1. Thoracoabdominal approach to large adrenal tumors – when laparoscopic adrenalectomy is not enough: a retrospective four-year study

Abstract

Introduction: Laparoscopic adrenalectomy is considered to be the gold standard approach in the field of adrenal surgery. This technique offers advantages of great importance compared to variant laparotomy techniques. Notwithstanding, a technique that needs to be mentioned is the thoracoabdominal approach which facilitates the anatomic exposure of the retroperitoneum, adrenal gland, and great vessels and is strongly recommended for the surgery of large, malignant adrenal tumors. Aim: The objective of this study is to evaluate the effectiveness and outcomes of the thoracoabdominal approach in adrenal surgery for large adrenal tumors. By presenting our results and experiences, we aim to highlight the advantages of this technique in providing optimal anatomic exposure of the retroperitoneum, adrenal gland, and great vessels, and to establish its role as a viable alternative to laparoscopic adrenalectomy in complex cases. Materials and methods: We reviewed retrospectively the data of our patients admitted to our Surgical Department and treated surgically with the thoracoabdominal incision, performed by a single surgeon at our tertiary care center, over the period 01/01/20-31/12/23. Results: All patients had large retroperitoneal masses of varying complexity, requiring maximal surgical exposure. Seventeen patients in total underwent operation with the indication of unilateral adrenal tumor. Five of those patients underwent a laparoscopic tumor resection, while 12 patients were treated by thoracoabdominal approach. Our study group included 7 women (58%) and 5 men (42%), with average age 57 years. The mean maximum tumor diameter was 10.8 cm. Conclusion: The advantages of the thoracoabdominal approach regarding the exposure of the operative field in challenging cases, together with the low incidence of complications noted in our experience, render this technique an excellent alternative if indicated.

Abstract

Aortocaval fistulas, typically linked to aortoiliac aneurysms, are rare vascular conditions. Currently, endovascular therapy is preferred treatment approach. However, endovascular therapy requires frequent and extended follow-up evaluations. Herein, we emphasize a noteworthy clinical finding in this disease: the presence of a pulsatile femoral vein. Additionally, we propose a novel, simple, and easily performed radiation-free method for postoperative assessment of fistula presence or resolution: duplex ultrasound of the femoral vein. While duplex ultrasound cannot substitute other diagnostic modalities, this technique offers a quick, cost-effective, and repeatable adjunct for monitoring.

Abstract

Transposition of inferior vena cava, or, left-sided inferior vena cava (LS-IVC) is a rare clinical entity, in which the inferior vena cava ascends along the left side of the abdominal aorta. Literature contains mainly clinical case reports. Although it is usually not associated with clinical symptomatology, this anomaly should be detected during preoperative planning to avoid iatrogenic injuries intraoperatively. We present a case of left-sided inferior vena cava encountered during laparoscopic lymphadenectomy in a 45-year-old man with previous laparoscopic hemicolectomy due to colon adenocarcinoma. Preoperative CT abdomen revealed the left-sided location of infrarenal IVC and laparoscopic trans-peritoneal aortic lymphadenectomy was decided. Intraoperatively, transposition of inferior vena cava was confirmed in accordance with the CT findings. Resection of lymph node block was conducted with no complications and with minimal blood loss. The postoperative course was uneventful, and the patient was discharged from the hospital the day following surgery. In conclusion, transposition of the inferior vena cava, although rare, constitutes an anatomical variant that should be identified preoperatively to decrease intraoperative risks. Several anatomical variants have been associated with left-sided inferior vena cava.

