Hereditary hemorrhagic telangiectasia (HHT) is a genetic disorder associated with abnormal angiogenesis and disabling epistaxis. Tranexamic acid (TA) has been widely used in the treatment of these severe bleeds but with no properly designed trial.

Exploration of the variation of treatment effect over time in randomized clinical trials with low event rates is limited by lack of power. A meta‐analysis on individual patient data from such trials can partly solve the problem, but brings other computational difficulties. Using an example in hypertension, we describe appropriate methods for graphical description and statistical modelling of treatment–time interactions in large data sets. Also, a method is developed for determining the total number of events required to detect treatment–period interactions of plausible magnitude. We conclude that trialists tend to overinterpret the observed data when looking for potential treatment–time interactions by visual comparisons of survival curves, failing to realize the substantial amounts of data that are needed for their detection and estimation. © 1998 John Wiley & Sons, Ltd.

The French social security has had a balanced budget as a recurring objective for several decades, with a specific target on the National Health Insurance (NHI) branch. Over the past several years, efforts have intensified, with a continuing policy on decreasing drug prices, developing ambulatory care to limit hospitalizations or reducing the reimbursement of some specialists. Decreasing the reimbursement rate of drugs reduces, as a direct effect, the prescription and delivery of the targeted drugs. However, the substitution effect in favor of still reimbursed drugs should not be forgotten, because it could minimize the intended effect in economic terms as well as in terms of public health. The recent French decision to terminate the reimbursement of Symptomatic Slow-Acting Drugs for Osteoarthritis (SYSADOAs) is an interesting case study for the impact of terminating the reimbursement a drug from a collective perspective. The aim of the present study was to investigate prescription substitutions in favor of analgesics and non-steroidal anti-inflammatory drugs (NSAIDs), both in terms of volumes and costs, as well as the occurrence of their potentially associated adverse effects (AEs) using a methodology based on access to national administrative databases.

L’objectif de retour à l’équilibre du Régime général de la Sécurité sociale repose notamment sur la réduction du déficit de la branche maladie. Les efforts en ce sens incluent une baisse du prix des médicaments, un développement des soins ambulatoires ou encore la réduction du remboursement de certaines spécialités. La baisse des taux de remboursement a pour effet direct de diminuer la prescription et la délivrance des traitements. Il faut cependant prendre en compte les éventuels reports de prescription vers des classes thérapeutiques toujours remboursées qui peuvent avoir des conséquences en termes d’économies et de santé publique. L’exemple du déremboursement de la classe des anti-arthrosiques symptomatiques d’action lente (AASAL) en France constitue un cas d’étude de l’impact d’un déremboursement du point de vue de la collectivité. L’objectif de l’étude était d’investiguer les reports de prescriptions vers les antalgiques et les anti-inflammatoires non stéroïdiens (AINS) et d’autres classes thérapeutiques associées, en termes de volumes et de coûts, ainsi que les éventuels événements indésirables.

Dans la perspective d’une médecine stratifiée, il est utile d’identifier des variables biologiques associées à une modification de l’effet d’un traitement, c’est-à-dire des biomarqueurs prédictifs de la réponse à une thérapie. Parallèlement à la question de la puissance de ces études, se pose la question de l’estimation de l’ampleur de la modification de l’effet. Si le plan expérimental en cross-over est fréquemment plus puissant que le plan en bras parallèles pour détecter un effet traitement, sa valeur ajoutée pour identifier et estimer une interaction entre une covariable et le traitement a été peu étudiée. L’objectif de cette étude est de caractériser l’apport du plan en cross-over versus le plan en bras parallèles, en termes de puissance statistique, pour l’identification de biomarqueurs prédictifs de la réponse au traitement.
