A 20-year-old pregnant woman with late-diagnosed atrial septal defect and pulmonary arterial hypertension (PAH) exhibited lack of efficacy during treatment with bosentan. After starting iloprost inhalation therapy, she showed functional improvement, but her pregnancy was terminated due to fetal distress. Following surgery, she resumed bosentan treatment for PAH, but unfortunately experienced severe hypoxia leading to cardiorespiratory arrest and ultimately passed away, indicating lack of efficacy to bosentan therapy.