Abstract

Abstract Background The incidence of the highly morbid and potentially lethal gangrenous cholecystitis was reportedly increased during the COVID-19 pandemic. The aim of the ChoCO-W study was to compare the clinical findings and outcomes of acute cholecystitis in patients who had COVID-19 disease with those who did not. Methods Data were prospectively collected over 6 months (October 1, 2020, to April 30, 2021) with 1-month follow-up. In October 2020, Delta variant of SARS CoV-2 was isolated for the first time. Demographic and clinical data were analyzed and reported according to the STROBE guidelines. Baseline characteristics and clinical outcomes of patients who had COVID-19 were compared with those who did not. Results A total of 2893 patients, from 42 countries, 218 centers, involved, with a median age of 61.3 (SD: 17.39) years were prospectively enrolled in this study; 1481 (51%) patients were males. Abstract

BackgroundFournier’s gangrene (FG) is a rapidly evolving necrotizing fasciitis of the perineum and the genital area, the scrotum as it most commonly affects man in the vast majority of cases. It is polymicrobial in origin, due to the synergistic action of anaerobes and aerobes and has a very high mortality. There are many predisposing factors including diabetes mellitus, alcoholism, immunosuppression, renal, and hepatic disease. The prognosis of the disease depends on a lot of factors including but not limited to patient age, disease extent, and comorbidities. The purpose of the study is to describe the experience of a general surgery department in the management of FG, to present the multimodal and multidisciplinary treatment of the disease, to identify predictors of mortality, and to make general surgeons familiar with the disease.MethodsThe current retrospective study is presenting the experience of our general surgery department in the management of FG during the last 20 years. The clinical presentation and demographics of the patients were recorded. Also we recorded the laboratory data, the comorbidities, the etiology, and microbiology and the therapeutic interventions performed, and we calculated the various severity indexes. Patients were divided to survivors and non-survivors, and all the collected data were statistically analyzed to assess mortality factors using univariate and then multivariate analysis.ResultsIn our series, we treated a total of 24 patients with a mean age 58.9 years including 20 males (83.4%) and 4 females (16.6%). In most patients, a delay between disease onset and seeking of medical help was noted. Comorbidities were present in almost all patients (87.5%). All patients were submitted to extensive surgical debridements and received broad-spectrum antibiotics until microbiological culture results were received. Abstract

Background The SARS-CoV-2 pandemic is still ongoing and a major challenge for health care services worldwide. In the first WSES COVID-19 emergency surgery survey, a strong negative impact on emergency surgery (ES) had been described already early in the pandemic situation. However, the knowledge is limited about current effects of the pandemic on patient flow through emergency rooms, daily routine and decision making in ES as well as their changes over time during the last two pandemic years. This second WSES COVID-19 emergency surgery survey investigates the impact of the SARS-CoV-2 pandemic on ES during the course of the pandemic. Methods A web survey had been distributed to medical specialists in ES during a four-week period from January 2022, investigating the impact of the pandemic on patients and septic diseases both requiring ES, structural problems due to the pandemic and time-to-intervention in ES routine. Results 367 collaborators from 59 countries responded to the survey. The majority indicated that the pandemic still significantly impacts on treatment and outcome of surgical emergency patients (83.1% and 78.5%, respectively). As reasons, the collaborators reported decreased case load in ES (44.7%), but patients presenting with more prolonged and severe diseases, especially concerning perforated appendicitis (62.1%) and diverticulitis (57.5%). Otherwise, approximately 50% of the participants still observe a delay in time-to-intervention in ES compared with the situation before the pandemic. Relevant causes leading to enlarged time-to-intervention in ES during the pandemic are persistent problems with in-hospital logistics, lacks in medical staff as well as operating room and intensive care capacities during the pandemic. This leads not only to the need for triage or transferring of ES patients to other hospitals, reported by 64.0% and 48.8% of the collaborators, respectively, but also to paradigm shifts in treatment modalities to non-operative approaches reported by 67.3% of the participants, especially in uncomplicated appendicitis, cholecystitis and multiple-recurrent diverticulitis. Conclusions The SARS-CoV-2 pandemic still significantly impacts on care and outcome of patients in ES. Well-known problems with in-hospital logistics are not sufficiently resolved by now; however, medical staff shortages and reduced capacities have been dramatically aggravated over last two pandemic years.

Abstract

Objectives: Complex abdominal wall reconstruction cases constitute a challenging issue, with high morbidity and mortality rates. Materials and methods: A young trauma patient presented abdominal dehiscence after multiple laparotomies. The fascial defect was managed with the use of a biological mesh, while initially primary wound closure was attempted. Due to cutaneous dehiscence, the use of negative pressure wound therapy was decided. Results: Granulation tissue formation was noticed on the eighth day while complete wound closure was achieved after 57 days. Conclusions:In conclusion, the combined use of biological mesh and negative pressure wound therapy is feasible in the management of complicated abdominal defects.

