This is one of the most often misunderstood facts by first-year students in probability theory and statistics. The first proof is concise using the moment generating function. We previously reported unusual adenomas with proliferative zones confined to the lower two-thirds of the crypt. The proliferative zones of colorectal adenomas have three patterns: 'lower,' 'superficial' and 'entire'. This study aimed to clarify the characteristics of each adenoma pattern. We investigated 2925 consecutive patients who underwent colonoscopy at our institute. All polyps that were removed were histologically examined using hematoxylin and eosin staining. The location of the proliferative zone was assessed for adenomas. Data were compared using Dunn's and Kruskal–Wallis tests. Colorectal adenomas with 'lower' proliferative zone often appeared similar to hyperplastic polyps (42.8%), and the frequency was significantly higher than that of adenomas with 'superficial' and 'entire' proliferative zones (p < 0.001). The mean sizes of adenomas were 2.4, 3.0 and 3.9 mm for 'lower,' 'superficial' and 'entire' proliferative zones, respectively. A significant gradual increase was observed. Regarding morphology, the proportion of type 0–I in adenomas with an 'entire' proliferative zone was significantly higher than that in adenomas with 'superficial' proliferative zone (p < 0.001). 4. Ply curving termination for enhancing tensile strength of composite ply drop-off: static evaluation and failure mechanism.

- Abstract
Composite structures are optimized in shape by terminating specific plies gradually. However, stress concentration at the ply drop-off can cause interlaminar delamination from the ply edge. In this study, Ply Curving Termination (PCT), which is introduced by locally curving fibers at 0° ply edges, is used to enhance tensile strength of the ply drop-off. To clarify the dependence of the PCT strength on the curved fiber angle (PCT angle), static tensile tests using ply drop-off specimens are conducted, followed by detailed damage observations using X-ray CT. The results indicates that the strength significantly increases with PCT but the fracture behavior differs depending on the PCT angle. Interestingly, specimens with large PCT angles fail from the position where the fibers curve, away from the ply edge. This PCT-specific failure mechanism is then elucidated using finite element analysis and the virtual crack closure technique, indicating that shear lag caused by the stiffness change due to fiber curving is important. Finally, using the revealed PCT failure mechanism, PCT angles that are the most effective in enhancing tensile strength is discussed. [ABSTRACT FROM AUTHOR]

5. Meningeal Carcinomatosis Presenting with Bilateral Loss of Vision.

Meningeal carcinomatosis (MC) has an extremely poor prognosis and can present with various neurological symptoms. A 68-year-old man presented to our hospital with a 1 month history of headache and nausea followed by sudden decrease in vision in both eyes. Whilst being examined in the ophthalmology department he lost consciousness and had a generalised tonic clonic seizure. Accordingly, he was transferred to the Emergency Department. Head magnetic resonance imaging showed hydrocephalus. Abdominal contrast-enhanced computed tomography scanning reported changes suggestive of gastric carcinoma. Cerebrospinal fluid cytological examination showed numerous atypical cells, leading to the diagnosis of MC. An upper gastrointestinal endoscopy revealed an advanced gastric tumour. Systemic chemotherapy was initiated, however, he died within 16 days of admission. At autopsy, poorly differentiated adenocarcinoma was identified in the subarachnoid space, however it had not invaded the brain parenchyma or optic chiasm. This is the first report of loss of vision being the first presenting symptom of new-onset gastric carcinoma with MC. Although rare, MC should be suspected in cases where patients present with sudden loss of vision and symptoms of meningeal irritation, where there are no ophthalmological findings to explain the vision loss. 6. Kinetic modeling of IG-110 oxidation in inert atmosphere with low oxygen concentration for innovative high-temperature gas-cooled reactor applications.

