2. Is Family Support A Factor in Patient Adherence to Immunosuppressıie Treatment? A Descriptive Study.

Objective: Non-adherence to immunosuppressive treatment can cause graft rejection in kidney transplantation patients. The reasons for nonadherence have been shown to be side-effects of the drugs, compliance fear, and lack of family support. The aim of this study was to determine the effect of family support on drug adherence. Material and Method: This study was conducted on 98 patients followed up in the polyclinic of a public hospital following kidney transplantation. The data were obtained from a patient information form, the Perception of Family Support Scale (PSS-Fa), the Immunosuppressive Treatment Adherence Scale (ITAS), and the tacrolimus blood plasma level of the patients. Results: In patients aged =40 years, the PSS-Fa was determined to be high (p <0.05), and family support was determined to have no effect on time since transplantation, donor type and rejection attack (p >0.05). The mean ITAS points were 11.03±0.90 and the tacrolimus blood plasma level mean standard deviation was <2.47, indicating high drug adherence. A positive correlation was determined between age and ITAS points (p <0.05). No correlation was determined between the tacrolimus blood plasma level standard deviation mean value and the ITAS and PSS-Fa mean points (p >0.05). Conclusion: No relationship was determined between family support and drug adherence. Further studies can be recommended to evaluate groups including different transplantation types to investigate the effect of family support on drug adherence. [ABSTRACT FROM AUTHOR]

4. De novo extended-release Tacrolimus in Kidney Transplant Patients; Is it safe?

Background: This study aimed to investigate whether de novo extended-release tacrolimus (ERT) therapy is safe in kidney recipients. Methods: The study was single-center, retrospective, and included a total of 57 patients, including 30 patients in the ERT group (Group 1) and 27 patients in the immediate-release tacrolimus (IRT) group (Group 2). Demographic and laboratory characteristics of the patients were recorded. Complications such as acute drug toxicity, acute rejection, new-onset diabetes mellitus after transplantation, and development of hypertension, opportunistic infection, and hospitalization data were recorded. Results: The mean age of the patients was 46.23±14.2 years in the ERT group and 47.04±14.6 years in the IRT group. There were 21 (70%) males in group 1, while 20 (74%) patients in group 2 had a male gender (P=0.73). The rate of improved serum creatinine values in the first week postoperatively was similar in both groups. While the mean tacrolimus levels on postoperative day 1 were significantly lower in the group- 1 (P lt;0.05), there was no significant difference between tacrolimus levels on postoperative days 2-7. There was no significant difference between the groups regarding opportunistic infections, diabetes mellitus, and the need for hospitalization in the first six months of follow-up. Conclusion: Initiation of de novo extended-release tacrolimus therapy in kidney recipients is safe in the long term and preserves graft function.

Objective: Renal transplantation is a challenging process for the recipients. One of the important problems in this process is unwanted weight gain. This study aimed to determine the change in body mass index (BMI) and to evaluate the effect of recipient characteristics on BMI during one-year period after renal transplantation. Methods: The article was conducted in a retrospective design. In the study, files of 170 patients who underwent renal transplantation between 2015 and 2016 were reviewed retrospectively. T-test, ANOVA, and correlation analysis were used in the analysis of data. Results: It was determined that the patients had a tendency to have increased BMI after transplantation, with a higher rate in the first three months. The increase in BMI was higher in singles than in married participants (p=0.01 and p<0.05), and in men than in women (p=0.01 and p<0.05). It was determined that there was no significant relationship between BMI values and glomerular filtration rate, and creatinine levels of adult renal transplant recipients (p>0.05). Conclusion: In the study, it was determined that the increase in BMI was higher especially in the first three months after renal transplantation. For this reason, it is an important requirement to address the counseling and support to patients and their relatives regarding weight management in the early period. [ABSTRACT FROM AUTHOR]

