Zusammenfassung. In der klinischen Praxis gewinnen evidenzbasierte Behandlungsleitlinien zunehmend an Bedeutung. Innerhalb dieser finden sich jedoch nur unbefriedigende Informationen zur Diagnostik, speziell im Hinblick auf eine therapie-begleitende Diagnostik. Der Erfolg einer Therapie, ob Psycho- und/oder Pharmakotherapie, hängt jedoch von der Verzahnung von Therapie und Diagnostik ab, um z.B. den Therapieverlauf und -erfolg zu bewerten und bei Bedarf die therapeutische Strategie zu ändern. Der Ansatz des Evidence-Based Assessment (EBA) bietet hier ein hilfreiches Rahmenmodell und konkrete Vorschläge sowie Kriterien zur Realisierung dieser Verzahnung. Im Beitrag soll EBA vorgestellt sowie anhand ausgewählter Störungsgruppen illustriert werden.

The Liebowitz Social Anxiety Scale (LSAS) is the most frequently used instrument to assess social anxiety disorder (SAD) in clinical research and practice. Both a self-reported (LSAS-SR) and a clinician-administered (LSAS-CA) version are available. The aim of the present study was to define optimal cut-off (OC) scores for remission and response to treatment for the LSAS in a German sample. Data of N = 311 patients with SAD were used who had completed psychotherapeutic treatment within a multicentre randomized controlled trial. Diagnosis of SAD and reduction in symptom severity according to the Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders, 4th edition, served as gold standard. OCs yielding the best balance between sensitivity and specificity were determined using receiver operating characteristics. The variability of the resulting OCs was estimated by nonparametric bootstrapping. Using diagnosis of SAD (present vs. absent) as a criterion, results for remission indicated cut-off values of 35 for the LSAS-SR and 30 for the LSAS-CA, with acceptable sensitivity (LSAS-SR: .83, LSAS-CA: .88) and specificity (LSAS-SR: .82, LSAS-CA: .87). For detection of response to treatment, assessed by a 1-point reduction in the Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders, 4th edition, rating, a reduction of 28% for the LSAS-SR and 29% for the LSAS-CA yielded the best balance between sensitivity (LSAS-SR: .75, LSAS-CA: .83) and specificity (LSAS-SR: .76, LSAS-CA: .80). To our knowledge, we are the first to define cut points for the LSAS in a German sample. Overall, the cut points for remission and response corroborate previously reported cut points, now building on a broader data basis.

Background and objectives Premenstrual syndrome (PMS) is characterized by menstrual cycle-related affective, behavioral, and/or somatic symptoms. By applying the emotional Stroop task (EST) the current study examined if changes in processing emotional information, which have been demonstrated in affective disorders, are also present in PMS. Methods Via online screening, telephone interviews, and daily records over two months 55 women for the PMS group (on the basis of the specific inclusion criteria and a prospectively confirmed PMS) and 55 ‘non-PMS’ controls were recruited. All participants completed three emotional Stroop tasks (EST) with neutral and negative word, picture, and facial stimuli, during the follicular and luteal phase of the menstrual cycle. Results Mixed 2 × 2 univariate analyses of variance and post-hoc comparisons showed primarily a greater emotional Stroop effect with respect to picture and facial stimuli in the luteal menstrual cycle phase in women with PMS, compared to the control group. No significant group differences were observed for word stimuli. With respect to the facial stimuli, a kind of paradox effect was revealed (Stroop facilitation) in the PMS group. Limitations This study provides important information regarding cognitive processes in women suffering from PMS that have to be interpreted in the light of the following limitations: a limited representativeness of the sample, the determination of menstrual cycle phases based on symptom diaries but not hormone levels, and a limited interpretability of our results as causal relationships. Conclusions Our findings are in line with the assumption that alterations in cognitive-emotional processes are associated with PMS. Further research on the etiology of PMS should focus more on cognitive-emotional processing and its interaction with biological changes relating to the menstrual cycle.

Zusammenfassung. Theoretischer Hintergrund: Trotz des anhaltenden Forschungsinteresses an frühen Therapieerfolgen (Early Response), wurden frühe Verschlechterungen in bisherigen Untersuchungen eher vernachlässigt. Fragestellung: Diese Studie untersucht Merkmale und prädiktive Qualität einer frühen Verschlechterung für das Behandlungsergebnis in einer verhaltenstherapeutischen Hochschulambulanz. Methode: 1983 Patienten wurden anhand des Reliable Change Indexes (RCI) auf das Vorliegen einer frühen Verschlechterung der psychischen Belastung überprüft. Ergebnisse: Es fanden sich nur wenige charakteristische Merkmale einer frühen Verschlechterung (z. B. höhere Anzahl psychischer Störungen). Sich früh verschlechternde Patienten erreichten seltener Response (OR = 7,45) beziehungsweise Remission (OR = 11,43) und zeigten häufiger Verschlechterungen (OR = 40,68) und pathologische Abschlusswerte (OR = 5,02) zu Therapieende. Schlussfolgerung: Verschlechterungen in der frühen Therapiephase sind mit negativen Behandlungsresultaten assoziiert und sollten bei der Behandlungsplanung ambulanter Psychotherapien berücksichtigt werden.

