Using data from the 2022 Korea Community Health Survey (n = 13 320), this study investigated helmet use and related factors among Korean adults using personal mobility devices, without distinguishing between private and hired users. Among mobility device users, 32.1% responded that they always wore a helmet. The proportion of helmet use was 35.2% among men, 25.8% among women, 29.2% among those aged 19-44 years, 42.3% among those aged 45-64 years and 26.6% among those aged 65 years or older. Furthermore, those who drank less frequently and were physically active were more likely to wear helmets. Moreover, people who always wore a seat belt when driving a car or sitting in the rear seat and people who always wore a helmet when riding a motorcycle were more likely to wear a helmet while using electric personal mobility devices. Approximately one-third of users always wore a helmet. The helmet-wearing rate was related to general characteristics such as gender and education level, and to safety behaviors such as wearing a seat belt when driving a car, sitting in the rear seat of a car, or when riding a motorcycle. Our work investigated the radioprotection implications associated with the possession of a collection of uraniferous minerals. Considering different scenarios, we developed (and applied to an actual collection) specific formulas for radiation doses evaluation. We discussed the shielding necessary to reduce the gamma irradiation down to the required values. A mathematical model was developed to estimate the minimum air flow rate to reduce the radon air concentration below the reference values. The radiation risks associated to the handling of single specimens was also addressed, including hand skin irradiation and shielding capabilities of surgical lead gloves. Finally, we discussed the radiation risks associated to the exhibition of a single specimen. The results, compared to the safety standards of the EU Directive 13/59, show that the exhibition of uraniferous samples with activity of a few MBq do not need specific radioprotection requirements nor for the involved personnel nor for visitors., (© The Author(s) 2024. In the current study, CeO2: Ag (0 and 11 mol) nanoparticles (NPs) were synthesized by solution combustion method using Aloevera extract as reducing agent. As-obtained NPs were characterized by standard techniques. Bragg's reflections confirm the formation of a single-phase cubic structure of CeO2:Ag NPs. Crystalline size is calculated using both the W-H plot and Scherrer's equation. Crystallite size found to decrease with increase in the dopant concentration. EDAX pattern confirmed the presence of Ce, O and Ag. Direct energy band calculated using Wood and Tauc's was found to be in the range of 2.9-2.2 eV for 0 and 11 mol, respectively. Fourier transformation infrared spectroscopy (FTIR) analysis confirmed the presence of the functional groups. Total shielding efficiency (SET) will give the best representation of EMI shielding properties. SET values calculated for a wide range of wavelengths are found to be as follows: near infrared (1.65 × 102 dB), mid infrared (9.78 × 101 dB) and far infrared (6.32 × 101 dB), followed by microwave region (MW) (6.46 × 101 dB), ultra-high frequency (UHF) (7.31 × 101 dB), very high frequency (VHF) (8.27 × 101 dB), high frequency (HF) (9.26 × 101 dB), medium frequency (MF) (1.02 × 102 dB), low frequency (LW) (1.12 × 102 dB), very low frequency (VLF) (1.22 × 102 dB), ultra-low frequency (ULF) (1.42 × 102 dB) and extremely low frequency (ELF) (1.52 × 102 dB). Radiation protection is essential to minimize the harmful effects of ionizing radiation. Lead equivalent apron is one of the common protective devices to reduce the occupational risk of radiation exposure. The aim of the study is to analyse the percentage attenuation of 0.5 mm lead equivalent apron. Twenty aprons were used for calculating the percentage of attenuation. The computed tomography (CT) phantom was placed on the radiography and CT table at 1 m from the X-ray tube. The readings were taken from the phantom using a detector at 1-m distance with and without lead equivalent aprons. The readings were tabulated and analysed. The mean percentage attenuation of 0.5 mm lead equivalent apron is 95.66 ± 0.61 in radiography (100 kVp), 95.64 ± 0.57 in CT (130 kVp), respectively. The objective of this study was to study the dose reduction effect on occupational exposure of operators and public exposure other than operators when a backscatter shield and a rectangular collimator are used in conjunction with a handheld intraoral X-ray unit. The occupational exposure was reduced to 40% when the backscatter shield was attached to the cone-tip, to 13% when the rectangular collimator was attached and to 7.7% when the backscatter shield and rectangular collimator were used together. On the other hand, the public exposure was reduced to 20% when the rectangular collimator was attached, but the backscatter shield was not effective in reducing the public exposure. Attaching a backscatter shield is effective in reducing the occupational exposure, and a rectangular aperture is effective in reducing the occupational exposure, as well as the public exposure., (© The Author(s) 2023. An additional extremity monitoring using a ring badge must be appropriately conducted for inhomogeneous exposure around radiation workers' extremity. Commercially available glass dosemeters are characterized in terms of Hp(0.07) for the application of additional extremity monitoring. A series of experiments demonstrated that the response of the model GD-352M radiophotoluminescence dosemeter fully matched the IEC's criteria for an extremity dosemeter for medical personnel. Although