Although variation in effect sizes and predicted values among studies of similar phenomena is inevitable, such variation far exceeds what might be produced by sampling error alone. One possible explanation for variation among results is differences among researchers in the decisions they make regarding statistical analyses. A growing array of studies has explored this analytical variability in different fields and has found substantial variability among results despite analysts having the same data and research question. Many of these studies have been in the social sciences, but one small "many analyst" study found similar variability in ecology. We expanded the scope of this prior work by implementing a large-scale empirical exploration of the variation in effect sizes and model predictions generated by the analytical decisions of different researchers in ecology and evolutionary biology. We used two unpublished datasets, one from evolutionary ecology (blue tit, Cyanistes caeruleus, to compare sibling number and nestling growth) and one from conservation ecology (Eucalyptus, to compare grass cover and tree seedling recruitment). The project leaders recruited 174 analyst teams, comprising 246 analysts, to investigate the answers to prespecified research questions. Analyses conducted by these teams yielded 141 usable effects (compatible with our meta-analyses and with all necessary information provided) for the blue tit dataset, and 85 usable effects for the Eucalyptus dataset. We found substantial heterogeneity among results for both datasets, although the patterns of variation differed between them. For the blue tit analyses, the average effect was convincingly negative, with less growth for nestlings living with more siblings, but there was near continuous variation in effect size from large negative effects to effects near zero, and even effects crossing the traditional threshold of statistical significance in the opposite direction. In contrast, the average relationship between grass cover and Eucalyptus seedling number was only slightly negative and not convincingly different from zero, and most effects ranged from weakly negative to weakly positive, with about a third of effects crossing the traditional threshold of significance in one direction or the other. However, there were also several striking outliers in the Eucalyptus dataset, with effects far from zero. For both datasets, we found substantial variation in the variable selection and random effects structures among analyses, as well as in the ratings of the analytical methods by peer reviewers, but we found no strong relationship between any of these and deviation from the meta-analytic mean. In other words, analyses with results that were far from the mean were no more or less likely to have dissimilar variable sets, use random effects in their models, or receive poor peer reviews than those analyses that found results that were close to the mean. Alpine grassland vegetation supports globally important biodiversity and ecosystems that are increasingly threatened by climate warming and other environmental changes. Trait-based approaches can support understanding of vegetation responses to global change drivers and consequences for ecosystem functioning. In six sites along a 1314 m elevational gradient in Puna grasslands in the Peruvian Andes, we collected datasets on vascular plant composition, plant functional traits, biomass, ecosystem fluxes, and climate data over three years. The data were collected in the wet and dry season and from plots with different fire histories. We selected traits associated with plant resource use, growth, and life history strategies (leaf area, leaf dry/wet mass, leaf thickness, specific leaf area, leaf dry matter content, leaf C, N, P content, C and N isotopes). The Arctic is warming at a rate four times the global average, while also being exposed to other global environmental changes, resulting in widespread vegetation and ecosystem change. Integrating functional trait-based approaches with multi-level vegetation, ecosystem, and landscape data enables a holistic understanding of the drivers and consequences of these changes. In two High Arctic study systems near Longyearbyen, Svalbard, a 20-year ITEX warming experiment and elevational gradients with and without nutrient input from nesting seabirds, we collected data on vegetation composition and structure, plant functional traits, ecosystem fluxes, multispectral remote sensing, and microclimate. The dataset contains 1,962 plant records and 16,160 trait measurements from 34 vascular plant taxa, for 9 of which these are the first published trait data. By integrating these comprehensive data, we bridge knowledge gaps and expand trait data coverage, including on intraspecific trait variation. These data can offer insights into ecosystem functioning and provide baselines to assess climate and environmental change impacts. The Open Science (OS) movement is rapidly gaining traction among policy-makers, research funders, scientific journals and individual scientists. Despite these tendencies, the pace of implementing OS throughout the scientific process and across the scientific community remains slow. Thus, a better understanding of the conditions that affect OS engagement, and in particular, of how practitioners learn, use, conduct and share research openly can guide those seeking to implement OS more broadly. We surveyed participants at an OS workshop hosted by the Living Norway Ecological Data Network in 2020 to learn how they perceived OS and its importance in their research, supervision and teaching. Further, we wanted to know what OS practices they had encountered in their education and what they saw as hindering or helping their engagement with OS. The survey contained scaled-response and open-ended questions, allowing for a mixed-methods approach. We obtained survey responses from 60 out of 128 workshop participants (47%). Responses indicated that usage and sharing of open data and code, as well as open access publication, were the most frequent OS practices. Only a minority of respondents reported having encountered OS in their formal education. A majority also viewed OS as less important in their teaching than in their research and supervisory roles. The respondents' suggestions for what would facilitate greater OS engagement in the future included knowledge, guidelines, and resources, but also social and structural support. These are aspects that could be strengthened by promoting explicit implementation of OS practices in higher