Writing about Francis Bacon, Gilles Deleuze comments on the body as the 'zone of indiscernibility' between man and animal, especially 'as it is flesh or meat.' Bacon's fleshy zone, I argue, finds an early illustration in Madame Bovary, where Flaubert will also incorporate notions of 'animal life' as described by the nineteenth-century pathologist, Xavier Bichat. This essay will examine these fleshy zones as they relate to changing ideas of skin, tactility, and those, fluid permeable zones between human and animal and between individual bodies and the penetrating forces of others and the environment. Online social networks(OSNs) face the challenging problem of hate speech, which should be moderated for the growth of OSNs. The machine learning approaches dominate the existing set of approaches for hate speech detection. In this study, we introduce BiCHAT: a novel BiLSTM with deep CNN and Hierarchical ATtention-based deep learning model for tweet representation learning toward hate speech detection. The proposed model takes the tweets as input and passes through a BERT layer followed by an attention-aware deep convolutional layer. The convolutional encoded representation further passes through an attention-aware Bidirectional LSTM network. Finally, the model labels the tweet as hateful or normal through a softmax layer. The proposed model is trained and evaluated over the three benchmark datasets extracted from Twitter and outperforms the state-of-the-art (SOTA) (Khan et al., 2022; Roy et al., 2020; Ding et al., 2019) and baseline methods with an improvement of 8%,7% and 8% in terms of precision, recall, and f-score, respectively. BiCHAT also demonstrates good performance considering training and validation accuracy with an improvement of 5% and 9%, respectively. We also examined the impact of different constituting neural network components on the model. On analysis, we observed that the exclusion of the deep convolutional layer has the highest impact on the performance of the proposed model. We also investigated the efficacy of different embedding techniques, activation function, batch size, and optimization algorithms on the performance of the BiCHAT model.

5. Hector Berlioz's Neurophysiological Imagination.

There is, Berlioz writes in his essay "De la musique en général" (1837), "a strange agitation in my blood circulation: my arteries beat violently . . . a trembling overtakes my limbs and a numbness my hands and feet, while the nerves of sight and hearing are partially paralyzed." The composer here is not, as one might think, describing the symptoms of an opium overdose, but rather the powerful effect of "good music" on his own body. Challenging our tendency to dismiss Berlioz's musical writings as merely overheated Romantic effusions, I argue that the remarkable medical detail presented in his arguments reveals a hitherto neglected dimension of nineteenth-century engagement with the embodied effects of music. Contextualizing Berlioz's claims within the neurophysiology of his age, and in particular the physiological psychology of Cabanis, the anatomy of Bichat, and the acoustic theories of Lamarck, I recover the medical and scientific epistemes that motivated the composer's assertions about the power of music to effect embodied and emotional transformation--a constellation of ideas that I term his "neurophysiological imagination." An analysis in these terms of some of Berlioz's major compositional innovations reveals how both his writings and his music explore an explicitly neurophysiological dimension of early Romantic listening. Specifically, I propose that Berlioz repeatedly attempted both to represent and to induce an experience that was of great interest to the artists and audiences of his day: an overpowering mental and physical response to music experienced as an embodied, neurophysiological form of the sublime. Il presente saggio si propone di analizzare un aspetto dell’interpretazione schopenhaueriana della corporeità: il considerare il corpo come fenomeno unitario, e non come termine di un dualismo che a esso contrappone l’anima. Per Schopenhauer il corpo umano, pur nella sua pluralità di significati, è riconducibile a un’unità di fondo, l’essere oggettivazione della volontà. Il Wille è pensato anche come passione, tutt’uno con il Leib che ne riflette i moti. Inoltre, il corpo inteso come organismo e come insieme di affezioni e di pensieri è concepito come unione di parti che si relazionano e si suggestionano reciprocamente. Per Schopenhauer, questa interpretazione è comprovata e condivisa, dal punto di vista fisiologico, da Cabanis e Bichat, nei termini di coappartenenza di physique e moral da un lato, e di interconnessione tra la vie organique e la vie animale dall’altro. Il saggio si propone dunque di illustrare tale visione unitaria e tratteggiare in che misura queste diverse prospettive si incontrano.

12. Successful Closure of an Oro-Antral Communication, after Three Previously Failed Attempts, Using Bichat Fat Pad Flap: A Case Report.

