Abstract

Purpose The purpose of this study was to assess the impact of the free choice of ultrasound propagation velocity on ultrasound image construction to improve the completion rate and anatomical quality of fetal second-trimester ultrasound examination in obese women. Materials and methods This repeated cross-sectional single-center study retrospectively collected second-trimester ultrasound images of 88 obese women. During the first period, ultrasound examinations were performed in 44 women (mean age, 31.4 ± 5.9 [SD] years; range: 21.1 - 45.3 years) applying only the standard 1540 m/s tissue ultrasound velocity (group 1). During the second period, ultrasound examinations were performed in other 44 women (mean age, 31.4 ± 5.1 [SD] years; range: 20.6 - 41.6 years) with the operator free to choose among three available velocity settings (1420 m/s, 1480 m/s or 1540 m/s) for the scanning planes for the morphological images (group 2). All women underwent mid-trimester ultrasound examination at 20 to 24 gestational weeks. Two observers assessed the examinations in both groups for completeness, quality, and duration of fetal ultrasound examinations. Results No differences in age (P > 0.99), body mass index (P = 0.67), prevalence of previous cesarean delivery (P = 0.30) or gestational age at the second-trimester scan (P = 0.20) were found between the two groups. The mean cumulative duration of these ultrasound examinations was longer in group 1 than in group 2 (for both the complete (P = 0.04) and incomplete (P = 0.03) examinations). The quality of the anatomic images according to Salomon's criteria was less often acceptable in group 1 (5/44, 11.4%) than in group 2 (15/44, 34.1%) (P = 0.02). Conclusion Free choice of ultrasound velocity improves the overall performance of fetal second-trimester ultrasound examinations in obese women.

Abstract

Introduction: The objective was to assess the prognostic value of the systolic flow through the aortic isthmus in monochorionic pregnancies complicated by twin-twin transfusion syndrome (TTTS) treated by placental laser ablation. Material and Methods: Fetal echocardiography and outcome data of 105 cases of TTTS treated by laser photocoagulation of placental anastomoses were reviewed. Hemodynamic parameters were collected before and after treatment. The isthmic systolic index (ISI) was calculated as the peak systolic velocity/systolic nadir ratio. Results: A total of 105 laser coagulations were studied. Fetal echocardiography pre- and post-laser were available in 68 cases, including 55 with data on aortic isthmic Doppler. Survival rates were 17, 22, and 61% for 0, 1, or 2 twins, respectively. At least 1 twin was delivered alive in 83% of the pregnancies. The mean gestational age at surgery was 21 weeks (range 16-26). Median ISI values were similar for donor and recipient twins, before and after laser ablation (all p > 0.05). A lower recipient ISI before laser was related to early recipient demise within 24 h (p = 0.04). Discussion: A lower ISI before placental laser ablation for TTTS is associated with postoperative demise of the recipient twin.

Abstract

International audience; Objectives: To study the prevalence of burnout among non-health care workers (NHCW), the risk and protective factors and to quantify the risk of burnout.Method: We conducted a cross-sectional study on the 3142 NHCW of the University Hospital of Clermont-Ferrand. They received a self-assessment questionnaire.Results: Four hundred thirty seven (13.9%) NHCW completed the questionnaires. More than three quarter (75.4%) of NHCW was in burnout, with one in five (18.7%) having a severe burnout. Job demand was the main factor explaining the increase in exhaustion and overinvestment was the main factor explaining the increase in cynicism. Effort-reward imbalance (ERI) multiplied the risk of severe burnout by 11.2, job strain by 3.32 and isostrain by 3.74.Conclusion: NHCW from hospital staff are at high risk of burnout. The two major models of stress at work, the job demand-control-support and the ERI, were highly predictive of burnout, with strong dose-response relationships.

