Einleitung: Für die diaetologische Praxis sind standardisierte Assessment- und Outcomeinstrumente für verschiedene gastroenterologische Erkrankungen von Relevanz. Die vorliegende Bachelorarbeit beschäftigt sich mit der Problematik und hat basierend auf den vorhergegangenen Bachelorarbeiten „Grundlagen für die Entwicklung eines standardisierten diaetologischen Assessments für ausgewählte gastroenterologische Erkrankungen” und „Grundlagen für die Entwicklung standardisierter diaetologischer Assessments für Chronisch Entzündliche Darmerkrankungen und Reizdarm” derzeit vorliegende Messinstrumente untersucht. Methodik: Durch eine systematische Literaturrecherche in den Datenbanken Pubmed, Cinahl, Cochrane und PsychINFO wurde nach Assessment- und Outcomeinstrumenten, Messinstrumenten zu kritischen Nährstoffen und Zusatzdiagnosen bzw. -befunden sowie nach Reliabilitäts- und Validitätsstudien zu den Instrumenten gesucht. Alle relevanten Studien wurden nach COSMIN (Consensus-based Standards for the Selection of Health Measurement Instruments) bewertet und je nach Empfehlungsgrad der Studien wurden deren Messinstrumente mit der ICF-Diätetik gelinkt. Ergebnisse: Es wurden insgesamt 18 Assessment- und Outcomeinstrumente zu den gastroenterologischen Erkrankungen Kohlenhydratmalassimilation, Zöliakie, Pankreatitis, Leberzirrhose, Chronisch Entzündlichen Darmerkrankungen und dem Zusatzbefund psychische Belastungen untersucht. Davon entsprachen 17 Messinstrumente den vorgeschriebenen Kriterien, sodass diese nach der ICF-Diätetik gelinkt wurden. Dabei konnten 507 Items der Instrumente nach 181 verschiedenen Kategorien gelinkt werden. Diskussion: Die Ergebnisse der Untersuchungen zeigen die bestehenden Abweichungen und Diskrepanzen der einzelnen Assessment- und Outcomeinstrumente auf. Daraus ergibt sich die Relevanz für die Entwicklung standardisierter Messinstrumente für die gezielte Erhebung von ernährungsrelevanten Informationen zu gastroenterologischen Erkrankungsbildern. Basierend auf der vorliegenden sowie den beiden vorangegangenen Bachelorarbeiten sollen in einer nachfolgenden Bachelorarbeit ICF-Core-Sets für gastroenterologische Erkrankungen entwickelt werden. Methodology: A systematic literature search in the databases Pubmed, Cinahl, Cochrane and PsychINFO was conducted to search for assessment and outcome instruments, measurement instruments on critical nutrients and additional diagnoses or findings, as well as reliability and validity studies on the instruments. All relevant studies were examined and, depending on the assessment, the corresponding measurement instruments were linked according to the ICF-Dietetics. Results: A total of 18 assessment and outcome instruments for the gastroenterological diseases carbohydrate malabsorption, coeliac disease, pancreatitis, liver cirrhosis, inflammatory bowel disease and the finding of psychological burdens were examined. Of these, 17 measurement instruments met the prescribed criteria so that they were linked according to ICF-Dietetics. In the process, 507 items of the instruments could be recorded according to 181 different categories. Discussion: The results of the investigations show the existing deviations and discrepancies of the individual assessment and outcome instruments. This leads to the relevance for the development of standardised measurement instruments for the targeted collection of nutrition-relevant information on gastroenterological disease patterns. Based on this and the two previous bachelor theses, ICF-Core-Sets for gastroenterological diseases are to be developed in a subsequent bachelor thesis.