10. Anaplastic testicular seminoma presenting as a hydrocele, in a 36-year-old patient with a family history of anaplastic seminoma

Abstract

Testicular cancer is the solid tumor with the greatest incidence in men between 15 and 44 years old. Its main histological type is germ cell tumor, that is divided into seminomatous and non-seminomatous tumors. Seminoma, consisting 55% of testicular cancer, manifests in the fourth decade of men's life and a rare type of it is the anaplastic seminoma (5% to 15% of testicular seminoma). Diagnosis is based on clinical examination, testicular ultrasound, magnetic resonance imaging, tumor markers measurement and pathological examination, while treatment of choice is radical orchiectomy, with or without retroperitoneal lymphadenectomy, potentially followed by radiotherapy or chemotherapy. We present the case of a 36-year-old male patient, whose father suffered from anaplastic seminoma and visited the hospital due to a painless hydrocele. The testicle was swollen and hard on palpation, while cytological examination of the drained fluid detected neoplasm, potentially seminoma. Tumor markers measurement, as well as abdominal and pelvic computerized tomography scans, were evaluated and the patient was staged as IIA, according to the American Joint Commission on Cancer. Radical orchiectomy with high ligation of the seminal vesicle was performed and the pathological examination showed anaplastic testicular seminoma. Postoperatively, four cycles of chemotherapy with bleomycin, etoposide and platinum (BEP) were performed and no signs of recurrence were present after 1 year. In conclusion, anaplastic seminoma has a good prognosis and is suggested to be treated with radical orchiectomy, with or without retroperitoneal lymphadenectomy, potentially followed by radiotherapy or chemotherapy.

Abstract

Pancreaticoduodenectomy has been established as the treatment of choice for the management of benign and malignant lesions of the pancreatic head, and pancreaticojejunal or pancreaticogastric anastomosis seems to be the safer choice for the management of the pancreatic duct. However, in certain seldom, but still existing circumstances, pancreatic duct ligation or occlusion with a chemical substance is a valuable and viable alternative. The aim of the current study is to compare these two methods of occlusion of the pancreatic duct regarding the endocrine and exocrine function of the pancreas and its histopathology.18 health mixed breed dog of both sexes were randomized in two groups: group A, in which the pancreatic duct was ligated and group B, in which the pancreatic duct was occluded with prolamine.None of the animals presented postoperatively steatorrhea and significant body weight changes. Peripancreatic inflammation at sacrifice, inflammatory cell infiltration and edema of the pancreas on the 15th postoperative day and 30th postoperative day were milder after occlusion with prolamine than after ligation. Ligation of pancreatic duct lead to significantly greater hyperamylasemia than prolamine occlusion every day until the 15th. Mild hyperglycemia presented from the first to the fourth day in both groups, which was associated with a significant drop in insulin. Glucagon remained within the normal values at all times during the experiment. None of glucose, insulin and glucagon differed between groups.Prolamine occlusion of the pancreatic duct causes milder hyperamylasemia and less extensive inflammation both macroscopically and microscopically than ligation.Pancreas, Pancreaticoduodenectomy, Hyperamylasemia.La ppancreaticoduodenectomia è riconosciuta come il trattamento di scelta per la gestione delle lesioni benigne e maligne della testa del pancreas, e l’anastomosi pancreaticodigiunale o pancreaticogastrica sembra essere la scelta più sicura per la gestione del dotto pancreatico. Tuttavia, in alcune rare circostanze, ma ancora esistenti, la legatura o occlusione del dotto pancreatico con una sostanza chimica è un’alternativa preziosa e praticabile. L’infiammazione peripancreatica al sacrificio era significativamente inferiore dopo l’occlusione con prolamina rispetto a dopo la legatura. L’infiltrazione di cellule infiammatorie e l’edema del pancreas sono stati più lievi dopo occlusione con prolamina rispetto a dopo la legatura il 15° giorno postoperatorio e il 30° giorno postoperatorio, mentre la fibrosi e l’atrofia si sono sviluppate approssimativamente allo stesso grado in 3 mesi dopo l’intervento dopo la legatura e l’occlusione della prolamina del dotto pancreatico . Non sono stati osservati cambiamenti quantitativi e qualitativi negli isolotti di Langerhans in nessun gruppo. La legatura del dotto pancreatico porta a iperamilasemia significativamente maggiore dell’occlusione prolaminica ogni giorno fino al 15. L’iperglicemia lieve si è presentata dal primo al quarto giorno in entrambi i gruppi, che è stata associata a un calo significativo dell’insulina. In seguito sia il glucosio che l’insulina sono rimasti entro i limiti normali, mentre il glucagone è rimasto sempre entro i valori normali durante l’esperimento. Nessuno di glucosio, insulina e glucagone differivano tra i gruppi.L’occlusione della prolamina del dotto pancreatico provoca iperamilasemia più lieve e infiammazione meno estesa sia macroscopicamente che microscopicamente rispetto alla legatura.

