Objective To establish an artificial intelligence model based on B-mode thyroid ultrasound images to predict central compartment lymph node metastasis(CLNM)in patients with papillary thyroid carcinoma(PTC). Methods We retrieved the clinical manifestations and ultrasound images of the tumors in 309 patients with surgical histologically confirmed PTC and treated in the First Medical Center of PLA General Hospital from January to December in 2018.The datasets were split into the training set and the test set.We established a deep learning-based computer-aided model for the diagnosis of CLNM in patients with PTC and then evaluated the diagnosis performance of this model with the test set. Result The accuracy,sensitivity,specificity,and area under receiver operating characteristic curve of our model for predicting CLNM were 80%,76%,83%,and 0.794,respectively. Conclusion Deep learning-based radiomics can be applied in predicting CLNM in patients with PTC and provide a basis for therapeutic regimen selection in clinical practice.

Computer-aided diagnosis systems (CADs) present valuable second opinions to radiologists in diagnosis. Many studies on thyroid nodules have proposed various CADs to provide a binary result, benignity or malignancy, for doctors, ignoring interpretability of more ultrasonic features that could be more useful. We develop an interpretable CADs (iCADS) that utilizes deep-learning networks’ classification power and interpretability potential of clinical guidelines, like TIRADS, a well-established scale for thyroid nodules. iCADS incorporates a main neural-networks model and six neural-network based interpreters. The outputs of the six interpreters are compared with TIRADS guidelines and the matched result will form a report, more than a benignity or malignancy result, for radiologists. Clinical images of 16,946 thyroid nodules from 5,885 patients were used to train the proposed iCADS. An extra experimental data set containing 501 images were used to test the performance of the model. For better illustrating the assistant ability of iCADS, we also recruited ten junior radiologists to make diagnosis decisions with or without the help of different versions of iCADS. The experiments demonstrated that iCADS can largely improve junior radiologists diagnosis with the help of interpreter strategy. These experiments are also the very first attempt to evaluate the effect of interpretability of deep-learning based CADs in clinical practice. Comparison experiments with other deep-learning based CADs and traditional CADs indicated that the interpreter strategy can easily be combined to other intelligent CADs without the loss of performance. The framework of iCADS can also inspire more research on the development of CADs.

Objective: To compare and analyze the clinical effects of ultrasound-guided percutaneous nephrolithotripsy and X-ray-guided percutaneous nephrolithotripsy in the treatment of complex renal calculi without hydronephrosis. Methods: Eighty patients with multiple stones without hydronephrosis were admitted at Department of Ultrasound Diagnosis, The first medical center of Chinese PLA General Hospital from January 21, 2020 to December 21, 2020 randomly divided into two groups: experimental group and control group, with 40 cases in each group. Patients in the experimental group were treated with ultrasound-guided percutaneous nephrolithotomy, while those in the control group were treated with X-ray-guided percutaneous nephrolithotomy. Venous blood was drawn before surgery and on the first day after surgery, and serum creatinine, urea nitrogen, blood β2-microglobulin, blood uric acid and other renal indexes were detected. Background Chronic lymphocytic thyroiditis (CLT) is an autoimmune disease commonly associated with papillary thyroid carcinoma characterized by a smaller primary tumor size at presentation. The efficacy and safety of ultrasound‐guided radiofrequency ablation (RFA) for papillary thyroid microcarcinoma (PTMC) coexisting with CLT is still unknown. Methods Sixty patients with unifocal PTMC were enrolled and classified into PTMC and PTMC+CLT groups (n = 30/group). CLT was diagnosed histopathologically. The ablation area exceeded the tumor margins, and was evaluated by US and contrast‐enhanced US (CEUS) for residual tumor to prevent recurrence. Three months after ablation, US‐guided core‐needle biopsy was performed to assess the presence of residual and recurrent cancer. Preoperative and postoperative data on patients and tumors were recorded and analyzed. Results There were no differences between groups in age, sex, preoperative tumor volume, ablation time, or ablation power (P > 0.05). There was also no significant difference in postoperative ablation zone volume between the groups at the 1‐, 3‐, 6‐, 12‐, and 18‐month follow‐ups (P > 0.05). The volume reduction rate significantly differed between the two groups at month 3 (P = 0.03). The ablation area could not be identified on US and CEUS at 9.8 ± 5.0 and 10.0 ± 4.8 months in the PTMC and PTMC + CLT groups, respectively (P = 0.197). No serious complications occurred during and after ablation. No residual cancer cells were found on biopsy after ablation. Conclusions RFA was effective in patients with PTMC+CLT, and its therapeutic efficacy and safety were similar to those in patients with PTMC without CLT.