The oxidation behavior of IG-110, a graphite core component, was investigated at temperatures ranging from 400 to 1000°C in a 10 ppm Ar/O
2 flow to simulate the oxidation process between the graphite core component and helium coolant with low O2 concentrations employed in advanced High-Temperature Gas-cooled Reactors (HTGRs). The results reveal that IG-110 undergoes significant mass loss at temperatures above 700°C, resulting in total mass changes of −1.5%, −5.3%, and −9.0% at 700, 800, and 1000°C, respectively, during a 10-hour oxidation period. No significant mass loss is observed below 600°C. To understand the oxidation mechanism of IG-110 under low O2 concentrations, we propose a kinetic model as the current chemical kinetic-controlled model does not fully explain the oxidation behavior observed in our research. Our analysis shows lower estimated reaction rates compared to studies at higher O2 concentrations; the activation energy values exhibit good agreement. The proposed kinetic model sheds light on the oxidation mechanism of IG-110 under 10 ppm Ar/O2 flow. This study provides new insights into the oxidation behavior of graphite core components in HTGRs and highlights the importance of controlling the O2 concentration in the helium coolant to prevent severe degradation of SiC-matrix fuel compacts. A detailed computational fluid dynamics code named JAEA Utility Program for Interdisciplinary Thermal-hydraulics Engineering and Research (JUPITER) for the thermal-hydraulics around the beam window (BW) of accelerator-driven system (ADS) was used to confirm the validity of the thermal-hydraulics design tool based on the ANSYS Fluent. The Fluent uses the Reynolds-averaged Navier–Stokes (RANS) model and can quickly calculate the turbulent flow around the BW as a BW design tool. First, the results of JUPITER were compared with the experimental results using a mock-up BW system in water to confirm the validity of JUPITER. This study confirmed that the numerical results are in good agreement with the experimental results. Thus, JUPITER could be used as a benchmark code. A benchmark simulation for the Fluent calculation was also performed using validated JUPITER to demonstrate the applicability of JUPITER as an alternative for experiments. Therefore, the mean values around the BW agreed with each other (e.g. the mean velocity profile for the stream and horizontal directions). Therefore, results confirmed that JUPITER demonstrated a good performance in validating the thermal-hydraulics design tool as a fluid dynamics solver. The purpose of this study was to compare the characteristics of testosterone polylactic-co-glycolic (PLGA) microspheres prepared by a paddle mixer or microfluidics device. The comparison was conducted by not only in vitro evaluation but also in vivo evaluation which has not been reported up to date. We discovered that, among the steps in microsphere preparation, the solvent removal process strongly impacted drug content, particle size and surface morphology. Spectroscopic measurements suggested that molecular interactions and crystallinity of the drug incorporated in the microspheres differed. For the drug release profile, although both mixer- and microfluidics-prepared samples showed similar sustained release of the incorporated drug for approximately one month in vitro, they exhibited different plasma concentration profiles in vivo. Together, our findings show that the preparation process, especially the solvent removal process, may affect the physicochemical characteristics of testosterone PLGA microspheres, leading to different in vivo performance. The complement component C5a contributes to the recruitment of immune cells to inflamed tissues and local inflammation. The proinflammatory cytokine interleukin (IL)-1β is also related to inflammatory disorders through inflammasome activation. However, the association between inflammasome activation and C5a is unclear. Human peripheral blood mononuclear cells (PBMCs) were stimulated with C5a and measured for IL-1β secretion by enzyme-linked immunosorbent assay (ELISA). The pro-IL-1β expression in cell lysates was also examined by Western blot analysis. Similarly, magnetic bead-isolated CD14
+ monocyte-depleted and lymphocyte-depleted PBMCs were stimulated with C5a, and immunoblot analysis was performed using an anti-cleaved-IL-1β (p17) antibody. FACS was performed to detect caspase-1-activated cells. C5a-stimulated PBMCs produced IL-1β in C5a concentration-dependent manner. The protein levels of pro-IL-1β in the cell lysates were significantly increased. Furthermore, the cleaved-IL-1β (p17) was faintly detected in the same lysates. Active caspase-1 was demonstrated in C5a-simulated CD14+ monocytes by FACS. Cleaved-IL-1β (p17) was demonstrated in the supernatant of C5a-stimulated PBMCs. Lymphocyte-depleted PBMCs stimulated with C5a but monocyte-depleted PBMCs produced cleaved-IL-1β (p17). C5a induced the production of mature IL-1β in PBMCs. The IL-1β production is mediated mainly by caspase-1 activation in CD14+ monocytes. These results suggest that C5a alone potentiates mature IL-1β production mainly in monocytes. Adequate conditioning results from various physical, environmental, and psychological factors in sports activities. In this study, we aimed to clarify the concurrence and relevance of injuries, psychological problems, and sleep disturbance in university football (soccer) players. Biomechanical characteristics and risk factors for those injuries were also investigated. Overall, 1,084 university football players participated in this cross-sectional study using a web-based health history questionnaire. Assessments were made via the presence of injuries for physical problems, the Athlete Psychological Strain Questionnaire (APSQ) for psychological problems, and the Athlete Sleep Screening Questionnaire (ASSQ) for sleep difficulties. There were 200 (19%) athletes with injuries; 413 (38%) of these were APSQ-positive, 246 (23%) were ASSQ-positive, and 468 (43%) athletes had no physical, psychological, or sleep problems. APSQ-positive respondents were significantly more likely to have a current injury than those who were APSQ-negative. No significant difference was observed between the number of ASSQ-positive and -negative respondents and current injuries. Those with sleep problems were significantly more likely to have psychological problems. More than 60% of university football players had one or more physical, psychological, or sleep problems. 12. Sustainable Cultural Preservation in Central Asia – Involving Local Communities as Caretakers of Archaeological Sites.

- Abstract
This article presents cultural conservation projects in Iran and Iraq, in each of which the social context of the archaeological site and the collection were also considered, in order to develop long-term conservation planning. The objective of one project is to improve the water supply for the local village population near an ancient salt mine in Iran in order to retain them as stewards of the archaeological site. Another project shows how digital media and different narrative perspectives are intended to appeal to different target groups in Iran and worldwide to raise awareness of the so-called salt mummies recovered from the ancient salt mine. International experts are also addressed via online formats to discuss the challenges of the ethical handling of human remains for conservation and exhibition. In Kurdistan (Iraq), young archaeology students are introduced to the topics of archaeological heritage management in order to recruit them as future caretakers. What all projects have in common is that relationships and cross-sector networks are being established to build a crisis-proof local community that takes care of its cultural heritage. In this way, projects can be implemented that are primarily carried out by local actors, thereby building up expertise that will endure. These projects are the initial attempts to link cultural preservation and social well-being and thus to make a contribution to sustainable development in line with the UN's 2030 Agenda for Sustainable Development. A reflexive referentially depends on another NP. Long-standing inquiries in sentence processing literature revolve around whether and how forming this coreference relation during online sentence processing is susceptible to similarity-based interference. To address these questions, we conducted four experiments using L-maze tasks. The results revealed (delayed) interference in ungrammatical sentences, indicating facilitatory interference. We also found that structural information associated with a distractor and related to clause finiteness may influence memory retrieval. Interference effects were not observed in grammatical sentences across all experiments. These results suggest that online reflexive resolution may be susceptible to interference only when the reflexive mismatches its structurally accessible antecedent in features, and that structural information may influence how a distractor interferes with memory retrieval during online sentence processing. With the emergence of molecular targeted agents and immunotherapies in anti-cancer treatment, a concept of optimal biological dose (OBD), accounting for efficacy and toxicity in the framework of dose-finding, has been widely introduced into phase I oncology clinical trials. Various model-assisted designs with dose-escalation rules based jointly on toxicity and efficacy are now available to establish the OBD, where the OBD is generally selected at the end of the trial using all toxicity and efficacy data obtained from the entire cohort. Several measures to select the OBD and multiple methods to estimate the efficacy probability have been developed for the OBD selection, leading to many options in practice; however, their comparative performance is still uncertain, and practitioners need to take special care of which approaches would be the best for their applications. Therefore, we conducted a comprehensive simulation study to demonstrate the operating characteristics of the OBD selection approaches. The simulation study revealed key features of utility functions measuring the toxicity-efficacy trade-off and suggested that the measure used to select the OBD could vary depending on the choice of the dose-escalation procedure. The objectives of this study were to retrospectively investigate the patient characteristics, treatment patterns, healthcare resource utilization (HCRU), and healthcare costs related to management of neurofibromatosis type 1 (NF1) in Japan. Cohorts of NF1 patients with or without plexiform neurofibromas (PN) were identified from the Medical Data Vision database in 2008–2019. Baseline characteristics, NF1 medications, HCRU, and associated costs were assessed using descriptive statistics. All-cause HCRU and costs following the first confirmed NF1 diagnosis date were analyzed per patient per year (PPPY) in Japanese Yen (JPY) and United States Dollar (USD). A total of 4394 NF1 patients without PN and 370 NF1 patients with PN were identified. The mean age was 35.0 and 36.9 years, respectively. The proportion of patients with PN treated with medications was higher than that in patients without PN (except for antirheumatic/immunologic agents). Analgesics/non-steroidal anti-inflammatory drugs were the most frequently prescribed NF1 medications (44.3% and 56.0% in patients without and with PN, respectively), followed by inpatient prescriptions of opioids/opioid-like agents (17.8% and 27.6%, respectively). Inpatient admissions accounted for the highest costs in both cohorts with the average cost PPPY being JPY 2,133,277 (USD 19,861) for patients without PN and JPY 1,052,868 (USD 9802) for patients with PN. NF1 is treated primarily with supportive care with analgesics/non-steroidal anti-inflammatory drugs being the most frequently prescribed NF1 medications in Japan. Findings underscored the unmet need and substantial economic burden among patients with NF1 and highlighted the need for new treatment options for patients with this disease. [ABSTRACT FROM AUTHOR]

