Life in the Anthropocene is characterized by many environmental problems, and unfortunately, more continue to emerge. Although much effort is focused on identifying problems, this does not necessarily translate to solutions. This situation extends to the training environment, where students are often adept at understanding and dissecting problems but are rarely explicitly equipped with the skills and mindset to solve them. Herein, a group of undergraduate students and their instructors consider the concept of becoming environmental problem solvers. We first identified themes associated with historical and contemporary environmental successes that emerged from our reading, or more specifically, we identify the elements that underlie environmental success stories. The key elements of success involved setting clear objectives, identifying the scale of the problem, learning from failure, and consulting diverse knowledge sources. Next, we reflected on the skills and mindset that would best serve environmental problem solvers and enable future successes. Essential skills include innovative and critical thinking, ability to engage in collaborative teamwork, capacity to work across boundaries, and resilience. In terms of mindset, key attributes include the need for courage, enthusiasm and commitment, optimism, open mindedness, tenacity, and adaptability. We conclude with a brief discussion of ideas for revising training and curriculum to ensure that students are equipped with the aforementioned skills and mindset. The ideas shared here should contribute to ensuring that the next generation of learners have the ability to develop solutions that will work for the benefit of the environment, biodiversity, and humanity. Solving environmental problems will increasingly fall to the next generation, so it is time to ensure that they are prepared for that task. La vie dans l'Anthropocene est caracterisee par de nombreux problemes environnementaux et, malheureusement, d'autres continuent d'apparaitre. Bien que de nombreux efforts soient consacres a l'identification des problemes, cela ne se traduit pas necessairement en solutions. Cela se repercute sur l'environnement de formation ou les etudiants sont souvent habiles a comprendre et dissequer des problemes, mais sont rarement explicitement equipes des competences et de l'etat d'esprit necessaires pour les resoudre. Ici, un groupe d'etudiants de premier cycle et leurs enseignants reflechissent a l'adoption du concept de devenir des personnes aptes a resoudre des problemes environnementaux. Les auteurs identifient d'abord les themes associes aux reussites environnementales historiques et contemporaines qui sont ressortis de leurs lectures--ou plus precisement, ils identifient les elements de la recette qui sous-tendent les reussites environnementales. Les elements cles du succes impliquaient de fixer des objectifs clairs, d'identifier l'ampleur du probleme, de tirer les lecons des echecs et de consulter diverses sources de connaissances. Ensuite, ils reflechissent aux competences et a l'etat d'esprit qui pourraient le mieux servir les personnes chargees de resoudre les problemes environnementaux et permettre de futurs succes. Les competences essentielles comprennent la pensee innovante et critique, la capacite a s'engager dans un travail d'equipe collaboratif, la capacite a travailler au-dela des frontieres et la resilience. En matiere d'etat d'esprit, les attributs cles comprennent le besoin de courage, d'enthousiasme et d'engagement, l'optimisme, l'ouverture d'esprit, la tenacite et l'adaptabilite. Ils concluent par une breve discussion des idees de revision de la formation et des programmes d'etudes afin de garantir que les etudiants soient dotes des competences et de l'etat d'esprit susmentionnes. Les idees partagees ici devraient contribuer a faire en sorte que la prochaine generation d'apprenants ait la capacite de developper des solutions qui fonctionneront dans l'interet de l'environnement, de la biodiversite et de l'humanite. La resolution des problemes environnementaux incombera de plus en plus a la prochaine generation, il est donc temps de s'assurer qu'elle est preparee a cette tache. - Abstract
BACKGROUND: Retinoblastoma is the most common intraocular cancer worldwide. There is some evidence to suggest that major differences exist in treatment outcomes for children with retinoblastoma from different regions, but these differences have not been assessed on a global scale. We aimed to report 3-year outcomes for children with retinoblastoma globally and to investigate factors associated with survival. METHODS: We did a prospective cluster-based analysis of treatment-naive patients with retinoblastoma who were diagnosed between Jan 1, 2017, and Dec 31, 2017, then treated and followed up for 3 years. Patients were recruited from 260 specialised treatment centres worldwide. Data were obtained from participating centres on primary and additional treatments, duration of follow-up, metastasis, eye globe salvage, and survival outcome. We analysed time to death and time to enucleation with Cox regression models. FINDINGS: The cohort included 4064 children from 149 countries. The median age at diagnosis was 23·2 months (IQR 11·0-36·5). Extraocular tumour spread (cT4 of the cTNMH classification) at diagnosis was reported in five (0·8%) of 636 children from high-income countries, 55 (5·4%) of 1027 children from upper-middle-income countries, 342 (19·7%) of 1738 children from lower-middle-income countries, and 196 (42·9%) of 457 children from low-income countries. Enucleation surgery was available for all children and intravenous chemotherapy was available for 4014 (98·8%) of 4064 children. The 3-year survival rate was 99·5% (95% CI 98·8-100·0) for children from high-income countries, 91·2% (89·5-93·0) for children from upper-middle-income countries, 80·3% (78·3-82·3) for children from lower-middle-income countries, and 57·3% (52·1-63·0) for children from low-income countries. 6. A Longitudinal Study of Mitral Regurgitation Detected after Acute Myocardial Infarction

