2. The French National Animal Health Surveillance Platform: an innovative, cross-sector collaboration to improve surveillance system efficiency in France and a tangible example of the One Health approach

Abstract

The French National Animal Health Surveillance Platform (NAHSP) was created in 2011. This network of animal health stakeholders was set up to improve surveillance efficiency for all health risks that threaten animal health, as well as zoonoses affecting human health. The NAHSP steering committee decides on the strategies and program of activities. It is composed of 11 institutions from both public and private sectors (policy-makers, scientific institutions, and representatives of farmers, veterinarians, hunters, and laboratories). A coordination team guarantees the implementation of the program and facilitates the activities of different working groups (WGs). Each WG is composed of technical experts with scientific, legal, and field knowledge from the sectors of animal health (livestock, companion animals, and wildlife), human health, and environmental health. Some WGs focus on a specific disease or health indicator, such as African swine fever or cattle mortality, while others cover cross-cutting topics, such as epidemic intelligence (EI), or specialize in aiding epidemiological investigations, such as the Q fever WG. The NAHSP stands out for its innovative approach because it is based on the concepts of consensus-building among participants, fostering collaboration, and embracing interdisciplinarity. Each proposal designed to improve surveillance is jointly developed by all the stakeholders involved, thereby ensuring its sustainability and acceptability among stakeholders. This process also has added value for decision-makers. As a pioneer platform, the NAHSP inspired the creation of two additional national surveillance platforms in 2018, one for plant health and the other for food chain safety. Both are organized in the same way as the NAHSP, which created a framework to place the emphasis on a One Health approach. For instance, four WGs are common to the three national surveillance platforms. This article aims to present this innovative approach to improve surveillance efficiency that could be of interest to other European countries or that could be rolled out at the European level.

Abstract

Deep convection plays a key role in the circulation, thermodynamics, and biogeochemical cycles in the Mediterranean Sea, which is considered to be a hotspot of biodiversity and climate change. In the framework of the DEWEX (Dense Water Experiment) project, the seasonal and annual budgets of dissolved inorganic carbon in the deep-convection area of the northwestern Mediterranean Sea are investigated over the period September 2012–September 2013 using a 3D coupled physical–biogeochemical–chemical modeling approach. At the annual scale, we estimate that the northwestern Mediterranean Sea's deep-convection region was a moderate sink of 0.5 mol C m−2 yr−1 of CO2 for the atmosphere. The model results show the reduction of oceanic CO2 uptake during deep convection and its increase during the abrupt spring phytoplankton bloom following the deep-convection events. We highlight the major roles in the annual dissolved inorganic carbon budget of both the biogeochemical and physical fluxes, which amount to −3.7 and 3.3 mol C m−2 yr−1, respectively, and are 1 order of magnitude higher than the air–sea CO2 flux. The upper layer (from the surface to 150 m depth) of the northwestern deep-convection region gained dissolved inorganic carbon through vertical physical transport and, to a lesser extent, oceanic CO2 uptake, and it lost dissolved inorganic carbon through lateral transport and biogeochemical fluxes. The region, covering 2.5 % of the Mediterranean, acted as a source of dissolved inorganic carbon for the surface and intermediate water masses of the Balearic Sea and southwestern Mediterranean Sea and could represent up to 22 % and 11 %, respectively, of the CO2 exchanges with the Atlantic Ocean at the Strait of Gibraltar.

Abstract

ABSTRACT Candida auris is a recently described emerging pathogen in hospital settings. Five genetic clades have been delineated, with each clade being isolated from specific geographic regions. We here describe the first transmission between 2 patients (P0 and P1) of a clade I C. auris strain imported into our burn intensive care unit from the Middle East. The strains have been investigated with whole-genome sequencing, which validated the high similarity of the genomes between isolates from P0 and P1. We repeatedly screened the two patients and contact patients (i.e., other patients present in the same hospital ward at the time of the first positive sample from P0 or P1; n = 49; 268 tests) with fungal culture and a C. auris-specific quantitative PCR assay to assess transmission patterns. We observed that P1 developed C. auris colonization between 41 and 61 days after potential exposure to P0 contamination, despite three negative screening tests as recommended by our national authorities. This study illustrates that transmission of C. auris between patients can lead to long-term incubation times before the detection of colonization. The recommended screening strategy may not be optimal and should be improved in the light of our findings. IMPORTANCE While large outbreaks of C. auris in hospital settings have been described, few clear cases of direct transmission have been documented. We here investigated the transmission of C. auris clade I between two patients with a 41- to 61-day delay between exposure and the development of colonization. This may lead to changes in the recommendations concerning treatment of C. auris cases, as an incubation period of this length is one of the first to be reported.

