痔瘻根治手術を受け再発し来院してきた39症例と，膿瘍切開を繰り返し来院してきた19症例の摘出瘻管について検索した．再発痔瘻では，膿瘍併存型痔瘻を示した症例は74%あり，膿瘍切開を繰り返した症例では63%に認めた．再発症例のうち組織検索可能例は22例あり，膿瘍切開を繰り返した症例は9例あった．術後痔瘻再発例に肛門腺を認めた症例は14%あり，膿瘍切開再発症例では56%に認めた．組織学的所見は双方ともに被膜様線維化組織に囲まれた好中球とリンパ球，形質細胞を主体とした膿瘍と周囲線維組織間を拡散する炎症性細胞の浸潤を認めた．脂肪組織と筋肉組織内は線維化が著明でリンパ球，形質細胞と少数の好酸球の慢性炎症性細胞の浸潤を認めた．このことから再発の原因は，一次口の位置を誤認して手術した場合と線維化組織に囲まれた分岐あるいは併存する炎症拡散性の膿瘍を遺残させた場合と考えられた．

術後遅発性出血は，創傷治癒過程の増殖期を中心に組織再構築期までおこる合併症である．その原因は創傷治癒障害，血中の凝固異常，線溶系機能の亢進によるものではなく，術後の正常の創傷治癒過程において細胞外マトリックス内の血管増殖因子活性により血管が新生されるときプラスミノゲン·アクチベータが活性され，この時期に一致して出血がおこると考えられる．血小板の変動は，創傷治癒サイトカインの変動と強い相関を持ち，特に組織再構築に関与し，血小板の機能障害がおこると治癒期に至るまで出血を経験する事がある．痔核血管は動静脈吻合と露出血管部に出血がおこりやすく，術後はマトリックス内に類似血管が新生されることにより出血しやすくなると推察される．痔核切除創を肛門管内に露出させない縫合手技により出血症例を減少させ，高確率で術後出血が予防できた．

目的, 対象と方法 : 一次口を別にする瘻管を2本以上をもつ痔瘻33例 (男性31人, 女性2人) 瘻管総数67本について瘻管を完全切除し, その組織像を検索した. 結果 : 多発性痔瘻では外括約筋を貫く肛門管に対し放射状に直走する瘻管, 組織型では瘻管形成型 (37/67) が多く, 一次口の位置では6時位 (18/67) に多かった. 瘻管内に肛門腺を認める症例は49% (33/67) であり, IV型痔瘻はなかった. 瘻管組織の特徴は, 膿瘍併存型痔瘻でも脂肪組織内に炎症細胞の浸潤を認めるものの, 膿瘍を強く形成する症例はなかった. 瘻管形成型, 膿瘍併存型痔瘻にみられる肛門腺は分葉型 (20/33) を多く認めた. 円柱上皮と移行上皮で構成される管壁では腺管破壊像と粘液産生能を認めたが, 扁平上皮化生で構成される肛門腺は, 管壁の破壊像, 粘液産生能はなく, 管壁周囲の炎症は軽度あるいは消退していた. 結論 : 多発性痔瘻の瘻管は, 炎症性細胞の浸潤は弱く, 肛門腺の関与する割合は半数以下であり, 肛門腺は感染に弱いが扁平上皮に化生するため炎症は消退しやすいと推測された.

便失禁症例,男性8人,女性10人について週1～2回の頻度で肛門管内を低周波電気刺激を行いその効果について検索した.治療期間は,平均116±39日であった.便失禁スコアーは男女ともに改善した.肛門内圧検査でも,低周波電気刺激により男性では肛門管最大静止圧は83%,女性では52%上昇し,最大随意収縮圧は男性63%,女性21%上昇した(p

難治性の慢性直腸肛門痛症例,男性19人,女性42人について肛門管内を双極刺激電極を挿入し低周波電気刺激を週1～2回,一回5分間行いその効果について検討した.これら症例のうち男性9人,女性12人について低周波電気刺激前後における左側肛門管組織血流量の測定を行った.症例の肛門内圧検査を行い随意最大収縮圧100mmHg以上の症例(機能正常群)と100mmHg以下(機能低下群)に分けて検討した.男性では,機能正常群が多く,女性では機能低下群が多かったが,慢性直腸肛門痛との関連はなかった.痛みが消失するまでの刺激回数は平均3.5±1.6回であり,98%に効果を認め,59%に痛みの完全消失が得られた.肛門管組織血流量,血流速度は低周波電気刺激により男性で64%,女性で33%の有意な増加がみられた(p

