1. The effects of irisin on the rat thoracic aorta: a histological study

Abstract

Irisin, a polypeptide hormone that is released from skeletal muscle in response to exercise found to improve endothelial functions, protect against endothelial injuries and change blood pressure which also affected blood vessels. The aim of this study is to study the histological changes of the rat thoracic aorta in response to irisin injection. Thirty rats were used. Then divided into two groups; the control group without irisin injection, and the irisin injected group was subdivided into four subgroups with different irisin concentrations (20, 40 and 160 nM, respectively) twice a week for four weeks, the control group and each subgroup consisted of 6 rats each. After 4 weeks all rats were sacrificed, and the descending thoracic aorta was treated for histological evaluation. Sections were stained with Hematoxylin and Eosin (H and E) and orcein stains. Morphometric measurement included: intima-media thickness (IMT), number of elastic lamellae and number of smooth muscle cells nuclei. Histological study has shown that intraperitoneal injection of different concentrations of irisin (20, 40 and 160 nM) in rats has increased intima-media thickness, number of smooth muscle cell's nuclei, and increase the number of elastic lamellae in media layer of the thoracic aorta in a dose dependent manner. Irisin has significantly affected the morphology of the wall of the rat thoracic aorta indicating a role for irisin in influencing the growth factors of the thoracic aorta walls and activate smooth muscle cells in the thoracic aorta layers.

Abstract

الملخص: أهداف البحث: إن معرفة الأجزاء التشريحية أثناء العمليات الجراحية يؤدي إلى تقليل المضاعفات، ومن هنا كان الهدف الرئيس للدراسة هو معرفة الأجزاء التشريحية التي يعتبرها أطباء الاختصاص مهمة؛ ويجب أن يكون المقيم المتدرب في مستوى السنة الثالثة قادرا على تحديدها أثناء العمليات الجراحية. بالإضافة إلى ذلك قمنا بدراسة العوامل التي قد تؤثر على آراء الأطباء الاختصاصيين. طرق البحث: شملت الدراسة أطباء اختصاصيين في التوليد والجراحة النسائية، الذين يعملون في مستشفيات يوجد فيها برامج تدريب للأطباء المقيمين وذلك في الفترة ما بين ١/٢/٢٠١٩ و٣٠/١٠/٢٠١٩. وطُلب من أطباء الاختصاص تقييم أهمية الأجزاء التشريحية التي يجب أن يكون المقيم المتدرب في السنة الثالثة قادرا على معرفتها أثناء العمليات الجراحية. ثم تم تحليل الاستجابات بناء على عمر الطبيب الاختصاصي وجنسه ونوع الممارسة إذا كانت في مستشفى عام أو خاص وعدد سنوات الخبرة وعبء العمل الجراحي. النتائج: استجاب للدراسة ١٦٥ اختصاصيا، وذلك بمعدل استجابة ٦٩.٣٪. وكان متوسط العمر ٤٦.١ سنة، ومتوسط عدد سنوات الخبرة ١٣.٤. بالإضافة إلى ذلك؛ صنف ٨٦.٦٪ من الاختصاصيين جميع الأجزاء على أنها ''أكثر أهمية''. لم تكن الأهمية حسب تصنيف االاختصاصيين مرتبطة بالجنس أو عدد سنوات الخبرة أو عبء العمل الجراحي. كما تم تصنيف أهمية ٦٣٪ من الأجزاء التشريحية ''أعلى'' من قبل الاختصاصيين المبتدئين مقارنة بالاختصاصيين الأكثر خبرة. الاستنتاجات: المعرفة بالتشريح الجراحي مهمة للتدريب في برامج الإقامة في التوليد والجراحة النسائية. إن تصور الاختصاصيين لأهمية الأجزاء التشريحية المختلفة يعكس فهمهم لمتطلبات التدريب. سلطت نتائجنا الضوء على الأجزاء التشريحية المهمة التي من المتوقع أن يعرفها الأطباء المقيمون في السنة الثالثة من التدريب أثناء العمليات الجراحية. قد يؤسس هذا البحث مدخلا لبحوث مستقبلية تشمل جميع سنوات التدريب وتعمل على تشكيل مرجعا لماهية المعرفة التشريحية المطلوبة لكل سنة تدريب. Abstract: Objectives: Intraoperative identification of anatomical structures can potentially reduce the risk of surgical complications. This study aims to report specialists' perspectives about the anatomical structures that third-year residents should be able to identify during surgical operations. In addition, the factors which may influence specialists' opinions are discussed. Materials and methods: This qualitative cross-sectional study was conducted on obstetricians and gynaecologists between 1/2/2019 and 30/10/2019. The specialists practising in a hospital with a residency programme were included, and were asked to rate the importance of structures that a third-year resident should be able to identify during operations. We performed a comparison of responses based on specialists' age, gender, practice type, years of experience, and surgical workload. Results: One hundred and sixty-five specialists were recruited with a response rate of 69.3%. The mean age of respondents was 46.1 years, and they had a mean experience of 13.4 years. Furthermore, 86.6% of specialists rated all the anatomical structures as “more important”. The importance of surgical structures, as rated by specialists, was not related to gender, years of experience, or surgical workload. The importance of 63% of the anatomical structures was rated higher by junior specialists than senior specialists. Conclusion: Knowledge of anatomical structures is vital for gynaecologic residency training. Specialist's perceptions of the importance of various anatomical structures reflect their understanding of the training requirements. Our results highlighted the important anatomical structures that third-year residents are expected to identify during surgical operations. Future research may establish a reference for the core anatomy knowledge essential for each training year.

