Abstract

Distribution Static Compensator (D-STATCOM) is a shunt compensation device used to improve electric power quality in distribution systems. It is well-known that D-STATCOM is a nonlinear, semi-defined and time-varying system. Therefore, control of D-STATCOM by the conventional control techniques is very difficult task. In this paper, the control of D-STATCOM is carried out by the neuro-fuzzy controller (NFC) which has non-linear and robust structure. For this aim, an experimental setup based on three-level H-bridge inverter is constructed. Phase angle control method is used for control of D-STATCOM's output reactive power. Control algorithm for this experimental setup is prepared in MATLAB/Simulink and downloaded to DS1103 controller card. A Mamdani type NFC is designed for control of D-STATCOM's reactive current. Output of NFC is integrated to increase tracking performance of controller in steady state. The performance of D-STATCOM is experimentally evaluated by changing reference reactive current as on-line. The experimental results show that the proposed controller gives very satisfactory performance under different loading conditions.

To develop a patient-centric approach for botulinum toxin injections in facial aesthetics, a group of clinicians met to provide opinions on an optimal treatment journey that uses PROs to quantify treatment benefits on patient quality of life.Patients and Methods: A multidisciplinary panel of 9 clinicians with expertise in facial aesthetic procedures convened for an advisory board that was preceded by and followed up with a structured, multistep consensus discussion. Based on current literature, the panel’s expertise, structured questions, and group discussion, panelists assessed, reconciled, and agreed upon on a patient journey for botulinum toxin treatment in facial aesthetics.Results: Panelists agreed that an optimal patient journey includes screening, assessment, treatment, posttreatment, and follow-up visits. A compact, easy-to-complete, and digital PRO questionnaire should be provided before the visit. During screening, thorough assessments are integral for a successful patient journey because they provide an opportunity to understand treatment goals, address patient concerns, discuss risks and benefits, obtain medication lists/medical history, and take pretreatment photographs. Treatment strategies should include discussing and educating on the approach/choice of botulinum toxin and ensuring patients are comfortable. Posttreatment, clinicians should request intense muscle movements to enhance product uptake and be available to address patient concerns. Finally, during follow-up, PRO questionnaires can be provided to gauge patient satisfaction with treatment, and pretreatment photographs can be provided to allow patients to track their progression. Follow-ups should be scheduled with new patients or those reporting low satisfaction.Conclusion: Establishing a relationship, being aware of the patient’s goals, and developing an individualized care plan allows for a structured, patient-centered treatment journey that promotes positive aesthetic outcomes.Keywords: patient satisfaction, patient-reported outcome measures, neurotoxins, body image, skin aging

Abstract

BACKGROUND: Retinoblastoma is the most common intraocular cancer worldwide. There is some evidence to suggest that major differences exist in treatment outcomes for children with retinoblastoma from different regions, but these differences have not been assessed on a global scale. We aimed to report 3-year outcomes for children with retinoblastoma globally and to investigate factors associated with survival. METHODS: We did a prospective cluster-based analysis of treatment-naive patients with retinoblastoma who were diagnosed between Jan 1, 2017, and Dec 31, 2017, then treated and followed up for 3 years. Patients were recruited from 260 specialised treatment centres worldwide. Data were obtained from participating centres on primary and additional treatments, duration of follow-up, metastasis, eye globe salvage, and survival outcome. We analysed time to death and time to enucleation with Cox regression models. FINDINGS: The cohort included 4064 children from 149 countries. The median age at diagnosis was 23·2 months (IQR 11·0-36·5). Extraocular tumour spread (cT4 of the cTNMH classification) at diagnosis was reported in five (0·8%) of 636 children from high-income countries, 55 (5·4%) of 1027 children from upper-middle-income countries, 342 (19·7%) of 1738 children from lower-middle-income countries, and 196 (42·9%) of 457 children from low-income countries. Enucleation surgery was available for all children and intravenous chemotherapy was available for 4014 (98·8%) of 4064 children. The 3-year survival rate was 99·5% (95% CI 98·8-100·0) for children from high-income countries, 91·2% (89·5-93·0) for children from upper-middle-income countries, 80·3% (78·3-82·3) for children from lower-middle-income countries, and 57·3% (52·1-63·0) for children from low-income countries. Abstract

