Thirteen boronated cyanometallates [M(CN-BR

To gain principal insight into fixation techniques of a posteromedial split fragment in bicondylar tibial plateau fractures. A computer simulation was performed, applying the finite-element method (FEM) to compare four methods of fixation of the posteromedial split fragment: lateral plate (model 1), lateral plate and kickstand screw (model 2), lateral plate and two antero-posterior lag screws (model 3), and lateral and posteromedial plate (model 4). The displacement of the fragment and material stresses in implants and bone under 2500 N axial load were analyzed. Maximal displacement of the posteromedial split fragment of 2.8 mm was found with a sole lateral plate. An added kickstand screw decreased the displacement to 1.46 mm. Added lag screws improved stability by a factor 4, with a maximal displacement of 0.76 mm. The double-plate configuration revealed 0.27 mm, a decrease of the displacement by a factor 10 compared to model 1. An additional analysis of posteromedial fragment displacements with osteoporotic bone, simulated by dividing the elastic modulus of the bone by a factor 2, turned out to be of relevant impact. For model 1, the calculations did not converge. The influence of bone quality was found to be 70% in model 2, 60% in model 3, and 40% in model 4. The results indicate that the additional fixation of a posteromedial split fracture by plate osteosynthesis might be advantageous in bicondylar tibial plateau fractures treated with lateral plating. This might be even more important in patients with low bone quality.

W(CNAr)₆ (CNAr = arylisocyanide) photoreductants catalyze base-promoted homolytic aromatic substitution (BHAS) of 1-(2-iodobenzyl)-pyrrole in deuterated benzene. Moderate to high efficiencies correlate with W(CNAr)₆ excited-state reduction potentials upon one-photon 445 nm excitation, with 10 mol % loading of the most powerful photoreductants W(CNDipp)₆ (CNDipp = 2,6-diisopropylphenylisocyanide) and W(CNDippPh^(OMe3))6 (CNDippPh^(OMe3) = 4-(3,4,5-trimethoxyphenyl)-2,6-diisopropylphenylisocyanide) affording nearly complete conversion. Stern–Volmer quenching experiments indicated that catalysis is triggered by substrate reductive dehalogenation. Taking advantage of the large two-photon absorption (TPA) cross sections of W(CNAr)₆ complexes, we found that photocatalysis can be driven with femtosecond-pulsed 810 nm excitation. For both one- and two-photon excitation, photocatalysis was terminated by the formation of seven-coordinate WII-diiodo [WI₂(CNAr)₅] complexes. Notably, we discovered that W(CNDipp)₆ can be regenerated by chemical reduction of WI2(CNDipp)5 with excess ligand present in solution.

In einem Hochendemiegebiet der alveolären Echinokokkose (AE), einer Zoonose verursacht durch den Fuchsbandwurm Echinococcus multilocularis wurde in Südwestdeutschland ein Feldhase (Lepus europaeus) während der offiziellen Jagd erlegt. Bei der amtlichen Fleischuntersuchung zeigten sich makroskopisch für eine AE suspekte, subkapsulär im Leberparenchym liegende, zystenähnliche Veränderungen von ca. 1–3 cm Größe. Auch im Rahmen einer an der Hasenleber durchgeführten Ultraschall- und MRT-Untersuchung ergab sich bildmorphologisch der hochgradige Verdacht auf eine AE. Um die Struktur des Inhaltes der Zysten nicht zu zerstören, wurde auf ihre Eröffnung verzichtet. Die histologische und immunhistochemische Untersuchung durch eine Referenzpathologie für Echinokokkose konnte die Verdachtsdiagnose der AE oder die Differentialdiagnose einer Zystizerkose, verursacht durch T. crassiceps oder T. pisiformis, nicht bestätigen. Die histopathologischen Befunde sprachen für das Vorliegen eines Cholangiofibroms (Von-Meyenburg-Komplex). Dieser Befund wurde anhand der Bildmorphologie des Ultraschalls und der MRT als Caroli-Krankheit eingeordnet. Der vorliegende Fallbericht zeigt, dass als Differentialdiagnose einer AE oder einer Zystizerkose auch an ein seltenes Cholangiofibrom mit der makroskopisch-/bildmorphologischen Ausprägung einer Caroli-Krankheit gedacht werden sollte.