Background: Mitral regurgitation (MR) is common following myocardial infarction (MI). However, the subsequent trajectory of MR, and its impact on long-term outcomes are not well understood. This study aimed to examine the change in MR severity and associated clinical outcomes following MI. Methods: Records of patients admitted to a single centre between 2016 and 2017 with acute MI treated by percutaneous coronary intervention (PCI) were retrospectively examined. Results: 294/1000 consecutive patients had MR on baseline (pre-discharge) transthoracic echocardiography (TTE), of whom 126 (mean age: 70.9 ± 11.4 years) had at least one follow-up TTE. At baseline, most patients had mild MR (n = 94; 75%), with n = 30 (24%) moderate and n = 2 (2%) severe MR. Significant improvement in MR was observed at the first follow-up TTE (median 9 months from baseline; interquartile range: 3–23), with 36% having reduced severity, compared to 10% having increased MR severity (p < 0.001). Predictors of worsening MR included older age (mean: 75.2 vs. 66.7 years; p = 0.003) and lower creatinine clearance (mean: 60 vs. 81 mL/min, p = 0.015). Change in MR severity was significantly associated with prognosis: 16% with improving MR reached the composite endpoint of death or heart failure hospitalisation at 5 years, versus 44% (p = 0.004) with no change, and 59% (p < 0.001) with worsening MR. Conclusions: Of patients with follow-up TTE after MI, MR severity improved from baseline in approximately one-third, was stable in around half, with the remainder having worsening MR. Patients with persistent or worsening MR had worse clinical outcomes than those with improving MR.

9. Harnessing Cu-64/Cu-67 for a theranostic approach to pretargeted radioimmunotherapy

- Abstract
Over the past decade, theranostic imaging has emerged as a powerful clinical tool in oncology for identifying patients likely to respond to targeted therapies and for monitoring the response of patients to treatment. Herein, we report a theranostic approach to pretargeted radioimmunotherapy (PRIT) based on a pair of radioisotopes of copper: positron-emitting copper-64 (64Cu, t1/2 = 12.7 h) and beta particle-emitting copper-67 (67Cu, t1/2 = 61.8 h). This strategy is predicated on the in vivo ligation between a trans-cyclooctene (TCO)-bearing antibody and a tetrazine (Tz)-based radioligand via the rapid and bioorthogonal inverse electron-demand Diels–Alder reaction. Longitudinal therapy studies were conducted in a murine model of human colorectal carcinoma using an immunoconjugate of the huA33 antibody modified with TCO (huA33-TCO) and a 67Cu-labeled Tz radioligand ([67Cu]Cu-MeCOSar-Tz). The injection of huA33-TCO followed 72 h later by the administration of 18.5, 37.0, or 55.5 MBq of [67Cu]Cu-MeCOSar-Tz produced a dose-dependent therapeutic response, with the median survival time increasing from 68 d for the lowest dose to >200 d for the highest. Furthermore, we observed that mice that received the highest dose of [67Cu]Cu-MeCOSar-Tz in a fractionated manner exhibited improved hematological values without sacrificing therapeutic efficacy. Dual radionuclide experiments in which a single administration of huA33-TCO was followed by separate injections of [64Cu]Cu-MeCOSar-Tz and [67Cu]Cu-MeCOSar-Tz revealed that the positron emission tomography images produced by the former accurately predicted the efficacy of the latter. In these experiments, a correlation was observed between the tumoral uptake of [64Cu]Cu-MeCOSar-Tz and the subsequent therapeutic response to [67Cu]Cu-MeCOSar-Tz.

