To study the changes in corneal microstructure and nerve fibers in patients with cornea verticillata in Fabry disease.The study included patients with a confirmed diagnosis of Fabry disease. All patients underwent standard ophthalmological examination. Corneal confocal microscopy was performed when slit-lamp examination revealed cornea verticillata.Cornea verticillata was found in 64.5% of patients. Corneal microscopy revealed hyperreflective deposits in the basal cell layer of corneal epithelium in all patients with cornea verticillata. Characteristic features were desquamation of epithelial cells with islands of epithelial cells deficiency, as well as hyperreflective intracellular deposits in the basal layer. Changes in keratocytes in the form of perinuclear white microdots were found in the stroma of 25.8% of patients. Endothelial layer was not changed and had normal cell density, reflectivity and morphology in all patients with cornea verticillata.Corneal confocal microscopy is a valuable diagnostic tool in patients with Fabry disease and may be important in evaluation of disease progression and monitoring of treatment efficacy.Изучить микроструктурные изменения, а также изменения нервных волокон роговицы у пациентов с вихревидной кератопатией при болезни Фабри.В исследование были включены пациенты с подтвержденным диагнозом болезни Фабри. Все пациенты прошли стандартное офтальмологическое обследование. В случае выявления вихревидной кератопатии при осмотре за щелевой лампой дополнительно проводили конфокальную микроскопию роговицы.Вихревидная кератопатия была обнаружена у 64,5% пациентов. При конфокальной микроскопии у всех пациентов с вихревидной кератопатией были выявлены типичные изменения эпителия роговицы. Характерными признаками были десквамация эпителиоцитов с участками отсутствия эпителиальных клеток поверхностного слоя, а также гиперрефлективные внутриклеточные включения в базальном слое роговицы. В строме у 25,8% пациентов были выявлены изменения кератоцитов в виде белых точек, окружающих ядро. Эндотелиальный слой роговицы у всех пациентов с вихревидной кератопатией не был изменен и имел нормальные плотность клеток, рефлективность и морфологию.Конфокальная микроскопия роговицы является ценным диагностическим методом при обследовании пациентов с болезнью Фабри, а также может иметь важное значение в оценке прогрессирования заболевания и мониторинге эффективности лечения.

IgG4-associated disease (IgG4-RD) is a systemic inflammatory disease characterized by tumorlike sclerosing masses in different organs. Differential diagnosis in orbital IgG4-RD includes majority of conditions, such as thyroid eye disease (TED), sarcoidosis, granulomatosis with polyangiitis, idiopatic orbital inflammation, limphoproliferative diseases and others. A case of IgG4-RD with different organs involvement and complicated differential diagnosis is presented. This case demonstrates very uncommon manifestation of IgG4-RD, when orbital involvement was very similar with TED. Systemic process was not recognized during a long period of time and diagnosis of IgG4-RD was established only after biopsy of abnormally increased lacrimal gland. Differential diagnosis included other systemic diseases, first of all sarcoidosis, GPA, and lymphoma. Biopsy results were consistent with the gold standard of diagnosis, e. g. more than 40% of plasma cells were IgG4 positive. This case demonstrates the necessity of orbital biopsy before starting immunosuppression to avoid inappropriate treatment strategy.IgG4-ассоциированное заболевание (IgG4АЗ) это системное воспалительное заболевание, которое, как правило, характеризуется формированием опухолеподобных склерозирующих очагов. Дифференциальный диагноз при поражении орбиты при IgG4АЗ довольно обширный и включает такие заболевания, как эндокринная офтальмопатия, саркоидоз, гранулематоз с полиангиитом, идиопатическое воспаление орбиты, лимфопролиферативные заболевания и др. В статье представлен клинический случай IgG4АЗ с поражением нескольких органов-мишеней и сложным дифференциальным диагнозом. Этот клинический пример является демонстрацией достаточно необычной манифестации IgG4АЗ, при которой ведущим проявлением было поражение орбит, схожее с таковым при эндокринной офтальмопатии. В течение длительного времени системный процесс не был распознан, и только после проведения биопсии аномально увеличенной слезной железы было диагностировано IgG4АЗ. Дифференциальный диагноз проводили с другими системными заболеваниями, в первую очередь гранулематозом с полиангиитом и саркоидозом, а также с лимфомой. Данные биопсии в представленном случае соответствовали золотому стандарту диагноза IgG4АЗ, т.е. более 40% плазматических клеток были IgG4-позитивны. Этот случай демонстрирует необходимость проведения биопсии патологически измененных тканей орбиты до назначения иммуносупрессивного лечения во избежание ошибок в выборе тактики лечения пациента.