Abstract Equine trypanosomosis is a complex of infectious diseases called dourine, nagana and surra. It is caused by several species of the genus Trypanosoma that are transmitted cyclically by tsetse flies, mechanically by other haematophagous flies, or sexually. Trypanosoma congolense (subgenus Nannomonas) and T. vivax (subgenus Dutonella) are genetically and morphologically distinct from T. brucei, T. equiperdum and T. evansi (subgenus Trypanozoon). It remains controversial whether the three latter taxa should be considered distinct species. Recent outbreaks of surra and dourine in Europe illustrate the risk and consequences of importation of equine trypanosomosis with infected animals into non-endemic countries. Knowledge on the epidemiological situation is fragmentary since many endemic countries do not report the diseases to the World Organisation for Animal Health, OIE. Other major obstacles to the control of equine trypanosomosis are the lack of vaccines, the inability of drugs to cure the neurological stage of the disease, the inconsistent case definition and the limitations of current diagnostics. Especially in view of the ever-increasing movement of horses around the globe, there is not only the obvious need for reliable curative and prophylactic drugs but also for accurate diagnostic tests and algorithms. Unfortunately, clinical signs are not pathognomonic, parasitological tests are not sufficiently sensitive, serological tests miss sensitivity or specificity, and molecular tests cannot distinguish the taxa within the Trypanozoon subgenus. To address the limitations of the current diagnostics for equine trypanosomosis, we recommend studies into improved molecular and serological tests with the highest possible sensitivity and specificity. We realise that this is an ambitious goal, but it is dictated by needs at the point of care. However, depending on available treatment options, it may not always be necessary to identify which trypanosome taxon is responsible for a given infection.

22. Designing a Syndromic Bovine Mortality Surveillance System: Lessons Learned From the 1-Year Test of the French OMAR Alert Tool

Abstract

Between May 2018 and 2019, a syndromic bovine mortality surveillance system (OMAR) was tested in 10 volunteer French départements (French intermediate-level administrative unit) to assess its performance in real conditions, as well as the human and financial resources needed to ensure normal functioning. The system is based on the automated weekly analysis of the number of cattle deaths reported by renderers in the Fallen Stock Data Interchange Database established in January 2011. In our system, every Thursday, the number of deaths is grouped by ISO week and small surveillance areas and then analyzed using traditional time-series analysis steps (cleaning, prediction, signal detection). For each of the five detection algorithms implemented (i.e., the exponentially weighted moving average chart, cumulative sum chart, Shewhart chart, Holt-Winters, and historical limits algorithms), seven detection limits are applied, giving a signal score from 1 (low excess mortality) to 7 (high excess mortality). The severity of excess mortality (alarm) is then classified into four categories, from very low to very high, by combining the signal scores, the relative excess mortality, and the persistence of the signal(s) over the previous 4 weeks. Detailed and interactive weekly reports and a short online questionnaire help pilot départements and the OMAR central coordination cell assess the performance of the system. During the 1-year test, the system showed highly variable sensitivity among départements. This variability was partly due not only to the demographic distribution of cattle (very few signals in low-density areas) but also to the renderer's delay in reporting to the Fallen Stock Data Interchange Database (on average, only 40% of the number of real deaths had been transmitted within week, with huge variations among départements). As a result, in the pilot départements, very few alarms required on-farm investigation and excess mortality often involved a small number of farms already known to have health or welfare problems. Despite its perfectibility, the system nevertheless proved useful in the daily work of animal health professionals for collective and individual surveillance. The test is still ongoing for a second year in nine départements to evaluate the effectiveness of the improvements agreed upon at the final meeting.

25. Pratiques de logistique circulaire au sein des TPE et PME : L’approche des ressources et des compétences comme clé de lecture

