Introduction: Cystic lymphatic malformations, also called cystic lymphangiomas, are very rarely found in the liver. Clinical cases. Two pediatric female preschool-age patients. presented with hepatomegaly due to multi-septated cystic lesions of the liver, who received treatment at Hospital Pediátrico Universitario William Soler, La Habana, Cuba. Results: We report two pediatric cases with giant cystic lymphatic malformation of the liver. In both cases, the diagnosis were based on imaging, laparoscopy and pathology. In one case the treatment was right hepatectomy, whereas in the other, sclerotherapy was performed, both with a favorable outcome. Conclusion: Despite its rarity, this diagnosis should be considered in pediatric patients with hepatic cystic lesions. The recommended treatment is surgical resection, but its indication and extent should be assessed individually for each patient. [ABSTRACT FROM AUTHOR]

Neonatal congenital leukemia manifests within the first 30 days of life and is exceedingly rare in this age group, but more prevalent among patients with Down syndrome. Its etiology is primarily genetic but is also linked to alcohol consumption, marijuana, and smoking. Differential diagnosis should be made with transient myeloproliferative disorder, a benign condition that resolves spontaneously. We present a clinical case of a full-term newborn, 16 days old, initially displaying symptoms suggestive of necrotizing enterocolitis. However, upon admission to the neonatal unit, the patient exhibited a greatly distended abdomen with severe hepatosplenomegaly, severe leukocytosis, anemia, and thrombocytopenia, accompanied by 100% blasts and laboratory findings consistent with tumor lysis syndrome. Management was initiated with hyperhydration, allopurinol, and rasburicase, but there was no improvement. Therefore, cytarabine was added. Flow cytometry indicated a megaloblastic pattern, indicative of myeloid leukemia. The patient required hospitalization for a month and a half, during which expected complications might arise. However, it was considered that the newborn experienced transient myeloid disorder, given the satisfactory clinical evolution, with reduced symptoms and severe leukocytosis. [ABSTRACT FROM AUTHOR]

In a 19-month long period there were outbreaks in which 16 exotic birds that belonged to the collection of the Cali Zoo died. Most of the affected animals were Australian parrots, of which a large percentage had sudden death. Mortalities occurred in enclosures with different characteristics, located in exhibition and internal areas of the Zoo. Necropsy, histopathology, and immunohistochemistry were performed in some animals that did not present an advanced state of decomposition, where the presence of a parasitic pneumonia with intraendothelial merozoites of Sarcocystis spp. was confirmed. Some of the individuals who survived, received combined antiparasitic, antibiotic, and antifungal therapy. Risk factors and important aspects of the management to prevent Sarcocystosis in animals kept under human care are mentioned, because this disease has high mortality in Old World birds, most of the time, without previous clinical signs. Glycogen storage diseases comprise inherited pathologies caused by enzyme and transporter abnormalities in glycogen synthesis and degradation. Clinical data include fasting lactic acidosis, hypoglycemia and hepatomegaly which can help guide the diagnosis. Diagnosis is suspected based on clinical presentation. In the past, definitive diagnosis required a liver biopsy to demonstrate glycogen deposition. However, mutational analysis of genes is now performed as a non-invasive approach. We present the case of a 19-month-old female patient, multiconsultant to the emergency department for episodes initially considered as focal crises (hypertonia of upper and lower limbs, sucking, fixed gaze) associated with hepatomegaly. She was evaluated by several specialties, with multiple managements without improvement. Finally, a diagnosis of glycogen storage disease was made by liver biopsy. [ABSTRACT FROM AUTHOR]

