1. Increased PAI-1 plasma levels and risk of death from dengue: no association with the 4G/5G promoter polymorphism

Abstract

Abstract Background Dengue virus infected patients have high plasminogen activator inhibitor type I (PAI-1) plasma concentrations. Whether the insertion/deletion (4G/5G) polymorphism in the promotor region of the PAI-1 gene is associated with increased PAI-1 plasma concentrations and with death from dengue is unknown. We, therefore, investigated the relationship between the 4G/5G polymorphism and PAI-1 plasma concentrations in dengue patients and risk of death from dengue. Methods A total of 194 patients admitted to the Dr. Kariadi Hospital in Semarang, Indonesia, with clinical suspected severe dengue virus infection were enrolled. Blood samples were obtained on day of admission, days 1, 2 and 7 after admission and at a 1-month follow-up visit. Plasma concentrations of PAI-1 were measured using a sandwich ELISA kit. The PAI-1 4G/5G polymorphism was typed by allele-specific PCR analysis. Results Concentrations of PAI-1 on admission and peak values of PAI-1 during admission were higher than the values measured in healthy controls. Survival was significantly worse in patients with PAI-1 concentrations in the highest tertile (at admission: OR 4.7 [95% CI 0.9–23.8], peak value during admission: OR 6.3 [95%CI 1.3–30.8]). No association was found between the PAI-1 4G/5G polymorphism, and PAI-1 plasma concentrations, dengue disease severity and mortality from dengue. Conclusion These data suggest that the 4G/5G polymorphism has no significant influence on PAI-1 concentrations in dengue virus infected patients and is not associated with the risk of death from dengue. Other factors contributing to the variability of PAI-1 plasma concentrations in patients with dengue need to be explored.

Abstract

The Antillean manatee (Trichechus manatus manatus), a subspecies of the West Indian manatee, inhabits fresh, brackish, and warm coastal waters distributed along the eastern border of Central America, the northern coast of South America, and throughout the Wider Caribbean Region. Threatened primarily by human encroachment, poaching, and habitat degradation, Antillean manatees are listed as endangered by the International Union for the Conservation of Nature. The impact of disease on population viability remains unknown in spite of concerns surrounding the species' ability to rebound from a population crash should an epizootic occur. To gain insight on the baseline health of this subspecies, a total of 191 blood samples were collected opportunistically from wild Antillean manatees in Belize between 1997 and 2009. Hematologic and biochemical reference intervals were established, and antibody prevalence to eight pathogens with zoonotic potential was determined. Age was found to be a significant factor of variation in mean blood values, whereas sex, capture site, and season contributed less to overall differences in parameter values. Negative antibody titers were reported for all pathogens surveyed except for Leptospira bratislava, L. canicola, and L. icterohemorrhagiae, Toxoplasma gondii, and morbillivirus. As part of comprehensive health assessment in manatees from Belize, this study will serve as a benchmark aiding in early disease detection and in the discernment of important epidemiologic patterns in the manatees of this region. Additionally, it will provide some of the initial tools to explore the broader application of manatees as sentinel species of nearshore ecosystem health.

The World One Health Congresses are biennial gatherings of approximately 1500 professionals from relevant international organisations, OIE, FAO, WHO, World Bank, leading scientific experts and researchers in the field of One Health, animal production and trade, food safety, animal health, human health and environmentology/ecology, government representatives in public health, human health, food safety, environmental health and global health security. The Congress is organized by the One Health Platform. This white paper summarizes highlights of the 5th International One Health Congress in Saskatoon, Canada, June 2018 and serves as a roadmap for the future, detailing several concrete action points to be carried out in the run-up to the 6th World One Health Congress in Edinburgh, Scotland, June 2020.

