Abstract The mechanism of resistance in carbapenem-resistant Enterobacteriaceae (CRE) has therapeutic implications. We comprehensively characterized emerging mechanisms of resistance in CRE between 2013 and 2016 at a health system in Northern California. A total of 38.7% (24/62) of CRE isolates were carbapenemase gene-positive, comprising 25.0% (6/24) bla OXA-48 like , 20.8% (5/24) bla KPC , 20.8% (5/24) bla NDM , 20.8% (5/24) bla SME , 8.3% (2/24) bla IMP , and 4.2% (1/24) bla VIM. Between carbapenemases and porin loss, the resistance mechanism was identified in 95.2% (59/62) of CRE isolates. Isolates expressing bla KPC were 100% susceptible to ceftazidime–avibactam, meropenem–vaborbactam, and imipenem–relebactam; bla OXA-48 like –positive isolates were 100% susceptible to ceftazidime–avibactam; and metallo β-lactamase–positive isolates were nearly all nonsusceptible to above antibiotics. Carbapenemase gene-negative CRE were 100% (38/38), 92.1% (35/38), 89.5% (34/38), and 31.6% (12/38) susceptible to ceftazidime–avibactam, meropenem–vaborbactam, imipenem–relebactam, and ceftolozane–tazobactam, respectively. None of the CRE strains were identical by whole genome sequencing. At this health system, CRE were mediated by diverse mechanisms with predictable susceptibility to newer β-lactamase inhibitors. Eremothecium coryli is a dimorphic fungus of the Saccharomycetes class. While species within this class are known to cause human infection, Eremothecium species have previously only been known as phytopathogens and never been isolated from a human sample. Combined effects of temperature (25, 30 and 35°C) and salinity (15, 25, 35 and 45 g L-1), on the body composition and energetic value in the juvenile spotted rose snapper (Lutjanus guttatus), were investigated over 90 days. Significant effects of temperature, salinity and their interaction on body composition and energetic value were analyzed. Low temperatures (25°C) significantly reduced the protein and increase lipid content in the body. In a temperature of 30°C, the highest energetic value was recorded, and in the highest salinity (45 g L-1), the energetic value was reduced. The salinity of 15 has the highest growth, protein content and energetic value. It was found that the optimal temperature-salinity combination for the best relationship of growth (3.8% d-1), protein (21.9%) and lipid (4.9%) content and energetic value (2.61 kJ g-1) was at 30 to 32.0°C and salinity of 35 g L-1. The effect of temperature was directly proportional to the increase in protein and inversely with the content of lipids in the body; salinity has its main effect negatively on these parameters in the highest salinity (45 g L-1). Congenital toxoplasmosis continues to be a public health threat. Even existing guidelines, publicly known, its implementation and lack of appropriate interpretation of serological tests in pregnancy is often observed. This leds to failure in opportunities for positive and known interventions to decrease the fetal risk due to Toxoplasma gondii infection. We reported herein a case series, with variable neurological and systemic compromise (respiratory distress, hepatosplenomegaly, enterocolitis, brain calcifications, thrombocytopenia, ascites, shock), even fatal, calling for awareness about the fact that despite the Zika epidemics in 2015-2016 in Brazil, Colombia and other countries, precisely toxoplasmosis, is a differential diagnosis still prevalent in these territories, that can leds to severe consequences, with neurological disability and risk of ocular damage, even lately. Freshwater species and their habitats, and transportation networks are at heightened risk from changing climate and are priorities for adaptation, with the sheer abundance and individuality of road-river structures complicating mitigation efforts. We present a new spatial dataset of road-river structures attributed as culverts, bridges, or fords, and use this along with data on gradient and stream order to estimate structure sensitivity and exposure in and out of special areas of conservation (SAC) and built-up areas to determine vulnerability to damage across river catchments in Wales, UK. We then assess hazard of flooding likelihood at the most vulnerable structures to determine those posing high risk of impact on roads and river-obligate species (fishes and mussels) whose persistence depends on aquatic habitat connectivity. Over 5% (624/11,680) of structures are high vulnerability and located where flooding hazard is highest, posing high risk of impact to roads and river-obligate species. We assess reliability of our approach through an on-ground survey in a river catchment supporting an SAC and more than 40% (n = 255) of high-risk structures, and show that of the subset surveyed >50% had obvious physical degradation, streambank erosion, and scouring. To accurately assess the threat that global climate change poses to marine systems, a detailed baseline of the current carbonate chemistry and other oceanographic conditions is required. Despite the heightened vulnerability of deep-sea communities to ocean acidification, there have been relatively few studies investigating the carbonate chemistry immediately above cold-water coral reefs. Here, we present data collected during five cruises from 2010 to 2014 in the northern Gulf of Mexico and quantify the carbonate system and other oceanographic parameters in offshore