatment of caesarean scar niche is mainly surgery and conservative. The former should be reserved for symptomatic patients, and those with secondary infertility and fertility treatment failure. Patients with residual myometrium thickness≥2.5mm may benefit from first-line hysteroscopic treatment, whereas a laparoscopic or vaginal approach could be offered in other cases.A pragmatic definition of caesarean scar niche as a disease including symptoms is the necessary prerequisite for the management of women. The treatment is mainly surgical, or conservative depending on the desire for subsequent pregnancy.

International audience; Infertility affects between 8 and 12% of reproductive-age couples worldwide. Despite improvements in assisted reproductive techniques (ART), live birth rates are still limited. In clinical practice, imaging and microscopy are currently widely used, but their diagnostic effectiveness remains limited. In research, the emergence of innovative techniques named OMICS would improve the identification of the implantation window, while progressing in the understanding of the pathophysiological mechanisms involved in embryo implantation failures. To date, transcriptomic analysis seems to be the most promising approach in clinical research. The objective of this review is to present the results obtained with the different approaches available in clinical practice and in research to assess endometrial receptivity in patients undergoing ART.; L’infertilité touche entre 8 et 12 % des couples en âge de procréer à travers le monde. Malgré l’amélioration des techniques d’aide médicale à la procréation (AMP), les taux de naissances vivantes restent toujours limités. En pratique clinique, l’imagerie et la microscopie sont actuellement largement utilisées, mais leur efficacité diagnostique reste limitée. En recherche, l’émergence des techniques innovantes dites OMICS permettrait d’améliorer l’identification de la fenêtre implantatoire, tout en progressant dans la compréhension des mécanismes physiopathologiques impliqués dans les échecs d’implantation embryonnaire. À ce jour, l’approche transcriptomique semble être la piste la plus prometteuse en recherche clinique. L’objectif de cette revue est de faire un état des lieux des résultats obtenus à ce jour avec les différentes approches disponibles en pratique clinique et en recherche pour évaluer la réceptivité endométriale chez les patientes prises en charge en AMP.

Desde su inicio en la decada de 1980, la fecundacion in vitro (FIV) convencional ha experimentado un progreso real con una gran expansion en todos los continentes. Hasta la fecha, se han logrado mas de 5 millones de embarazos en todo el mundo despues de la FIV convencional, que actualmente representa el 1-3% de los nacimientos en los paises desarrollados. Aunque los principios basicos de la FIV no han cambiado fundamentalmente, la llegada de nuevas tecnicas ha permitido una evolucion constante de las tecnicas de reproduccion asistida. Asi, la inyeccion intracitoplasmica de un espermatozoide (ICSI) ha llevado a un progreso considerable en el tratamiento de la infertilidad masculina. Ademas, los avances recientes en la criopreservacion, sobre todo gracias al desarrollo de la congelacion ultrarrapida (vitrificacion), han permitido una mejora significativa en las tasas de exito despues de la conservacion de ovocitos o embriones, permitiendo el uso de rutina de cultivos prolongados y la estrategia freeze-all. Ademas, la vitrificacion de los ovocitos ha abierto un nuevo campo para retener los ovocitos y preservar la fertilidad de las mujeres. Sin embargo, algunas situaciones, como los fracasos repetidos en la implantacion, la respuesta ovarica deficiente o la anomalia uterina, todavia no tienen una respuesta clara. Estas limitaciones de la tecnica son objeto de una intensa investigacion. En el contexto de la sociedad actual, surgen nuevos desafios, como el deseo de embarazo tardio o el tratamiento de la infertilidad en el contexto de la obesidad o el tabaquismo. Todos estos procesos estan siempre muy enmarcados en el nivel etico y legal para evitar posibles irregularidades.