Objective To investigate the short-term outcome of T1bN0M0 papillary thyroid cancer after ultrasonography-guided radiofrequency ablation(RFA). Methods Eighty-nine patients with T1bN0M0 papillary thyroid cancer who were treated with ultrasonography-guided RFA in our center from April 2014 to January 2019 were retrospectively analyzed.Trans-isthmus approach and moving shot technique were used during the RFA procedure.Ultrasonography and contrast-enhanced ultrasonography were performed before ablation,and immediately,1,3,6 and 12 months after ablation and then every 6 months thereafter. Results RFA was performed in 89 cases of papillary thyroid cancer,and no major complications were observed during the RFA.The mean follow-up was(18.8±7.3)months.The ablation zones decreased gradually during follow-up,and 38 ablation zones(42.7%)completely disappeared.The volume reduction rate was(99.2±2.3)% 30 months after ablation.During follow-up,2 patients(2.2%)developed tumor recurrence and 1 patient(1.1%)developed cervical lymph node metastasis. Conclusion Ultrasonography-guided RFA may be a safe and effective method for patients with T1bN0M0 papillary thyroid cancer.

Objective Papillary thyroid microcarcinoma (PTMC) has a good prognosis and a long survival time, surgery is the common treatment including total thyroidectomy (TT) and unilateral lobectomy (LT), but recent studies showed that TT does not show an advantage over LT for PTMC in preventing cancer recurrence and reducing mortality. Given this, the health‐related quality of life (HRQoL) has become one of the important factors that physicians must consider when making treatment decisions. The aim of this study was to compare the HRQoL of patients between undergoing TT and LT. Methods From October 2019 to December 2019, 69 PTMC patients were enrolled in our study, including 34 in the LT group and 35 in the TT group, respectively. We used three questionnaires which included the 36‐item short‐form health survey (SF‐36), thyroid cancer‐specific quality of life (THYCA‐QOL), and Fear of Progression Questionnaire‐Short Form (FoP‐Q‐SF) for each patient to evaluate their scores of HRQoL. Background and Objective: Intravascular ultrasound (IVUS) could take on a vital position when angiographic images are not clear enough to be precisely visualized or measured by computer-aided technology. This meta-analysis was designed to compare the benefits of IVUS-guided and angiography-guided percutaneous coronary intervention(PCI) strategies for improving clinical outcomes. Methods: PubMed, Embase, Web of Science, and Cochrane Library were searched for articles published from inception to 13th October, 2019. A comparative study of IVUS-guided and angiography-guided PCI strategies for patients with coronary bifurcation lesions was retrieved. The early endpoint events (≤1 year) and the late endpoint events (>1 years) were determined according to the follow-up time. The former included cardiac death, target lesion or vessel revascularization, stent thrombus, and major adverse cardiac events, while the latter included cardiac death. Adult neurogenesis plays a vital role in maintaining cognitive functions in mammals and human beings. Mobilization of hippocampal neurogenesis has been regarded as a promising therapeutic approach to restore injured neurons in neurodegenerative diseases including Alzheimer's disease (AD). Icarisid II (ICS II), an active ingredient derived from Epimedii Folium, has been reported to exhibit multiple neuroprotective effects. In the present study, we investigated the effects of ICS II on the proliferation and differentiation of neural stem cells (NSCs) and amyloid precusor protein (APP)-overexpressing NSCs (APP-NSCs) in vitro. Our results demonstrated that ICS II dose-dependently suppressed apoptosis and elevated viability of APP-NSCs. ICS II (1 μM) potently promoted proliferation and neuronal differentiation of NSCs and APP-NSCs. ICS II (1 μM) significantly upregulated Wnt-3a expression, increased the phosphorylation of glycogen synthase kinase-3β and enhanced the nuclear transfer of β-catenin. Moreover, ICS II also promoted astrocytes to secrete Wnt-3a, which positively modulates Wnt/β-catenin signaling pathway. These findings demonstrate that ICS II promotes NSCs proliferation and neuronal differentiation partly by activating the Wnt/β-catenin signaling pathway.

