This study investigated the determination of detailed microstructure modeling of the trochlea of the superior oblique muscle (SOM) using micro-computed tomography (micro-CT) and modeling of a potential prototype for a trochlea implant using three-dimensional (3D) printing. We dissected 15 intact orbits of 15 embalmed cadavers. The trochleae of the SOM were detached from the periosteum. The specimens were stained by immersion in a 15% Lugol's solution. Images were reconstructed using conventional scanner software. Measurement points were determined for the middle cross section. Points P1 and P2 were selected where the SOM adjoined the curvature of the inner trochlea. They defined the inner contact points of the SOM in the inner part of the trochlea curvature. On the back of the trochlea, points P3 and P4 were selected at the uppermost and lowest points in the inner parts of the straight trochlea, respectively. Origin O was defined on the arcuate line of P 1 P 2 ^ to generate the smallest-diameter circle consisting of P1, O, and P2. We then measured the angle from O P 1 ¯ t o O P 2 ¯ , and from O P 3 ¯ t o O P 4 ¯. We also measured the distances O P 1 ¯ , O P 2 ¯ , O P 3 ¯ , a n d O P 4 ¯ for the design of a potential trochlea implant prototype using 3D-printing and micro-CT-based modeling. The distances O P 1 ¯ , O P 2 ¯ , O P 3 ¯ , a n d O P 4 ¯ were 2.2 ± 0.7, 1.4 ± 0.5, 2.7 ± 0.9, and 2.5 ± 0.4 mm (mean ± SD), respectively. The angles from O P 1 ¯ t o O P 2 ¯ , from O P 2 ¯ t o O P 4 , - and from O P 3 ¯ t o O P 4 ¯ were 100.7 ± 14.4, 66.3 ± 18.0, and 98.9 ± 24.9 degrees, respectively. The present investigation demonstrates that the high-resolution CT is a powerful imaging technique for defining the true 3D geometry of a specimen and can potentially be used to create a 3D-printed trochlea implant. Purpose: Extrinsic trauma to the orbit may cause a blowout or orbital fracture, which often requires surgery for reconstruction of the orbit and repositioning of the eyeball with an implant. Post-operative complications, however, are high with the most frequent cause of complications being a mismatch of the position and shape of the implant and fracture. These mismatches may be reduced by computed tomography (CT) based modeling and three-dimensional (3D) printed guide. Therefore, the aim of this study is to propose and evaluate a patient-specific guide to shape an orbital implant using 3D printing. Design/methodology/approach: Using CT images of a patient, an orbital fracture can be modeled to design an implant guide for positioning and shaping of the surface and boundaries of the implant. The guide was manufactured using UV curable plastic at 0.032 mm resolution by a 3D printer. The accuracy of this method was evaluated by micro-CT scanning of the surgical guides and shaping implants. Findings: The length and depth of the 3D model, press-compressed and decompressed implants were compared. The mean differences in length were 0.67 ± 0.38 mm, 0.63 ± 0.28 mm and 0.10 ± 0.10 mm, and the mean differences in depth were 0.64 ± 0.37 mm, 1.22 ± 0.56 mm and 0.57 ± 0.23 mm, respectively. Statistical evaluation was performed with a Bland-Altman plot. Purpose: This study aims to systemically evaluate morphological printing errors between computer-aided design (CAD) and reference models fabricated using two different three-dimensional printing (3DP) technologies with hard and soft materials. Design/methodology/approach: The reference models were designed to ensure simpler and more accurate measurements than those obtained from actual kidney simulators. Three reference models, i.e. cube, dumbbell and simplified kidney, were manufactured using photopolymer jetting (PolyJet) with soft and hard materials and multi-jet printing (MJP) with hard materials. Each reference model was repeatably measured five times using digital calipers for each length. These values were compared with those obtained using CAD. Findings: The results demonstrate that the cube models with the hard material of MJP and hard and soft materials of PolyJet were smaller (p = 0.022, 0.015 and 0.057, respectively). The dumbbell model with the hard material of MJP was smaller (p = 0.029) and that with the soft material of PolyJet was larger (p = 0.020). However, the dumbbell with the hard material of PolyJet generated low errors (p = 0.065). Finally, the simplified kidney models with the hard material of MJP and soft materials of PolyJet were smaller (p = 0.093 and 0.021) and that with the hard material of PolyJet was opposite to the former models (p = 0.043). Originality/value: This study, to the best of authors' knowledge, is the first to determine the accuracy between CAD and reference models fabricated using two different 3DP technologies with multi-materials. Angiography and Doppler methods used for diagnosing vascular diseases give information on the shape of blood vessels and pointwise blood speed but do not provide detailed information on the flow fields inside the blood vessels. In this study, we developed a method for visualizing blood flow by using coherent synchrotron x rays. This method, which does not require the addition of any contrast agent or tracer particles, visualizes the flow pattern of blood by enhancing the diffraction and interference characteristics of the blood cells. This was achieved by optimizing the sample- (blood) to-detector (charge-coupled device camera) distance and the sample thickness. The proposed method was used to extract quantitative velocity field information from blood flowing inside an opaque microchannel by applying a two-frame particle image velocimetry algorithm to enhanced x-ray images of the blood flow. An x-ray particle image velocimetry (PIV) technique was developed to measure quantitative information on flows inside opaque objects. To acquire x-ray images suitable for PIV velocity field measurements, refraction-based edge enhancement was employed using detectable tracer particles with the object and detector separated by an experimentally determined optimal distance. The x-ray PIV method was applied to a flow in an opaque Teflon tube. Moreover, there is often substantial remission of the original tumor following NAC, and the original tumor may scatter as multifocal or occult lesions, making accurate tumor localization difficult in current clinical practice. Purpose To investigate the details of the flow structure of a plug-like jet that had a vortex ring in pulsatile stenotic phantoms using 4D flow MRI. Method Pulsatile Newtonian flows in two stenotic phantoms with 50% and 75% reductions in area were scanned by 4D flow MRI. Blood analog working fluid was circulated via the stenotic phantom using a pulsatile pump at a constant pulsating frequency of 1 Hz. The velocity and vorticity fields of the plug-like jet with a vortex ring were quantitatively analyzed in the spatial and temporal domains. Results Pulsatile stenotic flow showed a plug-like jet at the specific stenotic degree of 50% in our pulsatile waveform design. This plug-like jet was found at the decelerating period in the post-stenotic region of 26.4 mm (1.2 D). It revealed a vortex ring structure with vorticity strength in the range of ± 100 s − 1 . Conclusion We observed a plug-like jet with a vortex ring in pulsatile stenotic flow by in vitro visualization using 4D flow MRI. In this plug-like jet, the local fastest flow region occurred at the post-systole phase in the post-stenotic region, which was distinguishable from a typical stenotic jet flow at systole phase. Background: Although the measurement of turbulence kinetic energy (TKE) by using magnetic resonance imaging (MRI) has been introduced as an alternative index for quantifying energy loss through the cardiac valve, experimental verification and clinical application of this parameter are still required. Objectives: The goal of this study is to verify MRI measurements of TKE by using a phantom stenosis with particle image velocimetry (PIV) as the reference standard. In addition, the feasibility of measuring TKE with MRI is explored. Methods: MRI measurements of TKE through a phantom stenosis was performed by using clinical 3T MRI scanner. The MRI measurements were verified experimentally by using PIV as the reference standard. In vivo application of MRI-driven TKE was explored in seven patients with aortic valve disease and one healthy volunteer. Transvalvular gradients measured by MRI and echocardiography were compared. Results: MRI and PIV measurements of TKE are consistent for turbulent flow (0.666 < R2 < 0.738) with a mean difference of −11.13 J/m3 (SD = 4.34 J/m3). Results of MRI and PIV measurements differ by 2.76 ± 0.82 cm/s (velocity) and −11.13 ± 4.34 J/m3 (TKE) for turbulent flow (Re > 400). The turbulence pressure drop correlates strongly with total TKE (R2 = 0.986). However, in vivo measurements of TKE are not consistent with the transvalvular pressure gradient estimated by echocardiography. Conclusions: These results suggest that TKE measurement via MRI may provide a potential benefit as an energy-loss index to characterize blood flow through the aortic valve. The mammalian brain is far superior