Psychological factors, such as cognitive-emotional processes, are proposed to play an important role in the etiology of premenstrual syndrome (PMS). Our aim was to determine whether there are differences in emotion regulation between women with PMS and non-PMS controls. The study included 54 women who suffered from PMS (confirmed by prospective daily ratings during two menstrual cycles), as well as 52 non-PMS controls. All participants completed the Cognitive Emotion Regulation Questionnaire (CERQ) as an explicit and the affect misattribution procedure (AMP) as an implicit measure of emotion regulation. Each participant conducted the self-report as well as the experimental assessments twice, once during the follicular phase and once during the luteal phase of the menstrual cycle. In this article, the tsunami model TsunAWI (Alfred Wegener Institute) and its application for hindcasts, inundation studies, and the operation of the tsunami scenario repository for the Indonesian tsunami early warning system are presented. TsunAWI was developed in the framework of the German-Indonesian Tsunami Early Warning System (GITEWS) and simulates all stages of a tsunami from the origin and the propagation in the ocean to the arrival at the coast and the inundation on land. It solves the non-linear shallow water equations on an unstructured finite element grid that allows to change the resolution seamlessly between a coarse grid in the deep ocean and a fine representation of coastal structures. During the GITEWS project and the following maintenance phase, TsunAWI and a framework of pre- and postprocessing routines was developed step by step to provide fast computation of enhanced model physics and to deliver high quality results.

Zusammenfassung: Die Entwicklung von Klassifikationssystemen für psychische Störungen hat eine lange Tradition und wird von einer unverändert anhaltenden Diskussion zu grundlegenden Fragen begleitet. Dies betrifft v.a. die allgemeine Frage, wann überhaupt eine Störung vorliegt, sowie daran anschließend die Frage, welche und wie viele Störungen sinnvoll und notwendig sind. Im vorliegenden Beitrag wird ein Überblick über die mit den bisherigen Systemen Diagnostisches und Statistisches Handbuch Psychischer Störungen, 4.Aufl. (DSM-IV) und Internationale statistische Klassifikation der Krankheiten und verwandter Gesundheitsprobleme, 10.Ausgabe (ICD-10) verbundenen Paradigmen und Klassifikationsmethoden gegeben. Entgegen den ursprünglichen Erwartungen ist es nicht gelungen, auch nur eine der existierenden Diagnosen wissenschaftlich hinreichend zu validieren. Ebenso ist keine Alternative zum bisherigen deskriptiv-phänomenologischen Ansatz in Sicht. Die bevorstehenden Revisionen DSM-5 und ICD-11 werden keine konzeptuellen Neuerungen bringen, sondern lediglich einige neue Diagnosen einführen und die Kriterien einiger anderer Diagnosen modifizieren. Am Beispiel der Entwicklung von DSM-5 werden sowohl grundlegende Überlegungen als auch ausgewählte bevorstehende Änderungen kritisch diskutiert. Zudem werden Vorschläge für eine rationalere Entwicklung von Klassifikationssystemen vorgestellt

The German version of the Social Phobia and Anxiety Inventory (SPAI-G) is a validated measure for the detection of social anxiety disorder (SAD). The aim of the present study was to develop optimal cut points (OC) for remission and response to treatment for the SPAI-G.We used Receiver Operating Characteristic methods and bootstrapping to analyse the data of 359 patients after psychotherapeutic treatment. OCs where defined as the cut points with the highest sensitivity and specificity after bootstrapping.For remission, an OC of 2.79 was found, and for response, a change in score from pre- to posttreatment by 11% yielded best results.The OC we identified for remissionmay be used to improve the diagnostic utility of the SPAI-G. However, the cut point for response achieved only borderline-acceptable levels of sensitivity and specificity, calling into doubt their utility in clinical and research setting.Zusammenfassung Optimale Cut-Points für Remission und Response für die deutsche Version des Social Phobia and Anxiety Inventory (SPAI) Fragestellung: Die deutsche Version des Social Phobia and Anxiety Inventory (SPAI-G) ist ein validiertes Maß bei der Erkennung von Sozialer Phobie. Das Ziel der vorliegenden Studie war die Entwicklung optimaler Cut-Points (OC) für Remission und Response to treatment für das SPAI-G. Methode: Wir verwendeten Receiver Operating Characterstic Methoden und Bootstrapping, um die Daten von 359 Patienten nach Beendigung einer psychotherapeutischen Behandlung zu untersuchen. Ergebnisse: Als optimaler Cut-Point wurde der Cut-Point mit der höchsten Sensitivität und Spezifität definiert. Für Remission erreichte ein optimaler Cut-Point von 2.79 und für Response eine Prä-Post-Veränderung von 11% die besten Ergebnisse. Diskussion: Der von uns identifizierte optimale Cut-Point für Remission kann zur Verbesserung des diagnostischen Nutzens der SPAI-G genutzt werden. Der Cut-Point für Response erreichte allerdings nur grenzwertige Sensitivität und Spezifität, weshalb sein Nutzen für klinische und wissenschaftliche Zwecke zweifelhaft ist.

Several randomized controlled trials have identified early response to psychotherapy as a predictor for later treatment outcome among patients with depressive disorders. However, supporting evidence under routine conditions is rare. This study evaluated the predictive value of early improvement for final outcomes in psychotherapy among depressive patients in the naturalistic setting of a German university outpatient clinic.We used the method of percent symptom reduction to classify 639 patients with major depression or dysthymic disorder who underwent an average of 40.0 sessions (SD = 16.3) of naturalistic cognitive-behavioral therapy (CBT) as having either an early response or an early nonresponse.Early response was a good predictor for final response and remission regarding depressive symptoms (OR = 8.75 and OR = 5.32, respectively), as well as overall psychological distress (OR = 3.95 and OR = 3.16, respectively). Early nonresponse was distinctly associated with later deterioration of both depressive (OR = 9.56) and general psychological symptomatology (OR = 4.92).Early response to psychotherapy has high predictive qualities for positive later treatment outcome in depressive patients under routine CBT. Therefore, early treatment effects should be considered in clinical decision-making and treatment planning in everyday clinical practice.