strongBACKGROUND/strongThe diagnosis of thyroid cancer and distinguishing benign from malignant thyroid nodules by junior radiologists can be challenging. This study aimed to develop a computer-aided diagnosis (CAD) system based on the Thyroid Imaging Reporting and Data System (TI-RADS) to distinguish benign from malignant thyroid nodules by analyzing ultrasound images to improve the diagnostic performance of junior radiologists.strongMATERIAL AND METHODS/strongA modified TI-RADS based on a convolutional neural network (CNN) was used to develop the CAD system. This retrospective study reviewed 789 thyroid nodules from 695 patients and included radiologists with different diagnostic experience. Five study groups included the CAD group, the junior radiologist group, the intermediate-level radiologist group, the senior radiologist group, and the group in which the junior radiologist used the CAD system. The ultrasound findings were reviewed and compared with the histopathology diagnosis.strongRESULTS/strongThe CAD system for the diagnosis of thyroid cancer showed an accuracy of 80.35%, a sensitivity of 80.64%, a specificity of 80.13%, a positive predictive value (PPV) of 76.02%, a negative predictive value (NPV) of 84.12%, and an area under the receiver operating characteristic (ROC) curve (AUC) of 0.87. The accuracy of the junior radiologists in diagnosing thyroid cancer using CAD was similar to that of intermediate-level radiologists (79.21%ivs/i. 77.57%; P=0.427).strongCONCLUSIONS/strongThe use of ultrasound CAD based on the TI-RADS showed potential for distinguishing between benign and malignant thyroid nodules and improved the diagnostic performance of junior radiologists.

BACKGROUND The aim of this study was to investigate the accuracy of contrast-enhanced ultrasound (CEUS) enhancement patterns in the assessment of thyroid nodules. MATERIAL AND METHODS A total of 158 patients with suspected thyroid cancer underwent conventional ultrasound (US) and CEUS examinations. The contrast enhancement patterns of the lesions, including the peripheries of the lesions, were assessed by CEUS scans. The relationship between the size of the lesions and the degree of enhancement was also studied. US- and/or CEUS-guided biopsy was used to obtain specimens for histopathological diagnosis. RESULTS The final data included 148 patients with 157 lesions. Seventy-five patients had 82 malignant lesions and 73 patients had 75 benign lesions. Peripheral ring enhancement was seen in 40 lesions. The differences of enhancement patterns and peripheral rings between benign and malignant nodules were significant (p=0.000, 0.000). The diagnostic sensitivity, specificity, and accuracy for malignant were 88%, 65.33%, and 88.32%, respectively, for CEUS, whereas they were 98.33%, 42.67%, and 71.97%, respectively, for TC by conventional US. The misdiagnosis rate by conventional US was 57.33% and 34.67% by CEUS (p=0.005). With regard to the size of lesions, a significant difference was found between low-enhancement, iso-enhancement, high-enhancement, iso-enhancement with no-enhancement area and no-enhancement (p=0.000). CONCLUSIONS In patients with suspicious US characteristics, CEUS had high specificity and contributed to establishing the diagnosis. Therefore, CEUS could avoid unnecessary biopsy.