Background/Aims: An impaired immune response affects Coronavirus 2019 (COVID-19) disease progression. Immunities of both hemodialysis (HD) and Kidney Transplant (KTx) patients have already been suppressed. This study evaluated the prognostic laboratory results in HD and KTx patients with COVID-19. Methods: This retrospective, case-control study was conducted with PCR (+) COVID-19 HD and KTx patients and a control group. All patients were divided into two subgroups according to disease severity. Patients' demographic records and laboratory results were obtained from the follow-up files. Results: A total of 30 HD, 20 KTx patients, and 40 control groups were involved in the study. Gender and hospitalization duration did not differ between the groups. There was a 10% mortality rate in the KTx group and 27% in the HD group. Lung involvement in Computed Tomography (CT) was higher in HD patients (47%) than in KTx (25%). In subgroup evaluations, the most prominent laboratory values were fibrinogen in HD patients and LDH and Ferritin in KTx patients in determining disease severity. Conclusions: Early hospitalization and treatment implementations will be associated with a good prognosis in HD and KTx patients since CT and laboratory results are not predictive in these groups of patients during the COVID-19 pandemic.

9. Mean Arterial Pressure Control Impact on Early Graft Function in Kidney Transplantation.

- Abstract
Objective: For end-stage renal disease, kidney transplantation is the best therapy. Early graft function is critical for kidney transplant success. The purpose of this study was to see how mean arterial pressure (MAP) affected early graft function and biochemical outcomes. Methods: This was a retrospective study of transplanted patients from live-related donors at the Transplant Center Hospital, Tirana, Albania, between January 2007 and December 2022.The patients divided based on their mean MAP. One group had MAP above 100 mmHg (68 patients). The second group had MAP between 85-100 mmHg (74 patients) and the other group MAP less than 85 mmHg (52 patients). Serum creatinine decrease was observed during the first three post-operative days and the sixth day. For statistical computation, correlation analysis, analysis of variance test (ANO-VA), and multivariate analysis technique were used in this study. Results: On the first, second, and third postoperative days, the first and second groups’ mean serum creatinine values were comparable to the third group’s mean values. The mean MAP at the time of declamping was 108.64 mmHg for the first group, 95.28 mmHg for the second group, and 80.18 mmHg for the third group. The mean MAP ranged from 78 to 118 mmHg. There was no significant difference between the first two groups on the first, second, third, and sixth postoperative days, but there were significant differences between the first and second groups compared to the third group on the first, second, and third postoperative days. Conclusion: Early graft function requires a mean MAP greater than 85 mmHg and adequate fluid hydration. [ABSTRACT FROM AUTHOR]

Aim: In this study, we aimed to present demographic characteristics of the patients having undergone renal transplantation in our hospital between February 2017 and September 2020, to convey the experience of cadaver and living donor renal transplantation in our center, and to reveal the causes of allograft dysfunction in late posttransplant follow-ups. Material and Method: The study included 25 patients having undergone renal transplantation in our hospital and were followed up in the nephrology clinic between February 2017 and September 2020. Then, we retrospectively analyzed their demographic characteristics, clinical and laboratory findings, transplantation types, kidney donor characteristics, renal failure etiologies, pre-transplant dialysis modalities, post-transplant complications, rejection attacks, graft loss, and causes of mortalities. Results: The mean follow-up period of 25 renal transplant patients (17 males and 8 females with a mean age of 46.96±2.67 years) was 26.28±2.76 months. While living transplantation was performed in 16 patients, the others received cadaveric renal transplants. Five patients underwent pre-emptive renal transplantation, and three were recruited for transplant for the second time. The underlying cause of chronic kidney disease was unknown in 24% of the patients (first), while vesicoureteral reflux (VUR) and diabetes were the primary causes of the disease in 16% (second). The most prevalent cause of temporary or permanent impairment in allograft functions was urinary tract infection with 42.8%. It was more common in female patients (25%) and patients with a diagnosis of VUR (20%). Conclusion: Overall, our findings documented that urinary system infections are common following renal transplantation, which brings adverse effects on allograft functions. VUR-led renal failure and female gender are among the factors that facilitate urinary system infection. [ABSTRACT FROM AUTHOR]