To estimate efficacy and safety of endoscopic orbital decompression in patients with dysthyroid optic neuropathy (DON).19 patients aged 54.8±7.1 y.o. with DON were enrolled into the study.Visual acuity was from counting fingers till 0.9 (0.4±0.28) before surgery, it increased till 0.7±0.2 (This study is the first study of this kind in Russian Federation. Obtained results showed high efficacy and safety of endoscopic orbital decompression in patients with DON.Оценить эффективность и безопасность трансэтмоидальной костной декомпрессии орбиты (ТЭДО) при оптической нейропатии (ОН) у пациентов с эндокринной офтальмопатией (ЭОП).В исследование вошли 19 пациентов (38 глаз) с ОН при ЭОП, которым была выполнена ТЭДО в период с 2015 по 2018 г. Средний возраст составил 54,8±7,1 года.Острота зрения до операции составляла от счета пальцев у лица до 0,9 (в среднем 0,4±0,28), после хирургического вмешательства она повысилась в среднем до 0,7±0,2. Количество прочитанных таблиц Ишихара в среднем увеличилось с 5,8±7 до 13,3±7,6 (Данная работа является первым подобным исследованием в нашей стране. Полученные результаты убедительно демонстрируют эффективность и безопасность ТЭДО в лечении ОН у пациентов с ЭОП.

To evaluate the effectiveness of bony orbital decompression in patients with dysthyroid optic neuropathy (DON).The study analyzed 255 patients with thyroid eye disease (TED) and bony orbital decompression. Those among them who had DON as an indication for surgery were investigated further. Patients underwent standard ophthalmological examination, computer perimetry, color vision assessment using Ishihara tables, relative afferent pupillary defect (RAPD), computed tomography (CT) of the orbit, and in some cases optical coherence tomography (OCT) of the optic nerve.Final analysis included 31 patients (52 eyes). On 13 orbits, only lateral wall decompression was performed, and medial wall decompression was the only intervention in 7 orbits. In other cases, these techniques were performed either simultaneously - in 14 orbits, or alternately - in 18 orbits. In the postoperative period, all patients showed significant positive dynamics in terms of visual acuity, visual field, and proptosis. In all cases, decrease of the amount of orbital inflammation was observed. Exophthalmos significantly decreased after surgery and averaged 20.5±3.1 mm, which is 4.7 mm less than the initial one. All changes were statistically significant (Bony orbital decompression is an effective and safe treatment option for DON resistant to high doses of glucocorticoids. In the vast majority of cases, this intervention is the only way to improve and stabilize visual function in this severe category of patients.Оценить эффективность костной декомпрессии орбиты у пациентов с оптической нейропатией (ОН) при эндокринной офтальмопатии (ЭОП).Проанализированы данные 255 историй болезни пациентов с ЭОП, которым выполнена костная декомпрессия орбиты (ДО). Из них в дальнейшее исследование отобраны пациенты, у которых показанием для ДО являлась ОН. Обследование всех пациентов включало стандартное офтальмологическое исследование, компьютерную периметрию, оценку цветового зрения при помощи таблиц Ишихары, оценку относительного афферентного зрачкового дефекта (RAPD), в ряде случаев оптическую когерентную томографию (ОКТ) зрительного нерва. Всем пациентам проводили компьютерную томографию (КТ) орбит.В окончательный анализ результатов включен 31 пациент (52 глаза). На 13 орбитах проведена только латеральная ДО, на 7 — только медиальная. В остальных случаях эти методики выполнены либо одномоментно — 14 орбит, либо поочередно — 18 орбит. В послеоперационном периоде у всех пациентов отмечалась выраженная положительная динамика по показателям визометрии, периметрии, экзофтальмометрии. Во всех случаях наблюдали снижение активности воспалительного процесса в орбите. Экзофтальм после операции уменьшился и в среднем составил 20,5±3,1 мм, что на 4,7 мм меньше исходного. Все изменения статистически значимые (Костная декомпрессия орбиты является эффективным и безопасным способом лечения оптической нейропатии у пациентов с эндокринной офтальмопатией при резистентности к лечению глюкокортикоидами в высоких дозах. В подавляющем большинстве случаев это вмешательство является единственным способом повышения и стабилизации зрительных функций у этой сложной категории пациентов.

Encephalotrigeminal angiomatosis (Sturge-Weber syndrome) is a congenital sporadic disease characterized by angiomatosis of meningeal vessels, facial and eye capillaries. The article presents a clinical case of comorbidity between monolateral skin angioma, ipsilateral conjunctival and choroidal angiomas without intraocular pressure elevation. The case is unusual due to asynchronous manifestation of the clinical signs of Sturge-Weber syndrome. Facial angioma was diagnosed at birth, while conjunctival angioma appeared at the age of 45 - at the same time as monolateral hypermetropia, which may be the first symptom of diffuse choroidal angioma.Энцефалотригеминальный ангиоматоз, или синдром Sturge-Weber - врожденное, спорадически возникающее заболевание, характеризующееся ангиоматозом сосудов мозговых оболочек, капилляров лица и глаз. В статье описан клинический случай, демонстрирующий сочетание односторонней кожной ангиомы с ангиоматозом конъюнктивы и диффузной гемангиомой хориоидеи на ипсилатеральной стороне без признаков нарушения офтальмотонуса. Интерес данного клинического случая обусловлен тем, что имеет место разновременное появление клинических признаков синдрома Sturge-Weber. Кожная ангиома у пациентки была с рождения, ангиома конъюнктивы появилась в возрасте 45 лет, тогда же у нее появился гиперметропический сдвиг рефракции, что может быть первым проявлением диффузной гемангиомы хориоидеи.