the model GD-302M has excellent angular dependence, the material and the shape of energy compensation filter still need to be optimized to improve its energy dependence in the range between 30 and 100 keV. The combine use of both types of glass dosemeters for 'paired dosemeter' together with introduction of a simple algorithm may be a promising method to improve the response in the energy range below 20 keV., (© The Author(s) 2023. Extremity radiation monitoring is an important tool for the assessment of occupational exposures to staff at a variety of workplaces where ionising radiation is used. This work shows the feasibility of applying 3D printing for the development of customisable ring dosemeters. The rings were developed using two types of resin, hard and flexible and has the possibility of sterilisation using different techniques. The printed ring dosemeter was associated with BeO optically stimulated dosemeters. The energy and angular response were found within ±20% in the energy range from 24 to 662 keV and from 0° to 60° angle of incidence. This contributes to the reduction of measurement uncertainty when compared with currently used thermoluminescent detectors dosemeters. The new ring dosemeter showed a satisfactory response with respect to the performance criteria of the IEC 62387 Standard, in addition to providing improved ergonomics in relation to the commercial ring dosemeter., (© The Author(s) 2023. Because of reproducibility and repeatability problems with reference measurements made with an ionisation chamber in a self-protected GSRD1 irradiator, a comparison was made with alanine dosimetry for a whole-body mouse irradiation setup in a sterile box. The twisting of the cables in the cable duct and in the irradiator cell and the irradiation of the ionisation chamber connector are likely to have caused the problems encountered. These problems are not observed on other types of irradiators with more suitable cable passages. A difference up to 8.4% was observed between the alanine dosimetry and ionisation chamber. PyMCGPU-IR is an innovative occupational dose monitoring tool for interventional radiology procedures. It reads the radiation data from the Radiation Dose Structured Report of the procedure and combines this information with the position of the monitored worker recorded using a 3D camera system. This information is used as an input file for the fast Monte Carlo radiation transport code MCGPU-IR in order to assess the organ doses, Hp(10) and Hp(0.07), as well as the effective dose. In this study, Hp(10) measurements of the first operator during an endovascular aortic aneurysm repair procedure and a coronary angiography using a ceiling suspended shield are compared to PyMCGPU-IR calculations. Differences in the two reported examples are found to be within 15%, which is considered as being very satisfactory. Purpose: Minimizing hazardous drug (HD) contamination is critical for protecting the health of healthcare workers (HCWs) and patients. Alarmingly, widespread HD contamination has been documented across a variety of clinical settings. Quantitative wipe sampling presents significant time and cost barriers, resulting in routine monitoring adherence rates around 25%. Closed-system drug transfer devices (CSTDs) and qualitative point-of-care tests can be implemented to overcome these barriers., Methods: In this study, we tested the effects of the BD PhaSeal Optima (Becton, Dickinson and Company), a recently introduced CSTD, on HD contamination at 2 chemotherapy infusion centers. Wipe samples were taken at 29 workstations at each location prior to and a year following CSTD implementation. Additionally, traditional liquid chromatography with mass spectrometry (LCMS/MS) analyses were compared against a novel lateral flow immunoassay HD testing device (BD HD Check; Becton, Dickinson and Company) to determine the validity of the qualitative assay., Results: We found a 46% reduction in HD contamination after incorporating the CSTD into clinical workflows. Across time points and sites, HD contamination reported by the BD HD Check device was 91% accurate against LCMS/MS and 98% accurate within its limits of detection., Conclusion: Collectively, the evaluated CSTD and lateral flow immunoassay device may help to reduce HD contamination and provide real-time measures of contamination, respectively. We describe a partnership between an academic injury center and three U.S. fire departments to adapt and implement strategies for promoting smoke alarm programs. Each fire department identified the aims and parameters for a new promotion campaign for their smoke alarm programs. Promotion was directed toward residents in each department's catchment area who were eligible for the smoke alarm program. All three departments independently elected to use an automated telephone message to promote their smoke alarm programs. Uptake of smoke alarm installation services ranged between 0.02% and 2% of the calls placed. In Rochester, automated calls were sent to all residential landlines via the city's nonemergency call center; requests for smoke alarms increased by a factor of 7.5 in the month following the campaign. In Grand Rapids, automated calls were sent to 6% of the households served due to the telecommunications infrastructure; because of the limited reach, the overall number of requests remained unchanged, and the number of callers citing the automated calls was less than the number of requests callers reported were motivated by Grand Rapids' existing promotion strategies. In Cloquet, the automated calls were broadcast on a rolling basis to geographic clusters of households; although the number of requests remained unchanged, fire district