Sharks are among the oldest residents of the planet, they possess a unique value as top predators and constitute irreplaceable elements of marine ecosystems. Unfortunately, contemporary narratives widely presented in popular mainstream media have attached an utterly negative connotation to sharks, propagating an unsubstantiated and fabricated image of them as implacable and voracious predators. Recently a lot of attention is devoted to understanding the public perception towards sharks in order to promote their conservation given that a quarter of all shark species are facing extinction. This work assessed the current attitude of the public towards sharks on a global scale, utilizing modern technology through a single protocol that explored the importance of factors like culture, history, or educational level in shaping attitudes. We collected 13,800 questionnaires from 137 countries, with 25 countries presenting more than 100 answers each, representing in total 92% of the filled questionnaires. A generally positive attitude towards sharks emerged from our study, influenced significantly by several factors including knowledge and participation in marine conservation projects. Interestingly, shark attacks emerged as an important factor, with countries having high numbers of shark attacks exhibiting a highly positive attitude towards sharks, potentially because their citizens are more aware of the issue and the importance of sharks for the marine ecosystems. Guidelines for shifting public attitude towards sharks and consequently advancing shark conservation were also drawn.

10. Taking the next step in a collaborative project: a multimodal analysis of verbal and embodied actions at the computer

Abstract

This dissertation examines social and embodied actions used by participants in collaborative project-making at the computer. More specifically, this monograph studies the multimodal aspects of some recurring practices used by participants to take the next step in a collaborative setting. The practices studied are: first, proposing a next action, and displaying it as immediate or remote, second, pointing at or “in” the screen with physical and digital pointing gestures and establishing a joint focus of attention, and third, going from information-searching to typing. The analyses are carried out using multimodal Conversation Analysis as a methodology, and the data consist of 9 hours of English and French video-recorded data of university students working as pairs at the computer. The first part of the study examines the formats of proposals, and the embodied actions deployed along with them, produced by participants to propose a next action. Observations of the data show that three formats are predominantly used: Should/Shall we X, Let’s X, and Do we X. The analyses demonstrate that participants use the formats Should/Shall we X and Let’s X to propose immediate actions, and the format Do we X to propose remote actions. The analyses also put forward the role played by embodied actions to display the nature of the proposed next action. The second part studies physical and digital pointing gestures at or “in” the screen, and how referrers assess the establishment of a joint focus of attention after a pointing gesture. The analyses demonstrate that referrers rely on the sequential relevance of the recipients’ second pair part to establish whether a joint focus of attention has been established. Moreover, this part provides insight into the use of the mouse cursor as a resource to produce intelligible actions through digital pointing and highlighting gestures. The third part examines the unfolding of multimodal actions routinely employed by participants to search, Tiivistelmä Tässä väitöskirjassa tutkitaan vuorovaikutustilanteita, joissa kaksi henkilöä tekee yhdessä tietokoneen ääressä PowerPoint-esitystä. Tutkimus keskittyy puheen ja kehon sosiaalisiin toimintoihin, joita osallistujat käyttävät esityksen tekemiseen. Työ kuvaa tarkemmin kolmea vuorovaikutuskäytännettä. Ensimmäiseksi työssä kuvataan seuraavan toiminnan ehdottamista ja ehdotuksen merkitsemistä joko välittömästi tai myöhemmin toteutettavaksi. Toiseksi työ keskittyy tietokoneen ruudulla näkyvän asian osoittamiseen. Abstract

