Abstract

Objective: The soluble guanylate cyclase stimulator riociguat is approved for the treatment of adult patients with pulmonary arterial hypertension (PAH) and inoperable or persistent/recurrent chronic thromboembolic pulmonary hypertension (CTEPH) following Phase 3 randomized trials. The EXPosurE Registry RiociguaT in patients with pulmonary hypertension (EXPERT) study was designed to monitor the long-term safety of riociguat in clinical practice. Methods: EXPERT was an international, multicenter, prospective, uncontrolled, non-interventional cohort study of patients treated with riociguat. Patients were followed for at least 1 year and up to 4 years from enrollment or until 30 days after stopping riociguat treatment. Primary safety outcomes were adverse events (AEs) and serious adverse events (SAEs) coded using Medical Dictionary for Regulatory Activities preferred terms and System Organ Classes version 21.0, collected during routine clinic visits and collated via case report forms. Results: In total, 956 patients with CTEPH were included in the analysis. The most common AEs in these patients were peripheral edema/edema (11.7%), dizziness (7.5%), right ventricular (RV)/cardiac failure (7.7%), and pneumonia (5.0%). The most common SAEs were RV/cardiac failure (7.4%), pneumonia (4.1%), dyspnea (3.6%), and syncope (2.5%). Exposure-adjusted rates of hemoptysis/pulmonary hemorrhage and hypotension were low and comparable to those in the long-term extension study of riociguat (Chronic Thromboembolic Pulmonary Hypertension Soluble Guanylate Cyclase–Stimulator Trial [CHEST-2]). Conclusion: Data from EXPERT show that in patients with CTEPH, the safety of riociguat in routine practice was consistent with the known safety profile of the drug, and no new safety concerns were identified. © 2020 The Authors

Abstract

Objective: The soluble guanylate cyclase stimulator riociguat is approved for the treatment of adult patients with pulmonary arterial hypertension (PAH) and inoperable or persistent/recurrent chronic thromboembolic pulmonary hypertension (CTEPH) following Phase 3 randomized trials. The EXPosurE Registry RiociguaT in patients with pulmonary hypertension (EXPERT) study was designed to monitor the long-term safety of riociguat in clinical practice. Methods: EXPERT was an international, multicenter, prospective, uncontrolled, non-interventional cohort study of patients treated with riociguat. Patients were followed for at least 1 year and up to 4 years from enrollment or until 30 days after stopping riociguat treatment. Primary safety outcomes were adverse events (AEs) and serious adverse events (SAEs) coded using Medical Dictionary for Regulatory Activities preferred terms and System Organ Classes version 21.0, collected during routine clinic visits and collated via case report forms. Results: In total, 956 patients with CTEPH were included in the analysis. The most common AEs in these patients were peripheral edema/edema (11.7%), dizziness (7.5%), right ventricular (RV)/cardiac failure (7.7%), and pneumonia (5.0%). The most common SAEs were RV/cardiac failure (7.4%), pneumonia (4.1%), dyspnea (3.6%), and syncope (2.5%). Exposure-adjusted rates of hemoptysis/pulmonary hemorrhage and hypotension were low and comparable to those in the long-term extension study of riociguat (Chronic Thromboembolic Pulmonary Hypertension Soluble Guanylate Cyclase–Stimulator Trial [CHEST-2]). Conclusion: Data from EXPERT show that in patients with CTEPH, the safety of riociguat in routine practice was consistent with the known safety profile of the drug, and no new safety concerns were identified.

Abstract

The interpretation of pulmonary function tests (PFTs) to diagnose respiratory diseases is built on expert opinion that relies on the recognition of patterns and the clinical context for detection of specific diseases. In this study, we aimed to explore the accuracy and interrater variability of pulmonologists when interpreting PFTs compared with artificial intelligence (AI)-based software that was developed and validated in more than 1500 historical patient cases.120 pulmonologists from 16 European hospitals evaluated 50 cases with PFT and clinical information, resulting in 6000 independent interpretations. The AI software examined the same data. American Thoracic Society/European Respiratory Society guidelines were used as the gold standard for PFT pattern interpretation. The gold standard for diagnosis was derived from clinical history, PFT and all additional tests.The pattern recognition of PFTs by pulmonologists (senior 73%, junior 27%) matched the guidelines in 74.4±5.9% of the cases (range 56-88%). The interrater variability of κ=0.67 pointed to a common agreement. Pulmonologists made correct diagnoses in 44.6±8.7% of the cases (range 24-62%) with a large interrater variability (κ=0.35). Zusammenfassung: Das Kernsymptom des Ganser-Syndroms besteht im "Vorbeiantworten" auf einfache Fragen. Die Ursache dieses seltenen Syndroms ist unklar. Aktuelle Klassifikationssysteme zählen es zu den dissoziativen Störungen, wobei eine psychogene Ursache der Symptome angenommen wird. Anhand einer Literaturrecherche (n=151) wird jedoch gezeigt, dass das Ganser-Syndrom sehr häufig mit Hirnverletzungen assoziiert ist, wobei detaillierte bildgebende, neuropsychologische und neurologische Untersuchungen weitgehend fehlen. Wir stellen eine rechtshändige Patientin mit einem Ganser-Syndrom nach einem großen linkshemisphärischen Mediainfarkt vor. Die detaillierte neuropsychologische Untersuchung zeigte eine untypische Lateralisierung kognitiver Funktionen mit einer sog. gekreuzten Nichtaphasie und ausgeprägten frontal-exekutiven Funktionsstörungen. Abstract

