Purpose of Review: Pregnancy associated breast cancer (PABC) distinguishes itself as a different diagnosis due to its unique presentation, diagnostic timetable, and risk factors. The extended duration of treatment and the unfavorable prognosis linked to PABC can be attributed to the delay in diagnosis and treatment. The delay results from the difficulty in interpreting gravid breast examinations and clinicians' hesitancy to perform imaging and treatment procedures on expectant patients. To prevent unnecessary stress on the patient and to mitigate harm to the fetus, accurate and expeditious imaging is necessary. Recent Findings: As the number of women delaying childbirth continues to increase, it is projected that number of PABC diagnoses will also increase. The most common symptom associated with PABC is a palpable mass. Summary: This review provides a comprehensive discussion of the epidemiology and risk factors, clinical presentation, diagnostic challenges, and imaging evaluation of pregnancy associated breast cancer. Objectives: This study aims to synthesize current knowledge and outcomes related to pediatric auditory brainstem implantation (ABI) in children with severe inner ear malformations (IEMs). It highlights the clinical management practices, challenges, and potential future directions for consensus development in this field. Methods: A systematic review of findings presented at the Third International Pediatric ABI Symposium organized by the Hacettepe Cochlear Implant team between 3 and 5 September 2020 was conducted, incorporating data from 41 departments across 19 countries. Relevant clinical outcomes, imaging techniques, surgical approaches, and rehabilitation strategies were analyzed to identify key trends and variability in practices. Results: The review indicates that children receiving ABIs exhibit diverse auditory outcomes influenced by individual anatomical variations and developmental factors. Early implantation, particularly before the age of three, positively correlates with better auditory and language development. Multicenter experiences underscore the necessity of tailored decision-making, which considers both surgical candidacy and comprehensive rehabilitation resources. Discussion:: The variability in outcomes emphasizes the need for improved consensus and guidelines regarding eligibility, surgical techniques, and multidisciplinary rehabilitation approaches. Notable complications and the necessity for thorough imaging assessments were also identified as critical components affecting clinical decisions. Conclusion: A formal consensus statement is warranted to standardize best practices in ABI management. This will not only enhance patient outcomes but also guide future research efforts to address the remaining challenges in the treatment of children with severe IEMs. Enhanced collaboration among team members will be pivotal in achieving these objectives. In osteopetrosis, refractory jawbone osteomyelitis often occurs because of the progression of bone sclerosis, which makes the bone brittle. Herein, we report a case of refractory osteopetrosis-related maxillary osteomyelitis for which conservative therapy and surgical intervention were performed along with imaging evaluation. A 58-year-old woman had maxillary gingivitis for 2 years, for which her tooth was recently extracted. Subsequently, she developed sequestration and oroantral fistula and visited our department. Panoramic radiography, computed tomography, and magnetic resonance imaging (MRI) scans revealed left maxillary osteomyelitis, sinusitis, and sequestration. She had been diagnosed with osteopetrosis in childhood and exhibited sclerotic changes, which were likely caused by osteopetrosis. Bone scintigraphy using Tc-99m-HMDP revealed no increase in tracer uptake in the jawbone. Finally, the patient was diagnosed with osteopetrosis-related left maxillary osteomyelitis and sinusitis; hence, she underwent hyperbaric oxygen therapy, multiple surgeries, and antibiotic therapy. Hepatic dysfunction occurred during the treatment, and a mediastinal tumor was suspected. Thus, fluorodeoxyglucose-positron emission tomography was performed. Further, the patient was diagnosed with extramedullary hematopoietic foci associated with osteopetrosis and hepatic dysfunction due to hepatitis B, there was no Tc-99m accumulation in the maxilla, and hepatitis was treated concurrently. Six months after the final surgery for osteomyelitis, antibiotic therapy was discontinued. Notably, 2 years have passed without exacerbation of osteomyelitis. Moreover, bone scintigraphy using Tc-99m-HMDP did not reveal increased Tc-99m accumulation in the jawbone. In cases of osteopetrosis-related osteomyelitis, interpretation of findings should be done cautiously because Tc-99m may accumulate in an area with active bone metabolism and osteomyelitis may be masked. Refractory osteomyelitis with osteopetrosis requires careful interpretation of MRI and bone scintigraphy scans, and long-term follow-up is required. [ABSTRACT FROM AUTHOR]

The term congenital lung malformation (CLM) is used to describe a wide range of pathological conditions with different imaging and clinical manifestations. These anomalies stem from abnormal embryological lung development, potentially occurring across various stages of prenatal life. Their natural history can be variable, presenting in a wide range of severity levels and encompassing asymptomatic individuals who remain so until adulthood, as well as those who experience respiratory distress in the neonatal period. Through the PubMed database, we performed an extensive review of the literature in the fields of congenital lung abnormalities, including their diagnostic approach and findings. From our RIS-PACS database, we have selected cases with a final diagnosis of congenital lung malformation. Different diagnostic approaches have been selected, including clinical cases studied using plain radiograph, CT scan, prenatal ultrasound, and MR images. The most encountered anomalies can be classified into three categories: bronchopulmonary anomalies (congenital pulmonary airway malformations (CPAMs), congenital lobar hyperinflation, bronchial atresia, and bronchogenic cysts), vascular anomalies (arteriovenous malformation), and combined lung and vascular anomalies (scimitar syndrome and bronchopulmonary sequestration). CLM causes significant morbidity and mortality; therefore, the recognition of these abnormalities is necessary for optimal prenatal counseling and early peri- and postnatal management. Advances in imaging techniques have allowed an increasing number of multifocal/multicentric breast cancers (MMBC) to be detected at an early stage. Traditionally, MMBC has been considered to have a worse prognosis than unifocal breast cancer(UBC) and should be treated with escalation therapy. With the advancement of medical technology in recent years, the diagnosis, individualized treatment strategies, and the safety of breast-conserving surgery for MMBC need to be re-examined. This paper reviews the epidemiologic features, preoperative imaging assessment, pathological heterogeneity, molecular markers, and surgical approaches of MMBC, so as to deepen the knowledge of clinicians.