- Abstract
An oro-antral communication represents an abnormal connection between the oral cavity and the maxillary sinus. It occurs most often after tooth extractions, improper implant placement or incorrect management of the sinus lifts. Surgical repair is challenging and most practitioners usually choose the buccal advancement flap, the palatal flap and in some cases the buccal fat pad flap to close the defect. We present a 43 year-old female of a large oro-antral communication and associated chronic sinusitis which was succesfully manged by surgery. Previous interventions including 2 buccal advancement flaps, and a double layer closure using Collagen membrane and buccal advancement flap were unsuccesful. The stepwise intervention consisted on the complete cleaning of the sinus, using the Caldwell Luc technique, followed by the closure of the oro-antral communication using Bichat fat pad flap. The particular aspect was the proper integration of the buccal fat pad flap, after 3 failed attempts, without dehiscence or any other complications. The buccal fat pad flap can be succesfully used for closure of lage oro-antral communications, even when previous methods have failed and local tissue is of poor quality., Competing Interests: None declared

This essay tests the assumption that Diderot was unmentionable during the 1790s because of his association with atheist materialism, and explores how philosophes close to him protected themselves and their work. It focuses predominantly on the Idéologues, Cabanis, and Destutt de Tracy, and also on the physician Bichat and the philosopher-zoologist Lamarck. It identifies certain textual strategies commonly deployed by these writers, including firstly, the curious practice of naming certain officially acceptable luminaries and using them to cover obscure references to other unnamed 'génies', secondly, the creation of neologisms for their work ('biologie', 'idéologie'), and thirdly, exaggerated claims of originality. It suggests that the break generally perceived by modern scholarship between the eighteenth-century experimental texts exploring consciousness and matter, particularly those of Diderot, and the post-revolutionary thinkers has been overstated, perhaps because of the very success of the trace-covering strategies examined here. Objetivo: descrever, através de uma revisão de literatura, a cirurgia de bichectomia em relação aos seus benefícios, indicações e contraindicações, como também a técnica cirúrgica, abrangendo as considerações anatômicas e possíveis complicações relacionadas ao procedimento. Revisão de literatura: com as tendências da atualidade, a busca por uma aparência jovem ou correção da estética tem sido uma demanda constante, na qual também está inserida a Odontologia. O rejuvenescimento facial é uma área cada vez mais requisitada, sendo o perfil facial afilado o padrão estético mais aceito atualmente. Nesse contexto, existem várias técnicas cirúrgicas que buscam obter esse resultado. A bichectomia, por exemplo, é um procedimento com relatos antigos na literatura, mas que recentemente tornou-se popularizada devido aos seus resultados estéticos. É necessária uma avaliação minuciosa da face para a indicação do procedimento, este pode ser puramente estético ou funcional, onde o paciente acaba mordendo a mucosa jugal durante a mastigação. A cirurgia de bichectomia consiste na remoção da bola de Bichat, localizada externamente ao músculo bucinador e em frente a margem anterior do músculo masseter. Conclusão: a remoção cirúrgica da bola de Bichat promove uma diminuição do contorno das bochechas conferindo uma melhor harmonia facial, sendo uma opção viável com poucas complicações e baixa morbidade para pacientes saudáveis, com resolutividade para o comprometimento funcional e estético do paciente

