1. RESIST ACINETO test for the rapid detection of NDM and OXA acquired carbapenemases directly from blood culture in Acinetobacter species

Abstract

ABSTRACT Carbapenem-resistant Acinetobacter baumannii (CRAB) are increasingly reported worldwide and a leading cause of mortality associated with antimicrobial resistance. Their early detection, particularly in the cases of bloodstream infections, is crucial in attempting to initiate effective antibiotic treatment. The immunochromatographic assay RESIST ACINETO (Coris BioConcept) is a new test developed for the detection of OXA-23, OXA-40/58, and New-Delhi Metallo-beta-lactamase (NDM) carbapenemases in Acinetobacter spp. We evaluated this test on a collection of 121 Acinetobacter spp. clinical isolates, including 104 carbapenemase producers (97 carbapenemases targeted by the test) and 17 non-carbapenemase producers. The performance of the RESIST ACINETO test was evaluated according to the manufacturer’s recommendations from bacterial and blood cultures. The strains producing the carbapenemases OXA-23, -40, -58, or/and NDM were accurately detected from bacterial cultures and directly from blood cultures, with the exception of one OXA-23/NDM-1-positive Acinetobacter radioresistens isolate (only detected through standard culture). None of the non-carbapenemase producers tested positive. The RESIST ACINETO test demonstrated sensitivity/specificity of 100%/100% and 99%/100% on bacterial and blood cultures, respectively.IMPORTANCEThe incidence of bloodstream infections with carbapenem-resistant Acinetobacter baumannii (CRAB) could be very high in some countries such as the Balkans or Southeast Asia. In case of positive blood cultures with Gram-negative bacteria, the use of the RESIST ACINETO test could prove highly beneficial for the rapid identification of these imipenem-resistant bacteria and their antibiotic resistance mechanisms. In addition, it is now well established that New-Delhi Metallo-beta-lactamase (NDM) carbapenemase-producing isolates can have increased MICs of cefiderocol, which is an alternative treatment for these infections. This test may also allow the optimization of treatment based on the type of carbapenemase present. Finally, the RESIST ACINETO test is a rapid, easy-to-use, and cost-effective assay that demonstrates excellent performance in detecting the major acquired carbapenemases present in the Acinetobacter species.

Abstract

The incidence of campylobacteriosis has substantially increased over the past decade, notably in France. Secondary localizations complicating invasive infections are poorly described. We aimed to describe vascular infection or endocarditis caused by Campylobacter spp. We included 57 patients from a nationwide 5-year retrospective study on Campylobacter spp. bacteremia conducted in France; 44 patients had vascular infections, 12 had endocarditis, and 1 had both conditions. Campylobacter fetus was the most frequently involved species (83%). Antibiotic treatment involved a β-lactam monotherapy (54%) or was combined with a fluoroquinolone or an aminoglycoside (44%). The mortality rate was 25%. Relapse occurred in 8% of cases and was associated with delayed initiation of an efficient antimicrobial therapy after the first symptoms, diabetes, and coexistence of an osteoarticular location. Cardiovascular Campylobacter spp. infections are associated with a high mortality rate. Systematically searching for those localizations in cases of C. fetus bacteremia may be warranted.

Abstract

Cefiderocol is a siderophore-conjugated cephalosporin with potent activity against multidrug-resistant Gram-negative pathogens including Acinetobacter baumannii. The aim of this study was to evaluate cefiderocol testing methods on a relevant collection of 97 Acinetobacter spp. isolates. Commercialized broth microdilution methods (ComASP®, Liofilchem and UMIC®, Bruker), MIC test strips (Liofilchem) and disc diffusion using discs of three different brands (Mast Diagnostic, Liofilchem and Oxoid—Thermo Fisher Scientific) were compared with the broth microdilution reference method. None of the methods tested fulfilled acceptable criteria (essential agreement [EA] ≥ 90%; bias = ±30%) but both BMD methods achieved acceptable categorical agreement rates (CA = 95.9% [93/97, 95% CI 89.9–98.4] and CA = 93.8% [91/97, 95% CI 87.2–97.1] for ComASP® and UMIC®, respectively) and bias < 30% (−7.2% and −25.2% for ComASP® and UMIC®, respectively). The use of MIC gradient testing is strongly discouraged due to misclassification of 55% (n = 23/42) of resistant strains. Finally, the disc diffusion method could be used to rapidly screen for susceptible strains by setting a critical diameter of 22 mm.