education and by nurturing a more inclusive and equitable OS culture. Plant removal experiments allow assessment of the role of biotic interactions among species or functional groups in community assembly and ecosystem functioning. When replicated along climate gradients, they can assess changes in interactions among species or functional groups with climate. Across twelve sites in the Vestland Climate Grid (VCG) spanning 4 °C in growing season temperature and 2000 mm in mean annual precipitation across boreal and alpine regions of Western Norway, we conducted a fully factorial plant functional group removal experiment (graminoids, forbs, bryophytes). Over six years, we recorded biomass removed, soil microclimate, plant community composition and structure, seedling recruitment, ecosystem carbon fluxes, and reflectance in 384 experimental and control plots. The dataset consists of 5,412 biomass records, 360 species-level biomass records, 1,084,970 soil temperature records, 4,771 soil moisture records, 17,181 plant records covering 206 taxa, 16,656 seedling records, 3,696 ecosystem carbon flux measurements, and 1,244 reflectance measurements. Context-dependencies in species' responses to the same climate change frustrate attempts to generalize and make predictions based on experimental and observational approaches in biodiversity science. Here, we propose predictability may be enhanced by explicitly incorporating macroecological context into analyses of species' responses to climate manipulations. We combined vascular plant species' responses to an 8-year, 12-site turf transplant climate change experiment set in southwestern Norway with climate niche data from the observed 151 species. We used the difference between a species' mean climate across their range and climate conditions at the transplant site ("climate differences") to predict colonization probability, extinction probability, and change in abundance of a species at a site. In analyses across species that ignore species-specific patterns, colonization success increased as species' distribution optima were increasingly warmer than the experimental target site. Extinction probability increased as species' distribution optima were increasingly colder than the target site. These patterns were reflected in change in abundance analyses. We found weak responses to increased precipitation in these oceanic climates. Climate differences were better predictors of species' responses to climate manipulations than range size. Interestingly, similar patterns were found when analyses focused on variation in species-specific responses across sites. These results provide an experimental underpinning to observational studies that report thermophilization of communities and suggest that space-for-time substitutions may be valid for predicting species' responses to climate warming, given other conditions are accounted for (e.g., soil nutrients). As Open Science practices become more commonplace, there is a need for the next generation of scientists to be well versed in these aspects of scientific research. Yet, many training opportunities for early career researchers (ECRs) could better emphasize or integrate Open Science elements. Field courses provide opportunities for ECRs to apply theoretical knowledge, practice new methodological approaches, and gain an appreciation for the challenges of real-life research, and could provide an excellent platform for integrating training in Open Science practices. Our recent experience, as primarily ECRs engaged in a field course interrupted by COVID-19, led us to reflect on the potential to enhance learning outcomes in field courses by integrating Open Science practices and online learning components. Specifically, we highlight the opportunity for field courses to align teaching activities with the recent developments and trends in how we conduct research, including training in: publishing registered reports, collecting data using standardized methods, adopting high-quality data documentation, managing data through reproducible workflows, and sharing and publishing data through appropriate channels. We also discuss how field courses can use online tools to optimize time in the field, develop open access resources, and cultivate collaborations. Generality in understanding biodiversity responses to climate change has been hampered by substantial variation in the rates and even directions of response to a given change in climate. We propose that such context dependencies can be clarified by rescaling climate gradients in terms of the underlying biological processes, with biotic interactions as a particularly important process. We tested this rescaling approach in a replicated field experiment where entire montane grassland communities were transplanted in the direction of expected temperature and/or precipitation change. In line with earlier work, we found considerable variation across sites in community dynamics in response to climate change. However, these complex context dependencies could be substantially reduced or eliminated by rescaling climate drivers in terms of proxies of plant-plant interactions. Specifically, bryophytes limited colonization by new species into local communities, whereas the cover of those colonists, along with bryophytes, were the primary drivers of local extinctions. These specific interactions are relatively understudied, suggesting important directions for future work in similar systems. Functional trait data enhance climate change research by linking climate change, biodiversity response, and ecosystem functioning, and by enabling comparison between systems sharing few taxa. Across four sites along a 3000-4130 m a.s.l. gradient spanning 5.3 °C in growing season temperature in Mt. Gongga, Sichuan, China, we collected plant functional trait and vegetation data from control plots, open top chambers (OTCs), and reciprocally transplanted vegetation turfs. Over five years, we recorded vascular plant composition in 140 experimental treatment and control plots. We collected trait data associated with plant resource use, growth, and life history strategies (leaf area, leaf thickness, specific leaf area, leaf dry matter content, leaf C, N and P content and C and N isotopes) from local populations and from experimental treatments. The database consists of 6,671 plant records and 36,743 trait measurements (increasing the trait data coverage of the regional flora by 500%) covering 11 traits and 193 plant taxa (ca. 50% of which have no previous published trait data) across 37 families.