This article discusses the development of a software and hardware complex for primary diagnostics based on deep machine learning. The process of deep machine learning was carried out, the ANN and SVM algorithms were used. The results of deep machine learning were compared, two variants of data sets for training were collected. This article discusses prediction algorithms for the preliminary diagnosis of gastroenterological diseases. Made analysis of several algorithms such as KNN, Random Forest, DNN, ResNet. Currently, artificial intelligence algorithms are widely used in all industries and give their results. For example, it increases the efficiency of the staff and reduces the human factor. With the help of some algorithms, it is possible to solve the problem of diagnosing several diseases. Classification algorithms are used, with the help of classification, the diagnosis system quickly determines the disease and can help the medical staff quickly make a decision and make an accurate diagnosis for a specific disease. The effectiveness of algorithms in the field of medicine of gastroenterological diseases has been tested and the corresponding results have been obtained. A study was conducted on the composition of human saliva, the characteristics and function of saliva. The parameters of human saliva and how it affects human health have been studied. In this case, an analysis of algorithms for the preliminary diagnosis of a disease of the gastrointestinal tract was made. With the help of saliva, you can pre-diagnose diseases of the gastrointestinal tract. Einleitung: In der momentanen diätologischen Praxis besteht kein einheitliches Diaetologisches Assessment im Fachbereich der Gastroenterologie. Diese Bachelorarbeit baut auf den jeweiligen Resultaten der vorangegangenen Bachelorarbeit „Grundlagen für die Entwicklung eines standardisierten diaetologischen Assessments für ausgewählte gastroenterologische Erkrankungen“ auf und befasst sich mit den Krankheitsbildern der Chronisch Entzündlichen Darmerkrankungen und des Reizdarmsyndroms. Methodik: Alle 36 der vorher erhobenen Assessment- und Outcome-Instrumente wurden auf ihre Validität, sowie Reliabilität nach COSMIN (Consensus-based Standards for the Selection of Health Measurement Instruments) überprüft. Anschließend wurden die passend bewerteten Assessment- und Outcome-Instrumente, sowie die ernährungsrelevanten Gesundheitsinformationen der in der vorangegangen Bachelorarbeit durchgeführten Delphi-Befragung von Expert*innen dem Linking-Prozess unterzogen. Ergebnisse: Die Ergebnisse der qualitativen Bewertung der Assessment- und Outcome-Instrumente mittels COSMIN und die gelinkten ICF-Diätetik-Kategorien wurden tabellarisch dargestellt und zusammengefasst. Aufgrund des Resultats der Qualitätsbewertung konnten insgesamt 22 beurteilte Assessment- und Outcome-Instrumente neben den Ergebnissen der Delphi-Befragung in die ICF-Diätetik gelinkt werden. Diskussion: Zukünftig wären standardisierte Assessment- und Outcome-Instrumente zu den einzelnen Krankheitsbildern im Fachbereich Gastroenterologie (wie Chronisch Entzündliche Darmerkrankungen und Reizdarmsyndrom) von Vorteil. Diese könnten in der Praxis zur leichteren und schnelleren Identifizierung von Ernährungsproblemen und zum Dokumentieren des Erfolgsnachweises im Zuge der diätologischen Behandlung implementiert werden. Darüber hinaus könnte das Berufsfeld aus der Entwicklung von ICF-Diätetik-Core-Sets zu gastrointestinalen Erkrankungen einen Nutzen ziehen, welche den Qualitätsstandard in der diaetologischen Praxis vereinfachen würden. Das effizientere Vorgehen aller in Österreich praktizierenden Diätolog*innen hätte die Professionalisierung der Berufsgruppe und einen wachsenden Stellenwert der Diätologie im gesamten Gesundheits-wesen zur Folge und ist jedenfalls erstrebenswert. Weitere Forschung zu den in dieser Arbeit nicht bearbeiteten Krankheitsbildern muss in den nachfolgenden Jahrgängen betrieben werden, damit eine Grundlage für ein standardisiertes Assessment- und Outcome-Instrument im Fachbereich der Gastroenterologie gelegt werden kann. Introduction: There are no standardized dietetic assessments for the field of gastroenterology in the current dietetic field. This thesis builds on the respective results of the previous thesis “Basic principles for the development of a