DDS(drug delivery system)の概念を取り入れて開発された経皮吸収型鎮痛消炎貼付剤は炎症部位に近接した皮膚から薬剤が安定的に浸透作用し，消化器症状などの副作用がないために汎用されている。その一方で，同一部位に繰り返し貼付されることから，皮膚への高い安全性が求められる。今回我々は市販されている経皮吸収型鎮痛消炎貼付剤，(1)フェルビナク含有プラスター剤(フェルビナクP「EMEC」)(被験薬剤A)(2)インドメタシン含有プラスター剤(セラスター®)(被験薬剤B)，(3)ケトプロフェン含有プラスター剤(モーラス®テープ)(被験薬剤C)，(4)フルルビプロフェン含有プラスター剤(ヤクバン®40)(被験薬剤D)，および(1)の基剤(被験薬剤E)が皮膚に及ぼす影響について検討した。78名の被験者に被験薬剤を連続貼用し，2日，4日，6日後に視診，マイクロスコープによる拡大写真，経皮的水分蒸散量，皮膚角質水分量などを記録・測定評価した。これら5被験薬剤は経皮的水分蒸散量，皮膚角質水分量が高い被験薬剤C，Dと低い被験薬剤A，B，Eに分けられた。この違いは製剤の製造法の違いに起因している。また，これら検査値の高値は貼付剤によりふやけた状態(maceration)になったことを示し，潜在的な皮膚への刺激反応になると考えられた。発赤などの刺激反応等を総合するとEを除外した市販されている4製剤の中では被験薬剤Aが最も安全性が高いと考えられた。医療材料の安全性の評価には視覚的検査以外に経皮的水分蒸散量，皮膚角質水分量などによる評価も検討するほうが望ましい。

痔核手術で得られたヒト痔核組織に対し,炭酸ガス(CO2)レーザー,Ne : YAG(ヤグ)レーザー,半導体レーザーにより5W,7Wを5秒間照射しその影響を検索した.CO2レーザーはfocus beamとdefocussed beamで,ヤグレーザーと半導体レーザーでは,内照射と外照射を行い,さらに組織内にインドシアニングリーン(ICGlmg/ml)を注入し,レーザー光を照射した.CO2レーザーは,炭化凝固の少ない良好な創が得られ,ICG注入により深達は阻止された.ヤグレーザーと半導体レーザーの内照射では,CO2レーザーに劣らない創が得られ,外照射ではヤグレーザーは深達度が強いものの,色素依存性がないためICG注入により深達度は低下し,反対に半導体レーザーでは増強した.痔核血管に対するレーザー光の吸収はヤグレーザーが最も強く,半導体レーザー>CO2レーザーで弱かった.痔核手術にレーザーを用いる場合,それぞれの特性を生かした手術方法を考案する必要があると考えられた.

痔瘻根治手術で得られた一次口より二次口に至る組織学的所見について,広範囲膿瘍型7人,膿瘍併存型36人,瘻管形成型25人,瘻管閉鎖型21人について連続切片を作成し検索した.痔核手術で得られた組織の肛門腺19例を対照とした.瘻管内に肛門腺を認めたのは60%で肛門腺上皮には円柱上皮と,扁平上皮化生を示すものとがあった.広範囲膿瘍型,膿瘍併存型,瘻管形成型の円柱上皮肛門腺は,細胞の大きさ,腺管外径,内径とも対照と比べて有意に大きかった.扁平上皮肛門腺では広範囲膿瘍型,膿瘍併存型,瘻管形成型で腺管外径は対照と比較して有意に大きく,内腔は瘻管形成型で閉鎖箇所を認め瘻管閉鎖型で全例閉鎖していた.肛門腺由来の痔瘻形成では,肥大した円柱上皮肛門腺を通して異物が反復侵入し炎症を起こし膿瘍を形成,炎症の中で腺管は扁平上皮化生することにより管腔は閉鎖し,異物の進入をくい止め炎症は消退する.反対に脂肪組織へ炎症が波及すると膿瘍は拡大すると推察された.