Abstract

In the context of increasing antimicrobial resistance in Enterobacterales, the management of these UTIs has become challenging. We retrospectively assess the prevalence of antimicrobial resistance in Enterobacterales isolates recovered from urinary tract samples in France, between 1 September 2017, to 31 August 2018. Twenty-six French clinical laboratories provided the susceptibility of 134,162 Enterobacterales isolates to 17 antimicrobials. The most frequent species were E. coli (72.0%), Klebsiella pneumoniae (9.7%), Proteus mirabilis (5.8%), and Enterobacter cloacae complex (2.9%). The overall rate of ESBL-producing Enterobacterales was 6.7%, and ranged from 1.0% in P. mirabilis to 19.5% in K. pneumoniae, and from 3.1% in outpatients to 13.6% in long-term care facilities. Overall, 4.1%, 9.3% and 10.5% of the isolates were resistant to cefoxitin, temocillin and pivmecillinam. Cotrimoxazole was the less active compound with 23.4% resistance. Conversely, 4.4%, 12.9%, and 14.3% of the strains were resistant to fosfomycin, nitrofurantoin, and ciprofloxacin. However, less than 1% of E. coli was resistant to fosfomycin and nitrofurantoin. We identified several trends in antibiotics resistances among Enterobacterales isolates recovered from the urinary tract samples in France. Carbapenem-sparing drugs, such as temocillin, mecillinam, fosfomycin, cefoxitin, and nitrofurantoin, remained highly active, including towards ESBL-E.

Abstract

We report a case of infective endocarditis due to Neisseria sicca complicated by intracranial and popliteal aneurysms and hepatic and splenic infarcts in a patient with a bicuspid aortic valve. No predisposing factor other than poor dental condition was found. The patient fully recovered after antibiotic therapy, aortic and mitral valve replacement, endovascular occlusion of the middle-cerebral artery aneurysm, and surgical treatment of the popliteal artery aneurysm.

Abstract

Background Campylobacter spp. bacteremia is a severe infection. A nationwide 5-year retrospective study was conducted to characterize its clinical features and prognostic factors. Methods The study included patients with Campylobacter spp. bacteremia diagnosed in 37 French hospitals participating in the surveillance network of the National Reference Center for Campylobacters and Helicobacters, from 1 January 2015 to 31 December 2019. The goal was to analyze the effects of a delay of appropriate antibiotic therapy and other risk factors on 30-day mortality rates, antibiotic resistance, patient characteristics, and prognosis according to the Campylobacter species. Results Among the 592 patients, Campylobacter jejuni and Campylobacter fetus were the most commonly identified species (in 42.9% and 42.6%, respectively). The patients were elderly (median age 68 years), and most had underlying conditions, mainly immunodepression (43.4%), hematologic cancers (25.9%), solid neoplasms (23%), and diabetes (22.3%). C. jejuni and Campylobacter coli were associated with gastrointestinal signs, and C. fetus was associated with secondary localizations. Among the 80 patients (13.5%) with secondary localizations, 12 had endocarditis, 38 vascular, 24 osteoarticular, and 9 ascitic fluid infections. The 30-day mortality rate was 11.7%, and an appropriate antibiotic treatment was independently associated with 30-day survival (odds ratio, 0.47 [95% confidence interval, .24–.93]; P = .03). The median efficient therapy initiation delay was quite short (2 days [interquartile range, 0–4 days]) but it had no significant impact on the 30-day mortality rate (P = .78). Conclusions Campylobacter spp. bacteremia mainly occurred in elderly immunocompromised individuals with variable clinical presentations according to the species involved. Appropriate antimicrobial therapy was associated with improved 30-day survival.

To evaluate the performance of commercially available tests to determine the susceptibility of multidrug resistant (MDR) clinical Pseudomonas aeruginosa strains to cefiderocol.A collection of 150 clinical strains of P. aeruginosa resistant to ceftazidime, (CMI8 mg/L) imipenem (CMI4 mg/L) and ceftolozane/tazobactam (CMI4/4 mg/L), isolated from 2015 to 2022 was selected. Cefiderocol susceptibility was determined in parallel (i) by disc diffusion using MAST, Oxoid and Liofilchem discs deposited on Mueller Hinton agar batches (MHA) from Bio-Rad, BioMérieux, MAST, Becton Dickinson, I2A, and Oxoid, (ii) by MTS gradient strips (Liofilchem), and (iii) by EUMDROXF Sensititre microplates. MICs and inhibition zones were compared to the broth microdilution reference method (BMD) MICs.The MICDetermination of cefiderocol susceptibility in MDR P. aeruginosa clinical strains by Sensititre EUMDROXF microplates is an alternative to the reference BMD method. However, MIC values ± 1 dilution apart from the breakpoint (2 mg/L) should be controlled by BMD; whereas the use of MTS gradient strips is discouraged. Disc diffusion might be useful for screening, unfortunately many cefiderocol resistant strains are not detected.

Abstract

We construct modal outgoing Green's kernels for the simplified Galbrun's equation under spherical symmetry, in the context of helioseismology. The coefficients of the equation are C 2 functions representing the solar interior model S , complemented with an isothermal atmospheric model. We solve the equation in vectorial spherical harmonics basis to obtain modal equations for the different components of the unknown wave motions. These equations are then decoupled and written in Schrodinger form, whose coefficients are shown to be C 2 apart from at most two regular singular points, and to decay like a Coulomb potential at infinity. These properties allow us to construct an outgoing Green's kernel for each spherical mode. We also compute asymptotic expansions of coefficients up to order r − 3 as r tends to infinity, and show numerically that their accuracy is improved by including the contribution from the gravity although this term is of order r − 3 .