Abstract

In this article, we introduce a new soft finger with a pneumatic-actuated movable joint that is optimized and characterized in terms of the degrees of freedom (DoF), workspace, and fingertip force. The finger consists of one soft link as the body and the bending pneumatic joint as the actuator. Due to the additional translation and rotation movement capabilities of the joint, carried out by two stepper motors, the finger can bend in any direction while having different lengths, thanks to a configurable bending point. This results in more dexterity for the finger when dealing with a target inside its 3D workspace by increasing the number of configurations it can use to reach the target and exert force. The finite element method (FEM) and the Nondominated Sorting Genetic Algorithm II (NSGA-II) algorithm are applied to optimize the joint geometry and so maximize the bending angle and minimize the joint dimensions. Furthermore, the variations of each design parameter and consequent effects on the optimization objectives are analyzed. The optimal geometrical parameters are used to fabricate a prototype with silicone rubber. Tests on the bending angle and tip force variability are conducted on the prototype to validate the numerical modeling. The experimental results show that the finger exerts force up to 650 mN with a response time of fewer than 3 s. The stiffness of the finger can be changed by applying pneumatic pressure in the hollow space inside the link. This consequently varies the amount of applied force at the tipping point of the finger up to two times.

Abstract

We present a topology optimization (TO) method for a 1D dielectric metasurface, coupling the classical trend-fluctuations analysis (FTA) and the diamond-square-algorithm (DSA). In the classical FTA, a couple of device distributions termed Fluctuation or mother and Trends or father, with specific spectra is initially generated. The spectral properties of the trend function, allow to target efficiently the basin of optimal solutions. For optimizing a 1D metasurface to deflect a normally incident plane wave into a given deflecting angle, a cosine-like function has been identified to be an optimal father profile allowing to efficiently target a basin of local minima. However there is no efficient method to predict the father profile number of oscillations that effectively allows to avoid undesirable local optima. It would be natural to suggest a randomization of the variable which controls the number of oscillations of the father function. However, one of the main drawbacks of the randomness searching process is that, combined with a gradient method, the algorithm can target, undesirable local minima. The method proposed in this paper improves the possibility of the classical FTA to avoid the trapping of undesirable local optimal solutions. This is accomplished by extending the initial candidate family to higher quality offspring that are generated thanks to a diamond-square-algorithm (DSA). Doing so, ensures that the main features of the best trends are stored in the genes of all Offspring structures., 8 pages, 11 figures

Abstract

International audience; Mechanoluminescent materials have the property of emitting light when they are mechanically deformed. The paper deals with the potential use of these active substances to reveal interparticle contact force networks in granular media under mechanical loading. Preliminary uniaxial tensile tests were first performed on two types of longitudinal specimens for comparison purposes: pure epoxy resin and epoxy resin containing mechanoluminescent powders. Stress-strain curves showed that the powder acted as a reinforcement, but fractographic analysis by SEM revealed that debonding occurred between the epoxy matrix and powder grains during mechanical loading. Various two-dimensional cohesionless granular systems were then studied, using mechanoluminescent cylinders subjected to compression. Whereas uniaxial tensile tests featured homogeneous light emission, localized mechanoluminescence intensities were revealed in the contact zones between cylinders. The study shows that mechanoluminescent materials open perspectives for the identification of interparticle contact intensities in granular media.

Abstract

The fifth generation of mobile communications (5G) is expected to dramatically improve performance compared to preceding standards by offering very high bandwidths and low latencies. To provide this performance, heavy processing is required and must meet strong timing constraints. Reconfigurable computing, managing processing in software and exploiting reconfigurable hardware acceleration, is an innovative approach that should be considered for 5G for its capacity to combine high throughput and high flexibility. This paper presents a case study for Orthogonal Frequency Division Multiplexing (OFDM) computation reconfigurable offloading onto an Field Programmable Gate Array (FPGA). The implementation is based on Open Computing Language (OpenCL) that represents a versatile solution, as this language can be compiled for several architectures, provided that a Host+Accelerator structure is used. The objective of our study is to demonstrate that, by means of hardware offloading, the 5G architecture resources can reach high computational load, avoiding processing stalls and latency increase. Results show that around 15% of the software processing can be freed through hardware acceleration and reallocated to support other tasks.