20. Biopsy-proven BK virus nephropathy in kidney transplant patients: risk factors, prevalence and treatment approach.

Purpose: BK virus nephropathy (BKVN) is a latent infection and it is closely associated with immunsuppressive therapy. We aimed in this study to evaluate biopsy-proven BKVN and investigate frequency, risk factors and treatment management. Materials and Methods: In this study, 422 kidney transplant recipients were analysed retrospectively between April 2014 and April 2020 for biopsy-proven BK virus nephropathy. Group I included 16 kidney transplant patients with biopsy-proven BK nephropathy and group II included 36 kidney transplant patients with negative BK virus nephropathy. We aimed demographic, clinical features of kidney transplant recipients with BKVN (group I, n: 16) and non-BKVN (group II, n:36) were compared and the factors affecting of BKVN. Results: The mean age of grup I and group II were were 41±14.8 years and 39±15.2 respectively. The patients mean follow-up period of 43±11.2 months. Serum creatinine and proteinuria degree were significantly higher in the group with BKVN. In order to reduce the dose of immunosuppression in patients with BKVN, tacrolimus treatment was discontinued in 8 patients, and they were switched to everolimus + MMF + prednisone treatment, leflunamide + MMF + prednisone treatment in 4 patients, and sirolimus + MMF + prednisone treatment in 4 patients. The mean serum creatinine level of the patients who were followed up were observed as 1.78±0.98 mg/dl in group I. Conclusion: In our center, the prevalance of BKVN was found 3.92% during the study period. Reduction of dose immunosuppressive therapy is the most effective treatment. It is thought that there was no differences between Leflunamide and other approaches for treatment. Early diagnosis and screening (frequently intervals) seems to be most effective way for BKVN. 22. Urinary Tract Infection in Kidney Transplant Recipients: The predictors and two-year outcomes.

Background: Urinary tract infection (UTI) is the most common infection in kidney transplant recipients (KTR). Our aim in this study is to determine the prevalence, risk factors, and causative microorganisms of UTI. In addition, to compare the kidney functions of the patients in the 2nd year who developed and did not develop UTI after transplantation. Method: Two hundred sixteen patients underwent kidney transplantation in our center between July 2012 and March 2020. A total of 206 patients with 267 episodes of UTI were included in the study. The impacts of catheterization, hemodialysis duration, gender, posttransplant prolonged hospital stay on UTI development, and UTI on two-year allograft functions, were evaluated. Results: The mean age of the study patients was 34.5±12.7, and 43.7% of them were women. At least one UTI attack developed in 38.8% (80/206) of the KTR. Thirty-one KTR developed recurrent UTI (R-UTI). UTI incidence was found 38.8% in our cohort. Female gender, posttransplant prolonged hospital stay, presence of prolonged double-j stent and foley catheter durations were found associated with UTI development. The mean estimated glomerular filtration rate eGFR in KTR with UTI at 2-year post-transplant was significantly lower than KTR without UTI (71.2±29.2 vs 82.4±23.9; p=0.006). Low eGFR was more prominent among the KTR with R-UTI (69.9±31.6). Escherichia coli and Klebsiella pneumonia were the most frequently isolated microorganisms in our cohort. Conclusions: This study demonstrated UTI may have an adverse impact on allograft function in KTR, especially in KTR with R-UTI. [ABSTRACT FROM AUTHOR]