standardized dietetic assessment for selected gastroenterological diseases“ and deals with the selected gastroenterological clinical pictures of inflammatory bowel disease and irritable bowel disease. Method: In total, all 36 of the previously found assessment and outcome instruments were reviewed for their validity and reliability according to COSMIN (Consensus-based Standards for the Selection of Health Measurement Instruments). Following that the appropriately evaluated assessment and outcome instruments, as well as the nutrition-related health items of the Delphi survey carried out with experts in the field, were subjected to the linking process. Findings: The results of the qualitative evaluation of the assessment and outcome instruments using COSMIN and the used ICF dietetics categories were displayed in charts and summarized in the following work. Based on the findings in the rating, a total of 22 evaluated assessment and outcome instruments and the results of the Delphi survey could be linked to the ICF dietetics. Discussion: Henceforward, standardized assessment and outcome instruments for each individual disease in the field of gastroenterology (such as inflammatory bowel disease and irritable bowel disease) could be of great value. These could be implemented as a custom in active practice to identify alimentary issues more easily and swiftly and to be able to document the success of said dietary treatment. In addition, the professional field could benefit from the development of ICF dietetics core sets on gastrointestinal diseases, which would simplify the quality standard in dietetic practice. This efficient approach of all dieticians practicing in Austria would result in the professionalization of the field and a growing importance of dietetics in the entirety of the health care system and is in any case desirable. Further research on the gastroenterological diseases not dealt with in this thesis must be carried out in the succeeding work from the following years, so that a foundation can be laid for a standardized assessment and outcome instrument in the field of gastroenterology.

Introduction: There are no standardized dietetic assessments for the field of gastroenterology in the current dietetic field. This thesis builds on the respective results of the previous thesis “Basic principles for the development of a standardized dietetic assessment for selected gastroenterological diseases“ and deals with the selected gastroenterological clinical pictures of inflammatory bowel disease and irritable bowel disease. Method: In total, all 36 of the previously found assessment and outcome instruments were reviewed for their validity and reliability according to COSMIN (Consensus-based Standards for the Selection of Health Measurement Instruments). Following that the appropriately evaluated assessment and outcome instruments, as well as the nutrition-related health items of the Delphi survey carried out with experts in the field, were subjected to the linking process. Findings: The results of the qualitative evaluation of the assessment and outcome instruments using COSMIN and the used ICF dietetics categories were displayed in charts and summarized in the following work. Based on the findings in the rating, a total of 22 evaluated assessment and outcome instruments and the results of the Delphi survey could be linked to the ICF dietetics. Discussion: Henceforward, standardized assessment and outcome instruments for each individual disease in the field of gastroenterology (such as inflammatory bowel disease and irritable bowel disease) could be of great value. These could be implemented as a custom in active practice to identify alimentary issues more easily and swiftly and to be able to document the success of said dietary treatment. In addition, the professional field could benefit from the development of ICF dietetics core sets on gastrointestinal diseases, which would simplify the quality standard in dietetic practice. This efficient approach of all dieticians practicing in Austria would result in the professionalization of the field and a growing importance of dietetics in the entirety of the health care system and is in any case desirable. Einleitung: In der momentanen diätologischen Praxis besteht kein einheitliches Diaetolo-gisches Assessment im Fachbereich der