The article focuses on a study of virtual reference transactions of non-traditional Caucasian and African American library users. The data for the study originated from requests sent by distance education students to a reference desk. It was found that on average, 10.5 messages were sent by African American users, while 7.5 messages were sent by Caucasian users. The study found that the African Americans asked more questions in the second messages, while their Caucasian counterparts asked more technical questions. The study suggests that African American students need more library support.

Facial nerve paralysis can be difficult to treat because it presents a variety of functional, aesthetic, and psychosocial challenges. The goals of treatment include facial symmetry at rest, corneal protection, oral competence, restoration of voluntary and spontaneous facial movements, and minimal synkinesis. A multitude of static and dynamic procedures have been used to achieve these goals. Facial nerve reapproximation or interpositional grafting is associated with the best end results. The results of dynamic procedures are generally better than those of static procedures. Optimal reconstruction of the paralyzed face usually requires multiple surgeries with both types of procedures. Patients must be extensively counseled regarding expected results before they embark on what is an oftentimes lengthy reconstructive process. In this article, we discuss the anatomy of the facial nerve, the etiologic factors associated with facial nerve paralysis, the evaluation of the patient with facial paralysis, and the various surgical options for static and dynamic reconstruction of the paralyzed face. This project featured collaboration between a large public library system and an academic institution to address training needs related to the rise in the demand for quality consumer health information. A train the trainer program featuring hands-on training and Web-based support was designed and implemented. Public reference librarians received customized education and support to facilitate their efforts to train library users of consumer health information. The results of the collaborative effort included an increased awareness of cultural, linguistic, and literacy issues for effective provision of public library consumer health reference service along with a reaffirmation of the need for strategic community collaborations for health education. Persistent pulmonary hypertension of the newborn, or PPHN, represents a challenging condition associated with high morbidity and mortality. Management is complicated by complex pathophysiology and limited neonatal specific evidence-based literature, leading to a lack of universal contemporary clinical guidelines for the care of these patients. To address this need and to provide consistent high-quality clinical care for this challenging population in our neonatal intensive care unit, we sought to develop a comprehensive clinical guideline for the acute stabilization and management of neonates with PPHN. Utilizing cross-disciplinary expertise and incorporating an extensive literature search to guide best practice, we present an approachable, pragmatic, and clinically relevant guide for the bedside management of acute PPHN. Key Points PPHN is associated with several unique diagnoses; the associated pathophysiology is different for each unique diagnosis. Shared decision-making (SDM) is a collaborative approach to making decisions in health care, and is a cornerstone of person-centered care. While providers are increasingly expected to utilize SDM in routine practice, widespread and sustainable implementation has proven difficult, especially in the care of individuals diagnosed with serious mental illnesses, and physicians and patients continue to identify barriers to effective collaboration. To date, SDM research has largely focused on the provision of high-quality clinical information from doctors to patients to the neglect of what may be the most important, and transformative, aspect of SDM—the relationship itself. In this forum, the lack of attention to the relationship in SDM research and practice will be explored, along with the relational qualities that need to be in place to implement SDM in the care of persons with serious mental illness based on the findings from a mixed-methods, participatory research project.