Cornea verticillata is the typical sign of ocular involvement in Fabry disease and manifests by the whorl-like, linear opacities in the inferior part of the cornea. Aim. To study the frequency of cornea verticillata in patients with Fabry disease and it’s relation to the severity of the disease and the types of mutation in the GLA gene. Materials and methods. We studied 69 adult (over 18 years) patients with a classic form of Fabry disease that was confirmed by enzymatic and molecular genetic studies. There were 39 males and 30 females. The median age was 39 years [30.0; 50.0]. The severity of Fabry disease was assessed using the Mainz Severity Score Index (MSSI) with a maximum value of 76 points. Depending on the MSSI score, patients were classified into mild (40) clinical categories. Results and discussion. At least one classic symptom of Fabry disease was present in 88.4% of patients. The majority of patients had the missense mutations of the GLA gene. Cornea verticillata was found in 65.2% of patients and occurred with a similar frequency in males (56.4%) and females (76.7%; p=0.07). Cornea verticillata was the single classic symptom of Fabry disease in only 4.9% of cases, while the rest of the patients presented with angiokeratoma, neuropathic pain and/or hypohidrosis. The frequency of classic symptoms of Fabry disease, as well as renal disease (with the exception of terminal chronic renal failure), brain and heart damage was similar in patients with and without cornea verticillata. Median MSSI scores were also similar in patienths with and without cornea verticillata (20.0 and 18.5 points, respectively; p=0.92). Similar results were obtained in males (26.5 and 30.0 points, p=0.97) and females (16.0 and 16.0 points, p=0.45). The frequency of cornea verticillata did not differ in patients with different types of mutations in the GLA gene. Conclusion. Cornea verticillata occured in 65% of adult patients with Fabry disease, was usually accompanied by the other classic symptoms of the disease, and was not associated with the severity of the disease.

By the reaction of 5-(4-aminophenyl)-1,3,4-oxadiazolin-2-thione with alkyl halides 2-alkylthio-5-(4-aminophenyl)-1,3,4-oxadiazoles, 2–4 were obtained. By the reaction of compounds 2–4 with aromatic aldehydes, new Schiff bases 5a–d and 6a–d were synthesized. The structures of synthesized compounds are confirmed by the IR, UV, 1H NMR for all compounds, 13C NMR for compounds 3, 4, 5a–5d, 6a, 6d and X-ray for compound 3. All substances were tested in vitro for their antibacterial and antifungal activity. The results showed that Compound 2 exhibited remarkable activity against Candida albicans. Compounds 5а–d were found to exhibit a weak selective activity against Gram-positive bacteria of Bacillus subtilis and compounds 6а–d against Gram-negative bacteria of Escherichia coli.

Orbital decompression is a surgical procedure aimed at increasing the orbital volume and/or decreasing the volume of the orbital fat. The indications for orbital decompression are determined in the course of thorough eye examination. An important objective of examination of a patient with thyroid eye disease (TED) is determination of inflammation activity and severity. Orbital decompression is a surgical procedure that can be performed in both the active and nonactive stages of the disease. However, the indications for the surgery in these cases are different. Optic neuropathy and severe corneal disease are threatening complications that may lead to permanent visual loss and generally occur in the presence of active orbital inflammation. If treatment with high-dose corticosteroids has proven ineffective, an urgent surgical procedure consisting of orbital decompression and, in case of involvement of the cornea, eyelid and corneal surgery has to be performed. Owing to significant progress in technology, improvement of methods and accumulated experience over the past decade, the indications for bone orbital decompression have extended compared to the time when this procedure was used only in patients with extremely severe TED. The most common complication of the orbital decompression is the development or deterioration of previously existing binocular diplopia and strabismus. In addition, other parameters may change as well, including the position of the globe, the eyelids, the angle of deviation of the eye, and intraocular pressure. Thus, bone orbital decompression is a major step of a comprehensive, often multistage, system of rehabilitation of patients with severe refractory TED.