Objective: To compare the performance of the abdominal handheld point-of-care ultrasonography (POcus) Butterfly-iQ to gold standard transvaginal ultrasonography (Us) in identifying the position of intrauterine devices (lUDs) in the hands of a medical doctor specialised in ultrasonography. Methods: in this diagnostic accuracy study, a single operator conducted abdominal POCUS followed by conventional transvaginal US. Seventy patients utilising copper or hormonal iUDs were assessed between June 2021 and October 2022. iUDs were categorised as entirely within the uterine cavity or malpositioned. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy were calculated for detecting malpositioned iUDs, with conventional US results serving as the reference standard. Concordance rate and Kappa coefficient were computed to assess the agreement between the two ultrasound modalities. Results: among the 70 patients, 46 (65.7%) used copper iUDs, and 24 (34.3%) used hormonal iUDs. Conventional transvaginal US showed iUDs entirely within the uterine cavity in 56 (80%) patients and 14 (20%) iUDs were malpositioned. Of the 14 malpositioned iUDs seen by conventional US, POCUS identified 13 demonstrating a sensitivity of 92.9% (66.1-99.8). Of the 56 iUDs entirely within the uterine cavity shown by conventional US, only two cases were considered malpositioned by POCUS demonstrating a specificity of 96.4% (87.7-99.6). The concordance rate was 95.7%, and the Kappa value was 0.87 in differentiating between iUDs entirely within the uterine cavity and those that were malpositioned. Conclusion: Abdominal POCUS using Butterfly-iQ, when administered by an imaging specialist, exhibited excellent performance in confirming iUDs entirely within the uterine cavity. 17. Oxytetracycline resistance of Melissococcus plutonius strains in Japan.

- Abstract
European foulbrood caused by the Gram-positive lanceolate coccus, Melissococcus plutonius, is a major bacterial infection of honey bee brood. Oxytetracycline (OTC) has been used to combat this disease in some countries. Although OTC resistance was not previously reported despite its long-term use, the first case was finally detected in an M. plutonius strain isolated in Canada in 2019. In Japan, OTC has not been approved for use in the control of any honey bee diseases. However, after testing 77 M. plutonius strains isolated in Japan between the 1980s and 2010s, we identified four OTC-resistant strains with a minimum inhibitory concentration of OTC on agar medium of 16 µg/ml. The four strains were isolated in 2008 (1 strain), 2013 (1 strain), and 2015 (2 strains); i.e., before the Canadian OTC-resistant isolate. These results provide insights into the mechanisms underlying the development of OTC-resistant M. plutonius. [ABSTRACT FROM AUTHOR]

18. Improving fatigue characteristics of ply drop-off in composite laminate with ply curving termination.

- Abstract
Ply drop-off is a weak point of a tapered composite laminate, where stress concentration occurs and delamination initiates. By suppressing delamination from the ply drop-off, more optimized structural design and further weight reduction are possible. Ply Curving Termination (PCT), which locally changes the fiber direction at the edge of the dropped ply, is one of the promising delamination suppression mechanisms. To improve the technical maturity of PCT, this study conducted tensile fatigue tests using asymmetric taper configuration. Damage initiation, delamination propagation, and final failure were evaluated in detail by changing the PCT fiber angle and loading level, confirming that large PCT angles are effective in improving the fatigue properties of tapered laminates and that controlling the location of initial damage is necessary to maximize the effectiveness of PCT. The proliferative zone of colonic adenomas is confined to the upper third of the crypt or is scattered along its entire axis. In contrast, there are unusual adenomas with proliferative zones confined to the lower two-thirds of the crypt. We investigated the frequency and endoscopic features of adenomas with lower proliferative zones. We retrospectively reviewed consecutive patients who underwent colonoscopies between September 2022 and March 2023 at the Toyoshima Endoscopy Clinic. Colorectal polyps were endoscopically assessed using the Japan Narrow-Band Imaging Expert Team (JNET) classification. All resected polyps were histologically examined, and the proliferative zone locations were assessed in the adenomas. The frequency of adenomas with a lower proliferative zone was 1.8% (44/2420) in adenomas. Among these adenomas, JNET type 1 incidence was 43.2% (19/44), which was significantly higher than that in adenomas with a normal proliferative zone. Adenomas with a lower proliferative zone were diminutive (mean size: 2.5 mm) and prone to develop in the proximal colon. Colonic adenomas with proliferative zones confined to the lower two-thirds of the crypt often appear as diminutive, hyperplastic polyps. [ABSTRACT FROM AUTHOR]