- Abstract
IMPORTANCE: Early diagnosis of retinoblastoma, the most common intraocular cancer, can save both a child's life and vision. However, anecdotal evidence suggests that many children across the world are diagnosed late. To our knowledge, the clinical presentation of retinoblastoma has never been assessed on a global scale. OBJECTIVES: To report the retinoblastoma stage at diagnosis in patients across the world during a single year, to investigate associations between clinical variables and national income level, and to investigate risk factors for advanced disease at diagnosis. DESIGN, SETTING, AND PARTICIPANTS: A total of 278 retinoblastoma treatment centers were recruited from June 2017 through December 2018 to participate in a cross-sectional analysis of treatment-naive patients with retinoblastoma who were diagnosed in 2017. MAIN OUTCOMES AND MEASURES: Age at presentation, proportion of familial history of retinoblastoma, and tumor stage and metastasis. RESULTS: The cohort included 4351 new patients from 153 countries; the median age at diagnosis was 30.5 (interquartile range, 18.3-45.9) months, and 1976 patients (45.4%) were female. Most patients (n = 3685 [84.7%]) were from low- and middle-income countries (LMICs). Globally, the most common indication for referral was leukocoria (n = 2638 [62.8%]), followed by strabismus (n = 429 [10.2%]) and proptosis (n = 309 [7.4%]). Patients from high-income countries (HICs) were diagnosed at a median age of 14.1 months, with 656 of 666 (98.5%) patients having intraocular retinoblastoma and 2 (0.3%) having metastasis. Patients from low-income countries were diagnosed at a median age of 30.5 months, with 256 of 521 (49.1%) having extraocular retinoblastoma and 94 of 498 (18.9%) having metastasis. Lower national income level was associated with older presentation age, higher proportion of locally advanced disease and distant metastasis, and smaller proportion of familial history of retinoblastoma. Background and aims: Keratoconus (KC) is a degenerative eye disorder, which is leading to irregular corneal surface, thinning of cornea, changing the shape of a cone and decreased vision. The disease incidence between 1/500 to 1/2,000 people worldwide is estimated. Several genes associated with the disease have been studied, but evidence suggests a major role of visual system homeobox1 (VSX1) in the etiology of KC. The aim this study was to investigate the relationship between rs6050307 single nucleotide polymorphisms (SNP) in exon 1 of the VSX1 gene with keratoconus disease in Chaharmahal and Bakhtiari province. Methods: In this laboratory discriptivel study, rs6050307 polymorphisms of VSX1 gene in 100 KC patients and 100 healthy individuals have been studied. DNA was extracted using standard phenol chloroform method and rs6050307 were determined by polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP). Allele frequency of this polymorphism were analyzed using chi-square test and the relevance between allele frequency polymorphism were evaluated with the disease. Results: Genotype distribution of diferent allels of rs6050307 VSX1 gene between patient and control samples was not significant (P>0.05). Conclusion: Regarding to the findings in this research, it seems that rs6050307 of VSX1 gene not involved as a risk factor in the pathogenesis of keratoconus.