Abstract

Context. In a previous paper, we studied the effect of latitudinal rotation on solar equatorial Rossby modes in the β-plane approximation. Since then, a rich spectrum of inertial modes has been observed on the Sun, which is not limited to the equatorial Rossby modes and includes high-latitude modes. Aims. Here we extend the computation of toroidal modes in 2D to spherical geometry using realistic solar differential rotation and including viscous damping. The aim is to compare the computed mode spectra with the observations and to study mode stability. Methods. At a fixed radius, we solved the eigenvalue problem numerically using a spherical harmonics decomposition of the velocity stream function. Results. Due to the presence of viscous critical layers, the spectrum consists of four different families: Rossby modes, high-latitude modes, critical-latitude modes, and strongly damped modes. For each longitudinal wavenumber m ≤ 3, up to three Rossby-like modes are present on the sphere, in contrast to the equatorial β plane where only the equatorial Rossby mode is present. The least damped modes in the model have eigenfrequencies and eigenfunctions that resemble the observed modes; the comparison improves when the radius is taken in the lower half of the convection zone. For radii above 0.75 R⊙ and Ekman numbers E −4, at least one mode is unstable. For either m = 1 or m = 2, up to two Rossby modes (one symmetric and one antisymmetric) are unstable when the radial dependence of the Ekman number follows a quenched diffusivity model (E ≈ 2 × 10−5 at the base of the convection zone). For m = 3, up to two Rossby modes can be unstable, including the equatorial Rossby mode. Conclusions. Although the 2D model discussed here is highly simplified, the spectrum of toroidal modes appears to include many of the observed solar inertial modes. The self-excited modes in the model have frequencies close to those of the observed modes with the largest amplitudes.

Abstract

Study Analysis Group 21 (SAG21) of NASA's Exoplanet Exploration Program Analysis Group (ExoPAG) was organized to study the effect of stellar contamination on space-based transmission spectroscopy, a method for studying exoplanetary atmospheres by measuring the wavelength-dependent radius of a planet as it transits its star. Transmission spectroscopy relies on a precise understanding of the spectrum of the star being occulted. However, stars are not homogeneous, constant light sources but have temporally evolving photospheres and chromospheres with inhomogeneities like spots, faculae, plages, granules, and flares. This SAG brought together an interdisciplinary team of more than 100 scientists, with observers and theorists from the heliophysics, stellar astrophysics, planetary science, and exoplanetary atmosphere research communities, to study the current research needs that can be addressed in this context to make the most of transit studies from current NASA facilities like HST and JWST. The analysis produced 14 findings, which fall into three Science Themes encompassing (1) how the Sun is used as our best laboratory to calibrate our understanding of stellar heterogeneities ("The Sun as the Stellar Benchmark"), (2) how stars other than the Sun extend our knowledge of heterogeneities ("Surface Heterogeneities of Other Stars") and (3) how to incorporate information gathered for the Sun and other stars into transit studies ("Mapping Stellar Knowledge to Transit Studies"). Abstract

ObjectivesTo assess performance of disc diffusion, gradient tests and Vitek 2 system to determine the susceptibility of clinical Pseudomonas aeruginosa strains to ceftolozane/tazobactam (C/T) and ceftazidime/avibactam (CZA).MethodsTwo-hundred non-duplicate P. aeruginosa strains isolated by 47 French medical laboratories were selected to cover a wide range of C/T and CZA MICs. Performance of C/T disc (30/10 μg, Bio-Rad), CZA discs (10/4 μg) (Thermo Fisher and Bio-Rad), C/T and CZA gradient tests (Etest, BioMérieux; MIC Test Strip, Liofilchem), and AST-XN12 card of Vitek 2 system (BioMérieux) were compared with a broth microdilution (BMD) method (Thermo Fisher). MIC and disc results were interpreted using current EUCAST breakpoints.ResultsTwenty percent and 17% of strains were resistant to C/T and CZA, respectively. All the methods tested satisfactorily determined the susceptibility of P. aeruginosa to C/T [Category Agreement (CA) ≥95%] except the disc diffusion method. Because of the high rates of Major Errors (MEs) (12.5%), this latter method tends to overestimate the resistance. For CZA, only the gradient tests yielded more than 90% of CA. The Vitek 2 system and disc diffusion misclassified 18.1%, 10.1% (disc Bio-Rad) and 11.9% (disc Thermo Fisher) of susceptible strains, respectively.ConclusionsThe gradient tests (MIC Test Strip and Etest) and Vitek 2 card XN12 performed the best to determine the susceptibility of P. aeruginosa to C/T, whereas gradient tests were an acceptable alternative to BMD to assess CZA susceptibility.

