4. Mortality After Total Thyroidectomy for Amiodarone‐Induced Thyrotoxicosis According to Left Ventricular Ejection Fraction.

Abstract

Objective: To report cardiac outcomes after total thyroidectomy for amiodarone‐induced thyrotoxicosis according to the baseline left ventricular ejection fraction in a tertiary referral center. Study Design: Retrospective, monocentric. Setting: The tertiary health care system. Methods: Patients who underwent total thyroidectomy for amiodarone‐induced thyrotoxicosis between 2010 and 2020 with age >18 and available preoperative left ventricular ejection fraction were included in this study. Patients were dichotomized into: group 1 with left ventricular ejection fraction ≥40% (mildly reduced/normal ejection fraction), and group 2 with left ventricular ejection fraction <40% (reduced ejection fraction). Results: There were 34 patients in group 1 and 17 to group 2. The latter were younger (median 58.4 [Q1‐Q3 48.0‐64.9] vs. 69.8 years in group 1 [59.8‐78.3], p =.0035) and they presented more cardiomyopathy (58.8 vs. 26.5%, p =.030). Overall, the median time until surgery referral was 3.1 [1.9‐7.1] months and 47.1% underwent surgery after restoration of euthyroidism. Surgical complications accounted for 7.8%. In group 2, the median left ventricular ejection fraction was significantly improved after surgery (22.5 [20.0‐25.0] vs. 29.0% [25.3‐45.5], p =.0078). Five‐year cardiac mortality was significantly higher in group 2 (p <.0001): 47.0% died of cardiac causes versus 2.9% in group 1. A baseline left ventricular ejection fraction <40% and a longer time until surgery referral were significantly associated with cardiac mortality (multivariable Cox regression analysis, p =.015 and.020, respectively). Conclusion: These results reinforce the idea that surgery, if chosen, should be performed quickly in patients with left ventricular ejection fraction <40%. [ABSTRACT FROM AUTHOR]

Abstract

Abstract Background Using reduced doses of Cyclosporine A immediately after heart transplantation in clinical trials may suggest benefits for renal function by reducing serum creatinine levels without a significant change in clinical endpoints. However, these trials were not sufficiently powered to prove clinical outcomes. Methods In a prospective, multicentre, open-label, parallel-group controlled trial, 95 patients aged 18 to 65 years old, undergoing de novo heart transplantation were centrally randomised to receive either a low (130 Results At 12 months, the mean (± SD) creatinine value was 120.7 μmol/L (± 35.8) in the low-dose group and 132.3 μmol/L (± 49.1) in the standard-dose group (P = 0.162). Post hoc analyses suggested that patients with higher creatinine levels at baseline benefited significantly from the lower Cyclosporine A target. The number of patients with at least one rejection episode was not significantly different but one patient in the low-dose group and six in the standard-dose group required dialysis. Conclusions In patients with de novo cardiac transplantation, early Cyclosporine A dose reduction was not associated with renal benefit at 12 months. Abstract

Background Solid organ transplant (SOT) recipients are at risk of nocardiosis, a rare opportunistic bacterial infection, but prognosis and outcome of these patients are poorly defined. Our objectives were to identify factors associated with one-year mortality after nocardiosis and describe the outcome of patients receiving short-course antibiotics (≤120 days). Methods We analyzed data from a multicenter European case-control study that included 117 SOT recipients with nocardiosis diagnosed between 2000 and 2014. Factors associated with one-year all-cause mortality were identified using multivariable conditional logistic regression. Abstract

JCTCV:21(3), Aim: Long-term mechanical circulatory support is increasingly being used to deal with end-stage heart failure. The objective of this retrospective monocentric study was to evaluate the short- and mid-term evolution of patients receiving left ventricular assist device (LVAD) implanted by left thoracotomy without cardiopulmonary bypass at our center. Methods and results: Eighteen patients (17 males) aged 63.3 ± 7.3 years received LVAD between November 2012 and August 2016 (15 [83%] as destination therapy and 3 [17%] as a bridge to transplant). Aetiology was ischemic cardiopathy for 13 (72%) cases and dilated cardiomyopathy for 5 (28%) cases. The follow-up time was 17 months and survival at 12 months was 66% (73% in the subgroup of patients implanted as destination therapy), with acceptable postoperative complications. Conclusion: This approach is technically feasible without cardiopulmonary bypass and is a potential alternative to conventional medial sternotomy, especially for redo patients. However, its place has yet to be defined, in particular in patients who require a subsequent heart transplant.