Introduction. Pneumonia is a common infectious disease among children, very familiar to pediatricians and a major cause of hospitalization worldwide. Mycoplasma pneumoniae (M. pneumoniae) - atypical pathogen, is estimated to be responsible for approximately 30-40% of community-acquired pneumonias. The aim of the paper was to underline the diagnostic and treatment difficulties in a case of pneumonia with M. pneumoniae in a school-aged boy, with multiple presentations in the Emergency Service for respiratory difficulties. Material and method. We present the case of a 7 years and 10-month-old male patient, admitted to our clinic for wheezing, cough and dyspnea. Results. The clinical exam at admission pointed out influenced general status, wheezing, thoraco-abdominal swing, intercostal draft, sibilant and crackles, oxygen saturation 89%, tachycardia 134 beats/minute. The blood tests revealed mild leukocytosis, with neutrophilia, slightly increased inflammatory biomarkers. Considering the general status and the presence of functional respiratory syndrome, steroid and symptomatic anti-inflammatory treatment is initiated. However, the general condition remains stable, which required the widening of the spectrum of investigations with the performance of a chest CT noticing a pneumonic condensation with an air bronchogram located at the level of the right upper lobe. Considering the stationary respiratory functional syndrome and the radiological appearance of the pneumonia as well as the age, we performed serology for atypical germs and identified a positive titer of Ig M antibodies for M. pneumoniae by instituting Azithromycin po, but after 3 days, the evolution remains stationary, thus we changed the treatment with intravenous Levofloxacin, with a favorable subsequent evolution. Conclusions. Pneumonia with atypical pathogens such as M. pneumoniae is a well-defined and well-known pathology in the literature, but it still remains a condition that imposes multiple difficulties related to the diagnosis and therapeutic management of pediatric cases.

Abstract We successfully removed a rare ectopic pituitary adenoma from a 64-year-old female patient initially misdiagnosed with a chordoma. The tumor was located in the slope region and posed a challenge due to its rarity and location. Using neuro-navigation and neuro-endoscopic techniques, we performed a transnasal transsphenoidal resection and confirmed postoperatively that the tumor was a non-functional pituitary adenoma. Although the patient experienced temporary cortisol deficiency after surgery, she recovered well. To accurately diagnose ectopic pituitary adenomas of clivus, doctors must carefully evaluate the patient's imaging results and hormone levels. If necessary, nuclear medicine scans can also aid in diagnosis. Endoscopic transnasal-transsphenoidal surgery is the preferred treatment method, and preoperative multimodal imaging evaluation and intraoperative neuro-navigation technology are essential for a successful surgery.

Tooth extraction is the most common procedure in dental practice. However, in the long term, it may cause alveolar ridge atrophy. This systematic review aimed to evaluate the role of platelet-rich fibrin (PRF) in post-exodontic alveolar ridge preservation in terms of its effectiveness in the regeneration of bone tissue as assessed by imaging and its efficacy compared to physiological bone healing. The study is presented in accordance with the preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidelines. This systematic review was conducted using electronic databases such as PubMed, Scopus, Web of Science, and Science Direct. The gray literature search was conducted in the New York Academy of Medicine Grey Literature Report. All the studies in this systematic review were randomized controlled trials (RCTs). The risk of bias was performed according to the Cochrane Handbook for Systematic Reviews of Interventions 6.2 (RevMan 6.2). Considering the inclusion and exclusion criteria, we included 17 randomized clinical trials published up to 2022 investigating the efficacy of PRF in post-exodontic bone regeneration. Based on the results of clinical studies, it can be stated that despite not being statistically significant, PRF promotes neoformation and prevents bone loss between three and four months post-extraction. Abstract Background A comparison of changes in the long head of the biceps tendon for different types of rotator cuff tears has not been previously performed. Furthermore, the correlation between the thickening and degeneration of the long head of the biceps tendon and the cause of these changes have not been fully clarified. We evaluated the relationship between degenerative changes in the long head of the biceps tendon and rotator cuff tears in a rat model using imaging and histology. Methods Ninety-six 12-week-old Sprague–Dawley rats were divided into anterior (subscapularis tear), anterosuperior (subscapularis, supraspinatus, and infraspinatus tears), superior (supraspinatus and infraspinatus tears), and control groups. The long head of the biceps tendon was harvested at 4 or 12 weeks postoperatively. The cross-sectional areas of the intra- and extra-capsular components of the tendon were measured using micro-computed tomography, and the affected/normal ratio of the cross-sectional area was calculated. Masson’s trichrome staining and Alcian blue staining were performed for histologic analysis, with degenerative changes described using the modified Bonar scale. The correlation between the affected/normal ratio and Bonar scores was evaluated. Results The affected/normal ratio was higher for the anterior and anterosuperior groups than for the control group at 4 and 12 weeks. The ratio increased for the intra-articular portion in the superior group and for both the intra- and extra-articular portions in the anterior and anterosuperior groups. Degeneration considerably progressed in the anterior and anterosuperior groups compared with the control group from weeks 4 to 12 and was greater in the intra- than in the extra-articular portion. The ratio correlated with extracellular matrix score. Conclusions Subscapularis tears were associated with progressive thickening and degeneration of the long head of the biceps tendon at 4 and 12 weeks postoperatively, which was more significant in the intra- than in the extra-articular portion. Histologic evaluation indicated that the extracellular matrix likely caused these degenerative changes.