Abstract

The oscillations of a slowly rotating star have long been classified into spheroidal and toroidal modes. The spheroidal modes include the well-known 5-min acoustic modes used in helioseismology. Here we report observations of the Sun's toroidal modes, for which the restoring force is the Coriolis force and whose periods are on the order of the solar rotation period. By comparing the observations with the normal modes of a differentially rotating spherical shell, we are able to identify many of the observed modes. These are the high-latitude inertial modes, the critical-latitude inertial modes, and the equatorial Rossby modes. In the model, the high-latitude and critical-latitude modes have maximum kinetic energy density at the base of the convection zone, and the high-latitude modes are baroclinically unstable due to the latitudinal entropy gradient. Abstract

International audience; Temocillin is used for several years in some European countries but, only since 2015 in France. We assessed the susceptibility of Enterobacterales strains isolated from blood culture 1 year before (2014) and 2 years after (2017) its use in France. 1,387 strains were included by 17 clinical laboratories located throughout France: 363 in 2014 and 1,024 in 2017. The rate of resistance to temocillin was 4.6% and 26.7% in 3rd generation cephalosporin (3GC) susceptible and resistant strains respectively. Cephalosporinase-overproducer (COPE) strains were significantly more resistant to temocillin (37.7%) than ESBL-producer (ESBL-PE) (23.5%) (P < 0.01). The rate of temocillin resistance was correlated to the number of inactive beta-lactams. The rate of resistance to temocillin trend to increase from 13.9% in 2014 to 23.9% in 2017 (P < 0.01). Temocillin remains highly active against Enterobacterales but the trend in resistance should be assessed over time.

International audience; Objectives - Time to blood culture positivity (TTP), a routinely available parameter in automated blood culture systems, may be a proxy for infectious burden in patients with bloodstream infections. We aimed to study the association between TTP and infective endocarditis (IE), or death, in patients with Staphylococcus aureus bacteraemia. Methods - VIRSTA is a multicenter prospective cohort study that included all adult patients with S. aureus bacteraemia in eight university hospitals in France (2009-2011). We analyzed data from four centers which collected data on TTP. Regression models were used to study the association between TTP and definite IE (Duke-Li criteria), and 30 day-mortality. Results - We included 587 patients with S. aureus bacteraemia: mean age was 65.3±16.3 years, 420/587 patients (71.6%) were male, 121/587 (20.6%) died, and 42/587 (7.2%) had definite IE. Median TTP of first positive blood culture was 13.7 h (interquartile range, 9.9-18). On multivariate analysis, 30-day mortality was associated with TTP≤13.7 h (74/295 (25.1%) vs 47/292 (16.1%), P=0.02), as well as old age, McCabe score, methicillin resistance, stroke, pneumonia, and C-Reactive Protein. TTP was also independently associated with IE, but with a U-shape curve: IE was more common in the first (TTP18 h, 8/146, 5.5%) quartiles of TTP, P=0.002. Conclusions - TTP provides reliable information in patients with S. aureus bacteraemia, on the risk of IE, and prognosis, with short TTP being an independent predictor of death. This data readily available at no cost may be used to identify patients who require specific attention.

Abstract

Helioseismology is the study of the solar interior using observations of oscillations at the surface. It suffers from systematic errors, such as a center-to-limb error in travel-time measurements. Understanding these errors requires a good understanding of the nontrivial relationship between wave displacement and helioseismic observables. The wave displacement causes perturbations in the atmospheric thermodynamical quantities which perturb the opacity, the optical depth, the source function, and the local ray geometry, thus affecting the emergent intensity. We aim to establish the most complete relationship up to now between the displacement and the intensity perturbation by solving the radiative transfer problem in the atmosphere. We derive an expression for the intensity perturbation caused by acoustic oscillations at any point on the solar disk by applying the first-order perturbation theory. As input, we consider adiabatic modes of oscillation of different degrees. The background and the perturbed intensities are computed considering the main sources of opacity in the continuum. We find that, for all modes, the perturbations to the thermodynamical quantities are not sufficient to model the intensity. In addition, the geometrical effects due to the displacement must be taken into account as they lead to a difference in amplitude and a phase shift between the temperature at the surface and intensity perturbations. The closer to the limb, the larger the differences. This work presents improvements for the computation of the intensity perturbations, in particular for high-degree modes, and explains differences in intensity computations in earlier works. The phase shifts and amplitude differences between the temperature and intensity perturbations increase towards the limb. Abstract

International audience; In this work, we provide an algorithm to compute efficiently and accurately the full outgoing modal Green's kernel for the scalar wave equation in local helioseismology under spherical symmetry. Due to the high computational cost of a full Green's function, current helioseismic studies rely on single-source computations. However, a more realistic modelization of the helioseismic products (cross-covariance and power spectrum) requires the full Green's kernel. In the classical approach, the Dirac source is discretized and one simulation gives the Green's function on a line. Here, we propose a two-step algorithm which, with two simulations, provides the full kernel on the domain. Moreover, our method is more accurate, as the singularity of the solution due to the Dirac source is described exactly. In addition, it is coupled with the exact Dirichlet-to-Neumann boundary condition, providing optimal accuracy in approximating the outgoing Green's kernel, which we demonstrate in our experiments. In addition, we show that high-frequency approximations of the nonlocal radiation boundary conditions can represent accurately the helioseismic products.