69歳男性の肛門部基底細胞上皮腫の1例を報告した。肛門部7°～11°の方向に45mm×25mm, 高さ10mmの黒色腫瘤があり, 一部線状に肛門管へ浸潤し, 歯状線直上まで存在していた。生検にて基底細胞上皮腫の診断を得, 辺縁5mm離し, 脂肪織と外肛門括約筋を一部含めて切除後, V-Y advancement subcutaneous flapにて創を一期的に閉鎖した。術後排便良好。鑑別診断として, cloacogenic zone (移行帯) 由来の類基底細胞癌があげられる。

回腸のblind loopに発生した多発性腺癌の1例を報告した.症例は77歳女性で52歳時 (25年前) に回腸末端炎によるイレウスに対して, 回腸横行結腸吻合術を受けた.昭和60年9月より, 胃潰瘍にて近医で治療をうけていたが, 昭和62年4月, 胃潰瘍穿孔による腹膜炎で緊急手術が施行された.広範囲胃切除術ならびに胃十二指腸吻合術 (Billroth I法) とともに, 回腸上行結腸のblind loop部の切除と回腸横行結腸吻合術を行った.切除標本の肉眼所見では, 回盲弁 (Bauhin弁) の変形と高度の狭窄が見られ, blind loopを形成している回腸粘膜に多発性の潰瘍性病変がみられた.組織学的には, 多発性潰瘍6個のうちの3個に分化型腺癌が認められ, 腸間膜まで浸潤していた.Crohn病や, 結核などの所見はなかった.術後の経過は順調であったが, 11か月目に他院にて, 癌再発で死亡した.

Adenosquamous cell carcinoma of thje stomach is uncommon, pasticularly that occurring in the residual stomach at the site of the gastrojejunostomy. This paper dealed with a case of adenosquamous cell carcinoma at the site of gastrojejunostomy 31 years after gastrectomy. A 57-year-old man who had had distal gastrectomy and gastrojejunostomy (Billoroth II) for benign duodenal ulcer was admitted to our hospital complaining of epigastric pain. Carcinoma at the site of gastrojejunostomy was found as a result of various examinations, and total resection of the residual stomach and combined resection of the pancreas and the transverse colon were performed. The infiltrative and ulcerative tumor at the site of gastrojejunostomy was 5.5×4.0cm in size. Histology revealed adenosquamous cell carcinoma. Six months after surgery the patient died of peritonitis carcinomatosa and the liver metastasis of the carcinoma.

The purpose of this study is to make clear changes to participate in "DARC". For this purpose, we made research to use the same measure of previous study for DARC members at fifteen sites in Japan. As a result we found characteristic changes of drug dependents those who participate in DARC. Between terms of drug abuse and terms of participate in DARC, we can find weak positive correlation coefficient (R=.387**). People, whose terms of drug abuse are long, have tendencies to DARC participation for a long time. But there is also a tendency that the longer a person DARC participation, the longer he or she quit drugs. On the other hand, there is no correlation coefficient between terms of drug free and those of drug abuse. From this fact, we find that it is not only the short term drug dependent who quit drugs. The recovery of drug dependents in DARC is caused through life experiences on trust in fellow members. It is not a straight path but it is suggested that the drug free dose not change dramatically in one or two years. The changes in these years are the crossroad of re-abuse later, or the turning point to recover and to continue the drug free. The anxieties about life in these years make questions for meanings of life, and this is a chance to make possibilities for choices of new life not to have drugs. In dependents those who are recovering to DRAC participation, they make fails over again, however, we think the changes of spirituality through DARC participation.

腹部大動脈瘤-腸管瘻は腹部大動脈瘤破裂の特殊な形であり予後不良で手術成功例は稀である. 今回われわれは腹部大動脈瘤-S状結腸瘻の一手術例を報告する. 症例は腹部大動脈瘤の診断にて経過観察中であった58歳男性で, 腹痛・腰痛・乏尿を主訴に来院, 臍部を中心に手挙大の拍動性腫瘤を触知, 疼痛を訴え緊急入院した. 入院後5日目大量下血を生じショックに陥った. 腹部大動脈瘤-腸管瘻と診断し動脈瘤切除と right axillo-bifemoral bypass 術および Hartmann 手術を行った. 緊急手術を施行し術後発症した後腹膜膿瘍は持続的経皮ドレナージにて治癒したが術前より合併していた拡張型心筋症のため術後4か月半で死亡した. 腹部大動脈瘤は破裂や消化管瘻などの合併症をきたす前に早期手術を施行することが最重要であると思われた.