12. [Comparison of birth weights of children born after slow frozen embryo replacement versus fresh embryo transfer]

It is already known that children born after slow frozen embryo replacement have a significantly higher birth weight compared to children born after fresh embryo transfer. Similar data have been reported related to frozen embryo transfer using an open vitrification system. However, few data relative to birth weight using a complete embryo closed vitrification system has been reported. The purpose of this study was to know if frozen embryo in closed vitrification system is associated with a higher birth weight compared to fresh embryo replacement.This was a monocentric retrospective cohort study, 371 children were issued from fresh embryo replacement and 127 from vitrified embryo transfer.All singletons born after fresh or vitrified embryo transfer between January 2011 and April 2015 were included. Births from the vitrified or fresh transfers of egg or sperm donation, and preimplantation genetic diagnosis were excluded. In addition, pregnancies with more than one gestational sac at the first ultrasound were excluded. An analysis of covariance (ANCOVA) was used for multivariate analysis.Mean birth weight was 205g higher in the frozen embryo compared with fresh embryos transfer groups (3368g vs. 3163g respectively, P0.001). This difference remained after multivariate analysis adjusted on confounding factors such as gestational age, maternal age, maternal body mass index (BMI), tobacco exposure, number of embryo transferred and birth order (P0.001)..Embryo frozen in closed vitrification system is associated with a higher birth weight compared to fresh embryo replacement. Our findings are consistent with the previous studies related to slow freezing and open vitrification systems data. The effects of controlled ovarian stimulation (COS), ex vivo culture conditions and cryopreservation systems on birth weight of children born should be further explored.

Introduction: Weight loss amongst women with polycystic ovary syndrome (PCOS) is crucial to reduce the risk of endocrine, reproductive and metabolic complications including hirsutism, menstrual disturbances and cardiovascular disease. With approximately 50% of women with PCOS being overweight or obese, effective dietary management of weight in PCOS is essential. However, there is inconsistent evidence as to whether specifically modified diets (e.g. reduced carbohydrate diets) are more effective at achieving weight loss amongst women with PCOS than are conventional healthy hypocaloric diets. Material and Methods: A systematic review and meta-analysis of randomized controlled trials that had compared weight and BMI between women with PCOS who had undergone either a specifically modified diet or a conventional healthy hypocaloric diet were performed. Six electronic databases were searched, a manual search of the reference lists of the included studies was carried out and authors were contacted for additional information. Nine studies with a total of 395 participants (all with a body mass index [BMI] ≥30) were included in the meta-analysis. The effect size used was the mean difference in post-intervention weight and BMI between participants who had undergone a specifically modified diet and participants who had undergone a conventional healthy hypocaloric diet. Results: There were no differences between groups in post-intervention weight (mean difference 1.26, 95% confidence interval (CI) -0.92 to 3.43, p = .26; heterogeneity I2 = 50%, p = .04) or BMI (mean difference 0.15, 95% CI -0.93 to 1.23, p = 0.79; heterogeneity I2 = 44%, p = 0.10). Subgroup analyses according to the presence of a dietary run-in period (a period at the start of the study during which all participants are placed on an identical diet in order to equalize them on variables influenced by diet), intervention duration and type of diet and a sensitivity analysis according to study quality were not significant. Conclusions: Whilst the results should be interpreted in light of the moderate heterogeneity observed, they suggest that specifically modified diets offer no added benefit for weight loss in women with PCOS over conventional healthy hypocaloric diets. The findings of this meta-analysis may promote the unification of guidelines for the dietary management of PCOS and allow clinicians to be confident in prescribing conventional healthy hypocaloric diets for weight loss amongst their PCOS patients.