Synthesizing trait observations and knowledge across the Tree of Life remains a grand challenge for biodiversity science. Species traits are widely used in ecological and evolutionary science, and new data and methods have proliferated rapidly. Yet accessing and integrating disparate data sources remains a considerable challenge, slowing progress toward a global synthesis to integrate trait data across organisms. Trait science needs a vision for achieving global integration across all organisms. Here, we outline how the adoption of key Open Science principles-open data, open source and open methods-is transforming trait science, increasing transparency, democratizing access and accelerating global synthesis. To enhance widespread adoption of these principles, we introduce the Open Traits Network (OTN), a global, decentralized community welcoming all researchers and institutions pursuing the collaborative goal of standardizing and integrating trait data across organisms. We demonstrate how adherence to Open Science principles is key to the OTN community and outline five activities that can accelerate the synthesis of trait data across the Tree of Life, thereby facilitating rapid advances to address scientific inquiries and environmental issues. Lessons learned along the path to a global synthesis of trait data will provide a framework for addressing similarly complex data science and informatics challenges.

Proxy-based reconstructions and modeling of Holocene spatiotemporal precipitation patterns for China and Mongolia have hitherto yielded contradictory results indicating that the basic mechanisms behind the East Asian Summer Monsoon and its interaction with the westerly jet stream remain poorly understood. We present quantitative reconstructions of Holocene precipitation derived from 101 fossil pollen records and analyse them with the help of a minimal empirical model. We show that the westerly jet-stream axis shifted gradually southward and became less tilted since the middle Holocene. This was tracked by the summer monsoon rain band resulting in an early-Holocene precipitation maximum over most of western China, a mid-Holocene maximum in north-central and northeastern China, and a late-Holocene maximum in southeastern China. Our results suggest that a correct simulation of the orientation and position of the westerly jet stream is crucial to the reliable prediction of precipitation patterns in China and Mongolia.

In a rapidly changing climate, alpine plants may persist by adapting to new conditions. However, the rate at which the climate is changing might exceed the rate of adaptation through evolutionary processes in long-lived plants. Persistence may depend on phenotypic plasticity in morphology and physiology. Here we investigated patterns of leaf trait variation including leaf area, leaf thickness, specific leaf area, leaf dry matter content, leaf nutrients (C, N, P) and isotopes (δ 13 C and δ 15 N) across an elevation gradient on Gongga Mountain, Sichuan Province, China. We quantified inter- and intra-specific trait variation and the plasticity in leaf traits of selected species to experimental warming and cooling by using a reciprocal transplantation approach. We found substantial phenotypic plasticity in most functional traits where δ 15 N, leaf area, and leaf P showed greatest plasticity. These traits did not correspond with traits with the largest amount of intraspecific variation. Plasticity in leaf functional traits tended to enable plant populations to shift their trait values toward the mean values of a transplanted plants' destination community, but only if that population started with very different trait values. These results suggest that leaf trait plasticity is an important mechanism for enabling plants to persist within communities and to better tolerate changing environmental conditions under climate change.