The conventional markers for hepatocellular carcinoma (HCC), α-fetoprotein (AFP) and des-γ-carboxy prothrombin (DCP), have several limitations; both have low sensitivity in patients with early-stage HCC; low sensitivity for AFP with HCC after eliminating hepatitis C virus (HCV); low specificity for DCP in patients with non-viral HCC, which is increasing worldwide; low specificity for AFP in patients with liver injury; and low specificity for DCP in patients treated with warfarin. To overcome these issues, the identification of novel biomarkers is an unmet need. This study aimed to assess the usefulness of serum protein kinase C delta (PKCδ) for detecting these HCCs. PKCδ levels were measured using a sandwich enzyme-linked immunosorbent assay in 363 chronic liver disease (CLD) patients with and without HCC. In both viral and non-viral CLD, PKCδ can detect HCCs with high sensitivity and specificity, particularly in the very early stages. Notably, the value and sensitivity of PKCδ were not modified by HCV elimination status. Liver injury and warfarin administration, which are known to cause false-positive results for conventional markers, did not modify PKCδ levels. Subthreshold depression impairs young people's quality of life and places them at greater risk of developing major depression. Cognitive behavioral therapy (CBT) is an evidence-based approach for addressing such depressive states. This study identified subtypes of university students with subthreshold depression and revealed discrete profiles of five CBT skills: self-monitoring, cognitive restructuring, behavioral activation, assertive communication, and problem solving. Using data from the Healthy Campus Trial (registration number: UMINCTR-000031307), a hierarchical clustering analysis categorized 1,080 students into three clusters: Reflective Low-skilled, Non-reflective High-skilled, and Non-reflective Low-skilled students. Non-reflective Low-skilled students were significantly more depressed than other students (p <.001). The severity of depression seemed to be related to the combination of self-monitoring skills and other CBT skills. Considering the high prevalence of poor self-monitoring skills in persons with autism, the most severe depression was observed in the significant association between Non-reflective Low-skilled students and autistic traits (p =.008). These findings suggest that subthreshold depression can be categorized into three subtypes based on CBT skill profiles. Posterior expansion of the utricular endolymphatic space (ES) is a finding occasionally observed, and often occurs in bilateral ears. The clinical significance of posterior expansion of the utricular ES is not clear. To investigate the clinical significance of posterior expansion of the utricular ES detected on magnetic resonance imaging (MRI). Among 957 patients who underwent contrast-enhanced MRI to investigate the presence of endolymphatic hydrops (EH), clinical findings in cases with posterior expansion of the utricular ES were evaluated and the significance of their existence is considered. Posterior expansion of the utricular ES was detected in 30 ears from 20 cases (7 males, 13 females; mean age 48.7 years). The ears with posterior expansion had a significantly lower incidence of vestibular EH than those without, and this tendency was more prominent in cases in bilateral ears. Some cases with posterior expansion had vestibular symptoms and were diagnosed with Ménière's disease, although EH was not detected in their vestibules. Posterior expansion of the utricular ES was associated with Ménière's disease without vestibular EH. Children and youth from refugee backgrounds have complex language experiences owing to their journeys away from their homelands often taking them through multiple contexts. This study was motivated by a desire to better understand the language resources of students newly arrived as refugees in Australia and their embodied and contextualised experiences of their languages. Language portraits are a tool used to generate insights into language experiences. For this study, the language portrait tool was extended beyond the body to include contexts within which languages are experienced (school, home, the digital domain, other). Participants were 41 secondary students, who arrived as refugees from South Asia and the Middle East and were either enrolled in, or recent graduates of, an intensive English language programme. Findings were derived from (1) coding of languages according to embodied experience and context and (2) thematic analysis of students' transcribed discussion. Patterns across the cohort indicated that these students primarily experience their thinking as monolingual in their L1. However, the coding of sensory zones indicates that multilingualism is experienced as a multisensory flow of viewing, listening, speaking, eating and otherwise experiencing. Left upper lobectomy (LUL) with left superior pulmonary vein (LSPV) resection alters the left atrium (LA) physiological states and LA hemodynamics associated with thrombosis, although this underlying mechanism is poorly understood. Therefore, we investigated the effects of LSPV resection and associated LA physiological changes on LA hemodynamics using four-dimensional computed tomography image-based computational simulations. Three cases were considered: the LA before and after LUL extracted from computed tomography images and artificial LSPV resection without physiological changes. Particle flowability can be improved by admixing with particles smaller than the original main particles. However, the effects of the admixed particle coating structure for the main particle surface on the improvement of flowability are not fully understood. In this study, we investigated the effects of coating structures prepared under various mixing strength conditions (compression force and rotational speed) on particle compaction flowability. The main and admixed particles for various mass ratios were mixed in various mixing strength conditions and the compacted packing fractions of the mixed particles were measured. SEM images and SAXS were used to analyze the surface coating ratio, coating diameter, and fractal dimension of the admixed particles on the main particle surfaces. The difference in fractal dimension calculated from the SAXS data and the coating diameter ranges of the admixed particle agglomerates obtained from SEM images showed that the improvement mainly depended on coating structures in agglomerates 30–60 nm in size of the admixed particles. An optimal coating structure for improving the compaction flowability was an average gap between agglomerates on the main particle surfaces of around 2–3 times the coating diameter. The effects obtained from resistance training depend on the exercise range of motion (ROM) performed. We aimed to examine the acute effects of different exercise ROM resistance training on the plantar flexor muscles. Eighteen healthy untrained male adults participated in three conditions: calf raises in 1) partial condition [final (short muscle length) partial ROM], 2) full condition (full ROM), and 3) control condition. The ankle dorsiflexion (DF) ROM, passive torque at DF ROM, passive stiffness of muscle-tendon unit, and maximal voluntary isometric contraction (MVC-ISO) torque were measured before and immediately after the interventions. There were significant increases in DF ROM, passive torque at DF ROM, and a decrease in MVC-ISO, but no significant interaction in passive stiffness. Post hoc test, DF ROM demonstrated moderate magnitude increases in the full condition compared to the partial (p = 0.023, d = 0.74) and control (p = 0.003, d = 0.71) conditions. Passive torque at DF ROM also showed moderate magnitude increases in the full condition compared to the control condition (p = 0.016, d = 0.69). MVC-ISO had a moderate magnitude decrease in the full condition compared to the control condition (p = 0.018, d=−0.53). Resistance training in the full ROM acutely increases joint ROM to a greater extent than final partial ROM, most likely due to stretch tolerance. [ABSTRACT FROM AUTHOR]