Neovascular age-related macular degeneration (nAMD) is the most frequent cause of pigment epithelial detachment (PED). In the clinical routine the treatment of fibrovascular PED (fPED) and serous vascularized PED (svPED) with intravitreal vascular endothelial growth factor (VEGF) inhibitors has a restricted prognosis.There are limited data on the long-term outcome of PED under anti-VEGF therapy. Therefore, this study recorded the course of treated PEDs in nAMD eyes over a period of 5 years.All eyes with fPED or svPED that underwent anti-VEGF medication between 2006 and 2015 were retrospectively analyzed regarding the clinical course and the morphology seen on optical coherence tomography (OCT). The inclusion criteria were the detection of a PED on OCT, the angiographic verification of nAMD, a documented clinical history over 5 years and a good image quality.A total of 23 eyes from 22 patients met the inclusion criteria. After 5 years a significant deterioration of visual acuity (VA) was seen in all eyes (p = 0.007) and in the subgroup of cases with fPED (p = 0.045). In the eyes with svPED the decline of VA was not significant (p = 0.097). In the collective study group a statistically significant reduction of PED height (p = 0.006) and an increase of PED diameter was measured (p = 0.002). In the subgroup analysis the decrease of PED height and increase of PED diameter were significant for cases with svPED (p = 0.004, p = 0.013, respectively) but were not statistically significant for fPED eyes (height: p = 0.616; diameter: p = 0.097). In 17 (74%) eyes fibrosis or atrophy were seen on the final assessment of OCT images.After 5 years of anti-VEGF therapy for nAMD-associated PED the VA declined in half of the eyes and the OCT showed an unfavorable morphology in 3/4 of the cases. The average number of visits and injections was distinctly lower than in clinical trials and other real-life analyses. In summary, we observed an undertreatment with a worse functional and anatomical outcome in our clinical routine compared to other studies.HINTERGRUND: Die neovaskuläre altersabhängige Makuladegeneration (nAMD) ist die häufigste Ursache für das Auftreten einer Pigmentepithelabhebung (PED). Die Therapie der fibrovaskulären PED (fPED) und serös-vaskularisierten PED (svPED) mit intravitrealen VEGF(„vascular endothelial growth factor“)-Inhibitoren hat in der klinischen Praxis eine eingeschränkte Prognose.Die Datenlage zum PED-Langzeitverlauf ist überschaubar. Diese Arbeit erfasst den Verlauf therapierter PEDs bei nAMD über einen Zeitraum von 5 Jahren.Bei allen Augen, die im Zeitraum von 2006 bis 2015 aufgrund einer fPED oder svPED eine Behandlung mit Anti-VEGF-Präparaten erhielten, erfolgte eine retrospektive Analyse des klinischen Verlaufs und der Morphologie mittels optischer Kohärenztomographie (OCT). Es galten die folgenden Einschlusskriterien: OCT-Nachweis einer PED, angiographische Bestätigung einer nAMD, klinische Dokumentation über 5 Jahre sowie gute Bildqualität.Bei 23 Augen von 22 Patienten wurden die Einschlusskriterien erfüllt. Nach 5 Jahren zeigte sich im Gesamtkollektiv und in der Subgruppe der Augen mit fPED eine signifikante Verschlechterung des Visus (p = 0,007; p = 0,045). Bei den Augen mit svPED war die Visusabnahme nicht signifikant (p = 0,097). Im Gesamtkollektiv wurde eine statistisch signifikante Reduktion der PED-Höhe (p = 0,006) bei gleichzeitig signifikanter Zunahme des PED-Durchmessers (p = 0,002) gemessen, wobei innerhalb der Subgruppen die Abnahme der PED-Höhe und Zunahme des PED-Durchmessers nur für die Messwerte der svPED signifikant waren (p = 0,004; p = 0,013). Bei den Augen mit fPED war die Veränderung der OCT-Parameter nicht signifikant (p = 0,616; p = 0,097). Bei 17 (74 %) der Augen fand sich bei der finalen OCT-Beurteilung eine Fibrose oder Atrophie.Nach 5 Jahren Anti-VEGF-Behandlung bei nAMD-assoziierter PED zeigten sich in der Hälfte der Fälle eine Visusabnahme und in drei Viertel der Augen eine ungünstige OCT-Morphologie. Die mittlere Anzahl an Injektionen und Visiten war niedriger als in klinischen Studien und anderen Real-life-Erhebungen. Insgesamt beobachteten wir eine Unterbehandlung mit schlechterem funktionellem und anatomischem Outcome in unserer klinischen Praxis verglichen mit anderen Studien.