Long-term monitoring, space-for-time substitutions along gradients, and in situ temperature manipulations are common approaches to understand effects of climate change on alpine and arctic plant communities. Although general patterns emerge from studies using different approaches, there are also some inconsistencies. To provide better estimates of plant community responses to future warming across a range of environments, there have been repeated calls for integrating different approaches within single studies. Thus, to examine how different methods in climate change effect studies may ask different questions, we combined three climate warming approaches in a single study in the Hengduan Mountains of southwestern China. We monitored plant communities along an elevation gradient using the space-for-time approach, and conducted warming experiments using open top chambers (OTCs) and plant community transplantation toward warmer climates along the same gradient. Plant species richness and abundances were monitored over 5 years addressing two questions: (1) how do plant communities respond to the different climate warming approaches? (2) how can the combined approaches improve predictions of plant community responses to climate change? The general trend across all three approaches was decreased species richness with climate warming at low elevations. This suggests increased competition from immigrating lowland species, and/or from the species already growing inside the plots, as indicated by increased biomass, vegetation height or proportion of graminoids. At the coldest sites, species richness decreased in OTCs and along the gradient, but increased in the transplants, suggesting that plant communities in colder climates are more open to invasion from lowland species, with slow species loss. This was only detected in the transplants, showing that different approaches, may yield different results. Whereas OTCs may constrain immigration of new species, transplanted communities are rapidly exposed to new neighbors that can easily colonize the small plots. Thus, different approaches ask slightly different questions, in particular regarding indirect climate change effects, such as biotic interactions. To better understand both direct and indirect effects of climate change on plant communities, we need to combine approaches in future studies, and if novel interactions are of particular interest, transplants may be a better approach than OTCs.

Benthic macrofauna is one of the most widely used biological groups to assess the ecological status of marine systems. Lately, attention has been paid to similar use of benthic foraminifera. In this study, distribution patterns of benthic foraminiferal and macrofaunal species were investigated simultaneously in 11 fjords in southeastern Norway in order to assess correlations and responses to environmental conditions. Selected fjords allowed to investigate contrasted environmental conditions from low total organic carbon (TOC) content (sediment TOC < 2.7%) in normoxia (bottom-water [O 2 ] > 2 mL O 2 .L -1 ) up to high TOC content (> 3.4%) in severe hypoxia (< 0.5 mL O 2 .L -1 ). Environmental parameters comprised bottom-water dissolved oxygen, grain size, total organic carbon, total nitrogen (TN), pigments and depth below threshold (DBT). Foraminiferal and macrofaunal community data were significantly correlated (Procrustes analysis m 2  = 0.66, p = 0.001). Hence, benthic foraminiferal distribution patterns mirror those of benthic macrofauna. However, as opposed to the foraminifera, macrofauna was not recorded at the most oxygen-depleted stations and, hence, was more sensitive to severe oxygen depletion. With regard to assigning species to ecological groups for ecological quality status assessment, the results suggest that species, e.g. Spiroplectammina biformis (foraminifera), Scalibregma inflatum (macrofauna), may exhibit different ecological requirements depending on their habitat. Considering the observed congruent patterns of benthic foraminifera and macrofauna, palaeo-communities of benthic foraminifera could be used as indicators of reference conditions for benthic macrofaunal community structure. Plant functional traits vary consistently along climate gradients and are therefore potential predictors of plant community response to climate change. We test this space-for-time assumption by combining a spatial gradient study with whole-community turf transplantation along temperature and precipitation gradients in a network of 12 grassland sites in Southern Norway. Using data on eight traits for 169 species and annual vegetation censuses of 235 turfs over 5 yr, we quantify trait-based responses to climate change by comparing observed community dynamics in transplanted turfs to field-parameterized null model simulations. Three traits related to species architecture (maximum height, number of dormant meristems, and ramet-ramet connection persistence) varied consistently along spatial temperature gradients and also correlated to changes in species abundances in turfs transplanted to warmer climates. Two traits associated with resource acquisition strategy (SLA, leaf area) increased along spatial temperature gradients but did not correlate to changes in species abundances following warming. No traits correlated consistently with precipitation. Our study supports the hypothesis that spatial associations between plant traits and broad-scale climate variables can be predictive of community response to climate change, but it also suggests that not all traits with clear patterns along climate gradients will necessarily influence community response to an equal degree., (© 2016 by the Ecological Society of America.)