Abstract

Background The effects of physical activity (PA) in disease prevention and therapy have well-known effects on lower-limb osteoarthritis (OA), decreasing pain and improving function. Objective We aimed to describe the level and factors affecting PA practices of people with knee OA. Design Prospective epidemiological study. Setting In all, 548 people with knee OA were interviewed by use of self-administered anonymous questionnaires. Main outcome measurement The main outcome was physical activity level evaluated by the International physical activity questionnaire (IPAQ) (short version). Secondary outcomes included sociodemographic and clinical data, comorbidities, and barriers to and facilitators of practicing regular PA evaluated by 24 specific elements. Results The mean (SD) age of the study population was 67.6 (7.9) years; 73.9% were women and 30.9% had obesity (mean [SD] body mass index [BMI] 28.2 [5.7] kg/m2). Multi-joint OA affected 92% of the population, and 71.6% had comorbidities. The mean (SD) visual analog scale score for pain intensity was 4.5/10 (2.5), which was 51.4% better than the patient acceptable symptom state (PASS). The mean (SD) Western Ontario and McMaster Universities Osteoarthritis Index function score was 36.6/100 (20.7), which was 57.5% better than the PASS. In total, 67% of patients used analgesics, half of them at least once a week. According to the IPAQ, 42.6% of patients reported high, 38.6% moderate, and 18.8% low PA level; the median IPAQ total activity score was 2628 metabolic equivalent of task (MET)-min/week and time spent sitting was 257.1 min/day. Only one third of participants received non-pharmacological treatment corresponding to the latest recommendations. Variables significantly related to inactive or minimally active PA levels were BMI (P = 0.0294), sex (P = 0.0008), and biomedical barriers, related to self-efficacy (P = 0.0118). Conclusions The OA study population was less active, more sedentary, and had more comorbidities and more barriers to PA practice than the overall population.

Abstract

International audience; BACKGROUND: The objective was to examine the role of a sustained virological response (SVR) on major adverse cardiovascular events (MACEs) in patients with compensated hepatitis C virus (HCV) cirrhosis.METHODS: Patients with the following criteria were enrolled in 35 French centers: (1) biopsy-proven HCV cirrhosis; (2) Child-Pugh A; (3) positive viremia; and (4) no prior liver complication, and then prospectively followed. All patients received HCV treatment after inclusion. MACEs included stroke, myocardial infarction, ischemic heart disease, heart failure, peripheral arterial disease, cardiac arrest, and cardiovascular death. SVR, defined as negative viremia 12 weeks posttreatment, was considered as a time-dependent covariate, and its effect on MACE occurrence was assessed. The median follow up was 57.5 months, ending in December 2015.RESULTS: Sixty-two of 878 (7.1%) patients presented a total of 79 MACEs. The main predictive baseline factors of MACEs were Asian ethnic origin, history of MACEs, arterial hypertension, diabetes mellitus, current smoking, low serum albumin level, high total bilirubin level, and low platelet count. In multivariate analysis, SVR was associated with a decreased risk of MACEs (hazard ratio=0.35, 95% CI 0.09-0.97, P=.044), whereas Asian ethnic origin, arterial hypertension, smoking, and low serum albumin level remained predictive of MACE occurrence. The 5-year survival rate was 60.1% versus 87.5% in patients who did versus those who did not present a MACE (PCONCLUSIONS: In patients with compensated HCV-related cirrhosis, Asian ethnic origin, arterial hypertension, smoking, and low serum albumin are independent predictive factors of cardiovascular events, whereas an SVR is associated with a decreased rate of cardiovascular events.

Abstract

Gelatin is used to make hard capsules and has to meet strict dissolution specifications to guarantee homogeneous drug delivery. The aging of gelatin induces a decrease of its dissolution rate due to the formation of intra- and intermolecular cross-links. Cross-link formation has shown to be dependent on the environmental conditions of production, such as temperature and humidity, and therefore on the geographic origin of production. Gelatin structure consists of an amorphous phase (coil structure) and a crystal phase (triple-helixes) and is very sensitive to environmental changes. The present work aims at understanding the role and effects of structural changes during aging on gelatin dissolution, taking into account the origin of production. The molecular structure of pig skin gelatins from three different production sites of the same company were studied by differential scanning calorimetry, synchrotron circular dichroism and 1H nuclear magnetic resonance. It turned out that aging induced the melting of triple-helixes, an increase of random coils and, probably, β-turn conformations. The gelatin structure varied with the origin of production and thus affected the dissolution rate. Gelatins with non-compliant dissolution rates exhibited a higher amount of amorphous phase after aging than compliant gelatins. Although it was not possible to formulate any certain interpretation of the synchrotron circular dichroism results regarding gelatin dissolution, this technique is able to differentiate compliant dissolutions from non-compliant ones, even before gelatin aging.