Abstract

JCTCV:21(2), Objective: To assess hospital morbi-mortality of isolated aortic valve replacement (AVR) in a monocentric cohort of octogenarians. Methods: Between 2002 and 2014, 769 octogenarian patients, male in 46.4% (n=357), with a mean age of 82.7±2.4 years [range: 80–94.4], underwent isolated AVR with implantation of a biological valvular prosthesis in our center. The main surgical indication was aortic stenosis in 79.3% of patients. Patients who underwent redo surgeries were excluded from the study. Results: The hospital mortality rate was 1.95% (n=15) with a Euroscore 2 average of 2.7±3 [range: 0.9–41.2]. Mean intensive care unit stay was 3.1 ±4.5 days with a total hospital stay of 13.5±7.9 days. The main postoperative complications included blood transfusion (62.5%), renal failure (15.6%) requiring dialysis in 2.7 % of the cases, para-prosthesis aortic insufficiency (1 to 2/4) (3.4%), prolonged artificial ventilation (>48h) (2.9%), pace-maker implantation (1.4%), stroke (1.2%) and mediastinitis (0.5%). Multivariate predictors of hospital mortality were cardiogenic shock (OR=34.8 [4.2–215.9]), and a BMI greater than 30 (OR 4.5 [1.3–16.2]). Conclusion: Currently, aortic valve replacement in octogenarians is a reliable surgery with a low hospital morbi-mortality rate. It also provides effective identification of patients at risk.

Abstract

International audience; Molecular mechanisms underlying vascular and inflammatory cell network at endothelial and macrophage levels are still unclear. Here we found that microvascular inflammation associates with changes in Notch signaling at endothelium/monocyte interface including loss of endothelial Notch4 and the acquisition of the Notch ligand Dll4 in both cell types. We showed that endothelial DLL4 induces circulating monocytes to polarize into a M1-type pro-inflammatory macrophages (CD40highCD64highCD200Rlow HLADRlowCD11blow) eliciting the production of IL-6. Dll4 and IL-6 are both Notch-dependent and are required for macrophage polarization through selective down and upregulation of M2and M1-type markers, respectively. Subsequently, we investigated the ability of DLL4 to interfere with M2 polarization. We found that DLL4 triggers a specific alteration of the IL-4 induced M2 phenotype through a significant inhibition of M2 markers (CD11b, CD206, CD200R). DLL4 also induces caspase3/7-dependent apoptosis specifically in M2 differentiating macrophages while DLL1 had no effect. DLL4 signals via Notch1 and DLL4mediated apoptosis is Notch-dependent. Fully differentiated M2 macrophages became resistant to DLL4 action. DLL4 upregulates gene expression, upon M2 upon differentiation, affecting the Notch pattern (Notch1, 3, Jag1) and activity (Hes1), transcription (IRF5, STAT1) that associates with decrease in Akt but not STAT6 phosphorylation. In conclusion, our findings reveal an interplay between DLL4/Notch and IL-6/IL-6R or IL-4/IL-4R signaling pathways supporting M1 differentiation and impairing M2 differentiation via apoptosis.

In nonoperated prosthetic valve endocarditis (PVE), long-term outcome is largely unknown. We report the follow-up of 129 nonoperated patients with PVE alive at discharge. Abstract

Background: Thrombocytopenia is a common disorder after heart or lung transplantation. Platelet transfusion is often required to maintain haemostasis but represents a specific cause of morbidity and mortality in this setting including alloimmunisation and graft rejection. Study design and methods: As part of a health-care quality improvement project, in a single-centre before–after pilot study, the relevance of a platelet transfusion saving strategy based on romiplostim administration after transplantation was assessed in patients with platelet count <100 × 109/L. Transfusions on days 28 and 90 were compared using propensity matched score for adjustment of demographic characteristics at baseline. The primary outcome was platelet transfusion until day 28 after transplantation. Results: Ninety-three patients were analysed (73 before vs. 20 after). The median [interquartile range] number of platelet concentrate was 1 [0;4.0] before versus 0.5 [0;2.0] in the after period, mean difference 0.5 confidence interval 95% [−0.7 to 1.7], p = 0.39. On day 28, median [interquartile range] red blood cell transfusion was significantly higher in the before versus the after period, 7 [2.0;13.5] versus 6 [1.5;8.5], mean difference 3.2 CI 95% [0.4-6.0], p = 0.02. At 6 months, the rate of patients with de novo anti-human leukocyte antigen alloimmunisation was 45% before versus 53% in the after period (p = 0.56). Deep venous thrombosis was detected in nine patients (12%) before versus seven patients (35%) in the after period (p = 0.04). Conclusion: Romiplostim did not significantly reduce platelet transfusion after heart or lung transplantation.