Background/aimTo evaluate the association of genetic polymorphisms APOE,APOEG-219T promoter, microtubule associated protein(MAPT)/tauexon 6 Ser53Pro, MAPT/tauHist47Tyr,IL-6572 G/C andIL-6RAsp358Alawith the risk of concussion in college athletes.MethodsA 23-centre prospective cohort study of 1056 college athletes with genotyping was completed between August 2003 and December 2012. All athletes completed baseline medical and concussion questionnaires, and post-concussion data were collected for athletes with a documented concussion.ResultsThe study cohort consisted of 1056 athletes of mean±SD age 19.7±1.5 years,89.3% male, 59.4% Caucasian, 35.0% African-American, 5.6% other race. The athletes participated in American football, soccer, basketball, softball, men’s wrestling and club rugby. A total of 133 (12.1% prevalence) concussions occurred during an average surveillance of 3 years per athlete. We observed a significant positive association betweenIL-6RCC (p=0.001) and a negative association betweenAPOE4 (p=0.03) and the risk of concussion. Unadjusted and adjusted logistic regression analysis showed a significant association betweenIL-6RCC and concussion (OR 3.48; 95% CI 1.58 to 7.65; p=0.002) and between the APOE4 allele and concussion (OR 0.61; 95% CI 0.38 to 0.96; p=0.04), which persisted after adjustment for confounders.ConclusionsIL-6RCC was associated with a three times greater concussion risk andAPOE4 with a 40% lower risk.

This article reports on a case of a glandular odontogenic cyst (GOC) in the posterior mandible of a 25-year-old male treated with peripheral ostectomy, bone allograft, and an implant-supported fixed dental prosthesis. During a routine 3-month postoperative visit, a radiograph suggested vertical bone loss on the implant in the location of the mandibular right first molar. The three-walled defect was surgically treated with debridement, bone allograft, and a membrane. Healing of this site has been uneventful. This case report demonstrates that implants can be successfully placed and restored in previously affected GOC sites.

In this study, Inconel 718 powder particles were successfully cold-sprayed on Inconel 718 substrate by using nitrogen gas for a repair application of aero engine components. The effects of substrate roughness and spray-angle on the deposition behavior of Inconel 718 particles were investigated. It has been found that the deposition behavior of Inconel 718 powder on Inconel 718 substrates is highly influenced by substrate surface roughness. Single powder particle interaction with substrates of different roughness showed that plastic deformation and interfacial material mixing is higher if powder particles interact with substrates of higher roughness. Consequently, substrates with lower roughness demonstrated many abnormalities (e.g. cracks, peeling-off) during the deposition. Substrate roughness is limited to influence the deposition efficiency of the first few layers. Besides, it is also observed that particle deformation and coating quality are significantly affected by the spray-angle because of asymmetric deformation of the particle due to additional tangential momentum. Moreover, it is observed that deposition efficiency is reduced with a decrease in spray-angle, while the coating porosity and coating roughness show an inverse trend. Furthermore, the effects of three input parameters, namely substrate preparation, spray angle and stand-off distance, on four outputs (responses), such as thickness, roughness, porosity and Vickers-hardness of the coating were studied using non-linear statistical regression analysis. The above mentioned four outputs were found to be significantly dependent on substrate preparation and spray-angle.