Background: Analysis of the tumour necrosis rate is an important method to evaluate the effect of neoadjuvant chemotherapy for osteosarcoma. However, at present, there is no specific imaging method to evaluate this effect. The purpose of this study was to evaluate the changes in blood supply after chemotherapy by measuring the CT enhancement rate and to analyse the correlation between the CT enhancement rate and the tumour necrosis rate. Methods: Patients with primary osteosarcoma of the extremities treated in our institute from 2016 to 2017 were analysed retrospectively. In total, 103 eligible patients were enrolled in the study, including 67 males and 36 females, with an average age of 17.7 (6–54) years. Sixty cases had tumour sites in the femur, 25 in the tibia, 10 in the humerus, and eight in other sites. All patients received neoadjuvant chemotherapy including methotrexate, cisplatin + doxorubicin and ifosfamide before surgical treatment. All patients underwent enhanced CT examination of the same tumour site before and after the neoadjuvant chemotherapy protocol. The CT value before and after chemotherapy was measured and the enhancement rate was calculated. The change in the CT enhancement rate after chemotherapy was analysed. Changes in the CT enhancement rate were compared between patients with a tumour necrosis rate greater than 90% and those with one 90% and 1.62 (median 1.52, range 1.00–2.41) in the group with a necrosis rate 90% and 1.53 (median 1.43, range 1.00–2.83) in the group with a necrosis rate 90% decreased by 29.0% (median 28%, range −2.3–53%) (P

Morphologic Achilles tendon properties obtained via diagnostic ultrasound imaging are valuable in understanding Achilles tendon health and injury. Currently, limited information exists regarding Achilles tendon morphological properties amongst active aging adults based upon Victorian Institute of Sport Assessment (VISA-A) scores. Achilles tendon morphologic properties defined by VISA-A score groupings allow clinicians and researchers to compare data values amongst current patients. Purpose: Comparison of physically active aging adults Achilles tendon morphological properties with various VISA-A scores or a previous Achilles tendon rupture. A convenience sample of 121 participants (71 females, 50 males) at least moderately active and 50 years old, were recruited. Participants completed a VISA-A survey, and assigned groups by scores (Group 1: 90-100, Group 2: 70-89, Group 3: 45-69, Group 4: Previous Achilles tendon tear). Achilles tendon ultrasound imaging occurred at the malleolar line (The apex of the medial and lateral malleolus). Following imaging Achilles tendon cross-sectional area (CSA), thickness, and elastography were measured and analyzed. Participants with a previous Achilles tendon rupture displayed significantly larger tendon CSA and thickness compared with other groups (p<0.05). Individuals with VISA-A scores from 45-69 displayed significantly larger tendon CSA and thickness than participants with scores greater than 90 (p<0.03). No significant differences were noted for elastography between groups (p>0.05). Achilles tendon morphological differences exist based upon pain level in physically active aging adults. Diagnostic ultrasound may be used during assessment and rehabilitation of injured tendon tissue to inform about current tendon tissue properties.

Purpose Numerous studies have applied a variety of methods to assess paraspinal muscle degeneration. However, the methodological differences in imaging evaluation may lead to imprecise or inconsistent results. This article aimed to provide a pragmatic summary review of the current imaging modalities, measurement protocols, and imaging parameters in the evaluation of paraspinal muscle fat infiltration (FI) in MRI studies. Methods Web of Science, EMBASE, and PubMed were searched from January 2005 to March 2020 to identify studies that examined the FI of paraspinal muscles on MRI among patients with lumbar degenerative diseases. Results Intramyocellular lipids measured by magnetic resonance spectroscopy and FI measured by chemical‐shift MRI were both correlated to low back pain and several degenerative lumbar diseases, whereas results on the relationship between FI and degenerative lumbar pathologies using conventional MRI were conflicting. Multi‐segment measurement of FI at the lesion segment and adjacent segments could be a prognostic indicator for lumbar surgery. Most studies adopted the center of the intervertebral disc or endplate as the level of slice to evaluate the FI. Compared with visual semiquantitative assessment, quantitative parameters appeared to be precise for eliminating individual or modality differences. It has been demonstrated that fat CSA/total CSA (based on area) and muscle–fat index (based on signal intensity) as quantitative FI parameters are associated with multiple lumbar diseases and clinical outcomes after surgery. Conclusion Having a good command of the methodology of paraspinal muscle FI on MRI was effective for diagnosis and prognosis in clinical practice.