Abstract

A wide range of viruses from different virus families in different geographical areas, may cause immediate or delayed neuropathological changes and neurological manifestations in humans and animals. Infection by neurotropic viruses as well as the resulting immune response can irreversibly disrupt the complex structural and functional architecture of the central nervous system, frequently leaving the patient or affected animal with a poor or fatal prognosis. Mechanisms that govern neuropathogenesis and immunopathogenesis of viral infections are highlighted, using examples of well-studied virus infections that are associated with these alterations in different populations throughout the world. A better understanding of the molecular, epidemiological and biological characteristics of these infections and in particular of mechanisms that underlie their clinical manifestations may be expected to provide tools for the development of more effective intervention strategies and treatment regimens. Abstract

The four genetically divergent dengue virus (DENV) types are traditionally classified as serotypes. Antigenic and genetic differences among the DENV types influence disease outcome, vaccine-induced protection, epidemic magnitude, and viral evolution. We characterized antigenic diversity in the DENV types by antigenic maps constructed from neutralizing antibody titers obtained from African green monkeys and after human vaccination and natural infections. Genetically, geographically, and temporally, diverse DENV isolates clustered loosely by type, but we found that many are as similar antigenically to a virus of a different type as to some viruses of the same type. Primary infection antisera did not neutralize all viruses of the same DENV type any better than other types did up to 2 years after infection and did not show improved neutralization to homologous type isolates. That the canonical DENV types are not antigenically homogeneous has implications for vaccination and research on the dynamics of immunity, disease, and the evolution of DENV.

Abstract

Het influenzavirus ondergaat frequente antigene veranderingen die jaarlijkse aanpassing van het influenzavaccin door de WHO noodzakelijk maken. Voor dit doel genereert, verzamelt en analyseert het Nationaal Influenza Centrum (NIC, een samenwerkingsverband van het RIVM met de Erasmus Universiteit Rotterdam) in Nederland bij huisarts- en ziekenhuispatienten geisoleerde influenzavirusstammen. Het NIC vergelijkt deze stammen met de gebruikte vaccinstammen en met andere buitenlandse stammen. In zowel Nederland als elders in Europa was de influenzaepidemie van 1995/96 vroeg en van een gemiddelde omvang. Het subtype A(H3N2) overheerste maar ook A(H1N1)- en B-virussen werden gerapporteerd. De hoofdvarianten uit de seizoenen 1995 (zuidelijk halfrond) en 1995/96 (noordelijk halfrond) bleken in antigeen opzicht bij alle drie (sub)typen weinig of niet af te wijken van die uit 1994 en 1994/95, of van de gebruikte vaccinstammen. Zowel de immuniteit verworven door vroegere infecties als die opgewekt door vaccinatie zullen derhalve relatief goede bescherming hebben geboden tegen infectie met influenzavirus in 1995/96.

Abstract

In dit onderzoek werd de bruikbaarheid van een nieuw ontwikkeld, relatief goedkoop isolatie-systeem voor de huisvesting van SPF dieren vergeleken met de bestaande SPF-kweek die gekarakteriseerd kan worden als een klassiek strikt barriere-systeem. Dit rapport beschrijft een aantal waarnemingen met berekking tot fysiologische en microbiologische parameters bij SPF ratten gehouden tijdens kweek en experiment in beide SPF-units. Er werden geen duidelijke verschillen in de waarden voor de verschillende fysiologische en microbiologische parameters gevonden. Geconcludeerd wordt dat bouwkundig relatief eenvoudige isolatie-systemen goed bruikbaar lijken voor de kweek van SPF ratten en het uitvoeren van langdurende experimenten.