3. Ameliorative effects of curcumin and caffeic acid against short term exposure of waterpipe tobacco smoking on lung, heart and kidney in mice

Abstract

This study aims to evaluate the chemopreventive activity of two antioxidants (curcumin [CUM] and caffeic acid [CAF]), focusing on how these antioxidants could reduce cytotoxicity induced by short term secondhand exposure of waterpipe tobacco smoking. Forty-eight adult male BALB/c albino mice were equally divided into four groups. Antioxidants were delivered intraperitoneally, and the exposure to waterpipe smoking (WPS) was performed using a smoking machine. This experiment lasts for 14 consecutive days. Serum were collected from mice before dissection to quantify the activity of some liver enzymes, kidney function tests and proinflammatory cytokines. Lung, heart, and kidney were isolated and processed for light microscopy technique. Parallel treatment of CUM or CAF along with exposure to WPS showed less inflammation, less vacuolized, and more inflated alveoli, less deteriorations in cortex part of kidney, and less disintegration of cardiac myofibers in comparison to waterpipe only. Besides, CUM and CAF significantly reduced the activity of aspartate aminotransferase and proinflammatory cytokines. CUM and CAF were found to have anti-inflammatory and ameliorative effects against the cytotoxicity induced by exposure to waterpipe tobacco smoking, and CUM showed better chemopreventive activity than CAF.

Abstract

SUMMARY: Thorough knowledge of splenic artery course and morphology may help clinician to provide better practice. This Study aims at finding out if there was a relationship between splenic artery tortuosity index and age, sex, Body Mass Index (BMI) and abdominal cavity diameters. Routine abdominal Computerized Tomography (CT) scan images were retrospectively analyzed for 219 patients. Splenic artery tortuosity index was calculated. Abdominal cavity diameters were measured. Age, sex, and BMI were recorded. Splenic artery straight length (x) mean was 9.41 cm (SD 1.33). Splenic artery tortuous length mean was 15.15 cm (SD 3.31). Splenic artery tortuosity index mean was 1.63 (SD 0.36). Pearson correlation coefficient for Splenic artery tortuosity index vs. age was: 0.02 (P value 0.80). Splenic artery tortuosity index for females vs. males were 1.70 vs. 1.57 (P value 0.01). Pearson correlation coefficient for Splenic artery tortuosity index vs. BMI was 0.02 (P value 0.75). Pearson correlation coefficient for Splenic artery tortuosity index vs. abdominal cavity diameters were: Anterior-Posterior (AP) diameter -0.01 (P value 0.88) and transverse diameter 0.00 (P value 0.98). There may be a relationship between splenic artery tortuosity and female sex, but not with age, BMI and abdominal cavity diameters (AP and Transverse).

Abstract

It is well known that the most common sites for venous access are the superficial veins of the upper limb, particularly dorsal metacarpal veins and median cubital vein. Although dorsal metacarpal veins are the first choice for venous cannulation, there is scarce information about their anatomic variation. Hence, detailed anatomical information about these veins will improve the anatomic knowledge of the health care providers. Subsequently, this study was designed to study the dorsal metacarpal veins and to determine the most prominent dorsal metacarpal vein. A cross sectional study of 402 subjects (804 hands), was prepared to study the superficial veins on the dorsum of the hand among Jordanian students and staff of one of the major governmental medical colleges in Jordan, by using infrared illumination system. The obtained data was analyzed according to sex, sidedness, and handedness. Six locations of the most prominent dorsal metacarpal veins were identified. There was a significant relation between both females and males and the most prominent dorsal metacarpal vein (P=0.01). For the first time this study identified the most common location of the most prominent dorsal metacarpal vein in the fourth intermetacarpal space.

