El objetivo de este documento es actualizar las recomendaciones sobre la evaluación y el manejo de la afectación renal en pacientes con infección por el VIH del año 2010. La función renal debe monitorizarse en todos los pacientes e incluir la medida de la concentración sérica de creatinina, la estimación del filtrado glomerular (ecuación CKD-EPI), la medida del cociente proteína/creatinina en orina y un sedimento urinario. El estudio básico de la función tubular ha de incluir la concentración sérica de fosfato y la tira reactiva de orina (glucosuria). En ausencia de alteraciones, el cribado será anual. En pacientes tratados con tenofovir o con factores de riesgo para el desarrollo de enfermedad renal crónica (ERC), se recomienda una evaluación más frecuente. Se debe evitar el uso de antirretrovirales potencialmente nefrotóxicos en pacientes con ERC o factores de riesgo para evitar su progresión. También se revisan las indicaciones de la biopsia renal, cuándo enviar el paciente al nefrólogo y las indicaciones, evaluación y manejo del paciente en diálisis o del trasplante renal.

Digital, En el transcurrir de los procesos educativos las herramientas digitales y la tecnología han cambiado y revolucionado algunas de las actividades y costumbres arraigadas, que en algunos casos no fomentan el avance de los aprendizajes. Todas y cada una de las herramientas digitales han permitido el acercamiento un poco más en la mayoría de los ámbitos educativos y sociales. Generando diversas alternativas para la comunicación y nuevas formas de aprendizaje. Teniendo en cuenta los grandes cambios a nivel global, las herramientas digitales y tecnológicas son de gran ayuda para potenciar nuevos conocimientos y generar nuevas estrategias educativas. El contenido de este proyecto se fundamenta en las formas como se puede dar a conocer y explorar el arte y la pintura colombiana del maestro Fernando Botero, por medio de las nuevas tecnologías y herramientas digitales, como una new media art, desarrollada en un libro digital interactivo arte, tecnología y artística, para fortalecer el área de artística que permite a los estudiantes visualizar infinitas posibilidades y apropiarse de aprendizajes significativos. El libro digital o new media es un instrumento que permite a los estudiantes aprender de arte, de forma amena e interesante, por medio de actividades interactivas que se encuentran enfocadas en este caso a la temática de pintura, del artista Fernando Botero, debido a las anteriores afirmaciones se puede expresar que el arte es cultura y creatividad, la tecnología es innovación y crecimiento, la pintura es la expresión del ser y artística es desarrollo de habilidades, todas permiten la apropiación y crecimiento de nuevos aprendizajes dentro y fuera del aula, afianzando valores para ser mejores personas, In the course of educational processes, digital tools and technology have changed and revolutionized some of the activities and ingrained customs, which in some cases do not promote the advancement of learning. Digital, Esta investigación tuvo como objetivo principal implementar una estrategia pedagógica para fortalecer las competencias científicas en el área de las ciencias naturales, apoyada en New Media Art, en los estudiantes de grado quinto de la I.E. Daniel Alfonso Paz, basada en un diseño metodológico Cuasi- experimental, con una ruta investigativa mixta, de la cual existe una exposición, una respuesta y una hipótesis para contrastar, pero no hay aleatorización de los sujetos o grupos. Se inició a partir de una prueba diagnóstica para reconocer la realidad del fenómeno de estudio, sobre las falencias en los aprendizajes de las competencias científicas; para lo que se contó con una muestra intencionada probabilista de 30 estudiantes, acudiendo al requerimiento del diseño Cuasi- experimental, el cual no permite la aleatoriedad. El diagnóstico realizado permitió identificar los aprendizajes de pertinencia para el diseño de la estrategia pedagógica, que permitiría el fortalecimiento de las competencias científicas desde una articulación del arte y la tecnología. A partir de la implementación, se pudo evidenciar respuesta positiva de los estudiantes, generando un impacto significativo que permite validar el alcance de la investigación y da cuenta de la importancia de fortalecer las competencias digitales, puesto que generan transformación de los escenarios de enseñanza. Para finalizar, se reconoce la pertinencia de articular a los procesos de enseñanza el uso del arte y la tecnología; puesto que la pandemia ha obligado a los docentes a usar nuevas metodologías, que les exige explorar su creatividad y el uso de herramientas digitales, esto con el fin de promover los aprendizajes de una forma dinámica, innovadora y segura, The main objective of this research was to implement a pedagogical strategy to strengthen