26. Categorization of temocillin susceptibility using the disc diffusion method: Susceptible, Intermediate, Resistant or Undetermined?

Abstract

Background: Since the pandemic of ESBL (extended-spectrum beta-lactamases) and the emergence of carbapenemase-producing Enterobacterales there is a renewed interest in temocillin. However, as the molecule was little used, except in Belgium and UK, there are few guidelines for its antibiotic susceptibility testing (AST). We aim to assess the accuracy of the disc diffusion method (DDM) for temocillin susceptibility testing. Methods: Eight hundred eighty-eight Enterobacterales clinical strains of which 61.7% were resistant to 3 rd generation cephalosporin (3GC-R) were included. AST was performed using DDM in comparison to gradient strip tests (GST) and interpreted using the diameter breakpoint of 20mm and MIC breakpoint of 8 mg/L for DDM and GST as recommended by EUCAST-CASFM recommandations. Results: At breakpoint of 8mg/L, temocillin rates of susceptibility were 76.9%, 95.6% and 67.7% for overall, 3GC-S and 3GC-R strains respectively. Ninety-four (10.6%) discrepancies were noticed including 68 (72.3%) major errors and 26 (27.6%) very major errors. Sixty-eight (72.3%) of all errors correspond to inhibition diameters comprised between 17 and 23 mm. The presence of colonies within the inhibition diameter was noticed for 32 (5.3%) strains susceptible to temocillin. Conclusions: DDM lacks accuracy for strains displaying borderline inhibition zone diameter. We suggest applying the concept of area of technical uncertainty (ATU) for standard DDM. The strains displaying a diameter within the ATU should be tested by another method. The significance of colonies within the inhibition zone diameter should be explored.

The flow of plasma inside the SunThe activity of the Sun, including the occurrence of sunspots, is driven by magnetic fields that originate from the motion of charged plasma beneath the surface. Helioseismology uses acoustic oscillations to probe the Sun's interior, analogous to seismology's use of earthquakes to investigate Earth's interior. Gizonet al.analyzed helioseismology data from 1996 to 2019, covering two 11-year solar cycles. They measured the latitudinal and radial flow of plasma as a function of depth within the Sun and how it varies with time. Abstract

Context. The frequencies, lifetimes, and eigenfunctions of solar acoustic waves are affected by turbulent convection, which is random in space and in time. Since the correlation time of solar granulation and the periods of acoustic waves ($\sim$5 min) are similar, the medium in which the waves propagate cannot a priori be assumed to be time independent. Aims. We compare various effective-medium solutions with numerical solutions in order to identify the approximations that can be used in helioseismology. For the sake of simplicity, the medium is one dimensional. Methods. We consider the Keller approximation, the second-order Born approximation, and spatial homogenization to obtain theoretical values for the effective wave speed and attenuation (averaged over the realizations of the medium). Numerically, we computed the first and second statistical moments of the wave field over many thousands of realizations of the medium (finite-amplitude sound-speed perturbations are limited to a 30 Mm band and have a zero mean). Results. The effective wave speed is reduced for both the theories and the simulations. The attenuation of the coherent wave field and the wave speed are best described by the Keller theory. The numerical simulations reveal the presence of coda waves, trailing the coherent wave packet. These late arrival waves are due to multiple scattering and are easily seen in the second moment of the wave field. Conclusions. We find that the effective wave speed can be calculated, numerically and theoretically, using a single snapshot of the random medium (frozen medium); however, the attenuation is underestimated in the frozen medium compared to the time-dependent medium. Abstract

Little is known about the functional symptoms associated with Lyme borreliosis (LB) in Europe. We aimed to assess functional symptoms associated with presumed LB and to compare patients with and without confirmed LB.We performed a retrospective monocenter study. Patients consulting for presumed LB were included.Between November 2015 and June 2018, 355patients were included (mean age: 51years, 52% of women) of which 48had LB: erythema migrans (42%), early disseminated LB (50%; 35% of neuroborreliosis cases), and late disseminated LB (8%). The most frequently reported functional symptoms were neuropathic pain (23%), arthralgia (23%), and asthenia (17%). Other functional symptoms were rare (≤10%). Three hundred and seven (86%) patients did not have LB. Patients with confirmed LB reported fewer functional symptoms than patients without LB (1.8 (±1.7) vs. 3.6 (±2.5), P0.001) with a shorter duration of symptoms (3 months in 48% vs. 16% of cases, P0.001). They less often reported asthenia (17% vs. 59%, P0.001), widespread pain (10% vs. 31%, P=0.003), myalgia (10% vs. 32%, P=0.002), memory disorders (4% vs. 16%, P=0.03), irritability (2% vs. 23%, P±0.001), and sadness (0% vs. 16%, P=0.003).In patients consulting for presumed LB, patients diagnosed with LB had fewer and shorter functional symptoms than patients without LB.