BACKGROUND: At present, anterior cervical discectomy and fusion is the “gold standard” for treatment of cervical spondylosis. The cervical zero-notch anterior interbody fusion cage is gradually being promoted and applied in clinical practice, with good clinical efficacy and imaging results. OBJECTIVE: To evaluate the mid-term outcome of cervical spondylotic myelopathy treated with variable-angle zero-notch anterior interbody fusion system. METHODS: Totally 81 patients with cervical spondylotic myelopathy treated in First Affiliated Hospital of Guangzhou University of Chinese Medicine from January 2015 to January 2018 were included in this study. Among them, 42 patients received anterior cervical discectomy and decompression and fusion using variable-angle zero-notch anterior interbody fusion system (trial group); 39 patients received anterior cervical discectomy and decompression and fusion using traditional titanium plate-cage system (control group). Visual Analog Scale score, Japanese Orthopaedic Association score, Cobb-C angle and Cobb-S angle of cervical spine, interbody fusion, and complications were compared between the two groups. The protocols were approved by the Ethics Committee of First Affiliated Hospital of Guangzhou University of Chinese Medicine (approval No. JY2020199). RESULTS AND CONCLUSION: (1) Visual Analog Scale score and Japanese Orthopaedic Association score were significantly improved after surgery compared with those before surgery in both groups (P < 0.05). No significant differences in Visual Analog Scale score and Japanese Orthopaedic Association score were detected between the two groups (P > 0.05). (2) Cobb-S angle was significantly improved at 3 days and 30 months after surgery compared with that before surgery in both groups (P < 0.05). Cobb-C angle was significantly improved at 3 days after surgery compared with that before surgery in the trial group (P < 0.05). Cobb-C angle was significantly improved at 3 days and 30 months after surgery compared with that before surgery in the control group (P < 0.05). (3) The rate of complete fusion was 98% in the trial group and 100% in the control group. (4) The rate of the fusion cage sinking in the trial group was higher than that in the control group (24%, 3%, P < 0.05); the incidence of postoperative dysphagia was lower than that in the control group (2%, 38%, P < 0.05); and the incidence of adjacent intervertebral disc degeneration was lower than that in the control group (2%, 18%, P < 0.05). (5) The results have shown that variable-angle zeronotch anterior interbody fusion system for cervical spondylotic myelopathy can effectively improve the symptoms and reduce postoperative complications. However, the incidence of fusion cage sinking was higher than that of traditional titanium plate-cage system. Abstract Background The application of nano-hydroxyapatite/polyamide 66(n-HA/PA66) struts has become reliable in anterior cervical corpectomy and fusion (ACCF) as a source of sufficient segmental stability. This was a retrospective and long-term imaging evaluation of the n-HA/PA66 strut in 1-level ACCF. Methods The patients between June 2006 and December 2014, who underwent 1-level ACCF using an n-HA/PA66 strut, were reviewed. The neurological status was assessed using the Japanese Orthopedic Association (JOA) score and axial pain was evaluated using a Visual Analogue Scale (VAS) score and the radiographic parameters were determined by X-ray and 3-D CT examinations when necessary for the evaluation of bone fusion using the Brantigan scale and imaging characteristics. Results A total of 520 patients underwent one-level ACCF, with a mean follow-up (FU) duration of 72.38 ± 24.56 months. The level of surgery was C4 in 58 cases, C5 in 173 cases, C6 in 208 cases, and C7 in 81 cases. According to the Brantigan scale, on X-ray examination, the bony fusion rate was observed to be 40%, 70%, 93%, and 98% at 3 months, 6 months, 1 year and the final FU. An interesting radiographic appearance of the bone graft growth pattern was classified into three types. 95% of the patients accounted for types a and b. No significant differences were observed in age, hospitalization duration, surgical haemorrhage volume, or fusion rate among the types except in the percentage and sex of the patients among the types. Introduction Imaging plays significant role in diagnosing the male urethral stricture and helps the urologist in determining the treatment protocols. Retrograde urethrography (RGU) had been the standard imaging technique for anterior urethral stricture. Sonourethrography (SUG) as compared to the retrograde urethrography has the advantage of being radiation free. Aim of this study was to compare the sonourethrography findings in anterior urethral stricture with that of retrograde urethrography. Methods This was an observational study done in 46 patients with clinical symptoms related to lower urinary tract during the period of September 2019 to August 2020 after obtaining informed consent. Ethical clearance was received from Institutional Review Committee, Institute of Medicine. Patients were first evaluated by sonourethrography. Then, retrograde urethrography was done as per the standard protocol of the department. Data were collected in the predesignated proforma including the study variables on sonourethrography and retrograde urethrography and entered in IBM SPSS version 20. The lengths of strictures on sonourethrography and retrograde urethrography were compared. Results Among 46 patients 21 patients showed anterior urethral strictures on both SUG and RGU with bulbar region as the commonest location. SUG showed all the urethral strictures seen on RGU with significant positive correlation. SUG also showed spongiofibrosis in nine patients. Conclusion Sonourethrography, an radiation free imaging technique, showed all the strictures seen in retrograde urethrography with added benefit of detection of spongiofibrosis as well. Produced in the liver, Antithrombin III, now simply antithrombin (AT), is a vitamin K-independent serine protease inhibitor in the coagulation pathway. It is the most important primary physiologic inhibitor of thrombin in the human body. In addition to thrombin, AT also inhibits other coagulation serine proteases including VIIa, IXa, Xa, XIa, XIIa. (1-2) The deficiency of AT may be inherited or acquired. The incidence of inherited AT deficiency is about 1:2000-5000 in general population and is the least common of the three main anticoagulant deficiencies (the other two being protein C deficiency and protein S deficiency). (3) Its inheritance is generally in autosomal dominant fashion. The resultant procoagulant state leads to unprovoked recurrent venous thromboses and thromboembolic events such as deep vein thrombosis or pulmonary embolism which generally appear at the post-pubertal period, compared to the very low occurrence in the prepubertal period.