Derin öğrenme, makine öğrenmesinin bir alt çalışma alanı olup son yıllarda oldukça popüler hale gelmiştir. Derin öğrenme gerçek hayatta karşılaşılan problemlerde oldukça geniş bir uygulama alanına sahiptir. Özellikle çözümü zor ve karmaşık olan çok farklı alanlardaki problemlerin çözümünde kullanılmakta ve genellikle çok başarılı sonuçlar elde edilmektedir. Bu şekilde bilgisayarlar için oldukça zor bir problem olan nesne tanıma problemlerinde, derin öğrenme algoritmaları oldukça başarılı sonuçlar üretmektedir. Nesne tanımlama aslında, bir nesnenin eldeki görüntü içerisinde görüntünün neresinde olduğunu tespit ettikten sonra, bu sınırlar içerisindeki nesnenin görsel özelliklerinin kullanılması ile gerçekleştirilen eğitmenli bir sınıflandırma problemidir. Hâlihazırda nesne tanıma uygulamaları için en çok kullanılan derin öğrenme algoritması Evrişimsel Sinir Ağı (Convolutional Neural Network(CNN)), uzun yıllardır bilinen ve çok fazla uygulama alanına sahip çok katmanlı sinir ağlarının geliştirilmiş halidir. Evrişimsel Sinir Ağı, görüntünün segmentasyonu ve sınıflandırılması gibi klasik görüntü işleme problemlerinde şimdiye kadar çok iyi sonuçlar sergilemiştir. Bu çalışmada, nesne tanıma uygulamasına yönelik literatürdeki derin öğrenme yöntemleri kullanan çalışmalar incelenerek, bu yöntemlerin çeşitli açılardan kıyaslanması ve kullanılmalarına yönelik değerlendirmeler yapılmıştır.

Abstract

IMPORTANCE: Early diagnosis of retinoblastoma, the most common intraocular cancer, can save both a child's life and vision. However, anecdotal evidence suggests that many children across the world are diagnosed late. To our knowledge, the clinical presentation of retinoblastoma has never been assessed on a global scale. OBJECTIVES: To report the retinoblastoma stage at diagnosis in patients across the world during a single year, to investigate associations between clinical variables and national income level, and to investigate risk factors for advanced disease at diagnosis. DESIGN, SETTING, AND PARTICIPANTS: A total of 278 retinoblastoma treatment centers were recruited from June 2017 through December 2018 to participate in a cross-sectional analysis of treatment-naive patients with retinoblastoma who were diagnosed in 2017. MAIN OUTCOMES AND MEASURES: Age at presentation, proportion of familial history of retinoblastoma, and tumor stage and metastasis. RESULTS: The cohort included 4351 new patients from 153 countries; the median age at diagnosis was 30.5 (interquartile range, 18.3-45.9) months, and 1976 patients (45.4%) were female. Most patients (n = 3685 [84.7%]) were from low- and middle-income countries (LMICs). Globally, the most common indication for referral was leukocoria (n = 2638 [62.8%]), followed by strabismus (n = 429 [10.2%]) and proptosis (n = 309 [7.4%]). Patients from high-income countries (HICs) were diagnosed at a median age of 14.1 months, with 656 of 666 (98.5%) patients having intraocular retinoblastoma and 2 (0.3%) having metastasis. Patients from low-income countries were diagnosed at a median age of 30.5 months, with 256 of 521 (49.1%) having extraocular retinoblastoma and 94 of 498 (18.9%) having metastasis. Abstract

This article builds on ethnomethodological, conversation analytic research on object transfers: how participants hand over objects to one another. By analyzing video recordings of mundane (cars) and institutional interactions (laboratories), we focus on situations where an object is central to and talked about in the joint course of action. We focus on different organizations of object transfer and show that one embodied move is decisive, either a sequentially implicative ‘give’ or an arm extension designed as a stand-alone ‘take’. We examine the interrelationship between the organization of the object transfer and the broader course of action (e.g. request or offer sequence), which is either overlapping or intersecting. We demonstrate that by making the decisive move, either the participant initially holding the object or her recipient critically influences the progression and trajectory of the activity, and displays agency.

Abstract

This long-lived bivalve is endemic to the Mediterranean Sea, where it has a range from Spain to Turkeyalong the northern and southern coasts and coasts of the Mediterranean islands. Since 2016, adevastating and geographically widespread mass mortality event (MME) has impacted P. nobilispopulations throughout the Mediterranean Sea. Previous to the MME, the species was widespread andlocally abundant in some locations. The mortality is caused by a pathogen ( H. pinnae ) and theassociated die-offs have rapidly spread from the western (starting in Spain) to the easternMediterranean Sea in less than three years, causing mortality rates of 80-100% of the individuals inmost locations, including those with long-term P. nobilis monitoring programmes. There are a fewpopulations (less than ten subpopulations) that are known to remain pathogen-free and these aregeographically isolated and located in sites characterized by very specific environmental conditions(lagoons with little access to the sea and differing salinities).