Zusammenfassung. Mentale Vorstellungsbilder spielen bei zahlreichen kognitiven, motorischen und emotionalen Prozessen eine bedeutende Rolle. Die Spontaneous Use of Imagery Scale (SUIS; Kosslyn, Chabris, Shephard & Thompson, 1998 ) ist ein Selbstbeurteilungsinstrument zur Erfassung von interindividuellen Unterschieden hinsichtlich des alltäglichen Gebrauchs mentaler Vorstellungsbilder. In zwei Studien wurde eine deutsche Adaption der SUIS entwickelt und psychometrisch erprobt. Mittels konfirmatorischer Faktorenanalyse wurde in beiden Studien die Eindimensionalität der Skala nachgewiesen. Erste Validitätshinweise ergeben sich aus Beziehungen der SUIS zu Maßen von bildhaften (visuell-imaginativen) sowie verbal-gedanklichen kognitiven Stilen (Studie 1, N = 216). Mit der deutschen Version konnten auch positive Zusammenhänge mit Hypomanie repliziert werden. In der zweiten Studie (N = 447) konnte die interne Konsistenz der deutschen Version durch eine Erweiterung der Skala deutlich verbessert werden. Mit der erweiterten Version der SUIS liegt nun erstmalig für den deutschsprachigen Raum ein Instrument vor, das die Imaginationsneigung reliabel erfassen kann.

Zusammenfassung. Theoretischer Hintergrund: Die Exposition gilt als hoch wirksames Therapieelement in der Behandlung der Panikstörung mit Agoraphobie. Es ist jedoch weitgehend unklar, wie häufig und mit welchen Effekten verschiedene Modalitäten der Exposition (therapeutenbegleitet vs. patientengeleitet) in der Routineversorgung durchgeführt werden. Fragestellung: Es sollen Häufigkeit und Wirksamkeit der verschiedenen Durchführungsmodalitäten von Exposition in der Routinebehandlung der Panikstörung mit Agoraphobie untersucht werden. Methode: Bei N = 93 konsekutiven Patienten einer Hochschulambulanz mit der Hauptdiagnose Panikstörung mit Agoraphobie wurden Prä-Post-Veränderungen der Angstsymptomatik in Abhängigkeit von verschiedenen Expositionsmodalitäten erfasst. Die Auswirkungen von therapeutenbegleiteter und patientengeleiteter Exposition wurden getrennt für In-vivo-Exposition und interozeptive Exposition (IE) untersucht. Ergebnisse: Mit einer Kombination aus therapeutenbegleiteter und patientengeleiteter Exposition ließen sich tendenziell die besseren Therapieergebnisse erzielen, dieser Effekt ließ sich sowohl für die In-vivo-Exposition als auch für die IE nachweisen. Schlussfolgerung: Nach Möglichkeit sollten In-vivo-Exposition sowie IE sowohl therapeutenbegleitet als auch zusätzlich patientengeleitet (als selbständige Hausaufgabe) durchgeführt werden.

Zusammenfassung. Ein besseres Verständnis der Rolle von Prozessen und Stilen der Emotionsregulation (ER) im Kontext psychischer Störungen erscheint essentiell, um psychische Störungsmodelle und Behandlungskonzepte zu optimieren. Diese Studie überprüfte den Cognitive Emotion Regulation Questionnaire (CERQ) in einer klinischen Stichprobe von ambulanten Psychotherapiepatienten (N = 156) hinsichtlich seiner teststatistischen Güte sowie im Hinblick auf Zusammenhänge mit Psychopathologie. Der CERQ wies eine gute Reliabilität (.70 ≤ α ≤ .84) sowie faktorielle Validität auf. Im Vergleich zu einer Bevölkerungsstichprobe berichtete die klinische Stichprobe höhere Ausprägungen in dysfunktionalen und niedrigere Ausprägungen in funktionalen ER-Strategien. Mittels eines Strukturgleichungsmodells zeigte sich, dass unter Berücksichtigung der Skaleninterkorrelationen drei kognitive ER-Strategien einen signifikanten und inkrementellen Beitrag zur Vorhersage der Gruppenzugehörigkeit zur klinischen Gruppe leisten (Rumination, Planung, Andere beschuldigen). Die klinischen Subgruppen (depressive, Angst- und somatoforme Störungen) unterschieden sich nicht signifikant hinsichtlich des Einsatzes einzelner ER-Strategien. Der Einsatz des CERQ kann auch in klinischen Stichproben empfohlen werden, um transdiagnostisch relevante Prozesse einer veränderten Emotionsregulation zu untersuchen.