Objective: Tacrolimus is used in more than 80% of kidney transplant recipients due to its ability to avoid rejection. Irregularities in tacrolimus level may affect clinical outcomes by subjecting patients to adverse events associated with graft rejection or immunosuppressive therapy. There are two forms of tacrolimus; immediate-release (IR-T) and prolonged release (PR-T). This study is designed to compare the clinical follow-up of kidney transplant patients who are receiving prolonged-release (PR-T; Advagraf) and immediate-release (IR-T; Prograf) tacrolimus in the posttransplant first week. Methods: This study included 78 de novo, adult kidney transplant patients to prolonged-release tacrolimus 0.15 mg/kg/day (group 1, n=39) and immediate-release tacrolimus 0.15 mg/kg/day (group 2, n=39) for the first week in the posttransplant period. Demographic features, whole blood tacrolimus levels and kidney function were compared between the two groups. The presence of acute rejection and adverse events, antihypertensive drug use and arterial blood pressure of all patients were considered. Drug doses were determined according to the previously targeted tacrolimus level. SPSS 22 for Windows was used for statistical analysis. Results: Acute rejection was not seen in any patient and there were no adverse events in the posttransplant first week. However, group 2 patients were found to have higher tacrolimus levels on the posttransplant 1st, 4th and 7th days (p=0.02, p=0.009 and p=0.013 respectively). Serum creatinine levels were significantly increased in group 2 patients on the posttransplant 7th day (p=0.02). Systolic, diastolic or mean arterial blood pressure were not different between the groups. Conclusion: Prolonged-release tacrolimus is effective in preventing acute rejection when adequate blood levels are maintained, and appears promising as it makes it possible to avoid interindividual variation in absorption and early calcineurin inhibitor toxicity. This study was conducted in a descriptive and correlational type to determine the relationship between depression, anxiety, stress levels and compliance with immunosuppressive therapy in kidney transplant patients. Research data were collected between November 20 and December 10, 2021. At the end of the study, 95 patients were reached. Data analysis was performed by using SPSS 25 and all legal and ethical permissions were obtained. At the end of the study, the depression, anxiety, stress, DASS-21, and SAITTP total scores of the patients were determined as; 5.15±3.61, 5.16±3.78, 9.62±4.93, 19.94±10.37, and 11.42±3.29, respectively. It was determined that the mean depression scores of female patients were higher than that of male patients. It was concluded that self-employed patients had a higher depression score average than patients working as civil servants. It was determined that the mean SAITTP rank of patients with extended family structure was higher than patients with nuclear family structure. In this study, no statistically significant relationship could be found between the SAITTP scores of the patients and their depression, anxiety, and stress scores. No correlation was found between depression, anxiety, stress levels and compliance with immunosuppressive therapy in kidney transplant patients. Objective: To examine the causes of hypercalcemia developing after kidney transplantation and investigate its effects on graft functions. Methods: The results of 104 patients were explored retrospectively. Patients assigned according to calcium levels at 12th month after transplantation as hypercalcemia group (Ca2+ >10.2) and normocalcemia (Ca2+ ≤10.2) group. Glomerular filtration rates were calculated using the Chronic Kidney Disease Epidemiology Collaboration equation for each follow-up period. Results: A total of 104 patients, 30 (29%) females and 74 (71%) males, were included in our study. Patients were divided into two groups as hypercalcemic (Ca2+ >10.2) (n=30, 29%) and normocalcemic (Ca2+ ≤10.2) (n=74, 71%) according to their 12-month follow-up results. While there was no significant difference in alkaline phosphatase levels (ALP) (p=0.720) at the time of transplantation, a significant difference was found in ALP in the 12th-month measurements (p<0.001). Both parathormone levels at the transplantation time (p=0.006) and 12th-month follow-up results (p<0.001) were significantly higher in the hypercalcemia group. When we evaluated the graft functions of the patients, no significant difference was found between e-GFR levels in the 1st, 3rd, and 12th months. Conclusion: There is no association between posttransplant hypercalcemia and changes in graft function in kidney transplantation patients. [ABSTRACT FROM AUTHOR]