Background and objectivesAn invasive approach is recommended as the treatment of patients with non-ST elevated acute coronary syndromes (NSTE-ACS). However, it remains unclear that the optimal time of angiography and intervention for patients with NSTE-ACS at present. This study was designed to compare the effect of early and delayed invasive strategies on short-medium term prognosis in patients with those.MethodsPubmed, Cochrane Library and Embase were searched up to Dec-30-2018. Randomized clinical trials comparing an early versus a delayed invasive strategy in patients with NSTE-ACS were included. The primary endpoint (all-cause death and recurrent myocardial infarction) and secondary endpoint (major bleeding and recurrent revascularization), as well as composite endpoint were assessed by random or fixed effected meta-analysis with software RevMan 5.3 version after short-medium term follow up.ResultA total of six randomized clinical trials involving 4,277 early or delayed invasive strategies patients with NSTE-ACS were included in the meta-analysis. Time to coronary angiography varied from 0.5 to 24 h in the early invasive strategy and from 18.6 to 72 h in the delayed invasive strategy. There was a statistical difference in the primary endpoint of all-cause death among patients with NSTE-ACS between early and delayed invasive strategies (4.6% vs 6%; OR:0.76; 95% CI:0.58 to 1.00; P = 0.05; I2 = 0%), but not for recurrent myocardial infarction (6.0% vs 6.3%; OR: 0.94; 95% CI: 0.55 to 1.61; P = 0.82; I2 = 60%). The major bleeding in patients with NSTE-ACS was similar between both invasive strategies (2.7% vs 3.1%; OR:0.88; 95% CI:0.59 to 1.31; P = 0.54; I2 = 0%). However, the composite endpoint in the early invasive strategy patients with NSTE-ACS was significantly lower than that of the delayed invasive strategy (10.9% vs 13.9%; OR:0.76; 95% CI:0.63 to 0.92; P = 0.006; I2 = 0%), and the recurrent revascularization between both strategies was just the opposite (8.7% vs 5.9%; OR:1.5; 95%CI:1.15 to 1.97; P = 0.003; I2 = 0%).ConclusionThe systematic review and meta-analysis demonstrated that the early invasive strategy had a beneficial trend on all-cause death and significantly reduced the composite endpoint in patients with NSTE-ACS, but increased the rate of revascularization. Background. Contrast-enhanced ultrasound (CEUS) scanning can detect differences between thyroid tumors and surrounding tissues. However, enhancement patterns within nodules are insufficient for the diagnosis of thyroid carcinomas. The peripheral enhancement patterns of nodules may provide useful diagnostic information. The objective of this study was to investigate the diagnostic accuracy of the peripheral enhancement patterns during CEUS scanning of thyroid nodules. Material and Methods. 120 nodules with peripheral rings during CEUS and definite pathology confirmed by surgery were included in this study. The internal and peripheral CEUS enhancement patterns of these nodules were assessed, and the diagnostic value of CEUS was compared with the conventional ultrasound. The relationship of types of peripheral rings and sizes of nodules was analyzed, respectively. Results. There were 78 benign and 42 malignant nodules. Peripheral irregular ring performs well in detecting malignancy. It improves the diagnostic performance of CEUS by combining with internal enhancement patterns (diagnostic sensitivity of 97.6%, specificity of 98.7%, and accuracy of 98.3%) and adds value to conventional ultrasound (95.2%, 70.5%, and 79.2%). The sizes of the nodules with regular high-enhanced rings (2.34 ± 1.33 cm) were larger than the other three types of peripheral rings (P<0.05). Conclusions. Features of peripheral ring on CEUS are important for the diagnosis of thyroid cancer; they can further contribute to the accuracy combining with the internal enhancement pattern, which could avoid the unnecessary biopsy.

