The text "Presented by the 'Citizens' of Warren County, Ohio" begins on the left of the arms, near the bottom of the eagle's wings, and continues on the right of the arms. The text "TO THE" appears directly below the arms. Below that, in a gold outlined scroll, is the text "12th REGIMENT O.V.I.U.S.A." The flag's gold-knotted fringe is 4 cm in length

The flag has gold, plied fringe with knotted cut ends measuring 6 cm in length on its top, right, and bottom sides.