(4) The diagnosis is based on both quantitative and qualitative measurement of AT level.(5) The measured AT activity in functional tests in healthy subjects is generally around 80% to 120%, and AT level of less than 70% is considered as being AT deficiency. This evaluation should be made while not on anticoagulation therapy, because heparin decreases AT levels for up to 10 days following its discontinuation and warfarin increases its level. (6) Patients with inherited AT deficiency rarely develops renal failure which may be caused by renal vein thrombosis or glomerular injury associated with fibrin accumulation. (7) In these young patients with end stage renal disease (ESRD), renal transplantation is currently the best therapeutic option to improve the quality of life and to avoid the risk of complications of other renal replacement treatment particularly thrombosis of the hemodialysis access. Kidney transplantation in patients with an occluded iliac veins and inferior vena cava (IVC) is a very challenging surgery to perform.(8-9) Despite the reported success in few cases for children, kidney transplant surgeries with thrombotic diseases in adults remain very limited in the literature. Purpose Numerous studies have applied a variety of methods to assess paraspinal muscle degeneration. However, the methodological differences in imaging evaluation may lead to imprecise or inconsistent results. This article aimed to provide a pragmatic summary review of the current imaging modalities, measurement protocols, and imaging parameters in the evaluation of paraspinal muscle fat infiltration (FI) in MRI studies. Methods: Web of Science, EMBASE, and PubMed were searched from January 2005 to March 2020 to identify studies that examined the FI of paraspinal muscles on MRI among patients with lumbar degenerative diseases. Results: Intramyocellular lipids measured by magnetic resonance spectroscopy and FI measured by chemical‐shift MRI were both correlated to low back pain and several degenerative lumbar diseases, whereas results on the relationship between FI and degenerative lumbar pathologies using conventional MRI were conflicting. Multi‐segment measurement of FI at the lesion segment and adjacent segments could be a prognostic indicator for lumbar surgery. Most studies adopted the center of the intervertebral disc or endplate as the level of slice to evaluate the FI. Compared with visual semiquantitative assessment, quantitative parameters appeared to be precise for eliminating individual or modality differences. It has been demonstrated that fat CSA/total CSA (based on area) and muscle–fat index (based on signal intensity) as quantitative FI parameters are associated with multiple lumbar diseases and clinical outcomes after surgery. Background: Cholangiocarcinoma (CCA) is a relatively rare malignant biliary system tumor, and yet it represents the second most common primary hepatic neoplasm, following hepatocellular carcinoma. Regardless of the type, location, or etiology, the survival prognosis of these tumors remains poor. The only method of cure for CCA is complete surgical resection, but part of patients with complete resection are still subject to local recurrence or distant metastasis. Summary: Over the last several decades, our understanding of the molecular biology of CCA has increased tremendously, diagnostic and evaluative techniques have evolved, and novel therapeutic approaches have been established. Background: New patterns of progression under immune-oncology (IO) antibodies (mAb) have been described such as pseudoprogression. Except for melanoma, variations between studies reveal difficulties to establish their prevalence. Methods: This retrospective study enrolled patients participating in IO phase I trials at Gustave Roussy cancer center for solid tumors excluding melanoma. Radiological assessment according to iRECIST was correlated with prospectively registered patient characteristics and outcomes. Pseudoprogression (PsPD) was defined as RECIST-defined progression followed by stabilization or decrease at the next imaging, and dissociated response (DisR) as concomitant decrease in some tumor lesions and increase in others at a same timepoint. Results: Among 360 patients included, 74% received IO mAb combination: 45% with another IO mAb, 20% with targeted therapy and 10% with radiotherapy. The overall response rate was 19.7%. PsPD were observed in 10 (2.8%) patients and DisR in 12 (3.3%) patients. Atypical responses (AR), including PsPD and DisR, were not associated with any patient's baseline characteristics. Compare with typical responder patients, patients experiencing AR presented a shorter iPFS (HR 0.34; p < 0.001) and OS (HR 0.27; p = 0.026). Among the 203 patients who progressed in 12 weeks, 80 (39.4%) patients were treated beyond progression. PD was confirmed in 80% of cases, while 10% of patients presented a response. Conclusion: Pseudoprogression and dissociated response are uncommon patterns of progression. Their prevalence should be balanced with the rate of real progressing patients treated beyond progression. To date, there is little consensus on the role of pelvic imaging in assessing local disease extent during initial staging in patients with endometrial carcinoma, with practices differing widely across centers. However, when pretreatment assessment of local tumor extent is indicated, MRI is the preferred imaging modality. Preoperative imaging of endometrial carcinoma can define the extent of disease and indicate the need for subspecialist referral in the presence of deep myometrial invasion, cervical extension, or suspected lymphadenopathy. If distant metastatic disease is clinically suspected, preoperative assessment with cross-sectional imaging or PET/CT may be performed. However, most patients with low-grade disease are at low risk of lymph node and distant metastases. Thus, this group may not require a routine pretreatment evaluation for distant metastases. Recurrence rates in patients with endometrial carcinoma are infrequent. Therefore, radiologic evaluation is typically used only to investigate suspicion of recurrent disease due to symptoms or physical examination and not for routine surveillance after treatment. The American College of Radiology Appropriateness Criteria are evidence-based guidelines for specific clinical conditions that are reviewed annually by a multidisciplinary expert panel. BACKGROUND: Percutaneous vertebral body stenting system (VBS) can alleviate patients pain, recover the height of vertebral body, and correct local kyphosis, but there is no definite clinical study to show that it has