Abstract

Our aim was to investigate the variation in the vertebral levels of the origins of the celiac artery, superior and inferior mesenteric arteries, paired renal arteries, and common iliac arteries. We conducted a retrospective imaging study in a large public secondary hospital on a nonrandom sample of 227 participants. We consecutively included adult patients who had undergone computed tomography angiography of the abdomen and excluded patients with a history of any vertebral abnormality or whose images revealed evidence of a vertebral abnormality or a congenital anomaly of any of the branches of the abdominal aorta. The primary outcome was the frequency distribution of the vertebral levels of the landmarks. The secondary outcomes were the intercorrelations of the vertebral levels of the landmarks and their relationships with age, sex, weight, height, and body mass index. The celiac artery originated at T11/T12-L1/L2, followed by the superior mesenteric artery at T12-L2, the paired renal arteries at T12/L1-L2/L3, the inferior mesenteric artery at L2-L4, and the common iliac arteries at L3-L5. The vertebral levels of the landmarks were positively intercorrelated and stronger between proximate pairs. In addition, the vertebral levels of the landmarks were related to age, but not sex, weight, height, or body mass index. The intercorrelations suggest that a considerable proportion of the variation is accounted for by 'trickle-down' variation; variation in the vertebral level of a proximal landmark results in variation in the vertebral level of the immediate distal landmark. The overarching parameter remains unidentified.

Abstract

Background: The cephalic vein is one of the most distinguished superficial veins of the upper limb. Its clinical value lies in venous access. There is little known about the variation of its formation in relation to the anatomical snuffbox. Hence, anatomical variants in the origin of the cephalic vein are important in clinical practice. Subsequently, this study was designed to examine the variation of the cephalic vein formation in relation to the anatomical snuffbox. Materials and methods: A cross-sectional study of 438 subjects (722 hands), was prepared to study the cephalic vein among Jordanian students and staff of one of the major governmental Medical College in Jordan, by using infrared illumination system. The obtained data was analysed according to; gender, sidedness, and handedness. Results: Four sites for the formation of the cephalic vein in relation to the anatomical snuffbox were found. There was a significant relation between gender and sidedness, and the sites of formation of the cephalic vein (p < 0.0001 and p = 0.048, respectively). Conclusions: For the first time this study identified different sites for the formation of the cephalic vein in relation to the anatomical snuffbox. However, regardless of its sites of formation, the cephalic vein was running in 98% of the examined hands in the anatomical snuffbox.

Abstract

The dorsal metacarpal veins are frequently cannulated. Cannulation success is determined by several variable anatomic features. The objective of this study is to classify, for the first time, the anatomic variants of the dorsal metacarpal veins. In this cross-sectional study, 520 university students and staff were conveniently recruited. The dorsal metacarpal veins in 1040 hands were studied. Venous visibility was enhanced by either tourniquet application or near-infrared illumination. Variant patterns of the dorsal metacarpal veins were classified. The final analysis included 726 hands, for an exclusion rate of 30 %. Eight pattern types were identified. Three anatomic features informed the variation. Bilateral symmetry of the dorsal metacarpal veins was present in 352 participants (83 % of the total). The overall frequency distribution of variants in both hands was similar (P = 0.8). Abstract

The accessory navicular (AN) is an accessory ossicle anatomically located on the medial side of the foot, proximal to the navicular and continuous with the tibialis posterior tendon. It is occasionally a source of pain and local tenderness. Knowledge of the AN and its morphological variations can help identify the source of a patient's symptoms and prevent misinterpreting them as fractures. Foot radiographs from 1,240 patients who presented in two centers with chronic foot pain, or persistent pain developed after trauma, were retrospectively reviewed to determine the incidence and variations of the AN in relation to gender. The AN was found in 20.9% (259/1240). Among 259 feet with AN, Type 1 was identified in 25.4% (66/259), Type 2 in 42.4% (110/259) (20.0% (52/259) Type 2 A and 22.4% (58/259) Type 2B), and Type 3 in 32.0% (83/259). After 13 patients with incomplete medical records had been excluded, the remaining records showed that foot pain was associated with an AN in 10.6% of patients (26/246). In 1.2% of cases, two additional ossicles were found proximal to the navicular, possibly the result of multiple ossification centers that did not unite at the time of development. Patient symptomatology was related to the presence of an AN in 2% of patients with chronic foot pain. The AN could vary morphologically. Abstract

Objective Previous studies have identified that abnormal thyroid hormonal status is associated with worse prognosis especially in coronary artery disease (CAD). The present study further explores whether thyroid hormones associated with size of myocardial injury and extent of CAD in patients with acute coronary syndrome (ACS). Methods In this study, we enrolled 120 consecutive patients (52% male, mean age 54.2±6.4 years) who were admitted to ICU with ACS and having coronary angiography. All patients underwent testing for thyroid function status [thyroid stimulating hormone (TSH), free triiodothyronine (FT3) and free thyroxine (FT4)], cardiac troponin I (cTnI), cardiac enzymes, C-reactive protein (CRP). Risk profile, clinical characteristics and angiographic results (Gensini score) were also analyzed. Results 43.3% of patients presented with STEMI, 33.3% with NSTEMI and 23.3% unstable angina. CK-MB, troponin and CRP were significantly higher in STEMI compared with NSTEMI group (P Conclusions In ACS, the lower thyroid hormones levels are associated with larger myocardial injury, more severe CAD and higher rate of complications especially during their hospital course.