Abstract

International audience; The population dynamics of different Pseudo-nitzschia species, along with particulate domoic acid (pDA) concentrations, were studied from May 2012 to December 2013 in the Bay of Seine (English Channel, Normandy). While Pseudo-nitzschia spp. blooms occurred during the two years of study, Pseudo-nitzschia species diversity and particulate domoic acid concentrations varied greatly. In 2012, three different species were identified during the spring bloom (P. australis, P. pungens and P. fraudulenta) with high pDA concentrations (similar to 1400 ngl(-1)) resulting in shellfish harvesting closures. In contrast, the 2013 spring was characterised by a P. delicatissima bloom without any toxic event. Above all, the results show that high pDA concentrations coincided with the presence of P. australis and with potential silicate limitation (Si: N < 1), while nitrate concentrations were still replete. The contrasting environmental conditions between 2012 and 2013 highlight different environmental controls that might favour the development of either P. delicatissima or P. australis. This study points to the key role of Pseudo-nitzschia diversity and cellular toxicity in the control of particulate domoic acid variations and highlights the fact that diversity and toxicity are influenced by nutrients, especially nutrient ratios. Abstract

[Departement_IRSTEA]Ecotechnologies [TR1_IRSTEA]INSPIRE; International audience; In this paper, we introduce a geometric method for 3D reconstruction of the exterior environment using a panoramic microwave radar and a camera. We rely on the complementarity of these two sensors considering the robustness to the environmental conditions and depth detection ability of the radar, on the one hand, and the high spatial resolution of a vision sensor, on the other. Firstly, geometric modeling of each sensor and of the entire system is presented. Secondly, we address the global calibration problem, which consists of finding the exact transformation between the sensors' coordinate systems. Two implementation methods are proposed and compared, based on the optimization of a non-linear criterion obtained from a set of radar-to-image target correspondences. Unlike existing methods, no special configuration of the 3D points is required for calibration. This makes the methods flexible and easy to use by a non-expert operator. Finally, we present a very simple, yet robust 3D reconstruction method based on the sensors' geometry. This method enables one to reconstruct observed features in 3D using one acquisition (static sensor), which is not always met in the state of the art for outdoor scene reconstruction.The proposed methods have been validated with synthetic and real data.

Abstract

In prostate cancer, DNA methylation is significantly associated with tumor initiation, progression, and metastasis. Previous studies have suggested that soy phytoestrogens might regulate DNA methylation at individual candidate gene loci and that they play a crucial role as potential therapeutic agents for prostate cancer. The purpose of our study was to examine the modulation effects of phytoestrogens on a genome-wide scale in regards to DNA methylation in prostate cancer. Prostate cancer cell lines DU-145 and LNCaP were treated with 40 mu M of genistein and 110 mu M of daidzein. DNMT inhibitor 5-azacytidine (2 mu M) and the methylating agent budesonide (2 mu M) were used to compare their demethylation/methylation effects with phytoestrogens. The regulatory effects of phytoestrogens on DNA methylation were analyzed by using a methyl-DNA immunoprecipitation method coupled with Human DNA Methylation Microarrays (MeDIP-chip). We observed that the methylation profiles of 58 genes were altered by genistein and daidzein treatments in DU-145 and LNCaP prostate cancer cells. In addition, the methylation frequencies of the MAD1L1, TRAF7, KDM4B, and hTERT genes were remarkably modified by genistein treatment. Our results suggest that the modulation effects of phytoestrogens on DNA methylation essentially lead to inhibition of cell growth and induction of apoptosis. Genome-wide methylation profiling reported here suggests that epigenetic regulation mechanisms and, by extension, epigenetics-driven novel therapeutic candidates warrant further consideration in future "omics" studies of prostate cancer.

cited By 15; International audience; Hydrogels based on carboxymethylcellulose (CMC) and gelatin (GEL) crosslinked with glutaraldehyde were used to obtain interpenetrated- interconnecting polymer networks. They are designed to obtain controlled release polymeric drug systems. CMC and GEL were chosen for their biocompatibility and nontoxicity, which are compulsory conditions for polymers used in biomedical applications. By modifying the parameters of the crosslinking reaction, the obtained networks presented different crosslinking degrees and hence different swelling capacities. These properties determined the quantity of drug able to be loaded (0.25 g per gram of hydrogel). We obtained systems for which biologically active matter release was controlled by diffusion. The kinetics were zero-order during the major part of release period (∼500 min). These systems improve the bactericide activity compared with free drugs.