Objective To assess the effectiveness and safety of ultrasound-guided percutaneous radiofrequency ablation (RFA) for nonsurgical treatment of metastatic lymph nodes (LNs) in the neck from papillary thyroid carcinoma (PTC). Methods Totally 45 PTC patients who had been treated with total thyroidectomy and radioiodine therapy were enrolled in this retrospective study. A total of 71 metastatic LNs in the neck from PTC were confirmed by percutaneous biopsy. Follow-up consisted of conventional ultrasound,contrast-enhanced ultrasound,and thyroglobulin (Tg) measurement 1,3 and 6 months after RFA and then every 6 months. Results All 45 patients were successfully treated,without immediate or later major complications occurred. During the follow-up [(23±5) months;range:12-30 months)],there was no evidence of recurrence at ablated sites. The mean volume reduction ratio (VRR) was significantly reduced during the follow-up. Significant differences in the VRR were found between every two follow-up visits (P0.001). Furthermore,46 metastatic LNs (64.8%) completely disappeared and 25 metastatic lymph nodes (35.2%) remained as small scar-like lesions at the last follow-up visit. After RFA,mean serum Tg level decreased from (11.3±6.3)ng/ml (range:0.8-19.4 ng/ml) to (1.3±0.9)ng/ml (range:0.2-3.9 ng/ml) at the last follow-up visit (P0.001). Preoperative assessment of the cervical lymph node status is important in therapeutic schedule and further evaluations of prognosis for papillary thyroid carcinoma (PTC) patients. Our aim was to investigate the diagnostic values of conventional ultrasound (US), contrast-enhanced ultrasound (CEUS) features and the expression of MMP-9 of PTC in predicting the cervical lymph node metastases (LNM). In total, 156 patients with PTC confirmed by surgical pathology were included. Seventy-one patients had cervical LNM, while 85 patients had no LNM. The patients had cervical LNM (39.51 ± 13.29 years) were younger than those had no LNM (44.15 ± 10.94 years) (P = 0.02). Multivariate logistic regression results showed that tumor size ≥0.95 cm (OR = 13.47), ill-defined margin (OR = 4.31), internal heterogeneous low-enhancement (OR = 5.19) and ECE (OR = 25.25) were predictive for the presence of cervical LNM. The detection rate of ECE for the PTC with LNM by CEUS (81.48%, 44/54) was higher than by US (46.30%, 25/54). There was significant difference in MMP-9 intensity between PTC with and without cervical LNM (P = 0.000), and intense reactions (+++) were mainly found in the PTCs with LNM (80.95%, 17/21). In conclusion, the combination of conventional US, CEUS features and MMP-9 expression may serve as an effective tool for predicting the cervical LNM of PTC.

At the first stage, the on-line information fusion estimator for the unknown model parameters is presented based on the Recursive Instrumental Variable (RIV) algorithm and the Recursive Extended Least Squares (RELS) algorithm, which is realized by computing the average of local estimators for model parameter. At the second stage, the on-line information fusion estimator for the unknown variances is obtained using the correlation method, which is realized by computing the average of the local estimators for noise variances. At the third stage, the information fusion parameter estimator of MA model is presented using the correlation method and the dead zone Gevers-Wouters and LS algorithms.

Berberine is an isoquinoline alkaloid widely used in the treatment of microbial infections. Recent studies have shown that berberine can enhance the inhibitory efficacy of antibiotics against clinical multi-drug resistant isolates of methicillin-resistant Staphylococcus aureus (MRSA). However, the underlying mechanisms are poorly understood. Here, we demonstrated that sub-minimum inhibitory concentrations (MICs) of berberine exhibited no bactericidal activity against MRSA, but affected MRSA biofilm development in a dose dependent manner within the concentration ranging from 1 to 64 μg/mL. Further study indicated that berberine inhibited MRSA amyloid fibrils formation, which consist of phenol-soluble modulins (PSMs). Molecular dynamics simulation revealed that berberine could bind with the phenyl ring of Phe19 in PSMα2 through hydrophobic interaction. Collectively, berberine can inhibit MRSA biofilm formation via affecting PSMs’ aggregation into amyloid fibrils and thereby enhance bactericidal activity of antibiotics. These findings will provide new insights into the multiple pharmacological properties of berberine in the treatment of microbial-generated amyloid involved diseases.