Gastroenterologie. Diese Bachelorarbeit baut auf den jeweiligen Resultaten der vorangegangenen Bachelorarbeit „Grundlagen für die Ent-wicklung eines standardisierten diaetologischen Assessments für ausgewählte gastroente-rologische Erkrankungen“ auf und befasst sich mit den Krankheitsbildern der Chronisch Entzündlichen Darmerkrankungen und des Reizdarmsyndroms. Methodik: Alle 36 der vorher erhobenen Assessment- und Outcome-Instrumente wurden auf ihre Validität, sowie Reliabilität nach COSMIN (Consensus-based Standards for the Selection of Health Measurement Instruments) überprüft. Anschließend wurden die pas-send bewerteten Assessment- und Outcome-Instrumente, sowie die ernährungsrelevan-ten Gesundheitsinformationen der in der vorangegangen Bachelorarbeit durchgeführten Delphi-Befragung von Expert*innen dem Linking-Prozess unterzogen. Findings: In total, 48 valid and reliable assessment and outcome instruments concerning diseases such as carbohydrate malassimilation, celiac disease, inflammatory bowel disease, irritable bowel syndrome, pancreatitis and cirrhosis of the liver as well as selected critical nutrients and additional diagnoses could be ascertained. In referral to the Delphi questionnaire, only aspects on which experts reached a consensus of at minimum 60 % were considered relevant to a standardized dietetic assessment. Nutritionally valuable findings of the Delphi questionnaire were depicted tabularly. Discussion: Despite the existence of reliable and valid assessment and outcome instruments, there are no disease specific instruments being used in dietetic practice. A plethora of these instruments require additional research regarding reliability and validity for inclusion into a standardized procedure. Im Zuge dieser Bachelorarbeit werden die Anforderungen von Diaetolog*innen an ein standardisiertes gastroenterologisches Assessment erhoben sowie valide und reliable Assessment- und Outcome-Instrumente zu ausgewählten gastroenterologischen Erkrankungsbildern erfasst. Methodik: Eine systematische Literaturrecherche nach vorhandenen Assessment- und Outcome-Instrumenten und deren Reliabilitäts- und Validitätsstudien wurde in den Datenbanken PubMed, Cinahl, Cochrane und PsycINFO durchgeführt. Die erhobenen Instrumente wurden hinsichtlich ihrer Reliabilität und Validität beschrieben. Darauf aufbauend wurden im Rahmen einer Delphi-Befragung in zwei Runden die Anforderungen von Expert*innen an ein standardisiertes gastroenterologisches Assessment erhoben und nach ihrer Relevanz für die diaetologische Praxis bewertet. Als Expert*innen fungierten zwölf Diaetolog*innen in der ersten und neun Diaetolog*innen in der zweiten Befragungsrunde mit dem Schwerpunkt Gastroenterologie im klinischen Setting in Österreich. Ergebnisse: Es konnten insgesamt 48 valide und reliable Assessment- und Outcome-Instrumente zu den Erkrankungsbildern Kohlenhydratmalassimilationen, Zöliakie, chronisch entzündliche Darmerkrankungen, Reizdarmsyndrom, Pankreatitis und Leberzirrhose sowie zu ausgewählten kritischen Nährstoffen und Zusatzdiagnosen erhoben werden. Anhand der Delphi-Befragungen wurden jene erhobenen Aspekte als relevant für ein standardisiertes diaetologisches Assessment gewertet, die einen Konsens der Expert*innen von mindestens 60 % erreicht haben. Die ernährungsrelevanten Ergebnisse der Delphi-Befragungen wurden tabellarisch dargestellt. Diskussion: Obwohl bereits reliable und valide Assessment- und Outcome-Instrumente existieren, werden in der diaetologischen Praxis noch keine erkrankungsspezifischen Instrumente angewendet. Für eine Vielzahl an Instrumenten besteht weiterer Forschungsbedarf bezüglich der Reliabilität und Validität, um diese in ein standardisiertes Vorgehen inkludieren zu können. Auf Grundlage dieser Arbeit werden in einer weiterführenden Bachelorarbeit die eruierten Assessment- und Outcome-Instrumente nach COSMIN bewertet und die ernährungsrelevanten Aspekte mit der ICF-Diätetik gelinkt. Introduction: In the context of the dietetic process model, the dietetic assessment forms the basis of dietetic action. Currently, there exists no consolidated method among dietetic practice in Austria. Over the course of this thesis, the requirements of dietitians towards a standardized gastroenterological assessment as well as valid and reliable assessment and outcome instruments concerning selected