officials were pleased to reduce driving time between appointments which allowed volunteers to offer more home visit appointments. Automated telephone calls can be a valuable tool for promoting smoke alarm programs, but fire departments should carefully identify how dissemination strategies can best complement existing program efforts., (© The Author(s) 2022. Smoke alarms with lithium batteries have been marketed as long life or "10-Year Alarms." Previous work has drawn into question the actual term of functionality for lithium battery alarms. This article reports on observed smoke alarm presence and functionality in a sample of 158 homes that had participated in a fire department smoke alarm installation program 5 to 7 years prior to the observations. A total of 391 alarms were originally installed in the 158 homes that completed the revisit. At the time of the revisit, 217 of those alarms were working (54%), 28 were nonworking (7%), and 146 were missing (39%). Of the 158 homes that completed the revisit, n = 62 (39%) had all their originally installed project alarms up and working at the revisit. Respondents who reported owning their homes or who reported living in their home for 6 or more years were significantly more likely to maintain all of their project alarms than renters or those living in their homes for 5 or fewer years. Smoke alarm installation programs should consider revisiting homes within 5 to 7 years postinstallation to inspect and replace any missing or nonfunctioning alarms. This study aimed to investigate unwanted radiation exposure of radiosensitive organs during computed tomography (CT) scans of the hand. Furthermore, we investigated the effectiveness of a shield in reducing unwanted radiation exposure. In a prospective observational study, the patients were randomized to receive a shield during the hand CT scan. Two group of patients were further randomized to undergo multi-detector CT (MDCT) or cone-beam CT (CBCT). The radiation dose was measured in the target point and the nearest skin surface of radiosensitive organs by attached the thermoluminescent dosemeter chips. When shielding was performed, the radiation doses had decreased significantly with a mean of 79% (65-91%) in the MDCT group and 88.6% (85-93%) in the CBCT group. In the non-shielded group, the radiation doses to the radiosensitive organs decreased inversely with increasing distance, which means that the closer the target point is, the more unwanted radiation exposure increases. It is important to keep radiosensitive organs as far as possible from the target point during CT scan. In particular, it is necessary to shield the radiosensitive organs closer to the target point. Background: Healthcare workers are at risk of blood and body fluid exposures (BBFE) while delivering care to patients. Despite recent technological advances such as safety-engineered devices (SEDs), these injuries continue to occur in healthcare facilities worldwide., Aims: To assess the impact of an education and SEDs workplace programme on rates of reported exposures., Methods: A retrospective cohort study, utilizing interrupted time series analysis to examine reported exposures between 2005 and 2015 at a 600-bed hospital in Perth, Western Australia. The hospital wards were divided into four cohorts., Results: A total of 2223 records were available for analysis. The intervention was most effective for the first cohort, with significant improvements both short-term (reduction of 12 (95% CI 7-17) incidents per 1000 full-time equivalent (FTE) hospital staff) and long-term (reduction of 2 (CI 0.6-4) incidents per 1000 FTE per year). Less significant or consistent impacts were observed for the other three cohorts. Overall, the intervention decreased BBFE exposure rates at the hospital level from 19 (CI 18-20) incidents per 1000 FTE pre-intervention to 11 (CI 10-12) incidents per 1000 FTE post-intervention, a 41% reduction. No exposures resulted in a blood-borne virus infection., Conclusions: The intervention was most effective in reducing exposures at a time when incidence rates were increasing. The overall effect was short-term and did not further reduce an already stabilized trend, which was likely due to improved safety awareness and practice, induced by the first cohort intervention., (© The Author(s) 2019. In 2016, the EURADOS working group 12 started a project to gather reliable data concerning the behavior of active personal dosemeters (APDs) used by European hospitals in standardized pulsed and hospital fields. This paper concentrates on the tests that deal with the dose rate dependence of APDs in laboratory tests. The dependence on the dose rate was determined for 10 dosemeter types with continuous radiation of a 60Co source and pulsed radiation of the reference radiation facility of Physikalisch-Technische Bundesanstalt. Furthermore, irradiations were done with continuous radiation of a 137Cs source of the Karlsruher Institute of Technology and with continuous X-rays of the two radiation qualites in radiation beams emerging from the X-ray source assembly, RQR5 and RQR8, of Nofer Institute of Occupational Medicine. For the first time, APDs have been systematically investigated under laboratory conditions in a pulsed reference fields according to ISO/TS 18090-1 and the experience gained by these test procedures will be included in the new IEC/TS 63050., (© The Author(s) 2019. Medical personnel performing interventional procedures in cardiology and radiology is considered to be a professional group exposed to high doses of ionizing radiation. Reduction of the eye lens dose limit made its assessment in the interventional procedures one of the most challenging topics. The objective of this