A new type of flexible suspension tension structure (FSTS) is presented in this paper, which has the advantages of high-efficiency, convenient construction and wide application sphere, and it is also a kind of green and low carbon structure. In order to investigate the mechanical behavior of FSTS under the vertical load, series of structures under 10 groups of upward loads and 8 groups of downward loads are calculated with the matrix analysis method. The results show that the passive tensions of cables and displacement in the middle of span present approximate linear features under the downward loads. However, there are double broken linear features in FSTS under the upward loads. So this regularity can be utilized for the simple analysis and two-stage design calculation. Finally, the reliability of FSTS with the initial tension is investigated with the interval method. 81 kinds of structures with different initial tension are calculated, and the influence regularity of the initial tension in the back cable and the bottom cable are given. It is found that the deformation of FSTS is more sensitive to the initial tension of cables. Consequently, the initial tensions of cables are dominated by deformation of structures under design and construction.

Then the integrated model of a high-rise building is established, and the vibration responses of the floor where a precise instrument is fixed are calculated by inputting micro-vibration loads. The passive control method is used by putting several lead rubber bearings at the bottom of instrument and at the bottom of building separately, and the isolated vibration responses are calculated and analyzed. Some valuable conclusions are obtained by comparing the vibration responses of isolation system and non-isolation system with acceptable values according to the environmental vibration codes for precise instruments.

Reactive powder concrete (RPC) is one of new composite material, which is applied in the pilot project of the railways construction. According to the literature survey and test data report, the major factors which influence the reliability of 32m prestressed RPC girder in railway bridges are given in this paper. Furthermore, combining numerical simulation with finite-element method, the reliability of the prestressed RPC girder is calculated. The results show that the failure probability and reliability parameter can be obtained through direct sampling method and the latin hypercubic sampling method. The reliability parameter of fracture is 3.9 with two methods, while the reliability parameter of deflection control is 4.1 and 4.2 respectively with the two methods, and the reliability parameters meet the demands of norm of railway bridges. Consequently, the RPC is safety to be used in the railway bridges.

A detailed investigation of the absorption and CD signals of Ag(I)-cysteine (Cys) aqueous solutions at buffered or varying pH has allowed us to suggest that coordination polymers are formed upon mixing Ag(I) and Cys bearing a Ag(I)-Cys repeat unit. The formation of the coordination polymers are shown to be facilitated by both the Ag(I)···Ag(I) interaction and the interaction between the side chains in the polymeric backbone. The former allows for an immediate spectral sensing of Cys with enantiomeric discrimination capacity with both high sensitivity and selectivity, and the contribution of the side-chain/side-chain interaction serves to guide extended sensing applications by means of modulating this interaction. With our preliminary data on the corresponding Cu(I)-Cys and Au(I)-Cys systems that exhibited similar spectral signals, we conclude that the M(I)-SR coordination polymers (M = Cu, Ag, or Au) could in general function as spectral sensing ensembles for extended applications. This sensing ensemble involves the formation of coordination polymers with practically no spectral background, thus affording high sensing sensitivity and selectivity.

Zn-Al hydrotalcite exhibited fluorescence property without fluorescent substances intercalation. Its maximum excitation wavelength was 406nm and maximum emission wavelength was 523nm. In this study citric acid and adipic acid were introduced into the interlayer of LDH, which enlarged the interlayer spacing. It was found that the fluorescence property was influenced by anions in the interlayer of hydrotalcites. And it would be favorable to bind with other organic compounds to get photocatalysis materials for citric acid modified Zn-Al hydrotalcite. Meanwhile, the fluorescence intensity was enhanced greatly after modified by adipic acid, so it could be used as photocatalyst precursors.
