This report presents 2001 data on U.S. births according to maternal demographics (age, live-birth order, marital status, race, Hispanic origin, and educational attainment); maternal characteristics (medical risk factors, weight gain, and tobacco and alcohol use); pregnant women's medical care utilization (prenatal care, obstetric procedures, complications of labor and/or delivery, attendant at birth, and delivery method); and infant characteristics (gestation period, birthweight, Apgar score, abnormal conditions, congenital anomalies, and multiple births). Also presented are birth and fertility rates by age, live-birth order, race, Hispanic origin, and marital status. Data are shown on mother's state of residence, month and day of birth, sex ratio, and father's age. Trends in fertility patterns and maternal and infant characteristics are noted. The number of births, birth rate, fertility rate, and total fertility rates all declined 1 percent in 2001. The teenage birth rate reached another historical low. Birth rates for women in their 20s declined slightly. Rates for women age 30-44 years continued to rise. Births to unmarried women changed very little. Smoking by pregnant women was down again. Women were more likely to begin care in the first trimester of pregnancy. The cesarean delivery rate rose for the 5th consecutive year to 24.4 percent. Preterm and low birthweight levels rose in 2001. (SM)

Recognizing the importance of health care during the perinatal period, the topic for this issue of "Children in the Tropics" is prenatal development; this issue is intended for health educators for use in educating their clients about prenatal development, health care during pregnancy, labor and delivery, and the needs of the mother and neonate during the perinatal period. The sections are: (1) "Gestation," focusing on considerations for health care providers, psychological, social, and environment risk factors; (2) "Intrauterine Development," presenting a day-by-day account of organ formation and development, functions of the placenta, the amniotic fluid, methods of intrauterine stimulation, medication, drugs, and maternal diet; (3) "Surveillance of Pregnancy," detailing health care procedures and the impact of the expectant mother's family and social environment; (4) "Pregnancy and Childbirth: A Psychological and Physiological Experience," describing psychological needs during pregnancy, the role of the health team, physical and psychological aspects of childbirth, examination of the neonate, identifying gestational age, and breastfeeding; and (5) "The Health Team--An Interdisciplinary Team," presenting the educational needs of expectant mothers and their families and health care procedures during the perinatal period. Building community collaboration and commitment for educating students in urban districts was the theme of the January 29-31, 1992, meeting of the Urban Superintendents' Network. Presentations and discussions focused on the political nature of education, Chapter 1 legislation, the use of educational technology, and educational standards and testing. This publication includes excerpts from the keynote address by Bernard Watson speaking on the urgent need for collaborative action in solving problems in education. Another article features an exchange about business/education collaboratives between Deputy Secretary of Education David Kearns and meeting participants. Harold Hodgkinson presented data on urban demographics. Concurrent sessions were held on four topics: education of children prenatally exposed to drugs; school/community/business collaboration; staff development and renewal; and school restructuring and reform. This booklet examines what is known about the long-term effects of exposure in utero to alcohol and other drugs, as well as the educational implications of those effects. Research is synthesized on biological and medical risk factors, psychosocial risk factors, and the interaction of biology and environment. A section on implications for educational personnel discusses protective factors and facilitative processes to be built into the classroom. Implications for program development and administration are also discussed, listing needed services and outlining the need for transagency/transdisciplinary service delivery. Statistical data illustrate the incidence of babies who have been prenatally exposed to cocaine. The damaging effects of maternal cocaine use on the fetus, infant, and young child are described, including: (1) prenatal strokes, malformed kidneys and limbs, and deformed hearts and lungs; (2) physical problems, social and emotional problems, and impaired learning in the first years of life; and (3) problems with interpersonal relationships and school in the preschool and early school years. The damaging effects of cocaine on families and on social services, health services, and educational services are also noted. Four proposed solutions to this social problem are outlined, along with their possible drawbacks. (JDD)