Abstract

Introduction Acute ischemic cerebrovascular stroke (CVS) can be the first clinical manifestation of atrial fibrillation (AF), that is why large-scale screening programs for AF are currently implemented especially in individuals more than or equal 65 years old. Yet, to date, cost-effectiveness data have relied on assumptions of stroke rates observed in patients with established AF, while the true incidence rates of undiagnosed AF presenting as a stroke remain unknown. Purpose To estimate the incidence of patients with no history of AF who are presented with acute ischemic CVS and are found to be in AF at the time of presentation or developed atrial fibrillation during the hospital stay. Methods A retrospective analysis of all patients admitted with acute CVS to a tertiary care stroke specialized center in the period from 01/01/2014 till 31/12/2017. Ischemic CVS is confirmed by either multislice computed tomography (MSCT) or magnetic resonance imaging (MRI) of the brain. AF is documented by electrocardiography (ECG) that is made at admission or during hospital stay. Patients with history of AF on rate or rhythm-control therapy were excluded from the study. Results Of the 3299 patients admitted by acute ischemic CVS, 707 (21.43%) patients had history of AF and they were on medical rate or rhythm control therapy and thus were excluded from the study. Of the remaining 2592 patients eligible for the study, 1666 (64.27%) were males with a mean age of 56.06 years (±16.01). Regarding ECG, 2313 (89.24%) patients were in sinus rhythm and other non-AF rhythms and 211 (8.14%) presented with AF rhythm and 68 (2.62%) developed AF during their hospital stay with a cumulative incidence of (10.76%). Abstract

PANDA (Particles And Non-Destructive Analysis) is measuring system developed for non-destructive analysis of samples for safety, security and safeguards. The capabilities of the PANDA device were expanded by the addition of a thick silicon detector for β particle detection. The upgraded device can now be used for position-sensitive α − γ and β − γ coincidence measurements of various kinds of radioactive samples. The capability of the PANDA device in using the β − γ coincidence technique was tested using a mixed source of 134Cs and 226Ra. In addition, the ability of PANDA to locate nuclides emitting β particles from samples was tested using a combined sample containing a mixed 134Cs and 226Ra source and a pure 226Ra source placed a few centimeters apart from each other. The construction, commissioning and testing of the upgraded PANDA device is discussed.

Abstract

En el Departamento de Radiologia del Hospital de la Universidad de Jordania, Amman, Jordania, se llevo a cabo un estudio prospectivo entre el mes de julio de 2001 al mes de julio del 2015. Durante ese periodo se descubrieron cinco casos de vena cava inferior doble (VCID) en una cohorte de 7722 pacientes (3861 hombres y mujeres 3861, de 49,5 ± 16,9 anos, con un rango de edad de 16-78 anos). Los casos fueron diagnosticados por medio de venografia por tomografia computada espiral con contraste (TCV) y confirmados por medio de venografia por estudio tridimensional turbo. La mayoria de los pacientes (3166, 41 %) fueron remitidos para estadificacion y seguimiento de tumores malignos. Se presentaron complicaciones postoperatorias en 1777 pacientes (23 %), dolor abdominal no especificado en 1467 (19 %), evaluacion preoperatoria en 849 (11 %) y traumatismo en 463 pacientes (6 %). La venografia por resonancia magnetica (RMV) mostro una mayor sensibilidad, precision diagnostica, y resultando no invasiva para la evaluacion de la vena cava inferior (VCI). RMV es una modalidad mas util, no invasiva para la evaluacion de la VCI. VCID es una anomalia frecuente, encontrandose en nuestro estudio una incidencia de 0,064 %. Ademas se realizo una revision de la literatura, la embriogenesis, y la importancia de esta anomalia. Tambien, se proporcionaron cifras de muestras de los casos relevantes.

Abstract

The Bacillus thuringiensis δ-endotoxins (Bt toxins) are widely used insecticidal proteins in engineered crops that provide agricultural, economic, and environmental benefits. The development of insect resistance to Bt toxins endangers their long-term effectiveness. Here we have developed a phage-assisted continuous evolution selection that rapidly evolves high-affinity protein-protein interactions, and applied this system to evolve variants of the Bt toxin Cry1Ac that bind a cadherin-like receptor from the insect pest Trichoplusia ni (TnCAD) that is not natively bound by wild-type Cry1Ac. The resulting evolved Cry1Ac variants bind TnCAD with high affinity (dissociation constant Kd = 11-41 nM), kill TnCAD-expressing insect cells that are not susceptible to wild-type Cry1Ac, and kill Cry1Ac-resistant T. ni insects up to 335-fold more potently than wild-type Cry1Ac. Our findings establish that the evolution of Bt toxins with novel insect cell receptor affinity can overcome insect Bt toxin resistance and confer lethality approaching that of the wild-type Bt toxin against non-resistant insects.