scientific competencies in the area of natural sciences, supported by New Media Art, in fifth grade students of the I.E. Digital, La propuesta que se presentó, está diseñada para los grados 5° de primaria, el colegio Humberto Gomez Nigrinis de Piedecuesta Santander, para la asignatura de Educación Artística Se enfoca en la implementación de las TIC para hacer de la enseñanza- aprendizaje de las artes, un espacio creativo, llamativo, fuera de la cotidianidad del aula ya que en la institución los temas o contenidos del área, se enfocan solo en el dibujo técnico, dejando atrás las artes escénicas o demás disciplinas del arte. La propuesta buscaba potencializar y dinamizar la enseñanza-aprendizaje de las artes a través de las The New Media y apoyadas con la plataforma Classroom, que está implementada en la institución y el aprovechamiento de su riqueza tecnológica y comunicativa para el uso pedagógico de la misma. Un aula virtual en artística, en la cual se encuentran disponibles contenidos educativos en artes, referentes a la musica y la danza colombiana, del colegio Humberto Gomez Nigrinis. Las metodologías aplicadas fueron: ABP, Aprendizaje colaborativo familiar, Aprendizaje basado en competencias. Las metas alcanzadas fueron: Se Apoyaron y actualizaron los procesos de enseñanza de la Educación Artística en la institución mediante las herramientas digitales como lo es el aula virtual “Talentositos Hugonistas” a través de Classroom. Se incentivó el deseo de aprendizaje de las artes de una forma lúdica y se incluyeron distintos contenidos de las diferentes disciplinas artísticas. Digital, La presente investigación plantea con objetivo general, fortalecer la técnica de la pintura en la clase de artística por medio de situaciones de aprendizaje a través de la herramienta digital auto DRAW, con el propósito de dar respuesta a las necesidades de fortalecer la técnica de la pintura en la clase de artística en la Institución Educativa Nicanor Restrepo Santamaría, beneficiando con ello a los estudiantes al estar el docente en capacidad de mejorar su quehacer didáctico en las prácticas pedagógicas e inclusive podrá guiarlos más asertivamente en el uso de herramientas tecnológicas. Con este estudio, también se están apoyando las actividades de investigación educativa a través del desarrollo de las competencias referidas a la gestión de la información disponible en la Red. Desde una metodología mixta, y la aplicación de encuestas a una docente y 36 estudiantes, se llegó a interesantes resultados como, la evaluación del impacto de la aplicación de la herramienta digital auto DRAW en los estudiantes y docente del salón de clases del grado segundo de la institución fue positiva, por varias razones. En primer lugar, el tema estuvo comprendido por los estudiantes, lo que indica un buen nivel de conciencia acerca de su propio proceso de aprendizaje. En segundo lugar, orientó a la docente en el uso práctico de las tecnologías en educación, lo que se logró con una aplicación bastante conocida, como es la herramienta de modo que sirvió como muestra de cada actividad. Es importante destacar que, la implementación de esta propuesta también se puede justificar ya que, hoy en día el mundo está atravesando por una crisis de salud pública, generada por la aparición de una nueva neumonía llamada COVID-19, que de alguna manera está afectando en los actuales momentos a todos los sectores, político, económicos, sociales, culturales y educacionales a nivel mundial, lo que ha traído como consecuencia, la paralización de las clases en todos los continentes. Por lo tanto, esta alternativa de implementar la presente propuesta seria un factor clave para llevar la educación a los hogares de los niños, niñas, jóvenes y adultos a experimentar con la aplicación las herramientas digitales en los procesos de enseñanza y aprendizajes a distancia., The present investigation raises with general objective, to strengthen the painting technique in the artistic class through learning situations through the auto DRAW digital tool, with the purpose of responding to the needs of strengthening the painting technique in the artistic class at the Nicanor Restrepo Santamaria Educational Institution, thereby benefiting the students as the teacher is able to improve their didactic work in pedagogical practices and may even guide them more assertively in the use of technological tools. With this study, educational research activities are also being supported through the development of competencies related to the management of the information available on the Internet. From a mixed methodology, and the application of surveys to a teacher and 36 students, reached interesting results such as, the evaluation of the impact of the application of the auto DRAW digital tool on the students and teachers of the second-grade classroom of the institution was positive, for