work is to assess eye lens doses based on the whole-body doses using methods of computational dosimetry. Assessment included different C-arm orientations (PA, LAO and RAO), tube voltages (80 -110 kV) and efficiency of different combinations of protective equipment used in interventional procedures. Center position at the height of the thyroid gives best estimate of eye lens dose, with spreads of 11% (13%), 13% (17%) and 14% (13%) for the left (right) eye lens. The conversion factors of 1.03 (0.83), 1.28 (1.06) and 1.36 (1.06) to convert whole body to eye lens dose were derived for positions of first operator, nurse and radiographer, respectively. The aim of the study was to compare the absorbed doses and image quality of organ-based tube current modulation (OBTCM) and bismuth shielding of breasts and thyroid against regular tube current modulation in chest CT scan. An anthropomorphic phantom and MOSFET dosemeters were used to evaluate absorbed doses. Image quality was assessed from HU and noise. Relative to the reference scan, the average absorbed dose reduction with OBTCM was 5.2% and with bismuth shields 24.2%. Difference in HU values compared to the reference varied between -4.1 and 4.2 HU in OBTCM scan and between -22.2 and 118.6 HU with bismuth shields. Image noise levels varied between 10.0 to 26.3 HU in the reference scan, from 9.6 to 27.7 HU for the OBTCM scan and from 11.9 to 43.9 HU in the bismuth scan. The aim of this article was to verify the performance of the Mirion InstadoseTM dosemeter under clinical conditions and to compare its response in typical X-ray fields used during interventional and cardiology procedures with the TLD-100, usually used for radiation dosimetry. It was also objective of this study to verify the feasibility of using the InstadoseTM dosemeter response at the chest level for estimation of occupational eye lens dose in cardiology and interventional radiology. Initially the response of the dosemeter was tested using continuous X-ray beams and the results showed that the Instadose dosemeter present a satisfactory behavior of the most important dosimetric properties based on the tests as described in the IEC 62387 standard. The measurements performed in clinical conditions showed that the InstadoseTM dosemeter response was comparable to that of TL dosemeters used in interventional radiology and cardiology procedures and there is a correlation between the eye lens doses and the chest doses measured with the InstadoseTM. Based on the results obtained, we recommend the use of the InstadoseTM dosemeter for purposes of occupational whole-body monitoring of medical staff in interventional radiology and cardiology procedures., (© The Author(s) 2018. To evaluates the eye-lens radiation exposure of workers during medical interventional procedures and surgery in a military hospital as well as of the equine veterinarians. The measures represent the exposure in a normal workload schedule of ninety randomly selected workers over a 3-month period, extrapolated to 1 year. The eye-lens dosemeters were placed near the eye closest to the radiation source (Carinou, E., Ferrari, P., Bjelac, O. C., Gingaume, M., Merce, M. S. and O'Connor, U. Eye lens monitoring for interventional radiology personnel: dosemeters, calibration and practical aspects of H p (3) monitoring. A 2015 review. J. Radiol. Prot. 2015;35(3): R17-R34). Three models of eye-lens dosemeters (Dosilab, Landauer and IRSN) were assessed in term of ergonomics. The annual estimation of eye-lens doses did not reach the annual dose limit of 20 mSv revised by the ICRP, ranged from 0.00 to 18.12 mSv with a mean of 0.96 ± 2.28 mSv. However, these results cannot be representative of a heavy workload or incident situations for which radiation exposure to the eye-lens could exceed this limit. The IRSN dosemeter model was considered the most convenient.

A new proposed design of neutron shielding material-based on the commercial material Borotron UH050 with an addition of Al(OH)3-is evaluated in order to determine if its neutron and gamma shielding properties match those of a reference material, NS4FR. Neutron and gamma dosimetry measurements are performed, as well as neutron spectrometry measurements and Monte Carlo simulations. Negligible differences are found between the materials for neutron shielding, while significant differences are found for gamma shielding. The effect of Al(OH)3 addition to Borotron UH050 is to reduce neutron shielding properties while increasing gamma shielding properties. The resulting material is as efficient as NS4FR for neutron shielding but less efficient for gamma shielding-thicknesses 20% higher are required to match gamma shielding properties of NS4FR. Monte Carlo models of the materials are validated based on the performed measurements of neutron spectra and neutron and gamma ambient dose equivalent.

Galactic cosmic-ray-induced air showers constitute the largest source of radiation exposure for aircrew and passengers. To facilitate the evaluation of effective doses received by this population, a database containing dose rates of various radiation components in the atmosphere was established; the parameterization accounted for altitude, geomagnetic rigidity and solar modulation. Energy spectra of secondary cosmic rays and effective dose rates were calculated using the FLUKA Monte Carlo code. An auxiliary MATLAB program with a graphical user interface was developed to estimate the aviation route dose through interpolation and integration. The dose calculator was benchmarked by comparing its predictions with data from other assessments. Finally, a set of representative flight routes of substantial interest in Taiwan was selected and evaluated.