Abstract

Objectives To study any possible correlation between blood lead levels and some oxidative stress parameters of selected groups of lead-exposed automobile occupational Jordanian workers. Material and Methods Blood lead levels were determined for a total of 90 male automobile workers aged within the range of 25–45 years old along with the group of 20 agematched healthy males control. To get an idea about the antioxidant status of controls and lead-exposed workers, and to estimate the oxidative stress caused by exposure to lead, we measured the hydrogen peroxide (H 2 O 2 ) concentration, superoxide dismutase (SOD) activity, lipid peroxidation as malondialdehyde (MDA) and total antioxidant capacity of controls and workers. Results The study showed that lead levels in the case of workers were approximately 4–5 times as high as in controls 14.5–21 μg×dl –1 vs. 4.3 μg×dl –1 , respectively. There was a significant decrease (16–25%) in the level of plasma reduced glutathione and 21–33% decrease in total antioxidant capacity (TAOC) in all worker groups, as compared to controls. The results showed that the concentration of malondialdehyde (MDA) in plasma was higher (120–333%) in the case of workers than controls being the highest in automobile electronics and the lowest – in mechanics. Furthermore, there was 149–221% increase in hydrogen peroxide (H 2 O 2 ) concentration, and 26–38% increase in SOD activity in the case of workers compared to the control group. Conclusions There is a strong evidence for the associations between occupational lead exposure and various markers of oxidative stress in Jordanian automobile occupational workers. Abstract

The Very Energetic Radiation Imaging Telescope Array System (VERITAS) represents an important step forward in the study of extreme astrophysical processes in the universe. It combines the power of the atmospheric Cherenkov imaging technique using a large optical reflector with the power of stereoscopic observatories using arrays of separated telescopes looking at the same shower. The seven identical telescopes in VERITAS, each of aperture 10 m, will be deployed in a filled hexagonal pattern of side 80 m; each telescope will have a camera consisting of 499 pixels with a field of view of 3.5 deg VERITAS will substantially increase the catalog of very high energy (E > 100GeV) gamma-ray sources and greatly improve measurements of established sources., 44 pages, 16 figures

El objetivo fue determinar si el sexo, edad, hipertension, tabaquismo y la diabetes alteran el diametro transversal (DT) y area transversal (AT) de la parte ascendente de la aorta (AA), parte descendente de la aorta (AD) y tronco pulmonar (TP), medidos por angiografia por tomografia computadorizada (ATC). Examenes de ATC de 100 pacientes de 49,5±16,9 anos (rango 16­78 anos) fueron seleccionados entre enero del ano 2009 a mayo del ano 2011 por el Departamento de Radiologia, Hospital de la Universidad de Jordania, Facultad de Medicina de la Universidad de Jordania, Amman, Jordania para una evaluacion avanzada del DT y AT de la AA, AD y TP. Las mediciones se realizaron en el plano axial en el margen superior de las seis vertebras toracicas. Los pacientes fueron divididos en tres grupos segun edad. Se determino la existencia de significancia estadistica de los diferentes parametros entre los grupos etarios. La evaluacion de las razones tambien fueron consideradas. Abstract

Dentistry is a profession demanding physical and mental efforts as well as people contact, which can result in burnout. The level of burnout among 307 clinical dental students in 2 Jordanian universities was evaluated using the Maslach Burnout Inventory survey. Scores for the inventory's 3 subscales were calculated and the mean values for the students' groups were computed separately. Dental students in both universities suffered high levels of emotional exhaustion and depersonalization. The dental students at the University of Jordan demonstrated a significantly higher level of emotional exhaustion than their counterparts at the Jordan University of Science and Technology.

Abstract

Background Dental students are subjected to many stresses that may affect their achievement. The purpose of this study is to assess the mental and physical health of dental students in two Jordanian Universities. Methods A total of 265 dental students and 228 non-dental students from two Jordanian Universities participated in the study. They completed the survey questionnaire and their responses were used in calculating the 0-100 scores for the eight health concepts by linear transformations of scores. The ANOVA test was used to determine the significant differences among the student groups, and Tukey test was used for multiple comparisons among groups. All tests were carried out at 95% confidence level. Results The results indicated that the dental students of the Jordan University of Science and Technology were of better health than their counterparts at the University of Jordan. The health scores attained by the dental students of the two universities were less than those of non-dental students of the same age. Conclusions The physical and more significantly the mental health components of dental students should receive more attention, and further work is needed to detect the possible causes and find potent remedies for this problem. Abstract