The ability of cloud computing to provide almost unlimited storage, backup and recovery, and quick deployment contributes to its widespread attention and implementation. Cloud computing has also become an attractive choice for mobile users as well. Due to limited features of mobile devices such as power scarcity and inability to cater computationintensive tasks, selected computation needs to be outsourced to the resourceful cloud servers. However, there are many challenges which need to be addressed in computation offloading for mobile cloud computing such as communication cost, connectivity maintenance and incurred latency. This paper presents taxonomy of the computation offloading approaches which aim to address the challenges. The taxonomy provides guidelines to identify research scopes in computation offloading for mobile cloud computing. We also outline directions and anticipated trends for future research.

Abstract

Cutting force measurement in machining is a key issue for tool geometry optimization, tool condition monitoring, workpiece material machinability testing and other activities. In this work, the development of an innovative rotating dynamometer for cutting force measurement in milling is discussed. The device is capable to measure the forces acting on each cutting edge separately with a good dynamic response. Static and dynamic behavior of a simplified single-edge device was first investigated by means of static calibration procedure, pulse tests and preliminary cutting tests. Afterwards, pulse tests and cutting tests were performed on the complete measuring device clamped on a milling machine in order to evaluate the dynamic behavior of the system in operating conditions.

Abstract

Darier's disease is a rare, inherited autosomal dominant skin disorder caused by a mutation in the sarcoendoplasmatic reticulum calcium transporter (SERCA)-2-gene. In a number of pedigrees, Darier's disease closely relates with affective disorder. The most likely hypothesis for this is a susceptibility gene for affective disorder near the SERCA-2-gene. A 6.5-megabase region could be identified as a susceptibility locus. This region constitutes a susceptability locus also in affective disorder without Darier's disease. The underlying gene has not yet been identified., Der Morbus Darier ist eine seltene Hauterkrankung, die autosomal-dominant vererbt wird und durch eine Mutation im SERCA (sarco/endoplasmatic reticulum calcium transporter)-2-Gen verursacht wird. In einigen Stammbäumen ist das Auftreten des Morbus Darier eng mit dem Vorkommen affektiver Erkrankungen verknüpft. Abstract

This paper surveys and investigates the strengths and weaknesses of a number of recent approaches to advanced workflow modelling. Rather than inventing just another workflow language, we briefly describe recent workflow languages, and we analyse them with respect to their support for advanced workflow topics. Object Coordination Nets, Workflow Graphs, WorkFlow Nets, and an approach based on Workflow Evolution are described as dedicated workflow modelling approaches. In addition, the Unified Modelling Language as the de facto standard in object-oriented modelling is also investigated. These approaches are discussed with respect to coverage of workflow perspectives and support for flexibility and analysis issues in workflow management, which are today seen as two major areas for advanced workflow support. Given the different goals and backgrounds of the approaches mentioned, it is not surprising that each approach has its specific strengths and weaknesses. We clearly identify these strengths and weaknesses, and we conclude with ideas for combining their best features.

Abstract

A digital media recorder and a method of controlling such is shown. During attempts to copy media content such as audio or a video sequences, sub-sequences are extracted from an input media sequence. A digital fingerprint of the sub-sequence is calculated and compared with at least one first reference fingerprint from a database. The outcome of the comparison determines the action to take in the further processing. Recording of the input media sequence on the media carrier is either allowed or obstructed, e.g. disallowed. In the case recording of the sequence is allowed, the database is updated with information that the digital media sequence has been recorded. This has the effect of a "speed bump," which limits the rate at which those copies of media content can be made, i.e. the effect is that of prohibiting more than a given number of copies within a given time frame.

Abstract

A study of the effects of titania on selected properties of hafnia-rich mixed oxides in the system hafnia-zirconia-titania (HZT) was made in the region 5 to 20 mol percent titania. The studied properties included electrical conductivity, thermal expansion, and fracture strength and toughness. The effects of titania on the properties were studied for the reduced state as well as the oxidized state of the sintered mixed oxides. X-ray analysis showed that the materials were not always single phase. The oxidized compositions went from being monoclinic solid solutions at low titania additions to having three phases (two monoclinic and a titanate phase) at high additions of titania. The reduced compositions showed an increasing cubic phase presence mixed with the monoclinic phase as titania was added. The electrical conductivity increased with temperature at approximately 0.1 mhos/cm at 1700 C for all compositions. The thermal expansion coefficient decreased with increasing titania as did the monoclinic to tetragonal transformation temperature. The fracture strength of the oxidized bars tended to decrease with the addition of titania owing to the presence of the second phase titania. The fracture strength of the reduced bars exhibited a minimum corresponding to a two-phase region of monoclinic and cubic phases. When the second phases were suppressed, the titania tended to increase the fracture strength slightly in both the oxidized and reduced states. The fracture toughness followed similar trends.