Abstract

RÉSUMÉAlors que les politiques publiques et la communauté scientifique encouragent les pratiques d’économie circulaire, leur mise en œuvre opérationnelle reste à explorer dans les TPE et PME. Cette recherche exploratoire qualitative est basée sur l’étude de cas multiples par entretiens individuels semi-directifs menés auprès de 18 dirigeants de TPE et PME fortement impliquées dans l’économie circulaire. Nous étudions leur stratégie de circularité et plus précisément les ressources tangibles et intangibles qu’ils déploient. Nos résultats soulignent que ces TPE et PME disposent de compétences innovantes sur un marché de niche (compétences digitales, managériales, techniques). Ces dernières leur permettent de se démarquer de la concurrence par le biais des innovations dont elles sont porteuses. Par ailleurs, les activités liées au recyclage, la fabrication et la conception sont privilégiées au détriment d’autres maillons de la chaîne logistique circulaire (pratiques de partage ou de prêt, procédures de collecte, ou de reconditionnement après consommation). De par les difficultés opérationnelles rencontrées, liées notamment au financement, au développement d’une activité productive nouvelle ou d’un marché spécifique, nos résultats soulignent un réel besoin d’accompagnement des pouvoirs publics pour renforcer leur visibilité vis-à-vis des distributeurs, partenaires ou clients potentiels.

Abstract

This article explores the behaviour of small- and medium-sized enterprises (SMEs) regarding mandatory and voluntary measures established by the French government to reduce carbon dioxide (CO2) emissions generated by freight transport operations. Through semi-structured interviews with fourteen SMEs (five shippers, eight carriers and a consultant) located throughout France, this research examines the integration of sustainable development into organizational and decision-making practices since the introduction of these programmes on the French territory. Our qualitative study suggests that active environmental implications stem mainly from the company's internal dynamics, driven by its management, as well as end customers’ expectations. The voluntary policies seem to appeal more to SMEs than the mandatory measures implemented since 2013. This research shows that the carriers surveyed are highly environmentally proactive, regardless of their size. It also sheds light on techniques that could increase the efficiency and widespread adoption of governmental measures, in particular through the increasing use of on-board telematics., Cet article explore le comportement des petites et moyennes entreprises (PME) suite aux dispositifs obligatoires et volontaires mis en place par le gouvernement français pour réduire les émissions de CO2 générées par le transport de marchandises. Grâce à des entretiens semi-directifs réalisés auprès de quatorze entreprises réparties sur le territoire français (cinq chargeurs, huit transporteurs et un consultant), nous examinons la prise en compte du développement durable dans les pratiques organisationnelles et décisionnelles des PME depuis l’apparition de ces dispositifs. Notre étude qualitative suggère que les implications environnementales actives découlent principalement de la dynamique interne de l'entreprise, pilotée par sa direction, ainsi que des attentes des clients finaux. Ce sont les démarches volontaires qui semblent séduire davantage les PME par rapport aux dispositifs obligatoires mis en place depuis 2013. Nous identifions une forte proactivité environnementale des transporteurs interrogés, quelle que soit leur taille. Notre travail apporte également un éclairage sur les techniques qui permettraient d’accroître l’efficacité et l’adoption des dispositifs gouvernementaux, notamment via une utilisation croissante de la télématique embarquée.

30. Context of instauration and evolution under isavuconazole, a 2-year prospective real-life study

Abstract

To describe reasons for initiation and evolution under isavuconazole (ISZ), a 2-year prospective and observational study was performed. Anonymized data collected during the first 3 months of treatment were indications of treatment, efficacy, overall survival (OS), evolution of toxicity markers, and ISZ trough levels. Fifty-one (26 invasive aspergillosis, 16 prophylaxis, and 9 mucormycosis) patients started on isavuconazole. Isavuconazole was initiated upfront in 12/51 cases, especially to avoid toxicities from other antifungals. As second-line therapy (39/51 patients), isavuconazole was mostly initiated after toxicities of the previous treatments (66.7%; 26/39 cases). An improvement in toxicity markers was reported in most patients. However, five patients experienced adverse events. The mean ISZ trough levels measured from 179 samples collected in 37 patients was 3.33 ± 1.64 mg/l. The mean ISZ through levels was significantly lower (P = .003) in alloHSCT recipients (3.10 ± 1.45 mg/l) than in other patients (3.76 ± 1.88 mg/l) but still within the expected range of efficacy. After 12 weeks, the OS was 69.2% (n = 18/26) in the invasive aspergillosis intention-to-treat (ITT) group and 44.4% (n = 4/9) in the mucormycosis ITT group. After 2 years, the OS was respectively 46.2% (n = 12/26) and 33.3% (n = 3/9) in these two groups.