OBJECTIVES The present study investigates the role of dysfunctional cognitions in patients with chronic tinnitus. To explore different dimensions of tinnitus-related thoughts, a 22-item self-report measure, the "Tinnitus Cognitions Scale" (T-Cog), is presented. Furthermore, dysfunctional cognitions are examined as a possible mediator of the relation between tinnitus distress and depression. DESIGN The present study analyzes the cross-sectional data of 373 patients with chronic tinnitus. Parallel analysis and principal axis factoring are used to identify the factor structure of the T-Cog. Assumed mediating effects are tested using the asymptotic and resampling procedure. RESULTS Factor analysis reveals two factors interpreted as "tinnitus-related catastrophic thinking" and "tinnitus-related avoidance cognitions." Internal consistency is sufficient with a Cronbach's α of 0.88 for the total scale and 0.74 and 0.87 for the subscales. The authors find high associations between the T-Cog and other measures of tinnitus distress, depression, anxiety, and tinnitus acceptance, indicating convergent validity. With the exception of neuroticism, low correlations with personality factors are found, indicating discriminant validity. Patients with moderate or severe tinnitus distress report significantly higher scores of dysfunctional cognitions than patients with mild tinnitus distress. Tinnitus-related catastrophic thinking and tinnitus-related avoidance cognitions partially mediate the relation between tinnitus distress and depression. CONCLUSIONS Dysfunctional cognitions can play an important role in the degree of tinnitus distress. Catastrophic and avoidant thoughts contribute to the explanation of depression among tinnitus patients. The T-Cog is a reliable and valid questionnaire for the assessment of different dimensions of cognitions. Its use could provide information for identifying tinnitus patients who are particularly suitable for cognitive-behavioral therapy.

Zusammenfassung. Theoretischer Hintergrund: Verschiedene randomisiert-kontrollierte Studien konnten die Bedeutung frühzeitiger Therapieerfolge für das Therapieergebnis nachweisen. Entsprechende Erkenntnisse unter Routinebedingungen sind bislang selten. Fragestellung: Diese Studie untersucht Häufigkeit, prädiktiven Wert sowie Patientenmerkmale eines frühen Ansprechens auf die Therapie („Early Response“) in einer verhaltenstherapeutischen Hochschulambulanz. Methode: Wir klassifizierten anhand prozentualer Wertereduktion 1109 Patienten verschiedener Störungsgruppen hinsichtlich ihrer psychischen Belastung als Early Responder beziehungsweise Early Nonresponder. Ergebnisse: 48,2 % der Patienten zeigten frühe Therapieerfolge. Early Response erwies sich als Prädiktor für Response (OR = 6,36) und Remission (OR = 3,90). Early Nonresponse sagte eine Verschlechterung der Symptombelastung zu Therapieende vorher (OR = 12,14). Schlussfolgerungen: Frühzeitige Therapieeffekte sind im Routinesetting offenbar von hoher Bedeutung für das Therapieergebnis und sollten bei der Behandlungsplanung stärker berücksichtigt werden.

Background To investigate the short-term cost-effectiveness of cognitive-behavioral therapy (CBT) and psychodynamic therapy (PDT) compared to waiting list (WL). Methods The analysis was conducted alongside the SOPHO-NET multi-center efficacy trial. Patients were randomly assigned to CBT ( n =209), PDT ( n =207), or WL ( n =79). Resource use was assessed prior and during treatment to determine direct and absenteeism costs. Unadjusted incremental cost-effectiveness ratios (ICERs) were calculated based on remission and response rates. To visualize statistical uncertainty, cost-effectiveness acceptability curves (CEACs) were constructed based on adjusted net-benefit regression. Different values for the society׳s willingness to pay (WTP) were assumed. Results Both interventions were more efficacious than WL but were associated with increased direct costs besides intervention costs. Unadjusted ICERs per responder were €3615 for CBT and €4958 for PDT. Unadjusted ICERs per remitted patient were €5788 and €10,733. CEACs revealed a high degree of uncertainty: applying the 97.5% probability threshold, CBT proved cost-effective at a WTP ≥€16,100 per responder and ≥€26,605 per remitted patient. Regarding PDT cost-effectiveness only was certain for response at a WTP ≥€27,290. Limitations The WL condition is assumed to represent untreated patients, although the expectation to start treatment in the near future probably affects symptom severity and health care utilization. Conclusions At the end of treatment cost-effectiveness of CBT and PDT compared to WL is uncertain and depends on the societal WTP. The interventions may induce a more adequate utilization of other health care services – involving increased costs. Development of costs and effects in the long-run should be considered.