Four sequence type 664 (ST664) (serotype O:5) strains of Pseudomonas aeruginosa that were highly resistant to antibiotics, including ceftolozane-tazobactam and ceftazidime-avibactam, but were susceptible to colistin were found to harbor the gene encoding the rare class C β-lactamase PAC-1 on a chromosomally located Tn1721-like transposon. The bla(PAC-1) gene was associated with the 16S rRNA methylase determinant rmtF2, which confers pan-aminoglycoside resistance. These genotypically related strains were isolated in repatriated patients from Mauritius and Afghanistan and were close to a lineage reported in Nepal, Pakistan, and India.

Abstract

The immunochromatographic assay NG-Test Carba 5 (NG-Biotech) was evaluated with a collection of 107 carbapenemase-producing nonfermenters (CP-NF) (55 Pseudomonas spp., 51 Acinetobacter spp., and 1 Achromobacter xylosoxidans isolate) and 61 carbapenemase-negative isolates. All KPC, VIM, and NDM carbapenemase producers tested were accurately detected. Of the 16 IMP variants tested, 6 (37.5%) variants were not detected.

Retrograde-propagating waves of vertical vorticity with longitudinal wavenumbers between 3 and 15 have been observed on the Sun with a dispersion relation close to that of classical sectoral Rossby waves. The observed vorticity eigenfunctions are symmetric in latitude, peak at the equator, switch sign near $20^\circ$-$30^\circ$, and decrease at higher latitudes. We search for an explanation that takes into account solar latitudinal differential rotation. Abstract

Context. Large-scale equatorial Rossby modes have been observed on the Sun over the last two solar cycles. Aims. We investigate the impact of the time-varying zonal flows on the frequencies of Rossby modes. Methods. A first-order perturbation theory approach is used to obtain an expression for the expected shift in the mode frequencies due to perturbations in the internal rotation rate. Results. Using the time-varying rotation from helioseismic inversions we predict the changes in Rossby mode frequencies with azimuthal orders from m = 1 to m = 15 over the last two solar cycles. The peak-to-peak frequency change is less than 1 nHz for the m = 1 mode, grows with m, and reaches 25 nHz for m = 15. Conclusions. Given the observational uncertainties on mode frequencies due to the finite mode lifetimes, we find that the predicted frequency shifts are near the limit of detectability.

Abstract

Context. The interpretation of helioseismic measurements, such as wave travel-time, is based on the computation of kernels that give the sensitivity of the measurements to localized changes in the solar interior. These kernels are computed using the ray or the Born approximation. The Born approximation is preferable as it takes finite-wavelength effects into account, although it can be computationally expensive. Aims. We propose a fast algorithm to compute travel-time sensitivity kernels under the assumption that the background solar medium is spherically symmetric. Methods. Kernels are typically expressed as products of Green’s functions that depend upon depth, latitude, and longitude. Here, we compute the spherical harmonic decomposition of the kernels and show that the integrals in latitude and longitude can be performed analytically. In particular, the integrals of the product of three associated Legendre polynomials can be computed. Results. The computations are fast and accurate and only require the knowledge of the Green’s function where the source is at the pole. The computation time is reduced by two orders of magnitude compared to other recent computational frameworks. Conclusions. This new method allows flexible and computationally efficient calculations of a large number of kernels, required in addressing key helioseismic problems. For example, the computation of all the kernels required for meridional flow inversion takes less than two hours on 100 cores.

Abstract

The purpose of deep-focusing time--distance helioseismology is to construct seismic measurements that have a high sensitivity to the physical conditions at a desired target point in the solar interior. With this technique, pairs of points on the solar surface are chosen such that acoustic ray paths intersect at this target (focus) point. Considering acoustic waves in a homogeneous medium, we compare travel-time and amplitude measurements extracted from the deep-focusing cross-covariance functions. Using a single-scattering approximation, we find that the spatial sensitivity of deep-focusing travel times to sound-speed perturbations is zero at the target location and maximum in a surrounding shell. This is unlike the deep-focusing amplitude measurements, which have maximum sensitivity at the target point. We compare the signal-to-noise ratio for travel-time and amplitude measurements for different types of sound-speed perturbations, under the assumption that noise is solely due to the random excitation of the waves. We find that, for highly localized perturbations in sound speed, the signal-to-noise ratio is higher for amplitude measurements than for travel-time measurements. Abstract

In this study, the activity of 18 non-carbapenem antibiotics was evaluated against 100 extended-spectrum β-lactamase (ESBL)-producing Escherichia coli (ESBL-Ec) and 50 ESBL-producing Klebsiella pneumoniae (ESBL-Kp) isolated from urinary tract infections and bacteraemia in 2016. Minimum inhibitory concentrations (MICs) were determined using reference methods and the susceptibility profiles were defined according to European Committee on Antimicrobial Susceptibility Testing (EUCAST) 2017 recommendations. All of the ESBL-Ec isolates were susceptible to ceftazidime/avibactam and a great majority of them were susceptible to fosfomycin (98%), piperacillin/tazobactam (97%), amikacin (97%) and nitrofurantoin (96%). Mecillinam, cefoxitin and ceftolozane/tazobactam remained active against 92%, 83% and 78% of the ESBL-Ec isolates, respectively. Moreover, 100%, 94% and 90% of the ESBL-Kp tested were susceptible to ceftazidime/avibactam, amikacin and mecillinam, respectively. This study showed that there are non-carbapenem options (including orally administrable drugs) for the treatment of all of the situations of ESBL-Ec or ESBL-Kp infections, with ceftazidime/avibactam being the most efficient alternative.