In this study, we investigate the influence of dislocation strengthening in the metastable parent phase on the deformation-induced martensitic transformation behavior of a face-centered cubic (fcc) Co
20 Cr20 Fe34 Mn20 Ni6 high-entropy alloy. Annealed and hot-swaged specimens were prepared. In-situ neutron diffraction experiments captured an accelerated transformation kinetics in the hot-swaged specimen due to accumulated dislocations. The phase-specific macrostress development showed that the hexagonal close-packed $ \rvarepsilon $ ε -martensite predominantly accommodated the macroscopic plastic deformation of the annealed counterpart. Conversely, the matrix dislocation strengthening promoted cooperative plasticity of the $ \rgamma $ γ -matrix and the $ \rvarepsilon $ ε -martensitic phase, thus enhancing yield stress while preserving ductility. This paper firstly uncovers the role and significance of matrix dislocation strengthening on the mechanical behavior of metastable high-entropy alloys via in-situ neutron diffraction experiments. IgG4-related disease (IgG4-RD) is an immune-mediated systemic fibroinflammatory condition characterized by serum IgG4 elevation and IgG4-positive plasma cell infiltration into various organs. It generally occurs in elderly males. Pediatric cases have been reported, albeit rarely, accordingly lack of recognition of such cases could delay therapeutic intervention leading to poorer outcomes. The present review is a descriptive review of all published case reports, cohort studies, and reviews of pediatric IgG4-RD listed in PubMed. Characteristics of pediatric IgG4-RD were clarified, including sex, organ involvement, serological and histological findings, and treatment. We assessed how many published cases met current classification and comprehensive diagnostic criteria. The characteristics of pediatricIgG4-RD differed from adult IgG4-RD in terms of sex and involved organs. There was no clear male dominance in numbers of cases, and surface organ involvement such as ophthalmic diseases were more common in the pediatric IgG4-RD. Organ involvement tended to be indolent and unilateral, causing difficulty in definitively diagnosing pediatric IgG4-RD. Only about 20% of published cases met IgG4-RD classification or comprehensive diagnostic criteria. Physicians should be careful in diagnosing pediatric IgG4-RD after excluding mimickers. International collaboration toward high-quality evidence to support diagnosis and treatment of pediatric IgG4-RD is advised. [ABSTRACT FROM AUTHOR]

Various complications of arterial embolisation (AE) for postpartum haemorrhage (PPH) are reported. Endometritis (EM) frequently causes abscesses, increasing hysterectomy risk. However, risk factors for EM after AE for PPH are unclear. We explored these risk factors. We included patients who underwent AE for PPH in our hospital from 2005 to 2020 and compared those who did (EM group) and did not develop EM after AE (non-EM group) in a case-control study. Twenty patients met the study criteria; eight patients (40%) had EM. There were no differences in risk factors between groups involved in infection, such as premature rupture of membranes. However, the contrast medium extravasation rate on computed tomography scans before the AE procedure was significantly higher in the EM group (p=.019) compared to the non-EM group. The greatest EM risk factor was contrast medium extravasation before AE for PPH, determined by classification and regression tree modelling (relative risk: 4.5). What is already known on this subject? Reportedly, the clinical success rate of arterial embolisation (AE) for critical haemorrhage in obstetrics is high, around 90%. However, information regarding AE complications is limited. Endometritis is one of these complications, which not only causes prolonged hospitalisation but may also require further treatment, such as hysterectomy. However, the incidence rate and risk factors for EM remain unknown. What do the results of this study add? In this study, 40.0% of patients developed EM after AE for PPH. Extravasation of contrast medium was the top risk factor (relative risk: 4.5 compared to those without EM, p=.019). The second-leading risk factor was a bleeding volume greater than 2500 mL (relative risk: 4.5 compared to those without EM, p=.019). What are the implications of these findings for future clinical practice and/or future research? We created an EM prediction model using extravasation and a bleeding volume greater than 2500 mL. The model was 87.5% sensitive and 66.7% specific. This prediction model allows for the early detection and treatment of EM by recognising high-risk patients and providing intensive postpartum management. [ABSTRACT FROM AUTHOR]

Large bone defects that occur after resection of calvarial tumours are commonly remedied using titanium meshes or bone prostheses. However, these methods have several problems. While intraoperative extracorporeal radiotherapy for bone flaps could avoid these problems, there have been only a few reports wherein meningiomas were treated with 120Gy irradiation. Moreover, no reports are available on calvarial metastasis of sarcoma, and the therapeutic radiation dose remains uncertain. Here, we report a case of giant calvarial metastasis of myxoid liposarcoma treated with intraoperative extracorporeal radiotherapy at a dose of 50Gy. Improving the damage tolerance and reliability of ceramic artificial bone materials, such as sintered bodies of hydroxyapatite (HAp), that remain in vivo for long periods of time is of utmost importance. However, the intrinsic brittleness and low damage tolerance of ceramics make this challenging. This paper reports the synthesis of highly damage tolerant calcium phosphate-based materials with a bioinspired design for novel artificial bones. The heat treatment of isophthalate ion-containing octacalcium phosphate compacts in a nitrogen atmosphere at 1000°C for 24 h produced an HAp/β-tricalcium phosphate/pyrolytic carbon composite with a brick-and-mortar structure (similar to that of the nacreous layer). This composite exhibited excellent damage tolerance, with no brittle fracture upon nailing, likely attributable to the specific mechanical properties derived from its unique microstructure. Its maximum bending stress, maximum bending strain, Young's modulus, and Vickers hardness were 11.7 MPa, 2.8 × 10
‒ 2 , 5.3 GPa, and 11.7 kgf/mm2 , respectively. The material exhibited a lower Young's modulus and higher fracture strain than that of HAp-sintered bodies and sintered-body samples prepared from pure octacalcium phosphate compacts. Additionally, the apatite-forming ability of the obtained material was confirmed in vitro, using a simulated body fluid. The proposed bioinspired material design could enable the fabrication of highly damage tolerant artificial bones that remain in vivo for long durations of time. 34. Factors affecting the radius of curvature of circular braids.