Mental imagery can critically influence our emotional state. In contrast to commonly used explicit measures, implicit measures are promising for objectively assessing automatic emotional processes beyond deliberate control. In two studies with non-clinical samples, we tested the Affect Misattribution Procedure (AMP) to measure implicit affect induced by mental imagery. In a first study (N = 145), the implicit measure showed that mental imagery elicits significantly stronger negative affect than verbally processed stimuli (F(1, 144) = 3.94, p≤.05, η2p = 03). In Study 2 (N = 71), we refined the implicit measure and found that mental images can induce implicit affective reactions at least as strong as pictures. hs-CRP is elevated in depression (1), but evidence on decreases of hs-CRP during depression treatment or the role of hs-CRP in the prediction of response to depression treatment is still controversial (2,3). To date, no study has examined this association in patients with diabetes. As elevated hs-CRP increases the risk of diabetes complications in diabetes (4), we aimed to explore hs-CRP in patients with diabetes and major depression undergoing depression treatment based on the data of the Diabetes and Depression (DAD) study (5). Participants were randomized to 12 weeks (short-term phase) of diabetes-specific group cognitive behavioral therapy or sertraline treatment and followed up for 15 months (long-term phase). hs-CRP was assessed at baseline and at the end of the long-term phase with a latex-enhanced immunoturbidimetric method. Ethics approval and written informed consent were obtained (5). Information on baseline hs-CRP was available in 219 patients (mean age 48.1 ± 12.0 years, 62% female, mean HbA1c 9.25 ± 1.4% [78 ± 16.2 mmol/mol], 51.6% type 2 diabetes, median hs-CRP 0.33 mg/dL [interquartile range 0.10, 0.84]). Objectives Within a randomized controlled trial contrasting the outcome of manualized cognitive-behavioral (CBT) and short term psychodynamic therapy (PDT) compared to a waiting list condition (the SOPHO-Net trial), we set out to test whether self-reported attachment characteristics change during the treatments and if these changes differ between treatments. Research design and methods 495 patients from the SOPHO-Net trial (54.5% female, mean age 35.2 years) who were randomized to either CBT, PDT or waiting list (WL) completed the partner-related revised Experiences in Close Relationships Questionnaire (ECR-R) before and after treatment and at 6 and 12 months follow-up. The Liebowitz Social Anxiety Scale (LSAS) was administered at pre-treatment, post-treatment, and at 6-month and 1-year follow-up. ECR-R scores were first compared to a representative healthy sample (n = 2508) in order to demonstrate that the clinical sample differed significantly from the non-clinical sample with respect to attachment anxiety and avoidance. Results LSAS scores correlated significantly with both ECR-R subscales. Post-therapy, patients treated with CBT revealed significant changes in attachment anxiety and avoidance whereas patients treated with PDT showed no significant changes. Changes between post-treatment and the two follow-ups were significant in both conditions, with minimal (insignificant) differences between treatments at the 12- month follow-up. Conclusions The current study supports recent reviews of mostly naturalistic studies indicating changes in attachment as a result of psychotherapy. Although there were differences between conditions at the end of treatment, these largely disappeared during the follow-up period which is line with the other results of the SOPHO-NET trial. Wie bereits in vorherigen Kapiteln erwahnt, weisen Patienten mit medizinisch unerklarten Korperbeschwerden z. T. eine eher skeptische Haltung gegenuber Psychotherapie auf. In diesem ersten Modul des einzeltherapeutischen Behandlungskonzepts stehen deshalb daher verschiedene Strategien im Vordergrund, die dem Patienten das Therapiekonzept transparent machen sollen. Dazu gehort, dass mit dem Patienten zunachst die Therapieinhalte kurz umrissen werden. Damit der Patient sich validiert fuhlt, ist es zudem sinnvoll, seine Vorstellungen zu den Ursachen seiner Beschwerden zu explorieren. Die Klarung und Formulierung von Therapiezielen hilft dem Patienten, seine Therapieerwartungen realistisch zu reflektieren. Abschliesend sollten dem Patienten der Sinn und die Wichtigkeit eines Grundprinzips der kognitiven Verhaltenstherapie – die vom Patienten selbststandig durchgefuhrten therapeutischen Ubungen zwischen den Sitzungen – erklart werden.

In diesem Kapitel werden die ersten Ergebnisse zur Wirksamkeit des in Kap. 5 bis 12 dargestellten kognitiv-verhaltenstherapeutischen Behandlungskonzepts vorgestellt. Die Befunde basieren auf einer Stichprobe von Patienten mit multiplen unerklarten Korperbeschwerden, die in der Poliklinischen Institutsambulanz fur Psychotherapie der Johannes Gutenberg-Universitat Mainz entsprechend dem beschriebenen manualisierten Vorgehen behandelt wurden. Neben den klassischen Ergebnismasen zur Intensitat der Korperbeschwerden werden Veranderungen auch in anderen Patientenmerkmalen berichtet – wie z. B. hypochondrischen Einstellungen, korperbezogenen dysfunktionalen Kognitionen, Krankheitsverhalten, allgemeinen Psychopathologiemasen (z. B. Depressivitat, Angstsymptome) und der Beeintrachtigung des Patienten im alltaglichen Funktionsniveau. Abschliesend wird die Zufriedenheit der Patienten mit dem einzeltherapeutischen Behandlungskonzept beschrieben.

Das Thema Stress und Stressbewaltigung wurde bereits im sechsten Kapitel behandelt und wird nun im Therapieverlauf wieder aufgegriffen. Im zweiten Teil der Stressbewaltigung steht die Rolle von Gedanken im Zusammenhang mit Stresserleben im Vordergrund. Dementsprechend wird der Patient uber Strategien des geleiteten Entdeckens in der ersten Sitzung dieses Moduls in die Rolle von der Bewertung des Stressors und eigener Kapazitat zur Stressbewaltigung eingefuhrt. Verschiedene Formen des Umgangs mit Stress werden abgeleitet und individuelle Stressoren in der zweiten Sitzung exploriert. Die dritte Sitzung des sechsten Moduls widmet sich ubergeordneten kognitiven Schemata, die Stresserleben verstarken konnen – sog. Stressverscharfer. In der abschliesenden Sitzung des Moduls sollen das Repertoire des Patienten zur regenerativen Stressbewaltigung und die Fahigkeiten zur Erhohung der Stressresistenz erweitert werden, indem achtsamkeitsbasierte Entspannungsstrategien eingefuhrt werden.

Over recent years, the importance of knowledge transfer and dialogue processes in the field of climate sciences has received increased attention. As the fundamental basis for discussion in areas ranging from climate change to the sustainable use of resources, the task of transferring climate science knowledge into decision making processes and to society whilst acknowledging their respective priorities is both a vital and urgent challenge.