Objectives: To estimate normal linear dimensions and volume of spleen in Jordanians using ultrasonography, and to correlate splenic volume with age and body parameters: height, weight, body surface area (BSA), and body mass index (BMI). Methods: A prospective pilot study was conducted on 205 volunteers (115 males and 90 females) not known to have any conditions likely to be associated with splenomegaly. The study was performed at the Radiology Department, Jordanian University Hospital, Amman, Jordan, between December 2013 and August 2014. All linear dimensions of spleen were measured, and splenic volume (index) was calculated using the standard prolate ellipsoid formula (length × width × depth × 0.523). The splenic volume was then analyzed with age and body parameters using the Pearson’s correlation coefficient. Results: The mean (± SD) splenic dimensions were 10.72±1.37 cm in length, 7.40±1.52 cm in width, 4.40±1.47 cm in depth, and 184.15±79.56 cm 3 in volume. Men had larger spleens than women ( p less than 0.0001). Age had no significant effect on spleen volume (r=0.11, p =0.12). There was a significant moderate positive correlation ( p less than 0.0001), using Pearson’s correlation coefficient, between the spleen volume, and other parameters (height, weight, BSA, and BMI), with correlation coefficients exceeding 0.3. Abstract

We reported previously that skin flaps transplanted to the oral cavity in reconstructive surgery for oral cancer frequently acquired the gross appearance of buccal mucosa. The changes were shown to be reactive in nature. The "changed" flaps generally had a heavier infiltration of leukocytes in the dermis and appeared to have thicker epithelium. The present study quantifies these parameters, as well as the numbers of intraepithelial leukocytes. The flaps that had acquired the gross appearance of oral mucosa had significantly thicker epithelium, larger numbers of dermal leukocytes, and more intraepidermal inflammatory cells per unit length than flaps that retained the gross appearance of thin skin. No correlation was found between these changes and radiotherapy.

Abstract

Central venous access is important in both assessment and treatment of the patient. In modern clinical practice, a percutaneous approach is preferred. The well-established subclavian and internal jugular vein catheterization techniques, however, still carry the risk of major acute complications. In this article we describe a trial study of a percutaneous brachiocephalic vein catheterization technique conducted on a total of 74 cadavers undergoing autopsy. Relying on constant and easily recognizable anatomical landmarks, we performed a total of 128 catheterizations of the right and left brachiocephalic veins. The success of catheterization was ascertained by regional dissection. Our results show very high reproducibility with a success rate of about 97% on the first attempt of catheterization on both right and left sides. Equally important, collateral injury of neighboring structures was consistently absent. We propose the introduction of this rather overlooked approach into clinical practice. Clin. Abstract

Radial forearm skin flaps transplanted into the oral cavity in patients undergoing treatment for intra-oral tumors frequently acquire a gross appearance resembling that of buccal mucosa. This article reports studies of biopsies of the flaps in 53 patients, of which 21 showed changes at some stage in the follow-up period. The project aims to determine whether 1) the change in the epithelium to a mucosal form is genuine or reactive, 2) a creeping replacement of epidermis results from the surrounding buccal epithelium, 3) alterations in the epidermis could be associated with changes in the dermis, 4) the change is caused by radiotherapy. Examination of the biopsies by resin histology and transmission electron microscopy shows that the changes may be reversible: it suggests that they are reactive in nature, and related to the degree of dermal inflammation, rather than to the effect of radiotherapy. The dermis persists as such and is not replaced by buccal lamina propria.

Abstract

Bone and soft tissue infection in the foot of diabetic patients is a well-described issue in the literature. A sound anatomical knowledge of the foot anatomy and compartments is mandatory to understand the mechanisms of infection spread. We describe four cases of diabetic foot infection complicated by long ascending infection. All did not respond initially to antibiotic treatment and the usual surgical debridement and were cured only after excision of the infected tendons. We highlight a rare but serious complication of the diabetic foot disease not commonly seen by the surgical community. We hope that this report raises the awareness of this condition so that a prompt diagnosis is made and appropriate treatment started, thereby reducing the risk of major lower limb amputations.

Abstract

In this study scanning electron microscopy is used to document the surface features of superficial cells of buccal mucosa and to evaluate the usefulness of examination of oral swabs in monitoring radiotherapy in the mouth. Biopsy samples of buccal mucosa from 5 patients and oral swabs from 8 volunteers and 7 patients were examined. The surfaces of the buccal epithelial cells in healthy subjects were found to have a variable morphology. Cells nearer the surface showed a pattern of parallel microplicae or microvilli, while less mature cells showed more complex patterns such as irregular microplicae, microplicae forming a maze-like pattern or a mixture of microplicae and microvilli. Adjacent celis frequently exhibited different patterns, and superficial and deep surfaces of the same cell consistently had different appearances. In particular, all the appearances described by Robertson et al.: J. Submicrosc. Cytol., 19:515–521, 1987, following radiotherapy were noted in the normal subjects in the present study. The findings emphasize the need for caution in attributing particular cell-surface patterns to the effect of specific doses of radiation and suggest that scanning electron microscopy of oral swabs is unlikely to be useful in monitoring the progress of radiotherapy. © 1994 Wiley-Liss, Inc.