Abstract

This article starts from the observation that social scientists using video to study naturally-occurring interactions are often questioned about the reliability of their data, by wider audiences, but also by scholars who raised concerns early on about how the recording device would modify the participants' conduct. The study uses 47 video extracts in which workers filmed in their offices orient to the recording, analyzed from a conversation analysis perspective. I show that these sequences occur in two distinct sequential environments, corresponding to distinct sets of accomplishments. During the openings of encounters participants often discuss the meaning and features of the recording, and close the topic as they reach a form of agreement. I outline a pattern for such sequences. During the course of an encounter, they often use the recording not only as a resource to produce laughter in general, but also to achieve locally and sequentially relevant actions, such as closing a complaint or assessing an activity. By exposing the methods whereby participants "domesticate" the recording, I argue that while the recording is a specific circumstance that participants are aware of, and which requires some negotiations, which in turn may change their interactions, it nonetheless provides rich analytic material. Ich zeige, dass diese Bezugnahmen in zwei unterschiedlichen sequenziellen Kontexten stattfinden, die wiederum mit verschiedenen Arbeitsabläufen korrespondieren. Zu Beginn sprechen Teilnehmende oft über die Aufzeichnung, dies endet, wenn eine Art wechselseitiger Verständigung erreicht wurde; das Muster dieser initialen Bezugnahmen veranschauliche an einigen Beispielen. Im Verlauf der Aufzeichnung folgen dann zumeist humorvolle Bezugnahmen auf die Kamera, die zudem lokal und im Verlauf relevante Handlungen markieren, z.B. den Beginn oder das Ende einer Handlungskette. Ich zeige hier, wie Teilnehmende die Kamera "domestizieren", d.h. in welcher Weise sie sich der Videoaufzeichnungen bewusst und diese Gegenstand von Aushandlungsprozessen sind, die den Verlauf der Interaktionen ändern können, dass aber zugleich wichtiges Material resultiert: Zum einen hat die Studie ethische Implikationen, da sie die Erwartungen der Teilnehmenden in Bezug auf Datenschutz verdeutlicht und wie dieser kontinuierlich in ihren Interaktionen wieder relevant wird. Refractory infections are not common in head and neck region. As gossypiboma (retained surgical sponges) is also rare in head and neck surgeries, it is generally ignored as a potential diagnosis. In this article we aimed to call attention to gossypiboma cases in refractory maxillary infections. We present three cases of retained surgical sponges after head and neck surgery occurred in between 2003 and 2011. Abstract

Objective: The aim of the study was to investigate the prevalence of endometrial polyps in patients with uterine fibroids and associated factors of coexistence of these two pathologies. Materials and Methods: The medical records of 772 patients who underwent hysterectomy because of uterine fibroids were retrospectively reviewed. Patients were divided into two groups according to the presence of endometrial polyps in the histopathologic examination. Demographic, clinical and histopathologic findings of the patients with and without endometrial polyps were compared. Student’s t-test, Mann-Whitney U test, Pearson’s Chi-square test, and logistic regression analysis were used for statistical analysis. Results: The prevalence of the endometrial polyps in uterine fibroid cases was found 20.1% (n=155). Abstract