To estimate the frequency of lesions in the organ of vision in granulomatosis with polyangiitis (GPA) (Wegener's) and to determine their relationship to systemic diseases.The retrospective study enrolled 218 patients followed up at the E.M. Tareyev Clinic of Nephrology, Internal and Occupational Diseases, with a diagnosis of GPA. The frequency and association of ophthalmic manifestations with systemic involvement were statistically analyzed using PASW Statistics 18.The organ of vision was impaired in 48.1% of the patients with GPA. The most common manifestations were orbital space-occupying lesion (22.9%), conjunctivitis/episcleritis (14.7%), dacryocystitis (6.0%), and scleritis (4.6%). Orbital space-occupying lesions occurred more frequently in the local type of the disease (p=0.0003), and, on the contrary, the involvement of the conjunctiva and eyeball was seen in patients with the systemic types of GPA (p=0.02).The findings may suggest that the orbital lesion is an independent manifestation of GPA, which develops more commonly in its local type. Conjunctivitis/episcleritis is, on the contrary, more frequently seen in the active phase of the disease and generally in the involvement of other organs and systems.Цель исследования. Оценка частоты поражения органа зрения при гранулематозе с полиангиитом (Вегенера) - ГПА и определение их связи с системной патологией. Материалы и методы. В ретроспективное исследование включили 218 пациентов, наблюдавшихся в клинике нефрологии, внутренних и профессиональных болезней им. Е.М. Тареева с диагнозом ГПА. Статистический анализ частоты и взаимо-связи офтальмологических проявлений с системным поражением проводили при помощи программы PASW Statistics 18. Результаты. Орган зрения был поражен у 48,1% больных ГПА. Наиболее частыми офтальмологическими проявлениями служили объемное образование глазницы (22,9%), конъюнктивит/эписклерит (14,7%), дакриоцистит (6%) и склерит (4,6%). Объемные образования глазницы встречались достоверно чаще при локальном варианте заболевания (р=0,0003), а поражение конъюнктивы и глазного яблока, напротив, у пациентов с системными вариантами ГПА (р=0,02). Заключение. Полученные данные позволяют предположить, что поражение глазницы является самостоятельным проявлением ГПА, чаще развивающимся при локальном варианте. Воспаление конъюнктивы/эписклеры, напротив, чаще встречается в активную фазу заболевания и, как правило, при вовлечении других органов и систем.

One of the most common and potentially threatening manifestations of granulomatosis with polyangiitis (GPA) is orbital lesion.To study the clinical course and prognosis of orbital lesions of various localization in GPA patients. Material and me-thods. The study included 226 patients with GPA, 74 of them with orbital lesion. Ophthalmic examination consisted of visual acuity test, biomicroscopy, ophthalmoscopy, exophthalmometry and ocular mobility test. Visualization was done using multislice computed tomography and/or magnetic resonance tomography and/or ultrasound examination.Among the patients of the study group, in 35.1% the lesion was limited to dacryoadenitis, in 4.0% of patients - to myositis, and 70.8% had extensive inflammatory orbital masses. Patients with orbital masses had systemic disease in 51.1%, compared to 7.7% in dacryoadenitis (p=0.00). Clinical progression in patients with orbital masses was characterized by severe exophthalmos, periorbital swelling and hyperemia. Patients with dacryoadenitis, on the contrary, had only mild symptoms. Patients with orbital masses had unfavorable prognosis. There were only three patients with myositis in the study group, so the data on them is limited; their clinical symptoms included light periorbital swelling, exophthalmos, strabismus, painful binocular diplopia and eye movement restriction. The disease was recurring in two cases.Patients with orbital involvement in GPA may have different course of the disease depending on the localization of inflammation.Поражение орбиты является одним из самых частых и потенциально опасных для зрения проявлений гранулематоза с полиангиитом (ГПА). Цель - изучить особенности клинической картины при различной локализации поражения орбиты у пациентов с ГПА. Материал и методы. В исследование включено 226 пациентов с ГПА, 74 из них с поражением орбиты. Офтальмологический осмотр пациентов включал проверку остроты зрения, измерение уровня внутриглазного давления, исследование границ поля зрения, а также биомикроскопию, офтальмоскопию, экзофтальмометрию, исследование подвижности глазного яблока. В качестве визуализирующих методов применяли мультиспиральную компьютерную томографию, и/или магнитно-резонансную томографию, и/или ультразвуковое исследование. Результаты. У 35,1% пациентов поражение ограничивалось развитием дакриоаденита, у 4,0% - миозита, а у остальных 70,8% определялось объемное воспалительное образование орбиты. У последних заболевание носило системный характер - 51,1 против 7,7% (р=0,00) пациентов с дакриоаденитом. Клиническое течение было тяжелым и характеризовалось выраженным экзофтальмом, отеком и покраснением периорбитальных тканей. У пациентов с дакриоаденитом симптомы в целом были не столь выраженными. Исход заболевания также был хуже у пациентов с объемным образованием орбиты. Данные о пациентах с миозитом в рамках ГПА ограничены тремя пациентами. Из клинических симптомов в активной фазе наблюдались незначительный периорбитальный отек, экзофтальм, косоглазие, мучительное бинокулярное двоение, выраженное ограничение подвижности заинтересованных мышц. В двух случаях заболевание носило рецидивирующий характер. Заключение. Пациенты с поражением орбиты при ГПА могут характеризоваться различным клиническим течением заболевания в зависимости от локализации воспаления.