Abstract

Over the past three decades, a variety of programs have conducted extensive measurements of ocean properties at fixed stations throughout the water column, as well as in the surface ocean via oceanographic ships and ships of opportunity. Ships of opportunity were particularly used to determine the air-sea CO2 fluxes from automated measurements of sea-surface temperature, salinity, and CO2 fugacity. These underway measurements, often recorded at a frequency of every minute, generate large data files that need to be quality controlled, stored and analyzed. For practical use these data are often binned by 1° latitude x 1° longitude. Unfortunately, by doing so, there is a consequential loss of accuracy for these data sets. Here, using the original 2010 underway data sets of sea-surface temperature, sea-surface salinity, total alkalinity and total inorganic carbon, along the cruise track from Hobart (Tasmania) to Dumont D'Urville (Antarctica), we show what would had been a more appropriate sampling strategy for each of these properties, maintaining their full measurement accuracy, while improving their interpolation accuracy. Furthermore, this analysis illustrates a general methodology for objectively determining, under suitable conditions, the appropriate locations for each property measurement according to a required accuracy. These results should greatly facilitate future cruise preparation and reduce the cost of measurements, while improving their scientific value.

Abstract

International audience; The French National Reference Center for Invasive Mycoses and Antifungals leads an active and sustained nationwide surveillance program on probable and proven invasive fungal diseases (IFDs) to determine their epidemiology in France. Between 2012 and 2018, a total of 10,886 IFDs were recorded. The incidence increased slightly over time (2.16 to 2.36/10,000 hospitalization days, P = 0.0562) in relation with an increase of fungemia incidence (1.03 to 1.19/10,000, P = 0.0023), while that of other IFDs remained stable. The proportion of ≥65-year-old patients increased from 38.4% to 45.3% (P < 0.0001). Yeast fungemia (n = 5,444) was due mainly to Candida albicans (55.6%) with stable proportions of species over time. Echinocandins became the main drug prescribed (46.7% to 61.8%), but global mortality rate remained unchanged (36.3% at 1 month). Pneumocystis jirovecii pneumonia (n = 2,106) was diagnosed mostly in HIV-negative patients (80.7%) with a significantly higher mortality than in HIV-positive patients (21.9% versus 5.4% at 1 month, P < 0.0001). Invasive aspergillosis (n = 1,661) and mucormycosis (n = 314) were diagnosed mostly in hematology (>60% of the cases) with a global mortality rate of 42.5% and 59.3%, respectively, at 3 months and significant changes in diagnosis procedure over time. More concurrent infections were also diagnosed over time (from 5.4% to 9.4% for mold IFDs, P = 0.0115). In conclusion, we observed an aging of patients with IFD with a significant increase in incidence only for yeast fungemia, a trend toward more concurrent infections, which raises diagnostic and therapeutic issues. Overall, global survival associated with IFDs has not improved despite updated guidelines and new diagnostic tools.IMPORTANCE The epidemiology of invasive fungal diseases (IFDs) is hard to delineate given the difficulties in ascertaining the diagnosis that is often based on the confrontation of clinical and microbiological criteria. The present report underlines the interest of active surveillance involving mycologists and clinicians to describe the global incidence and that of the main IFDs. Globally, although the incidence of Pneumocystis pneumonia, invasive aspergillosis, and mucormycosis remained stable over the study period (2012 to 2018), that of yeast fungemia increased slightly. We also show here that IFDs seem to affect older people more frequently. The most worrisome observation is the lack of improvement in the global survival rate associated with IFDs despite the increasing use of more sensitive diagnostic tools, the availability of new antifungal drugs very active in clinical trials, and a still low/marginal rate of acquired in vitro resistance in France. Therefore, other tracks of improvement should be investigated actively.

45. Climatological variations of total alkalinity and total dissolved inorganic carbon in the Mediterranean Sea surface waters

Abstract

A compilation of data from several cruises between 1998 and 2013 was used to derive polynomial fits that estimate total alkalinity (AT) and total dissolved inorganic carbon (CT) from measurements of salinity and temperature in the Mediterranean Sea surface waters. The optimal equations were chosen based on the 10-fold cross-validation results and revealed that second- and third-order polynomials fit the AT and CT data respectively. The AT surface fit yielded a root mean square error (RMSE) of ± 10.6 μmol kg−1, and salinity and temperature contribute to 96 % of the variability. Furthermore, we present the first annual mean CT parameterization for the Mediterranean Sea surface waters with a RMSE of ± 14.3 μmol kg−1. Excluding the marginal seas of the Adriatic and the Aegean, these equations can be used to estimate AT and CT in case of the lack of measurements. The identified empirical equations were applied on the 0.25° climatologies of temperature and salinity, available from the World Ocean Atlas 2013. The 7-year averages (2005–2012) showed that AT and CT have similar patterns with an increasing eastward gradient. The variability is influenced by the inflow of cold Atlantic waters through the Strait of Gibraltar and by the oligotrophic and thermohaline gradient that characterize the Mediterranean Sea. The summer–winter seasonality was also mapped and showed different patterns for AT and CT. During the winter, the AT and CT concentrations were higher in the western than in the eastern basin. The opposite was observed in the summer where the eastern basin was marked by higher AT and CT concentrations than in winter. The strong evaporation that takes place in this season along with the ultra-oligotrophy of the eastern basin determines the increase of both AT and CT concentrations.