An urban fire department has been distributing free smoke alarms for more than 30 years. A community-academic partnership was developed to conduct a community intervention trial as part of the fire department's home visiting program. The trial comprised 170 canvassing events held across 12 census tracts; half of the census tracts were assigned to the treatment condition and received prepromotion of the home visit events. The objectives of this analysis were to identify environmental and programmatic predictors of 1) whether someone would be at home at the time of a visit, and 2) if at home, whether the resident would participate. A separate multilevel analysis was conducted to address each objective. The canvassing event served as the first level to account for variation in implementation of the program, with the census tract as the second level. All environmental and program characteristics were included as fixed effects in both models. Throughout 170 events, 8080 eligible residential addresses were visited, of which 3216 had someone at home, and 2197 homes participated in the program. Canvassing events held on weekends and during the evening hours was associated with higher odds of a resident being at home. Canvassing events without rain and held in the treatment census tract areas was associated with higher odds of resident participation. Environmental and programmatic factors can impact the reach of home visiting programs. These findings can contribute to emerging best practices for fire department home visiting programs.

Ultra-high intensity lasers in use are connected with ionizing radiation sources that raise a real concern in relation to installations, personnel, population and environment protection. The shielding of target areas in these facilities has to be evaluated from the conceptual stage of the building design. The sizing of the protective concrete walls was determined using computer codes such as Fluka. For the experiments to be carried out in the facility of the Center for Advanced Laser Technologies (CETAL), both proton beams with the energy of 100 MeV and electron beams with 300 MeV energy were considered to calculate the dimensions of structural shielding and to establish technical solutions fulfilling the radiation protection constraints imposed by the National Commission for Nuclear Activities Control., (© The Author 2016. The goal of this study was to develop a method to quantitatively assess the structural shielding performance of computed tomography (CT) rooms in Switzerland. The method was based on the comparison between calculated maximum allowed ambient dose rate (DR) and measured ambient DR in adjacent spaces. If the measured DR exceeds the calculated maximum allowed value, additional shielding is required; otherwise the shielding is sufficient. The maximum allowed ambient DR was calculated using two different shielding concepts either based on the tube loading of the scanner or on the accumulated dose length product of the scans. The method was validated for 36 spiral CT head protocols. The average ratio of the maximum allowed ambient DR between both concepts was 1.11 (range 0.57-2.14). Among 36 CT rooms, 7 room boundaries were insufficiently shielded. To better understand the distribution of deposited energy of beta and gamma rays according to changes in shielding materials and thicknesses when radionuclides are used for therapeutic nuclear medicine, a simulation was conducted. The results showed that due to the physical characteristics of each therapeutic radionuclide, the thicknesses of shielding materials at which beta-ray shielding takes place varied. Additional analysis of the shielding of gamma ray was conducted for radionuclides that emit both beta and gamma rays simultaneously with results showing shielding effects proportional to the atomic number and density of the shielding materials. Also, analysis of bremsstrahlung emission after beta-ray interactions in the simulation revealed that the occurrence of bremsstrahlung was relatively lower than theoretically calculated and varied depending on different radionuclides., (© The Author 2016. Cardiac catheterisation personnel are exposed to occupational radiation and its health risks. Little data exist regarding the efficacy of radiation-protective equipment from congenital catheterisation laboratories (CLs). The authors retrospectively reviewed data in which CL operators wore a radiation dosemeter during catheterizations on patients of >20 kg. A leaded under-table skirt was present in all cases. Three additional radiation-protective devices were utilised at operator discretion: a top extension to the under-table skirt, a ceiling-mounted shield and a disposable patient drape. Case details, operator position, fluoroscopy time, incident air KERMA in the patient plane (K, mGy) and dose-area product (DAP, µGy·m 2 ) were recorded. A total of 136 catheterizations over 8 months were included. Median operator dose (OpD) was 12 µSv (range 0-930) and indexed to K and DAP to correct for patient factors and case times. Indexed OpD decreased significantly with each additional shield used (14.8 vs. 1.3 nSv µGy -1 m -2 and 124 vs. 14 nSv mGy -1 with one and four shields, respectively, p < 0.001). This trend was not significant with operator at head-of-bed. Combinations that included the ceiling shield had the lowest indexed OpD. The patient drape did not further reduce OpD when all other shields were used (1.3 vs. 2.2 nSv µGy -1 m -2 , p = 0.5; 14 vs. 17 nSv mGy -1 , p = 0.4) and was associated with higher patient exposure indexed to weight and fluoroscopy time (4.5 vs. 3.1 µGy m 2 kg-min -1 , p = 0.009; and 0.51 vs. 0.38 mGy kg-min -1 , p = 0.01). Supplemental radiation barriers can decrease operator-absorbed radiation. A ceiling-mounted shield may provide greatest benefit. The recent publication of the Euratom Directive 2013/59, adopting the reduction of eye lens dose limits