Abstract

Interleukin-23 inhibition is effective in treating ulcerative colitis. Guselkumab is a dual-acting, human IgG1, interleukin-23p19 subunit inhibitor that potently neutralises interleukin-23 and can bind to CD64. We aimed to evaluate the efficacy and safety of guselkumab as induction and maintenance therapy in patients with ulcerative colitis.

7. Geographical heterogeneity in the disease characteristics and management of patients with inflammatory bowel disease, the preliminary results of a Chinese database for IBD (CHASE-IBD).

Abstract

Background: The incidence of inflammatory bowel disease (IBD) is rapidly increasing in China, a vast country with significant geographical differences. The socioeconomic status of Eastern China is significantly higher than that of Western China. Objectives: This study aimed to describe the geographical heterogeneity in the characteristics and management of patients with IBD in both Eastern and Western China. Design: This was a multicenter, cross-sectional study. Methods: Patients with IBD with ages ⩾18 years up to 18 January 2023 were included in the analysis from the Chinese database for IBD. Logistic regression was used to identify risk factors associated with surgeries among patients with IBD. Results: Among 8305 patients with IBD, the ratio of ulcerative colitis (UC) to Crohn's disease (CD) was 4.13 and 0.33 in Western and Eastern China, respectively. The median age at diagnosis of UC and CD was 40.69 and 28.58 years, respectively. There was a male predominance among patients with UC (54.3%) and CD (68.0%). The two regions exhibited a similar distribution of disease locations in UC. However, Western China had a higher proportion of L2 involvement (30.0% versus 19.1%) and more advanced disease behavior (B2 and B3) (48.8% versus 39.8%) than Eastern China. Patients with IBD in Western China received more 5-aminosalicylic acid and corticosteroids and fewer immunomodulators and biologicals. In terms of surgical risk, Eastern China [ versus Western China, odds ratios (OR): 5.36, 95% confidence intervals (CI): 2.96–9.68] was associated with a higher risk of surgery in UC, while Western China (versus Eastern China, OR: 3.39, 95% CI: 2.37–4.86) was associated with a higher risk of surgery in CD. Conclusion: Geographical heterogeneity exists in the disease characteristics and management of IBD in Eastern and Western China. These findings have the potential to guide the formulation of location-specific strategies aimed at enhancing the long-term outcomes of patients with IBD. [ABSTRACT FROM AUTHOR]

Abstract

In the DOA estimation of monostatic L-shaped array MIMO radar, Multiple Signal Classification algorithm is efficient. But the peak searching process of Multiple Signal Classification algorithm needs large amount of spectrum calculation. Focusing on the spectrum peak searching process of Multiple Signal Classification, an iterative search approach to reduce the calculation amount is proposed. The first- and second-order derivatives of Multiple Signal Classification spectrum functions are achieved and the calculation amount is analyzed. Two-dimensional Newton iteration methods are applied with multisearching threads and derivation information. The searching approach can greatly reduce the computational complexity of Multiple Signal Classification spectrum peak searching. The total calculation amount of the first and second derivatives is about 15 times of the spectrum function. However, in the two-dimensional searching, especially in the high accuracy processes, the amount of searched points can be reduced by ten hundreds times, and the computation is much lower than the common spectrum peak searching method. The simulation results show that when the search thread number reaches 100, the searching process can effectively achieve the entire spectrum peak and get the correct DOA estimation.