The lack of a precisely-dated, unequivocal climate proxy from northern China, where precipitation variability is traditionally considered as an East Asian summer monsoon (EASM) indicator, impedes our understanding of the behaviour and dynamics of the EASM. Here we present a well-dated, pollen-based, ~20-yr-resolution quantitative precipitation reconstruction (derived using a transfer function) from an alpine lake in North China, which provides for the first time a direct record of EASM evolution since 14.7 ka (ka = thousands of years before present, where the "present" is defined as the year AD 1950). Our record reveals a gradually intensifying monsoon from 14.7-7.0 ka, a maximum monsoon (30% higher precipitation than present) from ~7.8-5.3 ka, and a rapid decline since ~3.3 ka. These insolation-driven EASM trends were punctuated by two millennial-scale weakening events which occurred synchronously to the cold Younger Dryas and at ~9.5-8.5 ka, and by two centennial-scale intervals of enhanced (weakened) monsoon during the Medieval Warm Period (Little Ice Age). Our precipitation reconstruction, consistent with temperature changes but quite different from the prevailing view of EASM evolution, points to strong internal feedback processes driving the EASM, and may aid our understanding of future monsoon behaviour under ongoing anthropogenic climate change.

Forest-dependent biodiversity is threatened throughout the tropics by habitat loss and land-use intensification of the matrix habitats. We resampled historic data on two moth families, known to play central roles in many ecosystem processes, to evaluate temporal changes in species richness and community structure in three protected forests in central Uganda in a rapidly changing matrix. Our results show some significant declines in the moth species richness and the relative abundance and richness of forest-dependent species over the last 20-40 years. The observed changes in species richness and composition among different forests, ecological types, and moth groups highlight the need to repeatedly monitor biodiversity even within protected and relatively intact forests.

Background: Effort-related thrombosis of the axillosubclavian vein (Paget-Schroetter syndrome, PSS) is uncommon. It tends to affect young, active individuals and yet consensus on management is lacking. The aim here was to analyse late outcomes in a series of patients treated for PSS using a standard protocol., Methods: The medical records of patients treated for PSS over 16 years were analysed. Patients were divided into four groups according to their management. Clinical and functional outcomes were analysed regarding residual venous defects after treatment, and Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire scores, respectively., Results: Some 117 patients (52 men and 65 women, mean age 32 years) were included. Catheter-directed thrombolysis was successful in 42 of 56 treated patients. Thoracic outlet decompression surgery was performed in 95 patients. DASH scores improved significantly in patients treated surgically (P < 0.001 to P = 0·009); early surgery had a better outcome than delayed surgery (P = 0·040). Patients who were managed conservatively showed no improvement (P = 0·116). Where venoplasty was necessary, it was successful in 18 of 25 patients. A short duration of lysis (less than 24 h) increased the risk of rethrombosis (P = 0·020). The method of postoperative anticoagulation had no influence on the rate of rethrombosis., Conclusion: Patients treated surgically for PSS had better functional outcomes than those managed conservatively. Private forests harbor considerable biodiversity, however, they are under greater threat than reserved areas, particularly from urbanization, agriculture, and intense exploitation for timber and fuel wood. The extent to which they may act as habitats for biodiversity and how level of protection impacts trends in biodiversity and forest structure over time remain underresearched. We contribute to filling this research gap by resampling a unique data set, a detailed survey from 1990 of 22 forests fragments of different ownership status and level of protection near Kampala, Uganda. Eleven of the 22 fragments were lost over 20 years, and six of the remnants reduced in size. Forest structure and composition also showed dramatic changes, with six of the remnant fragments showing high temporal species turnover. Species richness increased in four of the remaining forests over the resample period. Forest ownership affected the fate of the forests, with higher loss in privately owned forests. Our study demonstrates that ownership affects the fate of forest fragments, with private forests having both higher rates of area loss, and of structural and compositional change within the remaining fragments. Still, the private forests contribute to the total forest area, and they harbor biodiversity including IUCN "vulnerable" and "endangered" species. This indicates the conservation value of the fragments and suggests that they should be taken into account in forest conservation and restoration.