to analyze the results of surgical treatment of patients with thyroid eye disease (TED), including the type of surgery, indications, and outcomes.Hundred thirty-nine patients with TED (112 females, 27 males) aged 45.8±13.6 years.Bony orbital decompression was performed in 91 patients (128 orbits). After the surgery, visual acuity increased from 0.84±0.47 up to 0.94±0.32 (р0.05), exophthalmos got reduced by the average of 3.4 mm. Strabismus surgery was performed in 15 patients. The absence of diplopia in the primary position of gaze was achieved in 12 cases and the degree of residual heterophoria averaged 3.33 PD (2-6 PD). Ocular motility improved from 10.7° up to 34.58°. Eyelid surgery was performed in 81 patients.1. Surgical treatment for thyroid eye disease is indicated in patients with visual impairment and/or significant decrease in the quality of life, if pathogenetic treatment fails. 2. The choice of surgical strategy depends on several factors, such as TED activity and severity, thyroid function, pathologic changes in the orbital tissues, and surgeon's experience and preference. 3. Current surgical techniques for thyroid eye disease enable correction of disease consequences, such as exophthalmos, optic neuropathy, impairment of ocular movement, and exposure keratopathy. 4. Adherence to the systematic approach that includes bony orbital decompression, strabismus surgery, and correction of lagophthalmos and eyelid retraction, ensures high-level functional and cosmetic rehabilitation in patients with TED.Цель — анализ результатов хирургического лечения эндокринной офтальмопатии (ЭОП), включая вид, показания и результаты хирургических вмешательств. Материал и методы. В исследование включены 139 пациентов (112 женщин и 27 мужчин) с ЭОП, средний возраст 45,8±13,6 года. Результаты. Костная декомпрессия орбиты (КДО) была выполнена 91 пациенту (всего 128 операций). В результате хирургического лечения острота зрения повысилась с 0,84±0,47 до 0,94±0,32 (р0,05). Экзофтальм после операции уменьшился в среднем на 3,4 мм. Операции на экстраокулярных мышцах были выполнены у 15 пациентов. В результате хирургического лечения в 12 случаях удалось добиться отсутствия двоения в первичном положении взора, а средняя величина гетерофории составила 3,33 (2—6) PD. Подвижность глаза улучшилась с 10,7 до 34,58°. Операции на веках были проведены 81 пациенту. Выводы. 1. Хирургическое лечение пациентов с ЭОП показано при нарушении зрительных функций и/или значительном снижении качества жизни при неэффективности патогенетически ориентированного лечения. 2. Выбор тактики хирургического лечения зависит от ряда факторов: тяжести и активности ЭОП, функции щитовидной железы и уровня гормонов, патологических изменений тканей орбиты, а также предпочтений и опыта хирурга. 3. Современные методики хирургического лечения ЭОП позволяют эффективно устранять такие последствия этого заболевания, как экзофтальм, оптическая нейропатия, глазодвигательные нарушения и экспозиционная патология роговицы. 4. Следование систематизированному подходу, включающему проведение КДО, устранение косоглазия, лагофтальма и ретракции век обеспечивает высокий уровень функциональной и косметической реабилитации пациентов с ЭОП.

Tumor borders are one of the most significant characteristics of any tumor, including that of the skin.To compare histological borders of periorbital skin tumors with their autofluorescence borders built from the analysis of non-induced protoporphyrin IX autofluorescence.The study group included 8 patients with skin tumors of the eyelids, periorbital region, eyebrow and zygomatic regions aged 54-88 years. The tumors varied in size from 2 to 8 mm and all displayed signs of basal cell carcinoma (BCC). At admission, all the patients underwent non-induced autofluorescence diagnosis. The images were processed with the «CancerPlot» program. During radio excision, the autofluorescent border of each neoplasm was marked with a surgical incision of about 5 mm long and 2 mm deep.Upon pathomorphological examination, solid BCC was identified in 7 cases. The remaining case was senile keratosis. All reference incisions were located in healthy tissues not farther than 1 mm from the tumor (or keratosis locus, correspondingly).By the example of facial BCC, an evident correlation was established between histological borders of the tumor and its native (non-induced) protoporphyrin IX autofluorescence.Одной из самых значимых характеристик опухоли, в том числе опухоли кожи, являются ее границы.Сопоставление гистологических границ новообразований кожи периорбитальной области с их аутофлуоресцентными границами, проводимыми на основе анализа спектров неиндуцированной аутофлуоресценции протопорфирина IX.В группу исследования вошли 8 пациентов с новообразованиями кожи век, периорбитальной, бровной и скуловой областей в возрасте от 54 до 88 лет. Размер новообразований варьировал от 2 до 8 мм. При поступлении пациентам проводилась аутофлуоресцентная диагностика без применения индукторов флуоресценции с обработкой изображений в программе «Канцерплот». В ходе радиоэксцизии новообразования вдоль его аутофлуоресцентной границы наносили хирургическую насечку протяженностью около 5 мм и глубиной 2 мм.При патоморфологическом исследовании базально-клеточный рак (БКР) солидного типа был выявлен в 7 случаях. Еще один случай был идентифицирован как сенильный кератоз. Хирургическая насечка во всех случаях злокачественного процесса располагалась в пределах гистологически неизмененных тканей, не накладываясь на опухолевый комплекс, но и не удаляясь от него более чем на 1 мм. В случае сенильного кератоза насечка также располагалась в 1 мм от гистологической границы процесса.Заключение. На примере узлового БКР кожи лица впервые наглядно продемонстрировано соотношение гистологических и аутофлуоресцентных границ опухоли, полученных без использования индукторов флуоресценции.