International audience; A new MALDI-TOF MS imipenem hydrolysis assay was developed to optimize detection of carbapenemase-producing strains of Pseudomonas aeruginosa. The method correctly discriminated carbapenemase producers (n=74) from non-producers (n=95) in 99.4% of cases, and successfully detected 100% GES-5 strains (n=5).

Abstract

Helioseismic holography is an imaging technique used to study heterogeneities and flows in the solar interior from observations of solar oscillations at the surface. Holograms contain noise due to the stochastic nature of solar oscillations. We provide a theoretical framework for modeling signal and noise in Porter-Bojarski helioseismic holography. The wave equation may be recast into a Helmholtz-like equation, so as to connect with the acoustics literature and define the holography Green's function in a meaningful way. Sources of wave excitation are assumed to be stationary, horizontally homogeneous, and spatially uncorrelated. Using the first Born approximation we calculate holograms in the presence of perturbations in sound-speed, density, flows, and source covariance, as well as the noise level as a function of position. This work is a direct extension of the methods used in time-distance helioseismology to model signal and noise. To illustrate the theory, we compute the hologram intensity numerically for a buried sound-speed perturbation at different depths in the solar interior. The reference Green's function is obtained for a spherically-symmetric solar model using a finite-element solver in the frequency domain. Below the pupil area on the surface, we find that the spatial resolution of the hologram intensity is very close to half the local wavelength. For a sound-speed perturbation of size comparable to the local spatial resolution, the signal-to-noise ratio is approximately constant with depth. Averaging the hologram intensity over a number $N$ of frequencies above 3 mHz increases the signal-to-noise ratio by a factor nearly equal to the square root of $N$. Abstract

Local helioseismology has so far relied on semi-analytical methods to compute the spatial sensitivity of wave travel times to perturbations in the solar interior. These methods are cumbersome and lack flexibility. Here we propose a convenient framework for numerically solving the forward problem of time-distance helioseismology in the frequency domain. The fundamental quantity to be computed is the cross-covariance of the seismic wavefield. We choose sources of wave excitation that enable us to relate the cross-covariance of the oscillations to the Green's function in a straightforward manner. We illustrate the method by considering the 3D acoustic wave equation in an axisymmetric reference solar model, ignoring the effects of gravity on the waves. The symmetry of the background model around the rotation axis implies that the Green's function can be written as a sum of longitudinal Fourier modes, leading to a set of independent 2D problems. We use a high-order finite-element method to solve the 2D wave equation in frequency space. The computation is `embarrassingly parallel', with each frequency and each azimuthal order solved independently on a computer cluster. We compute travel-time sensitivity kernels in spherical geometry for flows, sound speed, and density perturbations under the first Born approximation. Convergence tests show that travel times can be computed with a numerical precision better than one millisecond, as required by the most precise travel-time measurements. The method presented here is computationally efficient and will be used to interpret travel-time measurements in order to infer, e.g., the large-scale meridional flow in the solar convection zone. Abstract

International audience; The temporal covariance between seismic waves measured at two locations on the solar surface is the fundamental observable in time-distance helioseismology. Above the acoustic cutoff frequency (∼5.3 mHz), waves are not trapped in the solar interior and the covariance function can be used to probe the upper atmosphere. We wish to implement appropriate radiative boundary conditions for computing the propagation of high-frequency waves in the solar atmosphere. We consider the radiative boundary conditions recently developed by Barucq et al. (2017) for atmospheres in which sound-speed is constant and density decreases exponentially with radius. We compute the cross-covariance function using a finite element method in spherical geometry and in the frequency domain. The ratio between first-and second-skip amplitudes in the time-distance diagram is used as a diagnostic to compare boundary conditions and to compare with observations. We find that a boundary condition applied 500 km above the photosphere and derived under the approximation of small angles of incidence accurately reproduces the 'infinite atmosphere' solution for high-frequency waves. When the radiative boundary condition is applied 2 Mm above the photosphere, we find that the choice of atmospheric model affects the time-distance diagram. In particular, the time-distance diagram exhibits double-ridge structure when using a VAL atmospheric model.