from 150 to 20 mSv y -1 , calls for the development of new tools and methodologies for evaluating the eye lens dose absorbed by the medical staff involved in interventional radiology practices. Moreover, the effectiveness of the protective devices, like leaded glasses, which can be employed for radiation protection purposes, must be tested under typical exposure scenarios. In this work, eye lens dose measurements were carried out on an anthropomorphic phantom simulating a physician bound to perform standard interventional neuroradiology angiographic procedures. The correlation between eye lens doses, in terms of Hp(0.07), and the equivalent dose [again in terms of Hp(0.07)] monthly measured with thermoluminescent dosemeters placed above the lead apron at the chest level was studied, in the presence and in the absence of different types of leaded glasses., (© The Author 2015. The new limit of 20 mSv to the lens raises the need for further assessment of the equivalent dose to the lens for nuclear medicine and interventional radiology operators. (a) A measurement campaign was performed in nuclear medicine, (b) a routine monitoring was organised in interventional procedures and (c) the effectiveness of protective eyewear was assessed. In nuclear medicine, for photon fields, the adequacy of Hp(0.07) of dosemeter worn on the trunk is confirmed; with (90)Y, the annual values of Hp(3) measured in therapeutic session are <5 mSv. In interventional procedures, routine monitoring of the dose to the lens must be maintained where the values of Hp(0.07) dosemeter worn on the trunk are higher than one-third of the new limits. Stringent standards of hygiene must be applied in medical institutions, especially at operating blocks or during interventional radiology procedures. Medical equipment, including personal dosemeters that have to be worn by medical staff during such procedures, needs therefore to be sterilised. In this study, the effect of various sterilisation procedures has been tested on the dose response of extremity rings and of eye lens dosemeters in which thermoluminescent (TL) detectors (of types MTS-N and MCP-N, respectively) are used. The effects of medical sterilisation procedures were studied: by chemicals, by steam or by ultraviolet (UV), on the dose assessment by extremity rings and by eye lens dosemeters. Since it often happens that a dosemeter is accidentally machine-washed together with protective clothing, the effect of laundering on dose assessment by these dosemeters was also tested. The sterilisation by chemicals is mostly safe for TL detectors assuming that the dosemeters are waterproofed. Following sterilisation by water vapour, the response of these dosemeters diminished by some 30 %, irrespectively of the period of sterilisation; therefore, this method is not recommended. UV sterilisation can be applied to EYE-D™ eye lens dosemeters if their encapsulation is in black. In the past, the operational quantity Hp(3) was defined for calibration purposes in a slab phantom. Recently, an additional phantom in the form of a cylinder has been suggested for eye lens dosimetry, as a cylinder much better approximates the shape of a human head. Therefore, this work investigates which of the two phantoms, slab or cylinder, is more suitable for calibrations and type tests of eye dosemeters. For that purpose, a typical Hp(3) eye dosemeter was irradiated on a slab, a cylinder and on a human-like Alderson phantom. It turned out that the response on the three phantoms is nearly equal for angles of radiation incidence up to 45° and deviates only at larger angles of incidence. Thus, calibrations (usually performed at 0° radiation incidence) are practically equivalent on both the slab and the cylinder phantoms. However, type tests (up to 75° or even 90° radiation incidence) should be carried out on a cylinder phantom, as also for large angles of incidence the response on the cylinder and the Alderson phantoms is rather similar, whereas the response on the slab significantly deviates from the one on the Alderson phantom., (© The Author 2015. Sometimes, cardiac catheterisation and electrophysiological procedures, diagnostic and interventional, require an anaesthesiological support. The anaesthesiologist receives radiation doses depending on various factors, such as type of procedure and exposure modality, anaesthesiological technique, individual protective devices and operator experience. The aim of this study was to investigate the dose per procedure, the exposure inhomogeneity and the effective dose, E, of a senior anaesthesiologist in the haemodynamic laboratory of Ospedali Riuniti, Bergamo. The dose monitoring was routinely performed with sets of several thermoluminescent dosemeters and an electronic personal dosemeter. The study covered 300 consecutive procedures over 1 y. This study was aimed to assess patient dosimetry in interventional cardiology (IC) and radiology (IR) and radiation safety of the medical operating staff. For this purpose, four major Algerian hospitals were investigated. The data collected cover radiation protection tools assigned to the operating staff and measured radiation doses to some selected patient populations. The analysis revealed that lead aprons are systematically worn by the staff but not lead eye glasses, and only a single personal monitoring badge is assigned to the operating staff. Measured doses to patients exhibited large variations in the maximum skin dose (MSD) and in the dose area product (DAP). The mean MSD registered values are as follows: 0.20, 0.14 and 1.28 Gy in endoscopic retrograde cholangiopancreatography (ERCP), coronary angiography (CA) and percutaneous transluminal coronary angioplasty (PTCA) procedures, respectively. In PTCA, doses to 3 out of 22 patients (13.6 %) had even reached the threshold value of 2 Gy. The mean DAP recorded values are as follows: 21.6, 60.1 and 126 Gy cm(2) in ERCP, CA and