V elektrofyziologických studiích je substituce sodíku používaná jako metoda v extracelulárním prostředí. N-methyl-D-glukamin (megulamin, NMG) je pomocnou látkou díky své farmakologicky neaktivní povaze, která umožňuje blokovat proud Na+ na buněčné úrovni. V této studii jsme zkoumali změny týkající se vlivu skupin vláken na složený akční potenciál (compound action potential; CAP) během náhrady NMG. Náhrada vedla k významnému poklesu jak amplitudy, tak plochy CAP u každé nahrazené skupiny. Úplná náhrada nezmenšila plochu CAP v porovnání s částečnou náhradou. Bylo prokázáno, že různé poměry nahrazení Na+ v extracelulárním médiu s NMG způsobují změny v aktivitách ně-kte-rých nervových vláken, stejně jako blokádu vedení. Závěry jsou získané specifickou metodou výpočtu distribuce rychlosti nervového vedení. Částečná náhrada extracelulárního Na ovlivňuje rychle vodivé skupiny vláken, zatímco úplná náhrada ovlivňuje pomalu vodivé skupiny vláken. Autoři deklarují, že v souvislosti s předmětem studie nemají žádné komerční zájmy. Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů. PubMed: 22399124, Objectives: Intraoperative remifentanil administration results in acute opioid tolerance that is manifested by increased postoperative pain, opioid requirement and specifically periincisional hyperalgesia. The aim of this study was to investigate the effect of magnesium in preventing remifentanil-induced hyperalgesia. Methods: This study was performed on 60 (ASA I-II) patients planned for abdominal hysterectomy. Sixty patients were randomized into two equal groups. Before anesthesia, saline solution was given to the patients in group I (control group), 50 mg/kg i.v. magnesium in group II (magnesium group). Anesthesia was induced with 1 ?g/kg remifentanil combined with 4-5 mg/kg thiopental and 0.5 mg/kg atracurium, maintained with O.S MAC sevoflurane and 0.4 ?g/kg/min remifentanil in both groups. Sevoflurane concentration was titrated according to autonomic responses. Thirty minutes before the anticipated end of surgery, a 0.15 mg/kg bolus dose of morphine was intravenously. At the end of surgery, patients received tramadol i.v via a PCA device. Pain score, tramadol demand and delivery were assessed at 2, 4, 6, 12, 2 4 h after surgery. Total tramadol consumption were recorded for 24-48 h after surgery. Periincisional hyperalgesia was assessed by measuring pain threshold to pressure by using an algometer and electronic von Frey filaments before operation and at 28-48 h postoperatively. Abstract

Aim: DNA extraction is an important step from clinical samples for molecular diagnosis of tuberculosis by PCR. The aim of this study was to evaluate the efficacy of five DNA extraction methods [boiling, single step proteinase K, GuSCN lysis and izopropanol precipitation (Heliosis, METİS Company, TURKEY), DNA precipitation (Epicentre Technologies), and solid phase absorption (QIAamp DNA mini kit, QIAGEN, Valancia,CA)] in searching Mycobacterium tuberculosis DNA in smear positive sputum samples. Materials and Methods: A total of 50 sputum samples were extracted directly and after digested with 4% NaOH-NALC methods using 5 DNA extraction methods. All DNA extracts were studied by an in-house PCR method. Results: The rate of the positive detection for 5 extraction methods was 22% with boiling method, 38% with single step proteinase K, 38% with guanidium isothiocyanate lysis and isopropanol precipitation method (Heliosis, METIS ), 42% with DNA precipitation (Epicentre Technologies), and 58% with solid phase absorption (QIAamp). Conclusions: When the rate of positive detection is taken into consideration in smear positive patients, solid phase absorption method seems to be more proper to use routinely for DNA isolation from clinical samples.

49. COMPARATIVE ANALYSIS OF VARIOUS FLEECE CHARACTERISTICS OF NORDUZ AND ZOM SHEEP.

Abstract

This research aimed to evaluate the fleece samples obtained from the shoulder, rump and flank areas of Norduz and Zom sheep. The study consisted of 25 Norduz sheep and 24 Zom sheep. The longest hauteur fleece in Norduz and Zom sheep was obtained from the rump area (40.30±2.96 and 30.11±1.50 mm) and the longest barbe fleece was obtained from the rump area (58.22±4.70 and 42.28±3.10 mm). Upon examination of the fiber diameter of the fleece of the Norduz and Zom sheep, the thickest fleece was found in the rump area (35.73±1.13 and 33.01±0.86 μ), with the fleece in the other areas finer and closer together. In both breeds (Norduz and Zom), the fleece obtained from the flank had the highest elasticity (32.17±0.83 and 31.44±1.01%), and the fleece obtained from the rump had the highest strength (33.91±2.03 and 24.33±2.03 cN/tex). In both the Norduz and Zom sheep, the highest fleece yields were obtained from the flanks: 63.58±1.52 and 62.96±2.27%. This study revealed no marked variation in the fleece yields and characteristics of the Norduz and Zom sheep, which are thought to be a variety of the White Karaman sheep. [ABSTRACT FROM AUTHOR]