Abstract

Objectives: To determine the association between preterm birth (PTB) and maternal oral diseases during pregnancy. Subjects and Methods: This prospective study was performed by the neonatal and dental departments at Jordan University Hospital. The study included 100 women who gave birth to preterm singleton infants (born less than 37 complete weeks from last menstrual period) between January and July 2009. The control group included an equal number of women who delivered singleton, full-term infants on the same day or the day after the women in the study. The mothers’ demographic data were collected using a questionnaire, and an oral examination was conducted for each participant. Statistical analysis was performed using SPSS for Windows release 16.0 (SPSS Inc., Chicago, Ill., USA). Factors related to PTB were studied in univariate and multivariate logistic regression analyses. Results: Significantly higher DMFT (decayed, missing, filled teeth) index scores, Silness and Loe plaque index scores and Mühlemann tooth mobility index scores were associated with PTB. Mothers who did not have prepregnancy dental checkups had a significantly higher incidence of PTB. Fewer mothers in the PTB group visited dentists during their last pregnancy compared with controls. Conclusions: Oral health, especially healthy periodontium, is one reliable indicator for predicting a safe pregnancy outcome.

The dental students of the University of Jordan demonstrated a significantly higher (p < 0.05) level of emotional exhaustion than their counterparts in the Jordan University of Science and Technology. Conclusions The findings indicated that dental students in the Jordanian universities presented considerable degrees of burnout manifested by high levels of emotional exhaustion and depersonalization. Studies targeting students health and psychology should be carried out to determine the causes of burnout among dental students. The curricula of the dental schools in the two universities should be accordingly improved to minimize burnout among the students. Objectives: To evaluate the self-rated quality of life (QoL) and benefits associated with the use of bone-anchored hearing aids (BAHAs) and to identify potential improvements in comparison to the previous conventional hearing aids. Study Design: Cross-sectional. Setting: BAHA program in tertiary referral center. Patients: One hundred fifty-two adults who underwent BAHA procedure for more than 6 months. Intervention: Rehabilitative. Main Outcome Measure: Results of the Entific Medical Systems questionnaire. Results: One hundred seventeen responses (77%) were obtained. Overall satisfaction was excellent (median = 9). BAHA was rated very good/excellent in single-person conversation (85%), but fared less well in group situation (45%). BAHA was reported as better than the previous aid by more than 50% of patients within each subcategory where applicable (handling, 64%; comfort, 56%; cosmetic, 60%; sound, 68%; infections, 75%). Of those who reported improvement of ear infections, one quarter remained unsatisfied of improvement in QoL. Patients who used BAHA for 3 years or less (n = 45) had a similar rating to sound quality and conversation as those who used it longer (n = 72). Surprisingly, a subset of patients (n = 8) with poor bone-conduction thresholds (>45 dB) remained satisfied with the device (median = 8). Conclusions: The BAHA can improve QoL and hearing outcome with full acclimatization to the device being achievable within a reasonable amount of time. As with conventional aids, sound quality in group situations remains poor with the BAHA. Our results provide a predictive value during preoperative counseling of patients.

Abstract

Healing of large diabetic foot ulcers may be difficult, particularly if the blood supply and chronic infection do not allow primary suturing. Split-thickness skin graft is a simple reconstructive technique used to close large wounds. Phenytoin is known to promote healing mainly by increasing granulation tissue formation. The effectiveness of topical phenytoin in wound-bed preparation (WBP) for split thickness skin grafting has been examined in 16 patients with large diabetic foot ulcers. All patients were treated with standard wound bed preparation including debridement of necrotic tissue. Topical phenytoin (10 % w/w ointment) was applied for 2-8 weeks prior to performance of autografting. Clinical and histologic evaluations were performed. The graft survival was 100 percent In twelve patients, 80-90 percent in three patients take and 60 percent in one patient. Neither local nor systemic side effects were observed. The authors conclude that phenytoin ointment is a safe and efficacious treatment to enhance the survival of split-thickness skin grafts in large chronic diabetic ulcers.