PTCA procedures, respectively. Mean fluoroscopic times are 2.5, 5 and 15 min in ERCP, CA and PTCA procedures, respectively. The correlation between DAP and MSD is fair in CA (r = 0.62) and poor in PTCA (r = 0.28). Fluoroscopic time was moderately correlated with DAP in CA (r = 0.55) and PTCA (r = 0.61) procedures. Local diagnostic reference levels (DRLs) in CA and PTCA procedures have been proposed. In conclusion, this study stresses the need for a continuous patient dose monitoring in interventional procedures with a special emphasis in IC procedures. Background: Explosive ordnance disposal (EOD) technicians are often required to wear specialized clothing combinations that not only protect against the risk of explosion but also potential chemical contamination. This heavy (>35kg) and encapsulating ensemble is likely to increase physiological strain by increasing metabolic heat production and impairing heat dissipation. This study investigated the physiological tolerance times of two different chemical protective undergarments, commonly worn with EOD personal protective clothing, in a range of simulated environmental extremes and work intensities, Methods: Seven males performed 18 trials wearing 2 ensembles. The trials involved walking on a treadmill at 2.5, 4, and 5.5 km h(-1) at each of the following environmental conditions, 21, 30, and 37°C wet bulb globe temperature. The trials were ceased if the participants' core temperature reached 39°C, if heart rate exceeded 90% of maximum, if walking time reached 60min or due to volitional fatigue., Results: Physiological tolerance times ranged from 8 to 60min and the duration (mean difference: 2.78min, P > 0.05) were similar in both ensembles. A significant effect for environment (21 > 30 > 37°C wet bulb globe temperature, P < 0.05) and work intensity (2.5 > 4 > 5.5 km h(-1), P < 0.05) was observed in tolerance time. The majority of trials across both ensembles (101/126; 80.1%) were terminated due to participants achieving a heart rate equivalent to greater than 90% of their maximum., Conclusions: Physiological tolerance times wearing these two chemical protective undergarments, worn underneath EOD personal protective clothing, were similar and predominantly limited by cardiovascular strain., (© The Author 2015. We compared the frequency of lens opacities among physicians with and without occupational exposure to ionizing radiation, and estimated dose-response between cumulative dose and opacities. We conducted ophthalmologic examinations of 21 physicians with occupational exposure to radiation and 16 unexposed physicians. Information on cumulative radiation doses (mean 111 mSv) was based on dosimeter readings recorded in a national database on occupational exposures. Lens changes were evaluated using the Lens Opacities Classification System II, with an emphasis on posterior subcapsular (PSC) and cortical changes. Among the exposed physicians, the prevalences of cortical and PSC changes were both 11% (3/21), and the corresponding frequencies in the unexposed group were 44% (n = 7) and 6% (n = 1). For dose-response analysis, the data were pooled with 29 exposed physicians from our previous study. No association of either type of lens changes with cumulative recorded dose was observed. Our findings do not indicate an increased frequency of lens opacities in physicians with occupational exposure to ionizing radiation. However, the subjects in this study have received relatively low doses and therefore the results do not exclude small increases in lens opacities or contradict the studies reporting increases among interventional cardiologists with materially higher cumulative doses., (© The Author 2015. Doses to the eyes of interventional clinicians can exceed 20 mSv. Various protective devices can afford protection to the eyes with the final barrier being protective eyewear. The protection provided by lead glasses is difficult to quantify, and the majority of dosimeters are not designed to be worn under lead glasses. This study has measured dose reduction factors (DRFs) equal to the ratio of the dose with no protection, divided by that when lead glasses are worn. Glasses have been tested in X-ray fields using anthropomorphic phantoms to simulate the patient and clinician. DRFs for X-rays incident from the front vary from 5.2 to 7.6, while values for orientations reminiscent of clinical practice are between 1.4 and 5.2. Results suggest that a DRF of two is a conservative factor that could be applied to personal dosimeter measurements to account for the dose reduction provided by most types of lead glasses., (© The Author 2015. Objectives: The grain and compound feed industry entails inevitable risks of exposure to grain dust and its microbial content. The objective of this study was therefore to investigate task-dependent exposure differences in order to create knowledge basis for awareness and exposure reducing measures in the Norwegian grain and compound feed industry., Methods: A total of 166 samples of airborne dust were collected by full-shift personal sampling during work in 20 grain elevators and compound feed mills during one autumn season and two winter seasons. The personal exposure to grain dust, endotoxins, β-1→3-glucans, bacteria, and fungal spores was quantified and used as individual outcomes in mixed models with worker nested in company as random effect and different departments and tasks as fixed effects., Results: The exposure levels were highest in grain elevator departments. Exposure to endotoxins was particularly high. Tasks that represented the highest and lowest exposures varied depending on the bioaerosol component. The most important determinants for elevated dust exposure were cleaning and process controlling. Cleaning increased the dust exposure level by a factor of 2.44 of the reference, from 0.65 to 1.58mg m(-3), whereas process controlling increased the dust exposure level by a