several reasons. Digital, A través del presente trabajo, se pretende fortalecer el concepto de fracción en niños de grado 4° de primaria, de la Institución Educativa San Luis Beltrán, sede el cocal del municipio Neiva. A partir de aportes realizados por algunos autores sobre el tema y el estudio exploratorio realizado, se diseñó un objeto de aprendizaje mediado por los new media, esta manifestación de aprendizaje fue elaborado con el medio electrónico ExeLearning, abordando el concepto de fracción. Se aplicaron métodos de recolección de información, como encuestas y test.Se observó que la enseñanza del concepto de fracciones es una base para generar conocimiento matemático desde temprana edad en los estudiantes y el cual ira siendo fortalecido con los años siguientes sin embargo en necesario que el estudiante pueda crear bases sólidas desde los primeros años sobre el concepto, por lo tanto se apoyó la propuesta con las orientaciones de los lineamientos curriculares para el área de matemáticas (ministerio de Educación [MEN], 1998); al igual que se utilizó la resolución de problemas como metodología de aprendizaje. Una vez aplicado el objeto virtual se alcanzó mayor comprensión en el concepto de fracción y subtemas relacionados, fortaleciendo la argumentación de procedimientos de las situaciones problemas que se le plantearon y su solución adecuada, logrando así un mejor desempeño académico en la población de estudio, Through this work, it is intended to strengthen the concept of fraction in children of grade 4 of primary school, of the San Luis Beltrán Educational Institution, headquarters of the Neiva municipality cocal. Digital, La investigación presentada da cuenta de una experiencia educativa, en la cual se integraron las TIC y la educación artística como estrategia didáctica para fortalecer las habilidades comunicativas en estudiantes de grado cuarto de educación básica primara, pertenecientes al colegio técnico municipal Simón Bolívar ubicado en el municipio de Duitama – Boyacá, la cual fue aplicada en el segundo periodo académico del año 2020 (meses de junio y julio). Introduction and Objectives: Coronavirus disease 2019 (COVID-19) is caused by severe acute respiratory syndrome coronavirus 2. Atrial fibrillation (AF) is common in acute situations, where it is associated with more complications and higher mortality., Methods: Analysis of the international HOPE registry (NCT04334291). The objective was to assess the prognostic information of AF in COVID-19 patients. A multivariate analysis and propensity score matching were performed to assess the relationship between AF and mortality. We also evaluated the impact on mortality and embolic events of the CHA
2 DS2 -VASc score in these patients., Results: Among 6217 patients enrolled in the HOPE registry, 250 had AF (4.5%). AF patients had a higher prevalence of cardiovascular risk factors and comorbidities. After propensity score matching, these differences were attenuated. Despite this, patients with AF had a higher incidence of in-hospital complications such as heart failure (19.3% vs 11.6%, P=.021) and respiratory insufficiency (75.9% vs 62.3%, P=.002), as well as a higher 60-day mortality rate (43.4% vs 30.9%, P=.005). On multivariate analysis, AF was independently associated with higher 60-day mortality (hazard ratio, 1.234; 95%CI, 1.003-1.519). CHA2 DS2 -VASc score acceptably predicts 60-day mortality in COVID-19 patients (area ROC, 0.748; 95%CI, 0.733-0.764), but not its embolic risk (area ROC, 0.411; 95%CI, 0.147-0.675)., Conclusions: AF in COVID-19 patients is associated with a higher number of complications and 60-day mortality. - Abstract
Background: The use of combination antiretroviral therapy has led to dramatic improvements in the life expectancy of HIV-infected persons. As result, the HIV population is aging and increasingly facing illnesses typically seen in the elderly, such as chronic kidney disease (CKD)., Methods: A retrospective longitudinal study was conducted using data from years 2010 and 2014 in all HIV-infected persons enrolled at the Spanish VACH cohort. We analyzed the prevalence and the predictive factors for developing CKD (estimated glomerular filtration rate, eGFR<60mL/min/1.73m
2 )., Results: The CKD prevalence at baseline was 456/8968, 5.1% [4.6-5.6%]. Of 8512 HIV-positive individuals examined without CKD at baseline (73.7% male, median age 44 years-old), 2.15% developed CKD (eGFR<60mL/min/1.73m2 ). The odds ratios [95%CI] for the independent predictive factors identified were gender (male) 0.54 [0.39-0.75], age (per year) 1.08 [1.07-1.10], AIDS diagnosis 1.40 [1.03-1.91], protease inhibitor-based regimens 1.49 [1.10-2.02], hypertension 1.37 [0.94-1.99], diabetes 1.84 [1.33-2.55] and history of cardiovascular events 1.66 [0.96-2.86]., Conclusion: The prevalence and risk factors for CKD and its progression are high in the VACH cohort. Thus, preventive measures such as control of hypertension, diabetes and obesity, as well as efforts for avoiding exposure to nephrotoxic drugs, including some antiretrovirals, are warranted in this aging HIV population., (Copyright © 2019 Sociedad Española de Nefrología. - Abstract