16. Consciousness and its disorders.

The diaphanous nature of consciousness (as experienced by its owner) is more than matched by its opaqueness to analysis and description. And yet a descriptive psychopathology which did not include this concept would be difficult to imagine. Thus, during the nineteenth century, many ‘mental symptoms’ were defined in terms of introspective data from consciousness (e.g. hallucinations, depersonalization, dejd vu, etc.). The subjectivity of the madman became a private theatre where phantasms played dramas to which the psychiatrist had no direct access. All he could do was get the privileged seer to describe the experiences and then believe that the patient was telling the truth. It goes without saying that such belief is based on the assumptions that consciousness exists and that the patient's descriptive capacity has been spared by the disease. But in clinical practice things are more complicated than that. Sensations, images, feelings and impulses, often odd or never experienced before, must be recognized and put into words by a person who is simultaneously bemused, confused, upset, terrified or who may actually be participating in the inner drama. Therefore, what the psychiatrist may get is a prosaic, analogical, or bizarre paraphrase. Based on training and imagination, the interviewer will try to name and classify those descriptions that sound familiar. But what about the many which do not? In current practice, and due to the control of ready-made glossaries, it is likely that such descriptions will be quietly ignored. This is a pity for these unnamed experiences are also markers of disease. Only connect … In the history of Western medicine, from the school of Salerno to that of Freud, the content and structure of its four principal ingredients - its conception of the nature of man, its technical capacity to do medical research and to give treatment, its sense of religion, and the social structure of medical aid - have changed continually; but the exclusive concern of pathology with the physical side of man's being has not disappeared or changed. If one poses, for a science such as theoretical physics or organic chemistry, the problem of its relations with the political and economic structure of society, doesn't one pose a problem which is too complicated? Isn't the threshold of possible explanation placed too high? If, on the other hand, one takes a knowledge [savoir] such as psychiatry, won't the question be much easier to resolve, since psychiatry has a low epistemological profile, and since psychiatric practice is tied to a whole series of institutions, immediate economic exigencies and urgent political pressures for social regulation? Cannot the interrelation of effects of knowledge and power be more securely grasped in the case of a science as ‘doubtful’ as psychiatry? It is this same question that I wanted to pose, in The Birth of the Clinic apropos of medicine: it certainly has a much stronger scientific structure than psychiatry, but it is also very deeply involved in the social structures. Introduction Ours is not the first generation to be preoccupied with modernity, nor the first to search for clues as to when ‘our’ kind of medicine began. Conventional wisdom generally locates that origin in the Revolutionary Paris of the 1790s. Thus, R. H. Shryock's chapter on early-nineteenth-century French medicine is entitled ‘The emergence of modern medicine, 1800-1850’, and the ‘clinic’ described by Michel Foucault does not simply concern the activities of Bichat and Laennec, but also the ‘conditions of possibility of medical experience in modern times’. This French hospital medicine coincided with political and social upheaval and with ‘the end of the Enlightenment’, a fact which deepens the apparent chasm separating our medicine from that of the Enlightenment and before. If our own medicine derives in some important sense from the hospitals of Revolutionary France, the medical forms, patterns, and practices of the Enlightenment can be viewed in two antithetical ways: as having created the possibility of modernity; or, by way of reaction, the ultimate victim of it. The latter has been the more common historical attitude. Thus Shryock, while applauding eighteenth-century advances in mathematics, physics, chemistry, and the social sciences, discovered a variety of social and intellectual reasons why medicine lagged behind. 19. On lipoma of the buccal fat pad: Report of two cases and review of the literature.

Summary: The buccal fat pad (BFP) has been the subject of numerous publications regarding its anatomy and clinical implications, however our interest in the pathology was aroused by two cases of lipoma originating from the BFP that were particularly interesting as one lipoma was congenital and the other recurred. A search of the international literature revealed a further 27 cases of BFP lipoma dating from 1848 to 2002. This suggests it is a rare entity but the authors suspect it to be under reported because of unfamiliarity with the possibility and the various atypical characteristics that were observed. Firstly, BFP lipomas appear to be congenital relatively often. Also, many are histological variants such as the spindle-cell lipoma, which could be associated with a more diffuse growth in the various extensions. As the deep extensions are not routinely removed due to the difficulty of the procedure, this could result in incomplete resection and recurrence. A possible explanation is the hypothesis that the BFP has a different embryological origin than subcutaneous fat. More importantly, well-differentiated liposarcoma of the BFP has also been described, which may be clinically and histologically indistinguishable from spindle-cell lipoma. Therefore, the authors recommend a careful workup of every mass of the buccal space with consideration of the BFP as a possible origin. Detailed knowledge of the anatomy and extensive MR-imaging are paramount in guiding the surgical approach by visualising the extent of growth in the various extensions, and determining if the radiological picture is suggestive of liposarcoma. O rejuvenescimento facial é uma área cada vez mais requisitada, sendo o perfil facial afilado o padrão estético mais aceito atualmente. Nesse contexto, existem várias técnicas cirúrgicas que buscam obter esse resultado. A bichectomia, por exemplo, é um procedimento com relatos antigos na literatura, mas que recentemente tornou-se popularizada devido aos seus resultados estéticos. É necessário uma avaliação minuciosa da face para a indicação do procedimento, este pode ser puramente estético ou funcional, onde o paciente acaba mordendo a mucosa jugal durante a mastigação. A cirurgia de bichectomia consiste na remoção do corpo adiposo da bochecha também conhecido como bola de Bichat, localizada externamente ao músculo bucinador e em frente a margem anterior do músculo masseter. As contra indicações são as mesmas de qualquer cirurgia eletiva, tais como: pacientes com infecções locais; pacientes que fazem radioterapia e/ou quimioterapia; cardiopatias severas; imunossupressão; coagulopatia e nefropatias. Objetivo: Análise da cirurgia e seus benefícios e indicação e contra indicações na área odontológica. Metodologia: Foi realizada uma análise minunciosa de artigos científicos sobre a temática. Acessados na base de dados Scielo, Pubmed e Lilacs, foram aplicados descritores em português como: bichectomia, estética, cirurgia, e em inglês como: bichectomy, aesthetics, surgery. Considerações: A remoção cirúrgica da bola de Bichat promove uma diminuição do contorno das bochechas conferindo uma melhor harmonia facial, sendo uma opção viável com poucas complicações e baixa morbidade para pacientes saudáveis , sem condições médicas graves, cessando o comprometimento funcional e estético do paciente. A cirurgia de bichectomia consiste na remoção do corpo adiposo da bochecha também conhecido como bola de Bichat, localizada externamente ao músculo bucinador e em frente a margem anterior do músculo masseter. As contra indicações são as mesmas de qualquer cirurgia eletiva, tais como: pacientes com infecções locais; pacientes que fazem radioterapia e/ou quimioterapia; cardiopatias severas; imunossupressão; coagulopatia e nefropatias. Objetivo: Análise da cirurgia e seus benefícios e indicação e contra indicações na área odontológica. Metodologia: Foi realizada uma análise minunciosa de artigos científicos sobre a temática. Acessados na base de dados Scielo, Pubmed e Lilacs, foram aplicados descritores em português como: bichectomia, estética, cirurgia, e em inglês como: bichectomy, aesthetics, surgery. Considerações: A remoção cirúrgica da bola de Bichat promove uma diminuição do contorno das bochechas conferindo uma melhor harmonia facial, sendo uma opção viável com poucas complicações e baixa morbidade para pacientes saudáveis , sem condições médicas graves, cessando o comprometimento funcional e estético do paciente. Vale ressaltar que alguns pacientes devem ser encaminhados para o psicólogo antes do tratamento, pois conforme visto na literatura, há casos de um nível de exigência estética acima do normal, motivada por pertubações psicopatológicas.