Abstract

The paper addresses traffic management strategies in multi-service corporate communication networks. In particular, it proposes a unified congestion control scheme for mixed X.25/frame relay networks which meets users' requirements of effectiveness, fairness, and implementation efficiency. Key elements of the design include early detection of impending congestion by the various network resources, explicit notification of congestion information to the affected network services, and the use of adaptive rate and window control mechanisms by the various services to limit traffic admission into the network during overload. A comprehensive simulation test-bed was constructed for evaluating and tuning the proposed algorithms, and the results were verified against live networking equipment. Simulation results confirm the success of the congestion control strategy in maximizing network throughput, and minimizing users' packet loss rate and end-to-end delay under a wide range of congestion situations. The described algorithms have been successfully incorporated in the design of large multi-service data communication networks. >

Abstract

Background The empirical use of antibiotics in children with suspected meningitis is a common clinical practice worldwide that often leads to drug resistance. It is difficult to clinically differentiate bacterial when compared to viral meningitis until a culture study ofcerebral spinal fluid (CSF) Or CSF viral PCR study is performed. A ‘wait and see’ approach may lead to undesirable outcome. Bacterial Meningitis Score (BMS) is a tool that was developed to help physicians to differentiate between viral versus bacterial meningitis. Aim To determine the usefulness if any of BMS for discriminating between bacterial or viral meningitis is young children. Methodology We retrospectively reviewed the charts of all children (from birth till 14 years old) who were admitted with the diagnosis of meningitis to Hamad general hospital in last 2 years period. Result A total 120 patients (68% boys) with confirmed meningitis were reviewed during the study period. The mean age was (6.3±2.7 year). The majority of patients 112 (93.3%) had viral type meningitis while the remaining had bacterial meningitis (Strep Pneumia, Neisseria meningitis and H. Influenza). The sensitivity of BMS tool revealed a sensitivity of 100% (95% CI: 75.1 to 100.0) and a specificity of 60.9% (95% CI:50.1–69.7). Conclusion Our study shows that BMS is a simple, easy and highly sensitive tool that can differentiate bacterial from viral meningitis and it is use may limit the use of unnecessary antibiotic s and hospitalizations.

Abstract

Background and Aim: Diabetes was associated with twoto threefolds more with Hepatocellular carcinoma (HCC). We investigate the impact of comorbidity of diabetes on HCC presentation, progression and treatment outcome. Patients and Methods: Over 3 years, 1049 consecutive patients of HCC were evaluated; of them 347 patients were diabetics. The age, clinical features, aetiology of HCC and staging of the tumour at presentation, treatment decision and ultimate outcome were compared in diabetic to non-diabetic HCC patients. Results: HCC patients with DM (Group A, n =347, age = 60.3±7.3 years, sex (M:F) = 299:48) were compared with patient of HCC without DM (Group B, n = 702, age = 61.7±11.8 years, sex (M:F) = 608:94). Non alcoholic steatohepatitis was significantly more prevalent in Group A than Group B (p 0.05). Advanced HCC lesions were prevalent in group A than Group B (74.3 vs. 39.4%) – p < 0.05). During one year follow up, mortality rate was higher in Group A than Group B (60.2 vs. 45.3% – P < 0.05).

Abstract

Introduction Splanchnic arterial vasodilatation has related to hyperdynamic circulation and impaired natriuresis in advanced cirrhosis and was suggested to be responsible for the subtle sodium retention in pre-ascitic cirrhosis. α Adrenergic agonist may reverse this condition. Aim This study aimed to evaluate the effects of treatment with the α1-adrenergic agonist midodrine on systemic haemodynamics in non-azotemic cirrhotic patients. Method 154 cirrhotic patients were studied. The patients were classified into: absent ascites, mild to moderate diuretic responsive ascites and refractory ascites. Patients were randomly selected to receive either oral midodrine 10 mg, three times a day or placebo The following parameters were assessed for all patients: mean arterial pressure (MAP), cardiac output (CO), plasma rennin (Pl R) and renal resistive index (RRI) at baseline, 7 days after administration of oral midodrine 10 mg, three times daily, and 3 months after administration of oral midodrine 2.5 mg, three times a day. Results Midodrine administration induced a significant increase in MAP mention levels and P and significant decreases in CO mention levels and P in patients without and with mild to moderate ascites but not in patients with refractory ascites. As well as significant decreases in Pl R activity mention levels and P and RRI in patients without and with mild to moderate ascites but not in patients with refractory ascites Conclusion The administration of midodrine improves systemic haemodynamics associated with a suppression of the renin activity in cirrhotic patients without or with mild ascites. But had no effect in patients with refractory ascites

Abstract

Results: Out of 664159 women there were 22938 patients with hyperlipidaemia (3.5%) and 9312 patients with breast cancer. Out of the hyperlipidaemia patients, 530 patients developed breast cancer (2.3%) compared with 8782 patients developing breast cancer without hyperlipidaemia (1.4%). A logistic regression model accounting for time from first presentation to development of breast cancer showed that the presence of hyperlipidaemia increases the outcome of breast cancer by 1.64 times (95% C.I. 1.50-1.79). Conclusions: Whilst we appreciate numerous limitations of our methods, coupled with the main findings of the recent basic science research, our analysis further augments the case for the role of cholesterol in the development of breast cancer. Our data from a large clinical relevant sample further strengthens the argument to prospectively trial statins in the management of breast cancer.
