Abstract

Der Bestand von Rotmilanen (Milvus milvus) hat in der Schweiz während den letzten Jahren stark zugenommen. In anderen Ländern wie Deutschland, Frankreich und Spanien sind die Bestände gleichgeblieben oder haben sogar abgenommen. Die Gründe für die Zunahme sind nicht zweifelsfrei geklärt. Jagdverbote und eine Einschränkung von schädlichen Pestiziden haben den Beständen sicherlich geholfen. Der Rotmilan ist ein Kurzstreckenzieher und überwintert normalerweise auf der Iberischen Halbinsel und Südfrankreich. In den letzten Jahren überwintern aber immer mehr Tiere in ihrem Brutgebiet, so auch in der Region Wädenswil im Kanton Zürich. Möglicherweise auf Grund von Fütterungen durch Menschen und den zunehmenden schneefreien Wintern, müssen sie nicht ziehen. Die Frage stellt sich, ob es einen Zusammenhang zwischen den Vorkommen des Rotmilans und den Fütterungen in Wädenswil gibt. Mit einer wöchentlichen extensiven Zählung wurde ermittelt, dass über den Herbst/Winter 2018 sich durchschnittlich 16 Rotmilane pro Begehung im Untersuchungsgebiet befinden. Im November erreichte die Zählung Spitzen von bis zu 68 Tieren, wobei Doppelzählungen nicht ausgeschlossen sind. Mittels intensiver Zählung wurde das Aktivitätsmuster von einzelnen Tieren beobachtet. Es stellte sich heraus, dass Rotmilane zwei Drittel der Beobachtungszeit mit Ruhen verbringen. Mit einer Umfrage in der lokalen Bevölkerung wurde untersucht, wo die Rotmilane gefüttert werden. In der Nähe dieser Fütterungsstandorte haben sich die Tiere auf Schlafbäumen zusammengefunden. Zu guter Letzt wurde ein Anfütterungsversuch durchgeführt. Dabei wurde Fleisch aus ausgelegt und die Zeit gemessen, bis der erste Rotmilan das Futter entdeckte. Es dauerte zwei Stunden, bis ein Rotmilan vom Fleisch frass, nachdem schon ein Mäusebussard (Buteo buteo) gefressen hatte., The population of red kites (Milvus milvus) in Switzerland has increased sharply in recent years. In other countries, such as Germany, France and Spain, populations have remained stable or even decreased. The reasons for the increase are not clear. Hunting bans and restrictions on harmful pesticides have certainly helped the populations. The red kite is a shortdistance migrant and usually hibernates on the Iberian Peninsula and in southern France. In recent years, however, more and more animals have hibernated in their breeding area, including in the region of Wädenswil of Canton Zurich. Possibly due to feeding by humans and the increasing snow-free winters, they do not have to migrate. The question arises as to whether there is a connection between the occurrence of the red kite and feeding in Wädenswil. An extensive weekly census showed that an average of 16 red kites were present in the study area over the autumn/winter of 2018. In November the census reached peaks of up to 68 animals, but double censuses are not excluded. The activity pattern of individual animals was observed by means of intensive censuses. It turned out that red kites spend two thirds of the observation time resting. A survey of the local population investigated where the red kites were fed. Near these feeding sites the animals gathered on sleeping trees. Last but not least, a feeding experiment was carried out. Meat was laid out and the time measured until the first red kite discovered the food. It took two hours for a red kite to eat from the meat after a buzzard (Buteo buteo) had eaten.