Objective: Even after successful kidney transplantation, persistent hyperparathyroidism (HPT) with hypercalcemia is common, which is considered to be a risk factor for progressive bone loss, fractures, tubulointerstitial calcifications, vascular calcification, and development of graft dysfunction. The subtotal parathyroidectomy (PTX) is the standard treatment, but currently, it has been replaced by the calcimimetic cinacalcet. The aim of this single-center, retrospective study was to compare the long-term effects of PTX and cinacalcet on calcium, phosphorus, and parathyroid hormone (PTH) levels, as well as graft function in renal transplantation patients. Methods: The study population consisted of 24 patients followed between January 2004 and December 2020, 13 of whom underwent PTX and 12 of whom take cinacalcet therapy. The surgical group and the medical treatment group were compared. A control group with similar characteristics in terms of age, gender, transplant time, and kidney donor was formed, and the results were also compared with this group. The median PTH, calcium, phosphorus, creatinine levels, and eGFR values were recorded before/after PTX and cinacalcet therapy. Results: There was nonnegative effect of both PTX and cinacalcet groups on long-term and short-term allograft functions compared with control groups. Allograft functions were similar in comparison between PTX and cinacalcet groups. Conclusion: In conclusion, both cinacalcet treatment and PTX were found to be similarly effective and safe in reducing intact PTH and normalizing serum calcium in renal allograft recipients with hypercalcemia due to persistent HPT. Objective: We aimed to evaluate the quality of video content related to kidney transplantation on YouTube as a source of information. Method: 117 YouTube videos were included in this study. Utilization of Journal of the American Medical Association (JAMA) score, Global Quality Scale (GQS) and modified DISCERN score were approved for quality assessment. Results: Stand-alone health information websites comprised 35.9% of all video resources. The quality of the videos from universities/professional organizations/nonprofit physicians/physicians was generally better than the videos from other sources (p<0.001). According to the modified DISCERN score, the quality level of 72.6% of the video contents classified as poor. A positive correlation drew attention between the scores and the duration of videos (p<0.001). Conclusions: We think that the quality of the video content about kidney transplantation on YouTube is quite insufficient. The best quality of content was provided by universities/professional organisations/nonprofit physicians/physicians among all videos. [ABSTRACT FROM AUTHOR]

Amaç: Bu çalışmanın amacı nötrofil-lenfosit oranı (NLO) ve trombosit-lenfosit oranı (TLO) ile böbrek nakli sonrası akut rejeksiyon arasındaki ilişkiyi değerlendirmektir. Gereç ve Yöntemler: Ocak 2016 ile Ocak 2019 tarihleri arasında yürütülen bu geriye dönük çalışmaya ortanca yaşı 41 (aralık, 19-64) yıl olan toplam 36 yetişkin (33 erkek, 3 kadın) böbrek nakli alıcısı dahil edildi. Hastaların demografik özellikleri ve laboratuvar bulguları (nötrofil, lenfosit, trombosit, kreatinin, eGFR, serum ürik asit, C-reaktif protein) kaydedildi. Akut rejeksiyon Banff kriterlerine göre renal biyopsi ile tanımlandı. Bulgular: On altı (%44,4) hastada akut rejeksiyon gelişti. NLO (ortanca 3,75’e karşı 1,99; p=0,001) ve TLO (ortanca 125,59’a karşı 99,23; p=0,008) değerleri akut rejeksiyon grubunda kontrol grubuna göre anlamlı olarak daha yüksekti. Eğri altındaki alan, NLO için 0,822 ve TLO için 0,759 olarak hesaplandı. Cut-off değer NLO için >2,5 (%75 duyarlılık ve %75 özgüllük) ve TLO için >108 (%81 duyarlılık ve %65 özgüllük) olarak belirlendi. Tek değişkenli analiz, NLO >2,5 (Odds Ratio (OR)=0,267; %95 Güven Aralığı (GA)=0,089-0,803; p=0,019) ve TLO >108 (OR=0,231; %95 GA=0,066-0,810; p=0,022) ile akut rejeksiyon arasında güçlü bir korelasyon olduğunu ortaya koydu. Sonuç: Böbrek nakli hastalarında, yüksek NLO ve TLO değerleri ile akut rejeksiyon gelişimi arasında güçlü bir ilişki vardır. Basit, erişimi kolay, ucuz ve invaziv olmayan yöntemler olarak TLO ve özellikle NLO, transplant sonrası akut rejeksiyonu teşhis etmek için potansiyel testler olabilirler.