Marudai is a tool for making traditional braids, Kumihimo. It can produce braids of various cross-sectional shapes. Most of the braids are straight braids, but in a previous study, a circular braid was manufactured for the first time using a traditional technique called Marugenji-gumi. In this study, the same manufacturing method was applied to other Marudai techniques and the effectiveness of this method was verified. In addition, the factors affecting the radius of curvature were elucidated by comparing the structure of the braids produced from each technique. As the results of examining the unit mass, flexural rigidity, cross-sectional shape, and crimp curvature, it was found that there are two forces that promote and interfere with curvature, thus affecting the radius of curvature. This insight makes it possible to predict and design the presence or absence of curving, or a large or small radius of curvature, depending on the characteristics of each technique. 2 O2 and analysed using a human microarray, which revealed that heme oxygenase 1 (HMOX1), glutamate-cysteine ligase, early growth response 1, nuclear receptor subfamily 4 group A member 3 (NR4A3) and jun B proto-oncogene were upregulated, whereas superoxide dismutase 1 and catalase were not. Expression of NR4A3 rapidly increased after H2 O2 addition, and the 1.1B4 cells treated with siRNA targeting NR4A3 became sensitive to H2 O2 ; further, HMOX1 expression was strongly inhibited, suggesting that NR4A3 is an oxidative stress-responsive transcription factor that functions through HMOX1 expression in pancreatic islet β-cells. A decreased body mass index (BMI) over time is associated with a poor prognosis for patients on hemodialysis. We aimed to examine whether this association also applies to patients with peritoneal dialysis (PD). BMI change was defined as the percentage change in the BMI between the time of PD catheter insertion and six months after its insertion. The association between the BMI change and all-cause mortality or PD discontinuation from six months after PD catheter insertion until October 2021 was investigated. This retrospective cohort study included 122 patients (aged 61.1 ± 12.1 years; 90 males) who underwent PD catheter insertion between January 2008 and March 2020. The median follow-up period was 43.1 (21.2–78.8) months. The median six-month percentage change in the BMI was −2.14 (−5.56–1.84)%, and patients were categorized into tertiles based on their BMI changes. The fully-adjusted Cox regression analysis revealed a significantly higher rate of PD discontinuation or all-cause mortality (hazard ratio (HR): 2.48; 95%; confidence interval (CI): 1.41–4.37) in patients with the lowest tertile (T1, BMI change: < −4.13%) compared to patients with the middle tertile (T2, BMI change: −4.13%–0.67%). The risk was not significantly higher in patients with the highest tertile (T3, BMI change: >0.67%) than those in the T2 group (HR: 1.18; 95% CI: 0.66–2.11). A decreased BMI over time is independently associated with HD transfer or all-cause mortality among patients initiating PD, which highlights the importance of the 6-month BMI change as a novel prognostic marker. Many college students experience daytime sleepiness. Daytime sleepiness is reportedly associated with poor academic performance, suicide, depression, and traffic accidents. This parallel-group randomized controlled trial study aimed to examine the effect of self-care using transcutaneous vagus nerve stimulation (taVNS) in college students with sleepiness and depression. Thirty-four college students (age 21.5 ± 0.1 years, 21 men and 13 women) were assigned to a control or taVNS group. For taVNS, an electro-stimulator was used to deliver 100 Hz pulses at an intensity below each participant's pain threshold (3.0–8.0 mV) for 30 min. taVNS was performed 30 min before bedtime every day for 28 days. Daytime sleepiness, depression, study engagement, and sleep quality were measured using the Japanese version of the Epworth Sleepiness Scale (ESS-J), Japanese version of the Beck Depression Inventory-II (BDI-II-J), Japanese version of the Utrecht Work Engagement Scale for Students (UWES-S-J), Japanese version of the Pittsburgh Sleep Quality Index (PSQI-J), and Silmee™ Bar Type Lite. Autonomic nervous system activities were measured using high-frequency (HF) and low-frequency (LF) components of heart rate variability. Parasympathetic and sympathetic nervous system activities were indicated by the HF value and LF/HF ratio, respectively. The change in each group from day 28 to day 1 was calculated and analysed. The ESS-J, BDI-II-J, UWES-S-J, and PSQI-J (sleep quality and daytime dysfunction) scores improved, and the LF/HF ratio decreased in the taVNS group compared with the control group. Self-care using taVNS suppresses the sympathetic nervous system during sleep, which may improve sleep quality, daytime sleepiness, depression, and study engagement. [ABSTRACT FROM AUTHOR]

Thousands of glitter particles are potentially daily released into aquatic ecosystems, but their effects on the ecological processes of aquatic species remain unknown. The metallic coating of glitter could potentially reflect light and interfere with photosynthesis, so this study analysed the photosynthetic rates of the macrophyte
Egeria densa in the presence and absence of 0.04 g/L glitter using the light and dark bottle method. We collected 400 individuals ofE. densa and separated them into four equal-sized treatment groups (with and without glitter under iluminated and dark conditions). We found that the presence of glitter reduced the rates of net photosynthesis (P N) and gross photosynthesis (P G) ofE. densa by 34.61%, and 29.65%, respectively, compared with the control treatment without glitter, and also decreased the light intensity inside the bottles. These results suggest that the metal coating of glitter particles attenuates light penetration and increases water reflectance, wich interferes with underwater radiation and light absorption for photosynthetic processes. One of the defining properties of human language is the abundance of potentially unbounded dependencies between elements in a sentence. And yet, how speakers formulate dependencies in sentence production is still poorly understood. Here we examine the timing of verb planning in sentences involving across-the-board and parasitic gap constructions. Using a new task we call the Sentence-Word Interference task, we show that speakers plan the verb of a secondary clause before sentence onset, but selectively when producing across-the-board sentences and not when producing parasitic gap sentences. Based on this timing contrast, we argue that speakers plan verbs predominantly before the production of their dependents, but only when verbs and their dependents engage in both conceptual and direct syntactic relationships. More broadly, the current study suggests that sentence planning is constrained by syntactic relationships that are not reducible to conceptual relationships or to surface word order. This retrospective cross-sectional study aimed to investigate the association between autonomic parameters measured using the Kiritsu-Meijin device and visual-field defects in patients with open-angle glaucoma. A total of 79 eyes of 42 patients with open-angle glaucoma were enrolled in this study. Kiritsu-Meijin testing comprised three phases: sitting, standing, and sitting again (2 min, 2 min, and 1 min, respectively). Continuous electrocardiograms were recorded for five minutes. Autonomic parameters were extracted from the resulting data and analyzed, including activity, balance, reaction, switchover, and recovery; these are five representative parameters derived from Kiritsu-Meijin testing. Correlations between these parameters and mean deviation from Humphrey visual field testing were determined. Additionally, we used a linear mixed-effects model to observe sectoral differences in the relationship between total deviation and the Kiritsu-Meijin parameters. In this study, we focused on superior, central, and inferior total deviations. Significant positive correlations were observed between activity, balance, and recovery and mean deviation values (β = 0.29–0.38, p <.05). The β value between activity and inferior total deviation was higher than that between activity and superior total deviation (β = 0.22, p <.05). Balance did not show any sectoral differences (p >.05). Recovery was more strongly associated with central to inferior total deviation than superior total deviation (β = 0.17–0.25, p <.05). Our findings suggest that in patients with open-angle glaucoma, lower activity and recovery are associated with more severe central and/or inferior visual field defects in the superior quadrant. These results imply that measurements of autonomic function made with the Kiritsu-Meijin device may have clinical utility in the management of glaucoma. [ABSTRACT FROM AUTHOR]