surface-waters, the water column, and at deep benthic sites. Benthic sites containing the scleractinian cold-water coral L. pertusa occurred in waters with a relatively wide temperature range (6.8-13.6°C), low potential density ( σθ = 26.9 ± 0.3 kg m−3), low dissolved oxygen concentration (111.3 ± 2.0 μmol kg−1), low pHT (7.87 ± 0.04), low ΩARAG (1.31 ± 0.14), and a low availability of carbonate ions (94.4 ± 9.2 μmol kg−1) compared with L. pertusa habitats in other regions. Based on previous modelling and experimental results, these values place L. pertusa at the edge of its viable niche in the deep Gulf of Mexico. However, significantly elevated total alkalinity (+39-44 μmol kg−1) was detected above large L. pertusa mounds, suggesting that carbonate dissolution within the mounds may be partially ameliorating the direct effects of ocean acidification. Together, these results provide an important baseline for assessing future oceanographic changes in the Gulf of Mexico and for predicting the resilience of cold-water coral reefs to global climate and ocean change. Accordingly, no single institution can claim extensive experience, and management protocols are most probably based on either expert opinion or literature reports. Although vector-borne diseases are specific to the region of the host, there is a necessity for surveillance or reference laboratories to perform standardized, high-throughput testing capable of meeting the needs of a changing military environment and response efforts. The development of standardized, high-throughput, semiquantitative real-time and reverse transcription real-time polymerase chain reaction (PCR) methods allows for the timely dissemination of data to interested parties while providing a platform in which long-term sample storage is possible for the testing of new pathogens of interest using a historical perspective. PCR testing allows for the analysis of multiple pathogens from the same sample, thus reducing the workload of entomologists in the field and increasing the ability to determine if a pathogen has spread beyond traditionally defined locations. US Army Public Health Command Region- Europe (USAPHCR-Europe) Laboratory Sciences (LS) has standardized tests for 9 pathogens at multiple life stages. All tests are currently under international accreditation standards. Using these PCR methods and laboratory model, which have universal Department of Defense application, the USAPHCR-Europe LS will generate quality data that is scientifically sound and legally defensible to support force health protection for the US military in both deployed and garrison environments. This article describes the collaborative modeling process and the resulting water resources planning model developed to evaluate water management scenarios in the transboundary Rio Grande basin. The Rio Grande is a severely water stressed basin that faces numerous management challenges as it crosses numerous jurisdictional boundaries. A collaborative process was undertaken to identify and model water management scenarios to improve water supply for stakeholders, the environment, and international obligations of water delivery from Mexico to the United States. A transparent and open process of data collection, model building, and scenario development was completed by a project steering committee composed of university, nongovernmental, and governmental experts from both countries. The outcome of the process was a planning model described in this article, with data and operations that were agreed on by water planning officials in each country. Water management scenarios were created from stakeholder input and were modeled and evaluated for effectiveness with the planning model. [ABSTRACT FROM AUTHOR]

Background: Given the acknowledged problems in sepsis diagnosis, we use a novel way with the application of the latent class analysis (LCA) to determine the operative characteristics of C-reactive protein (CRP), D-dimer (DD) and Procalcitonin (PCT) as diagnostic tests for sepsis in patients admitted to hospital care with a presumptive infection. Methods: Cross-sectional study to determine the diagnostic accuracy of three biological markers against the gold standard of clinical definition of sepsis provided by an expert committee, and also against the likelihood of sepsis according to LCA. Patients were recruited in the emergency room within 24 hours of hospitalization and were follow-up daily until discharge. Results: Among 765 patients, the expert committee classified 505 patients (66%) with sepsis, 112 (15%) with infection but without sepsis and 148 (19%) without infection. The best cut-offs points for CRP, DD, and PCT were 7.8 mg/dl, 1616 ng/ml and 0.3 ng/ml, respectively; but, neither sensitivity nor specificity reach 70% for any biomarker. The LCA analysis with the same three tests identified a "cluster" of 187 patients with several characteristics suggesting a more severe condition as well as better microbiological confirmation. Assuming this subset of patients as the new prevalence of sepsis, the ROC curve analysis identified new cut-off points for the tests and suggesting a better discriminatory ability for PCT with a value of 2 ng/ml. Conclusions: Under a "classical" definition of sepsis three typical biomarkers (CRP, PCT and DD) are not capable enough to differentiate septic from non-septic patients in the ER. The vein of Galen aneurysmal malformation (VGAM) is an extremely rare arteriovenous malformation. The VGAM clinical manifestations vary depending on the magnitude of vascular compromise and the age at initial presentation. Neonates typically present with severe congestive heart failure. The water planning and management policies of the Rio Grande basin no longer respond to the sustainable needs of water users, environment, and international commitments of this transboundary basin between Mexico and the United States. This paper describes how groundwater banking through an in lieu method is one approach leading to better water management in this basin. In lieu groundwater banking is a conjunctive water allocation policy applicable to water users supplied from surface water and groundwater sources. A basin simulation model of the Rio Grande basin, built in the water evaluation and planning system (WEAP) software, was used to evaluate the groundwater banking policy. Two scenarios are discussed: a baseline scenario without new water allocation policies implemented and a groundwater banking scenario considering the in lieu groundwater banking method implemented in the Meoqui aquifer. Results show that groundwater banking can significantly improve water management in the basin, increasing system storage, improving water supply for users in the basin, and enhancing compliance with the treaty obligations. • Cerato-platanin protein has been identified and isolated from P. salina. • Hydrophobin protein has been identified and isolated from T. pinophilus. • CPs and HFBs are extracted from a fermented culture medium with seaweed biomass. • Self-assemble of this extracted proteins were influenced by the polysaccharides' seaweed. The marine fungi Paradendryphiela salina and Talaromyces pinophilus degrade and assimilate complex substrates from plants and seaweed. Additionally, these fungi secrete surface-active proteins, identified as cerato-platanins and hydrophobins. These hydrophobic proteins have the ability to self-assemble forming amyloid-like aggregates and play an essential role in the growth and development of the filamentous fungi. It is the first time that one cerato-platanin (CP) is identified and isolated from P. salina (PsCP) and two Class I hydrophobins (HFBs) from T. pinophilus (TpHYD1 and TpHYD2). Furthermore, it is possible to extract cerato-platanins and hydrophobins using marine fungi that can feed on seaweed biomass, and through a submerged liquid fermentation process. The propensity to aggregate of these proteins has been analyzed using different techniques such as Thioflavin T fluorescence assay, Fourier-transform Infrared Spectroscopy, and Atomic Force Microscopy. Here, we show that the formation of aggregates of PsCP and TpHYD, was influenced by the carbon source from seaweed. This study highlighted the potential of these self-assembling proteins generated from a fermentation process with marine fungi and with promising properties such as conformational plasticity with extensive applications in biotechnology, pharmacy, nanotechnology, and biomedicine. Background: A possible method of finding physiological markers of multiple sclerosis (MS) is the application of EEG quantification (QEEG) of brain activity when the subject is stressed by the demands of a cognitive task. In particular, modulations of the spectral content that take place in the EEG of patients with multiple sclerosis remitting-relapsing (RRMS) and benign multiple sclerosis (BMS) during a visuo-spatial task need to be observed.Methods: The sample consisted of 19 patients with RRMS, 10 with BMS, and 21 control subjects. All patients were free of medication and had not relapsed within the last month. The power spectral density (PSD) of different EEG bands was calculated by Fast-Fourier-Transformation (FFT), those analysed being delta, theta, alpha, beta and gamma. Z-transformation was performed to observe individual profiles in each experimental group for spectral modulations. Lastly, correlation analyses was performed between QEEG values and other variables from participants in the study (age, EDSS, years of evolution and cognitive performance).Results: Nearly half (42%) the RRMS patients showed a statistically significant increase of two or more standard deviations (SD) compared to the control mean value for the beta-2 and gamma bands (F = 2.074, p = 0.004). These alterations were localized to the anterior regions of the right hemisphere, and bilaterally to the posterior areas of the scalp. None of the BMS patients or control subjects had values outside the range of +/- 2 SD. There were no significant correlations between these values and the other variables analysed (age, EDSS, years of evolution or behavioural performance).Conclusion: During the attentional processing, changes in the high EEG spectrum (beta-2 and gamma) in MS patients exhibit physiological alterations that are not normally detected by spontaneous EEG analysis. The different spectral pattern between pathological and controls groups could represent specific changes for the RRMS patients, indicative of compensatory mechanisms or cortical excitatory states representative of some phases during the RRMS course that are not present in the BMS group. Background: Recent data have suggested that 18 million of new sepsis cases occur each year worldwide, with a mortality rate of almost 30%. There is not consensus on the clinical definition of sepsis