Abstract

The twenty-first century is the era of electronic warfare and information warfare. The focus is of the battle between all parties. CEEMD can link the time domain and frequency domain, describe the two-dimensional time–frequency characteristics of the signal, and draw the time–frequency diagram of the signal, so as to reduce the noise signal and improve the signal-to-noise ratio of the signal. The purpose of this paper was to study how to adjust the signal square spectrum bandwidth ratio in the subject of identifying the intra-pulse modulation of radar, so as to solve the problem of identifying the type of radar intra-pulse modulation. The experimental results in this paper show that the decomposition result of EEMD is incomplete and the signal reconstruction error is larger. Compared with the previous two methods, not only the CEEMD method can effectively suppress modal aliasing, but also the decomposition result is complete; the signal reconstruction error is very small, and the decomposition results close to ideal value. The interleaving filter with a bandwidth ratio of 1:2 can divide the 100 GHz channel spacing into asymmetric output spectra with bandwidths greater than 60 GHz and 30 GHz, which effectively improves the current mix of 10 Gb/s and 40 Gb/s The bandwidth utilization of the system illustrates the success of the simulation experiment. Abstract

In the DOA estimation of monostatic L-shaped array MIMO radar, Multiple Signal Classification algorithm is efficient. But the peak searching process of Multiple Signal Classification algorithm needs large amount of spectrum calculation. Focusing on the spectrum peak searching process of Multiple Signal Classification, an iterative search approach to reduce the calculation amount is proposed. The first- and second-order derivatives of Multiple Signal Classification spectrum functions are achieved and the calculation amount is analyzed. Two-dimensional Newton iteration methods are applied with multisearching threads and derivation information. The searching approach can greatly reduce the computational complexity of Multiple Signal Classification spectrum peak searching. The total calculation amount of the first and second derivatives is about 15 times of the spectrum function. However, in the two-dimensional searching, especially in the high accuracy processes, the amount of searched points can be reduced by ten hundreds times, and the computation is much lower than the common spectrum peak searching method. The simulation results show that when the search thread number reaches 100, the searching process can effectively achieve the entire spectrum peak and get the correct DOA estimation.

Abstract

This paper compares two approaches to tune PI controllers in a multivariable process. A modified Direct Synthesis method extends the technique to a multivariable system. Results show that the modified Direct Synthesis technique relies on model accuracy and suitability of the desired transfer function and process model. The effectiveness of this technique may break down when these conditions are not met. Genetic algorithms provide a method based on the evolution of species via natural selection. The GA method allows the use of a specific structure, such as PI controller, and then gets the best possible results for that structure. The concept has been adapted into an optimisation technique that can be used to find the best controller parameters to meet user defined transient specifications for a complex process. Thus, the robustness of the method allows it to outperform the former technique. Simulation results are shown to illustrate the comparisons between these two methods and the effectiveness of GAs.

19. Assessment of body composition-related imaging parameters indicative of sarcopenia in Chinese patients with Crohn's disease: correlation with disease severity and biologic efficacy.

Abstract

Objectives: To evaluate the prognostic value of body composition-related imaging parameters in assessing Crohn's disease (CD) severity and biological responses in Chinese patients., Methods: We retrospectively analyzed electronic medical records and Computed tomography (CT) images from 117 CD patients, including 90 with sarcopenia and 27 without. We calculated subcutaneous fat area (SFA), visceral fat area, skeletal muscle area (SMA), mesenteric fat index (MFI), skeletal muscle index (SMI), and muscle attenuation (MA). CD Activity Index (CDAI) score and Simple Endoscopic Score for CD (SES-CD) were used to evaluate inflammation and biologic efficacy. Correlation and comparative analyses were performed to determine associations between imaging parameters and clinical data. Receiver operating characteristic curve analysis evaluated the predictive performance of combined body composition indicators., Results: Sarcopenia was associated with higher CDAI scores and lower body mass index, albumin, and hemoglobin levels but was not associated with SES-CD or rates of clinical/endoscopic remission or response to biologic therapy. SMI was inversely correlated with CDAI score and SES-CD and positively correlated with albumin and hemoglobin. Endoscopy responders had higher SMA, MFI, SMI, and MA than non-responders. SES-CD improvement was positively correlated with MFI and MA and negatively correlated with SFA. The combined analysis of SMI, MFI, and MA yielded an area under the curve of 0.743 for predicting endoscopic response to biologic therapies in CD patients., Conclusions: SMI may indicate CD severity, while MFI and MA could predict biologic response.