To develop a therapeutic approach and to estimate the efficiency of complex treatment of corneal damage in patients with thyroid eye disease (TED).The study enrolled 44 patients (52 eyes) divided into 2 groups depending on the severity of corneal damage. Treatment of those with severe involvement included pathogenetic measures (pulse steroid therapy and/or radiation therapy) and surgery (orbital decompression, eyelid and corneal surgery).As the result of the treatment, orbital inflammation decreased and the state of the cornea improved in all patients.The treatment of corneal damage in patients with TED may differ depending on numerous factors, such as the severity of corneal damage and activity of orbital inflammation. Taking into account the potential danger of corneal involvement, one should make efforts to early detection and management of the risk factors.Цель - разработка тактики лечения и изучение эффективности комбинированного лечения пациентов с поражением роговицы при эндокринной офтальмопатии (ЭОП). Материал и методы. Пациенты (44 человека, 52 глаза) были разделены на 2 группы в зависимости от тяжести поражения роговицы. При тяжелом поражении роговицы (18 пациентов, 21 глаз) комбинированное лечение включало сочетание местной терапии, патогенетически ориентированного лечения (пульс-терапия глюкокортикоидами, лучевая терапия) и хирургические вмешательства (декомпрессию орбиты, операции на веках и роговице). Результаты. У всех пациентов в результате лечения было достигнуто снижение активности орбитального воспалительного процесса и значительное улучшение состояния роговицы. Заключение. Таким образом, лечение пациентов с поражением роговицы при ЭОП может быть различным и его необходимо проводить, учитывая множество факторов. Выбор лечебных мероприятий зависит прежде всего от активности орбитального воспаления и степени выраженности изменений роговицы. Кроме того, учитывая потенциальную тяжесть вовлечения роговицы в патологический процесс при ЭОП, необходимо выявлять факторы риска на ранних этапах и проводить мероприятия, направленные на их коррекцию.Цель - разработка тактики лечения и изучение эффективности комбинированного лечения пациентов с поражением роговицы при эндокринной офтальмопатии (ЭОП). Материал и методы. Пациенты (44 человека, 52 глаза) были разделены на 2 группы в зависимости от тяжести поражения роговицы. Precise instrument localization is of great importance in orbital decompression surgery. Different navigation systems were designed to fulfill this task and gained wide acceptance.to describe principal features and determine advantages of intraoperatively navigated orbital bony decompression.Data on orbital decompression surgery performed with or without intraoperative image guidance is presented. Each time two surgeons were involved - an ophthalmologist (lateral wall decompression) and otolaryngologist (medial wall decompression).In lateral wall decompression, electromagnetic navigation was mostly used to enlarge the created bone window upward and backward, which is particularly challenging in case of bone thinning in the posterior part of its inner rim. In transethmoidal decompression, the image guidance allowed a more extensive removing of the greater sphenoid wing as well as the posterior medial wall where it joins the lateral wall of the sphenoid sinus. As shown by MSCT, lateral rectus muscle dislocation into the created bone window was more significant in navigated procedures.This preliminary data convincingly shows advantages and potentials of the use of electromagnetic navigation in orbital decompression surgery, especially, for optic neuropathy in thyroid eye disease patients.При выполнении костной декомпрессии орбиты крайне важно четкое понимание локализации инструмента в момент манипуляции. Именно эту цель преследуют навигационные системы, получившие уже довольно широкое распространение. Цель - описание особенностей и выявление преимуществ костной декомпрессии орбиты с использованием интраоперационной навигации. Материал и методы. Под наблюдением находился пациент с эндокринной офтальмопатией, которому впервые в нашей стране была выполнена костная декомпрессия орбиты с использованием интраоперационной электромагнитной навигации. Вмешательство выполнялось двумя специалистами: латеральная стенка - офтальмологом, медиальная декомпрессия - ЛОР-хирургом. Результаты. При латеральной декомпрессии навигационный контроль использовали в основном на этапе расширения костного 'окна' кзади и кверху, особенно при истончении кости в задней части внутреннего края костного окна наружной стенки орбиты. При выполнении трансэтмоидальной декомпрессии использование навигационной системы позволило с большой точностью провести резекцию костного остова медиальной стенки орбиты в задних ее отделах, особенно в месте перехода и сочленения с латеральной стенкой клиновидной пазухи. По данным послеоперационной мультиспиральной компьютерной томографии выявлено более выраженное смещение наружной прямой мышцы в сформированное костное окно при использовании навигации. Заключение. Представленные предварительные данные убедительно демонстрируют преимущества и перспективность использования электромагнитной навигационной системы при декомпрессионных операциях на орбите, в частности при оптической нейропатии у пациентов с эндокринной офтальмопатией.