obvious advantages over percutaneous kyphoplasty (PKP). OBJECTIVE: To compare the short-term effect of VBS versus PKP in the treatment of osteoporotic vertebral compression fracture. METHODS: Forty patients with osteoporotic vertebral compression fracture who received VBS or PKP between January 2017 and December 2018 in the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine were included in this study. They were divided into a VBS group (n=15) and a PKP group (n=25) according to surgery method. According to whether fluoroscopic operation was performed in retroextension position, two subgroups were designated: VBS retroextension group (n=7) and PKP retroextension group (n=14). RESULTS AND CONCLUSION: Compared with the PKP group, the amount of bone cement injected into the vertebra was greater in the VBS group (P < 0.05). There were no significant differences in operation time, intraoperative blood loss, and leakage rate of cement between PKP and VBS groups (P > 0.05). Compared with preoperative situations, Visual Analogue Scale score was significantly decreased after surgery in both VBS and PKP groups, and further decreased at the last follow-up (P < 0.05). The height of anterior edge of the injured vertebra, the height of middle part of the injured vertebra and the ratio of the height of the anterior edge of injured vertebra to adjacent vertebra were significantly increased after surgery in both VBS and PKP groups (P < 0.05). In both VBS and PKP groups, the Cobb angle of vertebral compression was significantly decreased after surgery compared with before surgery (P < 0.05). After surgery, the Cobb angle of vertebral compression in the VBS group was smaller than that in the PKP group (P < 0.05). In the VBS group, local Cobb angle after surgery was significantly decreased compared with that before surgery (P < 0.05). There were no significant differences in operation time, intraoperative blood loss, amount of bone cement, bone cement leakage, cinical efficacy (VAS score), imaging evaluation {height of anterior edge of injured vertebra, height of middle part of injured vertebra, height ratio of anterior edge of injured vertebra to adjacent vertebra, Cobb angle of vertebral compression and local Cobb angle between VBS retroextension and PKP retroextension groups (P > 0.05). These results suggest that VBS and PKP are effective in the treatment of osteoporotic vertebral compression fractures. VBS can better correct compression kyphosis deformity when more bone cement is injected. The difference of therapeutic efficacy between two surgical approaches can be reduced with proper surgical position, keeping the surgical segment in the retroextension position. The coracoid process of the scapula is in close proximity to major neurovascular structures, including the brachial plexus and the axillary artery and vein. In addition, it serves as a major site of attachment for multiple tendons and ligaments about the shoulder. Isolated coracoid fractures are rare; however, they can be easily overlooked on routine shoulder radiographs. Importantly, when these fractures go undiagnosed, they are at high risk for nonunion. In this paper, we will review the relevant anatomy of the coracoid process, classification schemes for coracoid fractures, mechanisms of injury how these fractures typically present, multimodality imaging findings, and associated injuries. Radiological investigation should be an essential step in dental prevention, showing early dental lesions that can be treated early. In our group of patients was investigated the presence of dental and pulp illness with imaging in addition to the clinical examination.It was proved that a laborious initial investigation may highlight pathological processes existing at dento-periodontal structures that are not yet clinically manifest.

Highlights • Electrical resistance tomography (ERT) method is proposed to visually detect the defect in cementitious materials. • A similarity calculation based on color histogram is introduced to evaluate the image from ERT measurement. • The factors effecting on the image quality of ERT is analyzed and discussed. Abstract Electrical resistance tomography (ERT) method is proposed to visually investigated of the defect in cementitious materials. Specimens with one precast defect are made with different water-cement (w/c) ratio and detected using the embedded electrodes. The experimental images are assessed with a similarity evaluation method based on color histogram. In addition, the electrical resistivity and moisture content of the specimens are measured simultaneously to explore its effects on the quality of the imaging results. The results show that the moisture content is the main factor influencing the resistivity of the specimens in the early hydration reaction while the w/c ratio becomes the main factor effecting the resistivity after the early hydration reaction. For the multi-defect detecting, the influence of electrical soft field effect cannot be neglected. Purpose: To assess the predictive value for local tumor progression (LTP) of geometrical tumor coverage using the contrast-enhanced (ce-)CT images acquired before and within 24 h after radiofrequency (RF) ablation.Methods: Twenty patients (6 male and 14 female, median age 62 years) with 45 focal hypovascular liver metastases (16 colorectal carcinoma, 3 melanoma and 1 breast carcinoma) underwent RF ablation under CT-guidance and received a ce-PET/CT scan within 24 h post-procedure. Pre- and post-ablation ce-CT-images were aligned using an interactive procedure and used to verify the tumor coverage of the RF ablation. Results were correlated to LTP as recorded during follow-up performed every 2-3 months after the intervention (mean follow-up of 110 weeks) and compared to standard reading performed by three readers of the ce-CT images.Results: Eleven tumors (25%) showed LTP during the follow-up period. One lesion, which did not show LTP, was excluded from analysis due to the poor quality of the alignment. For the remaining, 29 (66%) tumors were completely covered by the ablation zone, 9 (20%) were not, and for 6 (14%) tumors the edges coincided with the edge of the ablation zone. The sensitivity, specificity, PPV and NPV for LTP of having incomplete tumor coverage or no apparent ablative margin versus standard reading of ce-CT were 100, 88, 73 and 100% versus 42, 88, 58 and 82%, respectively.Conclusions: Verifying the tumor coverage of liver metastases by an ablation zone through alignment of pre- and early post-ablation ce-CT images has a high predictive value for LTP. The greater tuberosity is an important anatomic structure and its integrity is important for shoulder abduction and external rotation. Isolated fractures of the greater tuberosity are often