Background: Although arctic lakes have responded sensitively to 20(th)-century climate change, it remains uncertain how these ecological transformations compare with alpine and montane-boreal counterparts over the same interval. Furthermore, it is unclear to what degree other forcings, including atmospheric deposition of anthropogenic reactive nitrogen (Nr), have participated in recent regime shifts. Diatom-based paleolimnological syntheses offer an effective tool for retrospective assessments of past and ongoing changes in remote lake ecosystems., Methodology/principal Findings: We synthesized 52 dated sediment diatom records from lakes in western North America and west Greenland, spanning broad latitudinal and altitudinal gradients, and representing alpine (n = 15), arctic (n = 20), and forested boreal-montane (n = 17) ecosystems. Diatom compositional turnover (beta-diversity) during the 20(th) century was estimated using Detrended Canonical Correspondence Analysis (DCCA) for each site and compared, for cores with sufficiently robust chronologies, to both the 19(th) century and the prior approximately 250 years (Little Ice Age). For both arctic and alpine lakes, beta-diversity during the 20(th) century is significantly greater than the previous 350 years, and increases with both latitude and altitude. Because no correlation is apparent between 20(th)-century diatom beta-diversity and any single physical or limnological parameter (including lake and catchment area, maximum depth, pH, conductivity, [NO(3)(-)], modeled Nr deposition, ambient summer and winter air temperatures, and modeled temperature trends 1948-2008), we used Principal Components Analysis (PCA) to summarize the amplitude of recent changes in relationship to lake pH, lake:catchment area ratio, modeled Nr deposition, and recent temperature trends., Conclusions/significance: The ecological responses of remote lakes to post-industrial environmental changes are complex. However, two regions reveal concentrations of sites with elevated 20(th)-century diatom beta-diversity: the Arctic where temperatures are increasing most rapidly, and mid-latitude alpine lakes impacted by high Nr deposition rates. We predict that remote lakes will continue to shift towards new ecological states in the Anthropocene, particularly in regions where these two forcings begin to intersect geographically.

Knowledge and control of the polymorphic phases of chemical compounds are important aspects of drug development in the pharmaceutical industry. Salmeterol xinafoate, a long acting beta-adrenergic receptor agonist, exists in two polymorphic Forms, I and II. Raman and near infrared spectra were obtained of these polymorphs at selected wavelengths in the range of 488-1064 nm; significant differences in the Raman and near-infrared spectra were apparent and key spectral marker bands have been identified for the vibrational spectroscopic characterisation of the individual polymorphs which were also characterised with X ray diffractometry. The solid-state transition of salmeterol xinafoate polymorphs was studied using simultaneous in situ portable Raman spectroscopy and differential scanning calorimetry isothermally between transitions. This method assisted in the unambiguous characterisation of the two polymorphic forms by providing a simultaneous probe of both the thermal and vibrational data. The study demonstrates the value of a rapid in situ analysis of a drug polymorph which can be of potential value for at-line in-process control.