Amaç: Bu çalışmanın amacı böbrek nakil sonrası dönemde lomber ve femoral kemik mineral yoğunluğundaki (KMY) değişiklikleri değerlendirmektir.Gereç ve Yöntemler: Bu retrospektif çalışmaya, Ocak 2016 ve Temmuz 2019 tarihleri arasında böbrek nakli yapılan ardışık 69 erişkin (>18 yaş) hasta dahil edildi. Hastaların demografik özellikleri ve laboratuvar bulguları (serum düzeltilmiş kalsiyumu, fosfor, alkalen fosfataz, kreatinin, eGFR, i-parathormon ve 25(OH) D vitamin düzeyi) ile ilgili veriler kaydedildi. KMY, dual enerji X-ray absorbsiyometri (DEXA) ile değerlendirildi.Bulgular: DEXA sonuçlarına göre, lomber ve femoral T skorları sırasıyla -1,1±1,3 ve -1,1±1,1 idi. Lomber değerlendirmede 12 (%17,4) hastada osteoporoz ve 24 (%34,8) hastada osteopeni saptandı. Femoral değerlendirmede 7 (%10,1) hastada osteoporoz ve 29 (%42,0) hastada osteopeni saptandı. İlk yıl, 1-2 yıl ve >2 yıl takip verilerine göre, sırasıyla lomber bölgede hastaların %4,3, %5,8 ve %7,2’sinde, femurda hastaların %2,9, %2,9 ve %4,3’ünde osteoporoz saptandı. Lomber bölge (p=0,197) ve femurda (p=0,971) KMY kaybı tespit edilen hastalar ile KMY ölçümü normal olan hastalar arasında kümülatif steroid dozu açısından anlamlı bir fark yoktu. Ayrıca hastaların almış olduğu indüksiyon tedavisi ile lomber bölge (p=0,671) ve femurda (p=0,126) gelişen KMY kaybı arasında anlamlı bir ilişki görülmedi.Sonuç: Sonuç olarak, nakil hastalarında 25(OH) D vitamini eksikliği oldukça yaygındır ve özellikle nakilden sonraki ilk yıl lomber bölgede belirgin olmak üzere KMY kaybı görülmektedir.

41. Anthropometric Parameter That Best Predicts the Relationship Between Obesity and Osteoporosis in Kidney Transplant Recipients.

Objective: This study aimed to investigate the relationship between obesity and osteoporosis in kidney transplant recipients (KTRs) using a body shape index (ABSI), a new anthropometric measurement parameter. Materials and Methods: In this cross-sectional study, a total of 130 KTRs were included. Laboratory and demographic findings, aetiology of kidney failure, time on dialysis, donor type, time after transplantation, additional diseases, menopausal status for women and medications were recorded from patients' files. Height, weight and waist and hip circumferences were also recorded from the files. ABSI was calculated using a specific formula. Bone mineral density (BMD) was measured using dual X-ray absorptiometry. Patients were divided into the following three groups based on the T-score: Normal, osteopenia and osteoporosis groups, according to the World Health Organization recommendations. Results: According to the BMD values, 24 patients (18.5%) had normal BMD, 82 patients (63.1%) were osteopenic and 24 patients (18.5%) were osteoporotic. Patients with osteoporosis have significantly higher parathormone hormone and ABSI and lower body mass index (BMI) than patients without osteoporosis (p<0.05). A positive correlation of BMI was found in the lumbar (r=0.294, p=0.002) and femoral T-scores (r=0.216, p=0.024). A negative correlation was found between ABSI and lumbar T-score (r=-0.237, p=0.014). Receiver operating characteristic curves showed that ABSI had the optimal power to discriminate patients with/without osteoporosis in both sexes. Conclusion: ABSI is found to be a more useful tool than anthropometric measurements in predicting osteoporosis in KTRs. [ABSTRACT FROM AUTHOR]