Abstract

Context. In time-distance helioseismology, wave travel times are measured from the two-point cross-covariance function of solar oscillations and are used to image the solar convection zone in three dimensions. There is, however, also information in the amplitude of the cross-covariance function, for example about seismic wave attenuation. Aims. Here we develop a convenient procedure to measure the amplitude of the cross-covariance function of solar oscillations. Methods. In this procedure, the amplitude of the cross-covariance function is linearly related to the cross-covariance function and can be measured even for high levels of noise. Results. As an example application, we measure the amplitude perturbations of the seismic waves that propagate through the sunspot in active region NOAA 9787. We can recover the amplitude variations due to the scattering and attenuation of the waves by the sunspot and associated finite-wavelength effects. Conclusions. The proposed definition of cross-covariance amplitude is robust to noise, can be used to relate measured amplitudes to 3D perturbations in the solar interior under the Born approximation, and will provide independent information from the travel times., accepted for publication in A&A

Abstract

International audience; This paper presents a hp−refinement method for a first order scalar transport- reaction equation discretized by a discontinuous Galerkin method. First, the theoretical rates of convergence of h− and p−refinement are recalled and numerically tested. Then, in order to design some meshes, we propose two different estimators of the local error on the spatial domain. These quantities are analysed and compared depending on the regularity of the solution so as to find the best way to lead the refinement process and the best strategy to choose between h− and p−refinement. Finally, the different possible refinement strategies are compared first on analytical examples and then on realistic applications for neutron transport in a nuclear reactor core.

Abstract

International audience; Evidence before this study We searched PubMed on June 30, 2016, for English-language cohort studies published since Jan 1, 1980, of patients with invasive listeriosis worldwide with the keywords " listeria " , " listeriosis " , " maternal " , and " neurolisteriosis ". Studies had to include epidemiological or clinical data on listeriosis. All clinical forms of infection were included (bacteraemia, neurolisteriosis, and maternal–neonatal infection). Host risk factors for listeriosis have been well identified, but the clinical features and prognostic factors of the disease are based on retrospective studies compiling heterogeneous data or random collected cases. Furthermore, no clinical trial has ever been done and medical management is not evidence based. Added value of the study Our study is the first prospective clinical study focusing on all forms of invasive listeriosis. The study is based on a national mandatory system that allowed the nearly complete capture of microbiologically proven cases. The study shows a higher burden of listeriosis than reported before: more than 80% of infected mothers experienced major fetal or neonatal complications (fetal loss, very high prematurity, early or late onset disease); only 39% of patients with neurolisteriosis survived and fully recovered. Abstract

Within the framework of a Discontinuous Galerkin spatial approximation of the multigroup discrete ordinates transport equation, we present a generalization of the exact standard perturbation formula that takes into account spatial discretization-induced reactivity changes. It encompasses in two separate contributions the nuclear data-induced reactivity change and the reactivity modification induced by two different spatial discretizations. The two potential uses of such a formulation when considering adaptive mesh refinement are discussed, and numerical results on a simple two-group Cartesian two-dimensional benchmark are provided. In particular, such a formulation is shown to be useful to filter out a more accurate estimate of nuclear data-related reactivity effects from initial and perturbed calculations based on independent adaptation processes.

Abstract

We present an error estimator for the S N neutron transport equation discretized with an arbitrary high-order discontinuous Galerkin method. As a starting point, the estimator is obtained for conforming Cartesian meshes with a uniform polynomial order for the trial space then adapted to deal with non-conforming meshes and a variable polynomial order. Some numerical tests illustrate the properties of the estimator and its limitations. Finally, a simple shielding benchmark is analyzed in order to show the relevance of the estimator in an adaptive process.

Abstract

Mutation-dependent overproduction of intrinsic β-lactamase AmpC is considered the main cause of resistance of clinical strains of Pseudomonas aeruginosa to antipseudomonal penicillins and cephalosporins. Analysis of 31 AmpC-overproducing clinical isolates exhibiting a greater resistance to ceftazidime than to piperacillin-tazobactam revealed the presence of 17 mutations in the β-lactamase, combined with various polymorphic amino acid substitutions. When overexpressed in AmpC-deficient P. aeruginosa 4098, the genes coding for 20/23 of these AmpC variants were found to confer a higher (2-fold to >64-fold) resistance to ceftazidime and ceftolozane-tazobactam than did the gene from reference strain PAO1. The mutations had variable effects on the MICs of ticarcillin, piperacillin-tazobactam, aztreonam, and cefepime. Depending on their location in the AmpC structure and their impact on β-lactam MICs, they could be assigned to 4 distinct groups. Most of the mutations affecting the omega loop, the R2 domain, and the C-terminal end of the protein were shared with extended-spectrum AmpCs (ESACs) from other Gram-negative species. Interestingly, two new mutations (F121L and P154L) were predicted to enlarge the substrate binding pocket by disrupting the stacking between residues F121 and P154. We also found that the reported ESACs emerged locally in a variety of clones, some of which are epidemic and did not require hypermutability. Taken together, our results show that P. aeruginosa is able to adapt to efficacious β-lactams, including the newer cephalosporin ceftolozane, through a variety of mutations affecting its intrinsic β-lactamase, AmpC. Data suggest that the rates of ESAC-producing mutants are ≥1.5% in the clinical setting.