In contrast with macroorganisms, whose geographical ranges are typically restricted, many microbial species appear to have cosmopolitan distributions. This observation has been explained as a consequence of ubiquitous dispersal caused by the enormous population sizes of microbial species. Recently, this "everything is everywhere, but, the environment selects" theory has been challenged by the detection of considerable regional genetic variability within microbial morphospecies. We demonstrate that, contrary to what is expected under ubiquitous dispersal, evidence of regional-scale metacommunity processes can be detected in microbial morphospecies. Our results imply that the microbial and macrobial world are structured by analogous processes.

Pither & Aarssen (2005) propose a null model approach to assess the proportion of niche specialist taxa along ecological gradients. They apply this methodology to a large data set of lacustrine diatom assemblages and conclude that a majority of the taxa are generalists on a pH gradient. This conflicts with previous work, which shows that many diatom taxa have a statistically significant relationship with pH. We demonstrate the methods used by Pither & Aarssen (2005) have a high Type II error for rare taxa, and that this problem is compounded by the non-uniform sampling of the pH gradient which effectively precludes acid-lake specialist diatoms from being recognized as such. We re-analyse the data used by Pither & Aarssen (2005) and show that most of the diatoms have a statistically significant relationship with pH, and we thus refute their conclusions that few diatom species are specialists.

Acute normovolaemic haemodilution (ANH) is widely used as part of a blood conservation strategy to minimize the use of allogenic blood in the peri-operative period. Its role has not been proven in a prospective randomized trial. The potential benefits must not blind clinicians to the possible hazards. We report a life-threatening complication of ANH prior to induction of anaesthesia for aortic aneurysm repair.

Oxygen isotopes are sensitive tracers of climate change in tropical regions. Abrupt shifts of up to 18 per mil in the oxygen isotope ratio of diatom silica have been found in a 14,000-year record from two alpine lakes on Mt. Kenya. Interpretation of tropical-montane isotope records is controversial, especially concerning the relative roles of precipitation and temperature. Here, we argue that Holocene variations in delta(18)O are better explained by lake moisture balance than by temperature-induced fractionation. Episodes of heavy convective precipitation dated approximately 11,100 to 8600, 6700 to 5600, 2900 to 1900, and <1300 years before the present were linked to enhanced soil erosion, neoglacial ice advances, and forest expansion on Mt. Kenya.

We describe the successful use of a guide wire and ureteral dilator to pass a tracheal tube into the trachea through the mouth in an awake, sedated patient when attempts to pass a larger tube over an Olympus LF1 fibreoptic bronchoscope were unsuccessful. Since the ureteral and renal dilators are available in a variety of sizes, if too large a tube is inadvertently mounted on the instrument this technique allows an appropriate sized tube to be inserted without the need to re-endoscope the patient.

Thiopental and fentanyl are commonly combined for induction of anesthesia. The effect of an analgesic concentration of fentanyl on the plasma concentration of thiopental to induce sleep was studied in 46 unpremedicated patients. As a measure of drug effect, sleep (the lack of response to open eyes to a verbal command) was used. Forty-six patients were randomized to receive thiopental infused to one of several predetermined plasma concentrations. Twenty-two of these patients also received a fentanyl infusion to a desired analgesic concentration of 1 ng/mL. Thiopental and fentanyl were infused by means of a pharmacokinetic model-driven infusion device (computer-assisted continuous infusion, CACI). Venous blood samples were taken from the contralateral antecubital fossa at 5 and 10 min after the start of the infusion. At 10 min, the patients' names were firmly spoken, and they were instructed to open their eyes. If they did not respond to this command, they were considered to be asleep. Only patients in whom the 5- and 10-min measured plasma concentrations of thiopental and fentanyl, respectively, were within +/- 30% of each other were used for the determination of the Cp50(asleep), the plasma concentration at which 50% of the patients were asleep. The Cp50(asleep) with and without fentanyl was calculated by logistic regression. The Cp50(asleep) for patients in whom concentrations were maintained within +/- 30% for thiopental alone (n = 17) was 7.32 micrograms/mL (95% confidence interval, 5.53-10.95); for thiopental in the presence of fentanyl (n = 18 with a measured fentanyl concentration of 1.27 +/- 0.5 ng/mL), this was 7.22 micrograms/mL (95% confidence interval, 4.83-10.15).(ABSTRACT TRUNCATED AT 250 WORDS)

Observations were made during the use of a Tuohy needle to perform deliberate dural puncture for the insertion of lumbar drains to improve operating conditions for intracranial aneurysm surgery. The most striking finding was the distance from identification of the epidural space to penetration of the dura. We postulate that this was because of tenting of the dura by the blunt Tuohy needle. This was facilitated by the absence of a negative epidural space pressure because an open system was used, which allowed time for pressure equilibration. This minimised the reactive forces across the dura. Aspiration and rotation of the Tuohy needle revealed dural puncture in some cases.