and, because of lack of training or simply unawareness, clinicians often miss or delay this diagnosis. This is especially worrying; since there is strong evidence supporting that early treatment is associated with greater clinical success. There are some difficulties for sepsis diagnosis such as the lack of an appropriate gold standard to identify this clinical condition. This situation has hampered the assessment of the accuracy of clinical signs and biomarkers to diagnose sepsis. Methods/design: Cross-sectional study to determine the operative characteristics of three biological markers of inflammation and coagulation (D-dimer, C-reactive protein and Procalcitonin) as diagnostic tests for sepsis, in patients admitted to hospital care with a presumptive infection as main diagnosis. Discussion: There are alternative techniques that have been used to assess the accuracy of tests without gold standards, and they have been widely used in clinical disciplines such as psychiatry, even though they have not been tested in sepsis diagnosis. During floods, river invertebrates may be swept downstream in large numbers. This so-called "catastrophic drift" leads to a major redistribution of animals, as well as reduced fitness and increased mortality among drifters. We present the first field evidence of the role of sediment movement in triggering catastrophic drift. Experiments indicate that the loss of invertebrates from the bed becomes exponential when shear stress reaches the threshold that entrains bedload. However, we found that low rates of bedload are sufficient to rapidly denude patches of riverbed of their invertebrates and so trigger mass drift. Such low bedload rates occur during small floods. As small floods occur frequently, our results suggest that episodes of catastrophic drift are frequent. This conclusion is counterintuitive, as the persistence of invertebrate communities on riverbeds suggests that such events cannot be truly catastrophic. Moreover, the drift losses that we observed occurred in the absence of significant geomorphic disturbance; this is inconsistent with the notion of catastrophic drift being a response to hydrological disturbance events. We argue that a new definition of catastrophic drift is needed, a definition based not on drift magnitude or the triggering role of sediment movement, but on the population consequences of downstream displacement. Background: Cognitive impairment is a common feature in multiple sclerosis (MS) patients and occurs in 60% of all cases. Unfortunately, neurological examination does not always agree with the neuropsychological evaluation in determining the cognitive profile of the patient. On the other hand, psychophysiological techniques such as event-related potentials (ERPs) can help in evaluating cognitive impairment in different pathologies. Behavioural responses and EEG signals were recorded during the experiment in three experimental groups: 1) a relapsing-remitting group (RRMS), 2) a benign multiple sclerosis group (BMS) and 3) a Control group. The paradigm employed was a spatial attention task with central cues (Posner experiment). The main aim was to observe the differences in the performance (behavioural variables) and in the latency and amplitude of the ERP components among these groups. Results: Our data indicate that both MS groups showed poorer task performance (longer reaction times and lower percentage of correct responses), a latency delay for the N1 and P300 component, and a different amplitude for the frontal N1. Moreover, the deficit in the BMS group, indexed by behavioural and pyschophysiological variables, was more pronounced compared to the RRMS group. Conclusion: The present results suggest a cognitive impairment in the information processing in all of these patients. Comparing both pathological groups, cognitive impairment was more accentuated in the BMS group compared to the RMSS group. This suggests a silent deterioration of cognitive skills for the BMS that is not usually treated with pharmacological or neuropsychological therapy. [ABSTRACT FROM AUTHOR]

Background In this prospective nation-wide survey of bloodstream isolates associated with first episode of FN in high-risk cancer patients from 14 US cancer centers (December 2016 and June 2018), viridans group Streptococci (VGS) were the most common Gram-positive isolate. We sought to clinically and microbiologically characterize VGS bloodstream infections (BSI). Methods Among 343 patients,we compared 90 with VGS vs 253 with non-VGS BSI. Minimum inhibitory concentrations for blood culture isolates were determined by broth dilution for selected agents at our reference microbiology laboratory (UNMC). Clinical data were electronically captured in RedCap, including local site isolate identification and confirmatory reference lab identification via MALDI. Categorical and continuous variables were assessed via chi-square and Mann–Whitney U tests, respectively. Results Ninety-two VGS isolates were identified among 90 FN patients, representing 27% of all BSI isolates. S. mitis or oralis comprised 64 (70%) of VGS. There were no differences between age, sex, and primary diagnosis (50% with AML) among the 2 groups; 1/3 were HSCT recipients. Fluoroquinolone prophylaxis was used in 64 (71%) vs. 139 (55%), P < 0.01, in VGS vs non-VGS groups. Critical illness composite (new need for pressor(s), mechanical ventilation or death within 