subtle and may not be detected on initial radiographs. Clinically, these patients display symptoms which mimic a full thickness rotator cuff tear. It is important to differentiate these two entities, as their treatment is different (typically nonsurgical management for minimally displaced fractures versus rotator cuff repair for acute full thickness rotator cuff tears). When greater tuberosity fractures are significantly displaced and allowed to heal without anatomic reduction, they can lead to impingement. This article will review greater tuberosity anatomy and function, as well as the clinical presentation and multimodality imaging findings of greater tuberosity fractures. • The changes in the tumor blood supply represented by the CT enhancement rate is a new evaluation method of the chemotherapy effect on osteosarcoma. • The blood supply changes based on CT evaluation are related with tumor necrosis rate. • The evaluation based on CT enhancement rate can be a reference for evaluating the chemotherapy effect of osteosarcoma. Analysis of the tumour necrosis rate is an important method to evaluate the effect of neoadjuvant chemotherapy for osteosarcoma. However, at present, there is no specific imaging method to evaluate this effect. The purpose of this study was to evaluate the changes in blood supply after chemotherapy by measuring the CT enhancement rate and to analyse the correlation between the CT enhancement rate and the tumour necrosis rate. Patients with primary osteosarcoma of the extremities treated in our institute from 2016 to 2017 were analysed retrospectively. In total, 103 eligible patients were enrolled in the study, including 67 males and 36 females, with an average age of 17.7 (6–54) years. Sixty cases had tumour sites in the femur, 25 in the tibia, 10 in the humerus, and eight in other sites. All patients received neoadjuvant chemotherapy including methotrexate, cisplatin + doxorubicin and ifosfamide before surgical treatment. All patients underwent enhanced CT examination of the same tumour site before and after the neoadjuvant chemotherapy protocol. The CT value before and after chemotherapy was measured and the enhancement rate was calculated. The change in the CT enhancement rate after chemotherapy was analysed. Changes in the CT enhancement rate were compared between patients with a tumour necrosis rate greater than 90% and those with one <90%. The average CT enhancement rates before and after chemotherapy were 1.68 (median 1.63, range 1.00–2.51) and 1.39 (median 1.28, range 1.00–2.83), respectively (P < 0.01). The average CT enhancement rate after chemotherapy decreased by 15.0% (median 16.7%, range −27.5–53%): 75 cases exhibited a decrease, three cases remained unchanged, and 25 cases exhibited an increase. The average enhancement rate before chemotherapy was 1.75 (median 1.68, range 1.18–2.51) in the group with a necrosis rate >90% and 1.62 (median 1.52, range 1.00–2.41) in the group with a necrosis rate < 90% (P = 0.068). The average CT enhancement rate after chemotherapy was 1.20 (median 1.21, range 1.00–1.53) in the group with a necrosis rate > 90% and 1.53 (median 1.43, range 1.00–2.83) in the group with a necrosis rate < 90% (P < 0.01). The enhancement rate of the group with a necrosis rate > 90% decreased by 29.0% (median 28%, range −2.3–53%) (P < 0.01); the enhancement rate of the group with a necrosis rate < 90% decreased by 3.8% on average (median 0.7%, range −27.5–44.5%) (P = 0.225). After receiving neoadjuvant chemotherapy, most patients with osteosarcoma of the extremities exhibited reductions in the CT enhancement rate. In cases where the tumour necrosis rate was greater than 90%, the tumour blood supply was significantly reduced. Background Immune checkpoint inhibitors are an important tool in the therapeutic strategy against metastatic non–small cell lung cancer (NSCLC); however, radiological evaluation is challenging due to the emergence of atypical patterns of responses. Several evaluation criteria have been proposed, such as the Response Evaluation Criteria in Solid Tumours (RECIST), version 1.1, immune -related RECIST (irRECIST) and iRECIST, but have not been systematically compared in a homogeneous population. Patients and methods We conducted a monocentric retrospective analysis of consecutive advanced NSCLC patients treated with an anti–programmed cell death-1 or anti–program death-ligand 1. Response patterns and the discordance between RECIST 1.1, irRECIST and iRECIST guidelines were described, and associations of response patterns and clinical outcome were explored. Results Overall, 160 patients treated between February 2013 and October 2016 were included. Atypical responses were observed in 20 patients (13%), including eight pseudoprogressions (PsPDs) (5%) and 12 dissociated responses (8%). Thirteen of the 20 patients demonstrated clinical benefit. Per the RECIST 1.1, 37 patients (23%) showed an objective response or stable disease, and 123 patients (77%) exhibited progression. Eighty progressive patients were assessable for irRECIST and iRECIST: 15 patients were assessed differently; however, only three (3.8%) mismatches with a theoretical impact on the therapeutic decision were identified. Patients with PsPD or dissociated response had higher overall survival than patients with true progression. Conclusion Atypical responses (PsPD/dissociated response) occurred in 13% of NSCLC patients under immune checkpoint inhibitors. Based on survival analyses, the RECIST 1.1 evaluation underestimated the benefit of immune checkpoint inhibitors in 11% of the progressive patients. The purpose of this study was to evaluate the available bone of the maxillary premolar area. Materials and methods. 31 patients, 10 men and 21 women, aged 40-70 years, with edentulous maxillary premolar area were selected for the study. A qualitative and quantitative analyze was performed using cone beam computed tomography (CBCT) in this patients. Results. Regarding the bone quantity, the patients were divided as follows: 26 (83.87%) had (subantral class – SA) 1, and the remaining, 5 (16.13%), had (subantral class – SA) 2. Regarding the bone quality, D3 type was found in 46 cases (74.19%), D4 type was found in 12 cases (19.35%), and D2 type was found in 4 cases (6.46%). Conclusions. The article offers information on the different types of diagnostic imaging used in the assessment of non-vascular mediastinal masses in children and infants in the U.S. Most often, non-vascular mediastinal masses are found in infants and children. Imaging plays an important role in patient management and correct diagnosis of the ailment. Different types of imaging modalities are available for the purpose including chest radiographs (CXR), ultrasound (US), and computed tomography (CT).