Pressure recordings were made during passage of a Tuohy needle from the interspinous ligament to the subarachnoid space for lumbar drain insertion. Epidural space pressures were always positive. Negative pressures were seen only at the moment of entry into the subarachnoid space. These were artefactual and were caused by tenting of the dura by the blunt Tuohy needle. Use of a closed measurement system such as this facilitates the development of large transdural pressure gradients because of the inability of the epidural space pressure to equilibrate with atmospheric pressure. This may contribute to ease of dural puncture.

The importance of carbon dioxide in the control of ventilation during exercise was tested by emptying CO2 stores by voluntary hyperventilation. Healthy subjects were studied after 3 min hyperventilation down to an end-tidal PCO2 of about 20 mmHg on a background of steady exercise at 75 W. Control runs were performed when the hyperventilation was made isocapnic by the addition of CO2. Following hypocapnic hyperventilation, there was a period when ventilation fell below control and this was accompanied by a fall in end-tidal PO2 (minimum 48 mmHg) and oximeter reading (minimum 73%). Ventilation rapidly returned to baseline following isocapnic hyperventilation and hypoxia was not seen. A mathematical simulation suggested that brain PCO2 recovered more slowly than arterial PCO2 and that at the times that ventilation was depressed central chemoreceptor PCO2 would have been low. We conclude that CO2 provides a crucial drive for maintaining adequate ventilation during steady exercise and that the central chemoreceptor may be involved.

A case of anaesthesia in a patient with myasthenic syndrome treated with 3.4 diaminopyridine is described. Despite symptomatic improvement and an improved electromyogram (EMG) on treatment, extreme sensitivity to neuromuscular block occurred with vecuronium. Antagonism of block was poor with an anticholinesterase, while oral 3.4 diaminopyridine improved neuromuscular transmission further. A combination of anticholinesterase with aminopyridine may be the antagonistic combination of choice in this condition.

Pharmacokinetics and blood concentrations of bupivacaine were studied after intercostal nerve blocks were performed intraoperatively using 1.5 mg.kg-1 in 11 neonates (age 0-28 days) and 11 infants between age 1 and 6 months. The study aimed to provide pharmacokinetic data that are limited in these age groups, and to identify any adverse effects of intercostal nerve block in infancy. Arterial blood samples were taken at 0, 5, 10, 15, 20, 30, 60, 120, 240, and 360 min. Whole blood bupivacaine was assayed by high-performance liquid chromatography. Peak blood concentrations were attained within 10 min in 18 of 22 subjects, and were 087 micrograms.ml-1 [corrected] +/- 0.56 micrograms.ml-1 (mean and SD) and 0.91 +/- 0.27 micrograms.ml-1 in neonates and infants, respectively. Pharmacokinetic variables in the two groups included elimination half-life (t1/2 beta): 132 +/- 59 min and 102 +/- 39 min; steady-state volume of distribution (Vdss): 2.56 +/- 0.76 l.kg-1 and 2.17 +/- 0.17 l.kg-1; and total body clearance (Clt): 16.93 +/- 9.32 ml.min-1.kg-1 and 15.71 +/- 6.99 ml.min-1.kg-1. There was no statistically significant difference between neonates and infants with regard to any of these parameters. Patients were further divided into those with acyanotic and cyanotic disease. Cyanotic infants were significantly heavier than acyanotic infants (P less than 0.05), but no other differences were demonstrated. No adverse effects resulting from the technique were identified.(ABSTRACT TRUNCATED AT 250 WORDS)