Carcinoma in adenoma of the papilla of Vater is extremely rare. We now report a case of adenocarcinoma in adenoma of the papilla of Vater and the clinicopathological findings are discussed. A 73-year-old Japanese woman was endoscopically diagnosed as a case of carcinoma combined with adenoma of the papilla of Vater. She underwent pancreatoduodenectomy and the postoperative course was statisfactory. Histological examination of the resected tissues revealed a superficial adenocarcinoma in the adenoma of the papilla of Vater. In certain cases, carcinoma of the papilla of Vater may develop from a pre-existing adenoma in the region. Therefore, we recommend that pancreatoduodenectomy should be done when an adenoma presents in this region.

Reported in this paper are two cases of peptic ulcer which developed in the gastric tube used for esophageal replacement following esophagectomy for esophageal tumors in adults. The results of our cases suggest that acid secretion from the mucosa of the gastric tube, in spite of truncal vagotomy and the state of the tube, seems to play important roles in the pathogenesis of peptic ulcer of the gastric tube after esophageal replacement, even though circulatory disturbances, due to postoperative irradiation and gastric tube formation, have been suspected as the cause. Therefore, in those cases of gastric tube stasis, surgical drainage of the gastric tube should be performed.

結腸に発生した腺扁平上皮癌の2例を経験したので若干の文献的考察を加え報告した.症例1は75歳男性.S状結腸から直腸S状部にび浸潤型の病変を認め, 術中所見で肛門側断端に癌の遺残が疑われたため直腸切断術を施行した.肉眼的進行度はP0, H0, N2, S2でStage IVであった.術後21日目に上部消化管出血に起因する多臓器不全にて死亡した.症例2は55歳女性.S状結腸に限局潰瘍型の病変を認め, 前方切除術を施行した.肉眼的進行度はP0, H0, N0, S2でStage IIであった.術後2年1カ月の現在再発の徴候なく健在である. いずれも病理組織学的に, 腺癌成分と扁平上皮癌成分が混在としており腺扁平上皮癌と診断された.その組織発生に関する可能性としては, 腺癌成分と扁平上皮癌成分の移行像がみられることなどから, 腺癌の直接扁平上皮化生説が有力と考えられた.

若年者胃癌22例23病巣について胃癌取扱い規約に基づいて臨床病理学的検索を行った. 年齢は20歳から29歳, 平均26.3歳で, 男女比は1.4: 1であった. 頻度は切除胃癌の2.9%で, 初発症状では消化性潰瘍を示唆するものが多かった. 占居部位はMが78.3%を占めた. 肉眼型分類では0型が52.2%で, 3, 4型は合計30.4%と低率であった. 組織学的分類では低分化型が87%と多かった. 組織学的進行程度はstage Iが50%, IVは13.6%と低率であった. 手術成績は切除率100%, 治癒切除率86.4%であり, 遠隔成績は切除例の実測5生率は76.5%で, 治癒切除例のそれは92.9%であった. 若年者胃癌治癒切除例の予後は一般胃癌よりも優れた成績であった.

A case of fuluminating Crohn's disease presented with diarrhea in the seventh month of pregnancy, accompanied with ileus, congestive heart failure and massive fresh blood in the stools. Cesarean section was combined with total colectomy and ileocolectostomy. The patient and baby were saved. TPN was performed for postoperative management of nutrition, and enteral nutrition using elemental diet was also effective in the phase between TPN and regular diet. Such cases should be dealt with by a cooperating team of surgeons, obstetricians, internists, and pediatricians.

Leiomyosarcoma of the residual stomach is extremely rare. Including the present case, there have been only six cases reported in Japan. A 71-year-old female who had undergone gastrectomy and gastroduodenostomy (Billroth I) because of gastric ulcer was admitted mainly with complaint of epigastralgia 11 years after the first surgery. By upper gastrointestinal series, gastrofiberscopic examination and CT-scan, she was diagnosed to have submucosal tumor of the residual stomach. Angiography of the celiac axis demonstrated a tumor stain with hypervascularity, which was supplied by the left gastric artery and the short gastric artery. Total resection of the residual stomach with dissection of the lymph nodes and reconstruction by Roux-Y method were performed. The resected tumor measuring 16×11×8cm in size showed exogastric growth, and no ulcer was observed on the mucasal surface of the tumor. Histology of the specimen revealed leiomyosarcoma. She showed metastasis in the liver 11 months after the second surgery. The clinical and pathological characteristics of leiomyosarcoma of the residual stomach are similar to that of leiomyosarcoma of the no-treated stomach.