Background: Thrombolysis remains the only proven therapy to benefit acute ischemic stroke (AIS) patients. Recent studies have introduced more sensitive outcome measures such as the shift analysis to detect a treatment effect in AIS trials and are also including imaging as a surrogate of injury. Methods: We conducted a cross-sectional, internet-based survey of academic neurologists regarding their attitudes, choices and understanding of various outcome measures in clinical trial design for AIS. The survey population consisted of neurologists who specialize in the care of stroke patients and are on faculty at university-affiliated hospitals in the USA. Results: 152 of 300 neurologists completed the survey. There were 79% men and 21% women. Among commonly used outcome scales in acute stroke, the most frequent ones selected for use as trial primary endpoints were the global statistic (59%), modified Rankin scale (mRS) (52%), and NIHSS (30%). When given choices about which outcome on the mRS would justify a therapeutic intervention, 54% chose a shift analysis of change in the distribution of outcomes and 39% chose a dichotomized outcome (mRS ≤2). A majority of respondents favored health transition states of 4–3, 3–2 and 2–1 on the mRS as clinically worthwhile. Only 2% of the respondents thought that a single transition point on the mRS was clinically meaningful. However, 20% of the respondents did not understand the shift analysis. In addition, nearly two thirds of the respondents believed that the presence of a mismatch on brain imaging is relevant to the success of neuroprotective agents. Conclusion: The majority of respondents accepted an analysis of the entire distribution of the mRS scores as an appropriate endpoint analytic technique in AIS trials and did not require the traditional dichotomized outcome to demonstrate a treatment effect; however, a better understanding of the shift strategy is needed. Congenital lung malformations are a spectrum of developmental anomalies comprised of malformations of the lung parenchyma, airways, and vasculature. Imaging assessment plays a pivotal role in the initial diagnosis, management, and follow-up evaluation of congenital lung malformations in the pediatric population. However, there is currently a lack of practical imaging guidelines and recommendations for the diagnostic imaging assessment of congenital lung malformations in infants and children. Objective: To summarize the guiding role of imaging evaluation of oblique lumbar interbody fusion (OLIF) in recent years., Methods: The reports of OLIF surgical imaging research at home and abroad in recent years were extensively reviewed and analyzed., Results: Preoperative imaging evaluation plays an important role in guiding the operation of OLIF, the placement of fusion Cage, the selection of indications, and the reduction of complications., Conclusion: Detailed preoperative imaging evaluation can correctly estimate the indications of OLIF, and avoid the nerve, blood vessel, and muscle injuries.

In the management of epithelial ovarian cancers, imaging is used for cancer detection and staging, both before and after initial treatment. The decision of whether to pursue initial cytoreductive surgery for ovarian cancer depends in part on accurate staging. Contrast-enhanced CT of the abdomen and pelvis (and chest where indicated) is the current imaging modality of choice for the initial staging evaluation of ovarian cancer. Fluorine-18-2-fluoro-2-deoxy-d-glucose PET/CT and MRI may be appropriate for problem-solving purposes, particularly when lesions are present on CT but considered indeterminate. In patients who achieve remission, clinical suspicion for relapse after treatment prompts imaging evaluation for recurrence. Contrast-enhanced CT is the modality of choice to assess the extent of recurrent disease, and fluorine-18-2-fluoro-2-deoxy-d-glucose PET/CT is also usually appropriate, as small metastatic foci may be identified. If imaging or clinical examination confirms a recurrence, the extent of disease and timing of disease recurrence then determines the choice of treatments, including surgery, chemotherapy, and radiation therapy. The American College of Radiology Appropriateness Criteria are evidence-based guidelines for specific clinical conditions that are reviewed annually by a multidisciplinary expert panel. The guideline development and revision include an extensive analysis of current medical literature from peer reviewed journals and the application of well-established methodologies (RAND/UCLA Appropriateness Method and Grading of Recommendations Assessment, Development, and Evaluation or GRADE) to rate the appropriateness of imaging and treatment procedures for specific clinical scenarios.