Abstract

Hepatitis E virus (HEV) is a common cause of acute hepatitis worldwide. In Europe, HEV is a zoonosis transmitted via contaminated pork meat or other pork food products. Genotype 3 is the most prevalent HEV type in the animal reservoir, as well as in humans. Despite an increased incidence of hepatitis E across Europe, much remains unknown about its spread, sources and transmission routes. A One Health approach is crucial to better understand the (molecular) epidemiology of HEV. HEVnet was established in April 2017 as a network and database for sharing sequences and accompanying metadata collected from human, animal, food and environmental sources. HEVnet members working in the public health, veterinary health, food, environmental and blood safety sectors have submitted 1,615 HEV sequences from nine countries as at January 2019. Most are from humans (89%), and sequences of animal (5%), food (6%) or environmental (0.3%) origin are rare. Metadata for human sequences capture mostly sex (93%), year of birth (92%) and sampling (100%); data on region of sampling (37%) and clinical information (hospitalisation 27%, symptoms 20% or mortality 8%) are limited. HEVnet aims to expand into a global network capable of performing cross-sectoral and supranational studies, with a joint repository of molecular and epidemiological data on HEV.

21. Human papilloma virus-associated squamous cell carcinoma of the larynx in an 18-year-old woman

BACKGROUND Human papilloma virus (HPV)-associated malignancies are considered to be sexually transmitted diseases. METHODS We report a HPV-positive larynx cancer in an 18-year-old female clarinet player, despite vaccination with the quadrivalent HPV-6-11-16-18-vaccine Gardasil (Merck Sharp & Dohme Corp., West Point, Pennsylvania). The patient showed no evidence of genito-oral infection but showed some evidence for oral-oral HPV transmission through the sharing of saliva-infested clarinet mouthpieces. A right vocal cord lesion of benign appearance was removed via free margin resection. RESULTS Histopathology revealed a microinvasive squamous cell carcinoma inside a zone of high-grade dysplasia that was positive for HPV-45. No tumor recurrence was observed during a 4-year follow-up evaluation. CONCLUSION Benign lesion appearance and quadrivalent HPV vaccine status do not exclude HPV-associated malignancies. In our patient, the Gardasil vaccine did not provide crossover protection against HPV 45 infection. HPV-associated disease may not necessarily be transmitted via sexual practice patterns alone.

24. Variable viral clearance despite adequate ganciclovir plasma levels during valganciclovir treatment for cytomegalovirus disease in D+/R- transplant recipients

Abstract

Abstract Background Valganciclovir, the oral prodrug of ganciclovir, has been demonstrated equivalent to iv ganciclovir for CMV disease treatment in solid organ transplant recipients. Variability in ganciclovir exposure achieved with valganciclovir could be implicated as a contributing factor for explaining variations in the therapeutic response. This prospective observational study aimed to correlate clinical and cytomegalovirus (CMV) viral load response (DNAemia) with ganciclovir plasma concentrations in patients treated with valganciclovir for CMV infection/disease. Methods Seven CMV D+/R- transplant recipients (4 kidney, 2 liver and 1 heart) were treated with valganciclovir (initial dose was 900-1800 mg/day for 3-6.5 weeks, followed by 450-900 mg/day for 2-9 weeks). DNAemia was monitored by real time quantitative PCR and ganciclovir plasma concentration was measured at trough (Ctrough) and 3 h after drug administration (C3h) by HPLC. Results Four patients presented with CMV syndrome, two had CMV tissue-invasive disease after prophylaxis discontinuation, and one liver recipient was treated pre-emptively for asymptomatic rising CMV viral load 5 weeks post-transplantation in the absence of prophylaxis. CMV DNAemia decreased during the first week of treatment in all recipients except in one patient (median decrease: -1.2 log copies/mL, range: -1.8 to 0) despite satisfactory ganciclovir exposure (AUC0-12 = 48 mg·h/L, range for the 7 patients: 40-118 mg·h/L). Viral clearance was obtained in five patients after a median of time of 34 days (range: 28-82 days). Two patients had recurrent CMV disease despite adequate ganciclovir exposure (65 mg·h/L, range: 44-118 mg·h/L). Conclusions Valganciclovir treatment for CMV infection/disease in D+/R- transplant recipients can thus result in variable viral clearance despite adequate ganciclovir plasma concentrations, probably correlating inversely with anti-CMV immune responses after primary infection.