Abstract

Het influenzavirus ondergaat frequente antigene veranderingen die jaarlijkse aanpassing van het influenzavaccin noodzakelijk maken. Voor dit doel coordineert de Wereldgezondheidsorganisatie (WHO) een mondiaal netwerk van virologische laboratoria. In dit kader vormen de Erasmus Universiteit Rotterdam (EUR) en het RIVM tezamen het Nationaal Influenza Centrum (NIC) voor Nederland. De EUR verzamelt recente influenzavirusstammen uit de Nederlandse diagnostische laboratoria, analyseert deze met serologische en moleculaire technieken, vergelijkt deze met referentiestammen en met de door de WHO voorgestelde vaccinstammen en zendt de stammen naar de WHO. Het RIVM voert in samenwerking met NIVEL een surveillance van respiratoire virusinfecties uit. De hieronder geisoleerde influenzavirusstammen worden eveneens onderzocht met serologische en moleculaire technieken en naar de WHO verzonden. Bovendien verzamelt en analyseert het RIVM influenzavirusstammen uit het buitenland en vergelijkt deze met de Nederlandse stammen. In heel Europa behalve Zwitserland was de influenza-epidemie van 1994/95 laat en klein. Stammen van influenzavirus B, A(H3N2) en A(H1N1) circuleerden naast elkaar. Uit de seizoenen 1994 (zuidelijk halfrond), 1994/95 en 1995 werden 84 stammen van het type B, 82 van A(H3N2) en 16 van A(H1N1) onderzocht. De hoofdvariant van het B-virus uit 1994/95 verschilde in antigeen opzicht significant zowel van de hoofdvariant uit de vorige epidemie van 1992/93 als van de vaccinstam die in 1994/95 werd gebruikt. Noch de immuniteit verworven door infectie, noch die opgewekt door vaccinatie zullen derhalve optimale bescherming hebben geboden tegen infectie met type B in 1994/95. De voornaamste variant van het A(H3N2)-virus in 1994/95 evenwel verschilde in antigeen opzicht niet van die in de vorige epidemie van 1993/94, noch van de vaccinstam voor 1994/95. Zowel de natuurlijk verkregen immuniteit als de influenzavaccinatie zullen dus optimaal hebben beschermd tegen infectie met A(H3N2) virus in 1994/95. De A(H1N1) virusstammen uit 1994/95 weken evenmin af van die uit vorige jaren of van de in 1994/95 toegepaste vaccinstam. Bij vergelijking van de antigene eigenschappen en de RNA-nucleotidensequenties van de Nederlandse influenzavirusstammen van 1994/95 met die uit het buitenland werden geen verschillen van betekenis gevonden tussen deze twee groepen. In onze studie hebben wij onder de influenzavirusisolaten uit Oost-Azie uit 1995 geen nieuwe variant aangetroffen. In het licht van de gegevens gepresenteerd in dit rapport lijkt het advies van de WHO voor de vaccinsamenstelling voor 1995/96 adequaat.

Dit rapport geeft de resultaten weer van de ontwikkeling van een monoclonaal antilichaam dat specifiek ratte IgA immunglobulinen herkent. IgA is een belangrijk immuunglobuline voor de bescherming van muscosale oppervlakken (o.a. op het niveau van de darm en de luchtwegen). Met behulp van het ontwikkelde reagens kunnen effecten van blootstelling aan schadelijke stoffen op de produktie van IgA worden vastgesteld, hetgeen van belang is om tot een nadere normstelling ten aanzien van deze stoffen te komen.

23. Beoordeling van het volksgezondheidsrisico door blootstelling aan BSE-infectiviteit van het destructiebedrijf Rendac Bergum

Voor het vaststellen en kwantificeren van het volksgezondheidsrisico van BSE infectiviteit voortkomende uit de activiteiten van het destructiebedrijf Rendac Bergum, werd een "risk assessment" onderzoek uitgevoerd. De stroom van infectiviteit die de fabriek binnenkomt werd gemodelleerd door gebruik te maken van een stroomdiagram. Het model werd geevalueerd door middel van een waarschijnlijkheidsbenadering van de risico-schatting om een beeld te krijgen van de onzekerheden in de ingevoerde parameters. De ingevoerde vaste parameters zijn steeds gekozen met het oogmerk een worst case situatie te beschrijven. De geselecteerde variabelen zijn gedefinieerd als een verdeling, en het resultaat vele malen berekend door middel van Monte Carlo simulatie. In 2001 zijn in Nederland in totaal 20 dieren positief getest voor BSE. Aangenomen is dat al deze dieren verwerkt zouden zijn bij Rendac Bergum als onderdeel van de SRM verwerking, en dat ze de infectiviteit bevatten van een volledig geinfecteerd dier. De mediane waarde voor de infectiviteit aan het begin van het proces werd bepaald op 3620 humane orale ID50 eenheden per jaar, met een spreiding van 36 tot 408000. De inactivatie door het destructie-proces werd bepaald op 99% van de totale infectiviteit. Van de resterende infectiviteit kwam de grootste hoeveelheid terecht in het eindproduct vlees- en beendermeel: 9 humane orale ID50 eenheden per jaar. Bij de huidige productie resulteert dit in 8 x 10-5 humane orale ID50 eenheden per kg vlees- en beendermeel. Al het vlees- en beendermeel wordt naar elders getransporteerd voor verbranding. BSE infectiviteit afkomstig van het destructiebedrijf kan via 3 routes in het milieu terechtkomen. Via het effluent van de waterzuiveringsinstallatie dat wordt geloosd in het Prinses Margriet kanaal, via het slib dat op land wordt gestort of wordt gebruikt als meststof, of via deeltjes in de gassen die in de omgevingslucht terechtkomen. In alle drie deze gevallen gaat het om extreem lage hoeveelheden BSE infectiviteit die geen significant risico opleveren voor mensen die in de omgeving van het destructiebedrijf wonen, werken of verblijven.