Anaplastic lymphoma kinase (ALK) gene rearrangement is detected in approximately 3–5% of non-small cell lung cancer (NSCLC) cases. Tyrosine kinase inhibitors (TKIs) targeting ALK rearrangement (ALK–TKIs) have shown significant efficacy and improved the survival of patients with NSCLC exhibiting ALK rearrangement. However, almost all patients exhibit disease progression during TKI therapy owing to resistance acquired through various molecular mechanisms, including both ALK-dependent and ALK-independent. Here, we review the mechanisms underlying resistance to second-generation ALK–TKIs, and the clinical management strategies following resistance in patients with ALK rearrangement-positive NSCLC. Treatment strategies following the failure of second-generation ALK–TKIs failure should be based on resistant mechanisms. For patients with ALK mutations who exhibit resistance to second-generation ALK–TKIs, lorlatinib is the primary treatment option. However, the identification of resistance profiles of second-generation ALK–TKIs can aid in the selection of an appropriate treatment strategy. In cases of ALK-dependent resistance mutations, lorlatinib could be the first choice as it exhibits the broadest coverage of mutations that lead to resistance against second-generation ALK–TKIs, such as G1202R, and L1196M. In cases of no resistance mutations, atezolizumab, bevacizumab, and platinum-based chemotherapy could be the alternative treatment options. [ABSTRACT FROM AUTHOR]

42. Influence of atherosclerotic burden on adult patients with ischemic moyamoya disease: combined analysis of two prospective cohorts.

This study aimed to determine the influence of atherosclerotic risk factors on initial and further cerebrovascular events in adult patients with moyamoya disease (MMD) by combined analysis of two prospective cohorts in which patients received pharmacotherapy alone and were prospectively followed-up for 5 years. In 71 patients, smoking status, home blood pressure, hemoglobin (Hb)A1c and low-density lipoprotein cholesterol (LDL-chol) were checked at inclusion and at further cerebrovascular event or at the end of 5-year follow-up. When a patient had daily smoking, increased HbA1c, increased LDL-chol, increased systolic blood pressure, or increased diastolic blood pressure, the patient was categorized as showing atherosclerotic burden. Angiographic disease progression was determined using changes on magnetic resonance angiography. Eleven patients showed angiographic disease progression and seven of these 11 patients experienced further cerebrovascular events during the follow-up period. The remaining 60 patients did not exhibit either condition. At inclusion, the incidence of atherosclerotic burden was significantly greater in patients without angiographic disease progression (80%) than in those with such progression (45%; p = 0.0249). For patients without angiographic disease progression, values or incidence of almost all variables showed significant interval decreases at the end of 5-year follow-up (p < 0.05). Adult patients with ischemic MMD who do not exhibit angiographic disease progression appear more strongly affected by atherosclerotic burden at the initial onset of cerebrovascular events than those exhibiting angiographic disease progression. A reduction in atherosclerotic burden by medical treatments for the former patients prevents further cerebrovascular events. [ABSTRACT FROM AUTHOR]