We report two tunable diode laser configurations emitting around 2.6 μm, where the gain is provided by a high-gain GaSb-based reflective semiconductor optical amplifier. The lasers are driven in pulsed mode at 20 °C, with a pulse width of 1 μs and 10% duty cycle to minimize heating effects. To demonstrate the broad tuning and high output power capability of the gain chip, an external cavity diode laser configuration based on using a ruled diffraction grating in a Littrow configuration is demonstrated. The laser shows a wide tuning range of 154 nm and a maximum average output power on the order of 10 mW at 2.63 μm, corresponding to a peak power of 100 mW. For a more compact and robust integrated configuration, we consider an extended-cavity laser design where the feedback is provided by a silicon photonics chip acting as a reflector. In particular, the integrated tuning mechanism is based on utilizing the Vernier effect between two thermally tunable micro-ring resonators. In this case, a tuning range of around 70 nm is demonstrated in a compact architecture, with an average power of 1 mW, corresponding to a peak power of 10 mW. [ABSTRACT FROM AUTHOR]

- Abstract
Nowadays, the success of a company is dependent to the novelty of the company in developing new items. Product design and engineering are a basic phase in developing new commodities which examines the product economically and technologically. In the proposed study, “Trust” is identified as an effective factor on the life cycle of the new designed product. This study addresses a simulation structure to generate all the possible trust modes between two agents over time and implements four prediction methods to forecast the trust value of the new item. The time horizon is considered to be short term and middle term, and 27 and 108 scenarios are designed, respectively, based on three categories involving high, medium and short trust. Here, three prediction techniques: conventional time series, artificial neural networks and adaptive neuro-fuzzy inference system, are recommended and compared. By comparing MAPEs of all prediction methods, the best technique is identified.

- Abstract
Otera’s distannoxane catalyst was found to promote the cleavage of carbamate groups from N-protected aziridines. This method enables the chemoselective cleavage of an aziridinyl N-carbobenzyloxy (Cbz) group in the presence of other N-Cbz groups. The selectivity is due to the longer, weaker N–C bond of aziridinyl carbamates, as inferred through IR and crystallographic analyses.

The last few years have seen an increased necessity to assess the cardiovascular risk factors on large samples of the population in Italy. A look at the citizens of a small rural town Brisighella is well known.Three hundred and five individuals of average age 40.8 years (+/-8.22), recruited between September 1988 and December 1989 (198 men, 107 women) were taken into consideration. Clinical and instrumental investigations were then performed (intern visits with data obtained from primary anthropometric, otorhinolaryngological and ocular variables, heart X-ray, electrocardiogram). The following analyses were also performed: haemochrome, ESR, uricemia, azotemia, glycemia, AST, ALT, bilirubinemia, protein electrophoresis, triglyceridemia, total cholesterol, HDL cholesterol, creatinine level and urine analysis. Women also received PAP-test. Another variable was lifestyle habits which may be considered a risk for cardiovascular disease. Hygienic and diet suggestions were made. After seven years, in 1997, 100 of these individuals (51 men, 49 women - mean age 42.7+/-6.5 years) were chosen at random and administered the same clinical and laboratory examinations as before.The analysis of the data show average BMI values 24.07+/-2.7 in men and 22.8+/-2.6 in women, inferior to those found in other Italian investigations. We observed an increase of BMI along with age, despite measures of preventive medicine, in married subjects and along with education and occupation level. Mean values of cholesterol level in men increased between the ages 30 and 39 and stabilised after 40 years; in women they increased only after the 5th decade of life.We observed a direct correlation between the increase of uricemia and triglyceride levels, triglyceride and cholesterol levels, BMI and triglyceride levels conferming the tight bond among these metabolisms and the importance of obesity as a risk factor. Uricemia levels correlate with systolic and diastolic blood pressure.

5-Fluorouracil is used as an antineoplastic agent in solid tumors. The study was conducted to analyze the effect of EDDA on synthesis of 5-fluorouracil with Tc. The Tc-5-flurouracil was formulated using stannous agent, and EDDA. This complex was stable for 4 h, with post labeling efficiency of 92 %. The distribution study in animal model showed that after 30 min 35 ± 8 % of injected dose cross the blood brain barrier and excreted through kidney with no sign of toxicity. It was concluded that the addition of EDDA modified the labeling side in 5-fluorouracil for Tc, which localized in brain and hence can be used further for brain imaging study. [ABSTRACT FROM AUTHOR]