Caroli's disease (CD) is a rare congenital disease, which presents with multifocal segmental intrahepatic bile duct dilatation. It was first described by Jacques Caroli in 1958 as a saccular or fusiform dilatation of the intrahepatic bile ducts. Objective: To characterize the population that has been diagnosed with Caroli's disease. Materials and methods: Systematic review. Results: 66 articles were analyzed. The age group most affected was children under 10 years old, where a higher prevalence was evidenced in males. This condition was associated with other comorbidities such as polycystic kidney in 20%. The most frequent manifestation was hepatomegaly (44.7%), followed by fever (42.4%), and abdominal pain in the right upper quadrant (41.2%). The most used diagnostic method was magnetic resonance imaging in 73.8% of the sample. The findings showed predominance of intra-hepatic dilatation in 76.5%. The most widely used treatment was antibiotic therapy to treat recurrences due to cholangitis. Conclusion: Caroli's disease has an extremely low incidence and occurs more frequently in the American continent, affecting mainly patients in the first decade of life, with a predilection for the male sex. Introduction: Multicentric Castleman's disease is a rare benign lymphoproliferative disorder that involves multiple enlarged lymph nodes in different areas of the body. Recently, a unique clinic pathological variant of this disease called TAFRO syndrome has been described. Case report: A 23-year-old male with a history of one year and six months of evolution characterized by generalized lymphadenopathy with, night diaphoresis, dyspnea and weight loss. At the third month, hypertension was diagnosed and 8 months later type 2 diabetes mellitus. After his admission, microcytic anemia, thrombocytopenia, renal dysfunction, pleural effusion and ascites were documented. Based on the findings of the physical examination, a computed tomography scan was performed, where multiple lymph nodes and hepatosplenomegaly were identified. A lymph node biopsy was performed, which reported Castleman's disease, considering then a diagnosis of TAFRO syndrome due to its association with thrombocytopenia, microcytic anemia, anasarca, fever, renal dysfunction, adenomegaly and hepatosplenomegaly. Conclusions: There are still doubts about whether it corresponds to a different disease or a subtype of the Castleman disease. It is more frequent in women, occurs in the middle-aged and elderly and given its heterogeneous presentation, it usually represents a diagnostic challenge. A case report of a 3-year-old boy with past medical history of intestinal partially treated amebiasis, is presented. The patient was admitted to Pediatric Unit, San Cristóbal Central Hospital, Táchira, Venezuela, with abdominal pain and fever. An abdominal bloating and a 3 cm palpable hepatomegaly below the right costal margin were assessed. Abdominal ultrasound revealed a liver enlarged In the right antero-superior area. A rounded space-occupying lesion, predominantly solid, with mixed-echo patterns, was assessed using ultrasound. The preliminary diagnosis Issued was of acute medical abdomen with hepatic space-occupying lesion considered amebic liver abscess or liver tumor, moderate hypochromic microcytic anemia, and malnutrition with short stature. During the case evolution, a first computerized tomography exploration was necessary in order to exploit the capacity of this imaging technique to scan an abscess as a peripheral pseudo-capsule showing rim enhancement. Nevertheless, this theoretical shape associated with abscesses on computerized tomography scans was unable to verify in this study. At this point, the mixed-echo patterns of the preliminary ultrasound study and the imprecision of the computerized tomography scan to categorize the lesion as an abscess or a tumor, do not allow establishing a definitive diagnosis. A management based on antibiotic therapy Is then proposed. The progression of the space-occupying lesion was performed using ultrasound and computerized tomography scans during the clinical evolution. The imaging controls probe a slight decrease of the liver lesion, which is diagnosed as a liver abscess. Percutaneous transhepatic drainage was performed. INTRODUCTION: Mauriac syndrome is an uncommon condition characterized by growth retardation, hepatomegaly and obesity in children with type 1 diabetes. CASE REPORT: We describe an 11 years old female patient with type 1 diabetes of 8 years duration treated with glargine and rapid-acting insulin. She presented gradually increased abdominal volume of approximately 3 months duration and poor glycemic control over 6 months. In addition, she exhibited polydipsia and polyuria. Physical examination revealed growth retardation, pubertal delay and hepatomegaly. Markers of viral or autoimmune hepatitis were negatives. Finally, liver biopsy reported abundant glycogen deposition. CONCLUSION: Although the Mauriac syndrome is an unusual condition, it should be suspected in patients with uncontrolled type 1 diabetes in order to establish a timely diagnosis and treatment. Introduction: Lysosomal acid lipase deficiency (LAL-D) is an inherited autosomal recessive entity that leads to the accumulation of cholesterol and triglyceride esters in the liver, spleen and other organs. The age of onset and rate of progression vary greatly, possibly explained by mutations of the LIPA gene. Clinical manifestations are the same as those of other hepatic, cardiovascular and metabolic pathologies which makes it difficult to recognize in clinical practice. Objective: The objectives of these guidelines is to help clinicians recognize the major groups at risk for LAL-D and to improve its diagnosis. Methodology: This document was designed as a consensus of experts in gastroenterology, hepatology, endocrinology, genetics, pathology and pediatrics. A review of the literature regarding clinical manifestations and tools for diagnosis of LAL-D was conducted and the nominal group technique was followed. Results: Diagnostic algorithms which facilitate suspicion and diagnosis of LAL-D were generated by consensus for each of the risk groups. Conclusions: This guide proposes algorithms for the diagnosis of LAL-D based on clinical consensus. The algorithms seek to optimize diagnosis for patients with this pathology. [ABSTRACT FROM AUTHOR]