Background: The life expectancy of HIV-infected individuals has dramatically improved with potent antiretroviral therapies. However, organ-specific toxicities of some antiretrovirals and persistent inflammation and immune activation due to residual virus replication account for a high burden of age-associated comorbidities in the HIV population., Methods: The prevalence of overt cardiovascular, renal and bone diseases as well as their major risk factors were cross-sectionally examined during the year 2014 in the VACH cohort, a large nationwide population of HIV-infected individuals in Spain., Results: A total of 10,897 HIV-infected patients were examined. Seventy-one point four percent were male and the mean age was 48 years. Mean time since HIV diagnosis was 15.8 years and mean time on antiretroviral therapy was 13.1 years. The proportion of patients with undetectable viral load was 87.1%, whereas 65.7% had CD4 counts>500 cells/mm
3 . Overall, cardiovascular, renal and bone disease were recorded in 4.7%, 5.9% and 2.8%, respectively. The prevalence of major risk factors was as follows: smoking 51.3%, alcohol abuse 7.8%, overweight/obesity 42.2%, diabetes 19.9%, dyslipidaemia 72.6%, hypertension 25.6%, and osteoporosis 11.1%. In the subset of patients older than 55 years-old (18%), all figures for overt disease and their major risk factors were significantly greater., Conclusion: Major age-related medical conditions and most of their risk factors are highly prevalent in HIV-infected individuals on long-term antiretroviral therapy in Spain. Osteoporosis has become an emerging comorbid condition in people living with HIV (PLWH). The increase in survival and the progressive aging of PLWH will make this complication more frequent in the near future. In addition to the traditional risk factors affecting the general population, factors directly or indirectly associated with HIV infection, including antiretroviral therapy, can increase the risk of osteoporosis. The present article is an executive summary of the document that updates the previous recommendations on the prevention and treatment of osteoporosis in PLWH. - Abstract
Objective: This consensus document is an update of metabolic disorders and cardiovascular risk (CVR) guidelines for HIV-infected patients., Methods: This document has been approved by an expert panel of GEAM, SPNS and GESIDA after reviewing the results of efficacy and safety of clinical trials, cohort and pharmacokinetic studies published in biomedical journals (PubMed and Embase) or presented in medical scientific meetings. Recommendation strength and the evidence in which they are supported are based on the GRADE system., Results: A healthy lifestyle is recommended, no smoking and at least 30min of aerobic exercise daily. In diabetic patients the same treatment as non-HIV infected patients is recommended. HIV patients with dyslipidemia should be considered as high CVR, thus its therapeutic objective is an LDL less than 100mg/dL. The antihypertensive of ACE inhibitors and ARAII families are better tolerated and have a lower risk of interactions. In HIV-patients with diabetes or metabolic syndrome and elevated transaminases with no defined etiology, the recommended is to rule out a hepatic steatosis Recommendations for action in hormone alterations are also updated., Conclusions: These new guidelines update previous recommendations regarding all those metabolic disorders involved in CVR. The aim of this article is to update the 2010 recommendations on the evaluation and management of renal disease in HIV-infected patients. Renal function should be monitored in all HIV-infected patients. The basic renal work-up should include measurements of serum creatinine, estimated glomerular filtration rate by CKD-EPI, urine protein-to-creatinine ratio, and urinary sediment. Tubular function tests should include determination of serum phosphate levels and urine dipstick for glucosuria. In the absence of abnormal values, renal screening should be performed annually. In patients treated with tenofovir or with risk factors for chronic kidney disease (CKD), more frequent renal screening is recommended. In order to prevent disease progression, potentially nephrotoxic antiretroviral drugs are not recommended in patients with CKD or risk factors for CKD. The document provides indications for renal biopsy and advises on the optimal time for referral of a patient to the nephrologist. The indications for and evaluation and management of dialysis and renal transplantation are also addressed.