to today's electronic circuits in intelligence and efficiency. Its functions are realized by the network of neurons connected via synapses. Much effort has been extended in finding satisfactory electronic neural networks that act like brains, i.e., especially the electronic version of synapse that is capable of the weight control and is independent of the external data storage. We demonstrate experimentally that a single metal-oxide-metal structure successfully stores the biological synaptic weight variations (synaptic plasticity) without any external storage node or circuit. Our device also demonstrates the reliability of plasticity experimentally with the model considering the time dependence of spikes. All these properties are embodied by the change of resistance level corresponding to the history of injected voltage-pulse signals. Moreover, we prove the capability of second-order learning of the multi-resistive device by applying it to the circuit composed of transistors. We anticipate our demonstration will invigorate the study of electronic neural networks using non-volatile multi-resistive device, which is simpler and superior compared to other storage devices. Abstract: Hemodynamics has been a very important factor in understanding and diagnosing various vascular diseases. Recently, the X-ray particle image velocimetry (X-ray PIV) method using speckle patterns of blood has been introduced as a new quantitative visualization method for blood flows without any seeding tracer or contrast agents. In this study, the peculiar optical characteristics of blood on the synchrotron X-ray imaging method, which were not presented in previous studies, were investigated in depth and systematically. The experimental conditions required for X-ray PIV application were found to be the distance between the sample and the scintillator (∼40cm), the thickening of the blood sample (>0.3mm), and hematocrit (20.0–80.0%). In addition, we verified that the X-ray PIV method is reliable as an advanced flow velocimetry by comparing the flow rate evaluated from the X-ray PIV result and the input flow rate supplied from a syringe pump with an error of less than 1%. Through this study, based on the understanding of contrast enhancement mechanisms of speckle patterns from blood, we could establish a trustworthy flow visualization method that can be used effectively in hemodynamic studies. A compact x-ray particle image velocimetry (PIV) system employing a medical x-ray tube as a light source was developed to measure quantitative velocity field information of opaque flows. The x-ray PIV system consists of a medical x-ray tube, an x-ray charge coupled device camera, a programmable shutter for a pulse-type x ray, and a synchronization device. Through performance tests, the feasibility of the developed x-ray PIV system as a flow measuring device was verified. To check the feasibility of the developed system, we tested a tube flow at two different mean velocities of 1 and 2 mm/s. The x-ray absorption of tracer particles must be quite different from that of working fluid to have a good contrast in x-ray images. Quantifying turbulence velocity fluctuation is important because it indicates the fluid energy dissipation of the blood flow, which is closely related to the pressure drop along the blood vessel. This study aims to evaluate the effects of scan parameters and the target vessel size of 4D phase-contrast (PC)-MRI on quantification of turbulent kinetic energy (TKE). Comprehensive 4D PC-MRI measurements with various velocity-encoding (VENC), echo time (TE), and voxel size values were carried out to estimate TKE distribution in stenotic flow. The total TKE (TKE sum ), maximum TKE (TKE max ), and background noise level (TKE noise ) were compared for each scan parameter. The feasibility of TKE estimation in small vessels was also investigated. Results show that the optimum VENC for stenotic flow with a peak velocity of 125 cm/s was 70 cm/s. Higher VENC values overestimated the TKE sum by up to six-fold due to increased TKE noise , whereas lower VENC values (30 cm/s) underestimated it by 57.1%. TE and voxel size did not significantly influence the TKE sum and TKE noise , although the TKE max significantly increased as the voxel size increased. TKE quantification in small-sized vessels (3–5-mm diameter) was feasible unless high-velocity turbulence caused severe phase dispersion in the reference image. [ABSTRACT FROM AUTHOR]

