Organizations attempt to keep pace with the dynamic, ever-changing environment through technology up-gradation, human resource restructuring, diversification of product or service lines, and other similar actions. In most cases, these actions have taken the form of organizational downsizing, leading to the loss of jobs. The primary purpose of this study is to propose an approach that organizations may adopt against the cascading impact of layoffs on separated employees (victims), associates and co-workers (survivors). Online platforms were used for the collection of data. A total of 864 statements were analysed. Sentiment analysis was conducted using the SEMRUSH software. Miles and Huberman's (1994, Qualitative data analysis: An expanded sourcebook) method was used to categorize the data obtained into meaningful categories to provide results that would support the research objectives. The analysis shows a considerable cascading psychological impact on the human resource ecosystem. This article puts forward the causal model, capturing the interplay of the effects of the stakeholders on one another and their social well-being. Further, the article proposes that the downsizing cycle be constituted such that it minimizes the impact of layoffs. Extant research on job loss has mostly considered one or two stakeholders at a time. We have attempted a multi-stakeholder approach. Epilepsy with myoclonic-atonic seizures, also known as Doose syndrome or myoclonic–astatic epilepsy (MAE), represents a challenging pediatric epilepsy syndrome, initially described by Doose et al. in 1970. MAE accounts for 1%–2.2% of juvenile-onset epilepsy cases, characterized by normal early development, followed by seizure onset between 7 months and 6 years. Seizure types include myoclonic, myoclonic-atonic, generalized tonic–clonic, and atypical absences, often accompanied by nonconvulsive status epilepticus. Electroencephalogram typically shows parietal-dominant rhythmic theta activity or a posterior 4 Hz pattern. While conventional antiseizure medications (ASMs) offer some efficacy, outcomes vary widely, and up to 50% of patients may experience refractory seizures and poor intellectual outcomes. The ketogenic diet (KD), a high-fat, low-carbohydrate diet inducing ketosis, has shown promise in reducing seizure frequency and severity in refractory epilepsy, including MAE. This case report describes a successful outcome with a South Indian style KD in a 3-year-old boy with Doose syndrome presenting with 20–25 daily head drop seizures despite multiple ASMs and steroid therapy. Initiation of a home-based KD utilizing traditional household ingredients significantly reduced seizure frequency by 60% within 1 month and 80% within 2 months and was 100% seizure-free in 3rd month following KD, alongside developmental improvements. The KD was well-tolerated, with improved nutritional status. This case underscores KD's potential as a viable complementary therapy for MAE, emphasizing the need for ongoing parental support and supervision to ensure dietary adherence and optimize outcomes. It is commonly believed that the KD is challenging for the South Indian population to follow due to the high carbohydrate content in South Indian cuisine. However, this case demonstrates that a KD can indeed be tailored around South Indian dishes, highlighting the uniqueness of this approach. When the diet is customized to consider a child's dietary habits, culture, and cuisine, it becomes easier to follow over the long term, leading to effective seizure control. Background: Muscle mass loss, impaired dietary intake, and poor nutritional status are hallmarks of patients with cancer. Cancer-associated malnutrition can lead to several negative consequences, including poor prognosis, reduced survival, increased therapy toxicity, reduced tolerance and compliance to treatment, and diminished response to drugs. The main aim of this study is to understand the prevalence of malnutrition using Global Leadership Initiative on Malnutrition Criteria (GLIM Criteria) and evaluate the impact of tailor-made nutritional intervention on the nutritional status of patients with cancer. Methods and Material: An interventional prospective study was conducted among 100 adult hospitalized cancer patients. The study was based on the Nutrition Care Process model, where nutritional assessment was done using GLIM criteria and Malnutrition Universal Screening Tool (MUST), and muscle mass was assessed using the Handgrip Dynamometer. Nutritional diagnosis was made based on assessment; nutritional intervention was provided through Individualized Nutritional Counseling & Oral Nutritional Support, and nutritional monitoring was done for patients during their stay in hospital using 24-hour daily dietary recall. Nutritional evaluation was done at the time of discharge using GLIM criteria, MUST, and Handgrip Strength (HGS). Data analysis was done using SPSS v24 statistical software (developed by IBM) to find out the Mean and Standard Deviation. Paired t test, Chi-square test, and Regression Analysis were used to analyze the qualitative variables. Statistical significance was defined as P < 0.05. Results: The current study evaluated 100 participants among whom 26% of participants were malnourished as assessed by MUST and 22% participants had poor muscle strength. A paired t -test was performed and it was found that the calorie intake (1504.1 ± 158.7) and the protein intake (56.08±7.01) were almost double after the nutrition intervention. The difference was found to be highly statistically significant with P < 0.01. Thus, nutrition intervention significantly (P < 0.01) improved calorie and protein intake. The overall mean MUST score at baseline was 0.39 which was reduced significantly (P < 0.000) to 0.17 at the end of the study. The HGS improved significantly (P < 0.000) from 10.02 to 25.51 after the nutrition intervention and it was evidenced by 2.5 times increase in HGS (25.51 ± 2.40) by the end of the study. Conclusion: Individualized nutritional support with continuous monitoring and counseling resulted in improvement in nutritional status as evidenced through their improved MUST score and HGS. The article offers a diversion from the ossification of the current scholarship on self-images and identity construction on social media by looking at the marketability and entanglements in ecologies of commerce and sociality that the crying videos lead to. Introduction: Drug-resistant epilepsy (DRE) is a challenging neurological condition in infants. Breastfeeding is widely regarded as the gold standard for infant nutrition due to its immunological and developmental benefits. However, in certain cases of genetic DRE, breastfeeding has been associated with increased seizure frequency, necessitating alternative nutritional strategies. This study represents the first investigation that evaluates the efficacy of a milk-based ketogenic diet (KD) in infants aged one to six months with genetic DRE focusing on seizure control, nutritional status, developmental progress, and safety, as measured by metabolic and biochemical parameters. As the first study of its kind, this research offers a unique contribution to the field, paving the way for further investigations into diet-based therapies for refractory epilepsy in early life., Methodology: This prospective study included eight infants with genetic DRE, aged one to six months. Baseline data on seizure frequency, nutritional status, developmental milestones, and biochemical parameters were collected. The KD was started at a 2:1 ratio based on a non-fasting KD protocol and was optimized based on need and tolerance. Nutritional status was assessed with the Strong Kids Nutrition Screening Tool. Seizure frequency was tracked daily, and urine ketone levels were monitored to confirm ketosis. Biochemical parameters were measured at baseline and after six months. Paired t-tests were used to analyze data., Results: At baseline, the mean seizure frequency was 16.5/day. After six months on the KD, the mean seizure frequency decreased to 4.6/day (p < 0.001). Nutritional status improved significantly. Urine ketone levels remained consistently high. Biochemical parameters, including triglycerides and random blood sugar, showed no significant changes, confirming the diet's safety., Conclusion: This study demonstrates that a milk-based KD is an effective and safe treatment for reducing seizures and improving nutritional and developmental outcomes in infants with genetic DRE. Persistent ketosis, indicated by large urine ketone levels, was a reliable biomarker of diet efficacy. Regular monitoring and careful parental counseling are essential for optimizing treatment outcomes in this vulnerable population.