30 days) was 6 (7%) vs. 44 (17%), P = 0.01, in the VGS vs non-VGS groups. Figure 1 displays an overview of antibiotic susceptibilities for 79 testable isolates. VGS susceptibilities to levofloxacin, penicillin, and ceftriaxone were 39%, 47%, and 94%, respectively. Conclusion VGS are common pathogens in FN patients. Prior fluoroquinolone prophylaxis use may be a risk factor. VGS BSI was not associated with increased critical illness compared with non-VGS. Maca tuber meal is used in fish diet formulations in Andean trout culture and knowledge of its effects on fish growth is paramount to healthy human food production. In the first experiment with rainbow trout alevins (0.096±0.002 g), starter diets were offered from first feeding until 15 weeks. We formulated high protein content (∼60%) semi-purified starter diets supplemented with 0%, 5%, 10%, or 15% maca tuber meal (control, M-5, M-10, and M-15 respectively). The second feeding trial was conducted with juveniles (1.56±0.02 g) fed one of three diets (control, M-15, and commercial) for 8 weeks. In the first experiment, fish fed M-10 and M-15 diets exhibited significantly higher growth rates than the other dietary groups. Survival was significantly improved in the groups fed diets supplemented with maca tuber meal (60.0–69.2%) in comparison with the group fed a control diet (21.7%). The second experiment showed a higher growth rate in the M-15 group compared with the control and a commercial diet fed group. Leucocyte numbers were increased by dietary supplementation of maca tuber meal. The findings of the present study suggest that a maca tuber meal inclusion at least 5% improves growth rate, feed utilization, immunity by increased leucocyte number, and survival of rainbow trout alevins and juveniles. Analysis of auditory scene relies on the appropriate division of the sensory input into meaningful events. The auditory system uses a sequential grouping strategy to segregate different acoustic events in the time domain. As a result of grouping, the organization of sound is expected to include acoustic events spanning different scales of time. It is unclear, however, at which moment the central auditory system begins to introduce changes in the neural representation of the auditory scene as a result of grouping different information units into a larger acoustic event. The present set of experiments examines this issue by using the mismatch negativity (MMN) component of event-related potentials. The MMN is elicited in response to changes in the previously registered auditory regularity whether or not the subject’s attention is focused on the auditory stimuli. By presenting three consecutive pairs of alternating tones in serial stimulus trains, we found that across-train repetitions of the tone elicited the MMN with inter-train intervals up to 240 ms. Beyond this interval, only within-train repetitions of the tone elicited the MMN, suggesting that pairs of tones were bound together into a single acoustic event (i.e., train of six tones) within a temporal window not much longer than 240 ms. Furthermore, a different pattern of responses was seen depending on the position of the deviant event within the train and the frequency distance between the alternating tones. These results suggest that grouping discrete auditory inputs may introduce changes in the neural representation of the auditory scene by applying rules to sounds spanning larger temporal scales. The article describes the case of a 3-year-old toddler who experienced severe pain due to an embryonal bladder/prostate rhabdomyosarcoma. The patient's complaint was a painful tenesmus secondary to the pelvic floor invasion of the mass. The case also describes ganglion impar phenol injection as an effective therapy strategy in a pediatric patient with refractory cancer pain.

Background: Despite significant advances in durable mechanical support survival, infectious complications remain the most common adverse event after ventricular assist device (VAD) implantation and the leading cause of early death after transplantation. In this study, we aim to describe our local infectious epidemiology and review short‐term survival and infectious incidence rates in the post‐transplantation period and assess risk factors for infectious episodes after transplantation. Methods: Retrospective single‐center study of all consecutive adult heart transplant patients from 2008 to 2017. Survival data were estimated and summarized using the Kaplan‐Meier method. We quantified and evaluated the difference in the incidence rate between patients with and without infection using a Fine‐Gray model. The outcome of interest is the time to first infection diagnosis with post‐transplant death as the competing event. Results: Among 278 heart transplant patients, 74 (26.5%) underwent LVAD implantation. Twenty‐one patients (28.3%) developed an infection while supported by an LVAD. When compared to patients supported by an LVAD without a preceding infection, BMI was significantly greater (31.2 vs 27.8 kg/m2, P = .03). Median follow‐up post‐transplantation was 3.01 years. Significant risk factors for the competing risk regression for infection after heart transplantation include LVAD infection (HR 1.94, [95% CI] 1.11‐3.39, P = .020) and recipient COPD (HR 2.14, [95% CI] 1.39‐3.32, P = .001) when adjusted for recipient age, gender, hypertension, diabetes mellitus, and body mass index.