Aufmerksamkeitsprozesse spielen in der Entstehung und Aufrechterhaltung somatoformer Beschwerden eine zentrale Rolle. Mithilfe von Verhaltensexperimenten und Metaphern werden dem Patienten im Rahmen der ersten beiden Sitzungen dieses Moduls die Funktion unseres Aufmerksamkeitssystems und die Wirkweise von Aufmerksamkeitslenkung und deren Einfluss auf korperliche Wahrnehmungen verdeutlicht. Geleitetes Entdecken spielt in diesen Sitzungen eine besonders wichtige Rolle. Das Modul wird mit einer dritten Sitzung zur Erarbeitung von individuellen Ablenkungsstrategien abgerundet. In diesem Kontext bietet es sich an, besonders ressourcenorientiert zu arbeiten und das Konzept von »Genuss und Genussstrategien« aufzugreifen. Genussstrategien konnen »zweckentfremdet« zugleich als Ablenkungsstrategien genutzt werden. Zugleich bietet es sich an, korperliche Aktivitat als Ablenkungsoption einzufuhren und damit bereits eine Grundlage fur den spater erfolgenden Abbau von Schonungs- und Vermeidungsverhalten zu legen.

Mithilfe eines Fallbeispiels soll dem Therapeuten verdeutlicht werden, wie die Anwendung des in diesem Manual beschriebenen Einzeltherapiekonzepts in der Praxis aussehen kann. Es werden zunachst Informationen uber die Symptomatik, Krankheitsanamnese und lebensgeschichtliche Entwicklung eines Patienten mit multiplen, chronischen unerklarten Korperbeschwerden vermittelt. Danach wird exemplarisch eine Verhaltensanalyse zum vorliegenden Patientenbeispiel beschrieben. Abschliesend werden die Therapieziele abgeleitet und der Therapieverlauf bzw. das Therapieergebnis geschildert. Elemente dieser Falldarstellung konnen auch als Orientierung fur die Erstellung eines Berichts zum Therapieantrag bei einer deutschen Krankenkasse dienen.

Schwierigkeiten in der Kommunikation in verschiedenen Lebensbereichen stellen bei einer Subgruppe von Patienten mit somatoformen Beschwerden ein wichtiges Problem dar. Sie konnen u. a. in der Beziehung zu Angehorigen, zu Freunden, zum (Ehe-)Partner oder zum Arzt auftreten. Diese Kommunikationsprobleme konnen mit der Entwicklung negativer Emotionen wie z. B. Frustration, Arger oder Enttauschung beim Patienten sowie seinem Umfeld einhergehen, die wiederum zur Aufrechterhaltung der korperlichen und begleitenden Beschwerden beitragen konnen. Es ist wissenschaftlich bewiesen, dass storungsubergreifend 40 % der Varianz im Therapieerfolg durch extratherapeutische Faktoren, einschlieslich der sozialen Unterstutzung und des sozialen Netzwerks, eines Patienten erklart werden. Adaquate soziale Unterstutzung ist zudem assoziiert mit der Symptomschwere und spielt eine Mediatorrolle zwischen kritischen Lebensereignissen und der Entwicklung von psychischen Symptomen. Deshalb kann angenommen werden, dass soziale Unterstutzung eine sehr wichtige Ressource und einen entscheidenden Schutzfaktor fur die Patienten darstellt.

Zunachst mag es ungewohnlich erscheinen, dass ein kognitiv-verhaltenstherapeutisches Behandlungskonzept mit einem Storungsmodell abgeschlossen und nicht eingefuhrt wird. Insbesondere fur Patienten, die zu Beginn der Behandlung stark ausgepragte somatomedizinische Annahmen zu ihren Korperbeschwerden aufweisen, kann es uberfordernd sein, wenn Sie mit einem biopsychosozialen Behandlungsmodell konfrontiert werden. Die Module des vorliegenden Behandlungsansatzes sind so aufgebaut, dass sie immer mit psychoedukativen Abschnitten eingeleitet werden, sodass dem Patienten das Storungsmodell uber den Therapieverlauf Schritt fur Schritt vermittelt wird. Das zusammenfassende Storungsmodell in der letzten Sitzung hilft dem Patienten, seine Erfahrungen aus der Therapie zu reflektieren und in einen Gesamtzusammenhang zu bringen. Das Storungsmodell als eine Art »Therapiebilanz« bietet dann einen guten Einstieg in die Erarbeitung von Strategien fur den Umgang mit zukunftigten »Ruckfall«-Situationen.

Aufgrund der haufig eher biomedizinisch gepragten Vorstellungen des Patienten zu den Ursachen seiner Korperbeschwerden bietet sich nach dem Aufbau ausreichender Therapiemotivation der Einstieg mit dem Thema »Stressbewaltigung« an. Viele Patienten erleben den Begriff »Stress« wenig bedrohlich und stigmatisierend. Aus der Alltagssprache sind viele Patienten mit dem Thema »Stress« vertraut. Eine einfuhrende Psychoedukation uber »Stress« erlaubt es, in die Behandlung auf einer biologisch-medizinischen Ebene einzusteigen, die in der Regel naher an den Erklarungen des Patienten fur seine Beschwerden steht. Aufgrund der nach aktuellem wissenschaftlichem Stand zentralen Bedeutsamkeit von Stresserleben in der Atiopathogenese unerklarter Korperbeschwerden sieht das vorliegende Manual vor, dass dieses Thema wiederholt aufgegriffen wird. Im ersten – eher psychoedukativen – Teil, der zwei Sitzungen umfasst, liegt der Schwerpunkt auf der Klarung der Funktionen von Stress und der Aufklarung des Patienten uber die menschliche Stressreaktion. Der zweite – eher kognitive – Abschnitt zu Stress und Stressbewaltigung folgt in 7 Kap. 10.