In thoracoabdominal aortic aneurysm repair, repairing the visceral and segmental arteries is challenging. Although there is a pre-hand-sewn and multi-branched graft based on the conventional image-based technique, it has shortcomings in precisely positioning and directing the visceral and segmental arteries. Here, we introduce two new reconstruction techniques using patient-specific 3D-printed graft reconstruction guides: (1) model-based technique that presents the projected aortic graft, visualizing the main aortic body and its major branches and (2) guide-based technique in which the branching vessels in the visualization model are replaced by marking points identifiable by tactile sense. We demonstrate the effectiveness by evaluating conventional and new techniques based on accuracy, marking time requirement, reproducibility, and results of survey to surgeons on the perceived efficiency and efficacy. The graft reconstruction guides cover the segmentation, design, fabrication, post-processing, and clinical application of open surgical repair of thoracoabdominal aneurysm, and proved to be efficient for accurately reconstructing customized grafts. Purpose: To evaluate the osseous anatomy of the proximal femur extracted from a 3D-MRI volumetric interpolated breath-hold (VIBE) sequence using either a Dixon or water excitation (WE) fat suppression method, and to measure the overall difference using CT as a reference standard. Material and methods: This retrospective study reviewed imaging of adult patients with hip pain who underwent 3D hip MRI and CT. A semi-automatically segmented CT model served as the reference standard, and MRI segmentation was performed manually for each unilateral hip joint. The differences between Dixon-VIBE-3D-MRI vs. CT, and WE-VIBE-3D-MRI vs. CT, were measured. Equivalence tests between Dixon-VIBE and WE-VIBE models were performed with a threshold of 0.1 mm. Bland–Altman plots and Lin's concordance-correlation coefficient were used to analyze the agreement between WE and Dixon sequences. Subgroup analyses were performed for the femoral head/neck, intertrochanteric, and femoral shaft areas. Results: The mean and maximum differences between Dixon-VIBE-3D-MRI vs. CT were 0.2917 and 3.4908 mm, respectively, whereas for WE-VIBE-3D-MRI vs. CT they were 0.3162 and 3.1599 mm. The mean differences of the WE and Dixon methods were equivalent (P = 0.0292). However, the maximum difference was not equivalent between the two methods and it was higher in WE method. Lin's concordance-correlation coefficient showed poor agreement between Dixon and WE methods. The mean differences between the CT and 3D-MRI models were significantly higher in the femoral shaft area (P = 0.0004 for WE and P = 0.0015 for Dixon) than in the other areas. The maximum difference was greatest in the intertrochanteric area for both techniques. Conclusion: The difference between 3D-MR and CT models were acceptable with a maximal difference below 3.5mm. Background: ERCP training models are very different in terms of anatomical differences, ethical issues, storage problems, realistic tactile sensation, durability and portability. There is no easy way to select an optimized model for ERCP training. If the ERCP training model could be made as a soft silicone model using 3D printing technique, it would have numerous advantages over the models presented so far. The purpose of this study was to develop an optimized ERCP training model using a 3D printing technique and to try to find ways for implementing various practical techniques.Methods: All organ parts of this model were fabricated using silicone molding techniques with 3D printing. Especially, various anatomy of the ampulla of Vater and common bile duct (CBD) were creatively designed for different diagnostic and therapeutic procedures. In order to manufacture each of the designed organ parts with silicone, a negative part had to be newly designed to produce the molder. The negative molders were 3D printed and then injection molding was applied to obtain organ parts in silicone material. The eight different types of ampulla and CBD were repeatedly utilized and replaced to the main system as a module-type.Results: ERCP training silicone model using 3D technique was semi-permanently used to repeat various ERCP procedures. All ERCP procedures using this model could be observed by real-time fluoroscopic examination as well as endoscopic examination simultaneously. Using different ampulla and CBD modules, basic biliary cannulation, difficult cannulation, stone extraction, mechanical lithotripsy, metal stent insertion, plastic stent insertion, and balloon dilation were successfully and repeatedly