Objective: To provide practical recommendations for the evaluation and treatment of metabolic bone disease in human immunodeficiency virus (HIV) patients., Participants: Members of scientific societies related to bone metabolism and HIV: Grupo de Estudio de Sida (GeSIDA), Sociedad Española de Endocrinología y Nutrición (SEEN), Sociedad Española de Investigación Ósea y del Metabolismo Mineral (SEIOMM), and Sociedad Española de Fractura Osteoporótica (SEFRAOS)., Methods: A systematic search was carried out in PubMed, and papers in English and Spanish with a publication date before 28 May 2013 were included. Recommendations were formulated according to GRADE system (Grading of Recommendations, Assessment, Development, and Evaluation) setting both their strength and the quality of supporting evidence. Working groups were established for each major part, and the final resulting document was later discussed in a face-to-face meeting. - Abstract
Background and Objective: The incidence of stroke in the very old is increasing. However, there are very few data regarding the differences in the management and outcomes in Spain., Material and Methods: We analyzed the clinical characteristics, diagnostic and therapeutic procedures, medical complications and intrahospitalary mortality in 463 subjects admitted because of ischemic stroke between 2009 and 2010. Very-old patients (≥ 85 year-old) were compared with younger patients., Results: Very old patients showed on admission higher frequency of atrial fibrillation (34 vs 19%, P>.001) and lower use of antiplatelet agents and oral anticoagulants (P<.001). Disability and stroke severity were higher among the very old (P<.001) and the use of diagnostic and therapeutic procedures was significantly lower (P<.001). The length of stay was longer in the very old (12 [6-24] days vs 8 [5-15], P<.001), as wells as the intrahospitalary mortality (27 vs 6%, P<.001). After a multivariate analysis, independent predictive factors of mortality were previous disability, measured by the modified Rankin scale (95% confidence interval [95%CI] 1.6-10.7), impaired level of consciousness (IC95% 2.1-13.9) and stroke severity measured by the National Institutes of Health Stroke Scale (95%CI 1.8-38.1)., Conclusions: Very old patients admitted with ischemic stroke undergo less diagnostic and therapeutic procedures. However, in our study, very old age was not an independent predictor of mortality, which was essentially determined by previous functional status and stroke severity., (Copyright © 2012 Elsevier España, S.L. - Abstract
Background and Objective: HIV-infected patients present accelerated cardiovascular disease (CVD) and CVD is among the most important causes of mortality in this population. We aimed to identify biomarkers and clinical factors associated with subclinical atherosclerosis in HIV-infected patients., Patients and Methods: Carotid intima-media thickness (cIMT) and cardiovascular biomarkers were measured in 235 HIV-infected patients. Individuals with a cIMT ≥ 75th percentile or plaque were classified as having subclinical atherosclerosis and compared with patients without subclinical atherosclerosis., Results: Age was 46 (11) years old. Mean cIMT was 0.58 (0.13)mm. Sixty-five (27.8%) patients had subclinical atherosclerosis. These subjects had more frequently lipodystrophy (OR:2.7; CI95%:1.4-4.9), immunosuppression (OR:2.5; CI95%:1.1-5.8), longer time to HIV diagnosis (≥ p50 [10 years], OR:1.4; CI95%:1.1-2.9), longer exposure to nucleoside analogues (≥ p50 [132 months], OR:3.2; CI95%:1.7-6) and to protease inhibitors (≥ p50 [24 months], OR:2.2; CI95%:1.1-3.6). They also showed higher levels of several biomarkers such as NT-proBNP (≥ p75 [72.6 pg/ml], OR:2.0; CI95%:1-4.1) and albumin/creatinine urine ratio (≥ p50 [5mg/g], OR:3.8; CI95%:1.3-11). After the multivariate analysis, subclinical atherosclerosis was associated with age (OR:6.6; CI95%:2.2-19.5; P=.001), a longer time to HIV diagnosis (OR:3.1; CI95%:1.0-11.0; P=.044) and immunosuppression (OR:2.8; CI95%:1-8.3; P=.048)., Conclusions: Among HIV-infected patients, time to HIV diagnosis and immunosuppression were independently associated with subclinical atherosclerosis. Patients with subclinical atherosclerosis showed increased levels of vascular damage biomarkers, especially albumin/creatinine urine ratio and NT-proBNP., (Copyright © 2011 Elsevier España, S.L.