Das vorliegende Therapiemanual ist modulartig gegliedert. Da die Wirksamkeit des Therapiekonzepts unter Einhaltung der Modulabfolge bzw. des strukturierten Vorgehens nachgewiesen werden konnte (► Kap. 6), empfehlen wir Ihnen, sich moglichst an der vorgeschlagenen Reihenfolge der Therapiebausteine zu orientieren. Das Manual umfasst insgesamt sieben Module (◘ Tab. 4.1), die sich auf 20 Einzeltherapiesitzungen aufgliedern.

Cognitive behavioral therapy (CBT) has been shown to be highly effective in the treatment of health anxiety. However, little is known about the effectiveness of group CBT in the treatment of health anxiety. The current study is the largest study that has investigated the effectiveness of combined individual and group CBT for patients with the diagnosis of hypochondriasis (N=80). Therapy outcomes were evaluated by several questionnaires. Patients showed a large improvement on these primary outcome measures both post-treatment (Cohen's d=0.82-1.08) and at a 12-month follow-up (Cohen's d=1.09-1.41). Measures of general psychopathology and somatic symptoms showed significant improvements, with small to medium effect sizes. Patients with more elevated hypochondriacal characteristics at therapy intake showed a larger therapy improvement, accounting for 7-8% of the variance in therapy outcome. CBT group therapy has therefore been shown to be an appropriate and cost-effective treatment for health anxiety.

OBJECTIVE This study compared the long-term efficacy of a diabetes-specific cognitive behavioral group therapy (CBT) with sertraline in patients with diabetes and depression who initially responded to short-term depression treatment. RESEARCH DESIGN AND METHODS A randomized controlled single-blind trial was conducted in 70 secondary care centers across Germany comparing 12 weeks of CBT with sertraline in 251 patients with type 1 or 2 diabetes (mean HbA1c 9.3%, 78 mmol/mol) and major depression (Structured Clinical Interview for DSM-IV [SCID]). After 12 weeks, treatment responders (≥50% reduction Hamilton Depression Rating Scale [HAMD-17]) were included in the 1-year study phase where CBT patients were encouraged to use bibliotherapy and sertraline patients received continuous treatment. We analyzed differences for HbA1c (primary outcome) and reduction (HAMD-17) or remission (SCID) of depression from baseline to the 1-year follow-up using ANCOVA or logistic regression analysis. RESULTS After 12 weeks, 45.8% of patients responded to antidepressant treatment and were included in the 1-year study phase. Adjusted HbA1c mean score changes from baseline to the end of the long-term phase (−0.27, 95% CI −0.62 to 0.08) revealed no significant difference between interventions. Depression improved in both groups, with a significant advantage for sertraline (HAMD-17 change: −2.59, 95% CI 1.15–4.04, P < 0.05). CONCLUSIONS Depression improved under CBT and sertraline in patients with diabetes and depression, with a significant advantage for sertraline, but glycemic control remained unchanged. CBT and sertraline as single treatment are insufficient to treat secondary care diabetes patients with depression and poor glycemic control.

Recent research indicates that a notable number of teachers are concerned with conditions of auditory impairment such as tinnitus, hyperacusis, and hearing loss. Studies focussing on characteristics and interdependencies of single hearing disorders (HD) are rare. This explorative study examines tinnitus, hyperacusis, hearing loss, and all possible combinations (tinnitus + hyperacusis; tinnitus + hearing loss; hyperacusis + hearing loss; tinnitus, hyperacusis + hearing loss) in German teachers. The impact of single HD on perceived distress, depending on the number and kind of comorbid HD, was of special interest. Information was collected via online survey and includes self-reported data as well as data from the Mini-Tinnitus Questionnaire (Mini-TQ). Results show that most of the 1468 participants (45%) suffered from two HD in different combinations, and the fewest (25%) were afflicted with only one HD. Considering the seven HD groups, most teachers (30%) suffered from all three HD. Across all groups, tinnitus was present in 1096, hyperacusis in 988, and hearing loss in 937 teachers. Multiple intergroup comparisons revealed that self-rated tinnitus-related distress rose significantly with the increasing number of HD. No significant differences were found for distress ratings of hyperacusis between the four groups including hyperacusis and between the four groups with hearing loss. In the Mini-TQ, groups including hyperacusis scored considerably higher than those excluding hyperacusis. The frequent prevalence of HD in German teachers points to a need of better noise prevention in German schools as one priority of occupational safety.