Objective: Alport syndrome is an inherited disease that occurs in 1/50,000 and is characterized by hematuria, kidney failure, deafness and ocular anomalies. Alport syndrome, which is inherited as X-linked recessive with a rate of 85%, may be inherited also as autosomal dominant or autosomal recessive. Defective type-4 collagen participating in the structure of glomerular basement membrane causes a progressive decline in kidney functions. This research investigates the results of end-stage renal failure in patients with Alport syndrome following kidney transplantation. Methods: In our Hospital Organ Transplant Center 11 kidney transplantations were performed in 10 patients with Alport syndrome between October 2010 and December 2020. The recipients were analyzed retrospectively for acute rejection, complication rate, graft and patient survival. Results: Eleven (0.88%) of 1,251 kidney transplants were performed in patients with Alport syndrome. Acute rejection did not occur in any patient after kidney transplantation and no medical or surgical complications were observed in the early postoperative period. One patient died 19 months after surgery because of pneumonia and sepsis while his graft was functional. Graft loss was observed in two patients. In one of these patients, graft loss developed due to drug incompatibility in the 11th month after kidney transplantation. In the other patient, graft loss was observed due to chronic allograft nephropathy in the 63rd month postoperatively. This patient underwent a second kidney transplant surgery from a living donor. Conclusion: Alport syndrome is a rare cause of chronic kidney failure. Kidney transplantation is an effective and successful treatment method for end-stage renal disease patients with Alport syndrome. [ABSTRACT FROM AUTHOR]