The studied transect is divided in four regions from North to South: the Subtropical Zone (STZ), the Subantarctic Region (SAR), the Antarctic Region (AAR) and the Coastal Antarctic Zone (CAZ). Latitudinal distribution of measured SST, SSS, AT, CT as well as calculated pH, CO2 parameters (seawater fugacity of CO2 (fCO2sw), difference between seawater and atmospheric fCO2 (ΔfCO2), CO2 flux (FCO2)) and satellite-derived Chlorophyll a (Chl-a) are discussed. We show that the variability of physical and carbonate parameters in the STZ and north of the SAR are related to the mesoscale activity. In the CAZ, the freshwater inputs from sea-ice melting strongly impact the variability of all parameters. The comparison between physical and carbonate parameters highlights that AT and CT are directly related to the latitudinal variability of SST and SSS. Study of the CO2 parameters shows that the transect is a sink of CO2 during February and March, with a mean FCO2 of -4.0 ± 2.8 mmol m-2 d-1. The most negative values of FCO2 are found in the STZ and SAR north of 50°S and in the AAR south of 62°S, where biological activity is high. New simple empirical relationships are developed for AT from SST and SSS and for CT using SST, SSS and atmospheric fCO2 (fCO2atm) for the austral summer in the studied area. Using high resolution SSS and SST from the SURVOSTRAL program, trends of AT and CT are determined in the SAR and the AAR from 2005 to 2019. SST, SSS and AT increase over this period in the SAR, which might be explained by the southward migration of the Subtropical Front. In the AAR, no clear trend is detected. CT increases by 1.0 ± 0.2 and 0.8 ± 0.3 μmol kg-1 yr-1 in the SAR and AAR respectively. The trend in the AAR is attributed to the increase in anthropogenic CO2 emissions in the atmosphere while, in the SAR, hydrographic changes also contribute to the increase. Using the coefficient associated with fCO2atm in the equation of CT, we estimate the impact of atmospheric CO2 increase on CT at 1.18 ± 0.14 μmol kg-1 yr-1 and 1.07 ± 0.13 μmol kg-1 yr-1 in the SAR and AAR respectively. Decreases in pH are observed in both regions (-0.0018 ± 0.0001 and -0.0026 ± 0.0003 yr-1 in the SAR and AAR respectively), indicating the sensitivity of surface waters in the area towards the development of ocean acidification processes under rising anthropogenic emissions.

50. Circulating Aspergillus fumigatus DNA Is Quantitatively Correlated to Galactomannan in Serum

Abstract

The performance of antigen galactomannan (GM) for diagnosing invasive aspergillosis (IA) is hampered by the occurrence of false-positive results. Quantitative PCR has been proposed to improve the diagnosis of IA. Therefore, we analyzed the value of performing a PCR test to the GM-positive serum sample. Using a quantitative PCR assay specific for Aspergillus fumigatus 28S ribosomal DNA, we retrospectively tested 422 GM-positive (Platelia Bio-Rad kit) serum samples collected over 1 year from 147 patients. The cases were classified based on EORTC criteria as “proven,” “probable,” and “no–IA” before availability of the PCR results. After exclusion of 65 samples for non-reproducibility of GM positivity (n = 62) or PCR inhibition (n = 3), 75 (21.0%) of the remaining 357 samples were PCR-positive. GM and fungal DNA showed a significantly positive correlation (p < 0.0001, R2 = 0.27, slope = 0.98 ± 0.19). At least one PCR-positive result was observed in 63.3% (31/49) of IA patients and in 13.2% (13/98) of non-IA patients (p < 0.0001). The PCR positivity was also associated with the presence of other microbiological criteria among the 44 patients with IA and complete mycological workup (p = 0.014), as well as a higher mortality rate at six months among the 135 patients with hematological conditions (p = 0.0198). Overall, we found a quantitative correlation between serum GM and circulating DNA with an increased likelihood of IA when both were positive. A PCR-positive result also supported a higher fungal load when GM was already positive. We advocate adding a PCR test for every confirmed GM-positive serum sample.