Granulomatosis with polyangiitis (GPA), formerly earlier known as Wegener's granulomatosis, is primary system necrotizing vasculitis of small and medium-sized vessels, which is characterized by the formation of foci of granulomatous inflammation. One of the target organs in GPA is the organ of vision. Its affection may not only reduce quality of life, but also may lead to the severest complications even to fatal outcome. The affection varies in severity from slight inflammation of the ocular superficial structures to severe involvement of all orbital and ocular structures to develop complete blindness, including bilateral one. GPA can affect any portion of the organ of vision. The changes are low specific, which makes early diagnosis and timely adequate therapy difficult. Overall all ophthalmologic manifestations can be divide into 4 groups: 1) lesions of the orbit and appendages; 2) those of the conjunctiva and fibrous tunic of the eyeball: 3) those of the retina: 4) those of the optic nerve. Orbital affection is characterized by the development of granulomas, dacryoadenitis with marked inflammatory infiltration of the surrounding tissues, including oculomotor muscles, and destruction of orbital bony walls. Nonspecific conjunctivitis and episcleritis are commonly encountered. Affection of the fibrous tunic of the eyeball is characterized by the development of necrotizing scleritis, peripheral ulcerative or stromal keratitis. The retina is rarely involved and mainly described as sporadic cases in the literature. The optic nerve is most commonly damaged due to the compression of the orbit by inflamed tissue; the lesion is more infrequently ischemic.Гранулематоз с полиангиитом (ГПА), ранее известный как гранулематоз Вегенера, - первичный системный некротизирующий васкулит сосудов мелкого и среднего калибров, характеризующийся развитием очагов гранулематозного воспаления. Одним из органов-мишеней при ГПА является орган зрения. Поражение его может не только существенно снижать качество жизни пациента, но и в редких случаях приводить к тяжелейшим осложнениям вплоть до летального исхода. Тяжесть поражения варьирует от незначительного воспаления поверхностных структур глаза до тяжелого вовлечения всех структур глазницы и глаза с развитием полной слепоты, в том числе двусторонней. При ГПА поражаться может любой отдел органа зрения. Изменения малоспецифичны, что затрудняет раннюю диагностику и своевременное назначение адекватной терапии. В целом все офтальмологические проявления можно разделить на 4 группы: поражение глазницы и придаточного аппарата, поражение конъюнктивы и фиброзной оболочки глазного яблока, поражение внутренних оболочек глазного яблока и поражение зрительного нерва. Поражение глазницы характеризуется развитием гранулем, дакриоаденита с выраженной воспалительной инфильтрацией окружающих тканей, в том числе глазодвигательных мышц, деструкцией костных стенок глазницы. Нередко встречаются неспецифические конъюнктивиты, эписклериты. Поражение фиброзной оболочки характеризуется развитием некротизирующего склерита, периферического язвенного или стромального кератитов. Внутренние оболочки глазного яблока поражаются редко и в литературе в основном описаны единичные случаи. Зрительный нерв чаще всего страдает в результате компрессии воспаленными тканями глазницы, реже поражение имеет ишемический характер.

We report a case of primary orbital osteoma originated from the sphenoid and notable for a mismatch between its giant size and mild clinical presentation. A lot of attention has been paid to the choice of surgical technique.В статье описан случай первичной остеомы орбиты, происходящей из клиновидной кости. Особенностями данного случая являются отсутствие выраженной симптоматики при больших размерах новообразования, а также метод хирургического удаления остеомы.

The article reveiws the problems of bony orbital decompression for thyroid-related orbitopathy. Indications for this kind of surgery and different approaches are discussed. Advantages and limitations of each method are analyzed. Special attention is paid to complications of different bony orbital decompression approaches, complication rate and its efficacy.В статье проанализирована отечественная и иностранная литература, касающаяся костной декомпрессии орбиты при эндокринной офтальмопатии (ЭОП). Описаны показания к проведению, различные виды декомпрессии орбиты. Проанализированы преимущества и недостатки каждого метода. Отдельно описаны осложнения различных методик, их частота и последствия, а также эффективность.

Full-thickness eyelid defect closure requires appropriate reconstruction, function recovery and acceptable cosmetic result. The aim of the study was to study the results of eyelid reconstruction after full-thickness resections in different benign and malignant tumors. 241 patients (63,5% females, 36,5% males) who underwent full-thickness resectioh of the lower or the upper eyelid due to tumor with primary defect closure were enrolled in the study. Eyelid resection was performed within visually clear tissue 2 mm away of the tumor border in benign and at least 3 mm in malignant lesions. Presented results of different techniques of eyelid reconstruction after full-thickness resections show good functional and cosmetic rehabilitation of patients and low complication rate.

To study the incidence and risk factors of ocular surface damage in thyroid eye disease (TED) and to determine histological changes underlying positive vital staining in this condition.Forty-six patients (92 eyes) with TED were included in this study. Routine ophthalmologic examination, Schirmer test I, vital staining and corneal sensitivity were performed. Fifteen patients with positive vital staining underwent impression cytology and incisional biopsy.Positive vital staining with lissamine green was observed in 56 eyes (60.9%), 30 patients (65.2%). The average degree of staining was 4.57 ± 0.44 (National Eye Institute Workshop grading system). Severe dry eye syndrome was found in 16%. The following histological changes of conjunctiva were revealed: significant epithelial dystrophy with cell polymorphism, goblet cells loss, excessive desquamation and epithelial keratinization with local leukocytic infiltration of substantia propria.According to our results dry eye syndrome is present in 65.2% of patients (60.9% eyes) with TED. Significant risk factors of ocular surface damage in TED were exophthalmos, lagophthalmos, palpebral fissure height and lower lid retraction. Positive conjunctival staining results from punctuate epithelial erosions and excessive desquamation of superficial cells. Histopathologic changes detected in conjunctiva consistent with dry eye and are not specific for TED.