Aim: Viral infections commonly affect kidney transplant recipients and may lead to graft failure and death. The aim of this study was to evaluate the antibody seroprevalence against viral agents in kidney transplant recipients. Material and Methods: The records of children who underwent kidney transplantation between 2008 and 2018 in Akdeniz University Faculty of Medicine were retrospectively reviewed. Epstein-Barr virus, cytomegalovirus, hepatitis A virus, hepatitis B virus, varicella, measles, rubella and mumps serologies evaluated before transplantation, were recorded. The clinical characteristics of seronegative and seropositive patients were compared, and factors that affected seropositivity were investigated. Results: The study included 253 children with a mean age of 16.7±6.23 years. The mean age at transplantation was 11.4±5.01 years. The seropositivity rates for vaccine-preventable viral infections varied: hepatitis B 89.7%, hepatitis A 60.5%, measles 78.7%, rubella 88.1%, mumps 61.2%, and varicella 71.9%. Cytomegalovirus seropositivity was 92.1% and Epstein- Barr virus seropositivity was 82.2%. Hepatitis B antibody positivity was 91.8% in patients undergoing hemodialysis, 94.5% in patients electrocheundergoing peritoneal dialysis, and 84.9% in pre-emptive transplantation patients (p=0.037). The mean age at transplantation was higher in patients with seropositivity for both cytomegalovirus and Epstein-Barr virus compared with seronegative patients (p<0.001 for both). The mean age at transplantation and diagnosis of glomerular disease was found to be effective for varicella seropositivity in multivariate regression analysis (OR 0.860, 95% CI: 0.808--0.915, p<0.001 and OR 2.502, 95% CI: 1.321--4.739, p=0.005, respectively). Conclusion: It is important to screen patients with chronic kidney disease in terms of vaccine-preventable diseases to identify risky groups of patients and to immunize these patients before end-stage kidney disease develops. Cytomegalovirus (CMV) is a common virus that causes opportunistic infections in many organ systems in immunocompromised patients. CMV infections are also an important problem in renal transplant patients. CMV infection often causes complications in gastrointestinal system (GIS). CMV duodenitis is a very rare involvement in the GIS. CMV infection can lead to complications, such as erosion, ulcer, adhesion, perforation and bleeding in the bulbus and duodenum. Our case was admitted with the signs of upper GIS bleeding 10 months after renal transplantation. Ulcers in the antrum and bulbus were observed in the endoscopic examination, typical incision bodies for CMV and intranuclear CMV immunoreactivity were detected in duodenal biopsies, and complaints regressed with antiviral treatment. Physical, emotional and economic aspects of transplantation are an important life experience for the patient and the family. Good organizational success in transplantation, surgery preparation and post-operative care is extremely important as well as biopsychosocial care and rehabilitation. This study describes the post-transplant experiences of patients who underwent kidney transplantation. In this study qualitative exploratory research design was used and collecting comprehensive information about the post-transplant psychosocial experiences of kidney transplant patients was aimed at. The sample of the study was consisted of 15 participants over the age of 18 who had organ transplantation at Mediterranean University Hospital Institute of Transplantation. Data were analyzed using content analysis for which nine themes were identified as challenges of life, changes in mood, changes in self-perception, the meaning of organ transplantation for the individual, interpersonal relationships, increased productivity, coping, perception of the organ, feelings towards the donor. In this study, detailed data about biopsychosocial lives, organ perception, donor perception and coping methods were obtained after kidney transplantation. Background: Acute and chronic calcineurin inhibitors (CNI) nephrotoxicity is a common concern in kidney transplant (KT) recipients. It is unclear whether diltiazem use can reduce CNI induced acute and chronic nephrotoxicity in (KT) recipients. In this study, we investigated the impact of diltiazem on 1-year allograft survival and function. Materials and Methods: This single-center retrospective study included 312 kidney transplant recipients and donors. Diltiazem receiving and diltiazem-free recipients were compared for 1-year allograft survival and functions. Available allograft biopsies were investigated for the evidence pieces of CNI induced nephrotoxicity. Factors may have a potential impact on allograft functions were evaluated (cytomegalovirus and polyoma BK viremia positivity, acute rejection episodes, donors and recipients ages and body mass indexes). A statistical package program was used for data analysis. P<0.05 was assigned significant. Results: Seventy-three recipients in diltiazem arm and 239 in diltiazem-free arm were compared. In diltiazem and diltiazem-free arms, 1- year mortality, allograft survival rates and CNI induced nephrotoxicity incidences were 4.1% vs 3.8% (P=0.89), and 13.7% vs 7.1% (P=0.08), 18.8% vs 10.5% (P=0.27), respectively. However, 12- month estimated glomerular filtration rate was worse in diltiazem arm (62.75 ml/dk/1.73m²) compared to diltiazemfree group (73.19 ml/dk/1.73m2) (P=0.03). CNI toxicity had a weak impact on low eGFR in regression analysis (P=0.055 and 95% confidence interval). Conclusions: Despite diltiazem use allows to CNI dose reduction, it might have undesirable impacts on longterm allograft functions, which is the main target of the allograft care. Aim: The aim of this study was to evaluate the association of neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) with acute rejection after kidney transplantation. Material and Methods: A total of 36 adult renal transplant recipients (33 males, 3 females) with a median age of 41 (range, 19-64) years were included in this retrospective study conducted between January 2016 and January 2019. Data on patient demographics and laboratory findings (neutrophil, lymphocyte, platelet, creatinine, eGFR, serum uric acid and C-reactive protein) were recorded. Acute rejection was defined via renal biopsy in accordance with Banff criteria. Results: Acute rejection occurred in 16 (44.4%) patients. NLR (median 3.75 vs. 1.99, p=0.001) and PLR (median 125.59 vs. 99.23, p=0.008) values were significantly higher in the acute rejection group than in the control group. Area under the curve was calculated to be 0.822 for NLR and to be 0.759 for PLR. Cut-off values were determined to be >2.5 (75% sensitivity and 75% specificity) for NLR and to be >108 (81% sensitivity and 65% specificity) for PLR. Univariate analysis revealed a strong correlation of acute rejection both with NLR >2.5 (Odds Ratio (OR)=0.267, 95% Confidence Interval (CI)=0.089-0.803, p=0.019) and PLR >108 (OR=0.231, 95% CI=0.066-0.810, p=0.022). Conclusion: In kidney transplant patients, there is a strong relationship between high NLR and PLR values and the development of acute rejection. As simple, easy-to-access, inexpensive and non-invasive methods, PLR, and particularly NLR, may be potential tests to diagnose posttransplant acute rejection. INSET: 212. [ABSTRACT FROM AUTHOR]