Objectifs L’infection par l’hpv a fait l’objet d’etudes aux resultats contradictoires concernant la survenue d’alteration du spermogramme, de fausses couches spontanees, de naissances avant terme et de diminution des taux de reussite en amp. La nature des genotypes d’hpv qui pourraient etre impliques dans des alterations du conceptus est encore debattue. Methodes Etude de cohorte, prospective, multicentrique, incluant 350 couples. Objectif principal Etudier l’association entre la presence d’une infection a hpv chez l’un des membres du couple (col uterin/sperme) et l’issue des grossesses obtenues en procreation medicalement assistee. La reussite de l’amp etait definie par l’obtention d’une grossesse aboutissant a la naissance d’un enfant viable et vivant. Objectifs secondaires Etudier les alterations du spermogramme, les genotypes specifiques d’hpv impliques, un lien potentiel entre hpv et les caracteristiques embryonnaires ainsi que la morbidite fœtale. L’analyse a ete realisee grâce a une modelisation avec ajustement sur un score de propension. Resultats Duree moyenne d’infertilite : 4,2 ± 2 ans. Moyenne d’âge des femmes 33 ± 4 ans, des hommes 36 ± 6 ans. Prise en charge : insemination intra-uterine dans 1,5 %, fiv dans 36,2 % et icsi dans 62,4 % des cas. Au moins un hpv etait present chez 26,9 % chez les femmes et 14,43 % chez les hommes. Un enfant vivant est ne dans 21,5 % des cas de meres hpv positives et dans 27,4 % de meres negatives (p = 0,3). Un enfant vivant est ne dans 36,6 % des cas de peres hpv positifs et dans 28,2 % de peres negatives (p = 0,2). Les couples exposes ont donne naissance dans 23,2 % a des enfants vivants et les couples non exposes dans 30,2 % des cas (p = 0,2). Vingt-cinq genotypes d’hpv ont ete trouves, les plus frequemment representes etaient a haut risque oncogene (16, 31, 51, 52, 53). Conclusion La presence d’hpv dans le sperme chez l’homme et/ou au niveau de l’endocol chez la femme ne constituait pas un facteur de risque d’echec en amp.

Summary The effect of human follicular fluid (hFF) on the cholesterol and phospholipid content and the movement characteristics of human spermatozoa were studied. Semen was selected by a discontinuous Percoll gradient and incubated during in vitro capacitating conditions with B2 medium supplemented with hFF 20%. Percoll pelleted spermatozoa were incubated in either B2 (B2-Percoll) or B2 supplemented with hFF (hFF-Percoll). In hFF-Percoll, we observed a time-dependent (24 h) decrease in both the cholesterol and phospholipid contents (cholesterol: 10.1 vs. 8.7 nmol 10-7 spermatozoa; phospholipids: 17.5 vs. 15.7 nmol 10-7 spermatozoa, P < 0.05). This decrease in cholesterol and phospholipids in human spermatozoa was concomitant with a high straight line velocity, a high progressive motility percentage and an increased value of lateral head displacement without any significant alteration of the spermatozoal membrane. No modification of the cholesterol: phospholipid ratio after 2 and 24 h of incubation in either B2-Percoll (0.61, 0.54) in hFF-Percoll (0.59, 0.63) was observed when compared with original control semen. It is suggested that the decrease in cholesterol and phospholipids in hFF-Percoll may be taken into account for the changes of membrane modification as part of the capacitation process.

- Abstract
En el 40-50% de las parejas que consultan por infertilidad se encuentra una enfermedad tubarica distal. La afectacion de la ampolla y la trompa provoca una modificacion de las relaciones tuboovaricas y una alteracion de la captura y la migracion del ovocito. En la actualidad, existen dos posibilidades terapeuticas para esta infertilidad tubarica: la cirugia reparadora mediante laparoscopia y la fecundacion in vitro. El tratamiento quirurgico consiste en una adherenciolisis, una fimbrioplastia o una neosalpingostomia, segun el tipo de afectacion tubarica. Las tasas de embarazo despues del tratamiento quirurgico reconstructivo son de alrededor del 30-40%. Es indispensable realizar una seleccion rigurosa de las pacientes a partir de los datos histerograficos y laparoscopicos para determinar las que sean candidatas a una cirugia reparadora o a una fecundacion in vitro. La valoracion de la trompa y sobre todo de la mucosa tubarica durante la laparoscopia es uno de los elementos fundamentales para tomar la decision terapeutica y en el pronostico de embarazo espontaneo. La cirugia es un complemento de la fecundacion in vitro y el objetivo consiste en combinar ambas posibilidades terapeuticas para mejorar la tasa de embarazo intrauterino.