On behalf of all the authors, I am pleased to share our third annual review on drug transporter science with an emphasis on articles published and deemed influential in signifying drug transporters' role in drug disposition in the year 2022. As the drug transporter field is rapidly evolving several key findings were noted including promising endogenous biomarkers, rhythmic activity, IVIVE approaches in transporter-mediated clearance, new modality interaction, and transporter effect on gut microbiome. As identified previously (Chothe et Cal. 2021, 2022) the goal of this review is to highlight key findings without a comprehensive overview of each article and to this end, each coauthor independently selected 1–3 peer-reviewed articles published or available online in the year 2022 (Table 1). Each article is summarized in synopsis and commentary with unbiased viewpoints by each coauthor. We strongly encourage readers to consult original articles for specifics of the study. Finally, I would like to thank all coauthors for their continued support in writing this annual review on drug transporters and invite anyone interested in contributing to future versions of this review. Many models for predicting various disease prognoses have achieved high performance without laboratory test results. However, whether laboratory test results can improve performance remains unclear. This study aimed to investigate whether laboratory test results improve the model performance for coronavirus disease 2019 (COVID-19). Prediction models were developed using data from the electronic healthcare record database in Japan. Patients aged ≥18 years hospitalized for COVID-19 after February 11, 2020, were included. Their age, sex, comorbidities, laboratory test results, and number of days from February 11, 2020, were collected. We developed a logistic regression, XGBOOST, random forest, and neural network analysis and compared the performance with and without laboratory test results. The performance of predicting in-hospital death was evaluated using the area under the curve (AUC). Data from 8,288 hospitalized patients (females, 46.5%) were analyzed. The median patient age was 71 years. A total of 6,630 patients were included in the training dataset, and 312 (4.7%) died. In the logistic regression model, the area under the curve was 0.88 (95% confidence interval [CI] = 0.83–0.93) and 0.75 (95% CI = 0.68–0.81) with and without laboratory test results, respectively. The performance was not fundamentally different between the model types, and the laboratory test results improved the performance in all cases. The variables useful for prediction were blood urea nitrogen, albumin, and lactate dehydrogenase. Laboratory test results, such as blood urea nitrogen, albumin, and lactate dehydrogenase levels, along with background information, helped estimate the prognosis of patients hospitalized for COVID-19. Constructive studies on symbol emergence systems seek to investigate computational models that can better explain human language evolution, the creation of symbol systems, and the construction of internal representations. Specifically, emergent communication aims to formulate a computational model that enables agents to build efficient sign systems and internal representations. This study provides a new model for emergent communication, which is based on a probabilistic generative model (PGM) instead of a discriminative model based on deep reinforcement learning. We define the Metropolis-Hastings (MH) naming game by generalizing previously proposed models. It is not a referential game with explicit feedback, as assumed by many emergent communication studies. Instead, it is a game based on joint attention without explicit feedback. Mathematically, the MH naming game is proved to be a type of MH algorithm for an integrative PGM that combines two agents that play the naming game. From this viewpoint, symbol emergence is regarded as decentralized Bayesian inference, and semiotic communication is regarded as inter-personal cross-modal inference. This notion leads to the collective predictive coding hypothesis regarding language evolution and, in general, the emergence of symbols. We also propose the inter-Gaussian mixture model (GMM)+ variational autoencoder (VAE), a deep generative model for emergent communication based on the MH naming game. In this model, two agents create internal representations and categories and share signs (i.e. names of objects) from raw visual images observed from different viewpoints. The model has been validated on MNIST and Fruits 360 datasets. Experimental findings demonstrate that categories are formed from real images observed by agents, and signs are correctly shared across agents by successfully utilizing both of the observations of agents via the MH naming game. Furthermore, scholars verified that visual images were recalled from signs uttered by agents. Notably, emergent communication without supervision and reward feedback improved the performance of the unsupervised representation learning of agents. [ABSTRACT FROM AUTHOR]

The coexistence of vascular malformations in the conus medullaris and cauda equina has been rarely reported, and the complex angioarchitecture in multiple arteriovenous lesions remains poorly understood. A 17-year-old woman presented with a sudden-onset, stepwise worsening of weakness and pain in the bilateral legs. Angiography revealed conus medullaris arteriovenous malformation and cauda equina arteriovenous fistulas. One of the drainers was shared between the coexisting lesions and harboured a varix. Targeted embolisation of a fistulous point in the conus lesion was performed with precaution to prevent occluding the common drainage route, which led to symptom improvement with angiographical diminishment of the varix. Recognising that communications between drainers can be observed in multiple spinal arteriovenous lesions is important in facilitating a safe embolisation. Cautious assessment of angiogram with fusion images of cone-beam computed tomography and volumetric T2 magnetic resonance imaging can help in establishing the diagnosis and treatment strategy. Efficient locomotion in terrestrial vertebrates relies on intra-limb coordination that emerges through interaction with the environment and passive interlocking of the musculoskeletal system. These structures have been well investigated in some mammals and birds, but the presence of similar structures in other terrestrial vertebrates has been under appreciated. This study addresses the functionality of the passive interlocking system in reptilian hindlimb locomotion by dissecting Crocodylus porosus and constructing a robot. Based on the dissection, we hypothesized that the passive interlocking mechanism consisted of multiple muscles to maintain the semi-erect limb posture and support its weight. The mechanism is provided by the caudifemoralis longus muscle, along with its tendon and the gastrocnemius externus, acting as a passive element that interacts with the ground reaction force. Accordingly, the feasibility of the interlocking function was demonstrated by a robot implementing the hindlimb and pelvic musculoskeletal system of crocodilians. This study provides new insights into the locomotion mechanism of crocodilians and emphasizes the importance of passive interlocking in efficient locomotion in terrestrial vertebrates. 49. Long-term speech perception performance in prelingually deafened adult cochlear implant recipients.

To investigate the postoperative long-term outcomes after an average of 9.2 years following cochlear implantation (CI) in prelingually deafened adults, along with preimplantation factors predicting postoperative outcomes. Twenty-six prelingually deafened adults who underwent CI at >18 years were compared with those who had undergone CI in childhood (<9 years) and were >10 years old. Outcome measures includedhearing thresholds, preoperative and postoperative aided hearing level (HL), speech discrimination score (SDS), and Categories of Auditory Performance (CAP) scores. Correlation analyses were performed on the following: SDS results, aided HL, school attendant status, implant manufacturers, and speech processor models. Improvement was achieved in the aided HL and SDS results, although these results were not better than those of the child group. CAP score was also statistically significantly improved after CI. Statistically significant correlation between the preoperative SDS and postoperative HL with CI results was observed. In other words, the better the preoperative SDS results, the better the postoperative SDS results. Prelingually deafened adults achieved considerable improvement through CI. It is important to understand that patients achieving better hearing with a well-fitted hearing aid and good SDS performance before surgery may be good candidates for CI. [ABSTRACT FROM AUTHOR]

We investigated potential predictive factors for visual prognosis in Japanese patients with endogenous endophthalmitis. Retrospective observational multicenter cohort study. We examined the characteristics of 77 Japanese patients with endogenous endophthalmitis and performed statistical analyses of these real-world data. The primary endpoint was the identification of factors associated with visual prognosis. We examined differences between patients in the better vision and legal blindness groups at 12 weeks after treatment initiation. The five risk factors for visual impairment at 12 weeks after treatment initiation were presence of pressure injuries, severe clinical symptoms (presence of eye pain and ciliary injection), pathogen identification, and poor best-corrected visual acuity at baseline. Staphylococcus aureus and fungus were associated with a better visual impairment outcome. Endogenous endophthalmitis remains a severe ocular infection; however, it can be managed with rapid treatments, as well as other advances in medical knowledge and technology. [ABSTRACT FROM AUTHOR]