gastroenterological diseases will be ascertained and identified. Method: Systematic literature research concerning existing assessment and outcome instruments including reliability and validity studies were conducted in databases such as PubMed, Cinahl, Cochrane and PsycINFO. The collected instruments were described regarding their reliability and validity. Based on this, the requirements and expectations of experts toward a standardized gastroenterological assessment were determined through a two-round Delphi survey and ranked according to their relevance for dietetic practice. Twelve dietitians participating in the first and nine in the second consultation, who had a focus on gastroenterology in the clinical setting in Austria served as experts during both rounds of questioning. Findings: In total, 48 valid and reliable assessment and outcome instruments concerning diseases such as carbohydrate malassimilation, celiac disease, inflammatory bowel disease, irritable bowel syndrome, pancreatitis and cirrhosis of the liver as well as selected critical nutrients and additional diagnoses could be ascertained. In referral to the Delphi questionnaire, only aspects on which experts reached a consensus of at minimum 60 % were considered relevant to a standardized dietetic assessment. Nutritionally valuable findings of the Delphi questionnaire were depicted tabularly. Discussion: Despite the existence of reliable and valid assessment and outcome instruments, there are no disease specific instruments being used in dietetic practice. A plethora of these instruments require additional research regarding reliability and validity for inclusion into a standardized procedure. In a further bachelor’s thesis based on the present one the assessment and outcome instruments discussed here will be evaluated according to COSMIN and the nutritionally valuable aspects will be linked to ICF-dietetic.

Анемический синдром – наиболее частое внекишечное осложнение у пациентов с патологией желудочно-кишечного тракта (ЖКТ), которое может значимо ухудшить качество жизни. В зависимости от сочетания патогенетических механизмов выделяют железодефицитную, анемию хронических заболеваний, В12-дефицитную и фолиеводефицитную анемию. Другие варианты анемии встречаются реже. Необходимо проведение комплексного лабораторного обследования для установления ведущего фактора в развитии анемии и подбора адекватной терапии. Парентеральные формы препаратов железа и витаминов более предпочтительны для данной категории пациентов из-за более высокой биодоступности, низкого профиля безопасности и минимального негативного влияния на ЖКТ. Препараты эритропоэтина и ингибиторы провоспалительных цитокинов применяются в основном для коррекции анемии хронических заболеваний. Последние хорошо себя показали при терапии пациентов с анемическим синдромом на фоне тяжело протекающих воспалительных заболеваний кишечника. Проводятся различные клинические исследования, направленные на внедрение новых препаратов, корректирующих анемию. Однако к настоящему времени опыт их применения практически отсутствует.В этом обзоре мы постарались объединить и систематизировать накопленную информацию о проблеме анемического синдрома при патологиях желудочно-кишечного тракта., Anemic syndrome is the most frequent extraintestinal complication in patients with gastrointestinal tract pathology, which can significantly impair the quality of life. Depending on the combination of pathogenetic mechanisms, the iron deficiency, anemia of chronic diseases, B12-deficiency, and folate deficiency anemia are distinguished. Other types of anemia are less common. It is necessary to conduct a comprehensive laboratory examination to reveal the leading factor in the development of anemia and select adequate therapy. Parenteral forms of iron and vitamin preparations are more preferable for this category of patients due to their higher bioavailability, low safety profile, and minimal negative effect on the gastrointestinal tract. Erythropoietin preparations and inhibitors of proinflammatory cytokines are used mainly for the correction of anemia of chronic diseases. The latter showed themselves well in the treatment of patients with anemic syndrome on the background of severe inflammatory bowel diseases. Various clinical trials are being conducted to introduce new drugs to correct anemia.