Orbital granulomatosis with polyangiitis (Wegener's granulomatosis, GPA), which is characterized by granulomatous inflammation with small-vessel vasculitis, can develop in local and generalized forms of the disease. The introduction of current immunosuppressive therapy regimens has improved the prognosis of the disease; however, there are immunosuppressive treatment-refractory forms of GPA, the morphology of which has been inadequately investigated. The paper describes a clinical case of refractory GPA involving the orbit, as evidenced by histological and immunohistochemical examinations. The specific feature of the case is the development of severe fibrosis with the accumulation of mainly type III collagen and the persistence of granulomatous inflammation and productive-destructive vasculitis.Гранулематоз с полиангиитом (гранулематоз Вегенера, ГПА) орбиты характеризуется развитием гранулематозного воспаления с васкулитом сосудов мелкого калибра, может развиваться при локальной и генерализованной формах заболевания. После введения современных схем иммуносупрессивной терапии прогноз заболевания улучшился, однако, сохраняются рефрактерные к терапии формы ГПА, морфология которых недостаточно изучена. Представлен клинический случай рефрактерного ГПА с поражением орбиты с гистологическим и иммуногистохимическим исследованием. Особенность заключается в развитии тяжелого фиброза с накоплением преимущественно коллагена III типа и персистенцией гранулематозного воспаления и продуктивно-деструктивного васкулита.

Granulomatous orbital cellulitis is one of the most common ocular manifestations of granulomatosis with polyangiitis (Wegener's). The process is often refractory to conventional immunosuppressive therapy and requires a more radical treatment approach. However, surgical experience with this type of patients is limited. There have been just a few reported cases of orbital decompression in such patients and many authors have doubted the appropriateness of the procedure, since it is associated with a high risk of potentially fatal complications. Our experience demonstrates the feasibility and effectiveness of orbital mass removal as to pain relief and exophthalmos reduction.Одним из самых частых офтальмологических проявлений гранулематоза с полиангиитом (Вегенера) является гранулематозное воспаление орбитальной клетчатки. Зачастую такое поражение рефрактерно к стандартной консервативной иммуносупрессивной терапии и требует более радикальных методов лечения. Однако опыт хирургического лечения таких пациентов ограничен. В литературе описаны единичные случаи декомпрессии орбиты у таких больных и их целесообразность оспаривается многими авторами с учетом высокого риска развития потенциально фатальных осложнений. Наш пример демонстрирует возможность и эффективность удаления орбитальных масс с целью уменьшения болевого синдрома и экзофтальма.

The goal of the study was to determine morphological changes of conjunctiva underlying positive vital staining in TED. 46 patients (92 eyes) with TED were included into the study. Besides routine ophthalmologic examination they underwent impression cytology and histology. According to our results positive conjunctival staining in TED is caused by punctuate epithelial erosions and excessive desquamation of superficial cells. Revealed changes are not specific for TED.

Risk factors of corneal involvement and association of keratopathy with HBD-2 gene expression were evaluated in patients with Thyroid Eye Disease (TED). 46 patients (92 eyes) were included into the study. Investigation included routine ophthalmological examination, palpebral measurements, Schirmer test I, vital staining, corneal sensitivity evaluation. The level of HBD-2 gene expression was evaluated in conjunctival cells using real time PCR. The following factors were found to be associated with corneal involvement in TED patients: activity of orbital inflammation, proptosis, lagophthalmos, palpebral fissure height, lower lid retraction, grade of conjunctival vital staining and low corneal sensitivity. Decrease of HBD-2 gene expression may result in corneal damage in TED patients.

The authors report the results of the first experience in using the stabilized hyaluronic acid preparation in patients with lagopthalmos in the presence of facial nerve palsy and thyroid eye disease and resultant keratopathy of varying degrees. The study included 21 patients, including 15 patients with facial nerve palsy and 6 with endocrine ophthalmopathy. The gel was injected externally to the levator aponeurosis and/or intramuscular, and/or under the pretarsal portion of the orbicularis oculi muscle, and/or subcutaneously. The use of this method led to a significant reduction of lagophthalmos and to a considerable corneal improvement. That of this procedure permitted avoidance of surgical intervention in some patients. The mean follow-up period after injection was 11.2 months (range 6-24 months).

In endocrine ophthalmolopathy, there may be persistent dysfunction of the organ of vision due to corneal lesion or optical neuropathy. As an illustration of severe corneal lesion in edematous exophthalmos decompensation, the authors give their own observation that shows it necessary to use in such cases complex treatment comprising pulse therapy with glucocorticoids, followed by a long-term dosage reduction and switching over to its oral administration, as well radiotherapy and surgery.