Hepatosplenomegaly (HS) is a physical sign that is frequently detected in the pediatrician's office. Normal hepatic size is based on values established by percussion, extending below the right costal ridge, or the length of the vertical axis estimated by imaging studies. HS can originate from five fundamental mechanisms: inflammation, excessive deposition, cellular infiltration, blood flow congestion, hepatobiliary and hematological obstruction. The personal experience of a mother that faced the Hurler-Scheie syndrome, strange disease suffered from his son already deceased, is exposed with the purpose to demonstrate that the observation of the changes occurring in a child, is vital for his development. The care toward a young child with this disorder is indispensable to achieve a childhood with life quality; therefore, the Cuban health system should continue designing strategies that guarantee an early diagnosis and the possibility of opportune therapeutic alternatives as the enzymatic substitution therapy. An unusual complication of infective endocarditis (IE) of the aortic valve is called kissing-abscess, caused by the impact of the flow of aortic regurgitation and subsequent contact of vegetations over the anterior mitral valve. The resulting abscesses on the mitral valve can further complicate forming pseud aneurisms. We present the case of a 36-year-old male patient with a 3-month history of fever. A diastolic murmur on the aortic chest wall area was listened and hepatomegaly was also found on the physical examination. A trans-esophageal echocardiography revealed severe aortic insufficiency and perforation of the non-coronary sinus with multiple vegetations, mean size of 4 mm. In addition, the anterior mitral valve had a hypo-echogenic image of 14mm in size consistent with an abscess with Doppler flow in its interior and a pseudo aneurism with multiple sub-valvular vegetations. The extension of IE from the aortic to the mitral valve is associated with high mortality rate needing a prompt diagnosis. [ABSTRACT FROM AUTHOR]

We describe the case of a 51-year-old woman with a seropositive, erosive, and non-nodular rheumatoid arthritis of 15 year of evolution. The patient had poor compliance with medical visits and treatment. She came to the clinic with persistent pancytopenia and spleen and liver enlargement. Liver and bone marrow biopsies were carried out and amyloidosis, neoplasias and infections were ruled out. We present the case of a hypertensive patient, who presents with pain in the right hypochondrium stabbing, associated with weight loss of approximately 60 pounds. Hepatobiliary ultrasound: Hepatomegaly with heterogeneous parenchyma secondary to multiple focal lesions, highly suggestive of metastatic liver disease. Contrasting abdominopelvic tomography: Hepatomegaly with irregular hypodense images towards the periphery of the liver suggestive of metastasis. We report the first case of aortic valve atresia with normal left ventricle was diagnosed at our institution. This malformation is rare in this variety, is the reason of submission. Clinical manifestations of fatigue during feeding, tachypnea, groan, pallor, cyanosis, precordial murmur, and hepatomegaly diminished distal pulses directed to suspected congenital obstructive left heart disease. Peliosis is a very rare benign entity which is characterized by blood filled cavities filled within solid organs. They occur primarily in the mononuclear phagocyte system although peliosis can also affect organs such as the lungs, pleura, kidneys, adrenal glands and stomach. Only one lesion or multiple lesions may occur, and their size is also variable. The etiology is unknown but is related to ingestion of, or direct contact with, drugs and various toxic substances such as steroids and oral contraceptives. It is associated with hematologic cancers and infectious diseases, particularly tuberculosis. In some cases it is idiopathic. We present the case of a 60 years old woman with a history of hypertension. Physical examination and various imaging studies showed a giant nodular hepatomegaly without splenomegaly. Laboratory studies showed disorders due to liver failure. The patient finally died of hypovolemic shock and hemoperitoneum. An autopsy determined the presence of parenchymal hepatosplenic peliosis of other than idiopathic etiology. Non-alcoholic liver disease (NAFLD) is the most common liver disease in affluent societies, affecting 2-8% of the general population, and it will be soon in our societies. It is generally asymptomatic or with a no specific picture of fatigue, hepatic pain or discomfort and hepatomegaly. It is suspected in cases with aminotransferase or imaging abnormalities. Liver biopsy is considered the gold standard for diagnosis. There is a close pathogenic relationship with obesity, type 2 diabetes, hyperlipidemia, metabolic syndrome and insulin resistance. Approximately 20-25% of the cases progress into cirrhosis with all its complications including hepatocellular carcinoma and the need for liver transplantation. Correction of insulin resistance with dietary measures and physical activity is generally beneficial. The efficacy of the multiple medications available remains to be demonstrated.