factor of 2.97, from 0.65 to 1.93mg m(-3). Process controlling was associated with significantly less grain dust exposure in compound feed mills and the combined grain elevators and compound feed mills, than in grain elevators. The exposure was reduced by a factor of 0.18 and 0.22, from 1.93 to 0.34mg m(-3) and to 0.42mg m(-3), respectively, compared with the grain elevators. Inspection/maintenance, cleaning, and grain rotation and emptying were determinants of higher exposure to both endotoxin and β-1→3-glucans. Seed winnowing was in addition a strong determinant for endotoxin, whereas mixing of animal feed implied higher β-1→3-glucan exposure. Cleaning was the only task that contributed significantly to higher exposure to bacteria and fungal spores., Conclusion: Cleaning in all companies and process controlling in grain elevators were the strongest determinants for overall exposure, whereas seed winnowing was a particular strong determinant of endotoxin exposure. Exposure reduction by technical intervention or personal protective equipment should therefore be considered at work places with identified high exposure tasks., (© The Author 2015. The purpose of this study is to evaluate the effectiveness of personal radiation shields currently worn in hospital and other diagnostic environments. This study was performed with four different radioisotopes; (18)F, (99m)Tc, (124)I and (131)I. (18)F results showed a decrease in dose with 0.5-mm Pb shielding but the reduction provided does not warrant its use clinically. (124)I testing demonstrated that dose enhancement can occur in greater shield thicknesses. PET isotope (124)I can be adequately shielded using 0.25-mm Pb equivalent aprons but any higher thickness increase the wearer's dose. As a result more shielding does not always equal more protection. The (131)I test showed that no dose reduction occurred, even when tested with up to 1.25-mm Pb equivalent shielding. The purpose of this article is to estimate lens doses using over apron active personal dosemeters in interventional catheterisation laboratories (cardiology IC, neuroradiology IN and radiology IR) and to investigate correlations between occupational lens doses and patient doses. Active electronic personal dosemeters placed over the lead apron were used on a sample of 204 IC procedures, 274 IN and 220 IR (all performed at the same university hospital). Patient dose values (kerma area product) were also recorded to evaluate correlations with occupational doses. Operators used the ceiling-suspended screen in most cases. The median and third quartile values of equivalent dose Hp(10) per procedure measured over the apron for IC, IN and IR resulted, respectively, in 21/67, 19/44 and 24/54 µSv. Patient dose values (median/third quartile) were 75/128, 83/176 and 61/159 Gy cm(2), respectively. The median ratios for dosemeters worn over the apron by operators (protected by the ceiling-suspended screen) and patient doses were 0.36; 0.21 and 0.46 µSv Gy(-1) cm(-2), respectively. With the conservative approach used (lens doses estimated from the over apron chest dosemeter) we came to the conclusion that more than 800 procedures y(-1) and per operator were necessary to reach the new lens dose limit for the three interventional specialties., (© The Author 2015. The number of coronary interventions increased substantially in the recent years. Although of great benefit to patients, these procedures can subject patients to considerable radiation doses. There is a legal framework for patient dose measurements in Croatia during radiological procedures, but in practice, it applies only occasionally. A quality control manual, established at the University Hospital Osijek, was accepted by other major cardiology centres in Croatia; besides checking the technical characteristics of the device, it provides constant measurement and analysis of patient doses in interventional cardiology. It also includes patient examination for radiation skin injuries in case of dose of >2 Gy. The aim of the study was to determine and compare patient radiation doses during cardiological interventions measured within 4 y in four major cardiology centres with the values proposed by the European Commission and other professional bodies. The aim is to estimate the possibility the new annual dose limit for eye lens to be exceeded and to study the impact of protective shield. Radiation exposure to medical personnel was evaluated with EDD-30 dosemeter in positions of operating surgeon, assisting doctor and nurse. At the operator's typical position for diagnosis and treatment of the urinary tract, the lens dose rates were 0.9 mSv h(-1) and 0.06 mSv h(-1) without and with lead shield. At the operator's position typical for percutaneous intervention dose rates were 1.9 and 0.02 mSv h(-1), respectively. At typical workload, the annual eye lens dose to the main operator without protective screen was estimated to be 29 mSv. With lead screen, operator lens dose can be reduced by a factor of 15-95 according to the procedure. This work reports on the use of two different Monte Carlo codes (GEANT4 and MCNPX) for assessing the dose reduction using bismuth shields in computer tomography (CT) procedures in order to protect radiosensitive organs such as eye lens, thyroid and breast. Measurements were performed using head and body PMMA phantoms and an ionisation chamber placed in five different positions of the phantom. Simulations were performed to estimate Computed Tomography Dose Index values using GEANT4 and MCNPX. The relative differences between measurements and simulations were <10 %. The dose reduction arising from the use of bismuth shielding ranges from 2 to 45 %, depending on the position of the bismuth shield. The percentage of dose reduction was more significant for the area covered by the bismuth shielding (36 % for eye lens, 39 % for thyroid and 45 % for breast shields)., (© The Author 2015.