Abstract

Each year, influenza viruses cause epidemics by evading pre-existing humoral immunity through mutations in the major glycoproteins: the haemagglutinin (HA) and the neuraminidase (NA). In 2004, the antigenic evolution of HA of human influenza A (H3N2) viruses was mapped (Smith et al., Science 305, 371-376, 2004) from its introduction in humans in 1968 until 2003. The current study focused on the genetic evolution of NA and compared it with HA using the dataset of Smith and colleagues, updated to the epidemic of the 2009/2010 season. Phylogenetic trees and genetic maps were constructed to visualize the genetic evolution of NA and HA. The results revealed multiple reassortment events over the years. Overall rates of evolutionary change were lower for NA than for HA1 at the nucleotide level. Selection pressures were estimated, revealing an abundance of negatively selected sites and sparse positively selected sites. Abstract

The glycoproteins of herpes simplex virus type 1 (HSV-1) are important targets for the immune system in the control of HSV-1 infections. The humoral and T cell responses to the glycoprotein (g)H-t(His):gL complex of HSV-1 were studied in seven HSV-1-seropositive and three HSV-1-seronegative healthy adults. In addition, responses to HSV-1 go, were determined. As antigens, purified soluble recombinant forms of the gH(t(His)):gL complex produced by insect cells and of gD(t) produced by yeast cells were used, In contrast to seronegative donors, sera of all seropositive donors contained gH(t(His)):gL-specific IgG, Using peripheral blood (PB) T cells, gH(t(His)):gl-specific proliferative T cell responses were detected in all seropositive donors. Culture supernatants of PB T cells stimulated with recombinant gH(t(His)):gL contained high levels of interferon-gamma and no detectable interleukin-4, indicating their Th1 phenotype. These results show that naturally acquired HSV-1 infection induces gH:gL-specific humoral and T cell responses.

Abstract

Since prehistoric times, the Bering Strait area (Beringia) has served as an avenue of dispersal between the Old and the New Worlds. On a field expedition to this area, we collected fecal samples from dabbling ducks, geese, shorebirds, and gulls on the Chukchi Peninsula, Siberia, and Pt. Barrow, Alaska, and characterized the subtypes of avian influenza virus present in them. Four of 202 samples (2%) from Alaska were positive for influenza A virus RNA in two independent polymerase chain reaction (PCR)-based screening assays, while all shorebird samples from the Chukchi Peninsula were negative. Subtypes H3N8 and H6N1 were recorded once, while subtype H8N4 was found in two samples. Full-length sequences were obtained from the three unique isolates, and phylogenetic analysis with representative sequences for the Eurasian and North American lineages of influenza A virus showed that one HA gene clustered with the Eurasian rather than the North American lineage. However, the closest relative to this sequence was a North American isolate from Delaware described in 2002, indicating that a H6 spillover from Asia has established itself in North America.