Husserl atribui ao mecanicismo de Descartes a responsabilidade de ter comprometido os desígnios mais universais da filosofia moderna, impregnando-os de um racionalismo objectivista. Pretendemos salientar que esta orientação, apesar de ter sido predominante, não impediu o aparecimento de várias tentativas (Stahl, Espinosa, Leibniz, Bichat, Diderot, etc.) que procuraram reconduzir a compreensão do mundo e do homem a uma maior proximidade dos dinamismos do mundo da vida. ABSTRACT: Husserl attributes to Descartes’ mechanicism the responsibility of having compromised the more universal objectives of modern philosophy, granting them an objectivist rationalism. We intend to emphasize that this orientation, regardless of having been predominant, did not constitute any impediment to the surge of several efforts (Stahl, Spinoza, Leibniz, Bichat, Diderot, etc.) that tried to redirect the comprehension of the world and man to a greater proximity to the dynamisms of the world of life.

Objective: To study the occurrence of cardiac, obstetric, fetal and neonatal complications in pregnant women with the Marfan Syndrome. Results: 16 women had a total of 20 pregnancies resulting in 1 late miscarriage, one intra-uterine fetal death, 4 preterm and 14 at-term deliveries. Of the 18 births of viable children 16 were born by Caesarean Section and 2 by vaginal delivery. One major cardiac complication occurred during the study: an aortic dissection at 37 weeks in a woman whose pregnancy was cons-reported and for which treatment with beta-blockers was started at 35 weeks. 8 intra-uterine growth retardations were diagnosed during pregnancy and 7 children were born small for gestational age. Neonatal complications were found related to maternal treatment with beta-blockers: 1 bradycardia, 1 hypoglycemia, and 1 respiratory distress. For women affected by the Marfan syndrome, when the aortic diameter is less than 45 mm, pregnancy can be reasonably considered in establishing a joint obstetrical and cardiologic follow-up and a therapy with beta-blockade.; Objectif : étudier la survenue de complications cardiologiques, obstétricales, foetales et néonatales chez les femmes enceintes atteintes du Syndrome de Marfan. Résultats : 16 femmes ont eu un total de 20 grossesses conduisant à 1 fausse-couche tardive, une MFIU, 4 accouchements prématurés et 14 accouchements à terme. Parmi les 18 accouchements d'enfants viables, 16 sont nés par césarienne et 2 par voie basse. Une seule complication cardiologique majeure est survenue durant l'étude : une dissection aortique à 37 SA chez une femme dont la grossesse était contre-indiquée et pour laquelle le traitement par bêta-bloquants n'avait pu être mis en place qu'à 35 SA. 8 RCIU ont été diagnostiqués au cours de la grossesse et 7 enfants sont nés hypotrophes. Les complications néonatales retrouvées sont en lien avec le traitement maternel par bêta-bloquants : 1 bradycardie, 1 hypoglycémie, 1 détresse respiratoire. Chez les femmes atteintes du syndrome de Marfan, lorsque le diamètre aortique est inférieur à 45 mm, une grossesse peut être raisonnablement envisagée en mettant en place un suivi conjoint obstétrico-cardiologique régulier et un traitement par bêta-bloquants.