This study investigated whether partner-related attachment characteristics differentially predict premature treatment termination as well as posttreatment and 1-year follow-up outcome in patients with social anxiety disorder treated with a manualized cognitive-behavioral therapy (CBT) or short-term psychodynamic therapy (PDT) in the SOPHO-NET (Social Phobia Psychotherapy Network) trial. Participants were 412 patients with social anxiety disorder (57% female) with a mean age of 35.4 years (SD = 12.1) who were randomized to either CBT or PDT. Partner-related attachment characteristics were measured using the revised Experiences in Close Relationships Questionnaire (ECR-R) at pretreatment. The Liebowitz Social Anxiety Scale was administered at pretreatment, posttreatment, and a 1-year follow-up. To address our research questions, linear regression models were applied. Furthermore, we compared CBT versus PDT patients within ECR-R quartiles. Treatment dropout did not differ between CBT and PDT and was not predicted by pretreatment attachment. In both treatment conditions, there was a trend for higher attachment anxiety to be associated with a more limited reduction in symptoms if controlling for pretreatment Liebowitz Social Anxiety Scale scores. Exploratory analyses showed that patients assigned to the highest quartile of the ECR-R-Avoidance distribution showed more benefit within the CBT condition posttreatment and at follow-up than the PDT condition. Our findings suggest that it may be useful to assess attachment characteristics in patients with social anxiety disorder before psychotherapeutic treatment. Patients characterized by very high pretreatment attachment avoidance (ECR-R-Avoidance >3.87) may specifically benefit more from CBT than from PDT. We examined the role of baseline patient characteristics as predictors of outcome (end-state functioning, response and remission) and attrition for cognitive therapy (CT) in social anxiety disorder (SAD). Beyond socio-demographic and clinical variables such as symptom severity and comorbidity status, previously neglected patient characteristics (e.g., personality, self-esteem, shame, interpersonal problems and attachment style) were analysed. Method Data came from the CT arm of a multicentre RCT with n = 244 patients having DSM-IV SAD. CT was conducted according to the manual by Clark and Wells. Severity of SAD was assessed at baseline and end of treatment with the Liebowitz Social Anxiety Scale (LSAS). Multiple linear regression analyses and logistic regression analyses were applied. Results Up to 37% of the post-treatment variance (LSAS) could be explained by all pre-treatment variables combined. Symptom severity (baseline LSAS) was consistently negatively associated with end-state functioning and remission, but not with response. Number of comorbid diagnoses was negatively associated with end-state functioning and response, but not with remission. Self-esteem was positively associated with higher end-state functioning and more shame with better response. Attrition could not be significantly predicted. Conclusions The results indicate that the initial probability for treatment success mainly depends on severity of disorder and comorbid conditions while other psychological variables are of minor importance, at least on a nomothetic level. This stands in contrast with efforts to arrive at an empirical-based foundation for differential indication and argues to search for more potent moderators of therapeutic change rather on the process level. Key Practitioner Message Personality, self-esteem, shame, attachment style and interpersonal problems do not or only marginally moderate the effects of interventions in CT of social phobia. Symptom severity and comorbid diagnoses might affect treatment outcome negatively. Die neue 5. Auflage des Diagnostic and Statistical Manual of Mental Disorders (DSM-5) fuhrt eine Diagnose mit der Bezeichnung „somatic symptom disorder“ (somatische Belastungsstorung) ein, bei der nicht mehr zwischen medizinisch unerklarten und erklarten korperlichen Symptomen unterschieden wird. Damit werden die bisherigen somatoformen Storungen als eigenstandige Storungsgattung infrage gestellt. Entgegen der ursprunglichen Intention wurden terminologische, konzeptuelle und praktische Probleme der bisherigen Klassifikation nicht gelost. Die Validitat der neuen Diagnose ist aufgrund ihres uberinklusiven Charakters fraglich. Weitere Kritikpunkte sind die suboptimale Auswahl psychologischer Storungsmerkmale, die Aufgabe eines einheitlichen Hypochondriekonzepts und handwerkliche Mangel bei den diagnostischen Kriterien in Form fehlender Ausschlusskriterien zur Differenzialdiagnose.

Cognitive reappraisal and acceptance are two presumably adaptive emotion regulation strategies in depression. More recently, self-compassion has been discussed as another potentially effective strategy for coping with depression. In the present study, we compared the effectiveness of self-compassion with a waiting condition, reappraisal, and acceptance in a clinically depressed sample, and tested the hypothesis that the intensity of depressed mood would moderate the differential efficacy of these strategies. In an experimental design, we induced depressed mood at four points in time in 48 participants meeting criteria for major depressive disorder. After each mood induction, participants were instructed to wait, reappraise the situation, accept their negative emotions, or employ self-compassion to regulate their depressed mood. Self-ratings of depressed mood were assessed before and after each mood induction and regulation phase. Results showed that the reduction of depressed mood was significantly greater in the self-compassion condition than in the waiting condition. No significant differences were observed between the self-compassion and the reappraisal condition, and between the self-compassion and the acceptance condition in patients' mood ratings. However, the intensity of self-rated depressed mood at baseline was found to moderate the comparative effectiveness of self-compassion and reappraisal with a trend of self-compassion being more effective than reappraisal in high depressed mood at baseline. These findings support the use of self-compassion as another adaptive emotion regulation strategy for patients with major depressive disorder, especially for those suffering from high levels of depressed mood.

Affective reactions to health-related information play a central role in health anxiety. Therefore, using ambulatory assessment, we analysed the time course of negative affect in a control group (CG, n = 60) which only rated their negative affect and an experimental group (EG, n = 97) which also rated the presence of somatic symptoms (e.g., back pain). By means of mixed regression models, we observed a decline of negative affect following the symptom self-ratings in the EG and a stable affect in the CG. The decline of negative affect was not moderated by the degree of health anxiety. Our findings might indicate that evaluating one's health status leads to a general reduction of negative affect in healthy individuals. The results of the study are in line with a bidirectional symptom perception model and underline the crucial role of affect regulation in the processing of health-related information.