Het model werd geevalueerd door middel van een waarschijnlijkheidsbenadering van de risico-schatting om een beeld te krijgen van de onzekerheden in de ingevoerde parameters. De ingevoerde vaste parameters zijn steeds gekozen met het oogmerk een worst case situatie te beschrijven. De geselecteerde variabelen zijn gedefinieerd als een verdeling, en het resultaat vele malen berekend door middel van Monte Carlo simulatie. In 2001 zijn in Nederland in totaal 20 dieren positief getest voor BSE. Aangenomen is dat al deze dieren verwerkt zouden zijn bij Rendac Bergum als onderdeel van de SRM verwerking, en dat ze de infectiviteit bevatten van een volledig geinfecteerd dier. In 2001, in the Netherlands, a total of 20 animals were tested positive for BSE. It was assumed that all of these animals would have been processed at the Rendac Bergum plant as part of the SRM processing, and that they had the infectivity of a fully infected animal. The median value of the infectivity entering the process was estimated to be 3,620 human oral ID50 units per year, with a range from 36 to 408,000. It was estimated that 99% of the infectivity entering the plant would be inactivated by the rendering process. Of the remaining infectivity, most was found to end up in in the Meat and Bone Meal (MBM) product: 9 human oral ID50 units per year. At the current production level this would result in about 0.00008 human oral ID50 units per kg MBM. Currently all of this MBM was sent offsite for disposal by incineration. Infectivity from the plant could enter the environment through one of three routes, via sludge used in landfill or spread on the land as fertiliser, waste water discharged to the canal, or as particles released to the air. In all 3 cases the amounts of infectivity were found to be very small and could not pose any significant risk to the public health.

RIVM rapport:Het influenzavirus ondergaat frequente antigene veranderingen die jaarlijkse aanpassing van het influenzavaccin door de WHO noodzakelijk maken. Voor dit doel genereert, verzamelt en analyseert het Nationaal Influenza Centrum (NIC, een samenwerkingsverband van het RIVM met de Erasmus Universiteit Rotterdam) in Nederland bij huisarts- en ziekenhuispatienten geisoleerde influenzavirusstammen. Het NIC vergelijkt deze stammen met de gebruikte vaccinstammen en met andere buitenlandse stammen. In zowel Nederland als elders in Europa was de influenzaepidemie van 1995/96 vroeg en van een gemiddelde omvang. Het subtype A(H3N2) overheerste maar ook A(H1N1)- en B-virussen werden gerapporteerd. De hoofdvarianten uit de seizoenen 1995 (zuidelijk halfrond) en 1995/96 (noordelijk halfrond) bleken in antigeen opzicht bij alle drie (sub)typen weinig of niet af te wijken van die uit 1994 en 1994/95, of van de gebruikte vaccinstammen. Zowel de immuniteit verworven door vroegere infecties als die opgewekt door vaccinatie zullen derhalve relatief goede bescherming hebben geboden tegen infectie met influenzavirus in 1995/96., As a consequence of frequent antigenic changes of the virus, the composition of the influenza vaccine is annually adapted by WHO. In this framework, the National Influenza Centre (NIC, a collaboration of RIVM with the Erasmus University Rotterdam) generates and collects influenza virus strains isolated in the Netherlands from patients presenting to general physicians and in hospitals. NIC compares these strains with the vaccine strains recommended by WHO and with other foreign strains. In Europe, including the Netherlands, the influenza epidemic of 1995/96 was early and of an average extension. Subtype A(H3N2) dominated but A(H1N1) and B-viruses were also reported. The antigenic reactivities of the three major influenza virus variants of the seasons 1995 (southern hemisphere) and 1995/96 (northern hemisphere) did not differ significantly from those of the seasons 1994 and 1995/96, or from the vaccine strains used in the 1995/96 season. Therefore, both the infection-acquired and the vaccine-induced immunity will have rendered a relative good protection against influenza virus infections in 1995/96.