Abstract Background Genital herpes is one of the most prevalent sexually-transmitted diseases, and accounts for a substantial morbidity. Genital herpes puts newborns at risk for very severe disease and also increases the risk of horizontal HIV transmission. It thus stands as an important public health problem. The recent availability of type-specific gG-based assays detecting IgG against HSV-1 and HSV-2 allows to establish the prevalence of each subtype. Worldwide, few data have been published regarding the seroprevalence in general populations of HSV-2, the major causative agent for genital herpes, while no data exist regarding the Swiss population. Methods To evaluate the prevalence of IgG antibodies against HSV-1 and HSV-2 in Switzerland, we used a population-based serum repository from a health examination survey conducted in the Western and Southern area of Switzerland in 1992–93. A total of 3,120 sera were analysed by type-specific gG-based ELISA and seroprevalence was correlated with available volunteers characteristics by logistic regression. Results Overall, seroprevalence rates were 80.0 ± 0.9% (SE, 95% CI: 78.1–81.8) for HSV-1 and 19.3 ± 0.9% (SE, 95% CI: 17.6–21.1) for HSV-2 in adults 35–64 year old. HSV-1 and HSV-2 seroprevalence increased with age, with a peak HSV-2 seroprevalence in elderly gentlemen, possibly a seroarcheological evidence of sexually transmitted disease epidemics during World War II. Risk factors for HSV-2 infection included female sex, marital status other than married, and size of town of residence larger than 1500 inhabitants. Unexpectedly and conversely to HSV-1, HSV-2 seroprevalence increased with educational level. HSV-2 infection was less prevalent among HSV-1 infected individuals when compared to HSV-1 uninfected individuals. This effect was most apparent among women at high risk for HSV-2 infection. Conclusions Our data demonstrate that by the early nineties, HSV-2 had spread quite largely in the Swiss population. However, the epidemiology of HSV-2 in Switzerland presents paradoxical characteristics, e.g. positive correlation with education level, that have not been observed elsewhere.

Abstract

Abstract: Background: Autochthonous hepatitis E is increasingly recognized as zoonotic infection in western countries. Serological assays have varying sensitivity and specificity. Methods: We implemented molecular testing to identify and characterize acute hepatitis E acquired in Switzerland. Results: Ninety‐three cases of mostly symptomatic acute hepatitis E acquired in Switzerland were documented by PCR between November 2011 and December 2016. Median HEV RNA was 7.5 x 104 IU/mL (range, 5.3 to 4.7 x 107 IU/mL). HEV genotyping was successful in 78 patients, revealing genotype 3 in 75 and genotype 4 in three patients. Phylogenetic analyses revealed a few limited geographical and temporal clusters. Of the 91 patients with available anti‐HEV IgM serology, four were negative; three of these were also IgG‐negative, likely as a result of immunosuppression, and one was IgG‐positive, a constellation compatible with HEV reinfection. Median age of the patients was 58 years (range, 20‐80 years); 71 (76.3%) were men and 49 of these (69.0%) were ≥ 50 years old. The clinical course was particularly severe in patients with underlying chronic liver disease, with fatal outcome in two patients. Six patients (6.5%) presented with neuralgic amyotrophy. Conclusions: Nucleic acid‐based diagnosis reveals HEV as a relevant cause of acute hepatitis in Switzerland. Middle‐aged and elderly men constitute the majority of symptomatic patients. Testing for HEV should be included early in the diagnostic workup of acute hepatitis and of neuralgic amyotrophy, a typical extrahepatic manifestation of HEV genotype 3 infection. Abstract

We analyzed changes in hepatitis B virus and hepatitis delta virus (HDV) viral loads (VL) during tenofovir-containing antiretroviral therapy among patients with a replicating HDV infection in the Swiss HIV Cohort Study. Only 28.6% experienced a ≥2.0 log reduction in HDV RNA, and 14.3% had undetectable HDV VL within 5 years. [ABSTRACT FROM AUTHOR]