Abstract

The aim of the current study was to evaluate the effect of ischemia-reperfusion injury on the liver's function and morphology during the establishment and progress of obstructive jaundice.80 Wistar rats were used for the purposes of the study and were allocated in four groups: JAUNDICE (obstructive jaundice), JAUN-ISC (obstructive jaundice and ischemia reperfusion), CONTROL (laparotomy) and ISCHEMIA (ischemia reperfusion).Obstructive jaundice, and ischemia-reperfusion injury following obstructive jaundice led to increased mortality, while no mortality was noticed in the control and ischemia groups. In the JAUN-ISC group, SGOT was significantly increased on the 10th day and SGPT was significantly increased on the 1st day compared to JAUNDICE group. Moreover, in the JAUN-ISC group, sinusoid dilation was significantly increased on the 5th and 10th days and neutrophil infiltration was significantly increased on the 10th day compared to the JAUNDICE group.A mild ischemia-reperfusion injury that in the normal liver led only to slight increase of hepatic neutrophil infiltration in the presence of obstructive jaundice led to increased hepatic biochemical markers (SGOT, SGPT) and increased hepatic sinusoid dilatation and enhanced neutrophil infiltration.Dilatation of sinusoids, Granulocytes infiltration, Oxaloxate, Pyruvate transaminase, Transaminase reperfusion.Con questa ricerca sperimentale si è voluto valutare l’effetto della lesione da ischemia-riperfusione sulla funzione e sulla morfologia del fegato durante l’instaurazione e il progresso di un ittero ostruttivo. Per la ricerca sono stati utilizzati 80 ratti Wistar, suddivisi in quattro gruppi: JAUNDICE (ittero ostruttivo), JAUN-ISC (ittero ostruttivo e riperfusione di ischemia), CONTROL (laparotomia) e ISCHEMIA (riperfusione di ischemia). RISULTATI: l’ittero ostruttivo e il danno da ischemia-riperfusione nell’ittero ostruttivo hanno portato ad un aumento della mortalità, mentre non è stata osservata mortalità nei gruppi di controllo e ischemia. Nel gruppo JAUN-ISC, la SGOT è aumentata significativamente al decimo giorno e la SGPT è aumentata significativamente al primo giorno rispetto al gruppo JAUNDICE. Inoltre, nel gruppo JAUN-ISC, la dilatazione dei sinusoidi è aumentata significativamente il quinto e il decimo giorno e l’infiltrazione di neutrofili è aumentata significativamente il decimo giorno rispetto al gruppo JAUNDICE. CONCLUSIONE: Una lieve lesione da ischemia-riperfusione, che nel fegato normale ha portato solo a un leggero aumento dell’infiltrazione epatica dei neutrofili, in presenza di ittero ostruttivo ha portato ad un aumento dei marcatori biochimici epatici (SGOT, SGPT) e ad un aumento della dilatazione epatica sinusoide e ad una maggiore infiltrazione di neutrofili.