achieved. Endoscopic sphincterotomy was also performed on a specialized ampulla using a Vienna sausage. After repeat procedures and trainings, all parts of organs including the ampulla and CBD modules were not markedly damaged or deformed.Conclusions: We made a specialized ERCP training silicon model with 3D printing technique. This model is durable, relatively cheap and easy to make, and thus allows the users to perform various specialized ERCP techniques, which increases its chances of being a good ERCP training model. We quantitatively evaluated breast tumor movement and volume changes between magnetic resonance imaging (MRI) scans in prone and supine positions. Twenty-seven breast tumor patients who received neoadjuvant systemic therapy (NST) for breast-conserving surgery were studied. Before and after NST, MRI scans in prone and supine positions were performed immediately. Tumor segmentation, volume, and position of tumors were evaluated in both positions. Average tumor volumes in prone and supine positions did not significantly differ (p = 0.877). Tumor movement from prone to supine positions from the origin of the bottom center of the sternum was strongly correlated with the distance from the tumor center to the chest wall (r = 0.669; p < 0.05). Tumor changes from prone to supine positions measured from the origin of the nipple depended on the location of the tumor in the breast. The prone-to-supine movement of all tumors from the origin of the bottom center of the sternum tended to move outward from the sagittal centerline of the body on the coronal plane, to the inside of the body on the sagittal plane, and outward and downward close to the body on the axial plane, which might help in planning operations using prone MRI in supine-position breast cancer surgery. Segmentation is fundamental to medical image analysis. Recent advances in fully convolutional networks has enabled automatic segmentation; however, high labeling efforts and difficulty in acquiring sufficient and high-quality training data is still a challenge. In this study, a cascaded 3D U-Net with active learning to increase training efficiency with exceedingly limited data and reduce labeling efforts is proposed. Abdominal computed tomography images of 50 kidneys were used for training. In stage I, 20 kidneys with renal cell carcinoma and four substructures were used for training by manually labelling ground truths. In stage II, 20 kidneys from the previous stage and 20 newly added kidneys were used with convolutional neural net (CNN)-corrected labelling for the newly added data. Similarly, in stage III, 50 kidneys were used. The Dice similarity coefficient was increased with the completion of each stage, and shows superior performance when compared with a recent segmentation network based on 3D U-Net. The labeling time for CNN-corrected segmentation was reduced by more than half compared to that in manual segmentation. Active learning was therefore concluded to be capable of reducing labeling efforts through CNN-corrected segmentation and increase training efficiency by iterative learning with limited data. Although hemodynamic influence of the subprosthetic tissue, termed as pannus, may contribute to prosthetic aortic valve dysfunction, the relationship between pannus extent and hemodynamics in the prosthetic valve has rarely been reported. We investigated the fluid dynamics of pannus formation using in vitro experiments with particle image velocimetry. Subvalvular pannus formation caused substantial changes in prosthetic valve transvalvular peak velocity, transvalvular pressure gradient (TPG) and opening angle. Maximum flow velocity and corresponding TPG were mostly affected by pannus width. When the pannus width was 25% of the valve diameter, pannus formation elevated TPG to >2.5 times higher than that without pannus formation. Opening dysfunction was observed only for a pannus involvement angle of 360°. Although circumferential pannus with an involvement angle of 360° decreased the opening angle of the valve from approximately 82° to 58°, eccentric pannus with an involvement angle of 180° did not induce valve opening dysfunction. The pannus involvement angle largely influenced the velocity flow field at the aortic sinus and corresponding hemodynamic indices, including wall shear stress, principal shear stress and viscous energy loss distributions. Substantial discrepancy between the velocity-based TPG estimation and direct pressure measurements was observed for prosthetic valve flow with pannus formation. [ABSTRACT FROM AUTHOR]