A 36-year-old man was transferred to our department because of massive malena followed by shock. Gastroduodenal endoscopy revealed no bleeding source in the stomach or the duodenum. RI-angiogram performed immediately after admission revealed a bleeding area at the jejunum and laparotomy was done. The bleeding lesion was detected by exploratory jejunotomy and a jejunectomy was successfully performed over 15 cm, including the lesion. RI-angiogram is useful as selective angiography in the diagnosis of intestinal bleeding and is less invasive.

Leiomyosarcoma of the stomach is a comparatively rare disease. Particulary, large leiomyosarcoma of the stomach, whose maximal diameter is no less than 15cm, is observed very seldom. The clinical entity of so-called “huge leiomyosarcoma of the stomach” is discussed in 36 cases reported in Japan, including 2 cases treated in our university hospital. Mass was felt in the epigastrium at the first examination in many patients (63.9%) with leiomyosarcoma of the stomach. Many tumors developed on the side of the greater curvature of the upper region of the stomach, and most of them showed exogastric growth. Ulcer fomation was observed on the mucosal surface of the tumors in 41% of 36 cases, similar to the general tendency for leiomyosarcoma. For diagnosis of the disease, roentgenogrophy, ultrasonography, CT-scan and angiography were effectively used, and angiography in particular enabled clarification of the vascularization of tumors, by which it was often possible to determine the organ in which the tumors intially developed. Metastasis to the liver was observed in 8 of 36 cases, and metastasis to the peritoneum in 2 cases. In one case that received partial resection of the stomach wall, recurrence was observed on the residual stomach wall. None of the 36 cases showed metastasis to the lymph nodes. However, metastasis to the lymph nodes was found in other published cases. From these results, it may be suggested that partial resection of the stomach wall is not sufficient in the operation for leiomyosarcoma of the stomach, and that gastrectomy of necessary and sufficient volume together with dissection of lymph nodes as applied in gastric cancer should be performed.

癌浸潤が肉眼的に胃, 十二指腸の両側におよんでいるとみなされた79例について, 臨床的ならびに病理組織学的に検索し, 切除度および術後再発を左右する因子について検討した.臨床的十二指腸壁に癌浸潤が確認された症例は79例中35例 (44.3%) で, その発生側はほとんどの症例で明らかでないが, 胃癌の十二指腸浸潤と考えるよりは, その進展の態度ならびに臨床的意義から, 胃・十二指腸境界部癌の概念で把握するのが妥当と思われる症例が6例みられた. この概念に該当する症例は肉眼的にBorrmann 1, 2, 3型である.十二指腸壁内先進部はmおよびsmにあり, リンパ管内蔓延が問題となる.転移では (8),(12) および (13) リンパ節が第一群リンパ節としての意義を有する.

In both conscious and anaesthetized dogs the adrenal cortical secretion in response to nicotine was directly measured. In conscious dogs the adrenal 17-hydroxycorticosteroid secretion rate increased, after the intravenous injection of 0·1 mg/kg of nicotine, from 0·12 ± 0·025 µg/kg/min to 1·61 ± 0·22 µg/kg/min (mean ±S.E.M.). Similar results were obtained in dogs anaesthetized with sodium pentobarbital. After hypophysectomy no increase in adrenal cortical secretion was observed following the administration of the same dose of nicotine. It is suggested that the adrenocortical response to nicotine is mediated through the adrenocorticotrophin release of the pituitary gland.

Conscious dogs were injected intravenously with 1.0mg/kg of pilocarpine or 0.3mg/kg of nicotine. Adrenal venous effluent was collected before and after the injection and analyzed for adrenaline and noradrenaline fluorimetrically. After the injection of pilocarpine a marked increase in adrenaline secretion with an inconsistent and slight increase in noradrenaline secretion was observed. Following the injection of nicotine the adrenaline and noradrenaline secretion increased markedly, the increase in the former being predominant.

Hypophysectomized dogs were used to study the effect of hemorrhage-induced hypotension upon the responsiveness of the adrenal cortex to adrenocorticotrophin (ACTH). Adrenal venous blood samples were collected under sodium pentobarbital anesthesia and were analyzed for 17-hydroxycorticosteroids (17-OHCS). ACTH was infused intravenously throughout each experiment. A sustained rise in adrenal 17-OHCS secretion was produced. Marked falls in arterial blood pressure and adrenal blood flow were induced by a removal of 20-25ml/kg of arterial blood. The rate of adrenal 17-OHCS secretion was decreased to the level of 28-65% of the pre-hemorrhage secretion rate.