The decision to offer ICSI is based on the quality of the semen, and the presence of severe oligospermia or azoospermia. In the case of severe asthenospermia, a second sample may be requested. In the case of complete asthenospermia, the fertilisation rate can be increased if the ICSI is carried out with spermatozoa selected using HOST (hypoosmotic swelling test). Not all morphological abnormalities have the same repercussions. Certain abnormalities should be excluded, in particular globozoospermia and macrocephalia. Secretory azoospermia may be known about or diagnosed on the first day of treatment, hence the advantage of freezing emergency straws in the event of severe oligoasthenoteratospermia. Meticulous microscopic observation of the ejaculate can allow the identification of occasional and even motile spermatozoa. Female infertility is no longer an indicator for ICSI treatment. In the event of IVF failure with normal sperm, switching to ICSI depends on oocyte quality and maturity. Regarding technical implementation of ICSI, equipment problems are as varied as the equipment is complex. It is conducted outside a sterilizer, thus requiring maintenance of the temperature, pH and sterility. Certain questions remain unanswered. Patients should be informed of the limited chances of success from ICSI, with high instances of aneuploidy and low pregnancy rates.

International audience; Circulating nucleic acids (cell-free DNA and microRNAs) have for particularity to be easily detectable in the biological fluids of the body. Therefore, they constitute biomarkers of interest in female and male infertility care. Indeed, in female, they can be used to detect ovarian reserve disorders (polycystic ovary syndrome and low functional ovarian reserve) as well as to assess follicular microenvironment quality. Moreover, in men, their expression levels can vary in case of spermatogenesis abnormalities. Finally, circulating nucleic acids have also the ability to predict successfully the quality of in vitro embryo development. Their multiple contributions during assisted reproductive technology (ART) make of them biomarkers of interest, for the development of new diagnostic and/or prognostic tests, applied to our specialty. Circulating nucleic acids would so offer the possibility of personalized medical care for infertile couples in ART.

International audience; OBJECTIVE:This study intended to compare frozen embryo transfer (FET) outcomes at blastocyst stage according to freezing methods, slow freezing versus vitrification and according to the type of endometrial preparation.PATIENTS AND METHODS:A total of 172 FET at blastocyst stage (day 5 or 6) were included retrospectively from April, 2007 to December, 2012. The FET outcomes from slow freezing (group 1, n=86) were compared with those from vitrification (group 2, n=86). More particularly, the survival rate after thawing, as well as implantation and pregnancy rates (clinical and ongoing pregnancy rates) were compared respectively between these two groups, after matching on women's age at freezing day, embryo number and embryo development stage for transfer. Resume Cette communication evalue les differents parametres morphologiques des ovocytes, zygotes et embryons concernant leur caractere predictif sur les chances de succes en FIV. Les dysmorphies ovocytaires ne semblent pas avoir un effet deletere sur la qualite embryonnaire et les taux de grossesse en FIV/ICSI. La multinucleation des zygotes est associee a une diminution des taux de grossesse et d’implantation dans les cycles de FIV avec transfert embryonnaire. Une fragmentation importante des blastomeres (> 25 %) est associee a de tres faibles taux de grossesse ( L’expression d’HLA-Gs dans le milieu de culture embryonnaire semble etre correlee a de meilleurs taux de grossesse en FIV.