Abstract

Background and study purpose: Hilar cholangiocarcinoma, also known as Altemeier-Klatskin tumor, is a rare malignancy that arises in the confluence of the hepatic ducts of the porta hepatis. The prognosis is rather poor. Several lesions mimic these tumors and lead to a misdiagnosis, resulting in radical hepatic resections. These lesions are known as Klatskin-mimicking lesions. We present our experience with the diagnosis and treatment of pathological lesions that can mimic a perihilar cholangiocarcinoma and establish an algorithm of treatment Methods: For the current retrospective study, a prospectively established bile-duct tumor database was analyzed by selecting patients with a preoperative diagnosis of perihilar cholangiocarcinoma. Results: In the last 22 years (from January 1, 1996, to December 31, 2017), 73 patients who were referred to our tertiary center with a primary diagnosis of a Klatskin tumor were treated. All patients underwent a thorough evaluation before deciding upon the treatment. However, only 58 cases had a confirmed preoperative diagnosis of hilar cholangiocarcinoma in the final histopathological examination. The final diagnosis in 15 patients differed from the primary cause for referral and the lesions were regarded as Klatskin-mimicking lesions. Conclusions: Clinicians should always highly suspect Klatskin-mimicking lesions when they evaluate a patient for a possible hilar cholangiocarcinoma in order to avoid a misdiagnosis and propose a proper treatment.

Abstract

Background Fournier’s gangrene (FG) is a rapidly evolving necrotizing fasciitis of the perineum and the genital area, the scrotum as it most commonly affects man in the vast majority of cases. It is polymicrobial in origin, due to the synergistic action of anaerobes and aerobes and has a very high mortality. There are many predisposing factors including diabetes mellitus, alcoholism, immunosuppression, renal, and hepatic disease. The prognosis of the disease depends on a lot of factors including but not limited to patient age, disease extent, and comorbidities. The purpose of the study is to describe the experience of a general surgery department in the management of FG, to present the multimodal and multidisciplinary treatment of the disease, to identify predictors of mortality, and to make general surgeons familiar with the disease. Methods The current retrospective study is presenting the experience of our general surgery department in the management of FG during the last 20 years. The clinical presentation and demographics of the patients were recorded. Also we recorded the laboratory data, the comorbidities, the etiology, and microbiology and the therapeutic interventions performed, and we calculated the various severity indexes. Patients were divided to survivors and non-survivors, and all the collected data were statistically analyzed to assess mortality factors using univariate and then multivariate analysis. Results In our series, we treated a total of 24 patients with a mean age 58.9 years including 20 males (83.4%) and 4 females (16.6%). In most patients, a delay between disease onset and seeking of medical help was noted. Comorbidities were present in almost all patients (87.5%). All patients were submitted to extensive surgical debridements and received broad-spectrum antibiotics until microbiological culture results were received. Abstract

Aim To evaluate the effect of local surgical adhesive glue (albumin/glutaraldehyde-Bioglue) on the healing of colonic anastomoses in rats. Methods Forty Albino-Wistar male rats were randomly divided into two groups, with two subgroups of ten animals each. In the control group, an end-to-end colonic anastomosis was performed after segmental resection. In the Bioglue group, the anastomosis was protected with extraluminar application of adhesive glue containing albumin and glutaraldehyde. Half of the rats were sacrificed on the fourth and the rest on the eighth postoperative day. Anastomoses were resected and macroscopically examined. Bursting pressures were calculated and histological features were graded. Other parameters of healing, such as hydroxyproline and collagenase concentrations, were evaluated. The experimental data were summarized and computed from the results of a one-way ANOVA. Fisher's exact test was applied to compare percentages. Results Bursting pressures, adhesion formation, inflammatory cell infiltration, and collagen deposition were significantly higher on the fourth postoperative day in the albumin/glutaraldehyde group than in the control group. Furthermore, albumin/glutaraldehyde significantly increased adhesion formation, inflammatory cell infiltration, neoangiogenesis, and collagen deposition on the eighth postoperative day. There was no difference in fibroblast activity or hydroxyproline and collagenase concentrations. Conclusion Albumin/glutaraldehyde, when applied on colonic anastomoses, promotes their healing in rats. Therefore, the application of protective local agents in colonic anastomoses leads to better outcomes.