Background Self-expandable metallic stent (SEMS) placement is a well-established palliative treatment approach for malignant gastroduodenal obstruction. In patients with a long (>10 cm) stricture, multiple stents placed in an overlapping fashion are often required. Purpose To investigate the outcomes of overlapping SEMS placement for the palliative treatment of malignant gastroduodenal obstruction in patients with a long (>10 cm) stricture. Material and Methods The medical records of 40 patients who underwent fluoroscopic overlapping SEMS placement for malignant gastroduodenal obstruction due to a long (>10 cm) stricture were reviewed. Results The technical and clinical success rates were 100% and 65.0%, respectively. The mean length of the stricture was 17.0 ± 4.7 cm and the mean number of stents placed in each patient was 2.2 ± 0.5. Metastatic cancer (odds ratio [OR], 0.315; P = 0.018), Eastern Cooperative Oncology Group (ECOG) score ≥3 (OR, 0.018; P = 0.006), and carcinomatosis with ascites (OR, 0.025; P = 0.017) were independent predictors of poor clinical success. The rates of minor and major complications were 27.5% and 2.5%, respectively. The median stent patency and survival were 33 days (interquartile range [IQR], 19-60 days) and 35 days (IQR, 19-73 days), respectively. An ECOG score ≥3 was an independent predictor of a poor survival outcome (hazard ratio, 4.681; P < 0.001). Conclusion Overlapping SEMS placement may be safe and effective for the palliative treatment of malignant gastroduodenal obstruction in patients with a long (>10 cm) stricture. Objectives: To evaluate clinical outcomes of fluoroscopic removal of retrievable self-expandable metal stents (SEMSs) for malignant oesophageal strictures, to compare clinical outcomes of three different removal techniques, and to identify predictive factors of successful removal by the standard technique (primary technical success).Methods: A total of 137 stents were removed from 128 patients with malignant oesophageal strictures. Primary overall technical success and removal-related complications were evaluated. Logistic regression models were constructed to identify predictive factors of primary technical success.Results: Primary technical success rate was 78.8 % (108/137). Complications occurred in six (4.4 %) cases. Stent location in the upper oesophagus (P=0.004), stricture length over 8 cm (P=0.030), and proximal granulation tissue (P<0.001) were negative predictive factors of primary technical success. If granulation tissue was present at the proximal end, eversion technique was more frequently required (P=0.002).Conclusions: Fluoroscopic removal of retrievable SEMSs for malignant oesophageal strictures using three different removal techniques appeared to be safe and easy. The standard technique is safe and effective in the majority of patients. Objectives: To validate 4D flow MRI in a flow phantom using a flowmeter and computational fluid dynamics (CFD) as reference.Methods: Validation of 4D flow MRI was performed using flow phantoms with 75 % and 90 % stenosis. The effect of spatial resolution on flow rate, peak velocity and flow patterns was investigated in coronal and axial scans. The accuracy of flow rate with 4D flow MRI was evaluated using a flowmeter as reference, and the peak velocity and flow patterns obtained were compared with CFD analysis results.Results: 4D flow MRI accurately measured the flow rate in proximal and distal regions of the stenosis (percent error ≤3.6 % in axial scanning with 1.6-mm resolution). The peak velocity of 4D flow MRI was underestimated by more than 22.8 %, especially from the second half of the stenosis. Sleep spindles are rhythmic patterns of neuronal activity generated within the thalamocortical circuit. Although spindles have been hypothesized to protect sleep by reducing the influence of external stimuli, it remains to be confirmed experimentally whether there is a direct relationship between sleep spindles and the stability of sleep. We have addressed this issue by using in vivo photostimulation of the thalamic reticular nucleus of mice to generate spindle oscillations that are structurally and functionally similar to spontaneous sleep spindles. Such optogenetic generation of sleep spindles increased the duration of non-rapid eye movement (NREM) sleep. Furthermore, the density of sleep spindles was correlated with the amount of NREM sleep. These findings establish a causal relationship between sleep spindles and the stability of NREM sleep, strongly supporting a role for the thalamocortical circuit in sleep regulation.