International audience; Embryo selection based exclusively on embryo morphological criteria does not allow currently to obtain satisfactory implantation rates. So, new time-lapse systems have been proposed to improve the rates of in vitro fertilization success. However, the profit/investment ratio of time-lapse systems and the interest of embryo morphokinetics evaluation remain clearly to be established by clinical, robust, prospective, and randomized trials. Consequently, morphological and morphokinetic approaches showed their limitations and justify the development of new non-invasive "omics" approaches. These approaches are very promising and have for main aim to identify new non-invasive biomarkers, in oocyte microenvironment and/or in embryo culture medium, predictive of oocyte and/or embryo quality as well as pregnancy rates. These approaches thus open the way to develop new diagnostic and/or prognostic tests for embryo viability based on the expression and/or the quantification of biomarkers of interest in assisted reproductive technology.

During the last years, the use of circulating nucleic acids (microRNAs and cell-free DNA) as diagnostic and/or prognostic tools in cancerology was widely documented. Likewise, in obstetrics and gynecology, the development of non-invasive prenatal testing based on the assessment of these biomarkers confirmed their growing interest in this speciality. In human reproduction, several studies were interested in the microRNAs, small non-coding RNA sequences, present in the ovarian follicle and their implication in folliculogenesis. Some of these microRNAs, as well as the vesicles which transport them, are easily detectable in the bloodstream and could be used as reliable biomarkers of interest in infertility care. Cell-free DNA level varies according to physiopathology and reflect the proportion of apoptotic and/or necrotic events occurring in the body. As a result, its quantification could give an additional help to the practitioners for ovarian functional status evaluation. Furthermore, these circulating nucleic acids could also constitute new predictive biomarkers of oocyte and/or embryo quality and represent a promising perspective for the prevention of in vitro fertilization implantation failures. In conclusion, these circulating nucleic acids open the way to the development of new diagnostic and/or prognostic innovative tests in order to improve in vitro fertilization outcomes.

Malgre de multiples essais, le traitement medical de l’infertilite masculine reste decevant. A l’exception des traitements antibiotiques ou anti-inflammatoires, le traitement par les gonadotrophines dans le cadre des insuffisances gonadotropes demeure la seule indication medicale de l’infertilite masculine. L’assistance medicale a la procreation (AMP) constitue donc le plus souvent le seul recours au traitement de l’infertilite masculine. Les caracteristiques du sperme definies comme convenables pour obtenir la fecondation ont ete determinees statistiquement: pour la fecondation naturelle les normes definies par l’OMS en 1992 sont: numeration egale ou superieure a 20 millions par ml, mobilite progressive superieure ou egale a 50% a la lere heure, morphologie presentant au moins 30% de formes normales. Les techniques d’AMP ne representent passtricto sensu un traitement curatif de l’infertilite masculine mais une demarche medicale permettant aux couples infertiles de realiser leur projet parental. Les techniques de preparation du spermein vitro en vue d’une AMP ont pour but de selectionner des spermatozoides mobiles a morphologie normale, d’eliminer le plasma seminal ainsi que les debris cellulaires et les spermatozoides morts mais aussi d’induire la capacitation des spermatozoides. Le choix entre les differentes techniques d’AMP disponibles repose essentiellement sur le nombre de spermatozoides mobiles recueillis apres preparationin vitro.

L'injection intracytoplasmique par ICSI d'un gamete immature (spermatide) a ete realisee dans plusieurs especes animales (souris, hamster, lapin, et bovin), ainsi que chez l'homme. Le recours a des spermatides pour ICSI concerne des patients chez lesquels aucun spermatozoide n'a ete observe dans l'ejaculat ou dans le testicule. Vanderzwalmen et al (1995) rapportaient chez l'homme la fecondation d'ovocytes et le clivage embryonnaire par l'injection en ICSI de spermatides allongees. Cela a ete confirme par l'obtention de quelques naissances a la suite de l'injection ovocytaire de spermatides rondes ou allongees extraites de l'ejaculat ou du testicule. Mais, aujourd'hui, l'utilisation de spermatides en ICSI demeure tres controversee. Le succes de I'ICSI avec une spermatide, son identification, ainsi que les problemes poses, sont l'object de cette revue.