Throughout Europe, except for Switzerland, the influenza epidemic of 1994/95 was unusually small and late. Strains of influenza virus B, A(H3N2), and A(H1N1) cocirculated. From both hemispheres a total of 84 strains of B virus, 82 of A(H3N2) virus, and 16 of A(H1N1) virus from the seasons 1994, 1994/95, and 1995 were examined. In antigenic respect, the major B virus variant of 1994/95 significantly differed from the major variant of the previous B virus epidemic in 1992/93 and from the vaccine strain used in 1994/95. Neither the immunity resulting from natural infection nor that induced by the vaccine will therefore have offered optimal protection against B virus infection in 1994/95. Het betrof in 195 gevallen het subtype A(H3N2) en in 12 gevallen het type B. De H3N2-virusstammen behoorden alle tot de nieuwe groep varianten, waarvoor het A/Beijing/32/92-virus de referentiestam is. Deze groep circuleerde ook al op kleine schaal in de eerste helft van 1993 met ongeveer dezelfde antigene eigenschappen. RIVM isolates influenza viruses in the NIVEL/RIVM surveillance of respiratory virus infections, sends them to WHO, and collects recent influenza virus strains from abroad. At RIVM, the foreign strains are examined with serological and molecular methods and compared with the Dutch virus strains. Of the seasons 1993 (southern hemisphere and tropical countries) and 1993/94 (temperate areas of the northern hemisphere), the influenza epidemics were almost exclusively caused by subtype A(H3N2). Of this period, 207 influenza virus strains originating from Singapore, Australia, New Zealand, Hong Kong, South Africa, Sweden, Norway, United Kingdom, France, Spain, and the Netherlands were analyzed. Of this collection, 195 strains belonged to subtype A(H3N2) and 12 to type B. All the H3N2 virus strains were related to the vaccine strain n/Beijing/32/92. This lineage of strains dates back to the sporadically circulating H3N2 virus strains of 1990. In this report a detailed picture is presented of the antigenic and molecular evolution of this lineage and of another, now (perhaps temporarily) extinct lineage. In 1993/94, we could distinguish three antigenic variants of A/Beijing/32/92-like H3N2 viruses. The main variant comprised 95% of the total number of tested strains. RIVM rapport:In dit onderzoek werd de bruikbaarheid van een nieuw ontwikkeld, relatief goedkoop isolatie-systeem voor de huisvesting van SPF dieren vergeleken met de bestaande SPF-kweek die gekarakteriseerd kan worden als een klassiek strikt barriere-systeem. Dit rapport beschrijft een aantal waarnemingen met berekking tot fysiologische en microbiologische parameters bij SPF ratten gehouden tijdens kweek en experiment in beide SPF-units. Er werden geen duidelijke verschillen in de waarden voor de verschillende fysiologische en microbiologische parameters gevonden. Geconcludeerd wordt dat bouwkundig relatief eenvoudige isolatie-systemen goed bruikbaar lijken voor de kweek van SPF ratten en het uitvoeren van langdurende experimenten.

Abstract

Background Treatment strategies for patients in whom HIV replication is not suppressed after exposure to several drug classes remain unclear. We aimed to assess the inter-relations between viral load, CD4-cell count, and clinical outcome in patients who had experienced three-class virological failure.Methods We undertook collaborative joint analysis of 13 HIV cohorts from Europe, North America, and Australia, involving patients who had had three-class virological failure (viral load >1000 copies per mL for >4 months). Regression analyses were used to quantify the associations between CD4-cell-count slope, HIV-1 RNA concentration, treatment information, and demographic characteristics. Predictors of death were analysed by Cox's proportional-hazards models.Findings 2488 patients were included. 2118 (85%) had started antiretroviral therapy with single or dual therapy. During 5015 person-years of follow-up, 276 patients died (mortality rate 5.5 per 100 person-years; 3-year mortality risk 15.3% (95% Cl 13.5-17.3). Risk of death was strongly influenced by the latest CD4-cell count with a relative hazard of 15.8 (95% CI 9.28-27.0) for counts below 50 cells per muL versus above 200 cells per muL. The latest viral load did not independently predict death. For any given viral load, patients on treatment had more favourable CD4-cell-count slopes than those off treatment. For patients on treatment and with stable viral load, CD4-cell counts tended to be increasing at times when the current viral load was below 10 000 copies per mL or 1.5 log(10) copies per mL below off-treatment values.Interpretation In patients for whom viral-load suppression to below the level of detection is not possible, achievement and maintenance of a CD4-cell count above 200 per muL becomes the primary aim. Treatment regimens that maintain the viral load below 10 000 copies per mL or at least provide 1.5 log(10) copies per mL suppression below the off-treatment value do not seem to be associated with appreciable CD4-cell-count decline.
